한국여성에서 자궁경부암 선별검사로서 자궁경부 세포진 검사와 인유두종바이러스 검사의 경제성 분석
□ 서 론
우리나라 국가암검진에서는 만 30세 이상의 모든 여성이 2년마다 자궁경부 세포검사를 받는 것을 권고하고 있다. 국가암검진의 수검률은 매년 높아지고, 이로 인한 자궁경부암의 사망률 및 유병률은 감소하고 있는 추세이다. 한편 2012년 대한부인종양학회 및 대한병리학회에서는 20~29세 여성은 1년마다 자궁경부 세포검사를, 30~70세는 2년마다 자궁경부 세포검사 및 인유두종바이러스 검사를 받는 것을 권고하고 있다. 그러나 자궁경부암 검사의 정확도(accuracy) 및 비용효과 등과 관련된 국내 연구가 제한적이므로 자궁경부암 선별검사의 유용성을 파악하는 것이 필요한 실정이다. 이에 본 연구에서는 일반 여성에서 자궁경부암 선별검사의 정확도 비교 및 경제성 분석을 실시하여 우리나라의 자궁경부암 진단 가이드라인을 생성하기 위한 근거를 제시하고자 한다.
□ 자궁경부암 선별검진의 진단 정확도
(1) 자궁경부암 선별검진의 진단 정확도 평가를 위한 국내문헌 분석
국내에서 시행되는 자궁경부암 선별검진의 진단 정확도(diagnostic accuracy)를 파악하고자 한국 성인 여성을 대상으로 무증상이면서 선별 목적으로 자궁경부암 검사를 수행한 문헌을 선정하여 CIN(cervical intraepithelial neoplasia) 1+를 기준으로 민감도, 특이도를 추출하였으며 참고표준검사로는 질확대경검사(colposcopy) 및 조직학적(histology) 검사를 고려하였다. 진단 문헌의 비뚤림 위험평가는 두 명 이상의 검토자가 QUADAS-Ⅱ를 이용하여 독립적으로 평가하였다. 진단 정확도에 대해서는 개별 문헌별 민감도와 특이도의 상호관련성을 모형화한 SROC 곡선과 민감도와 특이도의 통합 추정치 산출을 위한 이변량 랜덤효과모형으로 메타분석을 수행하였다.
최종 선정된 33편 문헌의 비뚤림 위험 평가 결과를 살펴보면, 대부분 문헌의 선택 비뚤림이 불명확하게(unclear) 평가된 경우가 많았으며 적용성 또한 비뚤림 위험이 높음으로 평가된 경우가 많아 결과 해석에 주의가 필요하다. 자궁경부 선별검사방법별 이변량 랜덤효과모형으로 메타분석을 실시하여 정확도를 추정한 결과, 자궁경부 세포검사에서 ASCUS 이상을 양성기준으로 하였을 때 민감도 0.84, 특이도 0.78 이었으며, 인유두종바이러스 검사의 진단 정확도는 민감도 0.77, 특이도 0.79로 추정되었다. 인유두종바이러스 검사를 방법별로 나누어 분석했을 때, HCⅡ 검사방법의 민감도는 0.76, 특이도는 0.76으로 분석되었다.
(2) 자궁경부암 선별검진의 진단 정확도 평가를 위한 국외문헌 분석
국외에서 시행되고 있는 자궁경부암 선별검사의 진단 정확도를 평가한 2차 문헌(체계적 문헌고찰, 메타분석, 의료기술평가)을 중심으로 문헌을 검토하였다.
