1. 연구배경
국내 정신질환실태역학조사에 의하면, 우울증의 평생유병률은 2001년 4.0%에서 2006년 5.6%로 증가하였으며, 일년유병률도 2001년 1.8%에서 2006년 3.2%로 증가하였다. 2009년 사망원인 통계(통계청)에 의하면 자살사망자수는 15,413명으로 2008년에 비해 2,555명(19.9%)이 증가하였으며, 1일 평균 42.2명꼴로 발생하며, 자살률(인구 10만명당)은 31.0명으로 전년도 대비 19.3% 증가하고 있다.
우울증과 자살의 증가로 사회적 문제가 된 우울증과 자살의 질병부담에 대한 개별 연구가 이루어지고 있으나 국민이나 사회에 정확하고 필요한 정보를 제공하는데 한계가 있어 이러한 개별연구의 통합과 임상전문가 등과의 다각적인 논의를 통해 사회에 필요한 정보를 국민과 사회에 제공하여 우울증과 자살에 대한 사회적 인식을 개선하고자 한다.
2. 방법
우울증과 자살에 대한 질병부담과 치료현황에 대해 지금까지 연구된 성과들 중 활용가능한 연구들을 통합하여 국민들에게 필요한 정보를 제공하기 위한 방법으로 round-table conference(RTC)를 하기로 하였다.
RTC의 역할은 사회에 필요한 정보를 국민과 사회에 제공, 필요한 후속연구 제시 및 정책 제안이다. RTC 과정은 기획, RTC 개최, 후속관리로 크게 나누어지며, 참석자 모두가 다각적인 논의과정을 거쳐 합의된 주요 메시지를 도출하는 방식으로 운영한다.
3. round-table conference (RTC)
1) RTC 운영결과
한국보건의료연구원과 우울증임상연구센터 공동 주최하여 보건복지부, 근거창출임상연구국가사업단과 대한신경정신의학회의 후원으로 “우울증, 자살 그리고 한국사회 round-table conference”가 8월 26일 서울대학교 의과대학 암연구소 이건희홀에서 개최하였다. 이 날 행사에는 정신보건센터 종사자, 청소년자살예방협회장, 관련 임상전문가 등이 참석하였다.
2) 발표내용
(1) 우리나라의 우울증 현황
현재 의학적 치료를 요하는 심각한 우울 장애를 앓고 있는 사람이 2백만 명을 상회할 것으로 추정된다. 그러나 적극적으로 의학적 치료를 받고 있는 환자는 이들의 10% 수준에 불과하며 자살률 증가, 의료비 상승, 삶의 질 저하 등과 같은 사회 경제적 지표로 반영되고 있다. 우울증에 대한 합리적 대책 수립을 위해 한국인의 우울증에 대한 보다 정확한 실태 조사를 더 이상 늦추어서는 안 될 상황이다.
(2) 우울증과 자살
자살이나 자살행동은 스트레스에 의한 정상적인 반응이 아니며, 우울증과 밀접하게 연관되어 있다. who에서는 우울증과 알코올, 약물남용에 대한 치료와 예방, 자살기도자에 대한 향후 치료와 관리를 통해서 자살률을 낮출 수 있다고 강조하였다. 계획자살기도군의 60%, 충동자살기도군의 72%에서 정신질환에 이환 된 적이 있는데, 이중에서 주요우울증과 알코올 의존․남용이 관련성이 가장 높은 것으로 나타났다. 따라서, 자살기도자의 주요우울증을 조기에 발견하고 치료하는 것이 자살예방에 중요한 점이라 할 수 있겠다.
(3) 우울증의 비약물학적 치료
우울증을 치료하기 위해 항우울제과 더불어 비약물학적 치료를 시행한다. 비약물학적 치료로 현재 임상적용이 가능한 방법으로는 운동, 독서치료, 인지행동치료, 대인관계치료, 단기 정신역동적 지지정신치료가 있다. 우울증에서 권고할 수 있는 자기관리 방법을 체계적 고찰에 의해 평가한 결과 근거 있는 것으로는 성요한 초(st. john’s wort), 운동, 독서치료, 광선치료이며, 제한된 근거로는 침, 광선치료, 마사지, 음이온, 요가, 이완요법, same, 엽산 등이다.
(4) 비약물적 치료와 항우울제 약물순응도의 관계
국내 실제 임상에서 사용되고 있는 비약물치료법의 현황과 항우울제 약물투약순응도와의 관련성을 건강보험심사평가원 청구자료를 이용하여 2006년에서 2008년 사이 일정 기준에 해당하는 환자를 대상으로 분석하였다.
