▢ 연구배경
인구고령화, 국민건강증진, 삶의 질 향상 등으로 새로운 의료기술의 수요가 높아짐에 따라 과학기술분야 중에서 보건의료분야에 투입되는 연구개발비는 지속적으로 증가될 것으로 전망되고 있다. 하지만 2012년 보건의료분야 연구개발비의 중복투자에 대해 국가과학기술위원회의 지적이 있었고, 검증되지 않은 신개발 의료기술의 무분별한 확산으로 인해 새로운 문제들이 유발되고 있다. 이에 따라 기존의 미래예측방법론과 의료기술평가 기반의 탐색활동 방법론을 다각도로 활용하여 신개발 의료기술에 대한 탐색활동을 체계적으로 수행함으로써 유용하고 효율적인 신의료기술에 대한 보건의료분야의 시장진입을 촉진하고 미래의 불확실성으로 인한 오차범위를 최소화하기 위한 전략수립이 필요한 시점이다.
유사한 문제들을 해결하기 위하여 해외 선진국에서는 Early Awareness and Alert (EAA) 시스템을 도입하여 수행하고 있다. 한국보건의료연구원에서도 국민의 건강증진을 위하여 의료시장 도입 직전에 있는 신개발 의료기술에 대한 탐색활동을 진행하여 보건의료분야의 안전성ㆍ유효성을 포함한 파급효과와 사회적인 잠재적 영향력을 선제적으로 분석하고 이를 확산하기 위한 방안마련 연구가 수행된 바가 있다.
국내 보건의료분야에서 EAA 시스템의 도입은 국가차원의 신개발 유망의료기술의 발굴 기회를 부여할 뿐만 아니라 보건서비스 및 임상연구를 개발하는 자원의 효율적 분배와 잠재적 위해 혹은 오남용 가능성이 있는 의료기술의 무분별한 확산 방지에 기여하는 것이다. 이러한 신개발 유망의료기술의 탐색활동이 국내 실정에 맞게 원활히 수행함으로써 실제적인 순기능이 강화될 수 있는 구체적인 지원과 방안 마련이 시급한 실정이다. 따라서 시범사업을 운영하여 신개발 유망의료기술 탐색체계를 확립하고 이를 적용한 탐색활동을 실제 수행하고 향후 독립적인 사업으로 안착하기 위한 기반을 조성할 필요가 있다.
▢ 연구목적
본 연구의 목적은 2013년에 이미 개발된 국내 신개발 유망의료기술 탐색활동 운영체계를 실제로 시범운영하여 국내 실정에 최적화된 탐색체계를 마련하고자 하는 것이다. 또한 신개발유망의료기술 탐색활동에 대한 확산체계를 구축하고 다양한 국내외 협력기반을 조성하고자 한다.
이를 위하여 제1세부 연구에서는 신개발 의료기술 탐색활동과 관련된 국내 유관기관과 국제적 정보 네트워크를 구성하여 국내 보건의료분야 산업의 활성화를 지원하기 위한 정보의 확산 체계를 구축하고 다양한 국내 보건의료 산업 활성화를 지원하고자 한다.
또한, 제2세부 연구에서는 신개발 유망의료기술 탐색활동을 시범적으로 운영하여 각 단계별 절차에 대한 지속적인 수정 및 보완작업으로 국내 실정에 맞는 신개발 유망의료기술 탐색체계를 마련하고 최적화된 절차를 통하여 선제적으로 보건의료 정책결정자, 보건서비스 제공자, 산업계 및 국민에게 잠재적 영향력 분석 보고서를 제작하여 확산하고자 한다.
▢ 연구방법
제1세부에서는 국내 신개발 의료기술 지원 및 발전방안을 모색하고자 현재 진행되고 있는 신의료기술평가 사전상담제도의 방향성과 절차들을 요약·도식화하여 기본 포맷을 구성하고, 참여연구진, 전문가, 신의료기술평가사업본부 담당자 등의 의견수렴 단계를 거쳐 안내 자료를 개발하였다. 유관기관 간 협력 기반을 조성하기 위하여 한국보건산업진흥원(KHIDI) 및 한국과학기술정보연구원(KISTI), 연구성과실용화진흥원과 회의를 통한 국내 개발단계 의료기술 발굴 및 지원방안을 모색하였다. 국제적 네트워크인 EuroScan의 공식적인 대면회의 참석(연 3회)을 통하여, 수평탐색(Horizon Scanning) 활동의 역량 강화와 내부 및 대외적 활동에 대한 상호 논의를 기반으로 회원국 간에 합의하여 주요 사안들을 결정하였다. 확산 체계 구축을 위한 홈페이지의 개발은 과업지시서를 기반으로 업체와 대면회의를 총 4회 이상 진행하고 실무자 간에 전자우편 및 유선 보고 등을 통하여 의견교환 및 의사결정을 진행하고 시연을 통한 수정작업을 수행하였다.
