▢ 연구 배경
외과적 응급수술이 필요한 복막염, 장허혈·괴사, 장천공, 복강내출혈 등의 급성복증은 적시에(timely) 수술하면 회복할 수 있지만, 수 시간 이상 지연되면 합병증 발생 및 사망에까지 이를 수 있는 응급질환이다. 그러나 응급수술은 대부분 젊은 외과의사들이 당직으로 담당하였기에 환자의 중증도에 비해 의료진의 전문성 및 지속성은 낮을 수밖에 없었다. 게다가 주간 정규수술, 외래 및 진료 등 일상 업무를 담당하는 젊은 외과의사들이 응급실의 외과 응급환자까지 진료하다 보니 환자의 위급성은 높음에도 불구하고 진료의 우선순위는 낮아져서 신속하고 적절한 외과적 최종치료 연결이 어려운 상황이다. 선진국에서는 2000년대 초반 이후로 응급수술전담팀(Acute care surgeon, ACS) 분과를 구축하여 외과계 응급 및 중증환자의 진료를 24시간 365일 연속적으로 전문의 중심의 전담팀으로 담당해왔다. ACS 모델을 도입한 국가에서는 일관성 있게 신속한 치료, 합병증 감소 등 치료결과 및 진료의 질 개선을 보고하였으며, 재원기간 및 진료비용 감소와 외과의사 만족도 향상을 보고하였다. 국내에서도 외과전공의 부족, 응급수술 지연, 정규수술을 하는 외과의사의 야간당직근무의 피로도 등의 의료환경의 변화를 겪으며, 현재 응급의료체계의 문제를 해결할 뿐만 아니라 병원의 질을 향상시키고 응급환자의 안전을 담보하기 위해 기관별로 ACS를 도입하고 있다. 이에 지역별 외과의료 인프라의 차이가 큰 국내 특수한 의료환경을 고려하고 국내 응급의료제도에 적합한 한국형 외과응급의료체계를 제안하기 위하여 응급수술전담팀 제도의 종합적인 분석 및 평가가 필요하다.
▢ 연구 목적
본 연구에서는 국내외 응급수술전담팀 제도 및 효과의 종합적 분석 및 평가를 통해 한국형 외과응급의료체계 모델을 제시하고자 하며, 이를 위한 세부목표는 다음과 같다. 첫째, 외국에서 응급수술전담팀 제도 도입 후 임상경과 및 재정효과를 분석한다. 둘째, 국내 응급수술전담팀 운영 병원의 제도 도입 전후 치료성과를 분석한다. 셋째, 국내 외과계 응급수술의 현황을 분석한다. 넷째, 국내 외과계 응급의료시스템의 현황 파악 및 문제점을 분석한다. 다섯째, 국내 응급수술전담팀 운영 의료기관의 운영, 성과 및 개선 방향에 대한 의견을 수렴한다. 여섯째, 외과계 응급의료체계 모델 구축을 위한 전문가 합의를 도출한다.
▢ 연구 방법
◯ 제 1세부과제: 국내외 응급수술전담팀의 효과 분석
1. 선진국 ACS 도입성과의 문헌적 근거수준은 체계적 문헌고찰로 파악하였다. 핵심질문은 “외국의 응급수술전담팀제도 도입 전후 임상적 안전성, 유효성 및 경제성은 어떠한가?” 이었으며, 문헌선택과정을 통해 문헌합성이 가능한 경우 양적분석인 메타분석을 하였으며 불가한 경우 질적검토를 적용하였다.
2. 국내 ACS 운영병원 성과분석을 위하여 ACS 제도를 도입한 전국 3개 상급종합병원(서울아산병원, 삼성서울병원, 영남대학교병원)의 전자의무기록조사를 통해 급성복증으로 응급수술을 받은 환자들을 대상으로 ACS 도입 전후 임상결과를 성향점수를 이용하여 분석하였다.
◯ 제 2세부과제: 국내 외과응급의료체계 현황 파악 및 인식도 분석
1. 외과계 응급수술 현황파악을 위하여 전국의 응급의료기관으로부터 전송되는 진료관련 정보를 실시간으로 수집하여 구축한 시스템인 국가응급의료이용자료 시스템(NEDIS, National Emergency Department Information System)을 이용하여 외과응급환자의 응급실 이용 현황을 확인하였다.
