▢ 연구 배경
국내에 로봇수술이 도입된 2005년 이후 가장 가파른 증가를 보인 분야는 비뇨기계로 전체 로봇수술 건수의 33.4%를 차지하고 있다. 그 중에서도 전립선암에서 로봇수술이 가장 활발히 이루어지고 있으며 로봇수술의 여러 장점으로 인하여 앞으로도 지속해서 그 사용이 증가할 것으로 예상되고 있다. 국외에서 발표된 체계적 문헌고찰연구결과에 따르면 로봇수술은 기존수술(복강경수술, 개복수술)에 비하여 기능적 결과 즉 성기능개선과 요자제 회복에 있어서 우수한 것으로 나타났고(Ficarra V 등, 2012), 본 연구원에서 수행한 체계적 문헌고찰에서는 기능적 결과뿐 아니라 안전성 측면에서도 더 안전하고, 재원일수와 출혈량 등 수술직후 결과도 향상시키는 것으로 분석되었다(이선희 등, 2014). 그러나 비용 효과적인 면에서 로봇수술은 아직까지 논란이 있다.
정부에서는 고액진료비가 발생하는 4대 중증질환에 대한 보장성확대를 추진 중에 있는데 로봇수술을 이용한 암수술도 4대 중증질환 보장성 강화 계획에 포함되어 있다. 그러나 국내의 자료를 활용한 경제성분석 결과는 보고된 바 없으며, 로봇수술의 보장성확대에 대하여는 찬반여론이 있는 실정이다.
▢ 연구 목적
본 연구는 국내 전립선암 환자에서 로봇보조 근치적전립선절제술의 임상적 효과를 기존수술법(복강경수술, 개복수술)과 비교하고 경제성 분석을 시행하여 국가 보건의료 정책 수립시 근거자료를 제공하고자 하였다.
▢ 연구 방법
첫째, 국내에서 수행되는 근치적전립선절제술 방법별 임상적 안전성 및 유효성을 파악하기 위하여, 2010년과 2011년에 5개 병원에서 임상병기 T1~T3 전립선암으로 진단받고, 근치적전립선절제술을 시행받은 864명의 의무기록 자료를 후향적으로 분석하였다.
둘째, 근치적전립선절제술을 받은 후의 건강상태에 따른 삶의 질을 조사하기 위하여 일반인 남성 400명을 대상으로 수술방법에 따른 건강상태를 자세히 묘사한 시나리오를 바탕으로 시간교환법을 사용하여 측정하였다.
셋째, 후향적 의무기록 조사 대상자 864 명중 비용자료가 확보된 682명을 대상으로 수술후 급여, 비급여를 포함하는 총 의료비용을 분석하였다.
마지막으로 근치적전립선암절제술 환자의 수술법간 경제성분석은 보험의료체계관점으로 분석기간 1년의 결정수형모형을 사용하여 비용-효용분석을 수행하였다.
▢ 연구 결과
⦁임계값 3,050만원을 기준으로 했을 때, 로봇수술은 다른 수술에 비해 비용효과적인 대안이라 할 수 없다.
⦁ 코호트 분석과 삶의 질 조사를 통해 확인하였을 때 로봇수술의 효과성이 기존 수술법에 비하여 효과적이었으나 차이를 나타내는 기능적 지표가 생존 등에 영향을 미치는 지표가 아니기 때문에 효용의 차이가 작은 반면, 로봇수술의 비용이 매우 고가이기 때문에 효과가 비용을 극복하지 못한 것으로 판단된다.
Ⅰ. 후향적 의무기록 자료를 이용한 임상적 안전성 유효성 비교
국내에서 수행되는 근치적전립선절제술 방법별 임상적 안전성 및 유효성을 파악하기 위하여, 2010년과 2011년에 5개 병원에서 임상병기 T1~T3 전립선암으로 진단받고, 근치적전립선절제술을 시행받은 864명의 의무기록 자료를 후향적으로 분석하였다.
