▢ 연구 배경
허혈성 심질환은 심근으로의 혈류 공급을 제한하는 관상동맥의 협착 및 폐쇄가 원인이 되는 질환으로서, 좁아진 관상동맥을 넓혀주는 재관류치료와 관상동맥 죽상경화반의 진행을 억제하는 약물치료가 주된 치료법으로 사용되고 있다. 일반적으로 관상동맥의 직경이 50% 이상 좁아질 경우에 혈역학적으로 의미있는 혈류 제한이 발생하기 때문에, 허혈성 심질환 및 관상동맥질환 분야에서 ‘폐쇄성 관상동맥질환 (obstructive coronary artery disease)’에 대한 연구가 많이 이루어져왔다.
하지만 최근 발표된 여러 연구에서 관상동맥의 협착 정도가 50%에는 미치지 못하는 관상동맥 죽상경화반(coronary atherosclerotic plaque)도 임상적으로 의미있는 심혈관계 합병증을 초래한다는 점이 밝혀지면서, ‘비폐쇄성 관상동맥질환(nonobstructive coronary artery disease)’에 대한 관심이 높아지고 있다. 특히 CT 관상동맥조영술이 널리 보급되면서 비침습적으로 관상동맥의 해부학적 구조와 죽상경화반으로 인한 관상동맥의 폐쇄 정도를 정확히 진단할 수 있게 되었고, 그 결과 비폐쇄성 관상동맥질환이 진단되는 환자가 급격히 증가하면서, 이러한 환자들에 대한 적절한 진료지침의 필요성이 강조되고 있다. 최근 비폐쇄성 관상동맥질환 환자에서 스타틴과 아스피린 등의 심혈관계 예방적 치료약제의 효과에 대한 일부 연구 결과들이 발표되었으나, 환자들의 임상적 특성에 따른 적절한 치료방침을 결정하기에는 근거가 매우 부족한 실정이다.
▢ 연구 목적
한국인에서 비폐쇄성 관상동맥질환 환자의 주요임상사건(사망, 급성 심근경색, 후기 재관류) 발생에 대한 영향 인자를 파악하고 환자의 예후 예측 모형을 개발하여 치료전략 수립에 객관적 근거를 제시하여 국내 의료정책 수립에 도움을 주고자 한다.
▢ 연구 방법
기구축 된 다기관 관상동맥조영술 코호트 자료(CARE-CCTA Registry)와 국민건강보험공단 자료 연계를 통하여 환자들의 예후를 추적하고, 주요 임상 사건에 대한 예측 인자를 로지스틱 회귀모형을 이용하여 확인한다. 확인된 예측인자를 이용한 예측모형과 위험지수를 구축하여 비폐쇄성 환자들의 위험도를 계층화하여 위험도에 따라 치료전략을 수립할 수 있게 한다.
▢ 연구 결과
⦁비폐쇄성 환자들에 대한 위험지수를 개발하였으며 위험지수의 범위는 0~14점 이었다.
⦁위험지수에 따른 환자를 계층화한 결과 위험지수 0~3점인 경우 비협착(협착률0%)군과 유사한 위험률을 가지고 위험지수 8점 이상인 경우에는 폐쇄성군과 유사한 위험률을 가진다.
Ⅰ. 대상자현황
본 연구에 사용된 자료원은 2007년부터 2011년까지 서울대병원에서 CT 관상동맥조영술을 받은 대상자 중 협착률이 1-49%인 환자로 전체 대상자 25,087명 중 6,243명이었다. 연구의 대상자로 선정된 환자 중 예측 모형 개발을 위하여 사용될 모든 변수의 값에 결측이 없는 환자는 4,132명이었다. 선택된 완전자료(4,132명)와 전체자료(6,243명)간의 기저특성 및 주요임상사건의 발생률에 차이가 없었다. 선택된 완전자료의 환자는 평균 연령이 59.7세이고 남자가 다수(68%)를 차지하였다. 입적 후 5년간 주요임상 사건의 발생률은 심근경색이 3%, 재관류술 1.7%, 사망 2.2%로 전체사건 발생률은 6.2%였다.
Ⅱ. 주요임상사건 발생 위험인자
유의한 주요임상사건 발생 위험 인자를 찾기 위하여 로지스틱 회귀모형을 이용하였다. 최종적으로 선정된 위험인자는 연령, 성별, 고혈압, 당뇨병, 헤모글로빈, Vessel score/CACS이었다. 위험률 예측 모형에 포함된 위험인자 중 가장 큰 영향력을 가진 인자는 연령과 헤모글로빈인 것으로 나타났다.
Ⅲ. 위험지수
위험률지수는 0(위험인자가 전혀 없는 경우)~14(모든 위험인자를 가진 경우)점까지 나타날 수 있다. 본 연구에서 사용한 자료의 위험지수는 Vessel score모형에서는 0점에서 12점까지, CACS를 함께 포함한 모형에서는 0에서 13점까지 분포하였다.
