▢ 연구배경
최근 10년 동안 우리나라의 심장질환 사망률은 지속적으로 증가하고 있으며, 그 중 허혈성 심장질환 사망률은 인구 10만 명 당 26~29명 수준으로 나타난다. 허혈성 심장질환을 진단하는 초기 진단 방법은 비침습적으로 검사하는 방법으로 심근 SPECT 및 관상동맥 CT 조영술 등이 있다. 하지만 현재 진료지침에서 저위험 및 중등도 위험군 흉통 환자들에게 우선적으로 권고되는 비침습적 검사는 심근 SPECT 및 운동부하 심전도 등의 기능적 검사(functional test)이며 해부학적인 검사(anatomical test)에 해당되는 관상동맥 CT 조영술은 저위험 및 중등도 위험군 환자들 중 기능적 검사를 시행하기 어려운 경우에 한하여 권고되고 있다. 그러나 해부학적 검사는 비교적 최근에 도입되어 기능적 검사에 비해 장기적으로 환자의 예후 개선 효과가 미미하여 대상 환자를 제한하여야 하는지에 근거가 부족하다.
▢ 연구목적
본 연구를 통해 CT의 진단적 가치뿐만 아니라 우리나라 실정에 맞게 현재의 흉통 평가 영상 진단법간의 비용효용성을 분석하고자 한다. 흉통을 호소하는 관상동맥질환의 저위험/중등도 위험 환자들에서 진단적 목적으로 시행한 관상동맥 CT 조영술과 심근 SPECT의 진단 정확도와 환자예후를 비교하고, 이를 바탕으로 두 검사법 간의 경제성 평가를 통해 우리나라 실태에 맞는 적절한 흉통 평가법에 대한 객관적인 자료를 마련하고자 한다.
▢ 체계적 문헌고찰 방법 및 결과
국내 및 국외 문헌 데이터베이스를 활용한 검색 결과 2016년 1월까지 출판된 총 3,021건 문헌 중 1,2차 문헌 선택배제 결과를 통해 최종 15개 문헌이 선택되어 평가되었다. 평가에 포함된 문헌들은 메타 분석을 실시하여 의료결과 값을 합성하고 비교하였다. 문헌 고찰 결과 관상동맥 질환의 저위험/중등도 위험 환자군에서 최초 진단검사로 관상동맥 CT 조영술을 선택할 경우 사망률과 주요심장사건 발생률에는 유의한 차이가 없었고, 심근경색은 유의하게 관상동맥 CT 조영술에서 적게 나타나는 것으로 나타났다. 12개월 이상의 장기추적관찰이 보고된 문헌의 경우에도 관상동맥 CT 조영술을 선택한 환자군에서 심근경색발생률이 유의하게 낮았다. 반면 침습적 관상동맥 혈관조영술, 혈관 재개통술의 발생률은 관상동맥 CT조영술을 실시한 군에서 유의하게 높은 것으로 나타났다. 흉통 증상에 따른 하위그룹 분석 결과, 급성 흉통 환자에서는 관상동맥 CT 조영술을 선택한 환자군의 혈관 재개통술 시술률이 기능적 검사를 시행한 환자보다 유의하게 높았으며 무통증 환자의 경우에는 관상동맥 CT 조영술을 시행한 환자에서 심혈관중재 시술률이 유의하게 높았다. 이를 통해 관상동맥질환 의심 환자에서 최초 진단검사로 관상동맥 CT 조영술을 선택하는 것이 기능적 검사를 선택하는 것과 최소 동등 이상의 유의성을 보임을 확인할 수 있었다.
▢ 경제성 평가 방법 및 결과
흉통을 호소하는 관상동맥질환의 저위험/중등도 위험 환자들에서 진단적 목적으로 수행한 관상동맥 CT 조영술과 심근 SPECT 간의 비용효과성을 비교하기 위하여 기구축된 전향적 다기관 무작위배정 임상시험 자료와 1년간의 국민건강보험공단 청구자료 및 통계청 사망자료를 연계하여 코호트 자료를 구축하였다. 경제성 분석은 보건의료체계관점으로 실시하였으며 의료비용과 비의료비용을 분석에 포함하였다. 효과지표로는 질보정생명년수 (QALY)를 사용하였으며 질보정생명년수 산출을 위해 건강관련 삶의 질 일반도구인 EQ-5D를 이용하였다.
코호트 입적일로부터 1년간의 비용 및 질보정생명년수 값을 확인한 결과, 관상동맥 CT 조영술군과 심근 SPECT군 간의 효용가중치 차이가 사전에 설정한 극소중요차이(minimally important difference, MID) 0.07보다 작아 두 군의 효과 차이가 없는 것으로 가정하고 비용효용분석 대신 비용최소화분석을 수행하였다. 총의료비용을 기반으로 한 기본분석에서 관상동맥 CT 조영술의 비용은 647만 원, 심근 SPECT의 비용은 746만 원으로 관상동맥 CT 조영술의 비용이 99만 원 더 낮아 비용효과적이었다. 심혈관계 관련 비용을 이용한 민감도분석 역시 관상동맥 CT 조영술이 심근 SPECT 보다 소요비용이 41만 원 낮아 비용효과적인 대안으로 나타났다.
하위그룹 분석은 관상동맥질환 위험도(10-29%, 30-59%, 60-90%)에 따라 비용최소화분석을 수행하였다. 관상동맥질환 위험도가 10-29%, 30-59%일 경우, 관상동맥 CT 조영술의 비용은 심근 SPECT의 비용보다 각각 50만 원, 59만 원 낮아 관상동맥 CT 조영술이 비용효과적임을 확인하였다. 그러나 관상동맥질환 위험도가 60-90%로 높을 경우, 심근 SPECT 대비 관상동맥 CT 조영술은 20만 원 더 소요되어 비용효과적이지 않았다.
