▢ 연구 배경
2009년 설립 이후 한국보건의료연구원은 올해로 설립 7주년을 맞이하였다. 그간 한국보건의료연구원은 국가 의료자원의 효율적 활용 및 국민 건강 향상을 위해 과학적 근거를 제공하고자 끊임없는 노력을 기울여 왔다. 또한 개원 기념 연례회의를 통하여 연구결과의 확산 및 보건의료 분야의 이슈 제공, 기관 홍보에 힘써왔다. 2016년 3월 24일 개최된 개원 7주년 기념 연례회의에서는 미래지향적 의료기술평가 연구 설계 방안을 모색하고자 최근 보건의료분야에서 각광받고 있는 정밀의학과 의료기술평가와의 관계를 살펴보았으며, 국가주도형 임상연구의 필요성과 발전방안을 모색함으로써 전문가들의 의견 교환의 장을 마련하고자 하였다.
▢ 연구 목적
본 연구를 통하여,
첫째, 한국보건의료연구원에서 행해지는 연구 사업을 대국민, 전문가 및 유관기관에 확산하고, 의료기술평가 및 신의료기술평가에 대한 정보를 제공하고 필요성을 강조하는 자리를 마련함으로써 한국보건의료연구원의 중요성 및 미래전략에 대해 알리고자 한다. 둘째, 보건의료 분야의 최근 이슈에 대한 아젠다를 도출하여 전문가들의 의견 교환의 장을 마련함으로써 미래지향적 의료기술평가 연구 방향을 모색하고자 한다. 셋째, 미래 보건의료 환경에 적응할 수 있는 국가주도형 임상연구의 발전방향을 모색하고자 한다. 넷째, 환자의 가치를 반영하는 연구를 수행하여 신뢰받는 한국보건의료연구원으로 거듭나기 위한 구체적인 실행 계획(action plan)을 모색함으로써 한국보건의료연구원이 국민건강 증진과 보건의료 발전에 기여하기 위한 방안을 모색하고자 한다.
▢ 연구 방법
Ⅰ. NECA 연례 학술회의 기획
2016년 1월~2월에는 행사를 위한 준비 작업을 하였다. 준비 과정에서는 기존에 개최되었던 NECA 행사 및 국제 심포지엄 사례들을 참고하여 개선방안을 반영하고자 하였다. 또한 2016년부터는 ‘연구 성과에 대해 발표하고 토론’ 하는 의미를 담고자 ‘연례회의’ 대신 ‘연례 학술회의’ 라는 명칭을 사용하였다. 연례 학술회의 개최 이후에는 결과를 검토 및 고찰하여 2017년도 연례회의 준비에 반영하였다.
1. NECA 연례 학술회의 아젠다 선정 및 프로그램 구성
2016년 NECA 연례 학술회의를 기획하기 위하여 첫째, 과거 NECA 연례회의의 개선점을 도출하여 미비했던 점을 보완하고자 하였으며, 둘째, 보건의료 근거창출을 선도하는 전문기관으로써 보건의료 관련 이슈를 검토하여 주제를 도출하였고, 셋째, 국외 유사기관의 Annual Conference 사례를 검토하여 몇 가지 주제 후보를 바탕으로 1차 기획안을 마련하였다. 주제 후보에 대하여 팀장 이상 급으로 구성된 운영 회의에서 최종 주제를 ‘국가주도형 보건의료 근거창출 미래전략’으로 선정하였다.
2. 발표연자 및 토론자 구성
연례 학술회의 기획 및 프로그램이 확정된 이후에는 각 Plenary에 부합하는 발표 연자 및 토론 후보자를 구성하였다. 전화 혹은 이메일을 통하여 후보자들을 섭외하였다.
3. 제작물 제작 및 홍보 활동
유관기관 각 담당자에게 연락을 취하여 홍보활동을 하였다. 주요 참석자를 섭외하기 위해 리스트를 마련하여 초청장 및 홍보물을 온·오프라인으로 발송하였고, 그밖에 주요 기관 및 관계자, 학교에 행사 포스터를 제작 및 배포하였다.
또한 연구원 홈페이지를 통해 NECA 연례 학술회의 개최를 알리고 참석자 사전 신청을 진행하였으며, 보도자료를 배포하여 NECA 연례 학술회의 프로그램 및 주요 내용을 배포하였다.
Ⅱ. 연례 학술회의 개최
NECA 연례 학술회의는 오전 9시 30분부터 오후 5시 40분까지 Plenary1, 2, 3으로 나누어 진행되었다. 사전등록과 현장등록을 통해 참석자를 받았으며, 참석자는 총 285명 이었다.
