▢ 연구 배경
사람유두종바이러스(human papilloma virus, HPV)는 자궁경부암의 주요 원인이며 암 발생의 위험도에 따라 고위험군과 저위험군으로 나뉜다. 고위험 유형은 대표적으로 16번과 18번이 있고 그 밖에도 31, 33, 35, 39, 45, 51, 52, 56, 58, 59, 66번이 자궁경부암의 원인이다.
자궁경부 세포검사는 자궁경부암 선별검사의 일차적인 표준검사법으로, 간편하고 고등급 병변에서 비교적 정확하고 경제적 이득 등의 장점이 있어 현재까지 인정된 방법 중 가장 효과적인 선별검사 방법으로 알려져 있다. 하지만, 높은 위음성률과 낮은 민감도 등으로 인해 새로운 선별검사방법에 대한 필요성이 제기되고 있고 HPV 검사가 그 대표적인 예이다.
HPV 검사의 경우 현행 비급여로 분류되어 있어 건강보험심사평가원 청구자료에서는 실제 임상현장에서의 사용 현황 및 결과를 확인하기 어렵다. 따라서 국내 임상현장에서 사용되고 있는 환자자료를 구축하여 자궁경부암의 조기검진을 위한 HPV 검사의 임상적 유효성을 확인할 필요가 있다.
▢ 연구 목적
다양한 HPV 유형을 전체 고위험군, HPV 16/18을 중심으로 한 고위험군과 그 외 고위험군으로 구분하여 각 군에 따른 CIN2+ (cervical intraepithelial neoplasia) 진단율을 확인하고, HPV 검사결과를 자궁경부암 검진에 선별 적용할 수 있는지를 확인하고자 한다.
▢ 연구 방법
전국 13개 상급종합병원의 의무 기록을 후향적으로 수집하고 분석하였다. 해당하는 연구대상자, 수집한 자료 항목과 분석 방법은 아래와 같다.
1) 연구대상자
- 2018.01.01.-2018.12.31. 동안 건강검진을 위해 전국 상급종합병원에 내원한 여성
- 자궁경부 세포검사와 사람유두종바이러스검사를 함께 시행한 여성
- 자궁경부 조직검사를 시행한 여성
2) 자료수집항목
- 나이, 키, 몸무게, 임신력, 분만력 등의 기본정보
- HPV 검사 기기 이름
- HPV 결과(유형)
- 자궁경부 세포검사결과, 자궁경부 조직검사결과
3) 분석방법
- HPV 검사 기기별 진단율 차이
- HPV 유형별 CIN2+ 진단율
- HPV 검사와 자궁경부 검사 혼용의 진단율
수집된 자료에 기초하여 HPV 검사 기기별, HPV 유형별 진단정확도를 산출하고, 자궁경부 세포검사와 HPV 검사 조합을 통한 최선의 선별전략을 확인하였다.
▢ 연구 결과
⦁ Cobas HPV 검사와 국내에서 시행되고 있는 다른 HPV 검사방법들 간의 CIN2+의 진단율에 차이는 없었다.
⦁ 자궁경부 세포검사와 고위험 HPV 검사를 같이 시행하면 민감도와 특이도를 향상시킬 수 있다.
Ⅰ. Cobas HPV 검사와 다른 HPV 검사방법들 간의 CIN2+의 진단율에 차이는 없었다.
1) Cobas HPV 16/18+으로 본 CIN2+의 민감도를 기준값으로 정의하였을 때, 다른 검사들과의 통계적 유의한 차이는 없었다.
Ⅱ. 자궁경부 세포검사에서 ASCUS (atypical squamous cells of undetermined significance) 이상으로 본 CIN2+의 민감도는 95.1%, 특이도는 17.1% 이다.
1) LSIL (low-grade squamous intraepithelial lesions) 이상으로 본 CIN2+의 민감도는 74.5%, 특이도는 60.5% 이다.
2) ASCUS 이상으로 본 CIN3+의 민감도는 95.1%, 특이도는 15.1% 이다.
III. HPV 16/18 양성 소견으로 본 CIN2+의 민감도는 41.5%이고 특이도는 86.1% 이다.
IV. 전체 고위험군 HPV 양성 소견으로 본 CIN2+의 민감도는 95.4%이고 특이도는 27.0% 이다.
V. CIN2+의 병변을 찾기 위해 다음과 같은 환자 분류 전략을 수립할 수 있다.
1) 자궁경부 세포검사에 상관없이 고위험군 HPV 양성인 환자
- 민감도 94.55%, 특이도 29.70%
2) 자궁경부 세포검사에서 LSIL 이상이거나 고위험군 HPV 양성인 환자
- 민감도 98.64%, 특이도 20.59%
3) 자궁경부 세포검사에서 HSIL 이상이거나 고위험군 HPV 양성인 환자
- 민감도 97.06%, 특이도 27.79%
▢ 결론 및 정책적 제언
기존의 자궁경부 세포검사 단독으로 자궁경부 상피내종양을 선별하는 것보다 HPV 검사를 동시에 시행하여 두 검사법의 단점을 보완하는 것이 민감도와 특이도의 측면에서 도움이 된다. 하지만, 아직까지는 HPV 검사 단독으로 자궁경부암 선별검사를 시행하는 것에는 한계가 있어 자궁경부 세포검사와 HPV 고위험군 검사를 같이 시행하는 것이 자궁경부암 조기 진단 측면에서 이득이고 또 안전한 방법이다.
