▢ 연구배경
최근 우리나라는 급속한 경제적 발전으로 국민들의 삶의 질이 높아짐에 따라 보건의료 분야에 대한 투자와 관심이 점점 커지고 있다. 이에 따라 관련 쟁점들이 급격하게 증가하여 사회적 갈등을 일으키고 있다. 보건의료 분야의 쟁점들은 전문적인 지식이 요구될 뿐만 아니라 이해당사자 간 의견이 첨예하게 대립하고 있어 이러한 쟁점들을 객관적이고 공정하게 해결할 수 있는 사회적 합의 장치가 필요하다.
한국보건의료연구원은 보건의료 분야에서 논쟁이 되고 있는 쟁점들에 대해 전문가 패널 혹은 시민 패널을 구성하여 이해 당사자 간의 입장 공유 및 숙의 과정을 거쳐 합의를 도출하려는 지속적인 노력 즉, 일관성 있는 사회적 대화를 지속하고자 하는 공론의 장인 NECA 원탁회의를 운영하고 있다. 2009년 ‘무의미한 연명치료 중단을 위한 사회적 합의안 제시’를 주제로 시작된 이래 2013년까지 총 9건의 원탁회의를 개최하였다.
▢ 연구목적
본 연구는 과거의 원탁회의 운영 경험을 바탕으로 운영 프로세스를 개선하고, 투명한 주제선정 절차, 참여대상의 확대, 결과 보고의 공개성 확보, 충분한 숙의 과정을 적용하여 총 4개 주제에 대한 원탁회의를 효과적으로 운영하고 합의 도출을 도모하고자 시도되었다.
▢ 연구방법
Ⅰ. NECA 원탁회의 기획
NECA 원탁회의 주제선정을 위하여 2013년 연구 과제를 대상으로 일차적으로 ‘NECA 원탁회의 주제수요조사’를 실시하였다. 주제수요조사 결과와 자체 발굴 주제에 대하여 사전에 개발된 체크리스트를 활용하여 연구본부 본부장과 팀장이 평가하여 최종 주제를 선정하였다.
Ⅱ. NECA 원탁회의 운영
NECA 원탁회의 운영은 운영위원회 구성, 발표자와 패널 구성, 사전회의, 본회의, 합의문 도출의 핵심 순서를 따르나 주제 및 패널의 특성에 따라 사전회의를 생략하는 등 특화하여 진행하였다. 운영위원회는 연구진과 주제 관련 전문가로 구성되었으며 원탁회의 일정 및 프로그램 결정, 의제 범위 설정, 핵심 질문 선정, 발표자와 패널 및 좌장 후보군을 선정하였다. 발표자는 원탁회의 주제에 대해 과학적인 근거자료를 바탕으로 자신의 의견을 발표하여 패널들이 주제 관련 쟁점에 대해 판단을 내리는데 도움을 주도록 하였으며, 패널은 근거의 평가, 토론 및 숙의 과정을 통해 쟁점에 대한 합의점을 모색하였다. 발표자와 패널이 구성되면 본회의 전에 사전회의를 개최하여 발표자와 패널에게 NECA 원탁회의의 목적과 진행과정에 대해 설명하였다. 본회의는 원탁회의의 가장 중요한 단계로 토론과 숙의를 통해 보건의료 분야의 쟁점에 대한 패널들의 합의된 의견을 도출하는 것을 최종 목표로 하여 진행하였다. 합의문은 연구진이 본회의에서의 토론과 숙의 내용을 정리하여 초안을 작성하고, 패널들의 검토를 통해 도출하였다.
Ⅲ. 후속조치
최종 합의문은 보고서, 보도자료, 논문 등의 형태로 대외적으로 공표되었다. 보고서 작성 및 공개 시에는 채텀하우스 룰(Chatham House Rule)에 따라 회의 참가자의 소신발언과 논의의 개방성 확보를 위해 논의된 내용은 공표하되 성명, 소속 등은 보고서에 공개하지 않는 규칙을 준수하였다.
