▢ 서 론
견관절 회전근개파열은 흔한 어깨 질환으로 통증과 기능장애를 초래하여 중고령기 삶의 질을 떨어뜨리는 대표적 근골격계 질환이다. 인구고령화와 건강수명의 강조, 진단기술의 발전으로 치료에 대한 수요와 효과적인 관리에 대한 관심이 증가하고 있으며, 수술을 포함한 다양한 치료법들이 개발되었다. 그러나 중고령기 환자의 회전근개파열의 적절한 치료 적응증에 대한 임상지침의 근거수준이 미약하고 객관적인 근거들이 부족한 실정이다. 따라서 본 연구의 목적은 회전근개파열 환자에 대한 보존치료와 수술치료의 비교 및 체계적 문헌고찰, 그리고 전문가 집단의 인식조사를 통해 임상 근거창출의 기반을 마련하고 향후 수술 및 비수술치료의 전향적 비교효과 관찰연구의 토대를 만드는 것이며 세부 목표는 다음과 같다.
1) 우리나라 회전근개파열 환자의 치료 현황과 추세를 파악한다.
2) 회전근개파열 치료에 대한 체계적 문헌고찰을 통하여 수술치료와 보존치료의 효과를 비교한다.
3) 실제 임상현장에서 치료방침 결정에 이견이 있는 부분파열 고도손상과 전층파열 중 소파열, 중파열 환자들에 대한 후향적 코호트 구축을 통하여 관절경적 회전근개봉합술과 보존치료의 효과를 비교한다.
4) 회전근개파열의 자연경과, 치료결과 및 치료방법 결정에 대한 전문가 인식을 파악한다.
▢ 회전근개파열 치료현황
회전근개파열 상병의 치료현황을 파악하기 위하여 2004년부터 2013년까지 10년간의 국민건강보험공단의 청구자료를 활용하였다. 분석대상자를 선정하기 위하여 청구자료에 어깨병변(M75) 또는 어깨 근육둘레띠의 근육 및 힘줄의 손상(S460)이 있거나, 견봉성형술(N0935), 견봉성형술 및 회전근개파열복원술(일차봉합술)(N0936), 견봉성형술 및 회전근개파열복원술(근 및 건성형이 동반된 경우)(N0937) 코드가 있는 50세 이상의 대상자의 명세서를 분석하였다. 비수술요법의 현황을 파악하기 위하여 상기한 조건에 부합하는 청구자료 중 주사치료, 물리치료, 운동치료, 침치료, 구술치료, 부황, 온냉경락요법 코드를 활용하여 분석하였다.
분석 결과 어깨병변 및 어깨의 근육둘레띠의 근육 및 힘줄의 손상으로 청구된 지급건수는 매년 10.0%~15.0%씩 증가하였으며, 견봉성형술(N0935), 견봉성형술 및 회전근개파열 복원술(N0936, N0937) 건수도 매해 1.3~1.9배씩 증가추세를 보였다. 주사 등 비수술요법 치료 현황도 매년 10.0%~15.0%씩 증가를 보였으며 2010년부터 급여가 시작된 침치료, 구술치료, 부황, 온냉경락요법 청구건수도 증가하는 경향을 나타내었다.
2013년을 기준으로 견봉성형술, 견봉성형술 및 회전근개파열 복원술 진료실 인원은 5만 5854명으로 나타났으며, 이는 2004년 1,555명보다 35.9배 가량 증가한 수치이다. 주사 등 비수술요법 진료실 인원은 208만 2658명으로 최초로 200만 명을 넘는 것으로 파악되었고, 2004년 대비 3.4배 증가하였다. 이 중 의과 진료를 받은 진료실 인원은 147만 3117명이었고 한방 치료를 받은 진료실 인원은 85만 5980명으로 나타났다.
▢ 체계적 문헌고찰을 통한 수술치료와 보존치료의 효과 비교
회전근개파열 치료에 대한 다양한 치료방법에 대한 효과 비교를 위하여 회전근개파열의 보존치료와 수술치료 무작위배정 비교임상시험연구를 중심으로 2014년 9월까지 Ovid-Medline, Ovid-Embase, Cochrane central, AMED, CINAHL, Pubmed 등 국외데이터베이스와 KoreaMed, 의학논문데이터베이스, 학술데이터베이스, 한국교육학술정보원 등 국내데이터베이스를 이용하여 문헌을 검색하였으며, 두 명의 독립적인 검토자가 사전에 정해진 문헌 선정 배제 전략과 문헌추출 서식을 활용하여 자료를 추출하고 Cochrane Risk of Bias 도구를 활용하여 독립적으로 문헌의 질을 평가하였다. 회전근개파열 치료방법의 다양성을 고려하여 치료법간 직접비교한 무작위배정 비교임상시험이 없는 경우에도 치료법간 비교를 위하여 간접비교 분석을 활용하였다. 최종 선정된 문헌은 8편이었으며, 연구에 따라 intention-to-treat(ITT) 분석과 per-protocol(PP) 분석이 혼재되어 있어 본 연구에서는 분석방법을 구분하여 실시하였다.