Ⅳ. 위험지수를 이용한 위험군 층화
비폐쇄성 환자를 위험지수에 따라 0~3점, 4~7점, 8점이상의 군으로 층화하여 주요임상사건에 대해서 무사건생존율을 추정한 결과 8점이상군이 두 모형에서 각각 0.782(Vessel score 이용), 0.796(CACS 포함)으로 폐쇄성 환자의 발생률과 차이를 보이지 않았다. 또한 0~3점 군의 경우에는 두 모형에서 모두 0.97로 협착정도가 0%인 군과 유사하였다.
▢ 결론 및 정책적 제언
현재까지 비폐쇄성 관상동맥 질환을 가진 환자에 대하여 위험률을 예측하는 모형은 본 연구가 최초이며 기존의 비폐쇄성 관상동맥질환 환자들 내에서도 환자들의 임상적 특성에 따라 주요임상사건의 발생 위험률이 상이하며 고위험 인자를 가지고 있는 환자를 구분하여 적절한 치료전략을 제시할 수 있는 객관적인 근거를 마련하였다.
▢ Background
Ischemic heart disease is caused by coronary artery stenosis and obstruction that restricts the supply of blood to the myocardium. Reperfusion that dilates the narrowed coronary artery and drug treatment that inhibits the progression of coronary artery atherosclerosis are mainly used as a treatment. In general, if the diameter of the coronary artery is narrowed by more than 50%, significant blood flow limitation occurs hemodynamically. Therefore, there has been many studies on 'obstructive coronary artery disease' in the field of ischemic heart disease and coronary artery disease. However, as recent studies have shown that coronary atherosclerotic plaque, which is less than 50% of coronary artery stenosis, also causes clinically significant cardiovascular complications, the interest on non-obstructive coronary artery disease is growing. In particular, as CT coronary angiography has been widely used, the anatomic structure of the coronary artery and the degree of occlusion of the coronary artery due to atherosclerotic plaques can be diagnosed accurately. As a result, patients diagnosed with non-obstructive coronary artery disease has been increased rapidly and the need for appropriate care guidelines for these patients is being emphasized. Recently, some studies on the efficacy of cardiovascular prophylactic drugs such as statins and aspirin have been published in patients with non-obstructive coronary artery disease. However, there is a lack of evidence to determine the appropriate treatment strategy according to the clinical characteristics of patients.
▢ Objective
The purpose of this study was to investigate the factors affecting the occurrence of major clinical events (death, acute myocardial infarction, late reperfusion) in Korean patients with non-obstructive coronary artery disease and to develop a prediction model for patient’s prognosis as well as to provide objective evidence for establishing the treatment strategy to help establish the domestic medical policy.
▢ Method
Through the linkage between the CARE-CCTA registry and the National Health Insurance Corporation data, the prognosis of patients can be followed-up. Predictive factors for major clinical events are confirmed using a logistic regression model. By establishing a predictive model and a risk index using the identified predictors, the risk of non-obstructive coronary artery disease patients can be stratified and the treatment strategy can be established according to the risk.
▢ Results
⦁ We developed a risk index for non-obstructive coronary artery
disease patients and the risk index ranged from 0 to 20.
⦁ Patients with a risk score of 0 to 4 have a similar risk to those with normal group (stenosis 0%) and those with a risk score of 10 or more have a similar risk to the obstructive coronary artery disease group.
Ⅰ.Subject status
The data used in this study included patients who had undergone CT coronary angiography at Seoul National University Hospital from 2003 to 2011 with a stenosis of 1-49%. The number of patients were 6,243 out of 25,087 patients. Of the patients selected for the study, 4,132 patients had all the variables used for the development of the predictive model. There were no differences in baseline characteristics and incidence of major clinical events between the selected complete data (4,132) and the total data (6,243). The age of the patients with the complete data selected were 59.7 years and majority of the patients were males (68%). The incidence of major clinical events for 5 years after entry was 3% for myocardial infarction, 1.7% for reperfusion and 2.2% for death, 6.2% for overall incidence.
Ⅱ. Major clinical event risk factors
A logistic regression model was used to investigate significant risk factors for major clinical events. Finally, the risk factors selected were age, hypertension, diabetes, hemoglobin, and Vessel score/CACS. Among the risk factors included in the risk prediction model, the age was the most influent factor and gender did not show statistical significance. However, the model was added because it is known that there is a gender difference in clinical cardiovascular events.
Ⅲ. Risk index
The risk index can range from 0 (no risk factor) to 15 (with all risk factors). The risk index of the data used in this study ranged from 0 to 12 in the Vessel score model and from 0 to 16 in the model including CACS.
IV. Risk stratification using risk index
The risk of major clinical events was estimated by stratifying 0 to 3 points, 4 to 7 points, and 8 points or more according to the risk index. As a result, the risk of major clinical events for 8 points or more group was estimated to be 0.782 (using Vessel score) and 0.796 (using CACS). The incidence of obstructive coronary artery disease was not different from the incidence of non-obstructive coronary artery disease. In addition, in the 0-3 point group, it was 0.97, similar to the 0% stenosis group.
▢ Conclusion and policy recommendations
To date, this is the first model to predict the risk for patients with non-obstructive coronary artery disease and even within existing non-obstructive coronary artery disease patients, the risk of major clinical events varies according to the patient's clinical characteristics. This study provides objective evidence which can offer appropriate treatment strategies by identifying the patients with high-risk factors.
