▢ 연구 배경
결장암은 결장에 생기는 악성 종양으로 우리나라 전체 암 발생의 7.18%를 차지한다. 결장암에서 가장 전통적인 수술법은 개복수술이나 1990년대 초 복강경 수술의 도입과 2000년대 초 로봇 수술의 도입으로 현재 국내에서는 세 가지 수술법이 모두 사용되고 있다.
식도암이란 식도에 생긴 암을 말하며, 국내에서 식도암의 발생률은 전체 암 발생의 0.9%으로 매우 낮은 편이다. 국내에서 식도암의 수술은 개흉수술이나 흉강경을 이용한 수술이 주를 이루고 있으며, 로봇수술은 도입 단계에 있다.
방광암은 방광에 생긴 악성종양을 말한다. 방광암은 국내 전체 암발생률 중 1.6%를 차지하며 비뇨기암 중 두 번째로 높은 발생률을 보인다. 방광암의 표준 수술법은 개복수술법이나 침습성이 높다는 단점이 있어 이를 극복할 수 있는 최소침습수술법이 연구되어 왔으며, 최근 들어 로봇수술을 이용한 최소침습수술이 시도되고 있다.
부신암은 부신피질에 발생하는 악성종양으로 국내 전체 암 발생의 0.1%로 발생이 매우 드물다. 부신암에서 로봇수술은 아직 도입단계이다.
신우암 및 요관암은 신우와 요관에서 발생하는 악성종양이며 국내 전체 암 발생의 0.38%를 차지한다. 신우요관암에서의 표준 수술법은 복강경 수술이며 로봇수술은 아직 도입 단계에 있다.
자궁암은 크게 자궁경부암과 자궁내막암으로 구분된다. 국내 암발생률 중 자궁내막암은 0.9%를, 자궁경부암은 전체 암 발생 중 1.7%를 차지하여 자궁경부암 발생률이 자궁내막암 발생률 보다 더 높다. 자궁내막암과 자궁경부암의 표준 수술법으로는 현재 개복수술 및 복강경수술이 널리 수행되고 있으며 로봇수술은 초기 자궁내막암과 초기 자궁경부암의 경우에 일부 적용되고 있다.
폐암은 폐에 생긴 악성종양으로 폐암은 국내 암 발생률 중 4위에 해당하며 약 10%를 차지한다. 폐암은 병기에 따라 비소세포폐암과 소세포폐암으로 구분되는데 수술적 치료는 비소세포폐암인 경우에 수행된다. 폐암에서의 표준수술법은 비디오흉강경 폐엽절제술이며, 로봇수술은 도입단계에 있다.
두경부암의 일부인 구강암은 구강 내 기관에 발생하는 암을 총칭하는 명칭이며, 인두암과 후두암은 각각 인두와 후두에서 발생하는 암이다. 구강암, 인두암, 후두암의 발생률은 각각 국내 전체 암 발생률 중 0.2%, 0.4%, 0.5%로 상대적으로 낮은 편이다. 인후두는 구조가 해부학적으로 복잡할 뿐 아니라 말하고 먹고 숨쉬는 등 중요한 기능을 수행하는 기관이라는 특성으로 인해 수술 시야 개선 및 기관 보존 등의 장점이 알려진 경구강로봇수술이 도입 단계에 있다. 인후두암은 조기에 림프절 전이가 일어나 암 병변 절제시 림프절 제거도 대부분 병행되며, 이때 후이개 접근을 이용한 로봇경부청소술은 수술후 환자의 기능과 미용에 효과를 볼 수 있다는 점 등으로 인해 관심을 받고 있다.
▢ 연구 목적
본 연구는 자궁암, 결장암, 식도암, 방광암, 폐 및 기관지암, 구강 및 인후두암, 부신 및 신우요관암에 대한 로봇수술의 임상적 안전성과 유효성을 평가함으로써 환자 및 보건의료 관련 종사자들에게 의사결정시 도움이 되는 과학적인 정보를 제공함을 목적으로 한다.
▢ 연구 방법
각 암종별(결장암, 식도암, 방광암, 부신 및 신우요관암, 자궁암, 폐 및 기관지암, 구강 및 인후두암)로 기존 수술법과 비교하여 로봇 수술법의 안전성 및 유효성 결과가 어떠한지 평가하기 위해 체계적 문헌고찰을 수행하였다. 국외의 3개 데이터베이스(Ovid-Medline, Ovid-EMBASE, Cochrane library)와 국내의 5개 데이터베이스(KoreaMed, KMbase, KISS, RISS, KisTi)를 사용하여 문헌을 검색하였으며, 검색전략의 적절성을 위해 임상전문가와 합의하에 검색전략을 수립하였다. 최종 선택문헌들의 비뚤림 위험 평가는 실험연구의 경우에는 Cochrane RoB를, 관찰연구의 경우에는 RoBANS ver 2.0을 사용하였다. 질평가후 최종 선택문헌의 치료효과크기를 추출하여 각 관심 의료결과에 대한 메타분석을 수행하였다.
▢ 연구 결과
⦁방광암에서 로봇수술은 개복수술에 비해 주요 합병증 및 전체 합병증 발생위험이 낮은 것으로 나타났고, 개별합병증 분석에서는 패혈증, 농양, 호흡부전의 발생위험이 더 낮았으나 협착 발생위험은 로봇수술 군이 개복수술 군보다 더 높았음.
⦁자궁내막암에서 로봇수술은 개복수술에 비해 전체 합병증, 수술중 합병증 및 수술후 합병증 발생 위험이 낮은 것으로 나타났고, 개별합병증 분석에서는 상처관련 합병증, 장폐색/폐쇄, 감염, 발열, 재입원, 중환자실입원, 수혈 발생위험을 낮추는 것으로 나타났으나 질구개 열개 발생위험은 더 높았음. 수술관련 결과분석에서 로봇수술은 개복수술에 비해 재원기간을 줄이고 출혈량을 감소시키는 효과를 보인 반면, 수술시간은 더 길었음. 복강경수술과의 비교에서는 로봇수술 군이 개복술로의 전환, 전체 합병증, 수술중 합병증, 수술후 합병증 발생위험이 낮은 것으로 나타났고, 개별합병증 발새 중 비뇨기계 손상 및 방광절개술 발생이 더 낮았음.
