평가배경
“폐암 경피적 냉동제거술”은 폐암에 대한 일차적 치료 및 재수술 치료를 목적으로, 아르곤 가스와 헬륨 가스를 냉동프로브에 삽입하여 동결/해동 주기를 통해 암조직을 파괴하는 의료기술이다. 국내에서는 신의료기술평가제도가 확립되기 이전인 2006년에 비급여로 등재되었으며, 이후 2011년 건강보험 보장성 강화 정책에 따라 급여로 전환되어 임상에서 사용하고 있다.
해당 의료기술은 행위 급여 항목 중에 과거 유관기관 수요조사에서 저가치 기술로 제기된 바 있어, 내부 모니터링을 통해 재평가 주제로 발굴하였으며, 재평가 대상선별을 위한 임상자문회의와 우선순위 심의를 거쳐 2022년 제8차 의료기술재평가위원회(‘22.8.12.)에서 재평가 대상으로 최종 선정하였다.
본 평가의 목적은 폐암 환자에서 냉동제거술의 안전성 및 효과성 등에 대한 의과학적 근거평가를 통해 의료기술의 적정 사용 등을 지원하고자 하였다.
평가방법
폐암 경피적 냉동제거술에 대한 안전성 및 효과성 평가를 위해 체계적 문헌고찰을 수행하였다. 모든 평가방법은 평가목적을 고려하여 “냉동제거술 통합 소위원회(이하 ‘소위원회’라 한다)”의 심의를 거쳐 확정하였다. 소위원회 구성은 흉부외과 2인, 호흡기내과 1인, 종양내과 1인, 영상의학과 3인, 비뇨의학과 2인, 신장내과 1인, 근거기반의학 2인 전문가로 구성하였다.
평가의 핵심질문은 “폐암 환자에서 경피적 냉동제거술은 임상적으로 안전하고 효과적인가?”이었고 안전성은 시술관련 합병증을, 효과성은 종양학적 결과, 폐 기능, 환자 관련 결과 지표로 평가하였다.
체계적 문헌고찰은 핵심질문을 토대로 국외 3개, 국내 5개 데이터베이스에서 검색하였으며, 문헌선정 및 배제기준에 따라 두 명의 검토자가 독립적으로 선별하고 선택하였다. 문헌의 비뚤림위험 평가는 Risk of Bias (RoB)와 Risk of Bias for Nonrandomized Studies (RoBANS Ver 2.0)을 사용하였고, 자료추출은 미리 정해놓은 자료추출 양식을 활용하였다. 문헌의 비뚤림위험 평가와 자료추출은 모두 두 명의 검토자가 독립적으로 수행하였으며, 의견 불일치가 있을 경우 논의하여 합의하였다. 자료분석은 질적 검토(qualitative review) 및 정량적 분석(quantitative analysis)을 수행하였다.
평가결과
체계적 문헌고찰 결과, 안전성 및 효과성 평가는 총 10편(무작위배정 비교연구 1편, 후향적 코호트 비교 연구 9편)을 토대로 검토하였다. 선택문헌의 연구대상자는 모두 표준 수술적 치료가 어려운 중증 폐암 환자(수술 부적합 환자 6편, 진행성/전이성 폐암 4편)였고, 비교기술은 폐엽 중 암이 발생한 부분만 절제하는 구역 절제술(sublobar resection), 초단파소작술(Microwave ablation), 고주파 열치료술, 완화치료 등이었다.
안전성
안전성은 총 9편에서 보고하였고, 시술관련 합병증, 시술 후 재원기간 지표를 검토하였다. 경피적 냉동제거술의 전체 결과를 검토하였을 때 시술과 관련된 사망은 발생하지 않았으며, 기흉, 객혈, 흉막삼출, 기관지흉강루, 혈흉, 통증 등의 다양한 합병증 사례들이 보고하였다.