비약물치료법의 연도별 처방양상을 확인한 결과 2006년과 2007년에 비해 2008년에는 크게 감소한 양상이 나타났다. 이는 3년간 항우울제 처방건수는 지속적으로 증가하는 양상과 대조적인 결과이다. 비약물치료의 치료횟수에 따른 항우울제 투약순응도를 살펴본 결과 남성은 비약물치료를 4회 이상 받은 경우 투약순응도가 비약물치료를 받지 않은 군에 비해 높아지는 경향성을 보였고, 여성은 3회 이상이면 투약순응도가 높아지는 경향을 보였다.
(5) 외국의 우울증으로 인한 자살 방지 대책
우리사회에서는 자살이 선택 가능한 하나의 대안인 것으로 인식되고 있으나 자살은 끝이 아니라 이후에 수많은 문제를 낳고 사회에 심리적인 충격과 함께 사회 경제적인 부담을 지운다. 실제로 자살은 치료 가능한, 치료를 해야 할 의학적인 문제이나 우리나라는 그에 대한 체계적인 역학 연구나 그에 바탕을 둔 구체적인 예방 프로그램은 존재 하지 않는다. 미국 학술위원회 의학연구소 (institute of medicine; iom)의 권고는 자살을 예방가능한 의학적인 상태로 보고 구체적인 권고를 제시하는 것으로 현재 자살 예방에 대한 관심이 커지고 있는 우리 사회에 던지는 의미가 크다고 할 수 있다.
3) 토의 내용
(1) 국내 우울증 현황
국내 우울증 환자는 국내 인구수를 바탕으로 추정 시 평생 우울장애를 한 번이라도 앓는 사람은 약 2백만 명으로 추정되고 있으나 현재 국내에서는 정확한 우울증 환자의 현황을 파악할 수 없어 이에 대한 대책마련이 필요하다. 또한 우울증 치료 중 비약물적 치료는 2007년까지 증가하였으나 2008년 이후로는 급격한 감소한 반면 항우울제는 매년 지속적으로 증가하는 양상을 나타났다. 이에 우울증 환자에 대한 정확한 치료현황과 함께 환자들이 치료를 받는데 장애요인을 파악 및 이를 방안을 마련하여 지속적인 치료로 우울증을 줄일 수 있도록 해야 할 것이다.
(2) 한국형 우울증 진단기준의 필요성
우리나라의 우울증 환자는 ‘우울하다’는 표현대신 ‘외롭다, 쓸쓸하다, 짜증난다, 참을 수가 없다’ 등의 표현과 더불어 신체증상이 많이 나타나는 반면 ‘우울하다’라는 표현을 하며 신체증상이 많지 않은 서구 환자와는 차이를 보인다. 국내에서 사용하고 있는 우울증 진단도구는 미국, 영국에서 개발한 도구로 우리나라의 문화적 특성을 반영하지 못하고 있다. 실례로 자살과 관련이 많은 우울증의 경우, 국내 유병률이 미국보다 2.5배 낮음에도 불구하고 자살률은 oecd에서 1위로 우울증과 자살의 관련성이 낮아 보인다. 이는 우울증이 과소 추계된 것으로 여겨지며, 따라서 국내 현실에 부합하는 한국형 우울증 진단기준을 마련하여 정확한 우울증 유병률을 파악하는 것이 필요하다.
(3) 자살에 대한 사회적 인식 변화 필요
자살은 통계청 등에서 공식적으로 사용하는 명칭이 ‘고의적 자해’이다. 이는 자살이 힘든 현실에서 도피하기 위한 개인의 선택인 고의성을 지닌 자해행동으로 사회에서 인식하기 때문에 자살을 기도하면 건강보험뿐만 아니라 사보험의 혜택도 볼 수 없는 실정이다.
이에 대해 사회적으로 자살이 치료가 필요한 의학적 문제라는 것에 공감대를 형성하여 치료를 받을 수 있는 분위기 조성이 필요하다. 아울러, 건강보험과 사보험의 문제에 있어서도 의학적 문제라는 점을 인식하여 자살기도자를 범죄자가 아닌 환자로 보험혜택을 받을 수 있도록 제도적 개선이 필요할 것으로 여겨진다.
(4) 자살 예방을 위한 사회적 개입 필요
자살예방을 위해 정부에서 시행하고 있는 지하철 스크린도어 확대설치, 한강 교각 정비 등의 여러 가지 정책에 대해 자살예방효과평가에 대한 연구를 통해 효과적인 정책이 지속적으로 시행될 수 있도록 근거를 마련하여 하여야 할 것이다. 자살사망자가 발생 시 핀란드의 예처럼 심리적 부검을 통해 자살 예방 대책이 사회나 국가차원에서 마련될 필요가 있다.