제2세부에서는 신개발 유망의료기술 탐색도구(H-SIGHT Toolkit)를 마련하고 체계적인 탐색활동 수행을 위하여 2013년 연구를 통해 마련된 운영 안을 바탕으로 관련 문헌 및 해외 사례조사를 실시하였다. 특히 EuroScan Toolkit, AHRQ (Agency for Healthcare Research and Quality) 프로토콜 등의 세부내용을 검토하고, 국내 상황을 고려하여 식별 정보원 수집 및 선정 기준, 여과단계의 기준 등을 마련하였다.
또한, 관련 전문가 설문조사를 통하여 우선순위 선정 정보지(information form) 및 우선순위 선정 평가지(scoring form)를 개발하였다. 우선순위 선정기준 및 각 기준별 가중치 설정을 위하여 각계 전문가(의료계, 정부/보험자, 학계/연구자, 법조계, 관련 협회, 산업계, 환자/소비자 단체 등) 54명을 대상으로 델파이 방법으로 평가항목별 우선순위를 조사하여 그 결과에 따라 항목별 가중치를 설정하였다. 신개발 유망의료기술 탐색 자료원 및 잠재적 영향력 평가 항목을 선정하기 위하여 심층면접 형식의 보건의료분야 전문가 자문을 통하여 정보 수집기준을 마련하였다. 탐색활동과 관련하여 신의료기술평가위원회 위원 및 한국보건의료연구원 분야별 전문위원회 위원을 대상으로 각 기술에 해당하는 전공분야를 고려하여 전문가 위원회를 구성하고, 각 의료기술에 대한 우선순위 선정 및 잠재적 영향력 평가 자문회의를 진행하였다.
자체 개발된 탐색도구를 적용하여 연 2회에 걸쳐 총 6건의 의료기술에 대한 탐색활동을 수행하였으며, 내부 연구진 회의, 동료심사, 우선순위 선정 정보지(information form) 작성, 전문가 위원회 구성과 자문회의 개최, 최종 선정된 의료기술의 잠재적 영향력 분석, 전문가 자문회의를 통한 잠재적 영향력 평가, 관련업체의 내용검토, 보고서 발행 및 언론 보도 배포 등을 통하여 관련 협회 및 보건의료분야 관계자들에게 정보를 확산하였다.
2014년 2월, 신개발 유망의료기술 탐색활동 개선방안을 마련하기 위하여 관련 협회의 추천을 받아 총 10인으로 구성된 산업계 전문 자문위원을 위촉하고, 시범사업 기간 동안 뉴스레터 및 탐색활동의 성과물을 우편으로 제공하였다. 2014년 11월 17일 산업계 자문회의를 개최하여 탐색도구 및 탐색체계와 향후 발전방안을 논의하고, 설문 기반의 의견개진 방식을 통하여 각 분야 및 산업체 전문가 평가를 수행하여 의견을 수렴하였다.
▢ 연구결과
Ⅰ. 1세부: 신개발 유망의료기술 탐색활동 운영체계 개발
국내의 보건의료 기술개발에 대한 탐색활동은 공공과 민간분야 모두에서 취약하며, 최근 미래 유망 과학기술에 대한 관련 이슈, 트렌드, 시그널 등의 광범위한 수평조사(Horizon scanning) 등이 강조되며 예측활동이 활성화되고 있다. 하지만, 국내 보건의료시스템 내에서 새로 개발되는 의료기술에 대한 초기 평가는 기술 개발자의 신청에 의해서 ‘신의료기술평가’ 단계에서 최초 평가되며, 그 이전 단계에서는 시장에 진입할 새로운 기술에 대한 능동적인 예측활동은 부재한 상황이다.
