▢ 연구 배경
코로나19는 2020년 현재 전세계적으로 가장 많이 확산된 감염병으로서 각국의 방역 노력에도 불구하고 확실한 치료제가 개발되지 않았고, 백신은 개발 초기의 단계에 있다. 한국은 성공적인 방역을 통해 국민의 건강을 보호해왔으나, 실제 임상 현장에서는 입원환자들의 폐렴 및 기타 장기 감염 관련 치료와 관련된 신속하고 근거에 기반한 임상진료지침 개발 요구가 높다. 현재 전세계에서 백신 개발 및 치료제 개발을 위한 임상연구가 활발하게 이루어지고 있어, 코로나 감염 확산 초기에 비해 많은 임상적 결과에 대한 논문들이 매일 발표되고 있다. 따라서 방법론적인 측면에서는 근거에 기반하여 신속하게 개발해야 할뿐만 아니라 지속적인 추가 근거의 확인과 권고문 업데이트가 필요한 상황이다. 다양한 임상 양상과 치사율을 보이는 코로나19와 관련하여 현재로서 지지 치료 외에 확립된 항바이러스제 표준 치료방법은 없으나, 최신 근거들을 바탕으로 하여 도움이 될 수 있는 항바이러스제 및 기타 약물 치료에 대해 이 지침을 통해 소개하고자 한다.
▢ 연구 목적
코로나19로 입원하는 환자들의 치료를 담당하는 의료진을 위해 최신의 치료방법에 대한 근거들을 검토하여 국내 상황에 맞는 근거기반 치료 임상진료지침을 신속하게 개발하는 것이다.
또한, 본 지침은 가이드라인 개발 방법론 측면에서는 코로나19와 같은 긴급한 상황 속에서 신속하면서도 최신의 근거를 기반으로 하는 가이드라인 개발 방법론을 탐색하여 적용하고자 한다.
▢ 연구 방법
본 지침은 가이드라인 개발 방법론 측면에서는 짧은 기간 내 신속한 권고안 개발을 위해 주요 국가 및 기관의 최신 가이드라인을 검토하여 활용하는 수용개작 방법을 기반으로 한다. 또한 최신 근거를 지속적으로 반영하는 살아있는 가이드라인(living guideline) 개발 방법론을 적용하고자 하였다. 가이드라인 개발 과정은 다음과 같다
○ 진료지침의 검색
검색원 선정 및 검색전략 구성 그리고 실제 문헌검색과 업데이트 전체 과정을 문헌정보검색전문가와 함께 논의하여 진행하였다. 2020년 6월 이후 발표된 논문들을 포함하는 진료지침들 혹은 지속적인 갱신을 하고 있는 living guideline들을 수용개작 대상으로 하기로 하였다.
○ 검색원
문헌 검색데이터베이스는 국외 PubMed와 EMBASE를 검색하였고 주요 진료지침 가이드라인 데이터베이스와 웹사이트를 검색하였다. 특히 진료지침 개발에 소요되는 시간을 고려했을 때, 출판 문헌으로 나온 가이드라인보다는 웹사이트가 실시간 근거 업데이트가 잘되고 있어 신뢰할 수 있는 주요 정부 기구와 기관 및 학회 사이트를 직접 검색하는 것이 주요하다고 판단하였다.
○ 검색전략
PubMed와 EMBASE를 기반으로 질환 및 치료제 검색용어 선정 및 진료지침 검색을 위한 필터를 적용하였다. 검색어는 ‘coronavirus’, ‘novel coronavirus’, ‘novel coronavirus 2019’, ‘2019 nCoV’, ‘임상진료지침’, ‘Wuhan coronavirus’, ‘Wuhan pneumonia’, ‘SARS-CoV-2’, ‘severe acute respiratory syndrome’, ‘treatment’, ‘therapy’, ‘antiviral’ 그리고 각 임상질문의 주요 치료제 용어를 기반으로 통제어와 자연어를 조합하여 사용하였다.
○ 진료지침 선택 기준
2020년 6월 이후에 나온 연구들이 코로나19 치료경험에 대해 질적으로나 양적으로 신뢰할 만한 정보를 담고 있어 가능한 최근 guideline이면서 방법론적 질이 높은 진료지침을 선택하기로 하였다. 주요 국가 및 기구에서 개발된 지침이면서 선정된 임상질문에 대한 권고를 포함하며 자주 업데이트 되고 있는 지침을 선정 대상으로 하였다.
○ 진료지침의 질 평가
진료지침의 질 평가는 AGREE 2 도구를 이용해서 3인씩 평가하였다. 평가자 간 변이를 줄이기 위하여 대한의학회에서 개발한 K-AGREE 평가서식을 활용하였다. AGREE 평가 시, 평가 결과의 재현성과 명확성을 확보하기 위하여 점수를 부여하는 데 근거가 된 내용을 코멘트 란에 기입하였으며, 평가자들의 평가 결과를 공유하며, 필요 시 오류나 실수에 의한 잘못된 평가 결과를 수정할 수 있는 재검토 과정을 거쳤다. 진료지침의 질평가 이후 AGREE 3개 영역에서 70점 이상을 받은 진료지침을 최종적으로 활용하기로 하였다.