2. 외과계 응급의료체계 현황조사를 위하여 45개 상급종합병원의 외과전문의를 대상으로 2차에 걸친 온라인설문조사를 수행하였다. 1차 설문조사는 45개 상급종합병원 외과 전문의 각 1인을 대상으로 하였으며, 2차 설문조사는 1차 설문조사 응답자 중 외과/중환자외과 또는 외상외과 전문의를 대상으로 수행하였다.
3. 응급수술전담제도를 운영하는 의료기관 근무자의 인식도를 분석하여 ACS 운영현황, 성과, 문제점 및 개선방안을 파악하기 위해 초점집단토론을 수행하였다. 대상은 응급수술전담팀 소속여부를 고려하여 각 5명으로 구성된 2개 집단 및 권역외상센터에 근무하는 외과전문의 5명 및 담당 정책당국자 1명으로 구성된 1개 집단이었다.
4. 외과계 응급의료체계모델구축을 위하여 2차에 걸친 델파이 조사를 통하여 전문가 의견을 수렴하였다. 학회 추천 21명 및 정책 및 보건전문가 9명으로 구성된 패널을 대상으로 외과계 응급의료 운영체계, 전담인력, 전담업무, 전담시설 및 재정지원에 대하여 의견을 파악 후 우선순위를 범주화하였다.
▢ 연구 결과
1. 선진국 Acute care surgery 도입 성과분석
1.1. 수술 후 합병증([OR], 0.69; 95% confidence interval [CI], 0.54-0.89; I2=67%) 및 복강경에서 개복으로의 전환율(OR, 0.43; 95% CI, 0.21-0.85; I2=68%)은 ACS 전담팀 도입 이전에 비해 이후에 유의하게 낮았다.
1.2. 응급실에 내원하여 외과의사가 최종치료를 결정하는 시간(-1.37 hours: 95% CI, -2.04 to –0.70) 및 수술실까지 가는 시간 (-2.43 hours: 95% CI, -4.02 to –0.83)과 외과응급환자의 병원재원일수(-0.53 days: 95% CI, -0.79 to –0.27)는 ACS 전담팀 도입 이후 유의하게 감소하였다.
2. 국내 Acute care surgery 운영병원 성과분석
2.1. ACS 운영 3개 병원의 총 2,200명의 환자를 후향적으로 분석한 결과, 응급실 내원 시점부터 응급수술 시작까지 걸린 시간은 ACS 도입 전 516분에서 도입 후 449분으로 67분이 단축되었고(p<0.001), 합병증 발생은 도입 전 38.9%에서 도입 후 31.2%로 감소하였다(p=0.002).
2.2. 응급수술 시간에 영향을 줄 수 있는 충수돌기 절제술 군을 제외한 기타 외과 응급수술을 받은 환자들을 대상으로 한 민감도 분석에서도 전체대상자와 유사하게 ACS 도입 후 응급수술까지 걸린 시간은 유의하게 단축되었다(도입 전 560분, 도입후 460분, p<0.001). 비악성종양군을 대상으로 하였을 때도 응급수술까지 걸린 시간은 ACS 도입 후 유의하게 단축되었으나, 내원 시 쇼크상태인 환자군만을 대상으로 하였을 때는 도입 전에 비해 도입 후 응급수술까지 걸린 시간의 차이는 유의하지 않았다.
3. 외과계 응급수술 현황분석
외과응급수술 현황을 연도별 및 지역별로 분석하였을 때 지역별 차이는 컸으며, 최근 5년간 큰 차이는 보이지 않았다. 2020년 1년간 총 응급수술건수는 서울 6,595건, 경기(서울 외 수도권) 6,654건, 강원 764건, 충남 1,848건, 충북 644건, 전북 1,361건, 전남 1,183건, 경북 1,403건, 경남 2,271, 제주 592건이 보고되었다.
4. 외과계 응급의료체계 현황 설문조사
4.1. 전체 45개 상급종합병원 중 응급수술전담팀/분과는 9개(20%), 외상전담팀/분과는 22개(48.9%), 중환자치료전담팀/분과는 19개(42.2%)에서 운영되고 있었다.
4.2. 대부분 ACS가 있는 경우 주간에 상주하는 전담전문의가 있었으나, ACS가 없는 경우 당직 on-call system으로 운영되었다. 야간에도 ACS가 있는 경우 절반이상이 전담전문의가 있었으나, ACS가 없는 경우 대부분 당직 on-call system으로 운영되었다.