▢ 연구결과: 위암 로봇수술 원탁회의
Ⅰ. 운영안 및 세부 운영 절차
한국보건행정학회와 공동으로 로봇수술 관련 NECA 원탁회의를 주최하기 위해 한국보건행정학회 임원진과 연구진은 운영위원회를 개최하였다(2014. 3. 6.). 운영위원회에서는 한국보건행정학회 임원진에게 NECA 원탁회의 목적과 운영절차를 설명한 후 세부적인 협력방안에 대해 논의하였으며, 발표자와 패널 후보군들을 선정하였다. 또한, 논의할 주제 범위를 설정하였으며 본회의 개최 일시와 장소를 결정하였다.
본회의는 『로봇수술의 명암과 비전』이라는 주제로 위암과 전립선암 로봇수술의 안전성과 유효성, 비용-효과성에 대해 논의하기 위해 연세대학교 의과대학 1층 강당에서 개최되었다(2014. 4. 11.). 본회의 주요 참석자는 한국보건의료연구원 연구책임자, 아주학교 의과대학, 연세대학교 의과대학 소속 전문가 3인의 발표자와 조선일보 기자를 포함한 대한위암학회, 대한비뇨기과학회, 카이스트, 보건산업진흥원, 한국환자단체연합회, 보건복지부 소속 전문가 패널 7인, 가톨릭대학교 의과대학 소속의 전문가인 좌장 1인이었다. 본회의는 약 4시간 동안 1부 주제발표와 2부 지정토론 및 자유토론으로 진행되었다.
Ⅱ. 본회의 결과
위암환자에 대하여 로봇수술과 복강경 수술을 비교한 결과 안전성과 유효성에서 큰 차이가 없었다. 미국의 연구결과 역시 로봇수술과 복강경 수술 비교 시 사망률, 합병증, 재원일수는 차이가 없었고, 다만 비용 측면에서는 로봇수술이 더 많이 소요되는 것으로 나타났다. 그러나 관련 문헌수가 적고, 장기간의 종양학적 결과가 확보되지 않아 로봇수술의 안전성과 유효성에 대한 결론을 내리기는 어려웠다. 현재 로봇 수술 장비는 모두 해외의 한 의료기 회사에 의존하고 있으므로 국산 로봇수술 장비 개발이 필요하다. 특히 의료보건산업육성과 기술경쟁력 향상 등이 현재 정부 기조이고, 최소 침습성 수술이라는 장점과 외과의사가 부족한 의료계 현실 등을 두루 고려해 볼 때 로봇수술은 앞으로 점점 더 확산되어 갈 것으로 전망된다. 로봇수술 장비를 효율적으로 개발하기 위해서는 의료전문가와 로봇개발 관련 전문가의 적극적인 상호협력이 필요하며, 국가적인 차원에서 장기적으로 규모 있는 투자가 병행되어야 한다. 로봇수술의 연구개발 활성화를 위해서는 유관기관들이 다음과 같은 역할을 해야 한다.
▪ 한국보건의료연구원: 아직 수행되지 않은 연구에 대한 비용효과 분석 등 관련 평가를 더 수행하고 지속적으로 과학적 근거들을 제시해야 한다.
▪ 식품의약품안전처: 후발주자인 국내 로봇 개발 회사가 기 개발된 장비와 본질적 동등성을 획득할 수 있도록 규제를 합리적으로 개선해야 한다.
▪ 건강보험심사평가원: 로봇수술에 대해 적정수준의 수가를 통한 보상과 급여화를 추진하여야 한다.
▪ 국내 의료기기 개발사: 한국의 미래를 이끌어 갈 국산 로봇을 개발하기 위하여 안정적 임상시험이 가능하도록 투자처를 확보하는 것이 필요하다.
Ⅲ. 후속조치
본 원탁회의는 정책토론회의 기능을 겸하였으므로 별도의 합의문을 작성하지 않고, 발표자와 패널들의 의견을 종합하여 논문으로 게재하기로 하였다.
▢ 연구결과: 전립선암 로봇수술 원탁회의

전립선암 로봇수술 원탁회의 운영안 및 세부 운영 절차는 위암 로봇수술 원탁회의 경우와 동일하다.
