만성 기도질환 환자에 대한 핵심치료방법인 흡입용 기관지확장제 및 흡입용 스테로이드제 사용에 대한 현황을 파악하고 중요한 임상적 정보를 제공하기 위하여 1) 건강보험심사평가원 청구자료를 이용하여 국내 흡입용 기관지확장제 및 흡입용 스테로이드제의 처방 현황 및 약물사용 패턴을 파악하고, 약물사용에 따른 질병발생을 비교하였다. 2) 환자 대상 설문을 수행하여, 흡입용 약물치료 사용 후 건강보험심사평가원의 청구자료 분석으로 파악하기 힘든 배뇨장애, 입마름의 빈도, 우울/불안감 등의 임상적 변화를 파악하였다.
1. 사용 현황 파악
건강보험심사평가원의 건강보험청구자료를 이용한 현황 분석 결과, 2006년부터 2010년 까지 5년간 흡입용 기관지 확장제 및 흡입용 스테로이드제로 건강보험심사평가원에 청구된 명세서는 33,276,2336건, 환자수는 2,651,982명, 신환자수는 1,099,536명인 것으로 나타났다. 명세서에서는 0~9세가 처방되는 빈도가 가장 많았으나, 환자, 신환자에서는 60대~70대가 가장 많이 처방받는 것으로 나타났다. 환자, 신환자 모두 천식으로 청구된 경우가 가장 많았으며, 처방된 약물의 약효군을 살펴보면 환자에서는 지속성 베타2 항진제/ 흡입용 스테로이드제(LABA/ICS)가 가장 많이 처방되었고, 신환자에서도 지속성 베타2 항진제/흡입용 스테로이드제(LABA/ICS)가 가장 높은 빈도를 나타냈으며 연도별로도 비슷한양상을 보였다.
흡입제 투약순응도를 살펴본 결과, 전체 추적관찰기간 2년에서 70%(511일)이상 처방받은 환자는 질환별로 기관지확장증은 4.30%, 결핵후유증은 6.22%, COPD는 4.07%, 천식은 1.63%였으며, 50%(365일)이상을 처방받은 환자는 기관지확장증은 9.74%, 결핵후유증은 11.79%, COPD는 8.64%, 천식은 4.27%로 천식의 투약순웅도가 가장 낮았다. 또한, 2년의 추적관찰기간 중에 천식환자의 79.95%가 약물을 변경하여 4개 질환중에는 가장 높았고, 70%(511일)이상 처방받은 그룹에서도 천식환자가 47.13%로 가장 높았다. 하지만, 50%(365일)이상 처방받은 그룹에서는 기관지확장증 환자가 62.47%로 가장 높았다.
2. 비교효과연구
흡입제 사용과 폐암 또는 후두암 발생과의 관련성을 평가하고자 코호트 내 환자-대조군 연구를 시행하였다. 연구 결과, ICS 비사용자에 비해 ICS 사용자의 폐암 발생 위험이 0.79배 낮게 나타났으며, ICS 누적사용량에 따른 폐암 발생 위험은 용량-반응 관계가 있었으며, 누적사용량이 증가함에 따라 폐암 발생 위험이 감소함을 알 수 있었고, 하위분석 결과 및 민감도 분석 결과도 유사하였다. 후두암 발생 위험도 ICS 사용군에서 감소하는 것으로 나타났으나, 통계적으로 유의하지 않았다. 흡입제 사용과 결핵 발생과의 관련성을 평가하고자 동일한 연구설계로 코호트 내 환자- 대조군 연구를 수행한 결과, ICS 비사용자에 비해 ICS 사용자가 결핵 발생 위험이 1.23배로 높게 나타났으며, ICS 누적사용량에 따른 결핵 발생 위험의 용량-반응 관계가 있었으며, 누적사용량이 증가함에 따라 결핵 발생 위험이 증가함을 알 수 있었으며, 이런 결과는 하위분석 및 민감도 분석 결과도 유사하였으냐, OCS 사용 유무에 따라 ICS와 결핵 발생 위험도는 차이가 있었다.