4.3. ACS 유무에 따른 전담 CT 및 수술실 등 자원 운영의 차이는 없었다.
4.4. 응급실진료 지연 원인은 ACS 유무별로 차이가 있었다. ACS가 있는 경우 응급실도착 이후 외과진료까지 지연요인은 대부분은 진료이외의 원인이었으나, ACS가 없는 경우 외과 진료의 지연이 지연의 주요한 원인이었다. 외과진료이후 수술시작 지연 및 타병원 전원의 주요 원인으로는 ACS가 있는 경우는 수술실, 병실, 중환자실 등의 시설부족이었으나, ACS가 없는 경우는 시설이외에 수술집도의 부재 및 지정 지연 등이 원인이었다.
4.5. 응급/중환자외과 및 외상외과 소속 전문의들이 응답한 연간 외과 응급수술건수는 연 300례이상(76.5%)이 많았으나, ACS 전담의 시행수술비율은 25%미만이 52.9%를 차지하였다. 응급실 내원 시 1시간 내 외과전담의 진료는 주간 76.5%, 야간/휴일은 64.7% 이었다.
5. Acute care surgery 종사자 중심 인식도 분석
5.1. 응급수술전담팀 종사자를 대상으로 초점 집단 토론을 시행한 결과 종사자들은 현재 응급수술전담팀의 도입 이후 객관적으로 검증된 긍정적인 임상결과 뿐만 아니라 타진료과 혹은 전원 과정에서 의사소통의 일원화와 교육 등의 방면에서도 성과를 내고 있다고 응답하였다.
5.2. 그러나, 응급수술전담팀의 장기적인 운영이 불투명하다고 여겨질 정도로 심각하다고 판단하고 있었으며 이는 부족한 인력과 시설, 재정, 비효율적 전원시스템 등 총체적인 원인에서 비롯된다고 생각하고 있었다.
5.3. ACS 운영의 문제점을 개선하기 위한 노력으로 시장과 경제 논리에 의존하기보다는 제도적 보완과 환자 안전 측면에서의 접근을 제시하고 있었으며 이는 수가 체계 개선이나 추가 지원방안 등의 재원 확보 및 신규 인력 채용과 기존 인력의 재배치를 통한 근무시간의 분배 등을 제안하였다. 특히 전원 네트워크의 효율성 제고를 바탕으로 한 중증 환자 진료병원 혹은 지역 거점 병원 설립 등의 방안을 제시하였다.
5.4. 권역 외상센터 근무자들은 외상센터 운영하는 방식의 장단점을 적절히 검토하는 것이 필요하며, 국가 지원 시 지나친 규제가 어려움이 될 수 있음을 제시하였다. ACS와 외상센터와의 통합 운영 시 ACS의 원개념에 맞는 의료 시스템의 확대나 기존의 확충된 인프라를 활용할 수 있다는 장점이 있으나 현행 수가 체계에서 발생할 것으로 예상되는 큰 폭의 적자나 행정적 문제점에 대한 검토가 필요하다고 응답하였다.
6. 외과계 응급의료체계 모델구축을 위한 전문가 델파이조사
6.1. 외과응급의료 운영체계로는 권역별 응급수술환자 전원시스템 구축이 가장 높았고(합의가능성 평균 4.47점, 순위기준 총점 86.3점), 응급수술 역량은 전담의료진(평균 3.79점, 총점 81.1점), 전담시설 및 환자순 기준 평가 및 권역별 외과응급의료 치료가능 병원 지정(평균 3.79점, 총점 72.6점)의 순으로 높았다.
6.2. 외과응급의료 전담인력 및 업무에 대해서는 응급실 전담근무 후 충분한 휴식시간 보장(중요도 평균 9.16점, 순위기준 총점 88.4점), 응급수술 전담의사의 24시간 상시대기(평균 7.79점, 총점 72.6점), 응급수술 전담외과의사로 구성된 전담부서(평균 7.74점, 총점 64.2점) 의 순으로 높았다.
6.3. 외과응급의료 전담시설 요구도는 외과응급환자 전용 수술실 확보(중요도 평균 8.63점, 순위기준 총점 95.8점), 중환자실 병상확보(평균 8.26점, 총점 81.1점)의 순으로 높았다.