⦁자궁경부암에서 로봇수술은 개복수술에 비해 상처감염, 요로감염, 발열 및 수혈 발생위험이 더 낮았던 반면, 질구개 합병증 발생위험은 더 높게 나타남. 수술관련 결과에서는 재원기간을 줄이고 출혈량을 감소시키는 효과를 보인 반면, 수술시간은 더 길었음. 복강경수술과의 비교에서는 수혈과 합병증 발생위험만이 로봇수술 군에서 유의하게 더 낮게 나타남.
Ⅰ. 결장암
1. 로봇수술 vs 개복수술
결장암에서 로봇수술 대 개복수술의 분석에 포함된 문헌은 총 1편, 대상자수는 총 135명이었다. 안전성 지표 분석 결과 수술 후 30일내 사망과 수술 후 30일내 전체 합병증은 유의한 차이가 없었다. 유효성 지표 분석 결과 재입원율, 수혈여부, 절제 림프절 수는 유의한 차이가 없었으며, 수술 시간은 로봇수술이 유의하게 길었고(median 191.7 vs 136.2, <.0001), 재원기간은 유의하게 짧았으며(median 5.0 vs 8.0, <.0001) 출혈량은 로봇수술이 유의하게 적었다(median 6.1 vs 94.8, <.0001).
2. 로봇수술 vs 복강경수술
결장암에서 로봇수술 대 복강경수술의 분석에 포함된 문헌은 총 5편, 대상자수는 총 640명이었다. 안전성 지표 분석 결과에서 로봇수술 군과 복강경수술 군 간에 유의미한 차이가 나타난 지표는 없었다. 유효성 지표 분석 결과에서는 로봇수술 군이 개복수술 군에 비해 식이 시작일(MD -0.69일, 95% CI -1.11, -0.26, I2=0%), 가스배출 시작일(MD -0.53일, 95% CI -0.76, -0.30, I2=48%), 배변 시작일(MD -0.62일, 95% CI -0.77, -0.47, I2=69%), 재원일수(MD -0.64일, 95% CI -1.10, -0.18, I2=48%)를 줄이고 출혈량(MD -19.49ml, 95% CI -27.10, -11.89, I2=0%)을 감소시키는 것으로 나타났으며, 원위부 절제연((MD 2.59cm, 95% CI 1.40, 3.77, I2=0%)도 긴 것으로 나타났다. 반면 수술시간은 로봇수술이 길었다(MD 51.98분, 95% CI 39.59, 64.37, I2=10%).
Ⅱ. 식도암
1. 로봇수술 vs 개흉수술
식도암에서 로봇수술 대 흉강경수술의 비교에 포함된 문헌은 1편이었으며, 총 대상자 수는 37명이었다. 안전성 및 유효성 결과지표에 있어 통계적으로 유의한 차이를 보인 지표는 없었다.
2. 로봇수술
식도암에서 로봇수술의 환자군 연구에 포함된 문헌은 총 9편이었으며, 총 대상자 수는 235명이었다. 안전성 지표에서 통합 추정치는 사망(5.3%, 95%CI 2.4, 11.3), 개흉수술로의 전환(15.3%, 95% CI 9.0, 24.7), 폐렴(10.9%, 95%CI 6.0, 19.2), 성대마비(24.2%, 95% CI 16.6, 34.0), 심방세동(11.5%, 95% CI 7.0, 18.1), 문합누출(18.9%, 95% CI 13.7, 25.4), 농흉(7.6%, 95% CI 3.2, 16.9), 유미흉(9.2%, 95% CI 4.8, 16.7), 상처감염(7.5%, 95% CI 3.4, 15.7)이었다. 유효성 지표에서 절제 림프절 수, 수술시간, 출혈량, 재원기간, 중환자실 입원기간, 인공호흡기 사용일수가 보고되었으며 문헌 간 차이를 보였다.
Ⅲ. 방광암
1. 로봇수술 vs 개복수술
방광암에서 로봇수술 대 개복수술의 비교에 포함된 문헌은 총 19편이었으며, 총 대상자 수는 2,446명이었다. 안전성 지표 분석결과, 90일 이내 주요 합병증 발생률(RR 0.57, 95% CI 0.42, 0.76)과 90일 이내 전체 합병증발생률(RR 0.73, 95% CI 0.65, 0.82)이 개복수술 군과 비교하여 로봇수술 군에서 유의하게 낮았다. 개별합병증 발생에 있어서는, 로봇수술 군이 개복수술 군과 비교하여 패혈증(RR 0.25, 95% CI 0.11, 0.58), 농양(RR 0.49, 95% CI 0.24, 0.99), 호흡부전 발생률(RR 0.19, 95% CI 0.04, 0.78)이 유의하게 더 적었으나 협착 발생(RR 1.99, 95% CI 1.21, 3.28)에 있어서는 개복수술 군이 로봇수술 군보다 더 낮았다. 유효성 지표 분석 결과에서는 로봇수술 군이 개복수술 군과 비교하여 출혈량이 약 505ml(95% CI -654.37, -356.21, I2=60%) 더 적었으며 수혈률(RR 0.32, 95% CI 0.20, 0.52, I2=79%)도 더 낮았다. 이밖에도 로봇수술 군은 개복수술 군과 비교하여 가스배출일과 정상식사시작일, 재원기간을 각각 약 0.6일(95%CI -0.83, -0.46, I2=94%), 약 1.3일(95% CI -2.05, -0.57, I2=62%), 약 1.2일(95% CI -2.18, -0.31) 줄이는 것으로 나타났으나, 수술시간의 경우에는 로봇수술 군이 개복수술 군 보다 약 102분(95% CI 80.35, 124.85) 더 길었다.