최종 선정된 59편의 문헌에 대하여 자궁경부암 선별검사별 정확도 분포를 살펴보면, 자궁경부 세포검사(ASCUS+)의 경우, 국내 요약 추정치보다 민감도가 낮았으나 특이도는 높았다. 국외문헌은 질병의 존재 여부에 대해 CIN 2+를 기준으로 파악 하였기에 국내 값보다 약간 낮은 경향을 보일 가능성이 있었으나, 이에 비해 특이도가 높게 추정되었다. 인유두종바이러스 검사방법별로 살펴보면, PCR방법은 국내 PCR방법에 비해 민감도와 특이도가 높았으며, HC Ⅱ는 몇 편의 낮은 수준을 제외하면 안정적으로 높은 수준을 나타내어 국내의 자궁경부 세포검사의 진단 정확도와 유사한 분포를 보였다. 병행검사의 진단 정확도는 문헌의 질이 낮은 2편을 제외하면 다른 검사법에 비해 민감도가 높았다. 국외 문헌을 통해 살펴본 선별검사 종류에 따른 정확도는 자궁경부 세포검사에 비해 PCR 방법, 병행검사 방법 등의 민감도가 높았다.
□ 자궁경부암 진단법의 경제성 분석
Ⅰ. 연구방법
국가 검진프로그램으로 고려할 수 있는 자궁경부암 검진전략에 대해 검사방법, 검사주기, 검사 시작 및 종료연령을 달리하여 경제성분석을 수행하였다. 검사방법으로는 자궁경부 세포검사를 하는 대안과 HPV triage(자궁경부 세포검사 이후 비정형 편평상피세포 발견 후 인유두종바이러스 검사시행)를 하는 검사를 고려하였다. 검사주기로는 1, 2, 3, 5년, 검사 시작연령은 20세 시작 및 30세 시작, 검사 종료연령은 평생 받는 경우와 79세에서 종료하는 조합을 고려하였다. 또한 HPV triage는 30세부터 시행하는 경우를 고려하였다.
비용-효용분석은 검진전략별 수명연장 수준과 삶의 질을 고려한 QALY를 적용하여 수행하였다. 분석관점은 사회적 관점에서 의료비용 및 시간비용, 간병비용, 교통비용 등을 모두 포함하였다. 분석주기는 1년으로 하고 분석기간은 평생으로 한 마콥 모형을 적용하였다.
변수의 불확실성 및 변동의 가능성이 있는 부분을 반영하기 위해서 수행한 민감도 분석 범위는 다음과 같다. 비용은 현시점에 발생하지만 편익이 먼 미래에 나타나 할인율이 크게 영향을 미칠 수 있기 때문에 할인율을 고려하였고, 전이확률(CIN 2/3 신환자가 1년 이내 자궁경부암으로 전이할 확률) 및 자궁경부 세포검사법의 진단 정확도를 달리하였을 때 비용효과 분석을 수행하였다.
Ⅱ. 연구결과
자궁경부암 검사방법, 검사주기, 시작 및 종료연령에 따른 24개의 자궁경부암 검진전략별로 1인당 비용과 생존연수 및 QALY를 산출하여 경제성을 확인하였다. 이를 위하여 효과 기준으로 정렬하여 우월성(dominance)과 확장된 우월성(extended dominance) 여부 및 점증적 비용-효용비를 산출하였다.
분석 결과 1인당 비용이 가장 낮은 전략은“PAP(5년 주기, 30세부터 79세까지)이며, 1인당 효과(LYG, QALY)가 가장 높은 전략은“PAP(1년 주기, 20세부터 79세까지)이었다. QALY를 기준으로 임계값이 3,000만원 수준인 경우“PAP(1년 주기, 30세부터 평생)”전략이 비용-효과적으로 파악되었다.