또한, 환자-교차설계로 흡입제 사용과 폐렴으로 인한 입원 및 응급실 방문의 위험도를 알아 본 결과, index date 이전 60-30일 사이의 대조기간에 ICS 단독사용은 폐렴으로 인한 입원 또는 응급실 방문을 위험을 1.73배 증가시켰으나, ICS와 LABA의 병용은 오히려 0.63배로 위험을 감소시켰으며, index date 이전 120-90일, 210-180일, 390-360일 사이의 대조기간에서도 유사한 결과를 보였고, 다양한 하위그룹분석와 민감도분석에서도 유사하였다.
3. 환자 대상 설문
흡입제 신환자를 대상으로 한 환자조사 결과, 90명의 환자가 분석 대상이 되었다. 흡입제 사용 전후 배뇨장애의 변화는 었었다. 입마름은 LAMA 사용군에서 사용 전 5.27에서 사용 12주 경과 시점에서 10.46으로 증가하였지만, ICS/LABA 사용군은 차이가 없었다. 흡입제 사용 12주 후 우울 지수는 9.61에서 10.85으로 증가하였고, 특히 ICS/LABA군에서 Anxiety 지수는 4.28에서 4.22로 감소하였고, LAMA군에서 3.42에서 4.48로 증가하였지만 두 군간의 변화량에 통계적으로 유의한 차이는 없었다.
또한, 주변모형 분석 결과 ICS/LABA 사용군과 LAMA 사용군에서 연령, 성별, 기저 FEV1, BMI, 흡연량을 보정한 후 시간에 따른 IPSS, HADS 각 도구 점수의 변화는 통계적으로 유의한 차이가 없었다. 하지만 입마름의 경우 ICS/LABA군에서는 시간에 따라 통계적으로 유의한 차이를 보이지 않았지만, LAMA군에서는 한달에 1.62점 증가하는 것으로나타났다.
본 연구는 건강보험심사평가원 청구자료를 사용하였으므로, 흡입제 사용 환자에게 중요한 변수인 흡연여부 및 폐기능검사 결과 등 다양한 임상적 변수를 갖고 있지 못하여 분석의 한계가 있어 환자-대조군 연구설계에서는 COPD 상병 등으로 매칭하여 이러한 한계를 극복하고자 하였다. 흡입용 기관지확장제 및 스테로이드는 천식, COPD 등 만성기도질환을 치료에 있어 기본이 되는 중요한 약제임에도 불구하고, 국내 약물사용현황 및 사용에 따른 성과비교가 미흡한 상황으로, 본 연구결과가 의료전문가 및 정책결정자에게 중요한 임상적 정보를 제공할것으로 사료된다.
To understand the current status of the use of inhaled bronchodilator and inhaled steroids, which are the core treatment for patients with chronic airway diseases, and to provide important clinical information, the study was conducted as follows: 1) the current status of the prescription for and the use of inhaled bronchodilator and inhaled steroids in the country was examined using the Health Insurance Review & Assessment Service's claims data, and the incidence of diseases depending on the drug use was compared. 2) Clinical changes such as disturbed urination, frequency of dry mouth, and depression/anxiety after the use of inhaled drug treatment, which
are difficult to find out from the analysis of the Health Insurance Review & Assessment Service's claims data, were examined by conducting the survey on patients.
1. Review of the current use
The analysis for current use by the Health Insurance Review & Assessment Service's health insurance claims data showed that the number of cases claimed between 2006 and 2010 for inhaled bronchodilator and inhaled steroids at the Health Insurance Review & Assessment Service was 33,276,366, the number of patients was 2,651,982, and the number of new patients was 1,099,536. In the claimed statements, the frequency for the prescription for 0~9 years patients was the highest, but the 60s~70s were mostly prescribed in patients and new patients. Asthma was the most frequent cause for prescription in both patients and new patients. For drug
effect groups of prescribed drugs, long-acting beta-2 agonist/inhaled steroids(LABA/ICS) was the mostly prescribed in patients and also in new patients, with similar tendency by year.
