▢ 서 론
패혈증으로 인한 높은 입원률, 사망률 및 비용으로 인하여 패혈증 관리에 대한 관심이 증가하고 있다. 최근 패혈증 진료지침에서는 패혈증 초기 소생 치료 이후 2차 감염이 일어나지 않는 환자에 대하여 항생제 중단여부를 판단할 때 프로칼시토닌 검사를 이용할 것을 권고하고 있다. 프로칼시토닌 검사법에 따라 불필요한 항생제의 사용을 줄임으로써 항생제 관련 합병증을 감소시키고 나아가 의료비용 절감의 효과도 기대가 되나, 국내 환자를 대상으로 한 비교연구가 없으며 프로칼시토닌 수준을 바탕으로 항생제 치료 기간이 단축되는 것에 대한 경제성 평가 또한 부족하다.
따라서 국내 패혈증 의심 또는 확진 환자를 대상으로 2차 감염이 일어나지 않을 때 항생제 중단 여부를 결정할 수 있는 지표로서의 프로칼시토닌 검사의 유용성을 파악하여, 프로칼시토닌 검사가 안전하게 항생제 치료 기간을 단축시키며 이러한 접근이 경제적으로 타당한지에 대한 근거가 필요하다.
본 연구에서는 패혈증 의심 환자에게 항생제를 투여할 때, 프로칼시토닌 수준을 바탕으로 항생제 치료 중단을 결정하는 것에 대한 임상적 효과를 평가하고 경제성 분석을 통하여 프로칼시토닌 검사와 관련한 정책 결정 및 의사 결정에 근거를 제공하고자 한다.
▢ 프로칼시토닌 검사의 임상적 효과 평가
Ⅰ. 체계적 문헌고찰
패혈증 환자에서 프로칼시토닌 수준에 따른 항생제 치료 조절의 임상적 효과와 일반치료의 효과를 비교하기 위하여 기존의 체계적 문헌고찰을 활용하였다. 미국 AHRQ가 권장하는 연구수행 방법을 토대로 사전에 정한 PICOTS-SD에 따른 체계적 문헌고찰 검색을 시행하고 검색된 문헌의 적절성을 평가하였다. 문헌검색 전략에 따라 EMBASE, Ovid-Medline, KoreaMed 등의 데이터베이스를 검색한 결과 총 134건이 확인되었으며, 이 중 선정 및 제외기준에 따라 최종 7건의 문헌이 선정되었다. 선정된 체계적 문헌고찰에 대해 AMSTAR를 이용한 문헌선택과 자료추출의 적합성, 문헌검색의 포괄성 여부 등의 평가가 수행되었으며, 질평가 결과를 바탕으로 Prkno 등(2013)의 문헌이 최종적으로 선정되었다.
선정된 문헌을 바탕으로 중환자실 setting과 무작위배정 임상시험을 선정기준에 추가하여 임상시험에 대한 검색을 수행하였다. 검색 문헌은 독립된 2인에 의해 1차와 2차 선택 및 배제 과정을 거쳐 최종 1개의 문헌이 선정되었으며, 선행 체계적 문헌고찰에 포함된 7개의 임상시험을 포함하여, 총 8개의 임상시험이 체계적 문헌고찰에 포함되었다.
선정 문헌에 대해 RoB(Risk of Bias) 도구를 바탕으로 비뚤림 위험도를 평가하였으며, 패혈증 감염 의심환자에서 프로칼시토닌 검사를 통한 항생제 치료 조절의 임상적 유용성을 일반치료와 비교하기 위해 전통적 메타분석과 베이지안 메타분석을 수행하였다.
프로칼시토닌 수준을 확인하면서 항생제 치료를 조절하는 치료법 자체의 특성으로 인해 이중 눈가림에 어려움이 존재하기 때문에 RoB를 이용한 비뚤림 위험을 평가한 결과, Jensen 등(2011)의 문헌을 제외한 대부분의 문헌에서 연구자 눈가림의 시행여부가 불분명하였다. 전통적 메타분석과 베이지안 메타분석을 이용하여 패혈증에서 프로칼시토닌 수준에 따른 항생제 치료조절과 일반치료를 정량적으로 비교하였다. 두 가지 메타분석 결과, 프로칼시토닌군과 일반치료군 간의 28일 사망률, 병원 내 사망률, 중환자실 재원기간에는 통계적인 유의성을 보이지 않았으나 항생제 사용기간에서는 두 군에서 유의한 차이를 보였다. 전통적 메타분석에서는 프로칼시토닌 군이 일반치료 군에 비해 항생제 치료기간을 2.43일 단축시켰으며(95% 신뢰구간: (-3.46, -1.40), p<0.0001), 베이지안 메타분석에서는 평균차의 사후 중앙값이 -2.39(95% 신용구간: (-3.79, -1.13))로 추정되었다. 프로칼시토닌 검사를 통해 항생제 치료를 조절하는 방법이 일반치료에 비해 항생제 사용일수를 줄일 수 있는 사후확률은 1로 나타났다.