본 연구의 선정제외기준에 부합하는 전체대상자를 분석한 결과 로봇수술은 기존 수술대비 출혈량이 적었으며 골반림프절 절제술과 신경보존술의 시행 비율이 더 높게 나타났다. 종양학적 결과를 살펴보면, 절제면 양성률은 복강경수술에서 가장 낮았으며, 생화학적 재발률은 1년, 2년, 3년 시점 모두에서 로봇수술이 가장 낮았으나 통계적으로 유의미한 차이는 없었다. 안전성 결과로써 합병증 발생은 로봇수술군에서 가장 적었으며, 전이와 사망은 세군 모두에서 보고되는 발생수가 매우 드물어 직접적인 비교분석은 수행하지 않았다. 기능적 결과로써 요자제 회복률은 로봇수술이 3개월 시점에서 89.4%, 3년 시점에서 95.0%로 가장 높았다. 연령, BMI 등 요자제 회복에 영향을 미치는 교란변수를 보정한 다변량 분석결과에서 로봇수술의 요자제 회복률이 복강경 수술에 비하여는 높았으나, 개복수술과는 차이가 없는 것으로 나타났다.
환자의 기저상태를 보정하기 위하여 비교적 결과에 영향을 많이 미칠 것으로 생각되는 변수(연령, BMI, Gleason 점수, PSA수치)를 기준으로 매칭한 대상군의 분석결과, 로봇수술은 기존수술법에 비하여 출혈량이 적은 것으로 나타났으며, 골반림프절 절제술과 신경보존술의 시행 비율이 더 높았다. 종양학적 결과로 절제면 양성률과 생화학적 재발률은 수술법간의 차이가 없었다. 안전성 결과로써 합병증 발생, 전이, 사망은 발생건수가 작아 직접 비교를 수행하지 못하였다. 기능적 결과로써 요자제 회복률은 로봇수술이 3개월 시점에서 88.7%, 3년 시점에서 95.3%로 가장 높았다. 연령, BMI 등 요자제 회복에 영향을 미치는 교란변수들을 보정한 다변량 분석결과에서 로봇수술의 요자제 회복률은 복강경 수술에 비하여 높게 나타났으나 개복수술과는 차이가 없는 것으로 나타났다.
국내 환자자료에서는 로봇수술이 기존수술에 비해 수술직후 결과(재원일수, 수술시간, 출혈량 등)와 기능적 결과(요자제 회복률)를 향상시키는 것으로 나타났는데, 이는 선행 의료기술평가보고서(HIQA 2011; NETSCC 2012) 및 체계적 문헌고찰(Ficarra, 2012)의 결과와 비슷하였다. 하지만 성기능회복 결과는 의무기록의 기록 미비로 본 후향적 코호트분석에서는 확인할 수 없었다.
생화학적 재발의 경우 국외에서 보고된 의료기술평가 보고서(HIQA 2011; NETSCC 2012) 및 체계적 문헌고찰(Ficarra, 2012; Novara, 2012) 결과와 유사하게 수술법간의 차이는 없는 것으로 나타났으나, 국내 체계적 문헌고찰(한국보건의료연구원, 2014)의 결과인 생화학적 재발의 정의에 따라 로봇수술군에서 재발률이 유의하게 낮은 것과는 다른 결과를 보였다. 로봇수술의 수술기법은 지속적으로 발전하고 그에 따라 치료결과도 더 향상되고 있는데, 본 연구는 2010년~2011년의 전립선암 환자를 대상으로 분석하였으므로 최근 향상된 수술결과를 반영하지 못한 부분이 존재할 수 있다. 또한 국외의 경우 최근에 발표되는 문헌은 5,000건 이상의 로봇수술을 시행한 숙련된 전문가의 수술결과가 보고되고 있는데 비하여 우리나라는 아직까지 로봇수술의 술기가 계속 발전하고 있는 현실이며, 2010년~2011년의 경우 이러한 상황도 일부 영향을 주었을 것으로 생각된다. 다만 전체대상자에서는 유의했던 생화학적 재발이나, 합병증의 차이가 매칭한 대상자에서는 차이가 없는 것으로 보아 수술법 선택에 있어서 환자상태가 영향을 주고 있는 것이라 추정할 수 있었다.
