우리나라의 30세 이상 성인의 연령표준화 이상지질혈증의 유병률은 35.8%, 45.1%, 48.3%로 점점 증가하는 추세를 보이고 있어 이상지질혈증은 우리나라의 주요 성인 질환으로 인식되고 있다.
미국에서는 8시간 공복 후 총콜레스테롤이 240 mg/dl 이상이거나 LDL 160 mg/dl 이상, 또는 중성지방 200 mg/dl 이상, HDL 40mg/dl 미만을 기준으로 하여 한 가지라도 충족시킬 경우를 이상지질혈증으로 정의하며, LDL을 primary target, 중성지방을 secondary target으로 치료의 방향을 권고하고 있다. 그러나 우리나라의 경우 서양에 비해 총콜레스테롤, HDL, LDL 등의 수치는 낮지만 중성지방은 높은 특징을 보이고 있으며, 연령대에 따른 특성도 총콜레스테롤, LDL, 중성지방이 40~49세 혹은 50~59세 구간까지 증가해 최고점을 이루며 이후 연령 증가에 따라 감소하는 양상을 보인다. 이로부터 혈중 지질농도는 인종간 차이가 존재함을 알 수 있다.
따라서 우리나라에 맞는 이상지질혈증 진단기준과 심뇌혈관질환에 대한 위험도를 산출하는 도구가 필요하다. 본 연구에서는 한국건강관리협회 소속 건강진단 센터 네트워크의 검사결과 자료를 건강보험심사평가원의 청구자료와 연계하여 이상지질 검사치를 중심으로 한 요인들과 실제 심뇌혈관질환 발생으로 청구되는 의료이용의 관계를 파악하려고 하였다. 구체적으로는 연계된 자료를 이용하여 국내 성인을 대상으로 심뇌혈관 질환 발생에 가장 영향력이 높은 이상지질 형태를 조사하고 심뇌혈관질환 발생에 있어 이상지질혈증에 대한 연령대별 진단 기준의 가능성을 모색하는 것이다.
본 연구 대상자들의 TC, HDL, LDL, TG, non-HDL 검사치의 평균값을 살펴보면 각각 180.8, 51.5, 107.3, 111.8, 129.3로 나타나고 있으며, 성별에 따른 연령대별 지단백 검사치들을 국민건강영양조사자료(2007년)와 미국 NHANES자료(2007 ~2008년)의 조사결과와 비교해본 결과 본 연구대상자들은 이상지질혈증에 있어서 이들 자료에 비해 좀 더 안정적인 수치들을 나타내고 있었다.
연구 대상자 42,275명 중 심뇌혈관 질환 발생자수는 589명(1.4%)으로, 남성이 1.6%, 여성이 1.2%로 남성이 조금 높은 발생률을 보이고 있다. 남성의 경우 30세 미만 연령대에서 심뇌혈관질환 발생률은 0.3%였으며 연령대가 증가할수록 발생률도 점점 증가하다가 70세 이상 연령대에서 12.1%로 가장 높은 발생률을 보였다. 여성의 경우도 30세 미만 연령대에서의 발생률은 0.1%였으며 연령대가 증가할수록 점점 증가하다가 70세 이상 연령대에서 10.1%로 가장 높은 발생률을 보였다.
지단백 검사치들의 상관계수를 고려하여, 지단백 검사치들을 하나씩 포함한 5개의 다변수 로지스틱 회귀모형을 적합시켜 본 결과, 심뇌혈관질환 발생에 대해서 남성의 경우 연령, 고혈압, uric_acid, TC, LDL, TG, non-HDL이 위험요인으로 파악되었으며, 여성의 경우 연령, 고혈압, BMI, TG가 위험요인으로 파악되었다. 연령이 증가할수록 심뇌혈관질환 발생위험은 증가하였으며, 검사자료의 경우 정상범위에 비해 비정상 범위에서 심뇌혈관질환 발생위험이 더 큰 것으로 나타났다.
심뇌혈관질환 발생까지의 시간을 고려한 콕스-비례위험모형결과와 비교해 본 결과 남성과 여성모두 로지스틱 회귀분석 결과와 동일한 결과를 제시하였다.
심뇌혈관 질환 발생에 대한 최종 예측모형에는 남성의 경우 연령, 고혈압, TG, non-HDL이 포함되었으며, 예측모형의 타당도는 c-통계량이 0.783, concordant rate가 73.5%로 나타났다. 여성의 경우 연령, 고혈압, TG가 예측모형에 포함되었으며, c-통계량과 concordant rate가 각각 0.817, 77.1%로 나타났다.