Ⅱ. 전향적 다기관 무작위배정 임상시험
중환자실 재원 환자 중 패혈증 의심 또는 확진 환자에 대해 프로칼시토닌 수치 변화를 기준으로 항생제 치료의 중단 또는 항생제 변경 등을 결정하는 경우, 항생제 사용일수 및 재원기간 등에 미치는 영향을 파악하기 위하여 서울대학병원, 삼성서울병원, 세브란스병원, 서울아산병원 총 4곳의 상급종합병원에서 전향적 임상시험을 수행하였다.
선정 및 제외기준에 따라 임상시험에 등록된 환자는 무작위배정 방법에 의해 프로칼시토닌군과 일반치료군으로 배정되었으며, 이 때 가이드라인을 기준으로 항생제 중단 강력 권고에 해당하나 1회라도 위반한 경우, 즉, 이틀 뒤까지 지속적으로 항생제를 사용한 경우와 항생제 투여 중단이 권고된 이후 연속 2회 이상 항생제가 사용된 경우는 overrule에 해당하게 되며, per-protocol(PP) 분석에서는 제외되었다. 임상시험에 등록된 환자는 등록 후 13일 간의 close monitoring 기간을 포함하여 사망 또는 병원 퇴원시점까지 관찰하게 되며, 일차 임상지표로 항생제 사용일수, 이차지표로 28일 사망률, 재원 내 사망률, 중환자실 입원기간 및 입원비용을 확인하였다.
임상시험 결과, 2014년 5월부터 2015년 6월까지 377명의 패혈증 의심환자가 선정 기준에 대한 스크리닝을 거쳐 이 중 총 57명의 환자가 임상시험에 등록되었다. 프로칼시토닌군의 경우 해당 군에 배정된 26명의 환자 중 중도탈락 및 overrule로 인하여 15명이 PP 분석에서 제외되었으며, 일반치료군의 경우 31명의 환자 중 3명이 제외되었다. 두 군에 포함된 환자의 기저특성을 비교한 결과 연령, 중증도, 처치내역 등 전반적인 기저특성의 분포가 비슷한 것으로 나타났다.
항생제 사용일수의 경우 프로칼시토닌 수치를 기반으로 항생제 중단 여부를 결정한 프로칼시토닌군이 일반치료군에 비해 짧은 것으로 나타났다. Intent-to-treat(ITT) 분석 시 프로칼시토닌군의 항생제 투여일수는 12.24일(sd=8.72)이었으며, 일반치료군의 투여일수는 16.06일(sd=10.4)로 3.82일이 단축되었으나 그 차이는 통계적으로 유의하지 않았다(p=0.1479). 반면, PP 분석에서는 프로칼시토닌군의 항생제 사용일수는 6.91일(sd=4.44), 일반치료군의 사용일수는 14.79일(sd=9.37)로 프로칼시토닌군에서 항생제 투여일수가 7.88일 유의하게 감소하였다(p=0.0013). 이차지표인 중환자실과 병원 재원일수는 ITT와 PP 분석 모두에서 프로칼시토닌군과 일반치료군이 비슷한 것으로 확인되었으며, 28일 사망률, 병원 내 사망률, 기계적 환기의 사용률 및 치료의 성공률도 두 군에서 유의한 차이를 보이지 않았다.