Ⅱ. 본회의 결과
로봇수술은 비뇨기과에서 가장 먼저 시작하였고 현재 가장 많이 시행되고 있다. 로봇수술은 개복술이나 복강경 수술에 비해 전립선암 수술 후 요실금, 성기능 감퇴와 같은 삶의 질 관련 부작용이 적고, 수술 중 출혈도 적다. 집도하는 의사 측면에서는 피로도가 적다는 장점도 있다. 또한 수술할 때 시야를 3차원으로 확대해 주는 기능이 있어 주변장기의 손상이 적으며 재원기간을 단축시키고 통증이 상대적으로 약하다. 체계적 문헌 고찰 및 메타 분석결과에서도 로봇수술은 기존수술에 비해 기능적 결과 측면에서 더 좋은 결과를 보이고 있다. 종양학적 결과에 있어서는 절제면 양성률은 발생위험의 차이가 없어 장기 추적 관찰된 결과가 필요하다. 전립선암에 대한 로봇수술은 안전성과 유효성은 과학적으로 검증되어 전립선암의 로봇수술 건강보험급여화가 우선적으로 검토될 수 있다. 이는 고소득층과 저소득층의 사이의 의료이용 양극화 극복에 도움이 될 것으로 기대된다. 위암 로봇수술과 마찬가지로 현재의 비용적인 부담을 낮추기 위해서는 국산 로봇수술 장비 개발이 시급하다. 향후 대안별 비용대비 효과를 평가하는 경제성 평가도 수행할 필요가 있다.

본 원탁회의는 정책토론회의 기능을 겸하였으므로 별도의 합의문을 작성하지 않고, 발표자와 패널들의 의견을 종합하여 논문으로 게재하기로 하였다.
▢ 연구결과: 치매 진단 원탁회의
Ⅰ. 운영안 및 세부 운영 절차
치매진단 관련 NECA 원탁회의에 대한 세부적인 운영에 대해 논의하기 위해 관련 임상 전문가들과 함께 운영위원회 및 사전회의를 개최하였다(2014. 6. 20.). 사전회의를 통하여 본회의는 2014년 9월 26일(금)에 『알츠하이머 치매 증상이 나타나기 전에 조기 진단이 가능한가?』 라는 제목으로 개최되었다.
Ⅱ. 본회의 결과
알츠하이머 치매의 원인은 유전적 소인과 환경적 요인에 의해 축적된 아밀로이드반(amyloid plaque), 타우 단백질(tau protein)과 같은 유해한 단백질의 침착으로 인한 뇌세포 손상이 원인으로 알려져 있다. 알츠하이머 치매의 증상은 기억력 저하, 방향감각 저하, 언어장애 등 여러 인지기능 이상과 성격변화, 우울 증상을 보이며 결국 모든 일상생활의 기능을 상실하게 된다.
알츠하이머 치매 진단을 위한 신경촬영법에는 MRI, CT와 같은 구조적 영상촬영법과 FDG-PET, 기능적 MRI(fMRI), 동맥 스핀 라벨링(arterial spin labeling, ASL)과 같은 기능적 영상촬영법으로 구분할 수 있다.
본 회의에서는 알츠하이머 치매 조기 진단을 위한 FDG-PET의 유용성이 논의되었다. 찬성하는 의견은 알츠하이머 치매의 병리 기전에 근거하여 병의 원인을 제거하기 위한 치료법을 개발하기 위해서 정확한 진단이 필요한 경우와 실제 임상현장에서 진단을 내리기 애매한 경우, 특히 기억장애성 MCI의 경우 FDG-PET 검사가 유용하게 활용될 수 있을 것이라고 피력하였다. 그러나, 알츠하이머 치매의 자연사와 병리, 조기치료 방법이 개발되지 않아 FDG-PET을 통한 진단이 임상적 유용성으로 연결되기 힘들고 조기 진단검사와 관련해 검사주기, 추후관리방법(예컨대 정량검사가 가능하다면 그 결과에 따른 차별적인 관리방법이 있는가 등)이 확립되어 있지 않은 점이 고려되어야 한다는 반대 의견이 있었다. 특히 FDG-PET을 건강검진 목적으로 활용하는 건에 대한 건강보험급여는 과잉진단 문제를 일으킬 수 있으므로 무증상 환자에게 알츠하이머 치매를 조기 진단할 필요가 있는지에 대해서는 지속적인 검토가 필요하다.
