연구목적
수면호흡장애 진단에서의 수면다원검사의 임상적 근거를 확인하고자 다음과 같은 세부 연구내용을 설정하였다. 첫째, 체계적 문헌고찰을 통하여 수면호흡장애 진단 시 수면다원검사의 진단정확도에 관련한 근거를 확인한다. 둘째, 의학교과서 및 관련 임상가이드라인 검토를 통하여 수면호흡장애 진단 시 수면다원검사의 의학적 근거를 확인한다.
연구방법
본 연구에서는 1차적으로 체계적 문헌고찰을 통해 수면다원검사의 과학적 근거(Scientific evidence)가 있는지를 검토하고, 근거를 찾을 수 없는 경우 의학적 표준(Medical standard)을 확인하는 절차로 연구를 진행하였다.
첫째, 체계적 문헌고찰을 통하여 수면호흡장애에서의 수면다원검사의 진단정확도를 평가하기 위해 PubMed(998건), KoreaMed(115건)를 검색하였으며, 이 중에서 수면호흡장애 진단 시 수면다원검사(Attended full night polysomnograhy)의 정확성에 관한 문헌이 있는지 검토하였다.
둘째, 의학적 표준(Medical standard)을 검토하기 위해 임상전문가(대한수면의학회, 대한수면연구학회 소속) 2인을 대상으로 자문회의를 통하여 검토할 교과서와 임상가이드라인을 선정하였다. 검토한 2권의 의학교과서는 Principles & Practice of Sleep Medicine(5th ed.)과 Sleep disorders medicine: basic science, technical considerations, and clinical aspects(3rd ed.)이었고, 임상가이드라인은 미국수면의학회에서 발간한 3편을 검토하였다(Practice Parameters for the Indications for Polysomnography and Related Procedures: An Update for 2005;Practice parameters for the respiratory indications for polysomnography in children; Clinical guideline for the evaluation, management and long-term care of obstructive sleep apnea in adults).
연구결과
수면호흡장애 진단에 있어서의 수면다원검사의 진단정확도에 대한 체계적 문헌고찰 결과 본 연구에서 설정한 선정배제기준을 충족하는 원저를 찾을 수 없었다. 수면다원검사가 의학수면호흡장애 진단을 위한 수면다원검사의 평가적으로 표준이 되는 검사법(gold-standard)으로 제시되고 다른 검사법(분할야간수면다원검사, 휴대용수면다원 검사 등)의 민감도/특이도 등을 보고한 연구가 다수였다. 그 밖에 수면다원검사의 결과 일치도, 검사 후 부작용, 임상적 판단의 중요성 등을 보고하고 있었다.
즉 체계적 문헌고찰을 통해 수면다원검사의 진단정확도에 대한 근거를 찾을 수 없어 본 연구에서는 의학적 표준(Medical standard)을 확인하기 위하여 자문 회의를 통해 선정된 교과서 및 임상가이드라인을 검토하였다. 검토한 모든 교과서 및 임상가이드라인에서는 수면호흡장애 진단 및 중증도 평가 시 수면검사실에서의 시행하는 수면다원검사가 가장 표준
적인 검사방법임을 명시하고 있다. 수면호흡장애를 평가하기 위해서는 수면다원검사 시 뇌파(EEG), 안전도(EOG), 턱 근전도(Chin EMG), 호흡기류(Airflow), 혈중산소포화도(Blood oxygen saturation)등의 지표를 확인해야 한다. 하지만 수면검사실에서 시행하는 수면다원검사의 경우에도 측정하는 밤 간의 차이(Night-to-night variability), 검사 - 재검사의 신뢰도(Test-retest reliability), 평가자간 및 평가자내 인식오류(Intra-rater, inter-rater event recognition errors) 등으로 인해 환자를 정확하게 진단/분류하지 못할 수 있다는 점이 지적되고 있다. 또한 수면다원검사의 기술정확도, 신뢰도가 보장되기 위
해서는 검사가 이루어지는 수면검사실 환경, 수면다원검사의 표준화, 전문적인 수면기사의 지속적인 감독 등의 조건이 충족되어야 함을 명시하고 있다. 특히 어린이의 경우 수면검사실에서 편안할 수 있도록 보다 주의 깊은 고려가 필요하다고 강조되고 있다.
수면다원검사가 수면호흡장애 진단 시 의학적 표준이 되는 검사법이라는 것은 본 연구에서 검토한 교과서 및 가이드라인에서 공통적으로 제시하고 있는 내용이었다. 하지만 수면다원검사를 수행하기 위해서는 많은 시간과 비용이 소요된다는 제한점이 있어 수면다원검사를 변경하는 방법이 제안되기도 하였다. 하룻밤에 수면호흡장애의 진단과 치료를 위한 CPAP
의 적정압력을 찾는 것을 모두 수행하는 분할야간검사(Split night)가 표준적인 수면다원검사의 대안이 될 수 있다고 하였으나 밤이 아니라 낮 시간 동안 검사를 시행하는 방법(Daytime nap)은 여러 한계점으로 추천되지 않았다.
앞서 언급하였듯이 수면다원검사는 수행 시 많은 시간과 비용이 발생하는 방법으로 사전검사를 통하여 수면호흡장애를 가지고 있을 확률이 높은 환자군을 선별한 후 수면다원검사를 시행하는 방안이 제시되기도 하였다. 또한 수면다원검사와 함께 과거력, 신체검사와 같은 사전검사 결과를 모두 함께 고려한 포괄적 수면검사를 통하여 관련 전문의가 수면호흡
장애를 진단해야 함을 언급하고 있다.
