▢ 연구 배경
2014년 제67차 세계보건총회에서는 “보편적 의료보장을 달성하기 위한 보건의료 중재 및 의료기술의 평가” 결의안을 채택하면서 의료기술평가가 보건재정 효율화의 핵심요소임을 강조하였다. 결의안을 통하여 의료기술평가는 의약품 및 의료기기에 대한 일차원적인 평가를 넘어 국민의 건강을 향상시키기 위한 공중보건 프로그램이나 보건의료체계와 관련한 정책까지 그 범위가 확장되고 있다.
전 세계적으로 흡연으로 인한 사망자 수가 연간 6백만 명에 이르며 우리나라도 2012년 기준 연간 약 58,000명이 흡연으로 인한 질병으로 사망한 것으로 알려졌다. 흡연은 비감염성 질환의 핵심 유해요인으로, 선진국에서는 이미 담배규제 정책 강화를 통하여 흡연으로 인한 사망 수를 줄이고 있지만 우리나라를 포함한 아․태평양 지역은 여전히 높은 흡연율을 보이고 있다.
Asia Pacific Regional Capacity Building for HTA(ARCH)는 아․태평양 지역의 의료기술평가에 대한 인식을 높이고 제도적, 기술적 역량을 육성·강화시키기 위하여 2013년에 조직되었으며, 2014년 1차 프로젝트의 주제로 담배규제정책 강화 및 금연지원 프로그램의 효율을 높이기 위해 의료기술평가를 금연분야에 적용하는 것을 채택하고 국제공동 연구를 진행하고 있다. 국내에서도 금연에 대한 관심이 높아지면서 이를 지원하기 위한 금연지원 프로그램 또는 정책 개발 노력이 활발해지고 있다. 그러나 국내에서 진행되고 있거나 향후 도입될 금연 프로그램 및 담배규제 정책에 대한 결과 예측 및 평가를 위한 도구 개발 혹은 관련 연구는 미비한 실정이다.
▢ 연구 목적
본 연구에서는 의료기술평가의 활용 및 적용범위를 확대하기 위한 ARCH 국제공동 연구에 참여함으로써, 이러한 도구의 국내 적용 가능성에 대한 연구뿐만 아니라 국제공동 연구 과정에서 한국의 현황 및 연구결과를 적극적으로 공유하고자 하며 이를 통해 한국보건의료연구원의 국제적 연구 전문성을 제고하고자 한다. 또한 담배규제 정책 평가 도구를 개발함에 있어서 국내에서 부족한 연구를 확인 및 도출하여 국내 금연지원 프로그램 및 담배규제 정책 발전에 기여하고자 한다.
▢ 연구 방법
I. ARCH 국제공동 연구 참여
ARCH는 2013년 개시회의에서 각 국가 상황에 맞는 의료기술평가 연구의 첫 번째 사례로 금연지원 프로그램 및 담배규제 정책 평가를 선정하고, 2014년 이와 관련한 의료기술평가의 방법론에 대한 교육 및 관련 연구 결과를 공유하기 위한 2차례의 워크숍과 1차례의 학술대회를 개최하였다.
Ⅱ. 의료기술평가의 국내 금연분야 적용
국외에서 의료기술평가를 금연분야에 적용한 사례로 영국 NICE와 브루넬대학교에서 개발한 금연 관련 Return On Investment(ROI) 도구를 선정하였다. 금연 관련 ROI는 국가 또는 지역 정부의 금연 정책과 프로그램들이 동시에 복합적으로 시행될 때, 그 효과를 손쉽게 예측할 수 있도록 지원하며 흡연자, 과거 흡연자, 사망의 건강상태로 구성된 ‘마코브 모형(Markov model)’을 기반으로 하고 있다. 금연 프로그램에 참여 여부, 금연 성공 여부 등에 따라 각 주기 건강상태의 보건의료 비용, 생산성 손실, 건강관련 삶의 질 등이 축적, 수집된다.
ARCH 국제공동 연구에서는 아시아 ROI 도구 개발에 있어서 필요한 자료 및 데이터의 목록을 구축하였으며, 한국에서도 이를 바탕으로 국내 자료원 및 데이터를 조사하였다.