아울러, 우울증과 자살에 대한 정확한 현황자료가 없는 현 시점에서 건강보험심사평가원의 청구자료, 통계청 사망자료, 각 병원의 emr 등 활용가능한 자료를 연계하여 현황파악 및 좀 더 구체적이고 폭넓은 자료를 생성할 수 있는 정책적 방안 마련이 조속히 이루어질 필요가 있다. 그리고 우울증, 자살기도는 사회적으로 낙인이 됨으로 이를 없애는 방안 모색이 시급하다.
4. 성과 확산
RTC에서 합의된 주요 메시지를 일반인과 임상전문가와 정책결정자들에게 확산하기 위해 대상별 접근방법을 달리하는 확산전략을 세웠다.
일반인과 사회에 주요 메시지를 전달하기 위해서 리플렛을 제작하여 배포하였으며, 임상전문가와 정책결정자를 위해 학술지 논문게재, 연구원 소식지인 근거와 가치에 원고를 게재하였으며 자료집을 요청하는 개인 또는 단체에게 개별적으로 우편 또는 이메일로 송부하였다.
5. 고찰
우울증과 자살의 질병부담에 대해 현재 활용 가능한 연구성과를 통합하여 임상전문가 등 관련 전문가들과의 다각적인 논의를 통해 사회에 필요한 정보를 도출했으며, 이러한 토론의 장을 앞으로 지속적으로 가질 수 있는 발판을 마련했다는 점에 의의가 있다. 그리고 자살을 개인적인 문제가 아닌 사회문제로 인식하여 해결방안을 모색하는 사회적 접근을 시도하였으며 이를 공론화하여 추후 이에 대한 사회적 관심을 유도할 수 있는 계기가 되었다.
우울증 치료에 제시된 자료가 건강보험심사평가원의 청구자료로 자료의 신뢰성 등이 문제가 되며 환자의 조작적 정의로 추정되었으며, 보험급여의 기준에 따라 청구의 형태가 바뀌는 등의 자료의 한계점으로 해석 시에 한계를 가지고 있다.
국내 현실에 부합하며 문화적 특성을 감안한 한국형 우울증 진단 기준 마련을 위한 연구와 청소년 등을 포함한 전 국민 대상 실태조사가 필요하다. 그리고 현 시점에서 활용가능하고 구체적인 정보를 알기 위해서는 건강보험심사평가원의 청구자료, 통계청 사망자료, 각 병원의 진료정보 등을 연계하여 연구에 활용할 수 있는 방안을 마련하는 정책연구가 필요할 것이다. 또한 정부에서 시행하고 있는 자살예방정책에 대한 자살예방효과평가 연구를 통해 효과성과 효율성 등을 파악하고 실효성 있는 정책이 지속적으로 유지될 수 있도록 근거를 마련할 필요가 있다.
6. 결론 및 정책제언
RTC를 통해 현재 의학적 치료를 요하는 심각한 우울 장애를 앓고 있는 우리 국민이 2백만 명을 상회할 것으로 추정되나 적극적으로 의학적 치료를 받고 있는 환자는 이들의 10% 수준에 불과하여 자살률 증가, 의료비 상승 등으로 이어지고 있다. 또한 자살기도자의 60~72%가 정신질환을 가지고 있어 자살은 고의적 자해가 아니라 치료가 필요한 의학적 문제인 것으로 나타났다. 따라서 자살은 의학적 관점과 사회적 관점에서 접근해야 하며, 효과적인 자살예방을 위해서는 의학적 접근이 중요하다는 점에 대해 사회적 인식변화가 동반되어야 한다.
우울증의 합리적인 대책 수립을 위해서는 한국의 문화적 특성을 반영한 진단도구 개발, 정기적 대규모 실태조사 시행과 이에 대한 감시체계를 마련할 수 있는 정책적 방안을 마련이 시급하다. 우울증과 자살 관련하여 현재 활용가능한 건강보험심사평가원, 통계청 등 많은 자료가 있으나 각 기관의 자료 연계가 용이하지 않다. 이에 우리나라에서도 공익적 목적을 위한 연구의 경우, 각 기관의 자료를 연계할 수 있는 방안을 정책적으로 마련해야 할 필요가 있다. 그리고 자살예방을 위해서는 지속적이고 광범위한 범국가적 자살예방정책에 대한 연구가 절실히 필요하다.