▢ 프로칼시토닌 검사의 경제성 분석
Ⅰ.비용최소화 분석
프로칼시토닌 수준을 바탕으로 항생제 중단을 결정하는 것이 병원 사망률 및 28일 사망률과 같은 최종성과에 영향을 미치지 않음을 체계적 문헌고찰과 전향적 임상시험에서 확인하고 비용최소화 분석을 수행하였다. 전향적 임상시험을 실시한 두개 병원자료를 토대로 프로칼시토닌군과 일반치료군에서의 1일 총 의료비용, 프로칼시토닌 검사비 및 항생제 비용, 중환자실 비용 등을 추출하여 비교하였다. 또한 건강보험 청구자료에서 패혈증 관련 항생제 비용과 임상시험에서 ITT 분석 대상 환자군의 결과값, 그리고 체계적 문헌고찰의 메타분석에서 얻은 결과를 추출하여 민감도 분석을 수행하였다.
전향적 임상시험에서 추출한 값을 바탕으로 비용최소화 분석을 수행한 결과, 패혈증 입원 한 건당 소요되는 평균비용은 프로칼시토닌군에서 1,952만원, 일반치료군에서 2,684만원으로, 프로칼시토닌군에서 732만원의 비용절감 효과가 있음을 확인하였다. 또한 프로칼시토닌 비용과 항생제 비용만을 비교하였을 때에도 일반치료군이 프로칼시토닌 군보다 약 8만원 더 소요되었다.
건강보험 청구자료를 이용한 민감도 분석을 수행하기에 앞서 전향적 임상시험의 환자와 청구자료의 환자군을 비교하였다. 당뇨, 심혈관계 질환 등에 있어서 임상시험 환자와 청구자료 환자군은 비슷한 정도의 분포를 보였으나 만성 폐질환, 만성 신질환, 전이성 고형암의 경우에는 임상시험 환자군에서 더 많은 환자가 있는 것으로 나타났다. 이는 임상시험에 참여한 4개의 병원이 모두 상급종합병원으로 청구자료의 환자군에 비하여 상대적으로 중증의 환자가 많기 때문인 것으로 여겨진다. 청구자료의 패혈증 입원환자에서 항생제 사용일수 및 비용을 산출하여 민감도 분석에 적용한 결과, 패혈증 입원 환자의 1일 평균 항생제 비용은 73,953원(sd=60,530)으로 임상시험 결과보다 높은 경향이 있어 일반치료군의 항생제 비용이 102만원으로 증가, 결과적으로 785만원의 비용절감효과가 나타났다. 또한 ITT 분석 대상 환자군을 바탕으로 민감도 분석을 수행하였을 때도 프로칼시토닌군에서 총 2,404만원, 일반치료군에서 총 2,644만으로 프로칼시토닌군에서 240만원의 비용절감효과가 있었으나 항생제와 프로칼시토닌 검사 비용만을 포함하여 비교하였을 때에는 프로칼시토닌군이 일반치료군에 비하여 약 9만원의 비용이 더 소요되는 것으로 나타났다. 마지막으로 체계적 문헌고찰의 베이지만 메타분석 결과를 이용하여 민감도 분석을 수행한 결과, 프로칼시토닌 수준을 활용한 항생제 치료 조절이 300만원의 비용절감효과가 있음을 확인하였다.
Ⅱ.재정영향분석
프로칼시토닌 검사는 현재 비급여 항목으로, 향후 급여 항목으로 전환될 가능성을 고려하여 2016년부터 2018년까지의 재정영향분석을 수행하였다. 프로칼시토닌 검사의 급여 횟수 및 검사가 임상현장에서 사용될 대체율를 고려하여 총 4개의 시나리오(급여 횟수 2회+대체율 10%, 급여횟수 2회+대체율 20%, 급여 횟수 3회+대체율 10%, 급여 횟수 3회+대체율 20%)를 검토하였다. 또한 최저비용의 항생제로 모든 패혈증 항생제 치료가 대체될 때, 임상시험의 ITT 분석 대상 환자군의 결과값을 대입하였을 때, 프로칼시토닌 검사의 적정한 급여비용을 적용하였을 때, 그리고 프로칼시토닌 검사의 급여횟수 및 대체율이 증가할 때에 대한 민감도 분석을 수행하였다.
고려한 모든 시나리오에서 프로칼시토닌 검사는 현재의 일반치료에 비해 재정절감의 효과가 있음을 확인하였으며 대체율이 변화함에 따라 재정영향이 민감하게 변화하였다. 프로칼시토닌 검사가 2회 급여되고 대체율이 10%일 경우와 3회 급여되고 대체율이 10%일 경우, 현행 대비 각각 216억, 214억원의 재정절감효과가 있을 것으로 나타났다. 또한 프로칼시토닌 검사의 대체율이 20%이고 급여 횟수 2회 또는 급여 횟수 3회일 경우, 현행 대비 각각 476억, 472억 원의 재정절감효과가 있을 것으로 파악되었다.