종합하면 본 원탁회의는 임상전문가 및 정책결정자가 알츠하이머 치매의 조기진단 가능성 및 유용성, 보험적용 등에 대해 공감대를 형성하는 계기가 되었다. 또한, 모든 환자 또는 일부 치매환자들에게 특정 영상검사를 바로 시행하거나 보험급여를 적용해야 한다고 결론지을 수 있을 만큼 타당한 근거는 없지만 치료적 의미가 있는 특정 적응증의 경우와 치료법 개발에 필요한 경우 이에 대해 긍정적으로 검토할 필요가 있다고 판단된다. 주로 논의된 FDG-PET는 고가의 검사이므로 보다 낮은 비용으로 선별검사로써 사용할 수 있는 여러 다양한 조기 검사방법들에 대한 연구·개발도 필요하는 의견이 있었다. 이를 위해서는 임상전문가 외에도 인지과학자나 뇌과학자와의 협업을 활성화하는 것이 도움이 될 것이다.
Ⅲ. 후속조치
NECA 원탁회의에서 발표된 내용과 토의된 내용은 추후 논문으로 작성하여 학술지에 게재할 계획에 있다. 또한 치매 진단 검사에 대한 국민들의 인식과 태도를 확인하고 치매 조기진단을 위한 FDG-PET에 대한 선호를 파악하고자 ‘치매 진단 검사에 관한 대국민 설문조사’를 수행하였다.
▢ 연구결과: 줄기세포
Ⅰ. 운영안 및 세부 운영 절차
줄기세포는 다른 치료법으로 불가능한 질병을 치료할 수 있다는 기대로 의학적인 측면에서 큰 기대를 받고 있고, 새로운 경제성장 동력으로 각광받고 있다. 하지만 줄기세포치료에 대한 안전성·유효성에 대한 과학적인 근거가 아직 명확하게 밝혀지지 않은 시점에서 의료현장에서 무분별하게 사용되고 있어 사회적인 문제로 대두되고 있다. 따라서 관련 학회 전문가와 기획회의를 통하여 환자에게 꼭 필요한 줄기세포치료에 대한 정보를 환자 정보집으로 제작하는 것으로 합의하였고, 학회 및 학계대표, 공공대표, 시민대표를 포함한 총 18명의 패널을 구성하여 원탁회의를 개최하였다. 이를 위해 한국줄기세포학회와 공동으로 운영위원회를 구성하고 기획회의(2014. 9. 17)를 개최하였으며, 이후 패널이 모여 환자 정보집 제작을 위한 사전회의(2014. 11. 18), 본회의(2014. 12. 23)를 거쳐 최종 콘텐츠를 마련하였다.
Ⅱ. 본회의 결과
주요 콘텐츠로는 줄기세포 기본 개념, 줄기세포 치료제 및 치료술에 대한 개발 및 승인과정, 줄기세포 임상시험에서 환자가 알아야 할 것, 줄기세포치료를 받는 환자의 체크리스트, 줄기세포치료 사후 안전성, FAQ로 정하였다. 세부적으로는 FAQ에 해당하는 8가지 Q&A를 제외하고 총 18가지 질문에 대하여 환자입장에서 꼭 알고 있어야 하며, 치료를 받기 전에 요구해야할 정보에 대해 정리하였다.
본회의에 참석한 패널들은 환자들 대부분이 줄기세포 치료제와 치료술을 혼동하고 있으며, 허가를 받은 치료제와 치료술이 아닌 임상시험 단계에 있거나, 무허가 시술에 노출되어 있음에 우려를 표하였다. 따라서 환자를 포함하여 전국민을 대상으로 이에 대한 주의가 필요함을 강조하고, 콘텐츠 곳곳에 이를 반영하였다.
Ⅲ. 후속조치
줄기세포치료에 관한 원탁회의의 최종 목표는 줄기세포치료를 앞두거나 관심 있는 환자들을 대상으로 꼭 알아야할 정보를 그들의 눈높이에 맞춰 제공하는 것이고, 이런 목적에 충실하고자 노력하였다. 최종 제작된 콘텐츠는 우리 원 및 한국줄기세포학회 등 온라인 웹사이트에 게재될 예정이며, 인쇄본은 식품의약품안전처, 국립보건연구원 등 공공기관과, 대학병원, 바이오, 제약기업 등 임상시험 수행기관 및 관련 학회에 배포할 예정이다. 또한 보도자료를 작성하여 배포하는 등 대국민을 대상으로 줄기세포치료에 대한 올바른 정보가 널리 확산될 수 있도록 다각도로 노력할 예정이다.
