▢ 연구 배경 잠복결핵 감염 치료는 중요한 국가 결핵 관리 정책 중의 하나로, 세계보건 기구의 결핵 통제 정책 전망에 따르면 결핵 퇴치를 위해 ‘활동성 결핵’의 신속한 진단, 치료와 함께, 잠복결핵 감염의 진단과 치료가 필수적이다. 국내에서도 결핵 조기 퇴치를 위해 잠복결핵 감염 검진 및 치료 정책과 지원이 확대되고 있다. 결핵 가족 밀접 접촉자 검진에서 5세 미만의 소아에서 잠복결핵 감염 검사와 치료를 진행하던 과거 정책은 최근 강화 되어 2014년 결핵 진료지침에서 35세 미만 연령으로 밀접 접촉자의 잠복결핵 감염 치료가 확대 되었고, 2017년부터 결핵 진료 지침에서는 65세 이하로 확대 되었다. 그러나 국내에서 잠복결핵 감염 정책의 근거 마련 연구는 부족한 실정이며, 특히 가족 밀접 접촉자에서 잠복결핵 감염 치료 연령 확대에 대한 비용-효과 분석 등은 수행되지 못하였다. 고령층은 결핵 발생률이 높은 연령군이면서, 잠복결핵 감염 치료로 인한 부작용 발생 위험도 높은 군으로, 국내의 높은 결핵 발생률과, 적극적인 잠복결핵 감염 치료 정책을 고려하면, 국내 자료와 정책을 기반으로 연령에 따른 잠복결핵 감염 치료에 대한 비용-효과 분석을 통한 근거마련이 필요하다. ▢ 연구 목적 이 연구의 목적은 국내 결핵 발생의 동적 수리모델 (dynamic model)을 개발하고, 가족 밀접 접촉자에서, 잠복결핵 감염 치료 정책의 연령별 비용-효과 분석을 통해 국내 가족 접촉자 잠복결핵 감염 진단과 치료 정책의 근거를 제공하는 것이다.
▢ 연구 방법 이 연구는 크게 아래와 같이 5가지 단계로 연구가 진행되었다. 첫째, 국내 결핵 수리 모델 설계 및 연령 구조와 접촉 행렬 적용 및 안정화를 진행하였다. 결핵 동적 수리 모델의 기본 구조인 S (Susceptible)-E (Pre-infectious)-I (Infectious) -R (Recovered)에서 출발하여, S-ES-EL-I-L-T 구조를 구성하고, 국내 2011-2018년 결핵 신고 자료 및, 가족 접촉자 조사 자료, 국가 잠복결핵 감염 코호트 분석 자료, 결핵 역학 문헌을 기반으로 모델 calibration을 진행하였다. 둘째, 안정화된 결핵 수리 모델을 이용하여 가족 밀접 접촉자에서 잠복결핵 감염 치료 연령 확대 정책에 따른 결핵 발생 수와 결핵으로 인한 사망자 수 예측을 위해 모의실험을 수행하였다. 이때 잠복결핵 감염 치료에 대한 연령군 별 확대 정책은, 치료를 하지 않는 경우 (대조군), 0-35세 치료, 0-55세 치료, 0-65세 치료, 0-70세 치료를 비교하였다. 셋째, 1인당 잠복결핵 감염 진단과 치료, 활동성 결핵 진단과 치료 비용을 확인하기 위해, 건강 보험 심사 평가원 수가 자료 (2020)와 건강보험 공단 청구 자료를 이용하였다. 잠복결핵 감염 및 활동성 결핵 진단 비용은 수가 자료를 이용하여 계산하였고, 잠복결핵 감염 및 활동성 결핵 치료 비용은 공단 청구 자료를 이용하여, 잠복결핵 감염과 활동성 결핵의 조작적 정의와 치료 완료의 조작적 정의를 적용하여 심결요양급여 총액으로 분석하였다. 넷째, 가족 접촉자 잠복결핵 감염 치료의 연령군 별 확대 정책에 따른, 비용-효과 분석을 위해, Incremental cost effectiveness ratio (ICER) 지표를 계산하였다. 