Ⅱ. 전립선암 환자의 삶의 질
일반인 남성 400명을 대상으로 근치적전립선절제술을 받은 후 건강상태에 따른 삶의 질을 조사하였다. 수술방법에 따른 건강상태를 자세히 묘사한 시나리오를 바탕으로 측정하였으며, 시간교환법을 사용하였다. 삶의 질 조사결과, 수술방법에 따른 건강상태는 개복수술로 인한 효용 감소가 복강경수술로 인한 효용 감소보다 크게 나타났다. 로봇수술에 비하여 개복수술을 수행한 경우 1년 중 약 9.5일, 복강경수술을 수행한 경우 1년 중 약 2.9일의 효용 감소(disutility)를 갖는 것으로 조사되었다. 전립선암 수술 후 부작용 발생에 따른 효용값을 조사한 결과 성기능 이상으로 인한 효용 감소가 요실금 문제로 인한 효용 감소보다 크게 나타났다. 부작용이 없는 상태에 비하여 성기능 이상이 있는 경우 1년 중 약 34일, 요실금이 있는 경우 1년 중 약 25.2일의 효용 감소(disutility)를 갖는 것으로 조사되었다. 근치적전립선암절제술후 1년 시점의 건강상태에 따른 효용값은 ‘암이 치료된 상태(0.826)>암이 재발된 상태(0.421~0.692)>전이암(0.129~0.269)’ 순으로 높았다. 암이 재발된 상태 내에서는 추가치료 여부에 따라 ‘치료 안함(0.692)>방사선치료/일정기간 호르몬치료(0.547)>평생 호르몬치료(0.421)’ 순으로 높았으며, 전이암 상태 내에서는 ‘초기암(0.269)>말기암(0.129)’ 순이었다.
Ⅲ. 비용분석
후향적 의무기록 조사 대상자 864명 중 비용자료가 확보된 682명을 대상으로 수술후 급여, 비급여를 포함하는 총 의료비용을 분석하였다. 전립선암 환자가 근치적전립선절제술 후 수술비용을 포함하여 1년 동안 소요하는 의료비용은 로봇 수술 약 1,800만원, 복강경 수술 약 840만원, 개복수술 620만원으로 로봇수술군이 가장 높았으며 기존 수술과 비교하여 2배 이상 높은 것으로 나타났다. 근치적전립선절제 이후 1년 동안 소요되는 의료비용 중 수술비용이 차지하는 비율이 가장 높았고, 수술비용을 제외한 나머지 소요비용에서는 큰 차이가 없는 것으로 나타났다. 수술방법에 따른 비급여 비율을 살펴본 결과 로봇수술군이 85.0%로 가장 높게 나타났으며, 복강경수술, 개복수술의 순으로 조사되었다.
Ⅳ. 경제성 분석
근치적전립선암절제술 환자의 수술법간 경제성분석은 보험의료체계관점으로 분석기간 1년의 결정수형모형을 사용하여 비용-효용분석을 수행하였다. 수술방법(개복수술, 복강경수술, 로봇수술)간 비용-효용분석 결과, 개복수술 대비 복강경수술의 ICER는 44,338,406원/QALY, 개복수술 대비 로봇수술의 ICER는 132,507,255원/QALY, 복강경수술 대비 로봇수술의 ICER는 252,141,944원/QALY으로 조사되었다. 즉, 안정훈 등(2011)이 제시한 임계값 3,050만원을 기준으로 할 때, 로봇수술은 다른 수술에 비해 비용효과적인 대안이라 할 수 없다. 일원민감도 분석 결과, 효용값과 로봇수술 비용이 민감한 변수임을 확인할 수 있었으나 로봇수술이 비용효과적이지 않은 기본분석 결과에는 영향을 주지 않았다. 또한 다원민감도 분석결과 역시 기본분석 결과와 유사하게 조사되었다.