미국, 캐나다, 호주와 뉴질랜드, 그리고 영국을 포함한 20여개의 유럽 국가에서는 이미 십 수 년 전부터 신개발 유망의료기술 탐색활동을 전담하는 조직을 운영 중이며, 독립형 탐색체계를 구축하고 있다. 특히 유럽 국가들의 경우, 국제적 네트워크인 EuroScan을 통하여 신의료기술 및 신개발 유망의료기술에 대한 정보 공유 및 정보 분석을 위한 방법론 개발을 위하여 협력하고 있다.
1. 국내 신개발 의료기술 지원 및 발전 방안 모색
한국보건의료연구원 신개발 유망의료기술 탐색연구팀은 한국보건산업진흥원 시행 ‘보건신기술(NET)인증제도’에 대한 이해를 통해 기관 간 정보교류 및 협력체계 구축이 필요하기 때문에, 신기술과 신의료기술 사이의 징검다리가 될 수 있는 「신의료기술평가 사전상담제도 안내자료」를 제작해 유관기관에 배포하였다. 또한, 보건신기술인증제도와 신개발 유망의료기술 탐색활동(Horizon Scanning Service of Innovative Global Health Technology, H-SIGHT)의 역할에 대한 명확한 기준을 수립하여 차별성을 강화함으로써 실제적인 산업계 지원을 위한 방법론을 강구하였다. 그러나 이러한 징검다리 역할만으로 보건신기술의 신의료기술평가 단계 진입 기간을 단축하거나 촉진하는 것에는 무리가 있어 산업계를 실질적으로 지원할 수 있는 방안을 모색하기 위해 유관기관을 중심으로 자문위원단을 구성하여 의견을 수렴하였다. 또한, 신개발 유망의료기술에 대한 잠재적 영향력 분석 결과가 신의료기술평가에 활용될 수 있도록 한국보건의료연구원 내 협력체계를 마련하기 위하여 내부 조정 및 회의를 진행하였다.
2. EuroScan 회원 활동
EuroScan은 유럽 국가를 중심으로 신개발 유망의료기술 탐색활동을 위해 조직된 의료기술평가 관련 국제적인 네트워크이다. 영국, 독일, 프랑스 등 유럽연합 주요국가와 호주, 캐나다가 회원국으로 참여하고 있으며, 한국보건의료연구원은 2013년 10월 EuroScan 회원가입을 승인 받고, 정식 회원 자격을 부여받았다. EuroScan은 매년 봄, 가을, 그리고 HTAi 연례 학술대회 기간을 포함하여 연간 총 3회의 정기 대면회의를 개최하고 있으며, 한국보건의료연구원은 2014년 봄부터 EuroScan의 회원국 자격으로 활동에 참여하고 있다.
EuroScan 활동은 내부활동(조직운영, 연구, 정보 및 지식 공유, 대외활동 전략수립 등)과 대외활동(업무협업 기관 INHATA, WHO 등과 교류)으로 구분되며, 세부 활동으로는 EuroScan Toolkit 업데이트, 회원국 간의 상호 활동에 대한 비교 분석, 회원기관 간 협업 사례 보고, EuroScan 홈페이지 이용현황 및 개편 방안에 대한 논의 등을 수행하였다. 대외활동으로는 국제기구 및 협의체(WHO, INAHTA, HTAi)와의 업무협약 체결을 통한 다양한 협력활동을 진행 중에 있으며, 한국보건의료연구원의 신개발 유망의료기술 탐색도구 개발과신개발 유망의료기술 탐색활동의 국제적 확산을 위한 활동을 펼치고 있다.
EuroScan 회원 가입 이후 적극적인 활동을 인정받아 2014년 10월, 집행위원회(Executive Committee) 선출 선거를 통하여 한국보건의료연구원 신개발 유망의료기술 탐색연구팀의 대표자가 간사(Registrar)로 선출되었으며, 임기는 2016년 9월까지이다.
3. 정보 확산을 위한 신개발 유망의료기술 탐색활동 자체 홈페이지 개발
국내에서 생소할 수 있는 신개발 유망의료기술 탐색활동을 소개하고, 보건의료분야 정책결정자, 의료진, 국민 등에게 신개발 의료기술에 대한 잠재적 영향력 분석 보고서 및 각종 소식지, 뉴스레터 등의 탐색활동 성과물들을 선제적으로 제공하기 위한 목적으로 홈페이지를 개발하였다.
