1. 목적
본 연구에서는 다음의 두 가지 목적을 통해 급성 심근경색증 환자의 효율적인 관리와 합리적인 의료비 지출을 위한 객관적 근거를 보건의료관련 종사자들에게 제공하고자 한다.
1) 급성 심근경색증에서 약물방출 스텐트와 비약물 금속스텐트의 효과 비교에 대한 체계적 문헌고찰을 통해 두 스텐트 간의 급성 심근경색증 환자에서의 임상적 효능 및 안전성을 평가한다.
2) 급성 심근경색증에서 약물방출 스텐트와 비약물 금속스텐트 간의 비용 최소화 분석을 통해 두 스텐트의 경제성을 평가한다.
2. 연구방법
1) 체계적 문헌고찰
본 연구의 목적에 맞는 선행 체계적 문헌고찰이 있는지 문헌고찰을 하였다. cochrane library, trip, sumsearch 전자데이터베이스를 이용하여 문헌 검색을 하였으며, 7개 국내 데이터베이스도 검색을 하였다. 검색된 체계적 문헌고찰에 대하여 amstar를 이용한 질 평가를 하였으며 본 연구의 목적에 맞는 새로운 체계적 문헌고찰을 위해 이용할 수 있는 선행 체계적 문헌고찰이 있는지 평가하였다.
본 연구의 목적에 맞는 새로운 체계적 문헌고찰을 위해서 2009년 9월까지 ovid-medline, embase, cochrane library 전자데이터베이스를 이용하여 문헌 검색을 하였으며, 7개 국내 데이터베이스를 검색원으로 활용하였다. 관련 유명 심장학회의 초록과 중요 웹사이트, 참고문헌 리스트는 손으로 검색(hand search)하였으며 필요하면 저자에게 직접 연락하였다.
두 명의 독립적인 검토자가 각 성과변수 별(사망, 재발 심근경색, 표적혈관 재관류술[tvr], 표적병변 재관류술[tlr], 스텐트 혈전증), 연구형태 별(무작위배정 비교임상 시험, 비무작위배정 비교임상 시험)로 자료를 추출 하였다. 메타분석은 inverse-variance random-effects 모델을 사용 하였으며 효과지표는 상대적위험도(rr)이었다. funnel plot, begg test, egger test를 사용하여 출판 비뚤림을 평가하였고 출판 비뚤림이 있다고 판단되는 경우 nonparametric trim and fill 방법을 사용하여 메타분석 결과 값이 변화하는지 검토하였다. 이질성을 평가하기 위하여 cochrane q statistic과 i2 statistic을 사용하였고 적절한 하위 그룹 분석과 민감도 분석, 메타 regression을 시행하였다.
무작위배정 비교임상시험 연구는 cochrane's risk of bias로, 비무작위배정 비교임상시험 연구는 minors로 질 평가를 실시하고, 체계적 문헌고찰 결과의 근거 수준을 결정하기 위하여 gradepro로 평가하였다.
2) 경제성 분석
경제성 분석을 위한 인구집단은 st분절상승을 가지는 급성 심근경색증 환자 중 스텐트 시술을 처음 받은 환자를 대상으로 하였다. 체계적 문헌 고찰 결과 주요 결과변수인 사망에 있어 두 스텐트 간에 유의한 차이가 나타나지 않아서 비용 효과성 분석이 아닌 비용 최소화 분석을 시행하였다. 비용 최소화 분석은 두 스텐트 간의 재시술률의 차이 등으로 인한 비용차이가 반영되었다. 사회적 관점으로 분석을 시행하였으며 분석 모형은 decision analytic model로 하였다. st분절상승을 가지는 급성 심근경색증은 만성질병이 아니고 응급을 요하는 질환이고 재시술도 1년 이내에 이루어지며 세 번째 시술이 일어날 확률은 극히 미미하므로 추적 기간은 1년으로 하였다.