Ⅲ. 설문조사 실시
행사 후 개선해야 할 점을 차년도 연례 학술회의에 반영하기 위하여 만족도 조사를 실시하였다.
Ⅳ. 후속조치
연례회의 후 유관기관 및 보건의료계 전문가, 정책고객들에게 성원에 대한 감사 및 적극적인 의견 수용, 확산에 최선을 다하겠다는 내용으로 후속 메일을 송부함으로써, 연대감을 형성하여 네트워크 구축이 될 수 있도록 하였으며, 학술회의 행사 영상자료를 Plenary별로 편집하여 연구원 홈페이지 및 블로그에 공개하였다.
▢ 연구 결과
설립 7주년을 맞이한 NECA는 2016년 3월 24일에 서울대학교치과병원 8층 대강당에서 ‘국가주도형 보건의료 근거창출 미래전략’을 주제로 제4회 NECA 연례 학술회의를 개최하였다. 이번 학술행사는 미래지향적 의료기술평가 연구 설계 방안을 모색하고자 최근 보건의료분야에서 각광받고 있는 정밀의학과 의료기술평가와의 관계를 살펴보고, 국가주도형 임상연구의 필요성과 발전방안을 모색함으로써 전문가들의 의견 교환의 장을 마련하고자 기획되었다.
행사는 Plenary 1, 2, 3으로 나뉘어 진행되었으며, Plenary 1에서는 개인의 유전체 및 진료정보를 고려한 맞춤의료와 건강, 생활환경, 습관 정보에 기반을 둔 사전적 건강관리가 포함된 맞춤형 예측 의료서비스인 ‘정밀의학’의 발전방향에 대해 모색하였다. ‘빅데이터 시대의 정밀의학’에 대해 정밀의학의 기본개념을 검토하고, 정밀의학을 실제 진료현장에 적용할 수 있도록 근거기반의학을 통한 과학적 근거제시의 필요성이 강조되었다. 또한. ‘정밀의학과 보건의료산업’에 대해 개인의 진료내역, 유전자정보, 생활습관 등 정밀의학 관련 다양한 데이터의 체계적 관리를 지원하는 국외 기업사례 및 환자의 유전체 분석을 통해 질병의 원인을 분석하여 임상적인 호전을 이끌어낸 연구사례가 소개되었다.
Plenary2는 ‘근거기반 보건의료 의사결정을 위한 임상연구의 발전 방향’을 주제로 국가지원 임상연구 결과를 근거로 한 급여기준 변경으로 의료비가 절감된 사례를 소개하며 임상연구 활성화의 중요성, 국가지원 2형 당뇨병 임상연구센터의 연구성과 소개와 함께 환자등록자료 구축을 통한 임상연구 활성화가 강조되었다. 한편 NECA의 국민건강임상연구사업은 고위험군 대상 공공보건의료 관련 중재, 비교효과연구, 임상연구 데이터 통합관리체계 구축 등을 추진함으로써 적정 의료서비스 제공 및 국민의료비 절감에 기여하고자 추진되고 있다는 점이 발표되었다.
Plenary3의 주제는 ‘NECA 3.0 지평을 넓히다’로서, 신의료기술평가가 국민의 의료서비스 보장과 의료기술개발 및 산업발전이라는 정책목표를 균형 있게 추구해 온 제도개선 사례, 근거기반 질병예방서비스 체계 구축에 NECA가 정책적으로 기여한 연구사례가 소개되었다. 한편, 국민속으로 다가서는 NECA가 되기 위하여 환자 가치 반영의 중요성과 환자참여 방안 등 앞으로 나아가야 할 방향을 제시하였다.
▢ 결론 및 정책적 제언
2016년 연례 학술회의를 통해 도출된 ‘국가주도형 보건의료 근거창출 미래전략’을 다음과 같이 네 가지로 요약할 수 있다.
첫째, NECA는 정밀의학과 같은 보건의료계의 새로운 동향을 주시하고, 근거기반에 기반한 정책적 의사결정 지원을 목표로 설립된 NECA의 기존 역량과 설립 가치의 교차하는 지점을 찾아내어야 할 필요가 있다. 이를 위해 의료기술평가 전문기관으로서 전문성을 개척하고 역량을 키워나가야 할 것이다.
둘째, 국민건강임상연구는 문제해결형 임상연구를 표방하고 있으며, 국내근거의 창출에 기여도가 높아 국가 정책에 실질적으로 활용될 수 있다. 따라서 국민건강임상연구사업을 통해 임상연구에 대한 국가 수준의 관심과 투자를 유도하기 위한 타당성 확보가 필요하며, 법과 제도로서 사업추진의 안정성을 확보하기 위한 노력이 필요하다.