초단파소작술과 비교한 연구(4편)에서 경피적 냉동제거술군의 기관지 흉강루 발생률(2편)은 유의하게 낮았고(오즈비(Odds ratio, OR) 0.18, 95% 신뢰구간(Confidence interval, CI) 0.03-0.93, I2=0%, p=0.04), 객혈 발생률(4편)은 경피적 냉동제거술군에서 높게 나타났으나 통계적으로 유의한 차이는 없었다(OR 2.09, 95% CI 0.58-7.83, I2=49%, p=0.28). 기흉, 흉막삼출, 혈흉, 호흡곤란, 폐출혈, 흉막염 등의 다른 지표는 두 군간 유의미한 차이가 없었다.
고주파 열치료술과 비교한 연구(2편)에서 기흉, 기관지 흉강루, 객혈, 호흡곤란 발생은 두 군간 유의미한 차이가 없었다.
구역 절제술(sublobar resection)과 비교한 연구(1편)에서 경피적 냉동제거술의 기흉과 객혈 발생은 각 37% (10/27), 22.2% (6/27)인데 반해 수술군은 모두 발생하지 않아 중재군에서 발생률이 높았다.
시술 후 재원기간은 총 4편에서 보고하였으며, 구역 절제술(Subloar resection)보다는 경피적 냉동제거술의 재원기간이 짧았고, 초단파소작술, 고주파 열치료술과 비교한 결과 유의한 차이가 없었다.
효과성
효과성은 총 9편을 토대로 단독과 병용치료로 구분하여 검토하였다. 효과성 지표는 i) 종양학적 의료결과로 생존(전체 생존율, 무진행 생존율, 무질병 생존율, 암특이 생존율)과 치료반응(고형암반응평가기준에 의한 평가결과(완전반응), 국소종양제어율, 재발/전이, 시술성공(technical success : 사전에 계획한 종양의 완전 소작)), ii) 폐 기능, iii) 삶의 질 등의 환자 결과를 지표로 정하였으며, 이 중 폐기능 및 환자결과를 보고한 연구는 없었다.
단독치료
전체 생존 결과는 총 5편에서 보고하였다. 폐암 경피적 냉동제거술의 3년 전체 생존율은 초단파소작술(2편) 및 구역 절제술(1편)과 각각 비교시 유의한 차이는 없었으며, 고주파 열치료술과 비교시(2편) 1편은 경피적 냉동제거술군에서 높았고(75% vs. 49.9%, 통계값 제시안됨), 다른 1편은 군 간의 유의한 차이는 없었다. 완화치료와 비교한 1편에서는 경피적 냉동제거술군에서 전체 생존기간이 유의하게 긴 것으로 나타났다.
3년 무진행 생존율은 2편에서 보고하였고, 폐암 경피적 냉동제거술과 초단파소작술을 비교한 결과(1편)는 두 군간 유의한 차이가 없었으며, 고주파 열치료술과 비교에서는 경피적 냉동제거술의 생존율이 더 높았으나 통계적 유의성은 보고되지 않았다. 3년 무질병 생존율, 암특이 생존율은 1편에서 보고하였으며, 폐암 경피적 냉동제거술은 구역 절제술, 고주파 열치료술과 유의한 차이는 없는 것으로 나타났다.
치료반응은 총 8편에서 다양한 지표로 보고하였다. 초단파소작술과 비교한 5편 중 4편은 대부분 지표에서 두 군간 유의한 차이는 없었고, 폐암 경피적 냉동제거술의 완전반응율은 초단파소작술에 비해 낮았으나 통계적 유의성은 언급이 없었다. 고주파 열치료술과 비교한 3편 중 2편에서 경피적 냉동제거술군의 재발 및 전이가 적거나, 완전소작률이 높았고, 다른 1편에서는 중재군에서 완전반응을 보인 환자비율이 낮았으나 통계적 유의성은 제시되지 않았다. 구역 절제술과 비교 결과(1편) 경피적 냉동제거술군이 재발 및 전이가 적거나 유사한 수준이었다.
병용치료
전체 생존은 2편에서 보고하였으며, 2편 모두 폐암 경피적 냉동제거술과 분자표적치료 또는 전신치료를 병용치료한 군에서 1년 전체 생존율 및 전체 생존기간이 유의하게 긴 것으로 보고하였다. 무진행 생존은 1편에서 보고하였고, 경피적 냉동제거술과 병용치료한 군에서 무진행 생존기간이 유의하게 긴 것으로 보고하였다.