본 연구의 경제성 분석에 국내 실정을 반영하기 위하여 건강보험심사평가원의 자료를 이용하였다. 요양개시일을 기준으로 2006년 1월 1일부터 2007년 12월 31일 까지의 청구 명세서 중 스텐트 시술을 받은 환자 중 주상병이 급성심근경색증(i21)인 환자를 대상군으로 하였다. 본 연구의 목적에 맞는 st분절상승을 가지는 급성 심근경색증 환자를 확인하기 위하여 두 가지 방법을 사용하였다. 응급으로 내원한 경우와 급성심근경색증 상병코드 중에서도 상세불명의 급성 심근경색증(i21.9) 코드가 있는 경우가 그것이다. 후자는 국내의 대형 대학병원 세 곳에서 st분절상승을 가지는 환자인 경우 사용하는 코드로 확인되어 사용하였다.
전이확률은 체계적 문헌 고찰 결과와 건강보험심사평가원의 분석자료를 이용하여 스텐트, 관상동맥 우회로술(cabg), 풍선혈관 성형술(balloon)로의 전이확률을 이용하였다.
비용은 국내 실정을 반영하기 위하여 건강보험심사평가원 자료를 이용하였고, 미시적 방법으로도 비용을 산출하였으며 2009년의 가치로 추계하였다. 민감도 분석을 위하여 건강보험심사평가원 자료에서 대상 환자군 선정에 사용한 정의를 달리 하여 산출된 값을 이용하였고, 토네이도 다이어그램과 monte-carlo simulation을 통한 확률적 민감도 분석을 시행하였다.
3) 합의 도출 방법
본 과제는 주제의 특성 상 심장내과의 전문적인 분야이므로, 심장학회의 추천을 받은 자문위원들에게 본 연구 결과에 대해 의견(endorse)을 받았다.
3. 연구결과
1) 체계적 문헌고찰
선행 체계적 문헌고찰의 검색 결과 총 9건의 문헌고찰이 검색되었으며, 질 평가와 각 문헌고찰이 포함하는 임상연구들을 확인한 결과 brar등의 체계적 문헌고찰이 본 연구의 목적과 잘 부합하였다. 새로운 체계적 문헌고찰은 brar등이 포함한 임상연구들을 이용하여 그 이후의 연구들에 대하여 새로이 검색하였다.
새로운 체계적 문헌고찰 결과, 총 49편의 문헌(52,503명)이 최종 선택 되었고, 이중 14편은 무작위배정 비교임상시험, 35편은 비무작위배정 비교임상시험이었다.
무작위배정 비교대조 임상시험에서 7,654명의 연구 결과를 통합하여 메타분석을 실시한 결과, 약물방출 스텐트와 비약물 금속스텐트는 사망(rr=0.88, 95%ci 0.70-1.11, p=0.28), 스텐트 혈전증(rr=0.93, 95%ci 0.72-1.21, p=0.59) 발생에 있어 유의한 차이를 보이지 않았다. 그러나 약물방출 스텐트는 비약물 금속스텐트에 비교하여 심근경색증 재발(rr=0.76, 95%ci 0.60-0.96, p=0.02), tvr(rr=0.48, 95%ci 0.41-0.56, p<0.0001), tlr(rr=0.42, 95%ci 0.33-0.54, p<0.0001)의 발생을 유의하게 감소시키는 것으로 나타났다. 출판 비뚤림과 이질성의 문제는 없는 것으로 나타났다.
