의료기술평가(Health Technology Assessment; HTA)는 많은 선진국에서 건강보험재정의 합리적인 분배를 위하여 사용되고 있는 연구방법이다. 오랜 기간 수행을 거친 전문인들이 공급하는 의료의 특성상 보험재정 효율화를 위한 여러 의사결정들은 전문인 공급자들의 지지와 동의없이 그 효과를 가지기 힘들다. 영국에서는 의사들이 익숙한 근거중심의료(Evidence Based Medicine; EBM)와 의사결정을 접목하는 한편 경제성평가를 포함한 사회적 가치평가과정을 NICE Appraisal이란 이름으로 제도화하여 파급효과가 큰 급여결정 대상을 선별하는데 공급자들이 동의하고 지지할 수 있도록 의료기술평가를 시행하고 있다. 미국에서는 오바마 대통령 당선 이후 의료보험의 합리화를 위해 비교효과연구(Comparative Effectiveness Research; CER)란 이름의 HTA와 유사한 형태의 연구들을 장려하고 있다. CER은 HTA 경험이 많은 유럽의 입장에서는 HTA의 미국식 버전으로 인식되고 있고 영국을 포함한 다른 많은 유럽 국가들에서도 비슷한 연구들을 HTA란 이름으로 수행하고 있다.
본 연구에서는 의료기술평가의 선진국이자 합리적인 의료체계를 갖추고 있다는 것으로 알려진 영국의 사례들에 대하여 영국의 저명한 연자들이 참여하는 심포지엄을 통해 영국의 경험들을 배우는 기회를 통하여 한국에서도 의료기술평가를 통한 합리적인 보험정책 의사결정방법을 모색하고자 하였다. 크게 England와 Wales를 중심으로 한 NICE의 의사결정과정과 Scotland의 의약품 연합(Scottish Medicines Consortium, SMC)에 의해 이루어지는 의사결정과정들로 나뉘어 연제가 발표되었으며 두 곳 모두 appraisal이라고 하는 영국의 독특한 의사결정형태를 가지고 있다. 임상의, 약사, 보건경제학자, 제약업계와 일반시민 대표 등이 모여 의료기술평가 결과와 그 의료기술의 사회적 가치를 합의해가는 과정으로 단순히 의사결정을 위한 위원회와는 겉모습은 같지만 참가주체나 논의방식이 다르다. 한국과 같이 학회나 단체의 추천을 받은 대표가 참여하는 것이 아니라 합리성을 갖춘 특정 지역의 시민을 개인자격으로 참여시키고 논의과정을 투명하게 공개하고(transparency) 이해하기 쉽게 설명함(accountability)으로써 합의된 의사결정의 정당성을 확보해 나가는 점이 특징이라 할 수 있다.
해외연자들이 강조한 부분은 근거에 기반한 의료시스템을 구축하기 위해 일반적으로 혼동되기 쉬운 assessment와 appraisal 및 의사결정 과정의 개념 차이를 명확히 해야 한다는 것이었다. 한국에서는 두 단어가 거의 구분되지 않은 채 “평가”로 번역되어 사용되고 있지만 영국에서 assessment란 기술평가를 의미하며, 특정 기술의 도입에 의해 기대되는 이득(benefit), 필요성, 영향력, 비용에 대한 근거자료를 만드는 과정의 하나로 의료기술평가 전문가들에 의해 수행된다. 반면 appraisal은 assessment에 의해 제공된 근거자료를 평가하는 별도의 과정으로 급여결정 등 보건의료 정책결정의 기반이 된다. 공식적인 결정은 어떠한 의료기술이나 치료법을 제공하는 것이 가장 효율적인지에 대한 판단으로 이러한 결정은 의료 체계 내에서 정책 결정권자가 내린다. 이를 한국의 현 시스템에 적용해 보자면 한국보건의료연구원이 assessment를 통하여 특정기술 도입에 관한 근거자료들을 만들어 내고 이런 자료들을 바탕으로 건강보험심사평가원이 운영하고 있는 약제, 치료재료, 행위 전문평가위원회들에서 appraisal을 거쳐 급여가 결정되는 형태가 이상적이라 할 수 있다. 미국의 CER과 영국의 HTA의 가장 큰 차이는 아마도 CER에는 appraisal이 명확히 없고 assessment의 결과를 전문인들인 공급자와 의사결정자들에게 제공하고 확산시키겠다는 수준에 머물고 있는 점이라 할 수 있다.
NECA hosted an international symposium with a theme of "Applying Health Technology Assessment on Decision-Making." The symposium brought together outstanding HTA experts of the United Kingdom, one of the leading HTA countries with the rational health care system. By learning a lesson from the UK experiences, we sought for a Korean style healthcare decision-making tool using HTA.
During the symposium, it was presented that there are two decision-making processes in the UK: one is a decision-making process by National Institute for Health and Care Excellence (NICE) in England and Wales and the other by Scottish Medicines Consortium (SMC) in Scotland. Besides an assessment step, both have an "appraisal" step within the decision-making process.
The appraisal is to reach an agreement on the result of the health technology assessment and its social value. Similar to general decision-making committees, the appraisal committee of the UK consists of clinicians, pharmacists, health economists, pharmaceutical industries and general public. The UK appraisal committee, however, is distinguished from other committees in terms of how to derive participants or how to arrive at the agreement. The appraisal committee members in Korea, for instance, are representatives recommended by relevant organizations and societies to guarantee procedural justification.
On the other hand, any rational expert not necessarily recommended by the professional society can participate in the UK appraisal committee. The committee ensures transparency and furthermore, procedural justification by opening and accounting for discussion to public.
To build an evidence based healthcare system, the HTA experts emphasized the importance of differentiating the commonly confused terms, “assessment”, “appraisal” and “the decision-making process". Currently in Korea, all three terms are commonly translated into “assessment. In U.K, “assessment” is commonly known as the health technology assessment. This further serves as a basis to produce evidence on the expected benefits, needs, impact and cost. On the other hand, “appraisal” is a term that indicates a separate process of assessing the evidence generated by the “assessment” process.
This further contributes to the reimbursement decision-making process and influences healthcare policies. Final decisions on whether introduction of a new health technology would be effective are made by the policy makers. Synthesizing evidence of health technology through assessment by NECA and determining coverage through appraisal based on NECA's evidence by committees in Health Insurance Review & Assessment Service (HIRA) would be ideal to apply the current Korean system.
The fact that CER has no clear appraisal process and focuses on provision and dissemination of assessment's results to professionals who are suppliers and decision-makers is the biggest difference between CER in U.S. and HTA in U.K.