▢ 결론 및 제언
2014년 원탁회의는 운영프로세스, 주제 선정의 투명성 확보, 참여대상 확대, 결과 보고서의 공개성 확보, 과정중심의 원탁회의 운영에 초점을 맞추어 수행되었으며, 위암환자에서 로봇수술, 전립선암 환자에서 로봇수술, 알츠하이머 치매 조기진단에 있어서 영상검사의 유용성, 줄기세포 치료에 대한 환자 정보집 발간을 주제로 하는 총 4건의 원탁회의가 진행되었다. 이에 대하여 앞서 기술한 개선사항들을 기준으로 평가한 성과와 개선사항은 다음과 같다.
첫째, 효과적인 운영 프로세스를 개발을 위하여 원탁회의 주제에 대하여 연구를 수행한 연구책임자를 세부 연구과제 책임자로 포함시키고자 하였으나, 원탁회의를 운영하고자 하는 자발적 수요가 적었고, 자발적인 수요가 있는 경우라도 평가에서 높은 점수를 받지 못하거나 최종 회의에서 선정되지 못하였다. 원탁회의에 대한 지속적인 홍보와 가시적인 결과물 생산으로 참여 희망률을 높이는 것이 중요하다고 할 수 있다. 이를 위하여 NECA 원탁회의를 브랜드화하기 위하여 「NECA 공명」이라는 명칭을 개발하여 2015년부터 본격 적용할 예정이다.
둘째, 주제선정의 투명성 확보를 위하여 체계적이고 정량적인 주제선정 절차를 운영하였다. 1차적으로 원탁회의 주제수요조사를 실시하였으며, 수요조사 결과와 자체 발굴 주제를 대상으로 기획위원회가 그 우선순위를 평가하였다. 주제후보군들의 우선순위는 과학적 근거수준 및 사회적 가치를 기준으로 하는 체크리스트를 개발하여 평가하였다.
셋째, 현재 보건의료분야의 화두는 환자중심 또는 환자 참여라고 할 수 있다. 원탁회의에도 환자 및 국민을 포함한 다양한 이해당사자의 참여를 도모하였다. 그러나 보건의료 분야의 쟁점들은 전문적인 지식을 요하는 것이며, 연구 수행 인프라의 제한으로 대규모 시민패널을 운영하는 것이 불가하였다. 그러나 최대한 다양한 이해당사가자 참여할 수 있도록 언론 기자, 정책결정자, 임상전문가, 기초과학자, 법률가 등 다양한 분야의 전문가로 패널을 구성하였고 주제에 따라서 환우회의 대표가 참석하기도 하였다. 앞으로 환자중심 원탁회의 운영을 위한 중장기 계획 수립 및 실행방안 모색이 필요하다.
넷째, 결과 보고서의 공개성을 확보하기 위해 패널 및 발표자에게 공개 수준을 사전에 확인하였으며 보고서 작성시에는 채텀하우스 룰(Chatham House Rule)에 근거하였다. 채텀하우스 룰은 활발한 논의가 진행될 수 있도록 논의의 내용은 공개하되 발언자의 실명은 공개하지 않는 것을 말한다.
다섯째, 기존의 원탁회의가 합의문 도출이라는 결과에 치중함으로 인해서 이해당사자간 의견이 첨예하게 대립하는 경우 합의문이 도출되지 못하게 되었고 이를 원탁회의의 실패로 여기는 경우가 있었다. 그러나 사회적 합의도모는 합의를 달성하지 못하는 경우에도 합의 과정 자체로 의미가 있으며 특히 토론문화가 척박한 국내 보건의료분야의 경우 더욱 그러하다고 할 수 있다. 따라서 2014년도에 개최된 원탁회의는 합의문 도출이라는 부담을 감소시키고 자유롭게 소신 발언을 하며 의견을 공유하여 쟁점에 대한 소통과 숙의의 장을 만드는데 주력하여 과정중심의 원탁회의가 운영되었다고 할 수 있다.
이상의 평가를 바탕으로 우리나라 보건의료 분야의 대표적인 합의방법론으로 NECA 원탁회의를 정착시키기 위해서는 다음과 같은 사항을 개선해야 한다.