여기에서 효과는 각 정책으로 인해 증가한 질보정수명(QALY)와 감소한 결핵사망자 수 (averted number of death)로 정의하였다. 질병 상태에 따른 질보정수명(quality adjust life year) 값의 weight는 기존 문헌 자료를 바탕으로 적용하였다 (활동성 결핵 상태 0.76, 잠복결핵 감염 치료 상태 0.99. 그 외 상태 1). 제어 전략은 ⓵ 잠복결핵 감염 치료를 하지 않는 경우 (no treatment)를 대조군으로 상정하고, 비용-효과 분석 비교 전략으로 ➁ 0-35세 치료 전략 ➂ 0-55세 치료 전략, ➃ 0-65세 치료 전략 ➄ 0-70세 치료 전략을 분석하였다. 다섯째, 비용-효과 분석에 대해, 수리 모델에 적용한 모수 값의 불확실성에 대한 영향을 확인하기 위해 민감도 분석을 시행하였다. 민감도 분석은 2019년부터 2040년까지 기간 동안 누적 ICER를 결과 값으로 설정하고, 모델에 사용된 모수들이 5% 범위에서 변할 때 결과가 달라지는 정도를 측정하였다. 밀접 접촉자 전체를 대상으로 잠복결핵 감염을 진단하고 0-35세와 0-65세를 대상으로 치료하는 두 가지 제어 전략에 관해 결과를 도출하였다
▢ 연구 결과 국내 결핵 신고 자료와 역학 자료를 바탕으로 가족 밀접 접촉자의 연령별 잠복결핵 감염 치료 효과를 확인하기 위한 동적 수리 모델을 확립하였다.
구축된 국내 결핵 수리모델을 통한 2040년까지 폐결핵 환자 발생 예측 모의실험 결과, 잠복결핵 감염 치료를 0-35세 그룹에서 진행했을 때, 치료를 하지 않았을 경우와 비교하여 1,748명, 0-65세 그룹에서 잠복결핵 감염 치료를 진행했을 때는 5,912명 환자 발생 감소가 예상되었다. 2020-2040년 사이 연령별 치료 전략에 따른 비용-효과 분석에서 0-35세 치료 전략에서 120,000원/QALY, 0-65세 치료 전략에서 5,750,000원/QALY 결과를 보여 비용-효과 적이었다.
1. 국내 폐결핵 동적 수리 모델 구축 결핵 동적 수리 모델 SEIR 을 기반으로, 연구 목적에 맞추어 수리 모델 구조를 구성하고, 국내 2011-2018년 결핵 신고 자료 및, 가족 접촉자 조사 자료, 국가 잠복결핵 감염 코호트 자료, 결핵 역학 문헌을 검토하여 모수를 추정하였다.
2. 수리 모델을 통한 폐결핵 환자와 사망자 수 예측 모의 실험 수리모델 모의 실험을 통한 환자 수 예측은 잠복결핵 감염 치료를 하지 않는 경우를 기본 전략으로 상정하였다. 이를 대조군으로 사용하여 각각 0-35세, 0-55세, 0-65세, 0-70세 연령 그룹을 치료하는 전략과 비교하여 분석하였다. 2019-2040년 사이의 누적 폐결핵 발생수를 비교했을 때, 0-35세 치료 그룹에서는 대조군 대비 1748명 환자 수가 감소하였고, 0-65세 치료 그룹에서는 5912명, 0-70세 치료 그룹에서는 7850명 폐결핵환자 발생이 감소하였다. 결핵으로 인한 사망자 수 감소는 0-35세 치료 그룹에서 대조군 대비 12명, 0-65세 치료 그룹에서 143명, 0-70세 치료 그룹에서 256명 이었다.