6.4. 외과응급의료체계의 재정지원 중 수가제도 전반에 대해서는 중요도 평균순서로 1) 정부의 지속성 있는 재정기반 제도마련(중요도 평균 8.79점, 순위기준 총점 71.6점), 2) 수가 난이도, 중증도 등에 따른 가산 필요(평균 8.74점, 총점 66.3점), 3) 외과 및 수술관련 수가 인상 필요(평균 8.42점, 총점 80.0점), 4) 새로운 수가제도 개발 필요(평균 7.79점, 총점 62.1점)의 순이었다.
6.5. 응급수술 인력 비용 지원과 관련하여서는 1) 차등지원 및 인센티브 지급(중요도 평균 8.26점, 순위기준 총점 67.4점), 2) 전담의료진에게 직접 인건비 지원(평균 7.89점, 총점 80.0점), 3) 인건비 등의 고정비용을 정부가 지원(평균 7.79점, 총점 71.6점) 등의 순이었다.
6.6. 응급수술 시설/장비보전 비용에서는 1) 국가가 응급수술 시설(수술실, 중환자병상 등) 및 치료장비 구입 및 운영 위한 비용 지원(중요도 평균 8.84점, 순위기준 총점 94.7점), 2) 통합사용 병상, 장비 등 지원(평균 7.95점, 총점 77.9점) 등의 순이었다.
▢ 결론 및 정책적 제언
외과응급질환은 적시에 치료받지 못하면 생명을 위협할 수 있는 중증응급질환이다. 외과응급질환은 최종치료 즉 응급수술을 적시에 받아야 비로소 치료효과를 볼 수 있다. 따라서 기존 응급의료체계가 응급환자의 관문인 응급실 내 의료인프라 의료/장비/서설에 투자했다면 이제는 본격적으로 최종치료로 이어질 수 있는 응급수술 의료인프라에 투자를 해야 할 시점이다.
본 연구를 통해 해외 뿐 아니라 외과응급수술전담팀을 꾸린 병원에서의 결과에서도 수술시간 단축 및 합병증이 감소되었음을 확인하였으며, 상급종합병원 대상 설문조사 및 초점집단토론에서도 공통적으로 응급환자의 전원사유는 중환자실, 수술실 부족 및 수술의사 부재가 대부분 원인으로 분석되었으며 이제는 과중한 업무가중 등으로 응급수술전담외과의사 및 전담팀의 필요성에 공감대를 형성하고 있었다. 또한 전문가 델파이 조사에서도 응급수술을 위한 시설,인력 지원이 필요하다는 의견에 동의하였다.
외과응급의료체계를 갖추기 위해서는 응급수술전문인력, 응급수술전용 수술실 및 중환자실 등은 필수적으로 마련되어야 한다. 특히 외과의사의 경우 지금과 같이 정규수술/외래/진료 등 일상업무를 하면서 응급수술을 하면 극심한 업무가중, 응급수술지연, 전문성 결여 등으로 위중한 환자를 대상으로 하는 외과응급의료체계의 발전을 기대하기 어렵다. 이미 선진국에서는 이 문제를 해결하기 위해 응급수술, 중환자 집중치료, 외상 등 세 분야의 외과 내 중증, 응급환자들을 진료하는 Acute care surgery 분과를 운영하고 있으며 그 성과는 매우 좋아 전세계적으로 확장되고 있으며 국내에서도 운영하는 병원이 증가하고 있다.
국내 상황은 외과전공의 지속적 감소 및 지역간 인력불균형이 심해 외과의료인프라가 매우 부족하다. 따라서 지역 의료인프라에 맞는 외과응급의료시스템 구축이 필요하다. 제한적인 외과필수의료 인력자원을 최대한 활용하기 위해 권역(지역)별 외과 응급수술이 필요한 중증환자의 수에 따라 외과응급진료가 가능한 병원을 레벨(I,II,III)로 지정해 지원하며, 네트워크를 형성하여 권역(지역)내 외과 응급의료체계를 함께 대처하여야 한다.
또한 이 시스템이 체계적이고 안정적으로 유지되기 위해 외과응급의료체계에 대한 지원이 필요한데 응급수술건수가 적어 단순한 수가인상만으로는 시스템 유지가 불가하여 응급수술에 필요한 인력/시설/장비를 보존할 수 있는 관리비와 함께 위험도 및 중증도가 높은 수술에 대한 수가 인상이 함께 이루어져야 한다.
필수영역인 응급수술이 필요한 외과계응급의료는 이제는 각 의료기관의 역량에 맡길 것이 아니라 현재 응급의료체계 개편을 통해 최종응급질환의 최종치료까지 이어질 수 있는 응급의료체계의 전반적인 개편이 필요하다.

