평가 배경
양악수술(double-jaw surgery)은 악안면 교정술(orthognathic surgery)의 일종으로 악교정 수술이라고도 칭하며, 구강 안쪽을 절개한 후, 상악(maxilla) 또는 하악(mandible)을 절골하여 적절한 위치로 이동시켜 턱 모양을 교정하는 수술법이다. 양악수술은 얼굴/턱 부위의 모양을 교정하고, 턱의 기능을 개선할 목적으로 개발되어 외모개선 목적으로도 활용되고 있으며 이 경우 국민건강보험 요양급여의 기준에 관한 규칙 [별표] 비급여 대상에 해당하는 ‘선택 비급여’항목이다. 해당 기술은 의료기술재평가사업에서 선택 비급여 항목으로 활용되는 국민생활밀착형 의료기술에 대한 근거를 제공하는 목적으로 선정되었다. 2020년 제11차 의료기술재평가위원회(2020. 11. 11.)에서 평가계획서가 심의되어 ’대국민 정보제안 목적‘의 주제로 선정되어 재평가를 수행하였다.
평가 방법
외모개선 목적으로 이루어질 때 악안면 교정수술(양악수술)의 안전성을 파악하기 위해 체계적 문헌고찰을 수행하였고 관련 세부수행방법은 소위원회에서 확정하였다. 소위원회는 성형외과(악안면 부위 전문) 2인, 구강악안면외과 2인, 치과교정과 2인, 마취통증의학 1인, 근거기반의학 1인 등 총 8인으로 구성되었다.
체계적 문헌고찰은 국외 3개, 국내 5개 데이터베이스에서 “악안면 교정술(orthognathic surgery)”이란 용어를 주요어로 하여 악안면 부위 수술들에 대하여 광범위하게 검색하였다. 두 명의 검토자가 독립적으로 문헌에서 악안면 교정술 중 양악수술법을 사용하고, 수술관련 안전성 지표를 보고한 문헌을 선별한 후 양악수술이 외모개선 목적이 아닌 의학적ㆍ기능개선 목적으로 시행된 경우에 대한 연구를 배제하는 방식으로 문헌을 선택배제하였다. 최근 10년동안 악안면 교정술(양악수술)의 안전성을 보고한 문헌들에서 각 문헌별로 보고한 안전성 지표를 항목별로 제시하고, 이를 바탕으로 대국민 정보문을 별도로 마련하였다.
평가 결과
외모개선 목적의 악안면 교정수술(양악수술)의 안전성을 보고한 최종 선택 문헌들은 총 136편이었다. 1차 자료원에 기반한 연구 129편, 2차 자료원인 기존 등록자료를 사용한 연구 7편이었다. 1차 자료에 기반한 연구 129편 중 양악수술 후 발생할 수 있는 부작용 및 합병증의 범위를 한정하지 않은 연구 23편, 특정 부작용 및 합병증 항목만을 대상으로 한 연구 96편, 수술법 간 비교 연구 5편, 기타 5편이었다.
안전성
양악수술의 안전성을 다룬 문헌들에서 가장 많이 다루고 있는 부작용 항목은 감각신경 관련 장애였다. 환자들이 호소하는 감각 장애에 대한 정의가 문헌마다 다른 방식으로 보고되었다. 감각신경 장애 발생률은 수술 직후 높은 발생률을 보이다가 시간이 경과함에 따라 감소하였다. 주로 하악에 대한 수술에서 해당 부작용을 보고한 사례가 많았다. 환자의 연령이 증가할수록 감각신경 장애의 발생이 증가한다고 보고하였다.
양악수술이 잇몸부위 절개를 동반하는 외과수술이라는 특성상 수술 후 나타날 수 있는 수술부위 감염, 통증, 출혈지속, 피부/근육 조직의 상처, 붓기, 염증 등과 같은 부작용 등이 주로 보고되었고, 골 절단 과정에서 의도하지 않은 골절 사례를 보고한 문헌들도 있었다. 구강 부위의 절단 과정에서 치아/잇몸 부위에 손상이 발생한 사례들도 보고되고 있었다. 절개 부위의 특성상 주로 상악수술에서 코와 눈이 연결된 기관들에 대한 장애 – 예. 누공, 구비강 개통, 눈물샘 폐쇄, 눈물 흘림 초래, 상악낭종 발생 등 – 에 대한 보고도 있었다. 양악 수술을 받은 환자에서 드물게 보고되는 사례들에 대한 증례 보고(폐기종, 기흉, 동공산대 등)도 있었다.
전신마취 과정을 필요로 하는 수술이므로 수술 후 메스꺼움 및 구토 등과 같은 증상이 빈번하게 보고되었다. 마취 시, 기관 삽관 과정과 관련하여 후두육아종 발생 사례, 삽관 절단 등과 같은 사례를 보고한 경우도 있었다.