셋째, 근거를 필요로 하는 외부의 변화에 민감한 대응이 필요하다. 근거기반 보건의료에서 정책적 의사결정을 담당하는 신의료기술평가제도는 이미 정책적 변화에 대응하여 제도 개선, 소통 증대 등의 민첩한 활동을 하고 있다. 그리고 근거기반 의사결정방법론은 공중보건 영역에서도 최근 수요가 높아지고 있고, NECA 역시 근거평가센터로서 연구활동을 통해 이를 지원해왔다. 이는 의료기술평가의 영역으로 그동안 주 활동분야였던 의학분야로 한정하지 않고, 궁극적으로 NECA의 설립목적에 맞게 역량을 강화해 가는 과정으로 볼 수 있다.
넷째, 근거기반 의사결정을 필요로 하는 보건의료 정책을 지원하는 상당히 포괄적인 개념이어서 구체적인 실현 방안이 모호할 수 있다. 이에 NECA 3.0의 핵심가치를 기반으로 실행전략을 세울 때, 가장 중요한 수혜의 대상은 환자 즉 국민이므로, 국민으로부터 신뢰를 얻기 위해 연구주제 선정이나 실행단계에서 국민의 참여를 모색 또는 확대해야 할 것이다. 또한 보다 효과적인 연구성과의 확산을 위해서는 연구결과의 쉬운 해석 제공등 국민과의 소통 증대에 관심을 기울여야 한다.
▢ Background
The National Evidence-based Healthcare Collaborating Agency (NECA) is celebrating its 7th anniversary since its establishment in 2009. Since its inception, the NECA has endeavored to provide scientific evidence for more efficient use of the nation’s medical resources and the improvement of public health. In addition, it has tried to disseminate research results, identify issues in the field of healthcare, and promote the institution through hosting annual memorial meetings. During the 7th anniversary annual meeting held in March 24, 2016, the relationship between precision medicine, which has received much attention in the field of health care, and medical technology assessment was examined in an effort to seek out future medical technology assessment research design ideas. Furthermore, there was also an effort to establish a platform for the exchange of opinions among professionals with regards to the need for state-led clinical research and development plans.
▢ Objective
The aims of this study were as follows: The first aim was to promote the importance and future plans of the NECA by sharing the research projects conducted within the agency with the public, professionals, and other relevant institutions, and by creating events that provide information and emphasize the need for medical technology assessments and the evaluation of new medical technologies. The second goal was to identify the direction of future-oriented research for medical technology assessments by creating an agenda of recent issues in the field of healthcare and establishing a platform for the exchange of opinions and research interests amongst professionals. The third goal was to identify the future direction of state-led clinical research that is applicable to the prospective healthcare environment. The final aim was to pinpoint ways the NECA can contribute to the development of public health and healthcare by establishing a specific action plan to conduct research that takes the patients’ values into consideration and become renowned as a credible institution.
▢ Methods
Ⅰ. Hosting NECA Annual Conferences
To ensure success, the event was planned several months in advance. The planning committee looked to previously held NECA events and international symposiums for improvement ideas. Also, starting from 2016, the title “Annual Conference” was used in place of “Annual Meeting” to reflect the proceedings which concern presentation and discussion of research achievements. After the annual conference, the results were reviewed and used in the preparation of the 2017 conference.
1. Agenda Selection and Program Development of the NECA Annual Conference
Several measures were taken to plan for the 2016 NECA Annual Conference. Firstly, potential areas of improvement from previous conferences were identified and utilized. Secondly, healthcare-related issues were reviewed to identify topics. After that, the first proposal was created based on potential topics after reviewing previous annual conferences held by similar institutions internationally and domestically. Of the possible candidates, the final topic, “Future Plan for State-led Healthcare Evidence Formulation” was chosen by the management committee consisting of team leaders and those of higher rank.
2. Speakers and Discussants
After the plan and program of the annual conference was confirmed, a speaker and discussants who are suitable for each plenary were chosen. The candidates were recruited over the phone or via email.
3. Production and Promotional Activities
Promotional activities were arranged by contacting the representative of each relevant institute. In order to recruit key participants, a list was created to distribute invitations and promotional materials both on- and off-line. In addition, event posters were created and distributed in key institutions and schools.
Furthermore, notification and pre-registration for the NECA Annual Conference was conducted through the researchers’ website. In addition, the NECA Annual conference program and primary contents were distributed through press releases.
Ⅱ. The Annual conference
The NECA Annual conference was held from 9:30 am to 5:40 pm with the conference split into Plenary 1, 2, and 3. Participants who pre-registered and registered on site were accepted, resulting in a total of 285 individuals in attendance.