▢ Background
Until recently, the most common delivery model for the care of these patients centered around a surgeon who was required to manage all surgical emergencies for a 12- to 24-hour interval, while concurrently working within the demands of a scheduled clinical practice. Although based in tradition, this system has multiple limitations, including interference with and required time away from a busy “scheduled” subspecialty practice; providing emergent surgery coverage throughout the night, with the high likelihood of still needing to engage in patient care during a busy “post-call” day; and a potential lack of coordinated and current academic expertise within the specific focus of acute care surgery.
The concept of acute care surgery (ACS) incorporates trauma, surgical critical care, and emergency general surgery. These conditions of emergency surgery include, but are not limited to, acute appendicitis, cholecystitis, diverticulitis, pancreatitis, intestinal obstruction, intestinal ischemia, intra-abdominal sepsis, incarcerated hernias and perforated viscous. These field require high quality, timely, and comprehensive care of critically ill surgical patients.
In Korea, with many social accident with delay of emergency surgery, the need to implementation of emergency surgical system is increasing. several institution already are adapting ACS system. Considering korean surgical emergency infrastructure, implementation of korea surgical emergency system in necessary.
▢ Objective
The aim of this study is to suggest Korean emergency surgery system through analysis the clinical effects of implementation of acute care surgery and reference to expert opinion. First, to analyze clinical outcomes and financial effects after the implementation of ACS system in foreign countries. Second, to analyze the clinical outcomes before and after the implementation of ACS system in Korea. Third, we analyze the current status of emergency surgery in Korea. Fourth, we analyze the current status and problems of Korean emergency surgery system. Fifth, we collect opinions on the operation, performance, and improvement direction of Korean ACS hospitals. Sixth, we derive expert consensus to suggest Korean emergency surgery system.
▢ Methods
◯ Subtask 1: Analyze the effectiveness of acute care surgery (ACS) in Korea and abroad
1. The level of evidence on the effectiveness of ACS in developed countries was identified through a systematic review. The key question was, "What are the clinical safety, effectiveness, and economic benefits before and after the implementation of emergency surgery team systems in foreign countries?" Through the literature selection process, meta-analysis, a quantitative analysis, was performed when it was possible to synthesize the literature, and a qualitative review was applied when it was not possible.
2. To analyze the performance of ACS hospitals in Korea, the clinical outcomes before and after the implementation of ACS were analyzed using propensity scores for patients who underwent emergency surgery for acute abdomen through an electronic medical record survey of three tertiary general hospitals that introduced the ACS system.
◯ Subtask 2: Identification of the current status of the Korean surgical emergency medical system and analysis of awareness
1. The National Emergency Department Information System (NEDIS), a system that collects treatment-related information transmitted from emergency medical institutions nationwide in real time to identify the current status of surgical emergency surgery, was used to check the emergency room utilization status of surgical emergency patients.
2. To investigate the current status of the surgical emergency medical system, we conducted two online surveys of surgical specialists at 45 tertiary general hospitals. The first survey was conducted with one surgeon from each of the 45 hospitals, and the second survey was conducted with surgeons specializing in surgery/critical care surgery or trauma surgery among the respondents of the first survey.
3. Focus group discussions were conducted among employees of ACS hospitals to identify the current status, performance, problems, and improvement measures. The subjects were two groups of five people each, considering whether they belonged to an emergency surgery team, and one group of five surgeons working in regional trauma centers and one policy official.
4. Expert opinions were collected through two Delphi surveys to establish the surgical emergency medical system model. A panel of 21 academics and 9 policy and health experts were asked for their opinions on the operational system, dedicated personnel, dedicated tasks, dedicated facilities, and financial support for surgical emergency medicine, and then categorized the priorities.
▢ Results
1. Analyze the performance of acute care surgery adoption in developed countries
1.1. Postoperative complications (odds ratio [OR], 0.69; 95% confidence interval [CI], 0.54-0.89; I2=67%) and laparoscopic to open conversion rates (OR, 0.43; 95% CI, 0.21-0.85; I2=68%) were significantly lower after the introduction of dedicated ACS teams compared to before.
1.2. The time from ER presentation to surgeon's final treatment decision (-1.37 hours: 95% CI, -2.04 to -0.70) and time to operating room (-2.43 hours: 95% CI, -4.02 to -0.83) and length of stay for surgical emergencies (-0.53 days: 95% CI, -0.79 to -0.27) were significantly reduced after the introduction of the ACS team compared to before.