3. 1인당 잠복결핵 감염 및 활동성 결핵 진단, 치료에 대한 비용 분석 건강보험 심사평가원 2020년 수가 자료를 이용하여 0-4세, 5세 이상에서 각각 1인당 잠복결핵 감염 및 활동성 결핵 진단 비용을 추정하였다. 잠복결핵 감염 진단 비용은 0-4세, 36,090원, 5세 이상 72,650원, 활동성 결핵 진단 비용은 0-4세 168,206원, 5세 이상 165,122원이었다. 1인당 잠복결핵 감염 치료 평균 비용은, 치료를 완료하였을 경우 180,499원 이었고, 연령에 따라 약간의 변화를 보였다. 1인당 활동성 결핵 치료 평균 비용은 치료를 완료한 그룹에서 3,461,975원이었고, 연령이 증가할수록 상승하는 경향이었다.
4. 가족 밀접 접촉자 잠복결핵 감염 연령별 제어 전략에 따른 비용-효과 분석 2020-2040년 까지, 제어 전략에 따른 누적 의료 비용을, 연령별, 1인당 잠복결핵 감염 진단과 치료 비용, 활동성 결핵 진단과 치료 비용 및 연 할인율 3%를 적용하여 산출하였다. 전 연령에서 잠복결핵 감염 검사를 시행하는 경우, 잠복결핵 감염 검사 및 치료비용은 2040년 기준 0-35세 치료 전략에서 2,908,000,000원 증가하고 0-65세 대상은 38,311,000,000원 증가하였다. 활동성 결핵 치료비용은 0-35세 치료에서 2,852,000,000원이 감소하고 0-65세에서 18,193,000,000원 감소하였다. 따라서 전체 의료비용은 0-35세를 대상으로 잠복결핵 감염을 치료하는 전략에서 55,000,000원이 증가하고 0-65세 치료 전략에서는 20,117,000,000원 증가하였다. QALY를 효과로 보고 ICER를 측정했을 때, 0-35세 치료 전략에서는 2040년 기준으로 비용이 55,000,000원 증가하고 QALY는 460이 증가하여 ICER 값이 120,000원/QALY에 해당하였다. 같은 조건으로 0-65세를 치료하는 전략에서 비용이 20,117,000,000원 증가하고 QALY는 3497이 증가하여 5,750,000원/QALY의 ICER 값을 보였다. 감소한 사망자 수를 효과로 정하고 모든 연령의 밀접 접촉자를 검사하는 경우 0-35세 치료 전략에서는 2040년 기준으로 비용이 55,000,000원 증가하고 사망자 수는 8.65명이 감소하여 감소한 사망자 한 명당 6,360,000원의 ICER 값을 얻었다. 0-65세 치료 전략에서는 비용이 20,117,000,000원 증가하고 사망자 수는 179.34명이 감소하여 ICER 값이 112,170,000가 되었다.
5. 민감도 분석 민감도 분석에서는, 최근 감염군에서 활동성 결핵으로 진행되는 비율 가 결과에 매우 민감하게 작용하는 모델 변수임을 알 수 있다. 다음으로는 감염율 (transmission rate) 와 평균 감염 기간의 역수에 해당하는 가 결과에 큰 영향을 주는 요소였다. 0-65세를 대상으로 치료하는 제어 전략에서 결핵으로 인한 사망 감소를 효과로 정의하는 경우는 결핵으로 인한 사망률 가 나 보다 중요한 역할을 하는 것을 확인하였다.
