1. 연구목적
류마티스 관절염 환자에서 항 CCP 항체 검사의 임상적 유용성에 대한 문헌 근거를 신속검토하여 관련 정책의 의사결정의 근거를 제공하고자 하였다.
2. 연구방법
본 연구에서는 국민건강영양조사 제4기 자료(2007-2009년)를 분석하여 국내 류마티스 관절염 환자 유병현황을 확인하고, 국외 의료기술평가보고서 및 가이드라인을 검토하여 선행 근거를 확인하였다. 그 다음으로, 미국 AHRQ에서 제안한 틀(그림 1)을 사용하여 기 발표된 체계적 문헌고찰을 검토하고, 한국인을 대상으로 한 문헌에 대한 기존의 체계적 문헌고찰에 추가적인 근거를 결합하여 최신의 근거를 제공하였다.
o 체계적 문헌고찰 분석(Overview of SR)
기 발표된 체계적 문헌고찰은 Pubmed와 The Cochrane Library에서 검색하였으며, 문헌 선택/배제 과정을 거쳐 선정된 총 9편의 체계적 문헌고찰에 대해 WHO나 AHRQ, COMPUS에서 선호하는 질 평가 도구인 AMSTAR로 문헌의 질평가를 수행하고 사전에 정의된 표준화된 양식을 이용하여 각 체계적 문헌고찰의 연구결과를 추출 및 정리 하였다.
o 한국인 대상 체계적 문헌고찰(Using SR)
한국인을 대상으로 한 항 CCP 항체 검사의 진단 정확도에 대한 임상적 효과 평가를 위해 기존의 체계적 문헌고찰을 활용한 체계적 문헌고찰(Using existing systematic review, using SR)을 수행하였다. 본 연구에서 2008년에 발표된 Lee 등의 체계적 문헌고찰연구에서 검색을 시행한 시점의 1년 전(2006년)부터 발표된 본 연구의 연구기준에 적합한 1차 연구를 MEDLINE, KoreaMed, 대한류마티스학회지에서 검색하였다. 문헌 선택/배제 과정을 거쳐 선정된 총 9편의 1차 연구에 대해 진단정확성 연구의 질평가 도구인 QUADAS II로 문헌의 질평가를 수행하였고, 사전에 정의된 표준화된 양식을 이용하여 각 체계적 문헌고찰의 연구결과를 추출하여 진단법 정확도에 대한 메타분석을 수행하였다.
3. 연구결과
o 국내 류마티스 관절염 환자 현황
우리나라의 류마티스 관절염 환자 현황을 파악하기 위하여 국민건강영양조사 제4기 (2007-2009년) 자료를 분석한 결과, 국내 류마티스 관절염 환자의 유병률은 평생 유병기준(지금까지 류마티스 관절염을 앓은 적이 있는 분율) 2.2%, 의사진단기준(평생 동안 의사로부터 류마티스 관절염을 진단받은 적이 있는 분율) 1.9%로 나타났다. 또한, 의사진단 기준 류마티스 관절염 환자의 평균 삶의 질 지수(EQ-5D index)는 0.803이었다.
이는 우리나라 일반인의 평균 삶의 질 지수인 0.933보다 낮은 값이며, 다른 질환과 비교 했을 때 고혈압(0.865), 당뇨병(0.852), 천식(0.842), 협심증(0.811) 보다 낮았다.
o HTA 보고서 및 가이드라인
류마티스 관절염의 진단을 위해 많은 국제적 임상 가이드라인과 국제적 보고서가 제시되어 왔으며, 국가와 기관마다 약간의 차이는 있겠으나 류마티스 관절염의 진단에 있어서 항 CCP 항체 검사가 필요하다고 일관되게 밝히고 있다.
아르헨티나 IECS(Augustovski F, 2007)에 따르면 항 CCP 항체 검사는 류마티스 인자가 음성이지만 류마티스 관절염이 의심되는 사람에게 사용될 수 있는 진단기준이며, 류마티스 관절염 진행에 대한 예후인자로 사용 시에 적절하다고 평가하고 있다. 프랑스 (HAS, 2007)에서도 항 CCP 항체 검사법은 류마티스 관절염의 진단과 예후평가에 적절하다고 하고 있으며, 프랑스 NHI에 항 CCP 항체 검사의 급여를 인정할 것을 제안했다.