기능 향상 정도를 보고한 문헌은 총 7편이었으며, ITT 분석군에서는 임상적으로 의미있게 관혈적 회전근개 봉합술 후 운동치료를 실시한 군이 소절개 회전근개 봉합술 후 운동치료를 실시한 군 및 운동치료 단독군에 비하여 유의하게 기능을 향상시켰고(평균차, 11.9와 11.0) 이외의 치료들 사이에는 유의한 기능 향상의 차이는 없었다. PP 분석군에서는 모든 치료방법간 유의한 기능 향상의 차이는 없었다.
관절가동범위는 전방굴곡과 외회전 변화를 제시한 2편의 연구가 분석대상이었으며 두 편 모두 ITT로 분석한 결과값 만을 제시하고 있어 PP분석은 시행하지 않았다. 분석 결과, 관절경적 회전근개 봉합술 후 초기 운동치료를 실시한 군이 소절개 회전근개 봉합술 후 초기 운동치료를 실시한 군보다 치료 후 1년 시점에서 전방굴곡과 외회전 범위가 약 9-10도 유의하게 높게 나타났으나 임상적으로 의미있는 차이는 아니었다.
통증 변화정도는 총 4편의 연구를 대상으로 분석이 시행되었으며 ITT 분석군에서는 관절경적 회전근개 봉합술, 관절경적 회전근개 봉합술과 자가혈치료술 병행, 견봉성형술, 관혈적 회전근개 봉합술이 운동 치료 단독보다 치료 후 1년 시점에서 통증이 감소되었다. PP 분석군은 혈소판 풍부 혈장을 사용한 관절경적 회전근개 봉합술이 소절개 회전근개 봉합술에 비하여 통계적으로 유의한 통증 개선을 보였으나(평균차, 0.4) 임상적으로 의미있는 정도의 차이는 아니었다.
▢ 후향적 코호트 자료를 활용한 성과분석
회전근개파열에 대한 보존치료와 수술치료 중 주로 사용되어지는 관절경적 회전근개 봉합술의 임상결과를 비교하기 위하여 서울 및 경기도 소재의 3개 대학병원에서 2008년부터 2013년까지 회전근개파열로 치료받은 50세 이상 환자 중 선정 배제 기준에 부합하는 대상자의 의무기록을 후향적으로 조사하여 환자등록자료를 구축하였다. 성과 변수는 치료 후 3개월, 6개월, 12개월 시점의 통증과 관절가동범위이며, 기본특성 변수를 활용하여 성향점수 매칭을 시행하였고, 매칭된 대상자를 바탕으로 일반화 추정방식 모형을 사용하여 변화 차이를 파악하였다.
총 연구대상자는 337명이었으며 보존치료법 157명, 수술치료법 180명이었다. 통증변화에 대한 일반화 추정방정식 모형 적합을 실시한 결과, 두 군 모두 시간의 흐름에 따라 통증이 감소하는 경향을 보였으며, 3개월과 6개월 시점에서 수술치료군이 보존치료군에 비해 통계적으로 유의하게 통증이 감소되는 것으로 나타났으나 12개월 시점에는 유의한 통증의 차이는 없었다. 관절가동범위 변화에 대한 일반화 추정방정식 모형적합을 실시한 결과, 3개월 시점에는 수술치료군이 보존치료군에 비해 평균 관절가동범위가 유의하게 작은 것으로 나타났으나 12개월 시점에는 통계적으로 유의한 차이가 없었다. 수술치료군에서 보존치료군으로 전환한 환자는 5.0%, 보존치료군에서 수술치료로 전환한 환자는 5.7%로 나타났으며, 12개월 시점에 수술치료군에서 재파열은 8.3%에서 보고되었다.