한편, 수술비용이 비용효과성에 미치는 영향을 확인하기 위하여 임계값 분석을 수행한 결과, 복강경수술은 약 70만원 감소 시 복강경수술이 개복수술에 비해 비용-효과적인 대안이 될 수 있으며, 로봇수술 비용이 약 900만원 감소 또는 830만원 감소할 경우 로봇수술이 개복수술 또는 복강경수술 대비 비용-효과적인 대안이 된다. 본 연구에서 수술 후 1년 동안 복강경수술 대 개복수술, 로봇수술 대 개복수술, 로봇수술 대 복강경수술의 비용과 효과를 분석한 결과, 1년 시점에서 삶의 질 개선 효과의 차이는 0.04~0.09 QALY로 크지 않은데 반해, 두 대안간 비용의 차이는 약 230만원~1200만원으로 크게 나타났다. 즉, 효과 차이는 작았고 지출되는 비용이 매우 크기 때문에 고가의 수술치료 비용이 두 군의 수술비용이 거의 유사해지는 수준 또는 약간만 높은 수준으로 감소해야 비용-효과적인 대안이 될 수 있다. 그러나 본 연구의 분석결과는 1년 시점의 결과이므로 해석에 주의할 필요가 있다.
▢ 결론 및 정책적 제언
본 연구는 4대 중증질환 보장성 계획에 맞추어 국내에 로봇수술이 도입된 2005년 이후 가장 가파른 증가를 보인 전립선암에서 로봇수술의 경제성분석을 시행하였다. 전립선암 환자의 근치적전립선절제술 방법(개복수술, 복강경수술, 로봇수술)간의 비용-효용분석 결과 임계값 기준 3,050만원 (안정훈 등, 2011)을 기준으로 할 때 로봇수술은 기존수술(개복수술, 복강경수술)에 비하여 비용-효과적이지 않았으며, 로봇수술 후 1년 동안의 소요비용이 약 830~900만원 감소할 경우 기존수술법에 비하여 비용효과적인 대안으로 나타났다. 이는 코호트 분석과 삶의 질 조사를 통해 확인하였을 때 로봇수술의 효과성이 기존 수술법에 비하여 효과적이었으나 차이를 나타내는 기능적 지표가 생존 등에 영향을 미치는 지표가 아니기 때문에 효용의 차이가 작은 반면, 로봇수술의 비용이 매우 고가이기 때문에 효과가 비용을 극복하지 못한 것으로 판단된다. 본 연구는 후향적 연구 설계를 통한 유효성 파악 및 단기결과를 통한 비용효용분석이므로 해석에 주의가 필요하며 앞으로 추가적인 연구를 통하여 본 연구결과를 토대로 향상된 결과를 도출해야 할 것이다.
▢ Background
Since the introduction of the first robotic platform in 2005, urology, which accounts for 33.4% of all robotic surgeries, is the fastest growing sector of robotic surgery in South Korea. Among these surgeries, robot-assisted radical prostatectomy (RARP) is most frequently used for patients with prostate cancer. Further, the application of RARP is expected to increase owing to its various advantages. According to a systematic review (SR) published in 2012 (Ficarra et al, 2012), RARP was superior in terms of functional outcomes such as the urinary incontinence recovery rate and potency recovery rates compared to conventional surgeries (laparoscopic radical prostatectomy [LRP]and open radical prostatectomy [ORP]). In addition, an SR (Lee et al. 2014), undertaken in 2003 at our research institute, showed that RARP was superior not only in terms of functional outcomes, but also in terms of safety, and was associated with improved immediate postoperative outcomes such as estimated blood loss. However, the cost-effectiveness of RARP remains under debate owing to high medical costs compared to that of conventional surgery.
The Korean government is currently extending coverage for four major diseases that incur high medical expenditure, and robotic surgery is included in the expanding health insurance benefit plan. However, no studies have reported an economic evaluation in Korea of Robotic surgery in prostate cancer using Korea data, and the provision to extend coverage for robotic surgery is under debate.
▢ Objective
To provide fundamental data for instituting the national health policy, this study evaluated the clinical efficacy and cost-effectiveness of RARP compared to that for LRP and ORP for localized prostate cancer in South Korea.
▢ Methods
To evaluate the clinical efficacy and safety of the surgical modalities (RARP, LRP, and ORP), first, we retrospectively reviewed the clinical data of 864 consecutive patients with prostate cancer who had undergone RP between January 2010 and December 2011 at 5 tertiary hospitals in South Korea, regardless of the modality.