1. Background
According to the epidemiological survey of psychiatric illnesses in Korea, the lifetime prevalence of depression increased from 4.0% in 2001 to 5.6% in 2006 and the 12-month prevalence also increased from 1.8% in 2001 to 3.2% in 2006. According to the 2009 statistics of the causes of death(Statistics Korea), the number of deaths by suicide was 15,413 which was an increase by 2,555(19.9%) compared to 2008.
Although individual studies of the disease burdens of depression and suicide are being conducted but they have limitations in providing accurate and necessary information to the people or society and thus we plan to integrate these individual studies and conduct multilateral discussion with clinical experts to provide the people and society with information necessary to society in order to improve social perceptions of depression and suicide.
2. Method
We decided to hold a Round-table Conference(RTC) as a method to integrate those pieces of information that are the most available among the outcomes of studies conducted thus far on the disease burdens of depression and suicide and the states of treatment of depression and suicide. The roles of the RTC are providing the people and society with information necessary to society, suggesting further research and proposing policies. The processes of the RTC are largely divided into planning, RTC holding and follow-up management and the RTC was operated to derive key messages agreed through comprehensive discussion among all attendees.
3. Round-table Conference (RTC)
1) Results of the RTC operation
The “Round-table Conference on depression, suicide and Korean society” was held on August 26 in Lee Kun-Hee Hall, in the Seoul National University Cancer Research Institute under the joint auspices of National Evidence-based Healthcare Collaborating Agency and the Clinical Research Center for Depression Clinical Study Center sponsored by the Ministry of Health and Welfare, National Strategic Coordinating Center for Clinical Research (NSCR) and Korean Neuropsychiatric Association. In the event, professionals from mental health centers, the president of the Association for Preventing Youth Suicide and related clinical experts participated.
2) Presentation
(1) Present state of depression in Korea
It is estimated that the number of Korean people suffering from serious depressive disorders that require medical treatment is over 2 million. However, patients who are actively receiving medical treatment are only around 10% of them and the results are being reflected on socioeconomic indexes such as increasing rates of suicide, increasing medical expenses and deteriorating quality of life. To establish reasonable countermeasures against depression, we should not further delay more accurate surveys of the present state of Korean people’s depression.
(2) Depression and suicide
Suicide and suicidal behaviors are not normal responses to stress but are closely related with depression. WHO emphasized that the rates of suicide could be reduced by the treatment and prevention of depression and alcohol or drug abuse and follow-up treatment and management of suicide attempters. Around 60% of planned suicide attempters and 72% of unplanned suicide attempters had mental disorders and attempts were associated with major depression, alcohol use disorders. Therefore, in order to prevent of suicide, it is important to the early detection and treatment of depression in suicide attempters.
(3) Non-pharmacological treatment of depression
To treat depression, non-pharmacological treatment is also conducted in addition to antidepressants. Non-pharmacological treatment methods that can be currently applied to clinics include exercises, reading therapy, cognitive behavioral therapy, human relation therapy and short-term dynamic supportive psychotherapy. Based on the results of evaluation by systematic review of self-control methods that can be recommended for depression, evidence based ones are St. John’s wort, exercise, reading therapy and light therapy, those with limited evidence are acupuncture, light therapy, massage therapy, negative air ionization, yoga, relaxation therapy, SAMe, folate and standard treatment include antidepressants and cognitive behavioral therapy(CBT).
(4) Relationship of non-pharmacological treatment and the adherence to
antidepressants
The present state of non-pharmacological therapies actually used in clinics in Korea and the relationship between non-pharmacological therapies and compliance to the administration of antidepressant were analyzed using billing data from the Health Insurance Review & Assessment Service(HIRA) with patients that met certain criteria between 2006 and 2008. The states of the prescription of non-pharmacological therapies by year were checked and based on the results, the prescriptions decreased greatly in 2008 compared to 2006 and 2007. This is in contrast with the state of the prescriptions of antidepressants that continuously increased for the three years.
(5) Measures to prevent suicide resulting from depression in foreign countries
In our society, suicide is perceived as one of alternatives that may be selected. However, suicide is not the end but brings about numerous serious problems later and gives socioeconomic burdens to society along with psychological shocks. Suicide is indeed a medical problem that can be must be treated but no systematic epidemiological studies of suicide or concrete prevention programs based on the studies exist in Korea. A recommendation by an academic committee in the USA, the Institute of Medicine (IOM) presents concrete recommendations considering suicide as a preventable medical condition and it can be said to quite meaningful to our society where attention to suicide prevention is currently increasing.