○ 최신 근거의 검색 및 선택
기존 진료지침의 근거표에서 선택된 근거문헌을 검토하여 수용하며, 추가 문헌의 검색원으로는 PubMed, EMBASE, Cochrane CDSR, 국내 DB는 KMBASE를 이용해 검색하였다. 코로나19의 근거문헌 출판이 신속하게 진행되는 점을 고려하여 preprint DB 중 MedRxiv, bioRxiv를 이용하기로 하였으나 실제 일차 검색 후 결과를 보았을 때, 충분한 양의 문헌이 출판되고 있어 MedRxiv, BioRxiv는 검색결과를 검토는 하였으나 포함시키지 않기로 했다. 검색어는 진료지침의 검색전략을 활용하되 치료제별로 검색어를 선별하여 검색전략을 마련하였다.
코로나19 치료제에 대한 문헌이 지속적으로 업데이트되고 있어, 검색은 1개월 단위로 업데이트하여 주요 근거를 확인하여 권고 개정에 반영하였다. 지속적 근거 업데이트 관리를 위해 문헌 선별과정에는 유료 체계적문헌고찰 반자동화 소프트웨어인 Covidence를 구매하여 활용하였다.
지침 선별기준과 마찬가지로 2020년 6월 이후에 발간된 코로나19 치료제에 대한 연구 중 임상질문에 해당하는 치료법을 중재로 포함하고 있는 인간대상 연구를 선정대상으로 하였다. 연구설계의 경우는 무작위배정임상시험연구 또는 comparative design의 관찰연구를 포함하되 임상질문별로 근거의 양을 확인한 후 결정하기로 했다.
○ 선택한 일차문헌의 비뚤림 위험 평가
기존 진료지침의 근거표에서 비뚤림위험이 평가된 근거문헌은 기준에 맞는지 검토하여 가능한 한 기존의 비뚤림위험 평가 결과를 수용하였다. 추가 검색된 근거의 질평가는 연구디자인에 따라 적합한 도구를 선택하고, 문헌당 2명의 연구자가 독립적으로 평가하며 의견 불일치시 합의하고 이를 통해 합의가 안될 경우에는 제3자의 의견을 받아 합의했다.
▢ 연구 결과
○ 진료지침의 선정 및 평가
진료지침 검색과정에서 수기검색을 통해 찾은 총 11개 진료지침 중 7개에 대해 AGREE 평가를 시행하였고, 이후 DB 검색을 통해 찾은 5개 진료지침 중 1편은 수기검색에서 찾은 지침과 겹쳐 나머지 4개의 지침에 대해서 추가로 AGREE 평가를 시행했다. AGREE 평가 주요 영역 3개에서 70점 이상을 만족하면서 최신성을 고려했을 때, 4편의 진료지침이 최종 선택되었다. 다만 평가된 11편의 진료지침의 권고비교표를 작성해서 다른 나라 권고 동향을 참고하는데 활용하였다.
○ 근거표 작성 및 최신성 검토
선정한 진료지침에서 선택배제 기준에 따라 선정 문헌을 포함한 근거표를 작성하였다. 질평가는 기존 가이드라인의 질평가 결과를 재검토하여 수용하고, 적절하지 않은 경우는 재평가하였다. 2020년 11월 9일과 2020년 12월 9일에 검색한 문헌을 검토하여 근거표에 추가하였다.
○ 권고문 도출 과정
임상질문 담당 실무위원들이 근거요약과 권고고려사항에 대한 검토를 마치고 권고문 초안을 작성하였고, 전체 회의에서 최종 권고문과 근거수준, 권고등급을 결정하였다. 이후 자문위원과 검토위원의 외부검토를 서면으로 받고 재논의 후 최종 권고문을 완성하였다.

▢ Background
Despite the preventive efforts being made globally against coronavirus disease 2019, the most widely spread infectious disease in the world as of 2020, no definitive cure has been developed and the vaccine is in the early stages of development. While South Korea has been successful in protecting its citizens from large-scale COVID-19 breakouts, healthcare providers at the coalface are demanding rapid and evidence-based clinical practice guidelines for treating inpatients with pneumonia and infections of other organs. Currently, there are many ongoing clinical studies aiming to develop vaccines and cures around the world. Compared to the early phase of the COVID-19 pandemic, many studies on clinical results are being published on a daily basis now. Therefore, it is necessary to develop an evidence-based methodology as well as continuously check for additional evidence and update recommendations. Since no standard antiviral treatment method other than supportive care has been established, we aim to provide guidelines on helpful antiviral and other drug treatments based on the latest evidence on COVID-19 in light of its clinical manifestations and fatality rates.
