▢ 국제교류 및 협력활동 배경
원활한 의료기술평가연구 수행을 위해서는 국제 유관기관과의 공조 및 협력이 중요하다. 국제사회는 지속가능 개발목표(Sustainable Development Goals, SDGs)를 설정했고, 사람은 누구나 경제적 상황과 관계없이 적절한 의료서비스를 제공 받을 수 있어야 한다는 ‘보편적 의료보장(Universal Health Coverage, UHC)의 실현’을 하나의 목표로 정하고 있다. 그리고 UHC의 실현을 위한 효과적 도구(tool)로써 의료기술평가의 도입 및 역할이 강조되면서 국제사회의 의료기술평가에 대한 관심이 지속적으로 증가하고 있다.
▢ 2016년 국제교류 및 협력활동 목적
2016년 한국보건의료연구원의 국제교류 및 협력은 중·저개발국가 대상 의료기술평가 저변 확대 활동 및 향후 유사 활동을 지속적으로 수행하기 위한 예산확보 방안을 마련하는데 집중하였다. 그리고 의료기술평가 관련 기존 국제협의체 활동을 유지하고, 국제사회가 한국보건의료연구원에 기대하고 있는 활동을 성실히 수행하는 것에 목적을 두었다.
▢ 2016년 국제교류 및 협력활동 성과
중·저개발국가 대상 의료기술평가 저변확대 활동은 실제 의료기술평가에 대한 한국보건의료연구원의 노하우를 확산하는 활동과 향후 이러한 활동을 지속하기 위한 추가 재원 마련 활동으로 진행되었다.
2015년 대우재단으로부터 지원 받은 기부금을 활용해 아시아태평양경제협력체(Asia Pacific Economic Cooperation, APEC) 소속 회원국들과 세계보건기구(World Health organization, WHO) 서태평양지역(Western Pacific Region, WPR) 내 회원국을 대상으로 의료기술평가 국제워크숍을 개최하였고, 이 활동의 결과를 2016년 APEC Health Working Group 2차 회의를 통해 공유하였다. 그 자리에서 향후 동일한 워크숍이 지속적으로 추진될 수 있도록 APEC의 협력과 지원을 호소하였다. 2016년에 새롭게 진행된 의료기술평가의 국제적 확산 활동으로는 태국 정부가 개최하고 저명한 국제개발 및 보건 단체들이 협력하는 Prince Mahidol Award Conference(PMAC)에 한국보건의료연구원이 공동개최기관으로 참여한 활동으로, 행사 기간 동안 2차례의 의료기술평가 관련 학술세션을 개최하여 PMAC에 참여한 중·저개발국가 공무원, 학자 등을 대상으로 의료기술평가에 대한 한국보건의료연구원의 경험을 공유하였다.
의료기술평가에 대한 중·저개발국가 대상 저변 확대 활동은 지속적으로 요구가 증가하고 있는 만큼 사업을 유지할 수 있는 추가적인 예산을 확보하는 것이 중요하다. 이를 위해서 2016년 한 해 동안 국제개발 및 보건분야 비정부 시민단체와 협력하여 의료기술평가 저변 확대 사업에 대한 제안서를 개발하고, 실제 제안 가능한 기관에 대해 조사도 실시하였다. 제안서의 방향은 UHC와 연결하여 UHC 실현을 위한 도구로써 의료기술평가에 대한 중·저개발국가 공무원 대상 교육훈련이 필요함을 강조하였다. 외교통상부 산하 KOICA(Korea International Cooperation Agency)의 경우 최근에 UHC를 주요 사업 내용에 포함시키면서 향후 KOICA가 의료기술평가의 국제적 저변 확대 활동에 관심을 가질 가능성이 높아졌다.
한국보건의료연구원의 연구역량 및 국제적 인지도가 높아짐에 따라 기존에는 개별연구자가 개별과제의 결과를 공유하는 차원에서 참석했던 HTAi(Health Technology Assessment international) 학술대회에서 한국보건의료연구원 세션을 개설하여 HTAi 참석자들 대상으로 교육 세션을 제공하는 수준으로 기관의 국제적 역할이 증대되었다.
2014년, 2015년에 각각 체결된 국제 유관기관과의 MOU에 대한 추가활동도 진행되었다. 영국 NICE(National Institute for Health and Care Excellence)와는 UHC를 주제로 개최된 PMAC 2016에 공동개최 기관으로 PMAC 프로그램 구성 및 운영에 협력하였다. 2015년에 MOU를 체결한 캐나다 CADTH와는 향후 기관 간 인력교류가 가능하도록 국제협력 담당자 간 지속적인 의견교류가 진행되었다. 우선적으로는 한국보건의료연구원에서 CADTH로 인력을 파견해 상호 간 경험을 공유하는 내용으로 기관 간 교류 가능성을 검토하였다.
이 외에 기존 협력하거나 기관회원으로 협의체 활동을 진행하고 있던 곳들과의 교류활동을 진행하였다. HTAsiaLink 사무국 활동, HTAi Policy Forum, INAHTA 활동을 통해 의료기술평가 관련 국제동향을 파악하고 한국보건의료연구원의 경험 을 공유하였다.