2. 로봇수술 vs 복강경수술
방광암에서 로봇수술 대 복강경수술의 비교에 포함된 문헌은 총 3편이었으며, 총 대상자 수는 279명이었다. 안전성 지표 분석 결과에서 로봇수술 군과 복강경수술 군 간에 유의미한 차이가 나타난 지표는 없었다. 유효성 지표 분석 결과에서는 로봇수술 군이 개복수술 군에 비해 출혈량을 약 293ml(95% CI -589.54, -7.33, I2=96%) 줄이는 것으로 나타났으며, 재원기간을 5.23일(95% CI -7.23, -3.22) 줄이는 것으로 분석되었다.
Ⅳ. 부신암
부신암에서 로봇수술과 기존수술을 비교한 문헌은 없었다.
Ⅴ. 신우요관암
1. 로봇수술 vs 복강경수술
신우요관암에서 로봇수술과 복강경수술을 보고한 1편의 문헌이 있었으며 포함된 총 환자수는 44명이었다. 이 문헌에서 로봇수술은 복강경수술만큼 안전한 것으로 나타났다.
2. 로봇수술 vs 복강경수술 및 개복수술
신우요관암에서 로봇수술, 복강경수술, 개복수술을 비교한 문헌이 1편 있었으며 포함된 총 환자 수는 20명이었다. 이 문헌은 로봇수술과 기존수술법을 직접적으로 비교하지는 않았지만, 출혈량, 재원일수를 줄이는 것으로 나타났다.
Ⅵ. 자궁암
1. 로봇수술 vs 개복수술
자궁내막암에서 로봇수술 대 개복수술의 분석에 포함된 문헌은 총 19편, 대상자수는 총 3,062명이었다. 자궁내막암에서 로봇수술은 개복수술에 비해 전체 합병증(RR 0.37, 95% CI 0.28, 0.49), 수술중 합병증(RR 0.40, 95% CI 0.23, 0.72) 및 수술후 합병증(RR 0.49, 95% CI 0.36, 0.68) 발생이 유의하게 적었으며 개별합병증 발생 중 상처 감염(RR 0.12, 95% CI 0.05, 0.33), 상처 열개(RR 0.11, 95% CI 0.03, 0.37), 상처 합병증(RR 0.18, 95% CI 0.10, 0.33), 장폐색/폐쇄(RR 0.18, 95% CI 0.10, 0.32), 감염(RR 0.29, 95% CI 0.16, 0.54), 발열(RR 0.21, 95% CI 0.06, 0.75), 재입원(RR 0.44, 95% CI 0.26, 0.73), 중환자실 입원(RR 0.16, 95% CI 0.05, 0.56), 수혈(RR 0.28, 95% CI 0.19, 0.40) 발생이 더 적었고 질구개 열개/분리 발생 위험(RR 3.03, 95% CI 1.35, 6.80)은 더 높았다. 수술관련 지표 중 재원기간(MD -2.85, 95% CI -3.28, -2.41, I2=91%) 및 출혈량(MD -159.62, 95% CI -189.73, -129.50, I2=83%)은 유의하게 줄였으나 수술시간은 더 길었고(MD 44.15, 95% CI 20.91, 67.39, I2=98%) 종양학적 결과변수(생존율, 재발)에서는 두 군간 유의한 차이가 보고되지 않았다. 환자보고 결과변수에서는 한 편의 문헌들에서 진통제 사용량이 로봇군에서 유의하게 적은 것으로 보고하였으며(Hoekstra et al, 2009) 일상생활 복귀일(Bell et al, 2008)도 로봇군이 유의하게 짧은 것으로 보고하였다(mean 24.1 vs 52.0, < .0001).
자궁경부암에서 로봇수술대 개복수술의 분석에 포함된 문헌은 총 14편, 대상자수는 총 994명이었다. 로봇 수술은 개복수술에 비해 전체 합병증, 수술중 및 수술후 합병증 발생에 있어 유의한 차이가 나타나지 않았으며 질구개 합병증은 로봇수술이 개복수술에 비해 경계수준으로 유의하게 높은 발생률을 나타냈고(RR 3.16, 95% CI 1.00, 9.99) 개별합병증 발생 중 요로감염(RR 0.27, 95% CI 0.09, 0.82), 상처 감염(RR 0.24, 95% CI 0.06, 0.89), 발열(RR 0.28, 95% CI 0.12, 0.69) 및 수혈(RR 0.12, 95% CI 0.06, 0.24) 발생률이 로봇군에서 더 낮게 나타났다. 수술관련 결과변수에서는 로봇수술이 개복수술에 비해 재원기간이 더 짧았고(MD -3.95, 95% CI -5.78, -2.12, I2=96%) 출혈량이 더 적었으나(MD -326.72, 95% CI -440.31, -213.12, I2=90%) 수술시간은 더 길게 나타났으며(MD 27.94, 95% CI 0.48, 55.39, I2=95%) 골반 림프절절제수가 로봇군에서 평균 3.71개 가량 유의하게 더 적게 나타났다(95% CI –5.88, -1.53, I2=52%). 전체생존율 및 무병생존율은 3편의 문헌에서 유의미한 차이를 보고하지 않았으며 사망률과 재발률은 일부 문헌들에서 유의미한 차이를 보고하였으나 기저상태가 군간 차이가 있고 추적기간도 두 군이 달라 통합분석을 시행하지는 않았다. 환자보고 결과변수는 각 한 편의 문헌들에서만 보고되었는데 로봇군이 개복군에 비해 통증 점수가 더 낮았고(mean 2.5 vs 3.5) 마약성 진통제 사용률도 더 낮았으며(82% vs 100%) 진통제 사용기간(MD -18.7, 95% CI –22.59, -14.81)이 더 짧았고 업무복귀일(MD -22.90, 95% CI –33.38, -12.42)도 유의하게 줄이는 결과를 보고하였다.