재정영향분석을 수행한 시나리오에서 현행 대비 프로칼시토닌 검사비용은 재정을 증가시켰지만 입원비용과 항생제 비용에서 재정절감의 폭이 커 전체적으로 재정절감의 효과가 나타났고 프로칼시토닌 검사의 급여횟수보다 검사를 사용하는 대체율이 재정에 더 큰 영향을 미치는 경향이 있었다. 항생제 비용, 임상시험의 ITT분석 대상 환자군의 결과값, 프로칼시토닌 검사의 급여비용, 검사의 급여횟수 및 대체율에 대하여 수행한 민감도 분석에서도 프로칼시토닌 수준을 이용한 항생제 조절 치료가 재정절감의 효과가 있음을 나타냈다.
▢ 결론 및 정책적 제언
패혈증 의심 또는 확진으로 중환자실에 입실하는 환자를 대상으로 시행한 전향적 다기관 무작위배정 임상시험을 통하여 국내 패혈증 환자에서 프로칼시토닌 검사를 이용하여 항생제 치료중단을 결정할 경우, 일반치료에 비해 항생제 사용일수를 약 8일 정도 감소시키지만 사망률과 입원기간은 유의한 차이가 없는 것으로 확인하였다. 비용최소화분석에서 패혈증 한건 당 비용은 프로칼시토닌 검사를 이용하여 항생제 치료중단을 결정했을 경우 일반치료에 비해 약 732만원의 비용절감효과가 있었다. 비급여 검사인 프로칼시토닌 검사가 급여화될 경우를 가정하여 프로칼시토닌 검사의 대체율 10-20% 및 검사횟수 2-3회인 급여 시나리오에 대한 재정영향분석을 수행하였을 때, 현재의 일반치료 대비 재정절감효과가 있는 것으로 나타났다.
본 연구는 국내 패혈증 환자를 대상으로 다기관 무작위배정 임상시험을 통해 프로칼시토닌 검사의 임상적 유용성을 파악한 최초의 연구이며, 이를 반영해 국내 상황에서 경제성 분석을 실시한 연구로 국내 프로칼시토닌 검사와 관련된 정책 결정 및 의사결정에 근거를 제공한 연구로 의미가 있다고 할 수 있다.
▢ Introduction
Sepsis management has been emphasized due to the high hospitalization rate, mortality, and costs associated with sepsis. The latest guidelines for diagnosis and treatment of sepsis recommend using procalcitonin test, when determining whether to discontinue the use of antibiotics for patients who do not experience secondary infections following initial resuscitation of sepsis. Since unnecessary use of antibiotics can be minimized by using procalcitonin test, reduction in antibiotics-related complications, and further, effectiveness in reducing medical costs can be expected. However, there are no comparative studies on Korean patients with regards to this subject matter and economic evaluations on reduction in antibiotics treatment duration based on procalcitonin levels are also lacking.
Therefore, it is necessary to generate scientific evidence that procalcitonin test can safely reduce the duration of antibiotics treatment and whether this approach is feasible economically by identifying the effectiveness of procalcitonin test as an indicator to determine the treatment course of antibiotics in sepsis suspected or confirmed patients without secondary infections in Korea.
This study aimed to evaluate the clinical effectiveness on determining discontinuation of antibiotics based on procalcitonin levels in sepsis suspected patients and to conduct an economic analysis in order to provide evidence for policy- and decision-making related to procalcitonin test.
▢ Clinical effectiveness of procalcitonin test
Ⅰ. Systematic Review
In order to make a comparison between clinical effectiveness of procalcitonin-guided treatment on antibiotic use and effectiveness of routine care in sepsis patients, existing systematic reviews were used. A systematic review search was conducted using pre-established PICOTS-SD according to the methodology recommended by AHRQ of the US and the suitability of the searched studies was evaluated. Following the search strategy, a total of 134 cases were identified from various databases, such as EMBASE, Ovid-Medline, and KoreaMed, among which a final total of 7 reviews were selected based on the selection and exclusion criteria.