Ⅲ. 국내 연구주제 개발: 금연과 삶의 질의 연관성 연구
금연 정책 또는 프로그램에 대한 경제성 분석을 진행하고자 할 때, 흡연으로 인한 사망률이 감소함으로써 얻는 삶의 양과 질의 변화를 반영하는 것은 중요하다. 이를 반영하기 위하여 질보정수명(Quality adjusted life year, QALY)을 효과로 사용하게 되는데, 국내에는 금연과 관련한 질보정수명을 추정할 수 있는 건강관련 삶의 질(Health Related Quality of Life, HRQoL)에 대한 연구가 미흡하여 국외 선행연구 자료를 사용하는 경우가 많았다. 건강관련 삶의 질은 문화적, 사회적 요인으로 인해 차이가 발생할 수 있기 때문에 이와 관련한 국내 자료 구축이 필수적이다. 따라서 본 연구에서는 국내 연구주제 개발에 있어 금연이 건강관련 삶의 질에 미치는 영향을 연구주제로 도출하고 실제 연구를 수행하였다.
제5기 국민건강영양조사 자료(2010-2012년) 중 19세 이상의 성인 자료를 활용하여 금연과 건강관련 삶의 질의 연관성을 파악하고자 하였다. 흡연상태에 따라 비흡연자, 흡연자, 과거 흡연자로 구분하였으며 과거 흡연자, 즉 성공적인 금연자는 평생 5갑(100개피) 이상 담배를 피웠으며 1년 이상 흡연을 하였으나 현재는 흡연하지 않는 사람으로 정의하였다. 종속변수는 EQ-5D 값이었으며 성별, 연령, 교육수준, 직업군, 결혼상태, 소득 등의 사회인구학적 특성과 만성질환 유무, 평소 스트레스 인지 정도, 활동제한 여부 등의 건강관련 특성으로 보정하였다. 국민건강영양조사의 자료 추출 특성을 고려하여 복합표본분석방법을 바탕으로 회귀분석을 진행하였다.
또한 국내 연구주제 개발 및 분석 결과를 공유하고 해당 분야 전문가의 의견을 듣고자 대한금연학회와 공동 세미나를 개최하였다. 2014년 11월 17일 인제대학교 서울백병원 강당에서 개최된 공동 세미나는 금연전문가 약 20명이 참여하였으며 영국의 금연 관련 ROI 도구 소개 및 건강관련 금연과 삶의 질과의 관계 연구에 대한 결과를 발표, 논의하였다.
▢ 연구 결과
I. ARCH 국제공동 연구 참여
1차 워크숍은 2014년 4월 10-11일 필리핀 마닐라에서 진행되었으며 의료기술평가의 개념과 방법론에 대한 소개가 있었다. 또한 각 국가의 담배규제 관련 현황을 공유하였으며 한국보건의료연구원에서도 국내 금연지원 프로그램 및 담배규제 정책에 대해서 소개하였다.
2차 워크숍은 8월 28-29일 양일간 인도네시아 발리에서 진행되었으며 정책 평가 도구로 사용할 수 있는 ROI 도구에 대한 교육이 이루어졌다. 담배와 관련한 우선순위 및 정책결정과 관련하여 동남아시아 담배규제 연합(South East Asia Tobacco Control Alliance, SEATCA)의 역할 및 세계보건기구의 담배규제기본협약(Framework Convention on Tobacco Control, FCTC)을 소개하여 담배규제와 관련한 국제적 흐름에 대한 검토가 있었다.
11월 21일 싱가포르에서 개최된 제1차 ARCH 학술대회에서는 지난 두 차례의 워크숍을 바탕으로, 담배규제 정책 평가 도구로써의 ROI 활용에 대하여 논의하고 각 국가에서 진행된 연구 결과를 공유하였다. 한국보건의료연구원에서는 국민건강영양조사 자료를 이용한 '금연과 삶의 질의 연관성에 대한 연구' 결과를 발표하였으며 흡연 상태가 삶의 질에 미치는 영향에 대한 아시아 최초 연구로 주목을 받았다.
Ⅱ. 의료기술평가의 국내 금연분야 적용
영국의 금연 관련 ROI에는 총 23개의 금연 프로그램이 포함되어 있으며 각 프로그램의 참여율, 금연 성공률 등을 변화시킬 때, 그 결과의 변화를 예측할 수 있다. 결과지표로는 순비용절감효과, 비용편익비, QALY 증가 당 비용 등이 포함되어 있으며 금연 정책 및 프로그램의 장․단기 효과를 모두 확인할 수 있다.