결론 및 제언
해당 소위원회에서는 현재 평가결과에 근거하여 다음과 같이 제언하였다.
악안면 교정수술(양악수술)의 가장 흔한 부작용인 감각신경 관련 장애는 수술 직후 흔하게 발생하고, 보통 반년 이상 경과 후 해소되는 경우가 많은데, 장기간 동안 환자가 해당 증상을 호소할 경우, 신경손상이 있는지를 주의깊게 살펴볼 필요가 있다고 제언하였다.
주로 하악수술을 평가한 문헌들에서 감각신경 장애 관련 부작용을 평가한 경우가 많았는데 이는 하악수술시 가장 많이 사용하는 하악지 시상분할 골 절단술(Sagittal Split Ramus Osteotomy, SSRO)에서 시상분할하는 하악지 부위가 감각신경이 지나가는 자리이기 때문에 신경이 늘어나거나 압박받을 가능성이 더 높기 때문으로 소위원회는 해석하였다. 그럼에도 SSRO는 턱의 전방, 하방 이동이 모두 가능한 유용성이 큰 방법으로 보편적으로 많이 사용되고 있다고 언급하였다.
잇몸 부위 절개가 동반되는 외과수술의 특성상 악안면 교정수술(양악수술) 후, 수술부위 감염, 통증, 출혈지속, 피부/근육 조직의 상처, 붓기, 염증 등과 같은 부작용을 예방하기 위해서는 수술 과정 뿐 아니라, 수술 후의 환자 관리 강화, 필요시 적절한 대응(예. 적절한 의약품 복용)도 중요하다고 제언하였다. 또한 전신마취와 관련한 이상반응이 드물지만 발생할 수 있으므로 환자의 상태에 대한 다각적 고려가 필요하며, 전신마취로 인해 흔히 발생할 수 있는 메스꺼움이나 구토 증상 등에 대해서도 환자에게 사전에 인지시킬 필요가 있다고 제언하였다.
2021년 제9차 의료기술재평가위원회(2021.09.10.)에서는 소위원회 검토 결과에 근거하여 의료기술재평가사업 관리지침 제4조제10항에 의거 “외모개선 목적의 악안면 교정수술(양악수술)”에 대해 다음과 같이 심의하였다.
본 평가를 통해 악안면 교정수술(양악수술)을 받는 환자들에 대한 다양한 부작용 사례들에 대한 현황, 발생률, 관련 요인 등 여러 측면에서 다양하게 결과를 제시할 필요가 있다. 또한 ‘외모개선 목적’으로 양악수술을 고려하고 있다면 수술의 적용 범위 및 과정, 수술로 인해 발생할 수 있는 여러 불편함과 부작용 발생의 가능성을 사전에 인지하고, 수술 전 담당 의사와 충분히 상담하고, 수술 후에도 전문가의 적절한 관리가 이루어지는 환경에서 수술이 이루어져야 한다고 제시하였다. 이와 관련하여 악안면 교정수술(양악수술)에 대한 평가결과를 대국민 정보로 별도 정리하여 제공하는 것에 동의하였다.

수술 후에도 정기적인 진료를 빠지지 않고 받으며, 몸에 이상 반응이 있을 경우, 주치의와 상담하는 것이 필요합니다.

Background
Double jaw surgery is a type of orthognathic surgery, where an incision is made inside the oral cavity, then the maxilla or mandible is cut and moved to a suitable location, so as to adjust the chin shape. Double jaw surgery was developed for the purpose of adjusting the facial or chin shape with improved chin function. Therefore, it is also used for cosmetic improvement; in this case, the technique is listed as part of Selective Non-payment in the Regulations on the Criteria of National Health Insurance Benefits (Annex). The technique was selected as the target of the HTR Project to provide evidence for the use of the technique as a public livelihood health technology in the Selective Non-payment list. The reassessment plan was deliberated by the 11th HTR committee of 2020 (Nov 11, 2020) and the theme was selected as the target of the public information provision, followed by the reassessment.
Methods
To assess the safety of double jaw surgery for the purpose of cosmetic improvement, a systematic review was performed. The details of the review were confirmed by the subcommittee. The subcommittee consisted of eight members; two plastic surgeons (specialized in mandibulofacial surgery), two oral and maxillofacial surgeons, two orthodontists, one anesthetists, and one specialist of evidence-based medicine.
For the systematic review, the term “orthognathic surgery” was used as the keyword on three overseas and five domestic databases, for an extensive search of mandibulofacial surgeries. Two reviewers independently screened and selected articles reporting on the use of double jaw surgery among orthognathic surgeries and the results of surgery-related indicators. Articles on double jaw surgery used for purposes other than cosmetic improvement, such as medical functional improvement, were excluded. The articles reporting on the safety of double jaw surgery in the past decade were analyzed per safety indicator per article, and the findings were used to prepare a leaflet of public information.