▢ 결론 이 연구에서는 국내 결핵 수리 모델을 이용한 모의 실험 결과, 가족 밀접 접촉자에서 잠복결핵 감염 진단과 치료가 결핵의 발생과 결핵으로 인한 사망을 줄이는 효과가 있음을 확인하였다. 잠복결핵 감염 치료 정책의 연령별 확대 정책의 비용-효과 근거 확인에서도, 0-35세 치료 전략 뿐 아니라 0-65세 치료 전략도 국내에서 비용-효과적인 정책으로 확인되었다. 주요어 결핵, 잠복결핵 감염, 가족 접촉자, 수리 모델, 비용-효과

▢ Background Treatment of latent tuberculosis infection (LTBI) is one of the important national tuberculosis control policies. According to the World Health Organization, the diagnosis and treatment of LTBI are essential for eradicating tuberculosis, along with prompt diagnosis and treatment of active tuberculosis. In Korea, national policies and support are expanding for screening and treatment of LTBI to eradicate tuberculosis. In the screening of close contact familial members, the past policy was limited to treat children with LTBI <5 years of age; then expanded to treat those <35 years of age from 2014; strengthened to treat those <65 years of age from 2017. However, the evidence for the expanded policy in Korea is rare, and cost-effectiveness analysis has not been conducted. The elderly have high incidence of tuberculosis and high risk of side effects from LTBI treatment. Considering the high prevalence of tuberculosis and national tuberculosis control strategies in Korea, it is necessary to establish evidence through cost-effectiveness analysis for the treatment of LTBI.
▢ Objective The purpose of this study was to develop a dynamic mathematical model of tuberculosis in Korea and to provide evidence for diagnosis and treatment policies of LTBI in close family contact through age-specific cost-effectiveness analysis. ▢ Methods This study was conducted in five stages as follows. First, the dynamic model for tuberculosis in Korea was designed considering the age structure and the contact matrix. Starting from the basic S (Susceptible) - E (Pre-infectious) - I (Infectious) - R (Recovered) model, the S-ES-EL-I-L-T structure was constructed. The model was then calibrated with national tuberculosis report data between 2011 and 2018, familial contact survey data, national LTBI cohort analysis data, and literature on tuberculosis epidemiology. Second, a simulation was performed using the stabilized model to predict the number of tuberculosis incidences and tuberculosis-related deaths according to the age extension policy for LTBI treatment in close familial contacts. In this case, the expanded treatment policy for each age group was compared among no treatment (control group), treatment of 0-35 years old group, 0-55 years old group, 0-65 years old group, and 0-70 years old group. Third, we used the Health Insurance Reviewers & Assessment Service (HIRA) data in 2020 and National Health Insurance Service (NHIS) data to measure the cost per capita in diagnosing and treating active tuberculosis and LTBI. The cost of diagnosis was calculated using the insurance fee data of HIRA, and the cost of treatment was calculated using the claim data of NHIS. Operational definitions of LTBI and active tuberculosis were used to analyze the total cost of medical care expenses. Fourth, the incremental cost-effectiveness ratio (ICER) index was calculated for cost-effectiveness analysis according to age group expansion policies for treatment of LTBI in familial contacts. The effects were defined as quality-adjusted life-year (QALY) and the number of averted deaths. The status-specific QALY was weighted based on the existing literature (active tuberculosis, 0.76; LTBI treatment status, 0.99; others, 1). As an intervention, four strategies were analyzed: (1) no treatment for LTBI (control), treatment of (2) 0-35 years old group, (3) 0-55 years old group, (4) 0-65 years old group, and (5) 0-70 years old group. Fifth, a sensitivity analysis was performed to find the effect of uncertain parameter values. The parameters were adjusted in the 5% range to calculate the cumulative ICER from 2019 to 2040. Two sets of sensitivity analyses were performed: diagnosis of LTBI among all close contact family members and treatment of LTBI for the 0-35 years old group and the 0-65 years old group. ▢ Results ● Based on the national tuberculosis report data and epidemiological data, a dynamic model was established to calculate the treatment effect of LTBI by age groups in family members. ● According to the model predicting the patients with pulmonary tuberculosis in 2040, treating LTBI in the 0-35 years old group was expected to reduce 1748 patients of active tuberculosis, and that in the 0-65 years old group was expected to reduce 5912 patients of tuberculosis compared to the control group. ● Between 2020 and 2040, it was cost-effective by showing KRW 120,000 /QALY for the 0-35 years old group treatment strategy and KRW 5,750,000/QALY for the 0-65 years old group treatment strategy.
