평가배경
갑상선암 수술, 위암 수술, 대장암 수술, 충수절제술, 자궁절제술 등의 수술은 관혈적, 내시경 또는 복강경에 의한 방식으로 수행되며, 해당 중재에는 1회용 절삭기가 이용되고 있다. 국내에서 이용되는 1회용 절삭기에는 초음파, 전파, 전파와 초음파, 다관절 다자유도 바이폴라 절삭기가 있으며 내시경, 복강경 또는 관혈적 수술에서 해당 에너지를 이용하여 조직의 지혈, 절개, 응고에 사용한다. 이 중 1회용 다관절 다자유도 바이폴라 절삭기의 경우 2019년 5월 선별급여 80%로 최초 등재된 이후 현재까지 사용되고 있다.
건강보험심사평가원에서는 선별급여의 적합성 평가를 위해 4종의 1회용 절삭기 사용(1회용 초음파 절삭기, 1회용 전파 절삭기, 1회용 전파와 초음파 절삭기, 1회용 다관절 다자유도 바이폴라 절삭기)의 임상적 효과성에 대한 근거 확인을 위하여 의료기술재평가를 본 원에 의뢰하였고(2022.9.6.), 2022년 제10차 의료기술재평가위원회(2022.10.14.)에서 전문적·심층적 검토를 통해 동 기술의 임상적 안전성 및 효과성 평가가 필요한 안건으로 심의하였다. 이에 본 평가는 체계적 문헌고찰을 통해 1회용 다관절 다자유도 바이폴라 절삭기의 임상적 안전성 및 효과성의 근거를 확인하여 정책적 의사결정을 지원하고자 수행하였다.
평가방법
동 기술에 대한 안전성 및 효과성 평가를 위해 체계적 문헌고찰을 수행하였다. 모든 평가방법은 평가목적을 고려하여 “1회용 절삭기를 이용한 수술 소위원회(소위원회)”의 심의를 거쳐 확정하였다. 평가의 핵심질문은 “갑상선암 수술, 위암 수술, 대장암 수술, 충수절제술, 자궁절제술 대상자에서 1회용 다관절 다자유도 바이폴라 절삭기 사용은 임상적으로 안전하고 효과적인가?”이며, 안전성은 합병증 및 이상반응 지표로, 효과성은 혈액 소실량, 수술 시간, 재수술률, 배액량, 재원 기간을 지표로 평가하였다.
체계적 문헌고찰은 핵심질문을 토대로 국외 3개, 국내 5개 데이터베이스에서 검색하여 문헌선정 및 배제기준에 따라 두 명의 검토자가 독립적으로 선별하고 선택하였다. 선택문헌이 있을 경우, 문헌의 비뚤림위험 평가, 자료분석, 근거수준 평가를 수행하였다. 평가결과 및 소위원회의 검토의견을 고려하여 의료기술재평가위원회에서 최종심의 후 권고등급을 제시하였다.
평가결과
1회용 다관절 다자유도 바이폴라 절삭기에 대한 안전성 및 효과성을 평가하기 위해 체계적 문헌고찰을 수행하였으나, 최종 선정된 무작위배정 임상시험연구는 없었다.
결론 및 제언
소위원회에서는 현재 문헌적 근거를 바탕으로 다음과 같이 결론을 제시하였다.
갑상선암 수술, 위암 수술, 대장암 수술, 충수절제술, 자궁절제술 등에서 1회용 다관절 다자유도 바이폴라 절삭기를 이용한 수술과 기존 지혈법을 이용한 수술을 비교한 무작위배정 임상시험연구는 확인되지 않았으며, 이에 대해 소위원회는 아래와 같은 의견을 제시하였다. 에너지를 이용한 절삭기 이용은 기술적 발전에 따른 자연스러운 세대 이전으로 보아야 하며, 수술시 절삭기 종류는 환자 및 혈관 상태에 맞게 수술자가 선택해서 사용하기 때문에 절삭기 간의 안전성과 효과성은 유사할 것으로 판단하였다. 따라서 1회용 초음파 절삭기와 1회용 전파 절삭기의 근거를 1회용 다관절 다자유도 바이폴라 절삭기의 근거로 고려하는 것에 문제가 없다는 의견으로 이에 1회용 다관절 다자유도 바이폴라 절삭기를 이용한 수술은 안전하고 효과적인 의료기술로 평가하였다.