Second, we assessed quality of life in terms of the state of health after RP. The quality of life of 393 normal men was measured mainly using Time-Trade-Off. The survey was based on a scenario that describes the state of health in detail and considers the surgical methods, short-term adverse effects following RP, disease-specific status, and additional treatments 1 year after RP.
Third, we analyzed total medical expenses, including postoperative insured and non-insured amounts for 682 of the aforementioned 864 patients.
Finally, we used the decision tree model to compare expected costs and quality-adjusted life years (QALYs) for RARP and LRP or ORP as per the Korean Healthcare system perspectives during the one-year horizon. We retrospectively reviewed a cohort of patients with localized prostate cancer to identify transition probabilities and costs. The status of health depended on the absence of evidence of disease, biochemical, metastasis, death, or major complications. A discount rate of 5% was applied to the cost and QALYs. The incremental cost-effectiveness ratios of RARP compared to that of LRP or ORP were calculated.
▢ Results
⦁With a threshold value of 30.5 million KRW, the cost-effectiveness analysis of different RP procedures showed that RARP was not cost-effective when compared to existing surgical procedures.
⦁ The effectiveness may not have offset the costs, since there was no profound difference in effectiveness between the different RP procedures, whereas RARP was significantly more expensive than its alternatives.
Ⅰ. Retrospective cohort study
Total 864 patients met the inclusion criteria of our study. Of 864 patients, 559 patients underwent RARP, 170 patients underwent LRP, and 135 patients underwent ORP. In perioperative outcomes, Patients treated with RARP had less operation time and high rate of treating pelvic lymphadenectomy and nerve sparing techniques compare to conventional surgery. In oncological outcomes, the positive surgical margins (PSM) rate was the lowest in patients who underwent LRP, but there was no significant differences in biochemical recurrence rate (BCR) according to modalities. In safety outcomes, the complication rates were the lowest in the patients who underwent RARP. In functional outcomes, RARP showed the highest urinary incontinence recovery rate, with 89.4% and 92.5% of the patients showing complete continence recovery by the 3-month and 3-year follow-up periods, respectively.

There was statistically significant differences in patients characteristics such as age, ASA class, prostate volume, clinical stage, so patients were matched based upon the covariates.
Total 864 patients, only 269 patients were matched. Of them, 133 patients underwent RARP, 68 patients underwent LRP, and 68 patients underwent ORP. In perioperative outcomes, Patients treated with RARP had less operation time and high rate of treating pelvic lymphadenectomy and nerve sparing techniques compare to conventional surgery. In oncological outcomes, there were no significant differences in BCR, and PSM according to modalities. The results of this study were similar to those reported previously in medical technology assessment reports(HIQA 2011; NETSCC 2012) and SR (Ficarra et al, 2012) that the postoperative and functional outcomes were superior in the RARP group. However, potency recovery rates could not be confirmed in this retrospective cohort analysis owing to the lack of written medical records. While, BCR rates results, which were similar to those reported in international SR (Ficarra, 2012; Novara, 2012) and HTA reports published between 2011 and 2012 (HIQA 2011; NETSCC 2012), were different compared to our results that there was no significant differences according to modalities. SR from the Korean Institute for Health and Medical Research reported that the rate of BCR of prostate cancer was significantly lower in the patients who underwent RARP. The surgical techniques and treatment outcomes of RARP have been improving constantly; however, by limiting the analysis to patients who underwent RP between 2010 and 2011, the study may have failed to reflect the improvement in surgical outcomes. Moreover, recent international publications report surgical outcomes from physicians who had performed > 5,000 RARP procedures. In contrast, RARP techniques were still under development in Korea at 2010~2011.
