평가배경
갑상선암 수술, 위암 수술, 대장암 수술, 충수절제술, 자궁절제술 등의 수술은 관혈적, 내시경 또는 복강경에 의한 방식으로 수행되며, 해당 중재에는 1회용 절삭기가 이용되고 있다. 국내에서 이용되는 1회용 절삭기에는 초음파, 전파, 전파와 초음파, 다관절 다자유도 바이폴라 절삭기가 있으며 내시경, 복강경 또는 관혈적 수술에서 해당 에너지를 이용하여 조직의 지혈, 절개, 응고에 사용한다. 이 중 1회용 전파 절삭기의 경우 복강경 등 내시경하 수술용으로 2014년 12월 일체형은 선별급여 80%, tip 교체형은 50%로 최초 등재되었고, 관혈적 일체형의 경우 2015년 4월 선별급여 80%로 최초 등재된 이후 현재까지 사용되고 있다.
건강보험심사평가원에서는 선별급여의 적합성 평가를 위해 4종의 1회용 절삭기 사용(1회용 초음파 절삭기, 1회용 전파 절삭기, 1회용 전파와 초음파 절삭기, 1회용 다관절 다자유도 바이폴라 절삭기)의 임상적 효과성에 대한 근거 확인을 위하여 의료기술재평가를 본 원에 의뢰하였고(2022.9.6.), 2022년 제10차 의료기술재평가위원회(2022.10.14.)에서 전문적·심층적 검토를 통해 동 기술의 임상적 안전성 및 효과성 평가가 필요한 안건으로 심의하였다. 이에 본 평가는 체계적 문헌고찰을 통해 1회용 전파 절삭기의 임상적 안전성 및 효과성의 근거를 확인하여 정책적 의사결정을 지원하고자 수행하였다.
평가방법
동 기술에 대한 안전성 및 효과성 평가를 위해 체계적 문헌고찰을 수행하였다. 모든 평가방법은 평가목적을 고려하여 “1회용 절삭기를 이용한 수술 소위원회(소위원회)”의 심의를 거쳐 확정하였다. 평가의 핵심질문은 “갑상선암 수술, 위암 수술, 대장암 수술, 충수절제술, 자궁절제술 대상자에서 1회용 전파 절삭기 사용은 임상적으로 안전하고 효과적인가?”이며, 안전성은 합병증 및 이상반응 지표로, 효과성은 혈액 소실량, 수술 시간, 재수술률, 배액량, 재원 기간을 지표로 평가하였다.
체계적 문헌고찰은 핵심 질문을 토대로 국외 3개, 국내 5개 데이터베이스에서 검색하여 문헌 선정 및 배제기준에 따라 두 명의 검토자가 독립적으로 선별하고 선택하였다. 선택 문헌의 비뚤림위험 평가는 Cochrane의 Risk of Bias (RoB)를 사용하여 두 명의 검토자가 독립적으로 수행하여 의견합의를 이루었다. 자료 분석은 정량적 분석(quantitative analysis)이 불가능한 경우 정성적(qualitative review) 분석을 적용하였다. 체계적 문헌고찰 결과의 근거 수준은 Grading of Recommendations Assessment, Development and Evaluation (GRADE) 접근 방법으로 평가하였다.
평가결과
1회용 전파 절삭기 평가에 포함된 문헌은 총 31편의 무작위배정 임상시험으로 갑상선암 수술, 위암 수술, 대장암 수술, 충수 절제술, 자궁 절제술을 받은 환자를 대상으로 1회용 전파 절삭기와 기존 방법(전기 소작기, 봉합 등)을 비교하여 안전성 및 효과성을 평가하였다.
안전성
1회용 전파 절삭기의 안전성은 모든 선택 문헌에서 보고하였으며, 1회용 전파 절삭기를 이용한 수술군과 기존 지혈법(전기 소작기, 봉합 등)을 이용한 군의 연구대상자는 각각 1,679명, 1,672명이었다.
문헌에서 보고된 합병증은 대부분 수술 적응증에 따른 합병증으로 전파 절삭기 이용에 따른 부작용은 별도로 보고되지 않았다. 소위원회에서는 모든 수술에서 전체 및 세부 합병증 중 출혈, 혈관 손상을 가장 핵심적인 지표로 평가하였고, 갑상선암 수술에서는 재발성 혹인 일시적 후두신경 마비를 핵심적인 지표로 평가하였다. 그 결과 중재군과 기존 지혈법 사이의 합병증 발생 차이는 없었다. 또한, 전체 합병증을 보고한 18편의 문헌을 대상으로 메타분석한 결과, 1회용 전파 절삭기를 이용한 수술군과 기존 방법을 이용한 수술군 사이의 합병증 발생 차이는 없었다(Risk ratio (RR)=1.27; 95% confidence interval (CI) 0.99, 1.62; I2=0%).