▢ 국제교류 및 협력활동 발전방향 모색
2016년 국제교류 및 협력활동을 통해 향후 개선사항을 검토하였다. 첫째, 국제교류 및 협력활동의 전문성 및 지속성을 확보하기 위한 인력 배치 및 양성이 필요하다. 또한 기관의 국제업무를 전담할 코디네이터가 필요하다. 그리고 국제협력을 위한 컨텐츠는 각 분야별 담당자 그룹(pool)을 구성해 내부 인력이 국제교류 및 협력업무에 참여할 수 있도록 해야 할 것이다. 둘째, 기존의 네트워킹 중심에서 컨텐츠 중심으로 국제교류 및 협력활동을 전환할 필요가 있다. 이를 위해서는 우선적으로 컨텐츠 개발이 필요하지만, 이 일에 참여하는 연구자들은 국제활동 참여 뿐 아니라 관련분야 전문가들과의 인프라 구축 기회를 얻게 될 것이다. 셋째, 한국보건의료연구원이 협력하고 있는 국외유관기관 및 국제협의체 등에 대한 객관적인 평가를 통해 국제협력 대상 기관 및 협의체에 대한 ‘선택과 집중’이 필요하다. 국제협력 그 자체에 의미를 부여하기 보다는 협력 내용과 기대효과를 파악해 협력대상 기관 및 협의체를 선별하고, 선정된 기관 및 협의체에 대해서는 현재 수준 이상의 적극적인 국제교류 및 협력활동을 추진하여, 기관의 국제적 기여 수준을 높일 필요가 있다.
▢ Background of international collaboration activity
It is well known that international collaboration is an important factor to achieve smooth and better work in health technology assessment. The Sustainable Development Goals (SDGs) has been introduced in the international society and it includes achieving Universal Health Coverage (UHC) which provides appropriate healthcare to all people without financial hardship. In order for achieving UHC it is emphasized that health technology assessment can be a useful tool; in turn, health technology assessment has been globally attractive.
▢ Purposes of international collaboration activity
National Evidence-based Healthcare Collaborating Agency’s (NECA) international collaboration activity in 2016 aimed to disseminate health technology assessment to low- and middle-income countries and to develop ways to get funds to sustain the similar activities in the future. Additionally, NECA has kept its collaboration activities with the international networks related to health technology assessment.
▢ Achievement of international collaboration activity in 2016
There were two main activities to disseminate health technology assessment to low- and middle-income countries. One was to share NECA’s experience and research outcomes in health technology assessment with the targeted countries, and the other was to develop ways to get funds to sustain dissemination activity from different sources. In 2015, with a financial support from the Daewoo Foundation, NECA successfully hosted an international workshop on health technology assessment for the member countries in Asia Pacific Economic Cooperation (APEC) and the World Health Organization (WHO) Western Pacific Region (WPR). Since the workshop was hosted under the name of APEC Health Working Group, the result of the workshop should be shared with the members of APEC Health Working Group; in turn, NECA attended the second meeting of 2016 APEC Health Working Group, presented the outcomes of workshop, and asked their further support and collaboration on the dissemination of health technology assessment globally. The new dissemination activity in 2016 was to co-host the Prince Mahidol Award Conference (PMAC) which has been run by the Thai government with the international bodies working for global development and global public health. During the Conference NECA arranged two academic sessions regarding health technology assessment to educate civil servants and experts in low- and middle-income countries and shared NECA’s experience and know-how in health technology assessment.
In order for financial support to sustain the dissemination activity of health technology assessment, NECA developed a proposal with a non-governmental organization which has many experience in the field of global development. The proposal focused on educating civil servants in low- and middle-income countries about health technology assessment as a tool for achieving UHC. In addition, NECA found that KOICA selected UHC as one of the topics for future activities, thus, we believe KOICA can be a good candidate as NECA’s new partner for the dissemination activity of health technology assessment in the near future.
With increasing NECA’s global awareness and research capacity, NECA opened a academic training session for the participants in the Annual Conference of Health Technology Assessment International instead of submitting abstracts of each research topic that NECA conducted.
Based on the MOU signed in 2014 and 2015, NECA has continued collaboration with the global partners. With NICE UK, NECA had participated in preparation to co-host PMAC 2016 which focused on UHC. With CADTH Canada that NECA signed MOU with in 2015, NECA discussed about exchanging human resources. NECA firstly asked CADTH to invite a researcher in NECA to CADTH as an initial approach.
NECA has kept its collaboration activity with international partners, such as HTAsiaLink, HTAi Policy Forum and INAHTA, in order to collect global information about health technology assessment and to share NECA’s experience with them.
▢ Seeking for future directions in international collaboration
Through the international collaboration activity in 2016, several improvement directions were found out. Firstly, it is needed to raise professionals and designate proper employees for international activities to improve and sustain NECA’s international collaboration. There is need of the coordinator who takes full charge of international collaboration. To share NECA’s research outcomes with the international partners, the researchers who conducted that research can be directly participating in international activity. Secondly, NECA’s international collaboration should shift from networking-oriented activity to content-oriented activity. With this change, each researcher in NECA can have an opportunity to participate in international collaboration activity, to improve their knowledge, and to build up relationship with international experts. Thirdly, given the limited resources, NECA should choice and concentration of international partners after an objective evaluation. Rather than just conducting international collaboration, NECA should chose suitable partners based on the consideration of contents and expectancy effect of collaboration. Once the partners have been confirmed, NECA should positively conduct collaboration activity and improve its contribution to the international society of health technology assessment.