첫째, 원탁회의 결과를 임상지침이나 보건정책에 반영하기 위한 노력이 필요하다. 이를 위해서는 보건복지부, 건강보험심사평가원, 국민건강보험공단 등 유관기관과 연계를 강화하여 사전기획 및 주제선정 단계부터 주제 수요조사나 포커스그룹 인터뷰 등의 방법을 통해 임상지침 또는 보건정책 결정에 필요한 주제를 발굴하는 것이 중요하다. 또한, 관련 의사결정자를 적극적으로 원탁회의에 참여시켜 소통과 협력 관계를 구축하고, 후속조치로 합의 결과를 임상 지침 및 정책에 반영하도록 노력해야 할 것이다.
둘째, 합의 원칙에 대한 구체적인 가이드라인 설정이 필요하다. 사전에 참여자들에게 서약서를 받고, 명확한 합의 원칙(예: 투표를 통해 패널 75% 이상 찬성 시 합의)을 설정할 필요가 있다.
셋째, 원탁회의의 발전방향 제시를 위하여 원탁회의에 대한 평가 방식을 개발하고 엄밀한 평가가 수행되어야 한다.
▢ Introduction
As rapid economic development improves quality of life for the general public, more attention have been paid in healthcare. Accordingly, the related issues have also dramatically increased, which has caused much social conflict. As healthcare issues require more special knowledgement and it might be strong conflict among interested parties, a social mediation mechanism based on social conversation and discussion such as the NECA Round Table Conference(RTC) is needed to objectively and impartially resolve such issues.
The NECA RTC is a one of the consensus meeting for public discussion among panels of experts or citizens toward an ongoing effort to elicit consensus and to pursue consistent social discussions on core controversial issues in the healthcare field through stakeholders' perspective sharing and deliberation.
It was first held in 2009 in order to "propose a social consensus for discontinuing meaningless life support." And total of 9 RTC have held until 2013.
▢ Research goals
This research aims to perform successfully the four theme of RTC by applying 1) improved administrative process, 2) the transparent process for topics selection, 3) comprehensive participation from various sectors for objective and comprehensive discussion, 4) ensuring publicity for the results report, and 5) sufficient deliberative processes.
▢ Research method
Ⅰ. Planning of the NECA RTC
To select topics for the NECA RTC, the survey of demanded topics for RTC was firstly conducted with a focus on research subjects in 2013. With the results of the survey and the developed theme, the final topic was selected through an evaluation on the results of the demand survey for theme and consensus by the director and research team leader.
Ⅱ. Operation of the NECA Roundtable Conference
The operation of the NECA Roundtable Conference, which is ordinarily carried out roughly in the order of organizing a steering committee, forming speakers and panels, conducting preliminary and plenary meetings, and drawing an agreement, was conducted in a specialized format in that it omitted a preliminary meeting based on the characteristics of a certain topic or panel. The steering committee, comprised of the head and members of a and topic-related experts, decided on the schedule and program for a roundtable conference, set up an agenda range, chose key questions, and selected presenters, panel participants, and the appropriate candidates for chairperson seats. Once presenters and panel participants were determined, a preliminary meeting was held before the plenary meeting so that they were informed of the purpose and progress of the NECA Roundtable Conference.
The plenary meeting was conducted with the final aim of eliciting agreed-upon views from the panels about the controversial health care issues through discussions and deliberations. Following the deliberations, the healthcare evidence dissemination team summarized what had been discussed, deliberated at the plenary meeting, and prepared a draft. Finally, the agreement was drawn up based on the review by the panel participants. The agreement had disseminated with the forms of the reports, press releases, and journal articles.
▢ Research result on the roundtable conference on robot surgery for the patient with stomach cancer
Ⅰ. Administrative procedure
NECA Roundtable Conference on robot surgery was held in cooperation with the Korean Society of Health Policy and Administration (KSHPA) and NECA.
The plenary meeting was held with the theme "Controversy of and Future Prospects for Robot Surgery" (April 11, 2014). The key participants of the plenary meeting were principal investigator of NECA project on the safety and effectiveness of robot surgery, expertise in medical field, reporter from daily news, representatives of academic associations, representative of the Korea Health Industry Development Institute, the Korea Alliance of Patients Organization, and the Ministry of Health and Welfare were involved in the meeting.