▢ 회전근개 치료 및 관리에 대한 전문가 인식조사
회전근개파열의 치료 및 관리에 대한 여러 분야의 전문가 인식을 파악하여 효과적인 치료 및 관리 프로그램 개발에 정보를 제공하고자 정형외과, 재활의학과, 류마티스내과, 마취통증의학과, 가정의학과 등 회전근개파열 환자를 진료하는 전문의 226명을 대상으로 2014년 11월 4일부터 2014년 12월 7일까지 온라인 설문조사를 실시하였다. 회전근개파열 환자에 치료를 제공하지 않고 자연경과관찰만 하는 경우 파열의 크기가 증가(146명, 64.6%)하고 일상생활수행정도가 악화(143명, 63.3%)될 것이라는 응답이 과반수를 차지했고, 84명(37.2%)이 통증이 증가할 것이라고 응답하였다. 보존치료를 제공하는 경우 파열의 크기는 변화가 없고(81, 35.8%) 통증은 감소되며(188명, 83.2%), 일상생활수행정도는 향상될 것(170, 75.2%)이라는 응답이 가장 많았다. 수술치료를 적용하면 통증이 감소하고(164명, 72.6%) 일상생활수행정도가 향상(174명, 77%)될 것이라는 응답이 가장 많았다. 어떤 치료도 적용되지 않는 경우 일상생활수행 정도가 악화될 것이라는 의견을 제외하고는 전문가 군간 공통의견이 발견되지 않았다. 회전근개파열의 치료관리를 수행하지 않는 경우 일반적인 자연경과에 대한 의료인간의 인식 일치정도를 묻는 질문에는 ‘논란이 있다’(112명, 49.6%)와 ‘매우 논란이 많다’(18명, 8%)라고 응답하여 반수 이상이 논란이 있는 것으로 인식하고 있었다.
수술치료 적용을 위한 의사결정에 있어 임상적으로 중요도를 가지는 요인으로는 환자의 나이가 4.45점으로 가장 높았고, 뒤를 이어 영상의학적 평가에 의한 파열 종류(4.37점), 환자의 일상생활수행정도(4.26점), 증상 및 이학적 검사의 회전근개파열과 일치도(4.22점), 허약(Weakness)(4.19점), 통증정도(4.14점), 직업(4.13점), 영상의학적 평가에 의한 파열크기(4.09점)순으로 나타났다.
환자 특성을 외상 유무, 증상 유무, 전층 파열 여부, 파열 크기 등에 따라서 13가지로 분류하여, 해당 환자의 상태가 수술치료와 보존치료 중 어떤 치료에 적절한 적응증으로 판단되는지 설문한 결과 응답자의 70% 이상이 동일한 치료법이 적절하다고 응답한 적응증은 비외상성 유증상 회전근개 전층파열에서 파열이 1cm 미만인 경우(166명, 73.5%), 비외상성 유증상 회전근개 부분파열 경도손상인 경우(213명, 94.2%), 외상성회전근개 전층 파열에서 파열이 1cm 미만인 경우(164명, 72.6%), 외상성 회전근개 부분파열 경도손상인 경우(201명, 88.9%) 보존치료가 적절하다는 응답이 전문가 50% 이상 답변한 다수의견이었다. 비외상성 유증상 회전근개 전층파열에서 파열크기가 5cm이상인 경우(166명, 73.5%), 외상성 회전근개 전층파열에서 파열이 3~5cm 미만인 경우(167명, 73.9%), 외상성 회전근개 전층파열에서 파열이 5cm 이상인 경우(191명, 84.5%)는 수술치료가 다수의견으로 나타났다. 비외상성 무증상 회전근개 전층 파열과 비외상성 유증상 회전근개 부분 파열(경도 손상)은 보존치료가 일차적이며 비용효과적 선택이라는 의견이 전문가 50% 이상 찬성하고 전문분과별 차이도 없는 공통의견으로 수렴되었으나, 그 밖의 경우는 전문분과별 차이를 보였다.
▢ 결론 및 정책적 제언
국내 회전근개파열 환자수와 치료 수요 증가에 따라 수술치료 및 보존치료 시행 빈도와 의료비가 지난 10년간 빠르게 증가해 왔고, 앞으로도 지속적으로 늘어날 것으로 예상된다. 체계적 문헌고찰과 후향적 관찰연구를 통해 회전근개파열에 대한 수술치료와 보존치료의 임상 결과를 비교하였을 때, 수술 및 보존치료 모두 치료 전에 비하여 기능상태, 통증, 관절가동범위 등이 유의하게 향상되었다. 그러나 수술치료와 보존치료 중 더 효과적인 치료법에 대해 결론을 내릴 수 있을 만큼의 양질의 근거는 부족하였다. 또한 전문가 설문조사를 통해 관련 전문분과 간 적절한 치료와 적응증에 대한 판단기준 불일치가 존재함을 알 수 있었다. 치료 성과에 대한 미약한 근거 수준과 전문가 간 이견으로 현재 임상 현장에서 많은 혼란이 야기되고 있는 실정이다.