1. Establishment of a dynamic mathematical model for pulmonary tuberculosis in Korea Based on the SEIR dynamic model for tuberculosis, the S-ES-EL-I-L-T model was established according to the research purpose. The parameters were determined by reviewing national tuberculosis report data between 2011 and 2018, family contact survey data, national LTBI cohort data, and epidemiological literature. 2. Simulation of pulmonary tuberculosis patients and the number of averted deaths through the mathematical model In predicting the number of tuberculosis patients, treatment strategies for the four groups were compared to the no treatment of LTBI strategy. The cumulative numbers of pulmonary tuberculosis patients between 2019 and 2040 were estimated to be reduced by 1748 patients, 5912 patients, and 7850 patients in the treatment strategy of 0-35 years old group, 0-65 years old group, and 0-70 years old group, respectively. The tuberculosis-related death was averted by 12 in the 0-35 years old group, 143 in the 0-65 years old group, and 256 in the 0-70 years old group, compared to the control strategy. 3. Analysis of per capita cost for diagnosis and treatment of tuberculosis Using the insurance fee data of the HIRA in 2020, we estimated the per capita cost of diagnosis for LTBI and active tuberculosis for children aged 0-4 and aged ≥5 years, respectively. The cost of diagnosing LTBI was KRW 36,090 for 0-4 years old, 72,650 KRW for ≥5 years old; the cost of diagnosing active tuberculosis was KRW 168,206 for 0-4 years old, and KRW 165,122 for ≥5 years old. The average cost of treatment for LTBI was KRW 180,499 per person when completed treatment, which showed slight changes according to age. The average cost of treatment for active tuberculosis was KRW 3,461,975 per person when completed treatment, which tended to rise as the age increases. 4. Cost-effectiveness analysis according to age-specific control strategies for LTBI in close family contact The cumulative medical cost in 2020-2040 was calculated by applying the cost of diagnosing and treating LTBI and active tuberculosis with an annual discount rate of 3%. In the case of screening for LTBI at all ages, the cost of diagnosing and treating LTBI increased by KRW 2,908,000,000 for the 0-35 years old group and by KRW 38,311,000,000 for the 0-65 years old group in 2040. The cost of treating active tuberculosis decreased by KRW 2,852,000,000 for the 0-35 years old group and KRW 18,193,000,000 for the 0-65 years old group. Therefore, the total medical cost increased by KRW 55,000,000 for the LTBI treatment strategy for the 0-35 years old group and KRW 20,117,000,000 for the 0-65 years old group. When ICER was measured with QALY as an effect, the cost increased by KRW 55,000,000 and QALY increased by 460 in the LTBI treatment strategy for the 0-35 years old group, corresponding to an ICER value of KRW 120,000/QALY. In the LTBI treatment strategy for the 0-65 years old group, the cost increased by KRW 20,117,000,000 and QALY increased by 3497, showing an ICER value of 5,750,000 won/QALY. In case of the number of averted deaths was set as the effect and close contacts of all ages were screened, the treatment strategy for the 0-35 years old group will increase the cost by KRW 55,000,000 and decrease the number of deaths by 8.65, resulting in an ICER value of KRW 6,360,000 for each averted death in 2040. In the treatment strategy for the 0-65 years old group, the cost increased by KRW 20,117,000,000 and the number of deaths decreased by 179.34, resulting in an ICER value of KRW 112,170,000 for each averted death. 5. Sensitivity Analysis In the sensitivity analysis, the progression rate of active tuberculosis in the recently infected group (q) was influential to the results. The transmission rate (β) and the reciprocal of the average infection duration (γ) also influenced the results. If the tuberculosis-related death was set as an effect, the death rate due to tuberculosis (d) played a more role than β or γ in the treatment strategy for the 0-65 years old group.
▢ Conclusion In this study, we conducted a simulation using a dynamic mathematical model of tuberculosis in Korea. The diagnosis and treatment of LTBI in close familial contact effectively reduced tuberculosis and tuberculosis-related death. In the policy of LTBI treatment, treating not only the 0-35 years old group but also the 0-65 years old group was found to be cost-effective.