치료반응은 1편에서 보고하였고, 폐암 경피적 냉동제거술과 분자표적치료를 병용치료한 군이 치료반응율(완전 또는 부분반응)이 분자표적치료만 한 군에 비해 유의하게 높았다.
경제성
폐암 경피적 냉동제거술의 경제성 관련 결과는 1편에서 보고하였다. 전이성 비소세포 폐암 환자에서 최상 지지요법 또는 전신요법에 경피적 냉동제거술(percutaneous multisite cryoablation)을 추가하였을 때 비용-효과적인 것으로 보고하였다.
결론 및 제언
소위원회는 현재 문헌에 근거하여 “폐암 경피적 냉동제거술”의 안전성 및 효과성 결과를 다음과 같이 제시하였다.
폐암 경피적 냉동제거술은 기흉(흉관삽관 요구된 기흉 포함), 객혈, 흉막삼출, 혈흉 등 대부분의 지표에서 비교시술과 유의한 차이가 없어 안전한 의료기술로 평가하였다. 폐암 경피적 냉동제거술의 효과성은 단독치료시 완화치료와 비교하여 유의하게 생존기간이 길었고, 다른 국소치료술(초단파소작술, 고주파 열치료술), 구역 절제술(sublobar resections)과 유사한 수준의 생존 결과, 치료반응을 보였다.또한, 병용치료는 연구 수가 적긴 하였으나 경피적 냉동제거술을 분자표적치료 또는 전신치료와 병용치료한 군에서 유의하게 생존기간 길었으며, 치료반응 결과가 좋았다.
따라서, 소위원회에서는 폐암 환자에서 경피적 냉동제거술은 안전하고, 다른 국소치료법(초단파소작술, 고주파 열치료술)과 비교하여 유사한 생존 및 치료반응을 보여 효과성이 있는 기술로 판단하였다.다만, 동 평가에 포함된 연구대상자의 중증도를 고려하여 표준적 수술이 어려운 상태의 고위험 폐암 환자에서 단독 또는 병용치료 시 경피적 냉동제거술을 국소치료법 중 하나의 대안으로 사용해 볼 수 있는 기술로 판단하였다. 수술이 가능한 조기 폐암 환자에서 안전성과 효과성을 판단하기에 문헌적 근거가 부족하다는 의견이었다.
2023년 제3차 의료기술재평가위원회(2023.3.10.)에서는 소위원회 검토 결과에 근거하여 의료기술 재평가사업 관리지침 제4조제10항에 의거 “폐암 경피적 냉동제거술”에 대해 다음과 같이 심의하였다.
의료기술재평가위원회는 임상적 안전성과 효과성의 근거 및 그 외 평가항목 등을 종합적으로 고려하였을 때, 국내 임상상황에서 표준적 수술이 어려운 상태의 폐암 환자를 대상으로 경피적 냉동제거술의 사용을 ‘조건부 권고함’으로 심의하였다.

Background
Percutaneous cryoablation of lung cancer, a sophisticated health technology, involves the destruction of cancerous tissues using freeze/thaw cycles. This process employs argon or helium gas inserted through a probe, aimed at both primary treatment and reoperation of lung cancer. Initially categorized as a non-benefit item in 2006, prior to the establishment of the new health technology assessment system in Korea, this technology underwent a transition to a benefit item in 2011 under the expanded health insurance coverage policy and has since been actively utilized in clinical practice.
Despite being previously considered a low-value technology in a demand survey by relevant organizations, percutaneous cryoablation was later earmarked for reassessment following internal monitoring. After undergoing a candidate screening by the Clinical Advisory Board and a priority review, it was selected for reassessment during the eighth session of the Committee of Health Technology Reassessment in 2022 (on August 12, 2022). The primary aim of this reassessment is to facilitate the appropriate application of this technology in clinical settings, grounded in robust medical and scientific evidence of its safety and effectiveness in patients with lung cancer.