비무작위배정 비교대조 임상시험 44,849명의 연구 결과를 추적 기간을 고려하지 않고 통합한 결과, 약물방출 스텐트는 비약물 금속스텐트에 비하여 사망(rr=0.82, 95%ci 0.73-0.91), tvr(rr=0.61, 95%ci 0.48-0.77), tlr(rr=0.44, 95%ci 0.32-0.60)의 발생을 유의하게 감소시키는 것으로 나타났다. 그러나 심근경색증 재발(rr=0.94, 95%ci 0.85-1.03), 스텐트 혈전증(rr=0.88, 95%ci 0.64-1.23) 발생에 있어서 두 스텐트 간에 유의한 차이를 보이지 않았다. 비무작위배정 비교대조 임상시험의 경우 추적 기간에 상관없이 메타분석을 하였을 때 결과의 이질성이 유의하게 높았기 때문에 1년 이내, 2년 이내 등으로 추적 기간을 나누어 메타분석을 시행하였다. 추적 기간을 구분하여 분석하였어도 약물방출 스텐트는 비약물 금속스텐트에 비하여 사망, tvr, tlr의 발생을 유의하게 감소시키는 것으로 나타났다. 그러나 심근경색증 재발과 스텐트 혈전증의 발생은 1년 이내에는 약물방출 스텐트를 사용한 환자들에서 비약물 금속스텐트를 사용한 환자 보다 유의하게 낮았으나, 2년의 추적 기간 때에는 두 스텐트 간의 차이가 없었다. 오히려 스텐트 혈전증의 경우는 2년 이상의 추적 기간을 가진 6,646명을 통합한 연구들에서 약물방출 스텐트를 사용한 환자들에서 비약물 금속스텐트를 사용한 환자보다 그 발생율이 유의하게 높게 나왔다. 출판 비뚤림과 이질성의 문제는 없는 것으로 나타났다.
무작위배정 비교임상 연구의 체계적 문헌고찰 결과의 근거 수준을 평가한 결과, 사망과 심근경색증 재발은 "moderate quality", tvr과 tlr은 "high quality", 스텐트 혈전증은 "low quality" 수준의 근거를 제공하는 것으로 평가되었다. 비무작위배정 비교임상 연구의 체계적 문헌고찰 결과의 근거 수준은 모든 성과변수들에 대해 “very low quality” 또는 “low quality”이었다.
2) 경제성 분석
건강보험심사평가원의 비용자료를 이용하였을 때, 한 환자 당 1년간 평균 비용은 약물방출 스텐트 시술 시 9,085,047원이었고 비약물 금속스텐트 시술 시 8,040,351원이었다. 두 스텐트 간의 비용 차이는 1,017,696원으로 약물방출 스텐트가 비용을 더 소요하는 것으로 나타났다. 본인 부담률을 보정하였을 때, 약물방출 스텐트 한 환자 당 1년간 평균 비용은 10,939,786원이었고 비약물 금속스텐트는 9,710,672원이었다. 두 스텐트 간의 비용 차이는 1,229,114원으로 나타났다.
미시적 비용의 방법을 이용하였을 때, 한 환자 당 1년간 평균 비용은 약물방출 스텐트로 시술 시 5,570,288원이었고 비약물 금속스텐트로 시술 시 5,305,411원이었다. 두 스텐트 간의 비용 차이는 264,877원으로 약물방출 스텐트가 비용을 더 소요하는 것으로 나타났다.
민감도 분석 결과, 첫 스텐트 시술을 받고 재시술을 받지 않는 환자들의 비용과 비율이 비용 최소화 분석 결과에 가장 큰 영향을 미치는 것으로 나타났다.
4. 연구의 한계
본 연구는 체계적 문헌고찰과 경제성 분석을 시행함에 있어 보다 정확한 방법론을 적용하고자 노력하였다. 그럼에도 불구하고 여러 가지 한계점을 지닌다. 첫째, 체계적 문헌 고찰에 포함된 문헌들은 대부분이 국외에서 연구된 문헌들이다. 둘째, 경제성 분석 시 시술 병변(single vessel, small vessel, long lesion, multi-vessel) 별로 구분하여 분석을 시행하지 않았다. 비용이 더 소요되는 long lesion이나 multi-vessel에서 약물방출 스텐트가 비약물 금속스텐트 보다 시행될 확률이 더 높기 때문에, 시술 병변을 고려하지 않은 경제성 분석 결과가 약물방출 스텐트에 더 불리하게 나올 가능성이 있다. 건강보험심사평가원 자료는 시술병변이나 혈관크기 등을 포함하지 않기 때문에 의무기록 조사를 하지 않는 한 이 한계점은 극복하기 어렵다. 이 연구는 국내의 대규모 레지스트리 자료인 kamir와 건강보험심사평가원 자료의 연계를 통해 한국형 근거자료를 제공함이 목적이었으나, 개인정보보호법의 이유로 자료 연계가 불가능하였다. 셋째, 경제성 분석 시 사용한 건강보험심사평가원 자료들은 약물방출 스텐트를 받은 환자들의 질환의 중증도나 동반질환을 전혀 고려하지 않은 한계점이 있다. 약물방출 스텐트는 중증도가 높은 환자에게 자주 사용되는 경향이 있고, 이 환자들은 다른 동반질환을 가지고 있을 확률이 높기 때문에 건강보험심사평가원의 자료로 비용을 추계한 경우, 그 비용이 비약물 금속스텐트를 받은 환자 보다 과대 추정되었을 위험이 매우 크다.
