▢ 연구배경 및 목적
스타틴은 심혈관계 질환의 일차 및 이차 예방약제로 아스피린과 더불어 임상에서 가장 많이 사용되고 있으며, 2013년 변경된 고지혈증 치료 가이드라인으로 스타틴의 사용이 증가할 것으로 예상된다. 따라서 스타틴의 장기 안전성 평가가 중요하나 임상현장에서 대규모 환자를 대상으로 이를 파악한 연구는 매우 부족한 실정이다. 또한 스타틴의 장기간 사용이 제 2형 당뇨병 발생 및 이에 따른 이차적 심혈관계 합병증 발생에 미치는 영향에 관한 연구 역시 부족한 상황이다. 이에 본 연구는 건강보험청구자료 및 국가건강검진자료를 이용하여 스타틴 사용과 당뇨병 및 심혈관계 질환 발생의 상관관계를 파악하고, 스타틴 사용기간, 누적용량 및 당뇨병의 위험인자에 따른 당뇨병 및 심혈관계 질환 발생률을 비교하고자 수행되었다.
▢ 연구방법
스타틴 사용 권고군(eligible)에서 스타틴 사용에 따른 당뇨병 발생 및 심혈관계 질환 발생 위험을 파악하기 위하여 국민건강보험공단의 건강보험청구자료, 자격자료, 국가건강검진자료 및 통계청 사망원인자료를 이용하여 후향적 코호트 연구를 실시하였다.
연구 대상자는 연구기간(2005.1.1.~2012.12.31.)동안 국가건강검진을 받은 40세 이상 수검자 중 1) 총콜레스테롤이 240mg/dL 이상이면서, 2) 심혈관계 질환 관련 입원 기록이 없는 경우를 포함 하였다. 대상자 중 연구 입적일 이전 3년 동안 암, 당뇨병, 심혈관계 질환이 있거나 지질저하제를 처방 받은 대상자는 제외하였다. 그 외에 연구 입적일 1년 이내 사망자, 입적일 6개월 이내 당뇨병 및 심혈관계 질환 발생 대상자, 스타틴 1회 사용자 등은 제외하였다.
연구기간 중 스타틴을 6개월 이내 최소 2회 이상 처방받은 경우 스타틴군으로 정의하였으며, 두 번째 스타틴 약물을 처방받은 명세서의 최초 요양개시일을 약물시작일(Index date)로 정의하였다. 연구기간 동안 스타틴을 한 번도 처방받지 않은 경우 비스타틴군으로 정의하였으며, 총콜레스테롤 240mg/dL 이상이 파악된 최초 검진 일을 연구 입적일로 정의하였다.
결과지표 중 1차 결과(primary outcome)는 제2형 당뇨병 발생(당뇨병 상병코드가 있으면서 경구혈당강하제 또는 인슐린을 처방받은 경우)이며, 2차 결과(secondary outcome)는 당뇨병 합병증 중 주요 심혈관계 질환 발생(심혈관계 질환 관련 사망, 심근경색입원, 뇌졸중입원)이었다.
스타틴 사용여부별 당뇨병 및 심혈관계 질환 위험도는 성, 연령, 공복혈당, 체질량지수, 흡연, 운동, 고혈압, 찰슨 동반상병지수(Charlson Comorbidity Index, CCI) 및 심혈관계 병용약물 등을 고려한 성향점수(Propensity Score, PS) 매칭 후 콕스비례위험모형을 사용하여 제시하였다. 또한 스타틴의 사용기간, 누적용량 (일일상용량(Defined Daily Dose, DDD))(사용일수)로 산출. 일일상용량이란 WHO에서 정의된 해당의약품의 주성분별로 주요 적응증에 대해 70kg의 성인이 1일 동안 복용해야 하는 평균 유지용량임.
, 당뇨병의 위험요인 및 스타틴의 종류별 하위그룹으로 분석하여 비스타틴군 대비 스타틴군의 하위그룹별 위험도를 파악하였다.