약제사용은 심뇌혈관질환 발생에 영향을 줄 수 있기 때문에 검진 이전 고혈압, 당뇨, 이상지질혈증, 항혈소판 약제를 사용한 적이 있거나 검진 이후 각각의 약제에 대한 순응도가 0.5 이상인 대상을 제외하고 하위그룹 분석을 수행하였다. 그 결과 남성의 경우 연령, 고혈압, TC, LDL, non-HDL이 위험요인으로 나타났으며 이는 전체연구대상자에서의 결과와는 다르게 TG가 제외된 결과이다. 여성의 경우는 전체 연구대상자의 결과와 동일하게 연령, 고혈압, BMI, TG가 위험요인으로 나타났다. 한편 예측모형에서는 남성의 경우 연령대, 고혈압, TC가 포함되었고, 여성에서는 연령대, 고혈압, BMI, TG가 포함되었다.
분석의 기반이 된 한국건강관리협회의 종합검진자료는 건광관리협회에 등록된 의원 및 병원의 자료이다. 즉, 조사대상자 추출이 무작위로 이루어진 것이 아니기 때문에 본 연구에서 구축한 코호트가 과연 한국인 전체를 대표할 수 있는지에 대해서는 의문이 있을 수 있으며 심뇌혈관질환 발생건수가 1,308건으로 분석을 위해 충분히 많은 수라고 볼 수 없을 수도 있다. 그러나 본 연구 목적에 부합하게 심뇌혈관질환 위험요인이 서양과 다르다는 것을 확인할 수 있는 결과를 보이고 있다.
본 연구의 Median follow time은 심뇌혈관질환 발생자의 경우 1.7년, 미발생자는 3.7년으로 고지혈증의 심뇌혈관질환에 대한 효과를 알아보기에 다소 짧은 기간이다. 그러나 이는 자료원이 된 심평원 연계자료가 법률상 문제로 자료 제공 범위가 5년으로 한정되어있기 때문에 발생한 한계이다. 또한 본 연구에서는 검진결과 변수 중 흡연, 음주, 식습관 등 생활습관 관련한 변수는 분석에 포함하지 못하였다는 한계가 있다. 따라서 향후 대규모의 환자 코호트를 구축하여 본 연구에서 포함하지 못한 다양한 교란요인을 고려한 연구가 요구되며, 20대의 경우 질환 발생군과 미발생군의 지질 분포 양상은 이 외의 연령대와 다르게 나타나고 있기 때문에 추가적 연구를 통해 지질 분포 양상이 연령대별로 미치는 영향력에 실제 차이가 있는지 확인이 필요하다.
이와 같은 한계에도 불구하고 본 연구는 한국인에서 성별에 따른 심뇌혈관 질환의 현황과 위험요인을 파악하였다는 것과 한국인의 심뇌혈관 질환에 영향력이 높은 이상지질혈증의 형태를 조사하였다는 점에서 의의를 가진다. 현재 우리나라에서는 미국 ATP-III 치료지침에 따라 LDL 콜레스테롤을 이상지질혈증 치료의 1차 표적으로 지정하고 있으나, 본 연구에 따르면 한국인은 남성의 경우 TC, LDL, TG, non-HDL이, 여성은 TG가 심뇌혈관질환 발생에 영향력이 큰 지질 항목임을 알 수 있었다. 이는 향후 한국인의 특성에 맞는 치료 지침과 대책을 확립하는 근거가 될 수 있다. 특히 성별로 고려해야 할 위험요인이 다르기 때문에 향후 추가연구들은 성별에 따른 지질형태의 서로 다른 상대적 위험을 파악할 수 있도록 해야 하며, 이러한 현상의 원인에 대해 탐색이 필요할 것이다
또한, 다양한 위험인자를 고려한 심뇌혈관 질환 발생 예측 모형을 유도하여 심뇌혈관 질환 위험인자를 가지고 있을 때 성별에 따른 질환 발생 위험도를 예측할 수 있도록 제시하였다는 점에서 의의가 있다. 향후 대규모 추가연구들을 통해 사건 발생 위험도 계산식을 확인하는 작업이 수행되어야 하며, 추가적으로 위험도 계산식을 연령대별로 고려하려는 접근이 있어야 할 것이다.
In Korea, the prevalence of dyslipidemia in the age-standardized population over 30 years old is showing an increasing trend from 35.8% to 45.1% to 48.3%. As indicated in these figures, dyslipidemia is recognized as one of the major adult diseases in this country.