2023년 제5차 의료기술재평가위원회(2023.5.12.)에서는 소위원회 검토 결과에 근거하여 의료기술재평가사업 관리지침 제4조 제10항에 의거 “1회용 다관절 다자유도 바이폴라 절삭기”에 대해 다음과 같이 심의하였다.
의료기술재평가위원회는 임상적 안전성과 효과성의 근거 및 그 외 평가항목 등을 종합적으로 고려하였을 때, 국내 임상상황에서 수술 시 1회용 다관절 다자유도 바이폴라 절삭기의 사용을 ‘조건부 권고함’으로 심의하였다.

Background
Advanced vessel-sealing systems have been developed and widely adopted in various specialties, including thyroid cancer surgery, gastric cancer surgery, colorectal cancer surgery, appendectomy, and hysterectomy. In Korea, the spectrum of disposable scalpels encompasses ultrasonic, bipolar, and ultrasonic and bipolar models, in addition to multi-degree of freedom (DOF) articulating laparoscopic instruments. Among these, the multi-DOF articulating laparoscopic instrument, first listed as a selective benefit item with an 80% co-payment in May 2019.
In 2022 (September 6, 2022), the Health Insurance Review and Assessment Service (HIRA) requested a reassessment of four types of advanced vessel-sealing systems (ultrasonic devices, energy-based vessel sealing systems, combination devices, multi-degree of freedom articulating laparoscopic instrument). These technologies were selected to be health technology reassessment agenda at the 10th Health Technology Reassessment Committee in 2022 (October 14, 2022). Accordingly, this assessment performed whether multi-DOF articulating laparoscopic instrument (such as Artisential) is clinically safe and effective in patients requiring surgery.
Methods
A systematic review was conducted to evaluate the safety and effectiveness of this advanced health technology. All assessment methods were finalized after deliberation by the “Advanced vessel-sealing systems Subcommittee” (“Subcommittee”), with consideration of the purpose of the assessment. The Subcommittee consisted of 7 experts: four surgeons, one obstetrician and gynecologist, one anesthesia and pain medicine, and one evidence-based medicine specialist.
The key question of the assessment was as follows: “Are multi-DOF articulating laparoscopic instruments clinically safe and effective for use in thyroid cancer surgery, gastric cancer surgery, colorectal cancer surgery, appendectomy, and hysterectomy?” Safety was based on complications and adverse events (AEs), and effectiveness was assessed based on blood loss volume, operating time, reoperation rate, drainage volume, and hospital stay length. Considering the purpose of this assessment, the study type was limited to randomized controlled trials (RCTs).
For the systematic review, three international and five domestic databases were searched based on the key question. The literature selection process involved two reviewers independently conducting the selection process according to predefined selection and exclusion criteria. The risk of bias (RoB) in the included studies was assessed two reviewers independently. All data were extracted by study unit. Any disagreements were resolved through a discussion with a third person to reach a consensus. Data were qualitatively analyzed if quantitative analysis was not possible. The systematic review results were assessed for the level of evidence using the Grading of Recommendations Assessment, Development, and Evaluation (GRADE) method.
Results
A systematic review was performed to assess the safety and effectiveness of the disposable multi-DOF articulating laparoscopic instruments. However, no RCTs were identified.
Conclusions and recommendations
Based on this assessment, the Subcommittee evaluated the following findings and opinions.
RCTs comparing surgeries utilizing multi-DOF articulating laparoscopic instruments with conventional hemostatic methods across thyroid cancer, gastric cancer, colorectal cancer surgeries, appendectomies, and hysterectomies, are notably absent. The Subcommittee suggests that the adoption of advanced vessel-sealing systems represents a progressive, generational change as technology advances. The choice of surgical tools, whether it be a scalpel or dissector, hinges on the surgeon’s discretion, guided by patient-specific factors and vascular conditions. Consequently, it is inferred that the safety and effectiveness of varying surgical instruments should be comparable. It is considered reasonable to extrapolate evidence from other advanced vessel-sealing systems to the multi-DOF articulating laparoscopic instruments. Thus, it was determined that the use of disposable multi-DOF articulating laparoscopic instruments is safe and effective.
The 5th Health Technology Reassessment Committee of in 2023 (on May 12, 2023) comprehensively reviewed findings based on the conclusions of the Subcommittee review.
The Committee determined the level of recommendation for the use of disposable multi-DOF articulating laparoscopic instruments as “conditionally recommends” based on the clinical settings and values in Korea.