민감도 분석으로는 할인율, 전이확률 및 자궁경부 세포검사법의 진단 정확도를 고려하여 영향을 살펴보았다. 그 결과 할인율 0% 및 3.5% 일때는 각각“PAP(1년 주기, 20세부터 평생)”및 PAP(1년 주기, 20세부터 79세까지)”가 비용효과적인 전략이었으며, 할인율 7.5%일 경우는“PAP(2년 주기, 30세부터 평생)”전략이 비용효과적이었다. 진단 정확도가 낮을 경우“PAP(2년 주기, 20세부터 79세까지)”전략이 비용효과적이었으며, 진단 정확도가 높은 경우“PAP(2년 주기, 20세부터 평생)”전략이 비용효과적이었다. 한편, 전이확률을 달리 하였을 때는“PAP(1년 주기, 20세부터 79세까지)”전략이 비용효과적이었다. 이상과 같이 민감도 분석 결과 검사 시작연령 및 검사 주기에 차이는 있었지만 공통적으로 자궁경부 세포검사가 비용효과적인 전략이었다.
□ 결론
국내 자궁경부암 선별검사의 진단 정확도 메타분석 결과 자궁경부 세포검사의 민감도와 특이도가 각각 0.84, 0.78이였으며 연구별 정확도 차이가 상대적으로 적은 반면, 국외의 경우는 인유두종바이러스 검사방법의 진단 정확도가 상대적으로 높았다. 그러나 국내문헌의 대부분은 고위험군을 대상으로 2,3차 진료기관에서 이루어졌으며, 연구별 진단 정확도의 변이가 컸고 질 평가 결과 해석에 주의가 필요하다는 한계가 있다. 국가검진프로그램으로 고려할 수 있는 자궁경부암 검진전략들 중 효과 지표를 QALY로 하고 임계값을 3,000만원 수준으로 할 때 자궁경부 세포검사를 1년 주기로 30세에서 평생동안 실시하는 대안이 가장 비용 대비 효과적이었다. 그러나 분석에서 고려된 국내 문헌의 선택비뚤림 및 선별방법의 비뚤림 위험을 배제할 수 없다. 또한 민감도에 포함된 항목에 따라 비용효과성이 높은 전략이 다르기에 이에 대한 고려가 필요할 것이다. 향후 국내 자궁경부암 중재법별 대표 정확도를 파악하며 이를 기초로 한 경제성 분석을 위해서는 선별검사를 목적으로 내원하는 무증상 여성을 대상으로 보다 다양한 검진기관의 자료를 확보하는 것이 필요하다.
□ Introduction
National Cervical Screening Programme in Korea recommends that a biannual cervical pap smear test interval is appropriate for women over 30 years. Conventional cytology is relatively simple and cheap method of cervical cancer screening and eventually has reduced the cervical cancer incidence and mortality. However, several studies in abroad have shown the relatively high false negative rate and reported that HPV testing is more sensitive than cytology. The recent updated guidelines from Korean Society of Gynecology Oncology and Korean Society of Pathologists in 2012 recommends that annual pap smear test in the age group 20~29 years and biannual screening of pap smear and HPV test in the age group 30~70 years.
Although the most accurate or cost-effective cervical cancer screening methods is different in each country, there is a paucity of evaluative research in Korea. Therefore this study aims to suggest the evidence for guidelines of cervical cancer screening by comparing accuracy of cervical cancer test methods and performing cost-effectiveness analysis.
□ Accuracy of cervical cancer screening methods
Ⅰ. Systematic Review of Korean literature
To ascertain the accuracy of cervical cancer screening methods in Korea, we performed a systematic review and meta-analysis among asymptomatic women for cervical cancer screening. We extracted sensitivity and specificity for the detection of cervical intraepithelial neoplasia 1 (CIN 1) or worse and the reference standard was colposcopy and histology. Study quality was determined by using the Quality Assessment of Diagnostic Accuracy (QUADAS-II) by independent reviewers. Summary ROC curve and bivariate random effects model were applied to synthesize diagnostic test accuracy. The rating of risk of bias among 33 studies were as follows.