Ⅱ. Quality of Life
According to a study that analyzed a target of 303 men for meeting logical consistency of the survey, the utility values are as follows. First, health status after surgery showed that the utility value reduced greatly in patients who underwent open radical prostatectomy (ORP) than in those who underwent LRP. The utility value was reduced by approximately 9.5 and 2.9 days per year, respectively, for ORP and LRP compared to that for RARP. Second, the utility value based on adverse effects after RP indicated that erectile dysfunction caused a greater reduction in efficacy than urinary incontinence. Owing to erectile dysfunction and urinary incontinence, efficacy reducedto approximately 34 and 25.2 days per year, respectively, in comparison to health state without adverse effects. Finally, the utility values were arranged in order from the highest to the lowest based on the state of health 1 year after RP as follows: no evidence of disease (0.826), biochemical recurrence (0.421-0.692), and metastasis (0.129-0.269). Further, according to additional treatment for recurrence, utility values were arranged in order from the highest to the lowest as follows: no treatment (0.692), radiation with/without hormone replacement therapy for a specific period (0.547), and lifelong hormone therapy (0.421). In addition, depending on the need for additional treatment for metastasis, utility values were arranged as follows: initial-stage (0.269)> terminal-stage (0.129).
Ⅲ. Cost Analysis
The data of 682 patients were collected using medical chart review in order to estimate total medical costs, including amounts covered with and without National Health Insurance. The medical costs of RP procedures, which included costs of the operation and postoperative management for 1 year, were the highest for the RARP group at 18 million South Korean Won (KRW), followed by approximately 8.4 million KRW for LRP and 6.2 million KRW for ORP. The cost of RARP was two-fold more expensive than the conventional surgeries, LRP and ORP. The cost of surgery accounted for the majority of the medical expenses incurred in the first year after RP. There was no significant difference between the costs, except for the cost of surgery. National Health Insurance did not cover 85.0% of the expenses in the RARP group, which had the highest proportion, followed by LRP, and ORP.
Ⅳ. Cost-utility Analysis
The economic efficiency of different surgical procedures for RP was measured using a decision-analysis model built over 1year, based on the perspective of the health insurance system. The cost-utility analysis results for the different surgical procedures were expressed in different incremental cost-effectiveness ratios as follows: 44,338,406 KRW for LRP over ORP; 132,507,255 KRW for RARP over ORP; and 252,141,944 KRW for RARP over LRP. In other words, considering the threshold value of 30.50 million KRW proposed by Hoon et al., RARP could not be considered as a cost-effective alternative, when compared to other surgeries. Although the utility value and RARP cost were confirmed as sensitive variables through one-way sensitivity analysis, the outcome did not affect the basic analysis, which demonstrated that RARP was not cost-effective. Moreover, multi-way sensitivity analysis demonstrated similar outcomes to those of the basic analysis.
Meanwhile, the threshold analysis to confirm the impact of surgery costs on cost-effectiveness demonstrated that LRP could be a more cost-effective alternative compared to ORP if costs for the LRP were to be reduced by 700,000 KRW. Similarly, RARP could be a cost-effective alternative to ORP or LRP if the costs could be reduced by approximately 9 million KRW, or 8.3 million KRW, respectively. In the present study, cost-effectiveness analysis of ORP over LRP, ORP over RARP, and LRP over RARP showed that the cost difference between the alternatives was significant at 2.3–12 million KRW, while the differences in the quality of life after 1year was non-significant at 0.04–0.09.
Specifically, due to large expenses and small differences in effectiveness, costly surgical treatments can be considered cost-effective only if the costs are reduced to levels similar to or marginally higher than those of the alternatives. However, the findings of the present study should be interpreted with caution, because our results were interpreted using data from a one-year period.
▢ Conclusions
This study analyzed the economic effectiveness of RARP for prostate cancer. Because the coverage plan was introduced for four severe diseases, RARP has become increasingly popular after its introduction in 2005. With a threshold value of 30.5 million KRW, the cost-effectiveness analysis of different RP procedures showed that RARP was not cost-effective when compared to existing surgical procedures. RARP could be cost-effective if the expenses could be reduced by 8.3–9 million KRW within the first year of RARP use.
The effectiveness may not have offset the costs, since there was no profound difference in effectiveness between the different RP procedures, whereas RARP was significantly more expensive than its alternatives. Moreover, while RARP was validated through cohort analysis and quality of life analysis and was shown to be more effective than conventional procedures, the functional index excluded factors such as survival. The results of the present study should be interpreted with caution as short-term data were used for cost-effectiveness analysis and a retrospective research design was used to assess the efficacy. Based on the results of our study, long-term prospective studies are necessary to yield superior results.