The selected systematic reviews were evaluated for suitability of literature selection and data extraction as well as comprehensiveness of the literature search using AMSTAR. Based on the quality evaluation, Prkno et al. (2013) was selected as the final candidate. Using the selected literature as the basis, search on clinical trials was conducted after adding ICU settings and randomized clinical trials to the selection criteria. Ultimately, one literature was selected as the final candidate through a process of primary/secondary selection and exclusion by two independent evaluators, and by including 7 clinical trials from precedent systematic reviews, a total of 8 clinical trials were included in the systematic review.
The risk of bias associated with the selected literature was assessed using RoB tool, while traditional and Bayesian meta-analyses were performed to compare the clinical effectiveness of procalcitonin-guided treatment on antibiotic use with routine care in sepsis suspected patients. According to the risk of bias assessment, since the characteristic feature of procalcitonin-guided treatment on antibiotic use made a double-blind difficult, most literature, with the exception of Jensen et al. (2011), were unclear on whether researcher blinding had been conducted. Meanwhile, traditional and Bayesian meta-analyses were used to quantitatively compare procalcitonin-guided treatment on antibiotic use with routine care.
The results of the two meta-analyses showed no statistically significant differences between the procalcitonin group and routine care group with respect to 28-day mortality, in-hospital mortality, length of stay in ICU, and length of stay in hospital; whereas, duration of antibiotics did show a significant difference between two groups. In the traditional meta-analysis, the procalcitonin group, compared to the routine care group, showed reduction of 2.43 days in the duration of antibiotics (95% CI: (-3.46, -1.40), p-value<0.0001), while in the Bayesian meta-analysis, posterior median of the mean difference was estimated to be -2.39 (95% CrI: (-3.79, -1.13)). The posterior percentage of procalcitonin-guided treatment on antibiotic use reducing the duration of antibiotics use compared to routine care was shown to be 1.
Ⅱ. Prospective, multi-center, Randomized Controlled Trial
A prospective clinical trial was conducted from 4 tertiary hospitals (Seoul National University Hospital, Samsung Medical Center, Severance Hospital, and Seoul Asan Medical Center) in order to identify the influences on the duration of antibiotics and length of hospital stay in sepsis suspected or confirmed patients hospitalized in the ICU when decisions are made on discontinuing or changing antibiotics treatments based on procalcitonin levels.
In accordance with the selection and exclusion criteria, the patients registered for the clinical trial were randomly assigned to the procalcitonin or routine care group. At this time, a guideline was used to overrule cases involving even single violation of strong recommendation for antibiotics discontinuation, meaning continuing to use antibiotics for two more days and using antibiotics for two consecutive times despite recommendation for discontinuation of antibiotics administration, with such cases being excluded from per-protocol (PP) analysis. The patients registered for the clinical trial were monitored at 28-days after onset of sepsis, discharge from ICU, and discharge from hospital, and the number of days of using antibiotics was identified as the primary clinical outcomes, while 28-day mortality, in-hospital mortality, length of stay in ICU, and hospitalization costs were identified as the secondary outcomes.
In terms of clinical trial results, a total of 377 sepsis suspected patients had passed through the screening process for the selection criteria between May 2014 and June 2015, among whom, a total of 57 patients were registered for the clinical trial. In the procalcitonin group, 15 patients, out of 26 who were assigned to this group, were excluded from PP analysis due to drop out or overrule, whereas in the routine care group, 3 out of 31 patients were excluded. As a result of comparing the baseline characteristics of patients assigned to the two groups, the overall distribution of baseline characteristics, such as age, severity, and medical history were shown to be similar.
The number of days using antibiotics indicated to be shorter in the procalcitonin group, for whom antibiotics discontinuation was determined according to procalcitonin levels, than that of the routine care group. Intent-to-treat (ITT) analysis showed that the number of days of antibiotics administration in the procalcitonin group was 12.24 days (sd=8.72) and 16.06 days (sd=10.4) in the routine care group, which indicated reduction of 3.82 days. However, the difference was not statistically significant (p=0.1479). On the other hand, PP analysis showed number of days of antibiotics administration in the procalcitonin and routine care groups to be 6.91 (sd=4.44) and 14.79 days (sd=9.37), respectively, showing significant reduction of 7.88 days of antibiotics administration in the procalcitonin group (p=0.0013). In terms of secondary indicators of length of stay in ICU and hospital, they were shown to be similar in both the procalcitonin and routine care groups by both ITT and PP analyses, while 28-day mortality, in-hospital mortality, utilization of mechanical ventilation, and treatment success rate were also shown to be similar between the two groups.