따라서 자궁경부암 검진 전략에 HPV 검사를 포함시켜야하고, 아직까지는 HPV 검사가 기존의 자궁경부 세포검사를 대체할 수는 없다. 또한, 국내 실정에 맞는 안전하면서도 비용-효과적인 측면에서 이득이 있는 HPV 검사와 자궁경부 세포검사의 조합을 찾아내는 전향적인 연구가 필요하다.
▢ Background
Human papillomavirus (HPV) is a major cause of cervical cancer and is divided into high and low risk groups according to the risk of cancer. The high-risk types are typically 16 and 18, and 31, 33, 35, 39, 45, 51, 52, 56, 58, 59, and 66 are the causes of cervical cancer.
Cervical cytology is the first standard screening test for cervical cancer screening and is known as the most effective screening method among currently recognized methods because of its advantages such as simple and relatively accurate and optimal gain in high grade lesions. However, the need for a new screening method has been raised due to high false negative rate and low sensitivity, and HPV test could be a representative.
HPV testing is not included in Korean National Health Insurance Service (KNHIS), so it is difficult to be analyzed the current status of use. Therefore, it is necessary to establish the clinical data of the HPV test for the early screening of cervical cancer by constructing the patient data used in the clinical field.
▢ Objective
The aim of this study was to assess the clinical value of hrHPV (High-risk human papillomavirus) genotyping as a supplemental triage strategy for cervical cancer screening compared with liquid-based cytology for cervical cancer screening in Korea.
▢ Methods
1) Study population
- Patients who visited the General Hospital for medical checkup in 2018.01.01.-2018.12.31.
- Patients who underwent cervical cytology and HPV test
- Patients who have undergone cervical biopsy
2) Data collection
- Basic characteristics including age, height, weight, pregnancy and labor
- HPV genotyping methods
- HPV genotyping (16/18 or others)
- Pap smear, pathologic finding
3) Triage strategies
- Difference of clinical value in HPV genotyping methods
- Difference of clinical value in HPV 16/18 with hrHPV
- Difference of clinical value in hrHPV HPV with cervical cytology
▢ Results
⦁ There was no difference in diagnosis rate of CIN2+ between the Cobas HPV test and other HPV test methods.
⦁ Combining cervical cytology and high-risk HPV testing can be cost-effective by improving sensitivity and specificity.
I. There was no difference in the diagnosis rate of CIN2+ (cervical intraepithelial neoplasia) between the Cobas HPV test and other HPV test methods.
1) When the sensitivity of CIN2+ was defined as the reference value with Cobas HPV 16/18+, there was no statistically significant difference from other tests.
II. ≥ASCUS (atypical squamous cells of undetermined significance), CIN2+ has a sensitivity of 95.1% and a specificity of 17.1%.
1) The sensitivity of CIN2+ of ≥LSIL (low-grade squamous intraepithelial lesions) is 74.5% and the specificity is 60.5%.
2) The sensitivity of CIN3+ of ≥ASCUS is 95.1% and the specificity is 15.1%.
III. The HPV 16/18+ findings revealed that CIN2+ had a sensitivity of 41.5% and specificity of 86.1%.
IV. Overall, hrHPV+ findings revealed a sensitivity of 95.4% and specificity of 27.0%.
V. A patient classification strategy can be established to identify lesions CIN2+.
1) HrHPV+ regardless of cervical cytology
- Sensitivity 94.55%, specificity 29.70%
2) Patients with ≥LSIL or hrHPV+ on cervical cytology
- 98.64% sensitivity, 20.59% specificity
3) Patients with ≥ASCUS or hrHPV+ on cervical cytology
- Sensitivity 97.06%, specificity 27.79%
▢ Conclusions
Rather than screening for cervical epithelial tumors with conventional Pap smears, supplementation with HPV tests is helpful in terms of sensitivity and specificity. However, screening by HPV alone is still limited, so a combination of Pap smear and HPV high-risk testing is a cost-effective and safe method.
Therefore, HPV testing should be included in the early screening of cervical cancer, and so far, HPV testing cannot replace the Pap smear. In addition, a prospective study is needed to find a combining of hrHPV test and cervical cytology that can benefit from safety and cost-effectiveness.