Most of studies were categorized into unclear in the selection criteria. In addition, the risk of bias in application was also high. Therefore, the caution should be exercised in the interpretation of results. Bivariate random effects meta analysis estimated the sensitivity and specificity for pap smear and HPV test as follows. The sensitivity and specificity was 0.84 and 0.78 respectively, in pap smear for detecting ASCUS or worse. In HPV test, the sensitivity and specificity was 0.77 and 0.79, respectively. According to HPV test methods, the sensitivity and specificity was 0.76, and 0.76 in HC II which was higher than that of HC I.
Ⅱ. Overview of systematic review in abroad
Overview of SR was conducted based on systematic reviews and meta-analysis of studies on the diagnostic accuracy of cervical screening methods. As a whole, the sensitivity of pap smear detecting ASCUS or worse was ranged 0.38~0.88, which was relatively low compared to pooled accuracy of studies in Korea.
As most foreign studies defined the disease endpoint CIN2+, sensitivity could be lower compared to Korean studies whose endpoint CIN1+. On the contrary, the diagnostic accuracy of HPV test was higher than that of Korean studies. The sensitivity was and specificity of HC II was generally higher than that of other screening methods.
□ Economic evaluation of cervical cancer screening strategy
Ⅰ. Methods
We assessed the cost-effectiveness of incorporate human papillomavirus (HPV) DNA testing into existing cervical cancer screening programme in South Korea. The model compared two management of screening methods: (1) Pap smear, (2) Triage with HPV DNA testing (HPV DNA screening test after atypical or abnormal pap results at routine cervical cancer screening). To compared current screening policy with new strategy: (1) screening interval (1 year, 2 year, 3 year, 5 year), (2) screening start age (20 year-old, 30 year-old) (3) screening period (life-time, till 79 year-old). We considered these kind of strategies combination.
We conducted cost-utility analysis applying QALYs to which takes into account life span expansion and the quality of life according to screening method, screening interval, screening start age.
With the societal perspective, patient time costs, caregiver costs, and transportation costs were all considered as well as medical costs. Markov model was used with one year cycle and life time analysis period.
Sensitivity analysis was conducted to reflect the uncertainty of variables. Sensitivity analysis was carried for discount rate, cervical cancer transition rate, screening efficacy.
Ⅱ. Result
Cost-effectiveness analysis of selected cervical cancer screening strategies with various screening interval, start age, screening period of each screening method. In order to assess the cost-effectiveness, the orders are based in effectiveness.
As a result, pap smear test with 5 year interval was most inexpensive strategy, pap smear test with 1 year interval was most effectiveness strategy.
In the South Korea, most of the interventions with an ICUR below KRW 30,000,000won/QALY are recommended routinely. Below on threshold 30,000,000won/QALY, pap smear test with 1 year interval (aged 30 to life-time) was most cost-effectiveness strategy.
Sensitivity analysis showed that discount rate, cervical cancer probability did not affect the result significantly. However, accuracy of cervical cancer screening led a change on whether to be cost-effectiveness.
□ Conclusions
To estimated the accuracy of cervical cancer screening methods in Korea, we performed a meta-analysis among asymptomatic women for cervical cancer screening. The estimated sensitivity and specificity for pap smear was 0.84 and 0.78, respectively, and the variability among studies using pap smear was relatively low compared with that of HPV test. In contrast, the diagnostic accuracy of HPV was higher than that of pap smear from studies performed in abroad. However, there was possibility of selection bias of domestic studies as most of them were targeted high risk population and were performed secondary or tertiary hospitals.
Cost-effectiveness analysis of selected cervical cancer screening strategies revealed that pap smear test with 1 year interval (aged 30 to life-time) was the most cost-effectiveness strategy with an ICUR below KRW 30,000,000won/QALY. It means that the period of age in cervical cancer screening should be setted as 30~life-time in terms of cost-effectiveness. However, we should consider that sensitivity analysis changed the most cost-effective strategy after applying for diagnostic accuracy.
For the estimation of representative diagnostic accuracy and economic evaluation of cervical screening methods, it is strongly warranted that there should be more studies in various medical institutions targeting among asymptomatic women for cervical cancer screening.