신개발 유망의료기술 탐색활동 홈페이지는 탐색활동을 위한 정보 제안이 가능하며, 탐색활동의 성과물을 확산하도록 기획하였다. 신개발 의료기술에 대한 탐색 정보들을 담아 내국인을 위한 국문판과 국외 확산을 위한 영문판으로 구성하여 다양한 이해관계자들이 정보를 쉽게 이해하고 접근할 수 있도록 하였다.
수차례의 수정 및 보완절차를 거쳐 국문 홈페이지(http://neca.re.kr/hsight)는 2014년 7월 29일 정식 오픈하였으며, 한국보건의료연구원 보도자료 배포를 통해 공식적으로 홍보하였다. 또한 9월에 영문 홈페이지(http://neca.re.kr/hsight/eng/)를 구현하여 현재 안정적으로 운영 중에 있다.
Ⅱ. 2세부: 신개발 유망의료기술의 잠재적 영향력 분석연구
신개발 유망의료기술 탐색체계를 구축하기 위하여 미국, 영국 등 선진국의 사례를 조사하였으며, 해당 분야 전문가를 국내에 초청하여 워크숍을 개최하고 탐색활동 및 절차에 대한 심도 있는 논의를 진행하였다. 또한 수요자들의 요구를 반영하고, 맞춤형 정보를 제공하기 위하여 2013년 연구를 기반으로 개발된 탐색도구를 활용하여 2014년 당해 연구기간동안 신개발 유망의료기술 탐색활동을 시범운영함으로써 수차례의 수정보완 과정을 거쳐 신개발 유망의료기술 탐색체계를 확립하고, 각 단계별 세부적인 내용들을 완성하였다.
본 연구에서는 국내 실정에 맞게 최적화된 신개발 유망의료기술 탐색도구를 적용하여 신개발 의료기술의 식별 및 여과단계를 통하여 분석대상을 선별한 후, 우선순위를 선정하고 이와 관련한 임상적 근거 및 전문가 의견을 바탕으로 해당기술에 대한 잠재적 영향력을 분석하여 확산하였으며, 이러한 전 과정에 대하여 전문가 평가를 수행하여 개선방안을 도출하였다.
1. 신개발 유망의료기술 탐색도구(H-SIGHT Toolkit) 마련
신개발 유망의료기술 탐색체계를 확립하기 위하여 미국, 영국 등 선진국의 사례를 조사하고, 선진국의 탐색전문가를 초청하여 개최한 워크숍에서 탐색활동 및 절차에 대해 심도 있는 논의를 진행하였다. 이를 바탕으로 수차례의 수정․보완 과정을 거쳐 국내 보건의료 환경에 맞는 탐색체계를 구축하고 탐색활동에 필요한 도구(H-SIGHT Toolkit)를 마련하였다.
(1) 식별(Identification)
선진국 사례 및 국내 실정을 반영하여 사전에 선정해놓은 다양한 탐색 정보원을 바탕으로 지속적이고 광범위한 모니터링을 실시하고, 신의료기술평가 신청자료 및 외부 정보제안을 통해 수집된 자료에 대한 식별과정을 거친다. 국내 의료시장도입 예상시점 및 혁신성의 정도를 고려하여 탐색대상을 식별하고 선정기준을 미충족하는 정보는 탈락된다.
(2) 여과(Filtration)
여과 단계에서는 식별된 의료기술에 대하여 내부 전문가 검토를 통하여 신개발 유망의료기술이 아닌 의료기술을 거르는 과정을 거친다. 이 때 선정여부는 적절성(관련성), 혁신성, 국내 의료시장도입 가능성을 기준으로 판단하며, 필요에 따라서는 전문가 자문을 활용할 수 있으며 3인 이상의 관련 분야 연구자의 합의가 필요하다. 배제된 의료기술에 대해서는 관련 자료를 보관하여 추후에라도 선정기준에 부합될 경우 다시 여과 단계를 거치게 된다.
(3) 우선순위 선정(Prioritization)
여과 단계를 통해 선별된 의료기술에 대해서는 탐색활동 담당자가 우선순위 선정을 위한 정보지(information form)를 작성하며, 이를 바탕으로 전문가 자문회의를 통하여 우선순위를 정한다. 우선순위 선정 시 고려사항으로는 질병부담, 임상적 효과, 혁신성, 경제적 효과, 수용가능성, 사회적 파급력, 근거의 양 등이 포함되며 평가항목별 가중치 및 총점 등을 고려하여 우선순위가 결정된다.