미국은 2010년부터 국립보건원(NIH)에서 지원하는 회전근개질환 워크그룹을 통해 정형외과와 재활의학과 전문의가 협업하는 다기관 전향적 추적 관찰 연구를 진행하여 근거 수준이 높은 연구 결과들을 발표하고 있다. 또한 AHRQ와 미국정형외과학회를 중심으로 회전근개 질환의 수술치료와 보존치료 적응증에 대한 전문가 합의를 통한 임상진료지침을 지속적으로 개발하여 개선하고 있다. 우리나라는 회전근개 질환의 진단과 치료에 있어 세계적인 수준의 임상결과와 연구논문을 발표하고 있어 국제적으로 주목받고 있으나, 이러한 노력에도 불구하고 전문가 합의와 표준적 임상지침은 미약한 형편이다. 따라서, 앞으로 전향적 비교효과 관찰연구를 통해 양질의 근거 창출 노력과 전문가 토의를 통한 합의 도출이 시급하다. 회전근개파열 치료에 대한 적응증에 영향을 미치는 요인에 대한 본 연구결과를 바탕으로 적절한 치료 대상군을 선별하고 의료비의 효율적 활용에 기여할 수 있는 양질의 임상 연구 및 경제성 분석 연구가 필요하다. 이를 위해서 첫째, 우리나라 회전근개질환 다기관 참여 전향적 코호트를 구축할 것을 제안한다. 둘째, 회전근개질환 진단, 치료, 예방 관리 및 보건 분야의 전문가들이 참여하여 토론과 합의도출을 위한 원탁회의의 개최를 제안한다.
▢ Introduction
Due to population aging, emphasis on prolonged and healthy lives, and advancement in radiologic diagnostics, the interest in the most effective treatment and management of rotator cuff tears has been growing in recent years, leading to the development of various novel or improved treatment methods, including surgeries. However, the clinical evidence for the proper therapeutic indications of rotator cuff tears in middle-aged and elderly patients is inadequate, and objective evidences are still lacking.
Therefore, the main objective of the present study was to compare the effects of conservative versus surgical treatment in patients with rotator cuff tears by: 1) identifying the status of and trends in treatments for patients with rotator cuff tears in Korea; 2) comparing the effects of surgical treatment versus conservative treatment through a systematic review of the literature on the treatment of rotator cuff tears; 3) comparing the effects of arthroscopic rotator cuff repair versus conservative treatment by establishing a retrospective cohort of patients with small and medium complete tears, as well as high-grade partial tears, for which the decision on the treatment method is difficult in the actual clinical setting; and 4) identifying the expert perspectives on the treatment of rotator cuff tears.
▢ Current state of treatments for rotator cuff tears
To identify the current state of treatments for rotator cuff lesion pathology , claims data from the National Health Insurance Service, covering 10 years between 2004 and 2013, were used. For the selection of the study subjects, claims statements of those 50 years or older who had shoulder lesions (code: M75), damages to the rotator cuff muscles or tendons (S460), acromioplasty (N0935), acromioplasty and rotator cuff tear repair (primary suture) (N0936), and/or acromioplasty and rotator cuff tear repair (accompanying myoplasty and tenoplasty) (N0937) were analyzed. The current state of non-surgical therapy was analyzed using the codes for injection therapy, physical therapy, exercise therapy, acupuncture, bead therapy, cupping therapy, and hot-and-cold meridian therapy in patients who met the above conditions.
The analysis results revealed that the number of claims paid for shoulder lesions and damages to the muscles and tendons of the shoulder rotator cuff increased by 10.0~15.0% annually, while the numbers of acromioplasty (N0935) and acromioplasty and rotator cuff tear repair (N0936, N0937) also showed increasing trends every year. Similarly, the state of non-surgical therapies, including injection therapy, also showed an annual increase of 10.0~15.0%, and the number of claims for acupuncture, dialectical royalty fee, bead therapy, cupping therapy, and hot-and-cold meridian therapy, which were covered starting from 2010, showed increasing trends as well.