브라질(da Mota, 2011)에서도 항 CCP 항체 검사가 류마티스 인자와 유사한 민감도와 높은 특이도로 인하여 그 진단적 마커로의 의미가 크다고 밝혔다. 또한, AHRQ의 CER 보고서(Wong, 2012)에서는 항 CCP 항체 검사가 진단되지 않은 근골격계 통증을 호소하는 어린이의 염증성 관절염이나 연결조직질환에의 유무에 대한 임상적 측정방법으로 쓰일 가능성이 있다고 말하고 있다.
또한 최근에 발표된 2010 ACR/EULAR 분류기준은 아직 타당도 검증이 되지는 않았지만, 1987 ACR 분류기준과 비교할 때 가장 다른 점은 항 CCP 항체 검사를 분류기준에 추가하였다는 것이며, 초기 류마티스 관절염 환자의 진단의 특이도를 높이는데 항 CCP 항체 검사가 기여할 것이라고 언급하고 있다.
o 체계적 문헌고찰 분석(Overview of SR)
기존에 발표된 항 CCP 항체 검사에 대한 9편의 체계적 문헌고찰을 분석한 결과, 검색한 DB나 검색기간, 연구 목적등의 차이가 있으며, AMSTAR에 의한 문헌의 질평가에 있어서 도 그 차이가 있었다. 하지만, 류마티스 관절염의 진단에 있어서 항 CCP 항체 검사가 류마티스 인자와 유사하거나 혹은 더 높은 수준의 민감도를 보이고, 특이도가 월등히 높은 것에 대한 이견이 없었다. 또한 항 CCP 항체 검사의 경우 1세대 보다는 2세대에서 민감도와 특이도가 더 높은 것으로 확인 되었으며, 조기의 류마티스 관절염 진단에 유의미하며, 추후 류마티스 관절염으로의 발전할 가능성을 예측하는데 도움을 주는 인자임을 결론짓고 있다.
특히, 적절성 평가 및 질평가 결과 최고 점수를 받은 Whiting 등(2010)에서 69편 문헌을 결합하여 류마티스 인자와 항 CCP 항체 검사를 비교한 결과, 항 CCP 항체 검사의 민감도는 0.67(95% 신뢰구간 : 0.64-0.70), 특이도는 0.95(95% 신뢰구간 : 0.94-0.96), 양성 우도비는 14.4(95% 신뢰구간 : 11.6-18.0), 음성 우도비는 0.35(95% 신뢰구간 0.32-0.38)로 특이도가 높았으며, 조기 류마티스 관절염 환자에서 15편의 코호트 문헌을 결합한 결과 항 CCP 2세대 항체 검사의 민감도는 0.57(95% 신뢰구간 : 0.51-0.63), 특이도는 0.96(95% 신뢰구간 : 0.93-0.97)였다. 그 다음 좋은 점수를 받은 Nishimura(2007)에서 항 CCP 항체 검사의 민감도는 0.67(95% 신뢰구간 : 0.65-0.68), 특이도는 0.95(95% 신뢰구간 : 0.95-0.96), 양성 우도비는 12.46(95% 신뢰구간 : 9.72-15.98), 음성 우도비 0.36(95% 신뢰구간 : 0.31-0.42)로 유사하였다.
o 한국인 대상 체계적 문헌고찰(Using SR)
최종 선정된 9편의 한국인 대상 항 CCP 항체 검사에 대한 1차 연구의 질평가 결과 연구유형이 환자대조군 연구 또는 단면연구였기 때문에 QUADAS II의 patient selection 영역에서는 비뚤림 위험이 높거나 불확실한 것으로 평가되었으며, 이를 제외한 항목의 경우 대부분의 연구에서 질이 높은 것으로 평가되었다. 즉, 최종 선정된 9편의 문헌 모두 전체적인 편향성이나 적용의 우려에서 낮음(low)으로 평가되어 적용이 가능한 것으로 평가되었다. 전체대조군을 대상으로 한 메타분석 결과, 통합 민감도는 0.76(95% 신뢰구간 : 0.73-0.79), 통합 특이도는 0.96(95% 신뢰구간 : 0.93-0.97), 통합된 민감도와 특이도로 계산된 양성 우도비(질병을 가지지 않은 사람이 이상소견을 보일 확률에 비해 질병을 가진 사람이 이상소견을 보일 확률)는 18.04(95% 신뢰구간 : 11.80-27.57), 음성 우도비(질병을 가지 지 않은 사람이 이상소견을 보이지 않을 확률에 비해 질병을 가진 사람이 이상소견을 보이지 않을 확률)는 0.25(95% 신뢰구간 : 0.23-0.28)로 나타났으며, 질환대조군을 대상으로 한 메타분석 결과, 통합 민감도는 0.79(95% 신뢰구간 : 0.73-0.83), 통합 특이도는 0.92(95% 신뢰구간 : 0.88-0.94), 통합된 민감도와 특이도로 계산된 양성 우도비는 9.43(95% 신뢰구간 : 7.00-12.71), 음성 우도비는 0.23(95% 신뢰구간 : 0.19-0.29)로 나타났다.