ARCH 국제공동 연구에서 구축한, 아시아 ROI 도구 개발에 필요한 자료 및 데이터의 목록을 바탕으로 한국에서도 국내 자료원 및 데이터를 조사하였다. 비용자료 중 흡연과 관련된 질병의 연간 비용 또는 보건제도 직접비용에 해당하는 흡연관련 단위비용에 대한 자료로 국민건강보험공단의 자료를 바탕으로 한 연구결과가 있었지만 국가 또는 지역 단위 금연 프로그램의 비용 자료는 미흡하였다. 또한 흡연자 및 흡연관련 질병 환자의 질병보정수명 자료는 있었지만 건강관련 삶의 질 자료는 부재하였다. 비교효과연구에 필수적인 흡연여부에 따른 질병에 대한 상대적 위험도 및 생존율 자료는 암, 심혈관계 질환, 호흡기계 질환 등 다양한 질환군에 걸쳐 연구, 발표되었다. 금연 프로그램 참여율 또는 프로그램을 통한 금연 성공률 등의 자료는 비용-효과 분석에 필수적이지만 국내에는 이와 관련한 체계적인 자료가 부재함을 확인하였다.
ARCH의 담배규제 ROI 도구 적용을 위한 최소 필요자료 목록에 따라 국내 자료를 파악한 결과, 우리나라는 건강보험 공단자료 또는 보험심사평가원 청구자료가 있어 전수자료의 활용이 가능하다는 장점이 있었다. 그러나 흡연 관련 질병 발생률 및/또는 유병률 자료이나 흡연여부에 따른 질병의 상대위험도 등의 자료에 대한 최신의 자료가 부재하여 담배규제 ROI 도구에 활용하기에 한계가 있었다. 또한 국가 또는 지역단위, 내지는 민간분야의 금연 프로그램과 프로그램에 참여하는 흡연자의 비율 또는 그에 따른 금연 성공률, 그리고 질병에 따른 건강관련 삶의 질에 대한 체계적인 자료가 부재하여 ROI 도구에 국내 자료를 적용하기에 어려움이 있었다.
Ⅲ. 국내 연구주제 개발: 금연과 삶의 질의 연관성 연구
성인 총 17,303명이 분석에 포함되었으며 비흡연자, 과거 흡연자, 그리고 현재 흡연자는 각각 55.7%, 17.3%, 그리고 27.0%를 차지하였다. 현재 흡연자의 연령은 평균 41.7세(표준오차 0.317)로 비흡연자(평균 45.6세, 표준오차 0.273) 및 과거 흡연자(평균 51.38세, 표준오차 0.393)의 연령보다 낮은 경향이 있었다. 흡연상태에 따라 EQ-5D 값에도 차이가 있었으며, 관련변수로 보정하기 전 현재흡연자의 EQ-5D 값은 평균 0.959(표준오차 0.002)로 과거 흡연자(평균 0.951, 표준오차 0.002) 및 비흡연자(평균 0.941, 표준오차 0.001)보다 높은 경향을 보였다. 성별, 나이, 학력, 직업군, 결혼상태, 소득 등의 사회인구학적 변수와 만성질환 유무, 평소 스트레스 인지정도, 활동제한여부 등의 건강 관련 변수로 보정한 결과, 과거 흡연자의 EQ-5D가 현재 흡연자보다 0.010 높았으며(95% 신뢰구간: 0.005-0.014), 비흡연자의 EQ-5D 역시 현재 흡연자보다 0.008(95% 신뢰구간 0.004-0.013) 높았다.
▢ 결론 및 정책적 제언
의료기술평가의 중요도가 높아짐에 따라 그 활용대상을 확대하기 위한 노력이 커지고 있다. 본 연구는 의료기술평가를 공중보건분야 중 금연분야로 확대했다는 것에서 그 의의가 있다 할 수 있다. 담배규제 정책 또는 금연 프로그램을 평가하기 위한 도구로써 영국의 ROI 도구의 국내 적용 가능성을 확인하였으나 이와 관련한 국내 자료가 미흡한 부분이 있어 ROI의 실제 적용은 어려웠다. 국내 자료를 수집하는 과정에서 금연과 삶의 질 간의 관련성을 확인한 연구가 부재한 것을 확인하였으며, 해당 자료를 확보하기 위하여 본 연구에서 국민건강영양조사 자료를 바탕으로 금연과 삶의 질 간의 연관성을 연구하였다. 의료기술평가의 적용대상 확대를 위해 수행된 첫 번째 연구인만큼 이와 관련한 후속 연구가 수행되어야 할 것이며 본 연구결과는 국내 금연 및 흡연예방 정책 발전 뿐 아니라 의료기술평가 발전에도 기여할 것으로 기대된다.