▢ 연구결과
연구대상자는 2,162,119명이었으며 이 중 스타틴군은 638,625명(29.5%), 비스타틴군은 1,523,494명(70.5%)이었다. 성향점수 매칭 이후 최종 분석에 포함된 대상자는 스타틴군과 비스타틴군 각 518,491명이었다. 매칭 이전 스타틴군의 평균연령은 56세로 스타틴군(52세) 보다 높았으며, 고혈압 및 심혈관계 병용약물 복용비율이 높았으나 성향점수 매칭을 통해 일반적 특성 간 불균형은 보정하였다.
스타틴군 및 비스타틴군에서 당뇨병 발생률은 1,000 person-year 당 각각 13.36 및 6.85로 스타틴군에서 높았다. 매칭 이후 콕스비례위험모형으로 분석한 결과 비스타틴군 대비 스타틴군의 당뇨병 발생위험도는 1.88(95% CI:1.85-1.93)이었다. 스타틴의 누적용량이 많은 군의 당뇨병 발생위험이 높았으며(HR:2.52, 95% CI:2.47-2.57), 사용기간이 2년이 넘는 경우 발생위험이 가장 높았다(HR:2.62, 95% CI:2.56-2.67). 당뇨병 위험요인별로(공복혈당장애, 체질량지수, 운동실천, 당뇨병의 위험인자 개수) 층화하였을 때도 위험요인여부와 무관하게 일관적으로 비스타틴군 대비 스타틴군에서 당뇨병 발생위험이 높았다.
한편 스타틴군은 비스타틴군에 비해 심혈관계 질환 발생위험도가 33% 낮았으며(HR: 0.67, 95% CI:0.61-0.74), 복용기간이 2년을 넘는 경우(HR:0.72, 95% CI: 0.62-0.82) 및 누적용량이 많은 경우에서 발생위험도가 유의하게 낮았다(HR:0.77, 95% CI: 0.68-0.87). 당뇨병의 위험요인별로 층화하였을 때 위험요인여부와 무관하게 일관적으로 스타틴군의 심혈관계 질환 발생위험도가 비스타틴군보다 낮았다.
▢ 결론 및 정책적 제언
현재까지 스타틴 사용과 당뇨병 발생을 다룬 대부분의 연구는 무작위 임상시험의 메타분석이나 제한된 인구를 대상으로 한 관찰 연구였다. 스타틴이 심혈관계 질환의 일차 및 이차예방의 주요약제로 광범위하게 사용되고 있는 현 시점에서 다양한 특성을 갖는 대규모 환자를 대상으로 스타틴과 당뇨병 발생의 연관성을 심도 있게 검토한 연구는 극히 부족한 실정이었다. 이에 비해 본 연구는 공공자료원을 이용한 실제 임상현장에서 대규모 인구를 대상으로 스타틴 사용에 따른 당뇨병 및 심혈관계 질환 발생의 위험도를 파악하였다. 그 결과 스타틴 권고군 중 스타틴을 사용하였고, 특히 사용기간이 길며 누적용량이 많은 군에서 제2형 당뇨병 발생위험은 비스타틴군보다 유의하게 높았으며, 심혈관계 질환 위험도는 비스타틴군보다 유의하게 낮았다.
최근에 발표된 미국의 스타틴 사용지침(2013 ACC/AHA guideline)은 10년 전의 사용지침(ATP-III guideline)에 비하여 스타틴 권고 대상자를 1.5배 이상 폭 넓게 포함하였기에 향후 스타틴 사용의 빈도는 더욱 증가될 것으로 보인다. 또한 국내에서도 식생활의 변화, 비만 인구의 급격한 증가로 인하여 심혈관계 질환 위험인자가 증가하면서 해당 질환에 대한 일차예방이 매우 중요시되는 상황이다. 따라서 본 연구는 심혈관계 질환 일차 예방을 위한 스타틴 사용이 권고되는 상황에서 대규모 스타틴 권고 대상자를 바탕으로 스타틴이 당뇨병 발생에 미치는 영향을 평가함으로서 스타틴 사용의 이득과 위험도(benefit-risk ratio)를 재평가하는 중요한 근거가 될 수 있을 것으로 판단된다. 또한 기존의 서양인을 대상으로 진행된 임상연구가 아닌 국내 환자를 대상으로 당뇨병 발생의 위험성을 평가하였기에 향후 보건의료 정책 반영이나 한국형 스타틴 사용지침을 정하는데 매우 유용한 자료로 활용될 것으로 사료된다.