▢ Objective
This study aimed to review evidence on the latest treatments for COVID-19 and outline evidence-based clinical practice guidelines for healthcare professionals; it also aimed to devise a guideline development methodology that can be applied quickly in different settings
▢ Methods
Regarding the guideline development methodology, we used the adaptation method wherein the latest guidelines from major countries and organizations are reviewed in a short period of time. In addition, we applied the living guidelines development methodology that is consistently being updated using the latest evidence. The guideline development process is described in the following sections.
○ Search practice guidelines
With the help of literature search specialists, we selected the database, devised a search strategy, and developed the process of literature search. We decided to include practice guidelines from papers published after June, 2020 or regularly updated living guidelines.
○ Search database
We searched international literature databases such as PubMed and EMBASE as well as major practice guideline databases and websites. We also searched the official websites of major government organizations, institutions, and conferences because the data on these resources are updated in real time.
○ Search strategy
We applied filters including disease and cure search words and practice guidelines to the PubMed and EMBASE databases. The search words were combinations of specific and informal words based on treatment terminologies from each clinical question : “coronavirus”, “novel coronavirus”, “novel coronavirus 2019”, “2019 nCoV”, “COVID-19”, “Wuhan coronavirus”, “Wuhan pneumonia”, “SARS-CoV-2”, “severe acute respiratory syndrome”, “treatment”, “therapy”, and “antiviral.”
○ Criteria for selection of guidelines
Since the studies released after June 2020 contain qualitatively and quantitatively reliable information on the COVID-19 treatment experience, we decided to select the latest practice guidelines with high methodological quality. We targeted guidelines developed by major countries and organizations that included recommendations for the selected clinical questions and are frequently updated.
○ Quality appraisal of guidelines
The quality of the practice guidelines was evaluated by three researchers using the AGREE (The Appraisal of Guidelines for Research and Evaluation) 2 tool. To reduce the variation between the evaluators, the K-AGREE evaluation form developed by the Korean Medical Association was used. During the AGREE evaluation, to ensure the reproducibility and clarity of the result, the evaluators were asked to describe the reason for awarding a score in the comment section, share evaluation results, and, if necessary, correct errors or mistakes via a review. After quality evaluation of the practice guidelines, we decided to use the guidelines that received more than 70 points in the three areas of AGREE.
○ Searching and selection of the latest evidence
The references selected from the reference tables of existing practice guidelines were reviewed before being included. For additional literature search, we used three international databases (PubMed, EMBASE, and Cochrane CDSR) and one domestic database (KMBASE). Considering the rapid publication of literature on COVID-19-related evidence, we initially decided to use two preprint databases, MedRxiv and bioRxiv. However, we decided to exclude them later because there was sufficient amount of literature from our primary search. For the search words, a search strategy was devised by selecting separate search words for each treatment.
As the literature on COVID-19 treatment is constantly being updated, the search will be updated on a monthly basis to check the main evidence and reflect the revisions of the recommendations. For constantly updating evidence, a semi-automated software for systematic review, Covidence, was purchased and used in the process of literature selection.
As with the guideline selection criteria, we decided to include studies on COVID-19 treatment that were published after June, 2020 and addressed the clinical questions on human subjects receiving interventions. For the study design, we decided to include randomized clinical trials or observational studies with comparative design after checking the amount of evidence for each clinical question.
○ Assessment of the risk of bias in the selected primary literature
The risk of bias of the references in the reference table of existing practice guidelines was reviewed to determine if they met the quality criteria. For the quality evaluation of the additionally searched evidence, an appropriate tool was selected according to the research design. Two researchers evaluated each paper independently. If they did not reach an agreement, an additional researcher’s input was used to help them arrive at a consensus.
○ Managing Conflicts of Interests
We set-up the policy of managing conflicts of interest (COI) and developed a format of COI disclosure including financial, intellectual and other potential conflicts. All members of guideline committee declared there were no potential COI related on interventions during the guideline development period.
▢ Results
○ Selection and evaluation of practice guidelines
Seven out of eleven practice guidelines found on manual search were subjected to the AGREE evaluation. As one of the five additionally searched practice guidelines overlapped with one of the guidelines found from the manual search, four more AGREE evaluations were performed. Finally, we selected four practice guidelines that are most up to date with a score of 70 or more in three major areas of the AGREE evaluation. A table of recommendations of 11 evaluated practice guidelines was prepared for comparison with the recommendation trends in other countries.
○ Preparation of evidence table and review of up-to-dateness
An evidence table of each clinical questions with the selected literature was prepared according to the selection and exclusion criteria for the practice guidelines. The results of quality evaluation of the existing guidelines were reviewed and reevaluated, if necessary. The studies retrieved on November 9 and December 9 were reviewed and added to the evidence table.
○ Recommendations and external review
The committee in charge of clinical questions reviewed the summary of evidence and recommendations; prepared the first draft of recommendations; and decided on the final recommendations, level of evidence, and recommendation level at a general meeting. Then, external reviews from advisory and review committees were received in writing. The final draft of recommendations was completed after another discussion.