In the United States, dyslipidemia is defined as meeting one of the following criteria: total cholesterol (TC) ≥ 240 mg/dl, low density lipoprotein (LDL) ≥ 160 mg/dl, triglycerides (TG) ≥ 200 mg/dl, or high density lipoprotein (HDL) <40mg/dl after eight-hour fasting. Dyslipidemia treatment with LDL as a primary target and TG as a secondary target is recommended in the United States. In Korea, on the other hand, the lipid profile is different such as lower HDL, TC, and LDL levels and higher TG level compared to western countries. As for characteristics by age group in Korea, TC, LDL, and TG increase to reach the peak in the age bracket of 40 to 49 years old or 50 to 59 years old, and then decrease in the later years of age, which indicates ethnic differences in the prevalence of this disease.
Due to these reasons, it is necessary to develop diagnostic criteria for dyslipidemia and a tool to calculate risks to cardiocerebrovascular diseases that better fit the situations in Korea. This study was aimed to investigate the relationship between the disease factors derived from dyslipidemic test values and health care utilizations claimed for the actual incidences of cardiocerebrovascular diseases by using the test results obtained from the Medical Examination Center Network under the umbrella of Korea Association of Health Promotion along with the insurance claim data obtained from the Health Insurance Review & Assessment Service. More specifically, these data were used to identify types of dyslipidemia with the biggest impact on the development of cardiocerebrovascular diseases in Korean adults and
to explore a possibility of establishing diagnostic criteria by age for dyslipidemia in association with development of cardiocerebrovascular diseases.
In the study results, the mean values of TC, HDL, LDL, TG, and non-HDL in the study subjects were 180.8, 51.5, 107.3, 111.8, and 129.3, respectively. Compared to the mean values of TC, HDL, LDL, and TG in Korean patients reported by the Korea Centers for Disease Control & Prevention in 2007 and National Health and Nutrition
Examination Survey in 2007-2008, the subjects in this study seemed to have been comprised of lesser dyslipidemia compared to the overall population in Korea.
Among the total 42,275 subjects in this study, 638 subjects (1.5%) developed cardiocerebrovascular diseases. The incidence was slightly higher in men (1.6%) compared to women (1.2%). By age, the incidence of cardiocerebrovascular diseases among the men was 0.3% in subjects aged below 30, which then increased gradually with the increasing age, reaching the highest rate of 12.1% in the age group of 70 years old or above. For women, the incidence was lowest in below 30 of 0.1% then increased with age and highest in 70 or above of 10.1%.
In this study according to multivariable logistic regression models, for men, age, hypertension, TC, LDL, TG, and non-HDL were identified as risk factors of cardiocerebrovascular diseases. For women, age, hypertension, BMI and TG were identified as risk factors of cardiocerebrovascular diseases. The risk to develop
cardiocerebrovascular diseases has increased in older ages. The study results also indicated that the out of normal range lab values were more associated with the development of cardiocerebrovascular diseases compared to the test values falling into the normal ranges.
In the final predictive model regarding the development of cardiocerebrovascular diseases, for men, age, hypertension, TG and non-HDL were included as variables with c-statistics of 0.783 and concordant rate 73.5%. Similarly, for women, age, hypertension, and TG were included in the final model with c-statistics of 0.817 and
concordant rate 77.1%.
Since the use of anti lipid drugs after lab test may influence the incidence of cardiocerebrovascular diseases, a subgroup analysis was added for the patients who never used a anti lipid drug after lab tests or whose drug compliance is under 0.5. In the subgroup analysis for men, age, hypertension, TC, LDL and non-HDL were identified as risk factors whereas age, hypertension, BMI and TG were identified as risk factors for women.
Limitations of this study are as follows: The comprehensive medical examination data, which served as basic data for this study, were provided by clinics and hospitals registered to the Korea Association of Health Promotion. This means that the study subjects were not randomly sampled, and this poses a question as to whether the cohort constructed in this study can represent the entire population in this country.
In this study, median follow-up time was 1.7 years for subjects who developed cardiocerebrovascular diseases and 3.7 years for subjects who did not. These periods are deemed rather short, and this can be another limitation for this study. Of particular note, since hyperlipidemia shows its impact after a long period of time, these periods are not exactly appropriate for the investigation of effects of hyperlipidemia on cardiocerebrovascular diseases. However, since the relevant law stipulates that the scope of data provided by the Health Insurance Review & Assessment Service should be limited to five-year information, this issue associated with source data is rather a legal problem.
Despite all these limitations, this study is still meaningful in that it has assessed the current status of cardiocerebrovascular diseases in Korea, identified risk factors of these diseases, investigated types of dyslipidemia with the biggest impact on cardiocerebrovascular diseases in Korean patients, and suggested a predictive model of the development of cardiocerebrovascular diseases.