다만 당뇨병 발생 부작용에 대한 우려로 스타틴 권고 대상자에게 스타틴 사용을 중단하거나 주저하는 경우 심혈관계 질환 예방 등 치료기회를 놓칠 수 있으므로 적절한 임상적 판단이 필요하다. 특히 동맥경화성 심혈관계 질환자 및 심혈관계 고위험군 환자에서는 부작용에 비하여 스타틴의 심혈관계 보호효과가 매우 크므로 이러한 환자군에서는 반드시 해석에 주의를 기울이는 것이 필요하다. 또한 본 연구는 임상변수가 제한적인 공공자료원을 이용한 후향적 코호트 연구이므로 이러한 제한점에 대한 충분히 해석상의 이해가 필요하며 향후 대규모 전향적 연구를 이용한 추가적인 결과 검토가 필요할 것이다.
▢ Background and purpose
Statins are primary and secondary preventive drugs for cardiovascular diseases. It is the most widely used agent along with aspirin and its use is expected to increase due to recent updates of guideline on the treatment of hyperlipidemia. Despite the fact that long term safety evaluation of Statin is crucial, only few studies are available with large patient cohorts in the clinical setting. Moreover, studies investigating the association between long term usage of Statins and the onset of type II diabetes as well as subsequent secondary cardiovascular complications are very limited. Therefore, this study was conducted to understand the relationship between the Statin use and the occurrence of diabetes and its complications as well as to compare the incident rate of diabetes and its complications in terms of duration of use, types, and doses of Statin based on the National Health Insurance claims data and the National Periodic Health Examinations data.
▢ Methods
A retrospective cohort study was performed using claims data, eligibility data, and the National Periodic Health Examinations data from the National Health Insurance Service, and cause of death data from the Statistics Korea to evaluate the risk of diabetes and cardiovascular complications related to Statin use in the Statin eligible group.
Subjects with more than 240mg/dl in total cholesterol and no history of medical use related to cardiovascular diseases were eligible among persons aged over 40 who received a health examination between 1/1/2005 and 12/31/2012.
Subjects who had cancer, diabetes, or cardiovascular diseases, or who were prescribed for lipid lowering agent in last 3 years before enrolling the study were excluded. Additionally, subjects who died within a year, who took Statin one time, or who have diabetes or cardiovascular diseases within 6 months after enrolling the study were excluded.
Statin group was defined as who were prescribed for Statin more than two times within 6 months during the study period. Index date was set to be the initial date of the first treatment in prescriptions with more than two treatments. Non-Statin group was defined as who were never prescribed for Statin during the course of study and the enrollment date was determined by when the total cholesterol level was reported as more than 240mg/dl for the first time.
Primary outcome was the incidence of type II diabetes (cases with disease code and prescribed with oral hypoglycemic agents or insulin) and secondary outcome was the incidence of major cardiovascular diseases among diabetes complications (death due to cardiovascular diseases, or hospitalization due to myocardial infarction or stroke).
Risks of diabetes and cardiovascular diseases based on Statin use was presented using the Cox's proportional hazards model after matching to the propensity score taking into account sex, age, fasting blood sugar, body mass index, smoking, exercise, hypertension, Charlson Comorbidity Index (CCI), and concomitant cardiovascular medications. Also, subgroup analyses were conducted based on the defined daily dose (DDD) of Statin, (DDD is defined as the average dose that an adult with 70kg needs to take for one day for the major indication with each principal component of the agent), duration of Statin use, and types of Statin and the metabolic risk in order to understand the degree of risk compared to the non-Statin group.