▢ Background
The 67th World Health Assembly 2014 has adopted the resolution titled, “Health intervention and technology assessment in support of universal health coverage” emphasizing the health technology assessment was the core element of health finance efficiency. The Health Technology Assessment (HTA) is being expanded beyond the assessment of drugs and medical devices to the public health program or health care system-related policies to improve the people’s health through this resolution.
Globally, the number of smoking-related deaths reaches up to 6 million people per year and in Korea, also approximately 58,000 people died from diseases caused by smoking as of 2012. Smoking is a key harmful factor for the non-communicable diseases (NCDs), and developed countries has been putting efforts to reduce the number of death caused by smoking through reinforcement of the tobacco control policy but the Asia-Pacific region including Korea still shows a high rate of smoking.
The Asia Pacific Regional Capacity Building for the Health Technology Assessment (ARCH) was organized in 2013 to raise awareness on the HTA and to cultivate and reinforce systematic and technical capacities. In 2014 as the first project, ARCH had adopted application of health technology assessment to non-smoking areas for tobacco control policy enforcement and for increasing the efficiency of non-smoking assistance program, while it is conducting an international joint research. In Korea, as the interest in quitting smoking is increased, such as smoking cessation program or policy development efforts are being vitally undertaken. However, development of tools to predict and evaluate the outcomes of related studies or the results of smoking cessation program and tobacco control policies being implemented or to be introduced in the future, are still limited in the country.
▢ Objectives
The purpose of the study was to examine the applicability of such tools in Korea by participating in an international joint research that aims expanding the scope of utilization and application of HTA, to investigate the domestic applicability of these tools, and to enhance the expertise of international researches of the National Evidence-based healthcare Collaborating Agency (NECA) by actively sharing the current conditions of Korea and study results in the process of the international joint research. Also the study aimed to contribute to the advancement of domestic smoking cessation programs and the development of national tobacco control policies by identifying and derivation of the studies lacking in developing assessment tools in Korea.
▢ Methods
I. Participation in ARCH International Joint Research
ARCH had chosen smoking cessation programs and tobacco control policies as the first cases of research for health technology assessment corresponding to the status of each nation at its inception meeting in 2013 and then held two workshops and an annual conference in 2014.
Ⅱ. Domestic Application of HTA in Tobacco Control Field
Tobacco Return On Investment (ROI) tool has been developed by UK National Institute of Health and Care Excellence (NICE) and Brunel University to support national or local government to easily predict the effectiveness of simultaneous implementation of integrated non-smoking policies and programs. The ROI tool is based on 'Markov model' consisted of smokers, past smokers, and the health status of death. Depending on whether to participate in a smoking cessation program and whether succeeded in the smoking cessation, data was to be accumulated and collected such as healthcare cost, productivity loss and health-related quality of life for each cycle.
ARCH international joint research had constructed the list of data and information required for developing Asian version of the ROI tool, and in Korea, domestic data source and data were investigated based on it.
Ⅲ. Development of Domestic Research Topic: Association between Quit-smoking and Health-Related Quality of Life
In terms of conducting an economic analysis on the smoking cessation policy or program, it is important to reflect the quantitative and qualitative changes of life obtained from reduction of smoking-induced mortality. In order to reflect the aforementioned, Quality Adjusted Life Year (QALY) was used as the effectiveness of such as smoking cessation programs, but as there were little domestic researches available for Health-Related Quality of Life (HRQoL) that could estimate the QALY associated with smoking, previous research commonly used data from overseas. As for the HRQoL, it is reported that HRQoL would show differences due to cultural and social factors, so that the domestic database construction is essential in this regard. Thus, this study had derived the effects of smoking cessation on the HRQoL as a research topic and conducted the research.
Using the 5th Korea National Health and Nutrition Examination Survey (KNHANES, 2010-2012), we examined the impact of quit smoking on the HRQoL. In the analysis, adult population at 19 years of age or older was included. Depending on the smoking status, they were categorized as non-smoker, current smoker, and ex-smoker. The dependent variable was EQ-5D value and it was adjusted for demographic characteristics such as gender, age, educational level, occupation, marital status and income level and health related characteristics such as presence/absence of chronic diseases, recognition level of daily stress, and limitation on activities. A regression analysis with consideration of the complex sample design of the KNAHES was performed.
Also we co-hosted a joint seminar with the Korean Society for Research on Nicotine and Tobacco to share the outcomes from this study and to hear experts' opinions in the related fields. About 20 experts in tobacco control had attended to this seminar which was held at the auditorium of Inje University, Seoul Paik Hospital on November 17, 2014, At the seminar the attending participants were introduced about non-smoking ROI tool of UK and also discussed on the results of HRQoL.