2. 로봇수술 vs 복강경수술
자궁내막암에서 로봇수술 대 복강경수술의 분석에 포함된 문헌은 총 22편, 대상자수는 총 3,512명이었다. 종양학적 결과변수인 생존율과 재발의 경우 두 군간 유의미한 차이를 보고한 문헌이 없었다. 안전성 지표 분석결과에서는 개복술로의 전환(RR 0.35, 95% CI 0.23, 0.52), 전체 합병증(RR 0.73, 95% CI 0.54, 0.98), 수술중 합병증(RR 0.24, 95% CI 0.12, 0.48) 및 수술후 합병증(RR 0.73, 95% CI 0.54, 0.98) 발생이 유의하게 적었으며 개별합병증 발생 중 비뇨기계 손상(RR 0.20, 95% CI 0.04, 0.91) 및 방광절개술(RR 0.21, 95% CI 0.06, 0.74) 발생이 더 적었다. 수술관련 결과변수에서는 로봇수술이 복강경수술에 비해 재원기간이 더 짧았고(MD -0.50, 95% CI -0.94, -0.07, I2=88%) 출혈량을 94ml 가량 감소시켰다(95% CI -105.46, -82.86, I2=51%). 진통제 사용량은 한 편의 문헌(Leitao et al, 2013)에서 로봇군이 복강경 군에 비해 유의하게 적은 것으로 보고되었다.
자궁경부암에서 로봇수술대 복강경수술의 분석에 포함된 문헌은 총 8편, 대상자수는 총 505명이었다. 통합분석 결과 자궁경부암에서 로봇수술은 복강경수술에 비해 수혈 발생 위험은 더 낮았으나(RR 0.24, 95% CI 0.06-0.93) 그 외 안전성 및 유효성 지표에서 유의미한 결과는 나타나지 않았다. 또한 환자보고 결과의 경우 한 편의 문헌(Soliman et al, 2013)에서 주사용 진통제 사용량이 로봇군에서 유의하게 적은 것으로 보고되었다.
Ⅶ. 폐 및 기관지암
1. 로봇수술 vs 개흉수술
폐암에서 로봇수술대 개흉수술의 분석에 포함된 문헌은 총 2편, 대상자 수는 총 239명이었다. 폐암에서 로봇수술과 개흉수술과의 비교에서는 안전성 및 유효성에 있어 통계적으로 유의한 차이를 나타낸 결과가 없었다.
2. 로봇수술 vs 흉강경수술
폐암에서 로봇수술대 흉강경수술의 분석에 포함된 문헌은 총 5편, 대상자 수는 총 405명이었다. 폐암에서 로봇수술과 흉강경수술과의 비교에서는 안전성 및 유효성에 있어 통계적으로 유의한 결과를 보인 지표가 없었다.
Ⅷ. 구강 및 인후두암
1. 로봇수술 vs 개경수술
인후두암 원발암에서 경구강로봇수술 대 개경수술의 분석에 포함된 문헌은 총 4편, 대상자수는 총 199명이었다. 인후두암 원발암에서 경구강로봇수술은 개경수술에 비해 유리피판을 이용한 재건술율(RR 0.15, 95% CI 0.03, 0.81)과 수술시 기관절개율(RR 0.20, 95% CI 0.08, 0.05)이 유의하게 낮았고, 구강섭식까지의 기간(95% CI -13.40, -7.00)은 약 10.2일, 삽관제거까지의 기간(95% CI -10.05, -2.81, I2=78%)은 약 6.4일, 재원기간(95% CI -10.79, -6.11)은 8.5일, 수술시간(95% CI -222.39, -49.41)은 약 135.9분 유의하게 짧았다.
인후두암 재발암에서 경구강로봇수술 대 개경수술의 분석에 포함된 문헌은 총 2편, 대상자수는 총 139명이었다. 인후두 재발암에서 로봇수술을 이용한 구제술은 개경수술에 비해 2년 무병생존율/전체생존율(74% vs 43%; 각 p=0.01, 0.02), 절제면양성률(RR 0.32, 95% CI 0.14, 0.73), 수술시 기관절개율(RR 0.33, 95% CI 0.22, 0.49), 수술시 피딩튜브삽입율(RR 0.48, 95% CI 0.34, 0.68), 6개월시점 피딩튜브의존율(RR 0.27, 95% CI 0.12, 0.61)이 유의하게 낮았다.
2. 로봇수술 vs 경구강레이저미세수술
인후두암 원발암에서 경구강로봇수술 대 경구강레이저미세수술의 분석에 포함된 문헌은 총 2편, 대상자수는 총 53명이었다. 경구강로봇수술과 경구강레이저미세수술과의 비교에서는 안전성 및 유효성에 있어 통계적으로 유의한 결과를 보인 지표는 없었다.
3. 로봇경부청소술 vs 기존경부청소술
구강 및 인후두암으로 인한 림프절 전이 및 의심 환자를 대상으로 로봇경부청소술대 기존경부청소술의 분석에 포함된 문헌은 총 3편, 대상자수는 총 109명이었다. 로봇경부청소술은 기존경부청소술에 비해 환자의 미용만족도(95% CI 0.42, 1.78, I2=77%) 점수가 약 1.1점 유의하게 높았고, 수술시간(95% CI 64.90, 111.01, I2=78%)은 로봇경부청소술에서 88분 유의하게 길었다.
▢ 결론 및 정책적 제언
결장암에서 로봇수술과 개복수술의 비교는 보고된 문헌이 1편뿐으로 평가하기에 연구문헌이 부족하였다. 복강경수술과의 비교에서는 가스 배출일, 배변시작일 등 기능적 변수에서 로봇수술 군이 더 좋은 결과를 나타냈으나 현재 시점에서 명확한 결론을 내리기에는 근거가 부족하였다. 따라서 로봇수술의 치료성과 평가를 위하여 장기적 관점에서의 잘 설계된 무작위배정임상연구가 필요하다.
식도암에서 로봇수술과 기존 수술법 간의 안전성과 유효성을 비교평가한 연구는 1편뿐이었다. 현재 식도암에서 로봇수술은 국내에서 거의 시행되고 있지 않지만, 추후 새로운 로봇수술 장비가 도입되면 흉부외과 영역에서 식도암 로봇수술은 증가 할 것으로 예상된다.