As of 2013, the number of patients who underwent undergoing acromioplasty or acromioplasty and rotator cuff tear repair was 55,854, while the number of patients treated by non-surgical therapies, such as injection therapy, was 2,082,658, exceeding 2 million for the first time. Among these patients, the number of patients who receiveding Western, or conventional, treatment was 1,473,117, while patients who receiving received Chinese, or alternative, medicine treatment was 855,980. Based on the claim data, which contained both the disease and surgery codes, the mean duration of hospital stay was 6.5 days for top-tier general hospitals, 14.6 days for general hospitals, 10.5 days for clinics, and 10 days for Oriental medical centers, with the duration of stay ranging from 8.1 to 18.3 days, depending on the type and location of the care facility.
▢ Comparison of the effects of surgical treatment versus conservative treatment through a systematic literature review
To compare the effects of the various treatment methods for rotator cuff tears, we conducted a thorough systematic review (until September 2014), using a total of 10 databases, including Ovid-Medline, Ovid-Embase, Cochrane central, AMED, CINAHL, and PubMed from overseas, and KoreaMed, Korean Medical database, academic journal database, and Korea Education and Research Information Service from Korea, with emphasis on randomly-assigned comparative clinical trials on conservative and surgical treatments of rotator cuff tears. Two independent reviewers extracted the data using a predetermined literature exclusion strategy and extraction format. Furthermore, the quality of the literature was assessed independently by using the Cochrane Risk of Bias tool. Considering the variability in rotator cuff tear treatment methods, an indirect comparative analysis was used to compare the treatments when randomly-assigned comparative clinical trials that directly compared the treatments were unavailable.
A total of 8 studies were finally selected, and since intention-to-treat (ITT) analysis and per-protocol (PP) groups were intermixed in some studies, analyses on both of these analysis groups were conducted in the present study.
Seven studies reported on the levels of functional improvement. In the ITT analysis groups, patients who received physiotherapy after open rotator cuff repairs showed clinically significant functional improvements compared to patients who received physiotherapy after mini-open rotator cuff repair and those who only received physiotherapy (mean differences, 11.9 and 11.0 points, respectively). There was no significant difference in the functional improvement between the other treatments. In the PP analysis groups, studies that included open rotator cuff repair were excluded, and no significant differences in the functional improvements were observed between all treatments, including for stand-alone physiotherapy.
In terms of the range of motion (ROM), two studies on forward flexion and external rotation were analyzed, and since both studies presented only the values analyzed by ITT, PP analysis was not performed. The analysis results revealed that the group that received early passive motion physiotherapy after arthroscopic rotator cuff repair showed a significantly higher ROM of approximately 9–10˚ in forward flexion and external rotation one year after the treatment compared to the group that received initial physiotherapy after mini open repair of the rotator cuff. However, the differences were not clinically significant.
For the level of change in terms of the pain, 4 studies were analyzed. In the ITT analysis group, arthroscopic rotator cuff repair, combination of arthroscopic rotator cuff repair and platelet-rich plasma therapy, acromioplasty, and open rotator cuff repair showed decreases in pain one year after treatment compared to e physiotherapy alone. In the PP analysis group, after excluding studies on physiotherapy that reported only the results of ITT analyses, arthroscopic rotator cuff repair using platelet-rich plasma were found to be associated with a statistically significant improvement in pain compared to mini open repair of the rotator cuff (mean difference, 0.4 points); however, the difference was not considered clinically significant.
▢ Outcomes analysis using retrospective cohort data
To compare the outcomes of arthroscopic rotator cuff repair, which is the most commonly used treatment for rotator cuff tears, and conservative treatment, a patient registration database was established by retrospectively investigating the medical records of subjects 50 years or older who received treatments for rotator cuff tears between 2008 and 2013 from 3 university hospitals located in Seoul and Gyeonggi-do and who met all the following selection criteria :patients who had high-grade partial (fiber disruption >50%) or small-to-medium-sized full thickness tears (≤3 cm) were included in this study. Patients were excluded if they had 1) severe adhesive capsulitis (<120° in forward flexion or <40° in external rotation), 2) neuropathy affecting the shoulder girdle muscles, or 3) pre-existing low-grade partial-thickness (fiber disruption < 50%) or large-to-massive-sized full-thickness tears (≥3 cm). The outcome variables included the pain and ROM at 3, 6, and 12 months after treatment, and propensity score matching was performed using basic characteristic variables, from which differences in the changes between different treatments were identified using a generalized estimation system model with the matched subjects as the basis.