▢ Results
I. Participation in ARCH International Joint Research
The first workshop was held in Manila, the Philippines from April 10 to 11 in 2014, where there was introduction of the concept and methodologies of health technology assessment. The regulatory circumstances of each country related to tobacco control had been shared, and the NECA also introduced smoking cessation programs and tobacco control policies in Korea.
The second workshop was held for two days from August 28 to 28 at Bali, Indonesia, and at the workshop, a training session was provided on ROI tool, which can be used a policy assessment tool as well. In connection to tobacco related priority setting and policy making, the role of South East Asia Tobacco Control Alliance (SEATCA) and the Framework Convention on Tobacco Control (FCTC) of WHO were introduced, leading to reviewing on the international trend related to tobacco control.
On November 21, at the first ARCH Conference held in Singapore, utilization of ROI as a tool for tobacco control policy assessment was discussed based on aforementioned past 2 workshops, and each country had shared the research outcomes undertaken in their locality. The NECA has presented the outcomes of 'Successful quit-smoking and health-related quality of life' in use of the data from the KNHANES which was spotlighted as the very first research in Asia on the effects of smoking on the health-related quality of life.
Ⅱ. Domestic Application of HTA in Tobacco Control Field
The UK tobacco ROI tool included 23 smoking cessation programs in total. As participation rate or smoking cessation success rate changed, the tool enabled to predict the changes of outcomes. For the outcome index, net cost saving effect, cost-benefit ratio and cost per QALY gained were included, and long-term/short-term effectiveness of non-smoking policies and programs were identifiable.
There were a variety of cost analysis based on the data of the National Health Insurance Service (NHIS) such as annual cost of smoking related diseases, or smoking-related unit cost applicable to the direct cost of health system, nevertheless the cost data of national or local level smoking cessation program was insufficient. In addition, there was available data for disease-adjusted life years (DALY) of smoker or patients with smoking-related diseases, whereas the data for HRQoL was absent. The data of relative risk and survival rate for diseases depending on smoking status, which are essential for the comparative study of effectiveness, were studied across various disease groups including cancer, cardiovascular diseases and pulmonary diseases. However, systematic data for participation rate in smoking cessation program or success rate of smoking cessation through such programs were absent in Korea.
From the results of identification of the domestic data according to the minimally required data lists for the ROI tool application, Korea had advantages that it can utilize the national claims data of the NHIS or the Insurance Review & Assessment (HIRA). However, as up-to-date data including prevalence and/or morbidity of smoking-related disease incidence and relative risk of disease was limited, it became the restriction for utilizing tobacco control ROI tool. Furthermore, due to the absences of data on the rate of smokers participating in smoking cessation programs provided at national, local, or private sector’s level, the success ratio of smoking cessation, and data on disease-induced HRQoL, it was difficult to apply such domestic data to the ROI tool.
Ⅲ. Development of Domestic Research Topic: Association between Quit-smoking and Health-Related Quality of Life
A total of 17,303 adults were included in the analysis and it was identified that non-smokers, ex-smokers, and current smokers were accounted for 55.7%, 17.3%, and 27.0% respectively. The mean age of current smokers was 41.7 years (standard error (SE) 0.317), showing a lower tendency to the ages of non-smokers (mean 45.6 years, SE 0.273) and ex-smokers (mean 51.3 years, SE of 0.393). There was a difference in EQ-5D values, depending on the state of current smokers smoking an average of 0.959 (SE 0.002), ex-smoker as an average of 0.951 (SE 0.002), and non-smokers as an average of 0.941 (SE 0.001). After adjusting with socio-demographic variables and health-related variables, EQ-5D of ex-smokers was higher than current smokers by 0.010 (95% confidence interval from 0.005 to 0.014), and EQ-5D of non-smokers was also higher than current smokers by 0.008 (95% CI 0.004 to 0.013).
▢ Conclusions
As the importance of HTA has been increasing, the efforts to expand its scope for utilization is also growing. This study may have had its significance in terms that it had expanded the HTA to tobacco control area in the public health field. The domestic applicability of the UK's ROI tool seemed very slight as there was insufficient domestic data to be used for assessing tobacco control policies or smoking cessation programs. In this regard, this study had investigated the association between quit-smoking and the HRQoL using the KNHANES. Since this study was the first study carried out for the expansion of application targets for HTA, a subsequent study should be conducted in connection to this study. The outcomes of this study is expected to contribute not only to advancement of the national policies for domestic smoking cessation and preventive policies for smoking but also to the advancement of HTA in Korea.