방광암에서 로봇수술은 개복수술과 비교하여 주요 합병증 발생위험 및 패혈증 등의 개별 합병증 위험을 낮추는 것으로 나타났지만, 무작위배정임상연구의 결과와 코호트 연구에서의 분석 결과에 차이가 있어 현 상태에서 확증적 결론을 내리는 것은 불가능하였다. 방광암에서 로봇수술이 시행 초기단계임을 고려할 때 학습곡선 극복 후의 전향적 무작위배정 임상연구를 통한 치료성과 평가가 필요하다.
부신암 및 신우요관암에서 로봇수술과 기존 수술법 간의 안전성 및 유효성을 평가하기에는 근거가 매우 부족하였다. 평가를 위해서는 이 분야에서의 근거의 축적이 필요하다.
자궁경부암 및 자궁내막암에 있어서 로봇수술은 개복수술과 비교하여 상처관련 합병증 및 감염, 발열 등의 합병증 발생위험을 낮추는 것으로 나타났으나, 질구개 합병증 발생률은 더 높았다. 종양학적 결과에서는 차이가 없었으며, 재원기간과 출혈량은 줄이는 효과를 보인 반면 수술시간은 더 길었다. 복강경수술과의 비교에서는 자궁내막암의 경우 로봇수술 군이 개복술로의 전환, 전체 합병증, 수술중 합병증, 수술후 합병증의 발생위험이 더 적게 나타났다. 따라서 특히, 자궁내막암의 경우에는 로봇수술이 기존수술에 비해 더 안전한 잇점을 보였으나 전체적으로는 포함된 문헌의 방법론적인 질이나 현존하는 근거의 수준이 높지 않아 자궁암에서 로봇수술의 임상적 영향에 대해 명확한 결론을 내리기에는 제한적이었다. 추후 연구에서는 수술 특성이나 수술자의 능력이 환자 결과에 미치는 영향을 고려하여 임상적 결과를 비교분석해야 하며 장기간의 전향적 연구나 잘 설계된 무작위배정 임상연구를 통해 임상적 안전성 및 효과성에 대한 입증이 필요하다.
폐 및 기관지암에서 로봇수술과 기존 수술법 간의 유의한 효과를 보인 지표는 없었으며 안전성 및 유효성을 명확히 평가를 위해서는 이 분야에서의 근거 축적이 필요하다.
구강 및 인후두암에서 경구강로봇수술 및 로봇경부청소술과 기존 수술법 간의 안전성과 유효성에 대한 확증적 결론을 내리기에는 근거가 부족하였다. 경구강로봇수술은 구강 및 인후두암에서 유망한 경향을 보였지만, 로봇수술의 임상적 영향을 명확히 규명하기 위해서는 잘 설계된 전향적 추가연구가 필요하다.
▢ Background
Colon cancer is a malignant tumor that occurs in the colon and accounts for 7.18% of the overall cancer incidence rate in Korea. Open surgery is the most traditional procedure used for colon cancer, but with the introduction of laparoscopic surgery in the early 1990s and robotic surgery in the early 2000s, all three surgical methods are currently being used in Korea.
Esophageal cancer is a cancer of the esophagus with a very low overall cancer incidence rate of 0.9% in Korea. Surgery for esophageal cancer in Korea mainly consists of open surgery or video assisted thoracoscopic surgery (VATS), and robotic surgery is currently at the introductory stage.
Bladder cancer is a malignant tumor of the bladder that represents 1.6% of overall cancer incidence rate in Korea and shows the second highest incidence among urological cancers. Open surgery is the standard method used for bladder cancer. However, due to the drawback of its high invasiveness, minimally invasive surgery has been studied, while minimally invasive surgery utilizing robotic surgery has recently been attempted.
Adrenal cancer is a malignant tumor of the adrenal cortex that is extremely rare with an overall incidence rate of 0.1% in Korea. Robotic surgery for adrenal cancer is still at the introductory stage.
Pelvic cancer and ureter cancer are malignant tumors of the renal pelvis and ureter that account for 0.38% of the overall cancer prevalence in Korea. Laparoscopic surgery is the standard surgical method for renal pelvis and ureter cancer, while robotic surgery is still at the introductory stage.
Uterine cancer is largely divided into cervical cancer and endometrial cancer. Of the overall cancer incidence rate in Korea, endometrial cancer represents 0.9% and cervical cancer represents 1.7%. Open and laparoscopic surgeries are the standard surgical methods for these cancers and are widely performed currently, while robotic surgery is partially applied for early endometrial cancer and early cervical cancer.
Lung cancer is a malignant tumor of the lung that has the fourth highest cancer prevalence in Korea of about 10%. Lung cancer is classified into non-small cell carcinoma and small cell carcinoma according to clinical stage; surgical treatment is performed only for non-small cell carcinoma. A standard surgical method of lung cancer is VATS lobectomy, while robotic surgery is at the introductory stage.
Oral cancer, a head and neck cancer, is the generic term for cancers that occur in the oral cavity, and pharyngeal cancer and laryngeal cancer are those of the pharynx and larynx, respectively. Oral cancer, pharyngeal cancer, and laryngeal cancer manifest relatively low overall cancer incidence rate in Korea at 0.2%, 0.4%, and 0.5%, respectively. The laryngopharynx is an anatomically complex structure that plays an important role in speaking, eating, and breathing; as such, transoral robotic surgery (TORS), which has the advantages of improved visualization of the operating field and organ preservation, has been introduced. Lymph node metastasis occurs in an early stage of laryngopharynx cancer, and lymph node removal is also mostly performed at the same time with cancer lesion resection. Robot-assisted neck dissection (RAND) via the retroauricular approach has gained interest due to its favorable effects on function and cosmesis in patients after surgery.
▢ Objective
This study aimed to investigate the clinical safety and effectiveness of robotic surgery in uterine cancer, colon cancer, esophageal cancer, bladder cancer, lung and bronchial cancer, oral and laryngopharynx cancer, and adrenal and renal pelvis and ureter cancer to provide scientific information helpful in the decision-making process of patients and public health workers.