Results
For the assessment of the safety of double jaw surgery for cosmetic improvement, 136 articles were found. Among them, 129 studies were based on primary data sources and 7 studies were based on secondary data sources. The 129 studies based on primary data sources included 23 studies reporting on postoperative side effects and complications without restriction, 96 studies reporting on specific side effects and complications, 5 comparative studies on different surgical methods, and 5 studies on others.
Safety
Regarding safety, the most frequently reported side effect of double jaw surgery in the studies was disorders related to sensory nerves. The degree of sensory disturbance experienced by patients varied across studies; it was high immediately after surgery, decreasing as time passed. The greatest number of cases were related to the mandible and the incidence was reported to increase with age.
As double jaw surgery is a surgical method involving the incision on the gum, the main side effects were postoperative surgical site infection, pain, continuous bleeding, skin or muscle tissue damage, edema, and inflammation. Some studies reported unintended fracture during bone cutting and other cases of damage to the teeth or gum area during oral incision. Most disorders related to organs connected to the nose and eyes were reported for maxilla surgery due to the characteristics of the incision area. Examples of these disorders include fistulous opening, oronasal communication, obstruction of the nasolacrimal duct, lacrimation, and maxilla cyst. In addition, there were case reports on rare cases of pulmonary emphysema, pneumothorax, corectasis, and others, in patients after double-jaw surgery.
As double jaw surgery requires general anesthesia, symptoms such as postoperative nausea and vomiting were frequently reported. There were even cases of granuloma and tube split in association with intubation for anesthesia.
Conclusions and Recommendations
Thus, based on the current level of evidence, the subcommittee suggested the following:
Disorders of sensory nerves, the most common side effect of double-jaw surgery, were shown to occur most frequently immediately after surgery, while they largely disappeared after around six months. It is therefore suggested that nerve damage should be carefully monitored when the patient complains of the related symptoms for a long period of time.
These disorders were mainly investigated as a side effect in studies assessing mandible surgery. The subcommittee attributed this to the higher probability of nerve stretch or pressure as the sagittal split ramus area in sagittal split ramus osteotomy (SSRO), the most commonly used method for mandible surgery, is where the sensory nerves pass. Nonetheless, SSRO was mentioned as the most general and highly useful method that allows both forward and backward chin movement.
Due to the characteristics of the surgery involving gum incision, postoperative surgical site infection, pain, continuous bleeding, skin or muscle tissue damage, edema, and inflammation may occur as a side effect of double jaw surgery. The subcommittee stated that, to prevent such side effects, consideration of not only the surgery process but also the reinforced postoperative patient management and appropriate response (e.g., the administration of suitable drugs) is necessary. In addition, although rare, adverse reactions to general anesthesia may occur, therefore multilateral interpretation of the patient state is necessary, while the patients should be informed of potential nausea or vomiting as a common effect of general anesthesia.
Based on the subcommittee report and according to act 4, paragraph 10 of the HTR Project Guideline, the conclusion of the 9th HTR committee of 2021 (Sep 10, 2021) on double jaw surgery was as follows:
Regarding the various side effects of double jaw surgery reported in past cases, the current state, incidence, and related factors should be presented through multidimensional analyses. Furthermore, in consideration of double jaw surgery for cosmetic improvement, one should be aware of the surgery scope and process and the various potential discomforts and side effects, while making adequate consultations with the surgeon before surgery. The surgery should be performed in an environment where appropriate management is provided by specialists after the procedure. For this, it was agreed that a public information leaflet should be prepared to present the results of double jaw surgery; the summary of which is outlined below.
Summary of Public Information
Double jaw surgery is a surgical method developed to adjust the facial or chin shape and improve chin function (e.g., chewing, speaking, and improvement in respiratory disturbance). The surgery involves general anesthesia, followed by oral incision, chin bone incision and fixation, and subsequent suture.
Double jaw surgery may entail various side effects and complications related to the chin bone incision and suture of surrounding tissue. Among the side effects, sensory disturbances are reported to occur mainly in surgery of the mandible. Other potential complications include surgical site infection, pain, continuous bleeding, gum tissue damage, edema and inflammation.
In making the decision regarding double jaw surgery, the patient should make adequate consultations with the specialists regarding his or her health, while ensuring that the surgery and subsequent admission occurs in an environment capable of rapid response to potential side effects (e.g., a resident anesthesiologist and a service of rapid emergency response, if necessary).
Likewise, the patient should participate in all periodic checkups post-surgery and, in cases of adverse response, consult the physician immediately.