A total of 337 study subjects were included in the analysis, including 157 who received conservative treatment and 180 who received surgical treatment. The result of applying the generalized estimating equation for the changes in pain demonstrated that both groups showed a tendency of decreased pain over time; at 3 and 6 months, the surgical treatment group showed a statistically significant decrease in pain compared to the conservative treatment group, whereas no significant difference in pain was observed at 12 months.
In terms of the ROM, the surgical treatment group had a significantly smaller mean ROM compared to the conservative treatment group at 3 months, while no significant difference was noted at 12 months.
A total of 5.7% and 5.0% of patients switched from surgical to conservative treatment and vice versa in the conservative and surgical treatment groups, respectively, while the re-tear rate at 12 months was reported to be 8.3% in the surgical treatment group.
▢ Survey of expert perceptions on the treatment and management of rotator cuff lesions
An online survey was conducted between November 4th and December 7th 2014 on 226 specialists from the fields of orthopedics, rehabilitative medicine, rheumatology, anesthesiology, and family medicine who treat patients with rotator cuff tears, to examine the multi-disciplinary expert perceptions on the appropriate management of rotator cuff tears, with the aim to establish effective treatment and management programs. In terms of the natural progression of rotator cuff tears, the most common perceptions were that the size of the tear would increase (n=146, 64.6%) and that the activities of daily living would worsen without treatment (n=143, 63.3%). In terms of the pain of these patients, most experts (n=84, 37.2%) responded that it would increase. Regarding conservative treatment, the most common perceptions were that the size of the tear would remain the same (n=81, 35.8%), that the pain would decrease (n=188, 83.2%), and that the activities of daily living would be improved (n=170, 75.2%). Finally, in terms of surgical treatment, the most common perceptions were that the pain would decrease (n=164, 72.6%) and that the activities of daily living would be improved after surgery (n=174, 77%).
Regarding clinicians choosing not perform therapeutic management of rotator cuff tears, the consensus between the doctors who do perform treatments was that general monitoring of progression was viewed as ‘controversial’ (n=112, 49.6%) or ‘highly controversial’ (n=18, 8%), suggesting that there is discrepancy between the experts.
With respect to factors with clinical significance for the decision-making of applying surgical treatment, as assessed on a 5-point Likert scale , the patient age had the highest score, with 4.45 points, followed by the type of tear based on the radiologic evaluation (4.37 points), level of patient activities of daily living (4.26 points), agreement between the symptoms and physical examination of the rotator cuff tear (4.22 points), weakness (4.19 points), pain severity (4.14 points), occupation (4.13 points), and size of the tear based on the radiologic evaluation (4.09 points).
Next, the patient characteristics were classified into 13 types according to the presence of injury, presence of symptoms, full-thickness tear, and the size of the tear, and a survey was conducted on whether representative sets of patient characteristics indicated surgical or conservative treatment. The indications for which more than 70% of the respondents agreed on the treatment totaled 7, including: conservative treatment for symptomatic non-traumatic full-thickness rotator cuff tear with tear size smaller than 1 cm (n=166, 73.5%); surgical treatment for symptomatic non-traumatic full-thickness rotator cuff tear with tear size bigger than 5 cm (n=166, 73.5%); conservative treatment for symptomatic non-traumatic full-thickness rotator cuff tear with mild impairment (n=213, 94.2%); conservative treatment for traumatic full-thickness rotator cuff tear with tear size smaller than 1 cm (n=164, 72.6%); surgical treatment for traumatic full-thickness rotator cuff tear with tear size smaller than 3~5 cm (n=167, 73.9%); surgical treatment for traumatic full-thickness rotator cuff tear with tear size bigger than 5 cm (n=191, 84.5%); and conservative treatment for traumatic partial rotator cuff tear with mild impairment (n=201, 88.9%).
▢ Conclusion and policy recommendations
The medical costs associated with surgical and conservative treatments of rotator cuff tears are constantly increasing in Korea. Although both surgical and conservative treatments for rotator cuff tears have been shown to significantly improve the functional state, pain, and ROM compared to before the treatment, strong evidence for determining which of the two treatment methods is more effective is still lacking. Furthermore, there are still disagreements between specialists who diagnose and treat rotator cuff tears in terms of the most appropriate treatment and the surgical indications, causing clinical confusion.
Therefore, based on the findings of the present study, further clinical trials and feasibility studies on the factors that influence the indications for the appropriate treatment of rotator cuff tears are needed to help select the appropriate treatment and to ensure effective utilization of medical expenditure.