(4) 잠재적 영향력 분석(Assessment)
우선순위 선정 단계를 거쳐 최종 선정된 의료기술은 현재까지의 근거 및 해당 분야 전문가 의견을 반영하여 국내 의료시장 도입 시 예상되는 긍정적 또는 부정적 잠재적 영향을 분석한다. 잠재적 영향력 분석 시에는 미충족 의료수요, 환자 건강상태 개선, 건강형평성 측면, 의료행태의 변화, 환자 및 임상의 측면에서의 수용가능성, 의료비용의 변화, 사회․윤리․법․정치․문화적 측면의 영향이 통합적으로 고려된다. 분석이 완료된 의료기술에 대해서는 해당 기술을 보유한 업체와의 논의를 통해 보고서를 수정․보완하는 과정을 거친다.
(5) 확산(Dissemination)
잠재적 영향력 분석이 완료된 연구결과는 수요자에 맞게 소식지, 보고서(간략보고서/심층보고서), 뉴스레터 등의 형태로 이메일과 홈페이지를 통하여 정보를 확산하게 된다. 확산 이후에도 상시 모니터링 및 피드백을 통하여 필요한 경우 관련 정보를 업데이트할 수 있다.

▢ Background
The increasing investment of research development expenditure in healthcare is predicted to continue in response to increased demand for new health technologies aimed at assisting aging populations, promoting public health, and improving quality of life. However, in 2012, the National Science and Technology Council (NSTC) has pointed out overlapping investments in research development expenditure on healthcare, and new problems are arising due to the spread of new health technologies. It is, therefore, necessary to establish a strategy to promote introduction of new, useful, and effective health technologies to the healthcare sector and to minimize the range of error due to uncertainty about the future. This may be achieved by systematic implementation of scanning activities for newly developed health technologies, making use of methodologies based on existing health technologies as well as future forecasting techniques.
In order to address these types of issues, developed countries have implemented an early awareness and alert (EAA) system. The National Evidence-Based Healthcare Collaborating Agency (NECA) performs horizon scanning activities relating to new health technologies just before their entry into markets and preemptively analyzes their safety and effectiveness in healthcare as well the potential impact of these technologies on society.
The horizon scanning activities, that provide opportunities to discover new and emerging healthcare technology at the natural level, were introduced in the domestic healthcare sector to contribute to an efficient allocation of health services and clinical research resources, and to prevent indiscriminate spread of health technologies with latent potential for harm or abuse. Supports and planning are urgently needed to strengthen the practical functioning of horizon scanning activities that identify new and emerging health technology by implementing in a manner appropriate to domestic conditions. Establishment of this horizon scanning system requires a pilot project and horizon scanning activities to build the groundwork for its safe use as an independent system in the future.
▢ Objectives
The purpose of this study was to develop an optimized horizon scanning system appropriate for Korea by implementing a pilot horizon scanning activities' operating system for new and emerging health technology developed in 2013.
In order to achieve this purpose, the first part of the study built collaboration with domestic agencies and an international network related to horizon scanning activities for new health technologies, constructing a system for dissemination of information to support activities of the domestic healthcare industry, and creating the basis of diverse domestic and international cooperation.
The second part of the study made continuous modifications to and took complementary measures for step-by-step procedures for the pilot study for horizon scanning activities of new and emerging health technology. This optimized horizon scanning procedure produced a customized report analyzing the potential impact of new and emerging health technology, which was disseminated to policy decision makers in healthcare, health service providers, the healthcare industry, and the public.
▢ Methods
As part of the first part study, a basic format was created for development of a fact sheet for the advanced consultation system at the Center for New Health Technology Assessment (nHTA) that would explore the current support and development plans for new domestic health technology by summarizing and making visual representations of its direction and procedures. The consensus of research staff, experts, the person leading the nHTA team, etc. was determined in an open discussion. In order to build the foundations of interagency cooperation, domestic health technology at the development stage was discovered and support measures were derived through meetings with the Korean Health Technology Portal Service (KHIDI), the Korean Institute of Science and Technology Information (KISTI), and the EuroScan, an international network, identifies key issues with its member countries via a consensus approach based on participation in official face-to-face meetings three times annually, which involve discussions about strengthening horizon scanning activities as well as the network’s internal and external activities. Development of a homepage for measures to build a dissemination system was based on task instructions developed during more than four face-to-face meetings with enterprises about project plans and progress.