2. Clinical outcomes of acute care surgery hospitals in Korea
2.1. In a retrospective analysis of a total of 2,200 patients among three ACS hospitals, the time from ER visit to the start of emergency surgery was reduced by 67 minutes, from 516 minutes before ACS to 449 minutes after ACS (p<0.001), and complications was reduced from 38.9% before ACS to 31.2% after ACS (p=0.002).
2.2. In a sensitivity analysis of patients who underwent other surgical emergencies, excluding the appendectomy group, which could affect emergency surgery time, the time to emergency surgery was significantly reduced after ACS (560 minutes pre-ACS, 460 minutes post-ACS, p<0.001) compared to before ACS, similar to the overall population. However, when only patients who were in shock at the time of visit were included, the difference in time to emergency surgery was not significant.
3. Analysis of surgical emergency surgery
When the status of surgical emergency surgeries was analyzed by year and region, there were large regional differences, and there was no significant difference in the last five years. The total number of emergency surgeries in 2020 was 6,595 in Seoul, 6,654 in Gyeonggi (metropolitan area outside Seoul), 764 in Gangwon, 1,848 in Chungnam, 644 in Chungbuk, 1,361 in Jeonbuk, 1,183 in Jeonnam, 1,403 in Gyeongbuk, 2,271 in Gyeongnam, and 592 in Jeju.
4. Survey on the status of surgical emergency system
4.1. Among forty-five tertiary general hospitals, emergency surgery teams/divisions were operated in 9(20%) hospitals, trauma teams/divisions in 22 (48.9%) hospitals, and critical care teams/divisions in 19 (42.2%) hospitals.
4.2. Most centers with an ACS had a resident specialist during the day, but without an ACS, the system operated as an on-call system. More than half of the centers with ACS had a dedicated specialist at night, but most without ACS operated as an on-call system.
4.3. There was no difference in the use of resources such as dedicated CT and operating rooms with and without ACS.
4.4. Reasons for delays in ER differed by ACS status. In the case of ACS, most of the delays from ER arrival to surgical care were due to non-medical reasons, but in the case of no ACS, the delay in surgical care was the main cause of delay. The main causes of delayed surgical start and transfer to other hospitals were lack of facilities such as operating rooms, wards, and intensive care units in cases with ACS, but in cases without ACS, the main causes were absence of a surgeon and delay in appointment in addition to facilities.
4.5. The number of annual surgical emergency surgeries responded to by specialists in emergency/critical care surgery and trauma surgery was more than 300 per year (76.5%), but the proportion of surgeries dedicated to ACS of less than 25% was 52.9%. ER visits seen by an attending surgeon within one hour was 76.5% during the day, and 64.7% during the night/holidays.
5. Awareness analysis of in ACS hospitals staffs
5.1. As a result of conducting focus group discussions with emergency surgery team members, the staff said that ACS has achieved positive objectively verified clinical outcomes since its introduction, as well as unified communication and education in other departments or throughout the entire process.
5.2. However, the long-term operation of the ACS is considered to be unclear, and they believe that this is due to a number of factors, including insufficient manpower, facilities, finances, and inefficient transfer systems.
5.3. Rather than relying on market and economic logic to improve the problems of ACS operations, they suggested an approach from the perspective of institutional strengthening and patient safety, which includes securing financial resources such as improving the payment system, additional support measures, and distributing working hours by recruiting new personnel and reassigning existing personnel. In particular, they suggested establishing critical care hospitals or regional base hospitals based on improving the efficiency of the transfer network.

Regional trauma center staffs suggested that it is necessary to properly examine the advantages and disadvantages of the way trauma centers were operated, and that over-regulation may be a challenge when providing government support. The advantages of integrating ACS with trauma centers are that it can expand the medical system in line with the original concept of ACS and utilize the existing expanded infrastructure, but it is necessary to review the large deficits and administrative problems that are expected to occur in the current payment system.
6. Expert delphi survey for Korean surgical emergency system
6.1. In terms of surgical emergency operation system, the establishment of transfer system for emergency surgical patients by region was the highest (average of 4.47 points for agreeability and total score of 86.3 points based on ranking), followed by dedicated medical staff (average of 3.79 points and 81.1 points), evaluation of dedicated facilities and patients by order, and designation of hospitals for surgical emergency medical treatment by region (average of 3.79 points and 72.6 points).