Methods
A comprehensive systematic review was conducted to evaluate the safety and effectiveness of cryoablation of lung cancer. The methodology was thoroughly developed after detailed deliberations by the “Integrated cryoablation subcommittee (hereafter referred to as the “Subcommittee”), composed of multidisciplinary experts including two thoracic surgeons, a pulmonologist, an oncologist, three radiologists, two urologists, a nephrologist, and two evidence-based medicine specialists.
The critical question guiding this assessment was, “Is percutaneous cryoablation a clinically safe and effective treatment for patients with lung cancer?” The safety assessment focused on procedure-related complications, while effectiveness was gauged in terms of oncologic outcomes, lung function, and patient-related outcomes.
In conducting the literature review, a thorough search was performed across three international and five domestic databases, based on the aforementioned key question. Two independent reviewers meticulously screened and selected relevant articles, adhering to the inclusion and exclusion criteria. The risk of bias (RoB) was assessed using the Cochrane RoB tool and the Risk of Bias for Nonrandomized Studies (RoBANS Ver 2.0), and data extraction followed a predetermined format. The RoB assessment and data extraction processes were independently executed by two reviewers, with any disagreements resolved through collaborative discussions to reach consensus. The data analysis encompassed both qualitative review and quantitative analysis.
Results
The systematic review included a total of 10 articles, comprising one randomized control trial (RCT) and nine retrospective cohort comparison studies. The study population in these articles consisted predominantly of patients with severe lung cancer for whom conventional surgical intervention may be challenging. This included six studies focusing on inoperable patients and four studies on progressive or metastatic lung cancer. Comparative interventions (controls) in these studies varied, encompassing microwave ablation, radiofrequency ablation, palliative care, and sublobar resection (resection of only the affected portion of the lung lobe).
Safety
The safety profile of percutaneous cryoablation for lung cancer was thoroughly assessed through a review of nine articles focusing on procedure-related complications and the duration of post-procedure hospital stays. Comprehensive analysis of these studies indicated that percutaneous cryoablation was not associated with any procedure-related fatalities. However, various complications such as pneumothorax, hemoptysis, pleural effusion, broncho-thoracic fistula, hemothorax, and pain were reported.
In comparative studies with microwave ablation (involving four studies), it was found that the incidence of broncho-thoracic fistula was significantly lower in the percutaneous cryoablation group (n=2), as evidenced by an odds ratio (OR) of 0.18 and a 95% confidence interval (CI) of 0.03–0.93, with no heterogeneity (I2=0%, p=0.04). Conversely, the incidence of hemoptysis was higher in the percutaneous cryoablation group (n=4), though this difference did not reach statistical significance (OR: 2.09, 95% CI: 0.58–7.83, I2=49%, p=0.28). The studies did not demonstrate significant differences in other complications like pneumothorax, pleural effusion, hemothorax, dyspnea, pulmonary hemorrhage, and pleurisy between the two groups.
In studies comparing percutaneous cryoablation with radiofrequency ablation (involving two studies), there were no significant differences observed in the incidences of pneumothorax, broncho-thoracic fistula, hemoptysis, pulmonary hemorrhage, and dyspnea between the groups.
In a study comparing percutaneous cryoablation with sublobar resection, the incidence rates of pneumothorax and hemoptysis in the percutaneous cryoablation group were 37% (10/27) and 22.2% (6/27), respectively, significantly higher than in the surgery group where no such cases were reported.
Regarding the length of post-procedure hospital stay, four studies reported that patients in the percutaneous cryoablation group generally experienced shorter hospital stays compared to those undergoing sublobar resection, but no significant differences were noted compared to the microwave ablation and radiofrequency ablation groups.
Effectiveness
The effectiveness of percutaneous cryoablation, either as monotherapy or in combination with other treatments, was evaluated based on nine articles. The parameters for assessing effectiveness included: 1) oncologic outcomes such as overall, progression-free, disease-free, and cancer-specific survival rates, along with treatment response based on the Response Evaluation Criteria in Solid Tumors [complete response], local tumor control rate, recurrence/metastasis, and procedure success [technical success: pre-planned complete ablation of tumor]);; 2) lung function; and 3) patient outcomes like quality of life. However, no studies reported on lung function and patient outcomes.