Ⅱ. Results from the plenary meeting
Comparisons of stomach cancer robot surgery and laparoscopic operation cases did not show any statistical differences in safety or effectiveness same as the other research results from the U.S. in mortality, complications, or the length of hospital stay. In terms of price, however, the robot surgery cost significantly more. Due to a lack of relevant literature and longitudinal data, it was difficult to draw a conclusive result based on these observations. For an efficient development of robotic surgery devices, an active mutual collaboration between medical experts and robot development-related experts is needed, and a long-term large-scale national investment must go hand in hand with these efforts. To promote the research and development for robotic surgery equipments, related organizations need to take the following roles:
▪ NECA should further conduct relevant evaluations such as cost-effectiveness analysis and should continue to present supportive scientific data.
▪ The Ministry of Food and Drug Safety should reasonably improve its regulations so that Intuitive Surgical, Inc. and local companies, as latecomers in robot surgery development, essentially achieve parity.
▪ The Health Insurance Review and Assessment Service should reflect a proper level of medical expenses for robotic surgery as compensation and wages.
▪ The company needs an investment source for clinical tests due to its poor financial status. It should strive to develop domestically manufactured robots that will lead the way for Korea’s health care field.
There will be an increase in the number of robotic surgeries in years to come, when taking many things into account ranging from the government keynote, which includes health care industry promotion and technological competitiveness enhancement, to the advantage of minimally invasive surgeries and the medical reality that there is a current lack of surgeons.
▢ Research results on the roundtable conference on prostate cancer robotic surgery
Ⅰ. Administrative procedure
The roundtable conference on the robotic surgery for the patient with prostate cancer was conducted in conjunction with the roundtable conference on stomach cancer robotic surgery under the theme of "Controversy of and Future Prospects for Robotic Surgery," so the operation plan and the detailed operational procedure were the same as those for the stomach cancer robotic surgery roundtable conference.
Ⅱ. Results of the plenary meeting
Robotic surgery was first used in urology and has been most commonly implemented. Compared to laparotomy or laparoscopic operation, robotic surgery shows fewer complication such as urinary incontinence, a decline in sexual functions, and less bleeding during surgery. There is also an advantage in that doctors who perform this type of surgery feel less tired. Furthermore, robotic surgery causes less damage to peripheral organs, reduces hospital stays, and is relatively less painful since it enhances the visual field into three dimensions during surgery. Even in a systematic literature review and meta-analysis results, robotic surgery shows better results in the aspect of functional performance than traditional surgery.
In terms of oncological results, longitudinal follow-up results are needed since did not show a difference in occurrence risks. As far as robotic surgery for prostate cancer, its safety and effectiveness has already been proven. First of all, the robotic surgery for prostate cancer should be placed into the health insurance wage system, and this would also help overcome the polarization of medical service usage between high- and low-income classes. As in the case of robotic surgery for stomach cancer, to reduce the current financial burden, it is urgent to develop domestically manufactured robotic surgery devices. An economic evaluation that appraises cost effectiveness per item also needs to be pursued.
▢ Research results on the roundtable conference on dementia diagnosis
Ⅰ. Administrative procedure
Steering committee meeting and a preliminary meeting was held to discuss a detailed operation of the NECA Roundtable Conference on Alzheimer's disease diagnosis on June 20, 2014. The participants were medical experts from the department of nuclear medicine, radiology, neurology and neuropsychology. The plenary meeting was held under the title "Is an early diagnosis possible before the symptoms of Alzheimer's disease emerge?" on September 26, 2014. The presenters were four clinical experts from the expert of he department of nuclear medicine, radiology, neurology. The panel participants were four clinical experts and policy makers.
Ⅱ. Results of the plenary meeting
The neuroimaging used to diagnose Alzheimer's disease includes: structural imaging, such as MRI and CT; functional imaging, such as FDG-PET, functional MRI (fMRI) and arterial spin labeling (ASL); and molecular imaging, such as amyloid PET.
Imaging diagnosis for detection Alzheimer's dementia in early stage could be helpful to develop treatment that actually removes the pathological mechanism and make an early-stage Alzheimer's diagnosis, such as mild cognitive impairment (MCI) which the disease is suspected, but it is hard to diagnose.