5. 결론 및 정책제언
52,503명을 포함하는 무작위배정 비교임상 연구과 비무작위배정 비교임상 연구를 포함하는 총 50편의 문헌 분석 결과, 재시술을 제외하고는 약물방출 스텐트가 비약물 금속스텐트에 비해서 임상적 효능과 안전성에 큰 차이가 없는 것으로 나타났다. 무작위배정 비교임상시험을 대상으로 한 체계적 문헌 고찰 결과 st분절상승을 가지는 급성 심근경색에서 약물방출 스텐트는 비약물 금속스텐트와 비교하여 안전성(사망과 스텐트 혈전증)에 차이가 없는 것으로 나타났다. 약물방출 스텐트는 비약물 금속스텐트에 비하여 심근경색증 재발을 감소시키는 것으로 나타났으나 그 유의성 정도는 크지 않았다. 비무작위배정 비교임상시험을 대상으로 한 체계적 문헌 고찰 결과 약물방출 스텐트는 비약물 금속스텐트에 비하여 사망을 유의하게 감소시키는 것으로 나타났으나, 심근경색증 재발과 스텐트 혈전증에 있어서는 전반적으로 두 스텐트 간 차이가 없는 것으로 나타났다. 약물방출 스텐트는 비약물 금속스텐트에 비하여 tvr, tlr의 발생을 연구 디자인과 무관하게 항상 유의하게 감소시키는 것으로 나타났다. 2년 이상 추적한 비무작위배정 비교임상 연구에서는 약물방출 스텐트가 스텐트 혈전증을 증가시킬 위험이 있다고 나타났으나 이는 비무작위배정 비교임상 연구를 통해 나온 결과이므로 결과 해석 시 주의가 필요하다.
두 스텐트 간의 경제성 분석을 시행한 결과 약물방출 스텐트가 비약물 금속스텐트 보다 약간 더 많은 비용을 소요하는 것으로 나타났으나 이는 재시술을 받지 않는 환자의 비용과 비율에 아주 민감하므로, 정확한 값 산출에 대한 추가 연구가 필요하다.
국내의 급성 심근경색증에서의 약물방출 스텐트와 비약물 금속스텐트 간의 임상적 효능, 안전성, 경제성을 확실하게 비교하기 위해서는 국내의 장기간 대규모 무작위배정 비교임상 시험이나 양질의 장기간 대규모 전향적 임상 연구가 필요하다. 또한 당뇨 등 동반 질환을 가지고 있는 고위험군 환자의 경우, 스텐트 간의 효과 및 안전성의 차이 등을 심층적으로 분석하여, 서로 다른 동반 질환을 가진 심근경색 환자를 대상으로 차별적 치료 접근이 필요한 지에 대해서도 연구가 필요하다.
1. Objectives
We aim to provide objective evidence to healthcare providers to efficiently manage patients with acute myocardial infarction withST-elevation (STEMI) by following detailed objectives.
1) Evaluate the clinical efficacy and safety of drug-eluting stents (DES) versus bare-metal stents (BMS) in patients with acute myocardial infarction through a systematic review and meta-analyses.
2) Perform an economic assessment comparing DES and BMS in patients with acute myocardial infarction through a cost-minimization analysis.
2. Methods
1) Systematic Review
We searched the Cochrane Library, TRIP database, SUM search database, and seven Korean databases to identify review articles comparing outcomes between DES and BMS in patients with acute myocardial infarction. Each review article was assessed and evaluated by two independent reviewers using AMSTAR, a measurement tool for the assessment of multiple systematic reviews.