4. 결론
HTA 보고서 및 가이드라인, 기존에 발표된 체계적 문헌고찰, 한국인 대상 문헌을 대상으로 한 체계적 문헌고찰을 수행해 본 결과, 류마티스 관절염 진단에 있어 항 CCP 항체검사가 필요하고 유용하다는 일관된 결론을 보이고 있으므로, 항 CCP 항체 검사는 류마티스 관절염 –특히, 초기 류마티스 관절염의 진단의 특이도를 증가시켜서 환자들의 조기 치료에 기여할 것으로 사료된다.
1. Study objective
This study was aimed to promptly review the literature regarding clinical usefulness of anti-CCP antibody tests in patients with rheumatoid arthritis to provide the basis of decision makings in the relevant policies.
2. Methodology
In this study, data from the Fourth Korea National Health and Nutrition Examination Survey (KNHANES Ⅳ, 2007-2009) were analyzed to verify the prevalence of Korean patients with rheumatoid arthritis, and the previous basis was confirmed based on the review of overseas medical technology assessment reports and guidelines. Next, the existing systematic reviews were reviewed by using a frame suggested by the US AHRQ(Figure 1), additional basis was combined with the existing systematic reviews in Korean population, and eventually the latest basis was suggested.
□ Overview of systematic review analyses
Previously published systemic reviews were retrieved from Pubmed and Cochrane Library. After the procedures of inclusion/exclusion of studies, a total of 9 reviews were selected for which quality assessment was performed by using AMSTAR, a quality assessment tool preferred by the WHO, AHRQ, and COMPUS. Results of these systematic reviews were extracted and assorted by using the pre-defined standardized form.
□ Systematic reviews in Korean population
To evaluate clinical effects regarding the diagnostic accuracy of anti-CCP antibody tests in Korean population, using existing systematic reviews were carried out by using the primary systematic review studies. In this study, the primary studies that have been published since 2006, one year before the search was initiated in the systematic review by Lee et al.(published in 2008), and that met the inclusion/exclusion criteria of this study were retrieved from the MEDLINE, KoreaMed, and the Journal of the Korean Rheumatism Association. A total of 9 primary studies selected through the study inclusion/exclusion procedures were assessed for quality through QUADAS II, a quality assessment tool for diagnostic accuracy studies, results from each of the systematic reviews were extracted by using the pre-determined standardized form, and meta analysis was performed for diagnostic accuracy.
3. Study results
□ Status of patients with rheumatoid arthritis in Korea
To understand the status of patients with rheumatoid arthritis in Korea, data from the Fourth Korea National Health and Nutrition Examination Survey (KNHANES Ⅳ, 2007-2009) were analyzed. The results indicated that the prevalence of patients with rheumatoid arthritis in Korea was 2.2% based on lifetime prevalence (a proportion of patients who have had rheumatoid arthritis at any time to date) or 1.9% based on the physician’s diagnosis (a proportion of patients who have been diagnosed with rheumatoid arthritis by a physician at any time in their life). In addition, mean quality of life (EQ-5D index) in patients with rheumatoid arthritis based on the physician’s diagnosis was 0.803, lower than the mean quality of life index of 0.933 in general population, and also lower than that of patients with hypertension (0.865), diabetes mellitus (0.852), asthma (0.842), and angina pectoris(0.811).
□ HTA report and guidelines
A number of international clinical guidelines and international reports have been presented for diagnosis of rheumatoid arthritis. Although there may be some differences among countries and institutions, these documents consistently support that it is necessary to perform anti-CCP antibody tests for diagnosis of rheumatoid arthritis.