▢ Economic analysis of Procalcionin test
Ⅰ. Cost-Minimization Analysis
The fact that procalcitonin-guided withdrawal of antibiotics does not influence the final outcome, such as in-hospital and 28-day mortality was confirmed via systematic review and prospective clinical trial, and cost-minimization analysis was performed. Based on the data from two hospitals where prospective clinical trial was performed, 1-day medical cost, cost of procalcitonin test, antibiotics cost, and ICU cost for both the procalcitonin and routine care groups were extracted and compared. Moreover, sensitivity analysis was performed using sepsis-related antibiotics costs from claims data of Health Insurance Review and Assessment (HIRA) and meta-analysis results from the systematic review.
The results of the cost-minimization analysis using values obtained from the prospective clinical trial showed that the average cost per sepsis hospitalization case in the procalcitonin and routine care group was 19.52 million and 26.84 million won, respectively, indicating the routine care group spent 7.32 million won more than the procalcitonin group. In addition, when only the procalcitonin and antibiotics costs were compared, the results also showed that the routine care group spent approximately 80,000 won more than the procalcitonin group.
Ⅱ. Budget Impact Analysis
Currently, procalcitonin test is a non-reimbursement item, and in consideration of being switched to a reimbursement item, budget impact analysis for years 2016 through 2018 was performed. By taking into account the number of reimbursements for procalcitonin tests and the substitution rate for the test being used in clinical settings, a total of 4 scenarios were examined, which were 2 reimbursements + substitution rate 10%; 2 reimbursements + substitution rate 20%, 3 reimbursements + substitution rate 10%, and 3 reimbursements + substitution rate 20%. Moreover, sensitivity analysis was also performed on when all sepsis antibiotics treatments are administered with minimal-cost antibiotics, when adequate reimbursement cost is applied for procalcitonin tests, and when the number of reimbursements for procalcitonin tests and the substitution rates increase.
It was confirmed in all of the scenarios considered that procalcitonin test had the effectiveness of budget savings compared to current routine care and changes in financial impact were sensitive to changes in the substitution rate. Procalcitonin test being reimbursed 2 times with substitution rate of 10% and being reimbursed 3 times with substitution rate 10% amounted to a budget savings effect of 21.6 billion and 21.4 billion won, respectively. Moreover, procalcitonin test with substitution rate of 20% and being reimbursed 2 times versus 3 times was determined to amount to a budget savings effect of 47.6 billion versus 47.2 billion won compared to current routine care, respectively.
In all of the scenarios, the cost of procalcitonin test increased the financial burden in comparison to current status, but showed an overall budgetl savings effect by increasing the margin of budget savings associated with hospitalization and antibiotics cost, and there was a tendency for the substitution rate of using the test having a bigger financial impact than the number of imbursements for the procalcitonin test. A similar outcome was observed from the sensitivity analysis performed on antibiotics cost, reimbursement cost of procalcitonin test, number of reimbursements for the test, and substitution rate.
▢ Conclusions and Implications
Through a multi-center prospective randomized controlled trial conducted on sepsis suspected or confirmed patients admitted to ICU, it was confirmed that using procalcitonin test on Korean sepsis patients to determine treatment course of antibiotics was able to reduce the duration of antibiotics by approximately 8 days, in comparison to routine care, however, no statistically significant differences in mortality and length of hospital stay were indicated. The cost-minimization analysis demonstrated for each case of sepsis, using procalcitonin test to determine discontinuation of antibiotics treatment resulted in a cost savings effect of approximately 7.32 million won, in comparison to routine care. The present study also performed budget impact analysis with the assumption that procalcitonin test, which is currently a non-reimbursement item, will become a reimbursement item. It was shown that when the substitution rate of procalcitonin test was set to 10-20%, with the test being performed 2 to 3 times, it had a budget savings effect over current routine care.
This study is the first research to identify the clinical effectiveness of procalcitonin test through a multi-center prospective randomized controlled trial conducted on sepsis patients and reflected the results in performing an economic analysis of current conditions in Korea. Therefore, the significance of the present study can be viewed as having provided evidence for policy- and decision-making related to procalcitonin test in Korea.