In addition, Ovid-Medline, EMBASE, the Cochrane Library, and seven Korean databases were searched for primary research articles comparing outcomes between DES and BMS in patients with STEMI, and presented or published through September 2009. Major conference proceeding databases were also searched and included the Scientific Sessions of the American College of Cardiology, the American Heart Association, the Transcatheter Cardiovascular Therapeutics, and the European Society of Cardiology. Finally, websites including cardiosource.com and TCTMD.com were also
searched for relevant studies. The references of prior review articles and meta-analyses were reviewed for additional possible studies and authors were contacted if needed. Randomized controlled trials (RCTs) and observational studies (non-RCTs) were included.
Two independent reviewers extracted variables of clinical endpoints (death, recurrent myocardial infarction, target vessel revascularization [TVR], target lesion revascularization [TLR], and stent thrombosis), and examined study types (RCTs and non-RCTs). The relative risk (RR) was calculated using the inverse variance method to pool the outcomes of each clinical endpoints. The average effects of eachoutcome and 95% confidence intervals (CI) were obtained using a random-effects model since this model provided wider, more conservative confidence intervals than would the fixed-effects model. It also allowed for generalization despite some degree of heterogeneity among patient samples included in the meta-analyses. A funnel plot, Begg test and Egger test were used to assess the presence of publication bias. If publication bias was suspected, the non-parametric trim and fill method were used to examine the impact of hypothetically imputed studies on the pooled estimates. The
Cochrane Q statistic and the I2 statistic were also used to assess the heterogeneity of RRs. Subgroup analyses, sensitivity analyses, and meta-regression were also performed. Two independent reviewers assessed the quality of included articles by using the Cochrane's risk of bias for RCTs, and the methodological index for non-randomized studies (MINORS). To determine the quality of evidence, GRADEpro was used.
2) Economic Assessment
The patient population for the economic assessment was stent-naive patients with STEMI. A cost-minimization analysis was performed because there was no significant difference between DES and BMS in the occurrence of death, which was the major clinical endpoint. The cost difference caused by the difference in the rate of revascularization between DES and BMS was reflected in this analysis. A decision analytic model with a societal perspective was developed to estimate the cost difference between DES and BMS. The time horizon used was one year because STEMI is an acute disease requiring emergency treatments and revascularization usually occurs within one year.
The National Insurance Claims Database obtained from the Health Insurance Review and Assessment Service (HIRA) was utilized to obtain transition probabilities and cost estimates reflecting the local situation. The patient population was defined as subjects with acute myocardial infarction who underwent stent procedures through
emergency department visits between January 1st, 2006 and December 31st, 2007.
The transition probabilities of DES, BMS, CABG, and balloons were obtained from the results of the systematic review and the analysis results of HIRA data. Costs in 2009 were obtained from the analysis results of HIRA data and the micro-costing method. Univariate sensitivity analyses were performed by presenting a Tornado diagram and probabilistic sensitivity analyses were performed.
3. Results
1) Systematic Review
Nine reviews and meta-analyses were identified. After assessing the quality of reviews, the systematic review performed by Brar, et al was chosen. Including the clinical studies considered in the review of Brar, et al and those identified through a new search, fifty studies were identified (N=52,503). Among them, fifteen studies were RCTs and 35 studies were non-RCTs.
A meta-analysis using data from 7,654 patients in RCTs showed that when comparing DES and BMS, there were no detectable differences in mortality (RR=0.88, 95%CI 0.70-1.11, p=0.28) or stent thrombosis (RR=0.93, 95%CI 0.72-1.21, p=0.59). On the other hand, a significant reduction was observed from the DES group in terms of the recurrence of myocardial infarction (RR=0.76, 95%CI 0.60-0.96, p=0.02), TVR (RR=0.48, 95%CI 0.41-0.56, p<0.00001), and TLR (RR=0.42, 95%CI 0.33-0.54, p<0.0001). There was no evidence of statistical heterogeneity or publication bias among these studies.