6.2. Regarding the staffing and tasks dedicated to surgical emergency, the important factors were ensuring adequate rest time after working in the ER (average of 9.16, total score of 88.4), 24-hour availability of emergency surgeons average of 7.79, total score of 72.6), and a dedicated department with surgeons dedicated to emergency surgery (average of 7.74, total score of 64.2).
6.3. The demand for facilities dedicated to surgical emergency care was high in order of dedicated operating rooms for surgical emergency patients (average of 8.63, total score 95.8) and intensive care beds (average of 8.26, total score 81.1).
6.4. Regarding the overall payment system among the financial support for the surgical emergency, in order of importance: 1) Establishment of a sustainable financial-based system by the government (average of 8.79 points, total score of 71.6 points), 2) Need to reinforce additional payment according to the difficulty and severity of the case (average of 8.74 points, total score of 66.3 points), 3) Need to increase surgical and surgery-related fees (average of 8.42 points, total score of 80.0 points), and 4) Need to develop a new payment system (average of 7.79 points, total score of 62.1 points).
6.5. Regarding the cost of emergency surgery personnel, the top three options were 1) differential support and incentives (average of 8.26, total score of 67.4), 2) direct support for personnel expenses to dedicated medical staff (average score of 7.89, total score of 80.0), and 3) government support for fixed costs such as personnel expenses (average score of 7.79, total score of 71.6).
6.6. For emergency surgical facilities/equipment maintenance costs, 1) the government supports the purchase and operation of emergency surgical facilities (operating rooms, intensive care beds, etc.) and treatment equipments (average score of 8.84, total score of 94.7 based on ranking), and 2) the government supports beds and equipment for integrated use (average of 7.95, total score of 77.9).
▢ Conclusions
Surgical emergencies are severe emergencies that can be life-threatening if not treated in a timely manner. Surgical emergencies can only be cured by receiving final treatment, i.e. emergency surgery, in a timely manner. Therefore, if the existing emergency system has invested in medical infrastructure medical care, equipment, and facilities in the emergency room, which is the gateway to emergency patients, it is time to invest in emergency surgery infrastructure that can lead to final treatment in earnest.
Through this study, we have confirmed that the results of ACS hospitals have shortened surgery time and reduced complications both at home and abroad. In the survey and focus group discussion of tertiary general hospitals, the reasons for transferring emergency patients were due to the lack of intensive care units, operating rooms, and absence of surgeons, and now there is a consensus on the need for specialized surgeons and dedicated teams due to overloaded workloads. In addition, the expert Delphi survey agreed that facilities and manpower support for emergency surgery are needed.
In order to have a surgical emergency system, it is essential to have emergency surgery specialists, operating rooms for emergency surgery, and intensive care units. In particular, it is difficult to expect the development of a surgical emergency system that targets critical patients due to extreme workload, delayed emergency surgery, and lack of expertise if surgeons perform emergency surgery while performing their daily duties such as elective surgery, and outpatient treatment. To solve this problem, developed countries are already operating ACS system that treat severe and emergency patients in three areas of surgery: emergency surgery, intensive care, and trauma, and the results are so good that they are expanding around the world, and the number of hospitals operating them in Korea is increasing.
In Korea, the surgical emergency infrastructure is severely lacking due to the continuous decline of surgical residents and the imbalance of manpower between regions. Therefore, it is necessary to establish a surgical emergency system that fits the local medical infrastructure. In order to make the most of limited surgical manpower resources, hospitals that can provide surgical emergency care are designated as levels (I, II, and III) according to the number of critically ill patients requiring surgical emergency surgery per region, and a network should be formed to deal with the surgical emergency system in the region.
In addition, support for the surgical emergency system is necessary to maintain the system in a systematic and stable manner, but the system cannot be maintained by a simple increase in the number of emergency surgeries due to the low number of emergency surgeries, so a fee increase for high-risk and high-severity surgeries should be made together with a management fee to preserve the personnel, facilities, and equipment required for emergency surgery.
Surgical emergency care, which is an essential area requiring emergency surgery, should not be left to the capabilities of each medical institution as it is, but an overall reorganization of the emergency system that can lead to the final treatment of emergency diseases is needed through the reorganization of the current emergency medical system. Implementation national surgical emergency system considering regional emergency surgical infrastructure in necessary.