▢ Results
Among the 2,162,119 of enrolled patients, the number of subjects in Statin group and non-Statin group was 638,625 (29.5%) and 1,523,494 (70.5%), respectively. The number of subjects included in the final analysis after propensity score matching was 518,491 each in Statin and non-Statin group. Before the matching, the mean age of Statin group was 56 year, higher than the non-Statin group (52 year), and the Statin group had higher rate of taking concomitant medications for hypertension and cardiovascular diseases. The differences in general characteristics were adjusted by the propensity score matching.
The incident rates of diabetes in Statin group and non-Statin group were 13.36 and 6.85 out of 1,000 person-year, respectively, indicating the Statin group had higher risk of diabetes. Analysis with the Cox's proportional hazards model after propensity score matching showed that the degree of incident risk for diabetes in Statin group compared to non-Statin group was 1.88 (95% CI: 1.85-1.93). Risk for diabetes increased in accordance with higher dose (HR: 2.52, 95% CI: 2.47-2.57) and the highest risk was observed in a group with Statin use longer than 2 years (HR: 2.62, 95% CI: 2.56-2.67). When stratified with diabetes risk factors (defects in fasting blood sugar, BMI, exercise, and number of diabetes risk factors), the Statin group had consistently higher incidence of diabetes regardless of the factors.
On the other hand, the Statin group appeared to have lower incident risk of cardiovascular diseases compared to the non-Statin group (HR: 0.67, 95% CI: 0.61-0.74). Subjects with high dose (HR: 0.77, 95% CI: 0.68-0.87) and subjects with longer than 2 years of administration (HR: 0.72, 95% CI: 0.62-0.82) presented significantly low degree of incident risk. Based on the subject characteristics regarding diabetes risk, the incidence of cardiovascular diseases was lower in the Statin group than the non-Statin group.
▢ Conclusions and Recommendations
Most studies so far on Statins and its role in diabetes onset have been conducted as meta-analysis of randomized clinical trials or observational studies with limited subject number. Currently, Statins are widely used as major primary and secondary preventive agents in cardiovascular diseases. Nonetheless, with our best knowledge, no study has been undertaken thoughtfully on Statins and its relationship with diabetes onset based on a large size of patient cohorts with various characteristics in daily clinical settings. In this study, by utilizing public resources, a large population was analyzed on the association between Statin use and incident risk of diabetes as well as cardiovascular diseases. In the patients recommended to use Statins, use of Statins, higher DDD, and longer period of administration were related to be significantly higher risk for type II diabetes compared to the non-Statin group, whereas risk for cardiovascular diseases was lower in the Statin group compared to the non-Statin group.
According to the recently published guideline on Statins in the United States (2013 ACC/AHA guideline), the number of patients with the indications of Statin use has been increased by more than 1.5 fold compared to the past guideline published over 10 years ago (ATP-III guideline), and the frequency of Stain use is expected to increase further. Moreover, primary prevention of cardiovascular diseases becomes crucial in Korea as the risk factors for those diseases increase due to changes in dietary life and rapid increase of obese people. Therefore, in today’s medical environment where Statin use is recommended for primary prevention of cardiovascular diseases, this study will provide fundamental information to reevaluate the benefit-risk ratio of Statin use from a large scale cohort with Statin use. In addition, this study was conducted to investigate the risk of diabetes onset in Korean population in contrast to previous clinical studies targeting Western population, suggesting this study can be highly informative when applying health policies or establishing a Korean guideline for Statin use.
As this study is a retrospective cohort study using public data with limited parameter, careful interpretation of the results with sufficient understanding of limitations is required, and further confirmation of the results is needed in the future through conducting a large scale prospective study.