▢ 결론 및 정책제언
체계적 문헌고찰 결과 관상동맥 CT 조영술은 기능적 검사에 비해 사망률, 심근경색, 주요심장사건(MACE) 등의 임상적 결과에 있어 최소 동등 이상의 유의성을 보였으며, 경제성 평가 결과 비용효과적인 대안으로 나타났다. 적절한 대상자에 관상동맥 CT 조영술을 활용함으로써 의료비 감소와 임상성과의 향상을 기대할 수 있을 것이다.
▢ Study Background
Chest pain, one of the most frequent symptoms that physicians encounter, is a symptom that warns of potentially dangerous myocardial ischemia and therefore warrants the highest diagnostic attention. Several noninvasive testing are utilized to determine which patients who present with chest pain have significant myocardial ischemia and which patients require invasive procedures. The emergence of coronary computed tomography angiography (CCTA) produced a substantial shift in diagnostic strategy for patients with suspected coronary artery disease (CAD), based on the diagnostic performance of CCTA and short-term outcomes following the test, relative to functional tests. Clinicians have rapidly adopted these noninvasive imaging studies for evaluation of patients with chest pain. This, in turn, has lead to the ever-increasing health-related cost spent in the evaluation of chest pain, necessitating the comparison of cost-effectiveness between each imaging modalities. Studies on this issue, whether by modeling or by retrospective analysis, has demonstrated that CCTA may be more cost-effective than myocardial single photon emission computed tomography (SPECT). However, the cost-effectiveness of these imaging studies has never been compared in a prospective, randomized clinical trial. In this trial, we compared the cost-effectiveness between CCTA and myocardial SPECT, the most widely used noninvasive imaging modality currently, in patients with intermediate risk chest pain.
▢ Objectives
This study aimed to compare the cost-effectiveness of the two most widely used noninvasive imaging test for evaluation of chest pain, coronary computed tomography angiography (CCTA) or myocardial single photon emission computed tomography (SPECT).
▢ Systematic review and meta-analysis
We searched PubMed, EMBASE and the Cochrane Library for randomized controlled trials (RCTs) that compared clinical outcomes during ≥6 months of follow-up between initial anatomical testing by CCTA and the initial functional testing in patients with suspected CAD. Occurrence of all-cause mortality, nonfatal myocardial infarction (MI) and major adverse cardiovascular events (MACE), and use of invasive coronary angiography (ICA) and coronary revascularization were compared between the two diagnostic strategies. Twelve RCTs, one case-control study and two prospective cohort studies were included (23,862 patients; mean follow-up duration, 20.5 months).
Patients who underwent CCTA as the initial noninvasive testing had lower risk of myocardial infarction compared to those who underwent functional testing (RR, 0.74; 95% CI, 0.55 – 0.99; P = 0.04). Risk of all-cause mortality and MACE did not differ statistically between the two diagnostic strategies. Compared to functional testing, the anatomical testing strategy increased the use of ICA (RR, 1.65; 95% CI, 1.05 – 2.60; P = 0.03) and coronary revascularization (RR, 1.57; 95% CI, 1.14 – 2.18; P = 0.006). Anatomical testing with CCTA, as the initial noninvasive diagnostic modality in patients with suspected CAD, resulted in lower risk of nonfatal MI than functional testing, at the expense of more frequent use of invasive procedures.
▢ Economic evaluation of noninvasive imaging test
Patients with 10~90% pre-test probability of coronary artery disease from 3 high-volume centers in Korea (n=965) were randomized 1:1 to either CCTA or myocardial SPECT as the initial noninvasive imaging test. The primary outcome was cost-effectiveness, analyzed using the downstream outcome following the initial noninvasive test, the quality-adjusted life years (QALYs) by the EuroQoL-5D questionnaire and all medical costs related to the evaluation and follow-up.
A total of 903 patients underwent testing with the initially randomized study (460 for CCTA and 443 for SPECT). There were 65 (14.1%) admissions for invasive coronary angiography in the CCTA group and 85 (19.2%) in the SPECT group (p-value=0.0412). There was no significant difference in the clinical events following the initial noninvasive test (21 for CCTA vs. 24 for SPECT, p-value=0.4554). Since the utility difference between CCTA and SPECT was smaller than the pre-defined minimally important difference, 0.07, cost-minimization analysis was carried out instead of cost-effectiveness analysis. In base case which was based on the total medical cost, mean cost for CCTA group was lower than that of SPECT group (6.470 million KRW vs. 7.460 million KRW) indicating that CCTA was the cost-effective alternative. In the sensitivity analysis, we utilized cardiovascular-related medical cost and found that similar results: SPECT was 0.99 million KRW more expensive than CCTA. In the subgroup analysis, we examined the cost-effectiveness of CCTA and SPECT based on pre-test probability using cardiovascular medical cost. When the pre-test probabilities were 10-29% or 30-59%, the cost of CCTA group was lower than that of SPECT by 0.5 million KRW, 0.59 million KRW, respectively, meaning that CCTA was the cost-effective alternative. However, in the group of pre-test probability with 60-90%, the cost for SPECT was 0.2 million KRW more expensive than that of CCTA, so that CCTA was not cost-effective.
▢ Conclusion
Anatomical testing with CCTA, as the initial noninvasive diagnostic modality in patients with suspected CAD, resulted in lower risk of nonfatal MI than functional testing, at the expense of more frequent use of invasive procedures. Although there was no differences of QALYs gained between CCTA and SPECT, CCTA tended to be lower cost in Korean patients with intermediate risk chest pain.