Argentina’s IECS (Augustovski F, 2007) notes that the anti-CCP antibody test can be used as a diagnostic criterion in people suspected to have rheumatoid arthritis although their rheumatoid factor is tested negative and that it is appropriate to use this test as a prognostic factor for the progress of rheumatoid arthritis. In France (HAS, 2007), anti-CCP antibody test is deemed appropriate for diagnosis and prognostic assessment of rheumatoid arthritis. It was suggested that the expenses of anti-CCP antibody tests should be reimbursed by the French NHI. In Brazil (da Mota, 2011), too, significance of the anti-CCP antibody test as a diagnostic marker was highly emphasized due to its high specificity as well as its sensitivity similar to that of rheumatoid factor. In addition, the AHRQ’s CER report (Wong, 2012) mentioned that this test could possibly be used clinically to detect inflammatory arthritis in children complaining musculoskeletal pain or the presence of connective tissue diseases.
Although the recently announced 2010 ACR/EULAR classification criteria are yet to be verified, the most notable difference from the 1987 ACR classification criteria is addition of the anti-CCP antibody test to the classification criteria where it is noted that the anti-CCP antibody test will contribute to a higher diagnostic specificity in patients with early rheumatoid arthritis.
□ Overview of systematic review analyses
Results from the analysis of the existing 9 systematic reviews on anti-CCP antibody tests indicated that their search DB, duration of search, and study objectives varied and that there were also differences in terms of quality assessment for literature by AMSTAR. Nevertheless, there was no objection to the fact that sensitivity of anti-CCP antibody is similar to or higher than that of the rheumatoid factor and anti-CCP antibody has a markedly high level of specificity in the diagnosis of rheumatoid arthritis. It was also confirmed that the second generation anti-CCP antibody has a higher sensitivity and specificity than the first generation. These reviews conclude that this test is relevant to early diagnosis of rheumatoid arthritis and can help predict the probability of further progress to rheumatoid arthritis.
Of particular note, the review by Whiting (2010) which received the highest scores in appropriateness and quality assessment combined 69 reports to compare rheumatoid factor and anti-CCP antibody. The results indicated that anti-CCP antibody had a 0.67 (95% confidence interval [CI] : 0.64-0.70) sensitivity and a 0.95 (95% CI : 0.94-0.96) specificity, with a positive likelihood ratio of 14.4 (95% CI : 11.6-18.0) and a negative likelihood ratio of 0.35 (95% CI : 0.32-0.38) which are also indicative of a high specificity. Fifteen cohort reports in patients with early rheumatoid arthritis were combined, from which the sensitivity and specificity of anti-CCP2 antibody were 0.57 (95% CI : 0.51-0.63) and 0.96 (95% CI : 0.93-0.97), respectively. The report by Nishimura (2007) that received the second best scores also showed similar results of anti-CCP2 antibody with a sensitivity of 0.67 (95% CI : 0.65-0.68), specificity of 0.95 (95% CI : 0.95-0.96), positive likelihood ratio of 12.46 (95% CI : 9.72-15.98), and negative likelihood ratio of 0.36 (95% CI : 0.31-0.42).
□ Systematic reviews in Korean population
Quality assessment was performed in the finally selected 9 primary studies on anti-CCP antibodies in Korean population, and the results indicated that bias risks in the patient selection area of QUADAS II were high or uncertain since the types of these studies were case control studies or cross-sectional studies. Except for this patient selection part, the studies were evaluated to have high quality in most of the other points. That is, all of the finally selected 9 primary studies had low levels of overall biases and application concerns, making these studies applicable.
Results from the meta analysis of overall control groups indicated that the pooled sensitivity was 0.76 (95% CI : 0.73-0.79), pooled specificity was 0.96 (95% CI : 0.93-0.97), and positive (the probability where a person with a disease may show an abnormality relative to the probability where a person without a disease may show an abnormality) and negative (the probability where a person with a disease may not show any abnormality relative to the probability where a person without a disease may not show any abnormality) likelihood ratios calculated from the pooled sensitivity and specificity were 18.04 (95% CI : 11.80-27.57) and 0.25 (95% CI : 0.23-0.28), respectively. Results from the meta analysis of disease control groups indicated that the pooled sensitivity was 0.79 (95% CI : 0.73-0.83), pooled specificity was 0.92 (95% CI : 0.88-0.94), and positive and negative likelihood ratios calculated from the pooled sensitivity and specificity were 9.43 (95% CI : 7.00-12.71) and 0.23 (95% CI : 0.19-0.29), respectively.
4. Conclusions
Results from the systematic review of HTA report and guidelines, existing systematic reviews, and systematic reviews of reports in Korean population support the consistent conclusion that the anti-CCP antibody test is both necessary and useful in the diagnosis of rheumatoid arthritis, and suggest that this test is expected to contribute to early treatment of patients with this disease by enhancing specificity in the diagnosis of rheumatoid arthritis, especially when the disease is in its early stage.