결론
본 연구를 통하여 검토한 교과서 및 임상가이드라인에서는 수면호흡장애 진단 및 중증도 수면호흡장애 진단을 위한 수면다원검사의 평가평가 시 수면검사실에서 수면기사의 감독 하에 시행하는 수면다원검사를 가장 표준적인 검사방법(gold standard)으로 명시하고 있음을 확인하였다. 수면다원검사의 기술정확도, 신뢰도가 보장되기 위해서는 검사가 이루어지는 수면검사실 환경, 수면다원검사의 표준화, 전문적인 수면기사의 지속적인 감독 등의 조건이 충족되어야 한다. 수면다원검사가 표준적인 검사법(gold standard)이지만 단독으로 사용되기 보다는 과거력, 신체검사와 같은 사전검사 결과를 함께 고려한 포괄적 수면검사를 통해 관련 전문의가 수면호흡장애를 진단하는 것이 적절하다.
Objective
To verify clinical evidence of polysomnography in the diagnosis of sleep related breathing disorder (SRBD), the following objectives were established for this study. Firstly, evidence of diagnostic accuracy of polysomnography for the diagnosis of SRBD was to be confirmed through systematic review.
Secondly, medical text books and relevant clinical guidelines were reviewed to confirm evidence of polysomnography in the diagnosis of SRBD.
Method
In this study, systematic review was performed primarily to investigate the existence of scientific evidence. If scientific evidence was not found, the study was to proceed to confirm medical standards. Firstly, as for the systematic review, PubMed (998 articles) and KoreaMed (115 articles) were searched to identify those on the accuracy of polysomnography (Attended full night polysomnograhy) for the diagnosis of SRBD. Secondly, to review medical standards for diagnosis of SRBD, text books and clinical guidelines were selected through a consultation meeting participated by two clinical
experts (Affiliated with the Korean Academy of Sleep Medicine and the Korean Sleep Research Society). Principles & Practice of Sleep Medicine (5th ed.) and Sleep disorders medicine: basic science, technical considerations, and
clinical aspects (3rd ed.) and three clinical guidelines published by the US Academy of Sleep Medicine (Practice Parameters for the Indications for Polysomnography and Related Procedures: An Update for 2005; Practice
parameters for the respiratory indications for polysomnography in children; Clinical guideline for the evaluation, management and long-term care of obstructive sleep apnea in adults) were reviewed.
Result
As a result of the systematic review regarding accuracy of polysomnography for the diagnosis of SRBD, no original article could be selected that met the inclusion/exclusion criteria established in this study.
Majority of the literature suggested polysomnography as a gold standard and reported other tests (Including split-night polysomnography and portable polysomnography) for sensitivity and/or specificity tests. In addition, reports pertaining to consistency in polysomnography results, side effects of polysomnography, and significance of clinical judgment could be found.
Since the evidence of diagnostic accuracy of polysomnography could not be found in the systematic review, we reviewed medical text books and clinical guidelines to confirm medical standard, all of which specified that polysomnography performed in a laboratory is the most standard test method for the diagnosis of SRBD and assessment of its severity.
To evaluate SRBD, polysomnographic indices such as electroencephalogram (EEG), electro-oculogram (EOG), chin electromyogram (Chin EMG), airflow, and blood oxygen saturation should be checked. It is also noted that even if
polysomnography is performed in a laboratory, there remains the possibility of inaccurate diagnosis/classification of patients due to night-to-night variability, test-retest reliability, and intra-rater, inter-rater event recognition errors. In addition, to ensure technical accuracy and reliability of polysomnography, it is specified that relevant conditions, including
atmosphere of laboratory, standardized method of polysomnography, and continuous supervision by a professional polysomnographic technician, should be satisfactory. It is emphasized that more consideration should be given for children to ensure that they can feel comfortable in the laboratory.
Polysomnography has been commonly suggested as a medical standard for the diagnosis of SRBD in the text books and guidelines reviewed in this study. However, due to limitations of polysomnography including requirement
of extensive time and cost, some of the references also suggested modifications to polysomnography. These specify that split night test that finds an appropriate continuous positive airway pressure overnight for diagnosis and treatment of SRBD can be an alternative to standard polysomnography. However, daytime nap method in which the test is performed during the day time instead of the night time was not recommended due to various limitations. As mentioned earlier, polysomnography requires extensive time and cost. Due to this reason, screening patients with a higher probability of SRBD through a preliminary test and then performing polysomnography in these patients was also suggested. In addition, it is noted that a comprehensive sleep test considering preliminary test results such as history and physical examination, on top of polysomnography should be performed for the diagnosis of SRBD by a physician specializing in sleep disorders.
Conclusion
It was confirmed that the text books and clinical guidelines reviewed in this study specified that polysomnography performed in a laboratory under the supervision of a polysomnographic technician was the gold standard for the diagnosis of SRBD and assessment its severity. To ensure technical accuracy and reliability of polysomnography, relevant conditions, including
atmosphere of the laboratory, standardization of polysomnography, and continuous supervision by a professional polysomnographic technician, should be satisfactory. Although polysomnography is the gold standard, it will be appropriate to perform a comprehensive sleep test considering preliminary test results such as history and physical examination, on top of polysomnography, for the diagnosis of SRBD by a physician specializing in sleep disorders, rather than relying on polysomnography alone.