For medication adherence of inhalants, bronchiectasis was 4.30%, sequela of tuberculosis 6.22%, COPD 4.07%, and asthma 1.63% in patients prescribed for over 70%(511 days); bronchiectasis 9.74%, sequela of tuberculosis 11.79%, COPD 8.64%, and asthma 4.27% in patients prescribed for over 50%(365 days), showing the lowest medication adherence in asthma, during the 2 years of follow-up period. Also, 79.95% of asthma patients changed their medication, which was the highest among 4 diseases, and also asthma patients were the highest as 47.13% in patients prescribed
for over 70%(511 days) during the 2 years of follow-up observation. However, bronchiectasis patients were the highest as 62.47% in patients prescribed for over 50%(365 days).
2. Comparative effectiveness study
The case-control study within the cohort was performed to evaluate the relationship between the inhalant use and the incidence of lung cancer or laryngeal cancer. The results showed that the risk for lung cancer incidence in ICS users was 0.79 times lower compared to ICS non-users, and the risk for lung cancer incidence by the cumulative use of ICS had a dose-response relationship, and the risk for lung cancer incidence was decreased as the cumulative use was increased, with similar results in the subgroup analysis and the sensitivity analysis. The risk for laryngeal cancer incidence was also decreased in the ICS user group, but not statistically significant.
The results of the patient-control study for the evaluation of the relationship between the inhalant use and tuberculosis incidence within the cohort with the same study design showed that the risk for tuberculosis incidence was 1.23 times higher in ICS users compared to ICS non-users, and the risk for tuberculosis incidence by the cumulative use of ICS had a dose-response relationship, and the risk for tuberculosis incidence was increased as the cumulative use was increased, with similar results in the subgroup analysis and the sensitivity analysis but with some differences in ICS and the risk of tuberculosis incidence depending on the use of OCS.
In addition, the risk for hospitalization and emergency room visit due to the inhalant use and pneumonia in the case-crossover design showed that the use of ICS only during the control period between 60-30 days before the index date increased the risk for hospitalization and emergency room visit due to pneumonia by 1.73 times, but the combined use of ICS and LABA was rather decreased the risk by 0.63 times, with similar results during the control period between 390-360 days, 210-180 days, and 120-90 days before the index date, with similar results in various subgroup
analyses and the sensitivity analysis.
3. Patients Survey
In the survey of new patients using inhalants, 90 patients were involved in the analysis. No urination disturbance was observed before and after the use of inhalants. The presence of dry mouth was increased in LAMA group from 5.27 before use to 10.46 after the 12-week use, but not changed in ICS/LABA group. The depression index was increased from 9.61 to 10.85 after the 12-week use of inhalants, and particularly the anxiety index was decreased from 4.28 to 4.22 in ICS/LABA and increased from 3.42 to 4.48 in LAMA group, but no significant difference was observed statistically in the changes between two groups.
Also, in the marginal model analysis, no significant differences were observed statistically in the changes of IPSS and HADS scores by time between ICS/LABA group and LAMA group after being corrected for age, gender, basal FEV1, BMI, and smoking. However, in case of dry mouth, no significant difference was observed statistically by time in ICS/LABA group, but increased 1.62 points a month in LAMA group.
This study used the Health Insurance Review & Assessment Service's claims data and thus had limitations in the analysis due to the lack of various clinical variables such as the presence of smoking and the results of pulmonary function tests, which are important variables in patients who use inhalant; in the case-control study design, matched for COPD (J41–J44) to overcome such limitations.
Although inhaled bronchodilator and inhaled steroids are important and core drugs in the treatment of chronic airway diseases such as asthma and COPD, the current status of domestic drug use and the comparison of performance after drug use are not sufficiently available. Thus it is considered that the results of this study provide important clinical information to medical professionals and policy decision-makers.