In non-RCTs with 44,894 patients, the use of DES was associated with significant reductions in mortality (RR=0.82, 95%CI 0.73-0.91), TVR (RR=0.61, 95%CI 0.48-0.77), and TLR (RR=0.44, 95%CI 0.32-0.60) compared with BMS. However, there was no significant difference between DES and BMS in the recurrence of myocardial infarction (RR=0.94, 95%CI 0.85-1.03) or stent thrombosis (RR=0.88, 95%CI 0.64-1.23). Since there was significant heterogeneity in the pooled data, a meta-analyses was performed by follow-up period (within one/two year(s) of the index stenting). The use of DES was still associated with significant reductions in mortality, TVR, and TLR. However, even though DES was associated with significant reductions in the recurrent myocardial infarction and stent thrombosis within one year of the index stenting, there were no differences between DES and BMS at two years of follow-up. Among six observational studies (N=6,646) with over two years' of follow-up, the use of DES was associated with a significant elevation of stent thrombosis compared with BMS. When studies were categorized by follow-up period and performed analyses, we found no evidence of statistical heterogeneity and publication bias.
The quality of the evidence derived from the RCTs was evaluated as "moderate" for mortality and myocardial infarction, "high" for TVR and TLR, and "low" for stent thrombosis. The quality of the evidence from non-RCTs was "very low" or "low" for all clinical endpoints.
2) Economic Assessment
Based on HIRA data, the average one-year cost per patient in the perspective of insurer was estimated to be 9,085,047 KRW and 8,040,351 for DES and BMS, respectively. The use of DES resulted in the higher costs of 1,017,696 KRW per person a year compared with BMS. From a societal perspective, the average costs of using DES and BMS in 2009 was 10,939,786 KRW per person a year and 9,710,672 KRW per person a year respectively. The use of DES was associated with the higher costs of 1,229,114 KRW per person a year compared with BMS. Using micro-costing method, the average costs of using DES and BMS were 5,570,288 KRW per person a year and 5,305,411 KRW per person a year respectively from a societal perspective. The use of DES was associated with the higher costs of 264,877 KRW per person a year compared with BMS. The cost difference between the use of DES and BMS was highly sensitive to incurred costs and the proportion of patients without revascularization.
4. Limitations
This study is subject to a number of limitations. First, most of articles included in this meta-analysis were based outside of Korea.
Secondly, an economic assessment considering various lesions that underwent procedures (single vessel, small vessel, long lesion, and multi-vessel) was no performed. DES are used more often in long lesions or multi-vessels which often result in worse outcomes and higher costs compared with BMS. Thus, an economic assessment that does not consider procedure lesions might produce results against
DES. the effect that differing procedures have is not a true picture. Since HIRA data does not include data presenting lesions or the diameter of vessels, this limitation is hard to overcome without specific chart reviews.
Thirdly, the disease severity or comorbidity condition of patients was not factored in when using HIRA data for economic assessments. Since DES are used more often in severe patients who may also have a comorbid disease, the estimated costs incurred in patients from DES group may be overestimated than from BMS group if
disease severity and comorbidity are not considered.
5. Conclusions
In this meta-analysis of 52,503 patients with STEMI from forty nine studies including RCTs and observational studies, the use of DES compared with BMS was associated with a significant reduction in TVR and TLR. DES and BMS did not show any differences in adverse effect outcomes such as death or stent thrombosis in RCTs. The use of DES in RCTs was associated with statistically significant reduction
in the recurrence of myocardial infarction, but the significance level was in the margin. In observational studies, the use of DES was associated with a significant reduction in mortality compared with BMS. However, there were no differences between DES and BMS in recurrent myocardial infarction and stent thrombosis overall. The use of DES was related with a significant increase in stent thrombosis compared with BMS in observational studies with over two years of follow-up. However, caution is needed to interpret this result because it is derived from observational studies and the evidence level was evaluated as "very low quality".
The use of DES appears to cost more than BMS. However, since this was very sensitive to the costs and proportions of patients without revascularization, additional studies are required to obtain more accurate estimates of costs and proportions of patients without revascularization after initial stenting. Large-scale, long term local RCTs or prospective studies with high quality are required to ascertain the clinical efficacy, safety, and economic effects of DES compared with BMS.