In the method of the second part study, the H-SIGHT Toolkit was reviewed for scanning activities of new and emerging health technology. For review, international case studies were examined and an investigation of related literature was conducted based on the operating plans developed in 2013. Particularly, the detailed contents of the EuroScan Toolkit and the Agency for Healthcare Research and Quality (AHRQ) Protocol were considered and, based on domestic conditions, criteria were prepared for collection and selection of sources used to identify and filter information.
In addition, surveys of relevant experts allowed development of priority information and scoring forms. In order to set prioritization criteria and weighted values for each criterion, prioritization was examined using a Delphi approach that targeted 54 experts from every field (healthcare, government/insurer, academia/researcher, law, related institute, industry, patient/consumer organization, etc.); a weighted value for each criterion was set according to the results. With advice from healthcare experts collected through in-depth interviews, criteria for information gathering were prepared to select scanning resources for new and emerging health technology as well as items for assessing their potential impact. Expert committees were organized with consideration of the specialties pertaining to each technology. Members of nHTA and expert committee members in each field at NECA were targeted for these committees. Members participated in advisory meetings as part of the evaluation process, prioritizing and examining potential impact of each health technology.
By implementing the in-house-developed H-SIGHT Toolkit, horizon scanning activities of health technology were performed twice consecutively with a total of six cases. These results were disseminated to related institutions and healthcare personnel through internal research staff meetings, peer review, prioritization information forms, expert committees, and advisory meetings; expert advisory meetings, reviews of the associated industries, publication of reports, and distribution of press releases were used to assess the potential impact of the health technologies finally selected by this process.
With recommendations from related institutes, a total of 10 industrial experts comprised an advisory committee that prepared plans for improvement of horizon scanning activities for new and emerging health technology in February 2014. After sending requests via post newsletters, horizon scanning activities reports, and the results of the pilot project, an industry advisory meeting was held on November 17, 2014. Industry experts and experts from each field performed assessments and provided opinions, and a consensus on the H-SIGHT Toolkit, the horizon scanning system, and plans for future development was reached through open discussions based on surveys and face-to-face meetings.
▢ Results
Ⅰ. Development of an operating system for search new and emerging health technology
Currently, searching for health and medical technology development is very vulnerable in both public and private areas. Although predicting future emerging science technology has emphasized wide horizon scanning of technology-related issues, trends, and signals, it is difficult to collect information on new health technology to file beforehand, because early assessment of newly developed health technology in Korean health system is conducted only when new health technologies apply for assessment.
In the United States, Canada, Australia, New Zealand, and 20 European countries including the UK, dedicated horizon scanning organizations for searching new and emerging health technology have been operating for at least 10 years, and independent horizon scanning system have also been built. Particularly, countries in Europe are devoting their efforts towards developing methodologies for sharing and analyzing information of new and emerging health technology.
1. Support and development of new domestic health technology
Construction of systems for information exchange and cooperation among organizations are necessary for understanding the new health technology certification system from the Korea Health Industry Development Institute. In addition, promotional materials from a previous consultation have been produced and distributed to relevant organizations. The goal was to develop a methodology to support the industry by establishing definite criteria for its role of seeking new and emerging health technology and reinforcing its differential characters. However, there were some difficulties in establishing a support system for seeking methods to support entry of new health technology into the new health technology assessment step. Therefore, an advisory committee of relevant organizations was created to collect opinions on methods to support the industry for system settlement in NECA and to shorten the assessment period of new health technology. It was also intended to prepare methods to allow the system to settle in the future.
Internal modifications and conferences were also used to develop an internal system for cooperating within NECA.
2. EuroScan activity
EuroScan is an international network organization designed to assess health technology by performing horizon scanning activities for new and emerging health technologies centering on countries in Europe. Member countries include major the EU countries, such as the UK, Germany, and France, as well as Australia and Canada; NECA obtained EuroScan membership approval in October 2013 and qualified for formal membership. EuroScan holds regular physical meetings 3 times annually, in spring, autumn, and the international Health Technology Assessment (HTAi) conference. NECA has participated as a member since spring, 2014.