▢ Methods
A systematic review was conducted to assess the safety and effectiveness outcomes of robotic surgery compared to conventional surgical methods for each cancer (colon, esophageal, bladder, adrenal, renal pelvis, ureter, uterine, lung and bronchial, and oral and laryngopharynx). Studies were searched using three international databases (Ovid-Medline, Ovid-EMBASE, Cochrane library) and five domestic databases (KoreaMed, KMbase, KISS, RISS, KisTi), and search strategies were established under agreement with clinical experts to ensure their adequacy. An assessment for risk of bias for the selected studies was performed utilizing the Cochrane RoB in experimental studies and RoBANS version 2 for observational studies. After extraction of the treatment effect size of the chosen studies after quality assessment, a meta-analysis was performed for each outcome of interest.
▢ Results
⦁ In bladder cancer, robotic surgery showed low risks of major and overall complications compared to open surgery and also lower risks of sepsis, abscesses, and respiratory failure in individual complication analyses; however, the risk of stricture was higher in the robotic surgery group than the open surgery group.
⦁ In endometrial cancer, robotic surgery manifested a lower risk of overall, intraoperative, and postoperative complications compared to open surgery and lowered the risks of wound, intestinal obstruction/atresia, infection, fever, readmission, ICU stay, and transfusion complications in individual complication analyses but a higher risk of vaginal cuff dehiscence. In surgery-related outcome analyses, robotic surgery reduced hospital stay and blood loss but prolonged operative time compared to open surgery. Compared with the laparoscopic surgery group, the robotic surgery group exhibited a lower risk of conversion to open surgery as well as overall, intraoperative, and postoperative complications and a lower risk of urinary damage and cystotomy among individual complications.
⦁ In cervical cancer, robotic surgery had a lower risk of wound infection, urinary tract infection, fever, and transfusion but a higher risk of vaginal cuff complications than open surgery. In surgery-related outcome analyses, robotic surgery shortened the hospital stay and blood loss but delayed operative time. Compared with laparoscopic surgery group, only transfusion and complication risks were significantly lower in the robotic surgery group.
Ⅰ. Colon cancer
1. Robotic VS. Open surgery
One study of 135 subjects with colon cancer was included in our comparison of robotic surgery to open surgery. As a result of safety index analyses, 30-day mortality and overall complications after surgery did not differ significantly. Effectiveness index analyses revealed that readmission rate, the need for transfusion, and the number of retrieved lymph nodes did not differ significantly, whereas robotic surgery significantly increased operative time (median 191.7 vs 136.2, <.0001), shortened the hospital stay (median 5.0 vs 8.0, <.0001), and decreased the blood loss (median 6.1 vs 94.8, <.0001).
2. Robotic VS. Laparoscopic surgery
A total of five studies with 640 subjects with colon cancer were included in our comparison of robotic surgery to laparoscopic surgery. No significant difference was shown in any of the indices between the robotic and laparoscopic surgery groups on safety index analysis. On the effectiveness index analysis, the robotic surgery group had a reduced time to regular diet (MD -0.69 days, 95% CI -1.11, -0.26, I2=0%), time to flatus (MD -0.53 days, 95% CI -0.76, -0.30, I2=48%), time to bowel function recovery (MD -0.62 days, 95% CI -0.77, -0.47, I2=69%), hospital stay (MD -0.64 days, 95% CI -1.10, -0.18, I2=48%), and blood loss (MD -19.49ml, 95% CI -27.10, -11.89, I2=0%) and an increased distal resection margin (MD 2.59cm, 95% CI 1.40, 3.77, I2=0%) compared to the open surgery group but a prolonged operation time (MD 51.98 mins, 95%CI 39.59, 64.37, I2=10%).
Ⅱ.Esophageal cancer
1. Robotic VS. Thoracoscopic surgery
One study with 37 subjects with esophageal cancer was included in the comparison of robotic and thoracoscopic surgery. No statistically significant difference was shown in safety and effectiveness outcomes.
2. Robotic surgery
Nine studies with a total of 235 subjects with esophageal cancer were included in our analysis of robotic surgery. On the safety indices, integrated estimates indicated 30-daymortality (5.3%, 95%CI 2.4, 11.3), conversion to open surgery (15.3%, 95% CI 9.0, 24.7), pneumonia (10.9%, 95%CI 6.0, 19.2), vocal cord paralysis (24.2%, 95% CI 16.6, 34.0), atrial fibrillation (11.5%, 95% CI 7.0, 18.1), anastomotic leak (18.9%, 95% CI 13.7, 25.4), empyema (7.6%, 95% CI 3.2, 16.9), chylothorax (9.2%, 95% CI 4.8, 16.7), and wound infection (7.5%, 95% CI 3.4, 15.7). On the effectiveness indices, the number of retrieved lymph nodes, operative time, blood loss, hospital stay, ICU stay, and number of patients requiring respirator use were reported. Differences were seen among the studies.
Ⅲ. Bladder cancer
1. Robotic VS. Open surgery
A total of 19 studies with 2,446 patients with bladder cancer were included in the comparison of robotic surgery to open surgery. The safety index analyses showed that the 90-day major and overall complication incidences (RR 0.57, 95% CI 0.42, 0.76; RR 0.73, 95% CI 0.65, 0.82) were significantly lower in the robotic surgery group than in the open surgery group. Inindividual complications, the robotic surgery group showed significantly lower incidences of sepsis (RR 0.25, 95% CI 0.11, 0.58), abscess (RR 0.49, 95% CI 0.24, 0.99), and respiratory failure (RR 0.19, 95% CI 0.04, 0.78) than the open surgery group, whereas the open surgery group manifested a lower stricture incidence (RR 1.99, 95% CI 1.21, 3.28) than the robotic surgery group. The effectiveness index analyses revealed that the robotic surgery group had less blood loss (MD 505, 95% CI -0.39, 7.08, I2=60%) and a lower transfusion rate (RR 0.32, 95% CI 0.20, 0.52, I2=79%) than the open surgery group. In addition, the robotic surgery group had a decreased time to flatus, decreased time to regular diet, and shorter hospital stay at 0.6 (95%CI -0.83, -0.46, I2=94%), 1.3 (95% CI -2.05, -0.57, I2=62%), and 1.2 (95% CI -2.18, -0.31) days, respectively, compared to the open surgery group. The operative time was approximately 102 minutes (95% CI 80.35, 124.85) longer in the robotic surgery group than the open surgery group.