효과성
1회용 전파 절삭기의 효과성은 혈액 소실량, 수술 시간, 재수술률, 배액량, 재원 기간을 결과 지표로 평가하였다.
혈액 소실량을 보고한 문헌은 24편으로 이 중 16편에 대해 메타분석을 수행한 결과, 1회용 전파 절삭기를 이용한 수술군이 기존 방법을 이용한 수술군에 비해 혈액 소실량이 통계적으로 유의하게 적었으나 통계적 이질성이 높아 해석에 주의가 필요하였다(Mean difference (MD)=-45.25ml; 95% CI -79.5, -11.0; I2=97%).
모든 문헌에서 수술 시간을 보고하였으며 이 중 22편에 대해 메타분석을 수행한 결과, 1회용 전파 절삭기를 이용한 수술군이 기존 방법을 이용한 수술군에 비해 수술 시간이 통계적으로 유의하게 단축하였으나 통계적 이질성이 높아 해석에 주의가 필요하였다(MD=-19.78분; 95% CI -28.1, -11.5; I2=98%).
재수술률을 보고한 문헌은 3편으로 군간 통계적으로 유의한 차이는 없었다(RR=0.48; 95% CI 0.02, 10.38; I2=0%).
배액량을 보고한 3편의 문헌 중 2편에서는 1회용 전파 절삭기를 이용한 군과 기존 방법을 이용한 수술 간 통계적으로 유의한 차이는 없었다. 나머지 1편의 연구에서 1회용 전파 절삭기를 이용한 수술군의 배액량이 증가하였으며 명확한 원인을 알 수 없으나 양극성 응고 조직에 인접한 염증 반응을 원인으로 보았다.
재원 기간을 보고한 문헌은 22편으로 이 중 13편에 대해 메타분석을 수행한 결과, 1회용 전파 절삭기를 이용한 수술군은 기존 방법을 이용한 수술군과 재원 기간의 차이는 없었다(MD=-0.10일; 95% CI -0.19, -0.01; I2=16%).
효과성 지표 중 혈액 소실량, 수술 시간, 배액량, 재원 기간은 연속형 변수로 제시하여 평균 및 표준편차로 자료추출이 이루어졌으나 몇몇 문헌에서는 중위수 및 범위, 신뢰구간, 표준오차 등으로 제시하여 이를 표준편차로 추정하여 민감도 분석을 수행하였으며 일관된 결과를 확인하였다.
결론 및 제언
소위원회에서는 현재 문헌적 근거를 바탕으로 다음과 같이 결론을 제시하였다.
갑상선암 수술, 위암 수술, 대장암 수술, 충수절제술, 자궁절제술 등에서 1회용 전파 절삭기를 이용한 수술을 기존 지혈법을 이용한 수술과 비교하였을 때, 전파 절삭기 장치에 따른 부작용은 확인되지 않았고, 합병증 및 이상반응 발생률은 유사한 수준이므로 1회용 전파 절삭기를 이용한 수술을 안전한 기술로 평가하였다. 그리고 1회용 전파 절삭기를 이용한 수술은 기존 방법과 비교하여 재수술률, 배액량, 재원 기간은 유사한 수준이고, 기존 지혈법에 비해 혈액 소실량과 수술 시간은 다소 감소하는 경향을 보여 1회용 전파 절삭기를 이용한 수술은 효과적인 기술이라고 평가하였다.
2023년 제5차 의료기술재평가위원회(2023.5.12.)에서는 소위원회 검토 결과에 근거하여 의료기술재평가사업 관리지침 제4조 제10항에 의거 “1회용 전파 절삭기”에 대해 다음과 같이 심의하였다.
의료기술재평가위원회는 임상적 안전성과 효과성의 근거 및 그 외 평가항목 등을 종합적으로 고려하였을 때, 국내 임상상황에서 수술 시 1회용 전파 절삭기의 사용을 ‘조건부 권고함’으로 심의하였다.

Background
Advanced vessel-sealing systems have been developed and widely adopted in various specialties, including thyroid cancer surgery, gastric cancer surgery, colorectal cancer surgery, appendectomy, and hysterectomy. In Korea, the spectrum of disposable scalpels encompasses ultrasonic, bipolar, and ultrasonic and bipolar models, in addition to multi-degree of freedom articulating laparoscopic instruments. Among these, the energy-based vessel sealing systems, particularly for endoscopic and laparoscopic surgeries, have been continually utilized since their designation as selective benefit items: the integrated type as an 80% co-payment item in December 2014, the replaceable tip type as a 50% co-pay item in December 2014, and the integrated type for open surgery as an 80% co-pay item in April 2015.