The activities of EuroScan member countries are classified into internal activities (organization operation, research, information and knowledge sharing, and strategizing external activities) and external activities (i.e., communication with other organizations such as the International Network of Agencies for Health Technology [INHATA] and the World Health Organization [WHO]). These activities include updating the EuroScan Toolkit, comparative analysis of mutual activities, cooperation case reports, and discussions on use and modification of the EuroScan homepage. In addition, to its other external activities, EuroScan has actively solicited opinions about and participated in activities with related and consultative organization (WHO, INHATA, HTAi) and is seeking to establish a strategy to lead Asian countries and expand the EAA system on the basis of development and EAA activity experiences. In order to share activity strategies between EuroScan members, its secretariat investigated future organization activity strategies in March, 2014 and shared information, such as the subjects and people in charge of the EuroScan DB and its homepage, as well as the current subjects of horizon scanning activities in order to prevent exchange or overlap between members.
The Executive Committee elected the representative of NECA H-SIGHT as a Registrar in October 2014 in acknowledgement of its activities after its member subscription in EuroScan; the Registrar will conduct activities of the Executive Committee for two years from 2015.
3. Development of H-SIGHT homepage
H-SIGHT homepage was developed with the main purpose of introducing activities of new and emerging health technology search (H-SIGHT) and to share information, such as potential impact assessment reports on newly developed health technology through domestic EAA system and products, such as various newsletters to policy decision makers of health and medical service, medical professionals, and national people preemptively. Therefore, the homepage was developed to provide and disseminate information. It was written in Korean to optimize information access and comprehension. It was also developed as English versions for external dissemination.
Through several modifications and complementary procedures, the Korean homepage of new and emerging health technology search (H-SIGHT) was officially opened on July 29, 2014. In addition, the English homepage (http://neca.re.kr/hsight/eng/) was released in September, 2014.
Ⅱ. The second detailed study: Analytical research on potential impact of new and emerging health technology
In order to build a horizon scanning system for new and emerging health technology, case studies were performed in the United States, the UK, and other developed countries. Experts in relevant fields were invited to Korea, workshops were held, and in-depth discussions were conducted relating to horizon scanning activities and procedures. Additionally, for the development of horizon scanning system for new and emerging health technology, in order to reflect consumer demands in advance and provide customized information, the H-SIGHT Toolkit, developed using the 2013 study results, was piloted to demonstrate horizon scanning activities for new and emerging health technology during the 2014 study period. In this way, through an iterative process of amendments and additions, the detailed contents were finalized for each stage.
In the present study, after identifying new health technology using a version of the H-SIGHT Toolkit optimized for domestic conditions, subjects for analysis were selected through filtering. After filtering, priorities were determined and an analysis on the potential impact of the technology was disseminated, based on related clinical evidence and expert opinions. An expert assessment of these procedures was carried out to prepare plans for improvement.
1. Preparing the H-SIGHT Toolkit
In order to build a horizon scanning system for new and emerging health technology, case studies were performed in the United States, the UK, and other developed countries. Experts in relevant fields were invited to Korea to participate in workshops and in-depth discussions relating to horizon scanning activities and procedures. A scanning system suited to the domestic healthcare environment was thus constructed through amendments and additions; finally, the H-SIGHT Toolkit necessary for horizon scanning activities was prepared.
(1) Identification
Based on a combination of cases in developed countries and domestic conditions, continued and widespread monitoring is performed based on various scanning sources selected in advance as well as identification of data collected through applications for new health technology assessment and external suggestions. Their expected timeline for entry into the domestic medical market and innovativeness are considered to identify targets for scanning; information that does not meet the selection criteria are removed from further consideration.
(2) Filtration
In this filtration step, an internal expert filters out health technologies other than new and emerging health technology. At this time, the selection criteria include relevance, innovativeness, and potential to enter the domestic market. Expert consultation is available if necessary, and agreement of at least three relevant investigators is required. Health technologies eliminated at this stage may go through this filtration step again after further development.
(3) Prioritization
The person in charge prepares an information form for the filtered health technology. Its priority is determined during an expert advisory meeting. Consideration items for prioritization include disease burden, clinical effect, innovativeness, economic effect, acceptability, social impact, and alternatives as necessary. The priority is ultimately determined by considering the total score of the weighted values of the assessment items.