2. Robotic VS. Laparoscopic surgery
A total of three studies with 279 subjects with bladder cancer were included in the comparison of robotic and laparoscopic surgery. The safety index analyses revealed no significant difference between groups. The effectiveness index analyses showed that the robotic surgery group had reduced blood loss (MD 293, 95% CI -589.54, -7.33, I2=96%) and hospital stay (MD 5.23, 95% CI -7.23, -3.22) compared to the open surgery group.
Ⅳ. Adrenal cancer
There was no study comparing robotic surgery and existing surgery in adrenal cancer.
Ⅴ. Renal pelvis and ureter cancer
1. Robotic VS. Laparoscopic surgery
Only one study with a total of 44 patients reported the use of robotic and laparoscopic surgery for renal pelvis and ureter cancer. This study showed that robotic surgery was as safe as laparoscopic surgery.
2. Robotic VS. Laparoscopic and open surgery
One study with 20 patients compared robotic, laparoscopic, and open surgery for renal pelvis and ureter cancer. Although it did not directly compare robotic and conventional surgery, robotic surgery reduced blood loss and hospital stay.
Ⅵ. Uterine cancer
1. Robotic VS. Open surgery
In endometrial cancer, a total of 19 studies with 3,062 subjects were included in the comparison of robotic surgery to open surgery. Robotic surgery had a significantly lower incidence of overall complications (RR 0.37, 95% CI 0.28, 0.49), intraoperative complications (RR 0.40, 95% CI 0.23, 0.72), and postoperative complications (RR 0.49, 95% CI 0.36, 0.68) compared to open surgery. Among the individual complications, robotic surgery also showed a lower incidence of wound infection (RR 0.12, 95% CI 0.05, 0.33), wound dehiscence (RR 0.11, 95% CI 0.03, 0.37), wound complications (RR 0.83, 95% CI 0.19, 3.66), ileus/bowel obstruction (RR 0.18, 95% CI 0.10, 0.32), infection (RR 0.29, 95% CI 0.16, 0.54), fever (RR 0.21, 95% CI 0.06, 0.75), readmission (RR 0.44, 95% CI 0.26, 0.73), ICU stay (RR 0.16, 95% CI 0.05, 0.56), and transfusion (RR 0.28, 95% CI 0.19, 0.40) but a higher risk of vaginal cuff dehiscence/separation (RR 3.03, 95% CI 1.35, 6.80). In peri-operative outcomes, hospital stay (MD -2.85, 95% CI -3.28, -2.41, I2=91%) and blood loss (MD -159.62, 95% CI -189.73, -129.50, I2=83%) were significantly decreased in robotic surgery, whereas operation time (MD 44.15, 95% CI 20.91, 67.39, I2=98%) was increased, while the oncologic outcomes such as survival rate or recurrence did not show a significant difference between the two groups. In patient report outcomes, Hoekstra et al (2009) reported that a significantly less pain medications was used in the robotic surgery group, while time to return to daily life (Bell et al, 2008) was also significantly shorter in the robotic surgery group.
In cervical cancer, a total of 14 studies with 994 subjects were included in the comparison of robotic surgery to open surgery. In safety outcomes, the incidence of overall complications, intraoperative complications, and postoperative complications showed no significant difference. Vaginal cuff complications (RR 3.16, 95% CI 1.00, 9.99) showed a marginally significantly higher incidence in robotic surgery compared to open surgery; among individual complications, the incidences of urinary tract infection (RR 0.27, 95% CI 0.09, 0.82), wound infection (RR 0.24, 95% CI 0.06, 0.89), fever (RR 0.28, 95% CI 0.12, 0.69), and transfusion (RR 0.12, 95% CI 0.06, 0.24) were lower in the robotic surgery than in the open surgery group. In peri-operative outcomes, robotic surgery showed shorter hospital stay (MD -3.95, 95% CI -5.78, -2.12, I2=96%) and less blood loss (MD -326.72, 95% CI -440.31, -213.12, I2=90%) than open surgery but longer operation time (MD 27.94, 95% CI 0.48, 55.39, I2=95%) and significantly lower number of retrieved pelvic lymph nodes (MD 3.71, 95% CI –5.88, -1.53, I2=52%). Overall survival and disease free survival were reported no significant difference in three studies, whereas mortality and recurrence rates differed significantly in some studies. However, it was reported that the outcome should be carefully interpreted because of differences in underlying conditions and follow-up duration between the two groups. Patient-reported outcomes included per one study showed a lower pain score (mean 2.5 vs 3.5), lesser amount of opioids use (82% vs 100%), shorter duration of analgesic use (MD -18.7, 95% CI –22.59, -14.81), and significantly shorter time to a return to work in the robotic compared to the open surgery group.
2. Robotic VS. Laparoscopic surgery
In endometrial cancer, a total of 22 studies with 3,512 subjects were included in the analysis of robotic versus laparoscopic surgery. No studies reported a significant difference in oncological outcomes (survival rate, recurrence) between the robotic and laparoscopic surgery groups. The incidence of conversion to open surgery showed a significantly lower in the robotic surgery group (RR 0.35, 95% CI 0.23, 0.52), overall complications (RR 0.73, 95% CI 0.54, 0.98), intraoperative complications (RR 0.24, 95% CI 0.12, 0.48), and postoperative complications (RR 0.73, 95% CI 0.54-0.98) in the robotic group compared to the laparoscopic group; of the individual complications, the incidence of urinary damage (RR 0.20, 95% CI 0.04, 0.91) and cystotomy (RR 0.21, 95% CI 0.06, 0.74) was lower as well. In perioperative outcomes, robotic surgery shortened the hospital stay (MD -0.50, 95% CI -0.94, -0.07, I2=88%) compared to laparoscopic surgery and had a reduced blood loss (MD 94, 95% CI -105.46, -82.86, I2=51%). One study (Leitaoet al., 2013) reported that significantly smaller amounts of fentanyl were used in the robotic group.