3) Discussion
(1) Present state of depression in Korea
Based on domestic population, the number of patients who have suffered from depression at least once during their lifetime is estimated to be around two millions. However, it is not possible to grasp the present state of depression patients in Korea accurately and therefore, countermeasures against this are necessary. Of depression therapies, non-drug therapies had increased by 2007 but drastically decreased since 2008 while antidepressant treatment showed a pattern of continuous increases every year. Along with accurate states of treatment of depression patients, hindering factors
against patients’ treatment should be grasped to prepare measure to remove the hindering factors in order to reduce depression patients through continuous treatment.
(2) Necessity of Korean style criteria for depression diagnoses
Depression patients in Korea frequently show bodily symptoms along expressions such as ‘lonely, solitary, annoyed or unbearable’ instead of an expression, ‘depressed’ while Western patients express ‘depressed’ and do not frequently show bodily symptoms. The depression diagnosis tools used in Korea have been developed in the
USA or in the UK and they do not reflect the cultural characteristics of Korea. For instance, the prevalence of depression which is closely related with suicide in Korea is as low as to be only 1/2.5 of that in the USA but the rate of suicide in Korea is the highest among OECD countries and thus it seems like that depression and suicide are not closely related because depression was underestimated. Therefore, it is necessary to prepare Korean style criteria for depression diagnoses that coincide with the reality in Korea in order to grasp the accurate prevalence of depression.
(3) Social perceptions on suicide should be changed
A term of suicide officially used by Statistics Korea is ‘intentional self-injury’. As such, suicide is perceived in society as self-injuring behaviors based on intentions which are individuals’ choices to escape from hard reality and thus those who attempt suicide can benefit neither from the national health insurance nor from private insurance. In relation to this, a bond of sympathy should be formed in society on the fact that suicide is a medical problem that must be treated in order to form an atmosphere where suicide attempters can be treated.
(4) Social intervention is necessary to prevent suicide
Evidence should be prepared so that effective policies can be continuously implemented through studies of the effects of various policies implemented by the government to prevent suicide such as increased installations of screen doors in subways and the rearrangement of bridges in the Han River. It is necessary to prepare measures to prevent suicide at the level of society or the country through conducting
psychological autopsy of suicide deaths as in the case of Finland. Furthermore, at the present time point where there is no accurate data on the present state of depression and suicide, it is necessary to grasp the present situations by linking utilizable data such as the billing data of the Health Insurance Review & Assessment Service, death data from Statistics Korea and EMR from individual hospitals and prepare policy measures to create more concrete and wide ranged data as soon as possible.
4. Implementation
We produced and distributed leaflets in order to deliver the major messages to the general public and society, published papers on journals for clinical experts and policy decision makers, published manuscripts on the ‘Evidence and Value’ which is a newsletter for researchers and sent data collections to individuals or groups requesting them individually by mail or e-mail.
5. Discussion
The RTC is meaningful in that the outcomes of studies on the disease burdens of depression and suicide that can be utilized now were integrated to draw information necessary to society through multilateral discussion with related experts including clinical experts and that a foothold to have this stage of discussion continuously in future has been prepared. In addition, a social approach to perceive suicide as a social problem instead of a personal problem has been attempted and it was made into a public opinion to prepare an opportunity to induce social attention to it in future.
The data presented for depression treatment are billing data from the HIRA and thus the reliability of the data was doubted and the data were assumed to be manipulated definitions by patients. The data also have limitations in interpretation due to limitations in the data such as changes in the form of billing depending on the criteria for insurance benefits. Actual state surveys on the entire people including youths are necessary to prepare Korean style criteria for depression diagnoses that conform to the reality in Korea and consider cultural characteristics. To obtain information that can be utilized at this moment and is concrete, studies of policies will be necessary to prepare measures to link the billing data of the HIRA, death data from Statistics Korea and EMR from individual hospitals together and utilize them in research.
6. Conclusions
Through the RTC, the number of Korean people who are currently suffering from serious depressive disorders that require medical treatment is assumed to be larger than four millions but those who are actively receiving medical treatment are only around 10% of the 2 million patients and this leads to increasing rates of suicide and
increasing medical expenses. Therefore, suicide should be approached from medical viewpoints and social viewpoints and changes on social perceptions should accompany in relation to the fact that medical approaches are important to effectively prevent suicide.
To establish reasonable countermeasures against depression, we should urgently develop diagnostic tools reflecting Korean cultural characteristics, conduct periodic large scaled surveys of actual states and prepare policy measures to prepare related monitoring systems. Although there are large amounts of data related to depression and suicide that can be utilized now such as those in the HIRA and Statistics Korea, it is not easy to link data from each institution with each other. Therefore, it is necessary to prepare measures to link data from various institutions in the case of studies for public good by policy in Korea too. To prevent suicide, studies of continuous and wide ranged pan-national measures to prevent suicide are urgently
needed.