Monotherapy
The overall survival rate was documented in five articles. In comparisons of 3-year overall survival rates, the percutaneous cryoablation group showed no significant difference compared to the microwave ablation (n=2) and sublobar resection (n=1) groups. In two studies comparing with radiofrequency ablation, one reported a higher survival rate in the percutaneous cryoablation group, while the other noted no significant difference between the groups. Against palliative care, the overall survival period was significantly longer in the percutaneous cryoablation group.
A 3-year progression-free survival rate was reported in two articles. Comparisons between percutaneous cryoablation and microwave ablation (n=1) showed no significant difference, whereas the comparison with radiofrequency ablation indicated a higher survival rate in the percutaneous cryoablation group, albeit without reported statistical significance. Disease-free and cancer-specific survival rates over three years also showed no significant differences compared to the sublobar resection and radiofrequency ablation groups.
Treatment response, as per various indicators, was reported in eight articles. Among five comparing with microwave ablation, four reported no significant differences in most indicators, and the complete response rate was lower in the percutaneous cryoablation group, though without statistical significance. Among three articles comparing with radiofrequency ablation, two reported lower recurrence and metastasis and higher complete ablation rates in the percutaneous cryoablation group, while the other reported a lower percentage of patients with complete response in the intervention group, again without statistical significance. In a comparison with sublobar resection, the percutaneous cryoablation group showed lower or similar levels of recurrence and metastasis.
Combination therapy
Overall survival rate was reported in two articles, both indicating significantly longer 1-year overall survival rates and overall survival periods in groups combining percutaneous cryoablation with molecular targeted therapy or systemic therapy. Progression-free survival, reported in one article, showed a significantly longer period in the combination therapy group.
Treatment response was documented in one article, where the combination therapy group (percutaneous cryoablation and molecular targeted therapy) demonstrated a significantly higher treatment response rate (complete or partial response) compared to the monotherapy group (molecular targeted therapy alone).
Cost-effectiveness
An exhaustive investigation into the cost-effectiveness of percutaneous cryoablation for lung cancer was conducted, based on the data presented in one article. This study revealed that adding percutaneous multisite cryoablation to either best supportive care or systemic therapy is a cost-effective approach for treating patients with metastatic non-small cell lung cancer.
Conclusions and recommendations
Following a meticulous review of the available literature, the Subcommittee deduced several key points about the safety and effectiveness of percutaneous cryoablation for lung cancer.
The technology was identified as a safe health intervention, with no significant differences in major safety indicators such as pneumothorax (including cases requiring chest tube insertion), hemoptysis, pleural effusion, and hemothorax, compared to other comparator interventions. Regarding its effectiveness, when used as monotherapy, percutaneous cryoablation of lung cancer was found to significantly prolong survival periods compared to palliative care and showed similar survival outcomes and treatment responses as other local treatment methods like microwave ablation, radiofrequency ablation, and sublobar resection. Although the study base was limited, the use of percutaneous cryoablation in combination with molecular targeted therapy or systemic therapy indicated better survival periods and treatment response outcomes.
Given these findings, the Subcommittee recognized percutaneous cryoablation as a safe and effective option for lung cancer patients, especially those at high risk or with conditions that complicate standard surgical approaches. It can be employed effectively either as monotherapy or as part of combination therapy. However, the Subcommittee also noted a lack of substantial evidence regarding the safety and effectiveness of this technology in patients with operable early-stage lung cancer.
In the third session of the Committee of Health Technology Reassessment in 2023 (on March 10, 2023), the committee drew conclusions regarding percutaneous cryoablation for lung cancer in line with Article 4-10 of the Guidelines for Management of Health Technology Reassessment. The Committee “conditionally recommends” the use of percutaneous cryoablation for lung cancer patients with conditions that make standard surgery difficult in the Korean clinical context. This recommendation is grounded in a thorough consideration of the clinical safety, effectiveness, and other relevant assessment criteria of the technology.