In cervical cancer, a total of eight studies with 505 subjects were included in the comparison of robotic and laparoscopic surgery. Pooled analyses showed that robotic surgery group had a lower risk of transfusion (RR 0.24, 95% CI 0.06, 0.93) than in the laparoscopic surgery group, but there were no significant difference in other safety and effectiveness outcomes. One study (Soliman et al, 2013) reported that the amount of intravenous analgesic use was significantly lesser in the robotic group.
Ⅶ. Lung and bronchial cancer
1. Robotic VS. Open surgery
A total of two studies with 239 subjects were included in the comparison of robotic and open surgery for lung cancer. In the comparison between robotic and open surgery for lung cancer, there was no statistically significant difference in the safety and effectiveness indices.
2. Robotic VS. Thoracoscopic Surgery
Five studies with 405 subjects with lung cancer were included in the comparison of robotic and thoracoscopic surgery. In the comparison between robotic surgery and thoracoscopic surgery for lung cancer, there was no statistically significant difference in safety and effectiveness indices.
Ⅷ. Oral and laryngopharynngeal cancer
1. TORS VS. Open neck resection
A total of four studies with 199 subjects with primary laryngopharyngeal cancer were included in the comparison of TORS to open surgery. In primary laryngopharyngeal cancer, TORS showed a significantly lower free flap reconstruction rate (RR 0.15, 95% CI 0.03, 0.81) and tracheostomy rate at the time of surgery (RR 0.20, 95% CI 0.08, 0.05) compared to open surgery and had a significantly shorter time to oral diet of 10.2 days (MD -10.2, 95% CI -13.20, -7.00), time to decannulation of 6.4 days (MD -6.4 days, 95% CI -10.05, -2.81, I2=78%), hospital stay of 8.5 days (MD -8.5, 95% CI -10.79, -6.11), and operative time of 135.9 minutes (MD -135.9, 95% CI -222.39, -49.41).
Two studies with 139 patients with recurrent laryngopharyngeal cancer were included in the comparison of TORS to open surgery. In recurrent laryngopharyngeal cancer, salvage surgery using robotic surgery manifested significantly lower 2-year disease-free survival rate (74% vs. 43%, p=0.01), 2-year overall survival rate (74% vs. 43%, p=0.02), positive margin rate (RR 0.32, 95% CI 0.14, 0.73), tracheostomy rate at the time of surgery (RR 0.33, 95% CI 0.22, 0.49), feeding-tube rate at the time of surgery (RR 0.48, 95% CI 0.34, 0.68), and feeding tube dependence rate at six-months after surgery (RR 0.27, 95% CI 0.14, 0.73) compared to open surgery.
2. TORS VS. Transoral Laser Microsurgery (TLM)
Two studies with 53 subjects with primary laryngopharyngeal cancer were included in the comparison of TORS to Transoral Laser Microsurgery (TLM). We found no statistically significant difference in the safety and effectiveness indices of TORS versus TLM.
3. Robot-Assisted Neck Dissection (RAND) VS. Conventional transcervical neck dissection
A total of three studies were included in the comparison of RAND to conventional transcervical neck dissection in 109 patients with lymph node metastasis or suspected lymph node metastasis due to oral and laryngopharyngeal cancer. RAND showed significantly higher cosmetic satisfaction by 1.1 points (95% CI 0.24, 1.78, I2=77%) than conventional transcervical neck dissection and a significantly longer operation time by 88 minutes (95% CI 64.90, 111.01, I2=78%).
▢ Conclusions
In colon cancer, only one study reported a comparison of robotic and open surgery; therefore, it was not sufficient for the assessments. Comparison of laparoscopic and robotic surgery revealed more favorable outcomes in functional variables such as time to flatus and time to bowel movement, but sufficient evidence to make a clear conclusion is currently lacking. Therefore, a well-designed randomized clinical study with a long-term perspective is needed to assess robotic surgery treatment outcomes.
In esophageal cancer, one study compared the safety and effectiveness of robotic and conventional surgical methods used. Robotic surgery is currently rarely performed for esophageal cancer in Korea, but its use is expected to increase in cardiothoracic surgery once new robotic surgery devices are introduced in the future.
In bladder cancer, robotic surgery exhibited a lowered risk of major and individual complications including sepsis compared to open surgery. However, the result of a randomized clinical study was discrepant from that of a cohort study, thereby making it currently impossible to draw a confirmed conclusion. Considering that robotic surgery for bladder cancer is at an initial stage of implementation, a treatment outcome assessment through a prospective randomized clinical study after overcoming the learning curve is necessary.
In adrenal cancer, renal pelvis and ureter cancer there was a substantial lack of evidence in the assessment of safety and effectiveness between robotic and conventional surgical methods. Therefore, the accumulation of further evidence is required.
In cervical and endometrial cancer, robotic surgery was shown to lower the risk of wound complications, infection, and fever compared to open surgery but manifested a higher incidence of vaginal cuff complications. Oncological outcomes did not show a significant difference, and robotic surgery reduced the hospital stay and blood loss but prolonged the operative time. In the comparison with laparoscopic surgery for endometrial cancer, the robotic surgery group exhibited a lower risk of conversion to laparotomy as well as overall, intraoperative, and postoperative complications. Particularly in endometrial cancer, robotic surgery may be safer than conventional surgery. However, methodological quality and the level of evidence of the included studies are not high; therefore, we are unable to make a clear conclusion regarding the clinical effects of robotic surgery in uterine cancer. Further prospective studies or well-designed randomized clinical studies are required to validate these findings.
In lung and bronchial cancer, no indices showed a significant effect between robotic and conventional surgical methods, and it is necessary to accumulate evidence in this field to more clearly investigate safety and effectiveness.
In oral and laryngopharynngeal cancer, there was not enough evidence to draw a definite conclusion regarding safety and effectiveness between robotic surgery and conventional surgical methods. TORS is promising for oral and laryngopharyngeal cancer, but a well-designed prospective study is also required to clearly elucidate the clinical effects of robotic surgery.