Ⅲ. Conducting the Survey
Right after the event, a participant satisfaction survey was conducted to identify potential areas of improvements for the next annual conference.
Ⅳ. Follow-up Measures
After the annual conference, letters were sent to relevant institutions, healthcare professionals, and policy customers, expressing appreciation for their support and stating that maximum effort would be put forth in applying and sharing their suggestions. This was done in hopes of creating a sense of solidarity and formulating a network. In addition, the video footage of the conference was edited for each plenary and published on the researchers’ website and blog.
▢ Results
To celebrate its 7th anniversary, the 4th NECA Annual conference was held on March 24th, 2016 on the 8th floor of the Seoul National University Dental Hospital on the subject of “Future Plans for Evidence Formulation of State-led Healthcare.” This conference aimed to examine the relationship between precision medicine and medical technology assessment to identify a future-oriented medical technology assessment research design plan and also to provide professionals with a place to exchange ideas while exploring the need for state-led clinical research and development plans.
The conference was divided into three plenaries. Plenary 1 focused on customized healthcare that covers individual DNA and medical information. The session also explored development ideas of precision medicine--a customized predictive healthcare that includes proactive healthcare focused on health, living environment, and habits. Emphasis was placed on the need to produce research from evidence-based medicine so that precision medicine of the big data age can be reviewed and applied to an actual medical situation. Also, in regards to precision medicine and the healthcare industry, the following case studies were introduced: (1) case studies of local and overseas companies that systematically handle a variety of data relevant to precision medicine such as personal medical history, DNA information, and lifestyle, and (2) research cases that utilized a patient’s DNA to analyze the cause of a disease and resulted in clinical improvement.
The theme of Plenary 2 was “development direction of clinical research for evidence-based healthcare decision making.” During the session, cases were introduced in which medical bills were reduced due to changes in payment standards caused by results of state-led clinical research. Other topics included emphasis on the importance of the execution of clinical research, introduction of research achievements from a state-led type 2 diabetes clinical research center, and the emphasis of the initiation of clinical trials from a collection of patient registration data. It was also mentioned that the NECA’s National Health Clinical Research Project contributes to providing suitable medical services and reduces public medical bills by arbitrating public healthcare for high risk groups, conducting comparative effect studies, and formulating an integrated management system for clinical research data.
Plenary 3 was entitled “Widening the Horizons of NECA 3.0” and introduced the institutional improvements that new medical technology assessment has initiated with the aims of ensuring the public’s healthcare, innovating medical technology, and developing industry. The policy contributions that the NECA has made in the formation of an evidence-based disease prevention service system were introduced as well. Session participants also discussed steps the NECA must take in order to connect with the public, such as the importance of considering the patients’ values and patient participation plans.
▢ Conclusions
“The Future for State-led Healthcare Evidence Formation” discussed at the 2016 annual conference can be summarized by the following four points.
First, the NECA must closely observe new developments in the field of healthcare such as precision medicine, and identify intersecting points with the NECA’s existing competency and values of establishment, which were developed to support policy decisions based on evidence. In order to do so, the NECA must establish its professionalism and develop its ability as an institute on medical technology assessment.
Second, the national health clinical research studies have been conducted using solution-oriented methods; since these studies significantly contribute to the discovery of evidence domestically, they can be effectively applied to national policies. Hence, there is a need to maintain feasibility to attract interest and investment on a national level for clinical research through the National Health Clinical Research projects, and efforts must be made to ensure the stability of the business from legal and regulatory standpoints.
Third, proper measures are needed which are sensitive to external changes that require evidence. The new medical technology assessment system, which informs policy-making decisions regarding evidence based healthcare, has already been affecting policies and increasing communications in response to political changes. Also, the demand for evidence-based decision-making methodology has increased in the field of public health, and the NECA has supported this through research activities as an evidence evaluation center. This area is in the field of medical technology evaluation, and can ultimately be seen as a process of strengthening the abilities of the NECA in a way that is suited to its foundation goals rather than limiting the primary subject area to the field of medicine.
Fourth, since supporting healthcare policy that requires evidence-based decision-making is a very broad concept, it can be difficult to suggest specific implementation plans. When using the core values of NECA 3.0 as the basis of the implementation plan, the most important beneficiary is the patient or, in other words, the public. Hence, efforts such as choosing research subjects to obtain the public’s trust or seeking out and expanding the public’s participation in the implementation procedure is needed. In addition, for more effective dissemination of research achievements, more efforts must be taken in providing easy interpretation of research results in order to increase communication with the public.