In 2022 (September 6, 2022), the Health Insurance Review and Assessment Service (HIRA) requested a reassessment of four types of advanced vessel-sealing systems (ultrasonic devices, energy-based vessel sealing systems, combination devices, multi-degree of freedom articulating laparoscopic instrument). These technologies were selected to be health technology reassessment agenda at the 10th Health Technology Reassessment Committee in 2022 (October 14, 2022). Accordingly, this assessment performed whether energy-based vessel sealing system (such as LigaSure) is clinically safe and effective in patients requiring surgery.
Methods
A systematic review was conducted to evaluate the safety and effectiveness of this advanced health technology. All assessment methods were finalized after deliberation by the “Advanced vessel-sealing systems Subcommittee” (“Subcommittee”), with consideration of the purpose of the assessment. The Subcommittee consisted of 7 experts: four surgeons, one obstetrician and gynecologist, one anesthesia and pain medicine, and one evidence-based medicine specialist.
The key question of the assessment was as follows: “Are energy-based vessel sealing systems clinically safe and effective for use in thyroid cancer surgery, gastric cancer surgery, colorectal cancer surgery, appendectomy, and hysterectomy?” Safety was based on complications and adverse events (AEs), and effectiveness was assessed based on blood loss volume, operating time, reoperation rate, drainage volume, and hospital stay length. Considering the purpose of this assessment, the study type was limited to randomized controlled trials (RCTs).
For the systematic review, three international and five domestic databases were searched based on the key question. The literature selection process involved two reviewers independently conducting the selection process according to predefined selection and exclusion criteria. The risk of bias (RoB) in the included studies was assessed two reviewers independently. All data were extracted by study unit. Any disagreements were resolved through a discussion with a third person to reach a consensus. Data were qualitatively analyzed if quantitative analysis was not possible. The systematic review results were assessed for the level of evidence using the Grading of Recommendations Assessment, Development, and Evaluation (GRADE) method.
Results
The systematic review incorporated a total of 31 randomized controlled trials (RCTs). The safety and effectiveness of these energy-based vessel sealing systems were evaluated in comparison to conventional surgical methods (such as electrocautery and suturing) across various surgeries, including thyroid cancer surgery, gastric cancer surgery, colorectal cancer surgery, appendectomy, and hysterectomy.
Safety
The safety of energy-based vessel sealing system was assessed based on 31 RCTs. Most complications were in accordance with surgical indications, there were no adverse events due to energy-based vessel sealing. The Subcommittee determined that bleeding and vascular injury were critical indicators. In thyroid cancer surgery, the incidence of recurrent or transient laryngeal nerve paralysis was evaluated as a critical indicator. The results showed no statistically significant differences between the intervention group and the control groups. As a meta-analysis including of 18 studies reporting overall complications, there was no differences in complication rates between the energy-based surgery group and the conventional surgery group (risk ratio [RR]=1.27; 95% confidence interval [CI] 0.99, 1.62; I2=0%).
Effectiveness
A meta-analysis of 16 of 24 studies reporting on blood loss found that the disposable energy-based vessel sealing group had statistically significantly less blood loss than the control group, but interpretation was cautious due to high heterogeneity (mean difference [MD]= -45.25 mL; 95% CI -79.5, -11.0; I2=97%).
All studies reported operating time, and a meta-analysis was performed on 22 of these. The results showed that the operative time was statistically significantly shorter in the energy-based group than in the control group. However, this finding also exhibited high statistical heterogeneity (MD= -19.78 minutes; 95% CI -28.1, -11.5; I2=98%).
The reoperation rate, a factor reported in three studies, showed no statistically significant difference between the two groups (RR=0.48; 95% CI 0.02, 10.38; I2=0%).
Regarding drainage volume, which was reported in three studies, two articles indicated no statistically significant difference between the intervention and control groups. However, one study reported an increased drainage volume in the energy-based group, which was interpreted to be an inflammatory response near the bipolar coagulation tissue.
In the meta-analysis of 13 studies out of 22 that reported on the length of hospital stay, no significant difference was observed between the energy-based group and the control groups (MD=-0.10 days; 95% CI 0.19, -0.01; I2=16%).
Conclusions and recommendations
Based on this assessment, the Subcommittee evaluated the following findings and opinions.
In surgeries such as thyroid cancer surgery, gastric cancer surgery, colorectal cancer surgery, appendectomy, and hysterectomy where a disposable energy-based vessel sealing system was employed, there were no complications associated with the energy-based systems. The incidence of other complications and AEs was found to be similar to that in surgeries using conventional hemostatic methods. Moreover, the energy-based group had similar reoperation rate, drainage volume, and length of hospital stay as the conventional group, and showed a slight reduction in blood loss and operative time. Thus, it was determined that the use of disposable energy-based vessel sealing systems is safe and effective.
The 5th Health Technology Reassessment Committee of in 2023 (on May 12, 2023) comprehensively reviewed findings based on the conclusions of the Subcommittee review.
The Committee determined the level of recommendation for the use of disposable energy-based vessel sealing systems as “conditionally recommends” based on the clinical settings and values in Korea.