But, it would be hardly beneficial for there to be no radical treatment measures, even with a good early diagnosis. In relation to the early diagnostic examination, a guideline for the check-up interval and treatment based on the result of the test.
And if the national coverage for the FDG-PET to detection Alzheimer's disease, an issue of overdiagnosis should be considered carefully.
In summary, this roundtable conference was thought to be instrumental in forming a shared view among clinical experts and policy-makers on the possibility and effectiveness of Alzheimer's early diagnosis, its insurance coverage, etc. Moreover, though there are no grounds valid enough to draw the conclusion that all Alzheimer's patients should be entitled to certain image examinations or insurance benefits, a positive review deems it necessary to explore cases having therapeutic importance in order to develop new treatment. Furthermore, there is a need for R&D on various early examination methods that can be used as low-cost selective checkups, since the above-mentioned FDG-PET is a high-cost examination. To this end, it would be helpful to vitalize collaboration with cognitive and brain scientists, as well as clinical specialists.
▢ Research results on stem cells therapy
Ⅰ. Administrative procedure
Stem cells are greatly anticipated due to the fact that they can cure the diseases that are incurable with other treatment measures, and they are attracting the spotlight as a new economic growth engine. However, reckless medical use of stem cells is emerging as a major social issue, even though the scientific grounds on the safety and effectiveness of stem cell therapy have not been clearly established. Therefore, it was decided to make a guidebook for the patient to give a information based on agreed contents which were discussed at this RTC. In order to publish the patient' guidebook, a steering committee was organized in conjunction with the Korean Society for Stem Cell Research and a planning meeting (September 17, 2014). A preliminary meeting for the production of the guidebook was held on December 23, 2014 with 18 participants from the medical expertises, public sector representatives, and citizen representatives.
Ⅱ. Results of the plenary meeting
The panel participants expressed concerns about the patients were confused about stem cell therapy with unauthorized therapies and exposed to the treatment in the clinical trial stage. Therefore, the participated panels emphasized that patient guidebook have to contain the evidence-based information to ensure the safety of the general public, as well as the patients.
Ⅲ. Follow-up measures
The ultimate goal of the roundtable conference on stem cell therapy was to provide the patients (who were either about to receive or were interested in stem cell therapy) with the information they must know about in an easily understandable manner. Such goals were pursued, and the final contents produced were to be posted on the website of the NECA, the Korean Society for Stem Cell Research, etc. Additionally, the printed guidebook were to be distributed to the public sector via the Ministry of Food and Drug Safety, Korean National Institute of Health, relevant associations such as university hospitals, biotech companies, and pharmaceutical companies, as well as press releases.
▢ Conclusions and suggestions
The NECA RTC on the four theme in 2014 were performed successfully and there is more need to be developed.
First, the transparency of selecting a topic was successful. In this roundtable conference, for a systematic and objective selection of a theme, a demand survey for roundtable conference topics was first conducted, and an executive staff evaluated and prioritized in the quantitative manner.
Second, multidisciplinary panel from various fields, including reporters, policy-makers, clinical experts, basic scientists, jurists, representative of patient association were formed for the RTC, although a large-scale citizen panel was not available.
Third, the openness of the results report were kept by following the Chatham House Rule which is not putting actual names and participant identities in the report.
Fourth, a roundtable conference had more focus on process rather than on results can be evaluated as successful as well, so that in this year's roundtable conference, panel participants were able to freely present and share their views without experiencing the pressure of eliciting an agreement, thus creating a venue for communication and deliberation on the issues at hand.
In conclusion, to raise the status of the NECA Roundtable Conference into a leading consensus meeting for Korea's healthcare field, the following areas should be improved. First, it is critical to discover the topics necessary for health care policy-making through various ways including topic demand surveys or focus group interviews. Second, links need to be strengthened among relevant organizations such as the Ministry of Health and Welfare, the Health Insurance Review and Assessment Service, and the National Health Insurance Service. Third, efforts should be made to foster a communicative and cooperative relationship by actively engaging relevant policy-makers in the roundtable conferences. Finally, a specific guideline needs to be established for the agreement principles. Roundtable conference results can be applied to decision making in healthcare policies through these efforts.
