1. 연구배경
뇌졸중은 전 세계적으로 사망의 주요 원인 중 하나로 우리나라의 경우 2011년 통계청 사망원인 자료에 의하면 뇌혈관 질환은 주요 사망원인 2위에 해당한다. 또한 뇌졸중은 한번 발생 이후 지속적으로 의료기관 이용이 필요로 하는 특징을 띄는데, 아직 국내에서 뇌졸중 환자의 의료이용이 어떤 패턴을 보이는지에 대한 연구가 부족하다.
본 연구는 환자조사자료와 건강보험심사평가원(심평원) 청구자료를 이용하여 국내 뇌졸중 환자의 진단 및 추적시기별 의료기관 종별 이용 현황을 비롯한 의료이용 현황 분석 및 의료이용 관련 요인 파악하고자 하였다.
2. 연구방법
(1) 환자조사자료 분석
우리나라 뇌졸중 환자의 일반적 특성을 파악하기 위하여 2009년 환자조사 자료를 이용하여 전체 대상자 중 ICD-10 기준 뇌졸중 상병에 해당하는 I60-62(뇌출혈), I63(뇌경색), I64(출혈 혹은 경색 불명), I69(뇌혈관질환 후유증)의 상병코드를 가진 환자를 대상으로 의료기관을 이용한 환자의 일반적 특성을 파악하였다. 일반적 특성으로 성, 연령, 의료기관 종류, 입원경로, 내원경위, 병상수, 진료과, 재원일, 치료결과, 퇴원형태, 진료비 지불방법에 대하여 기초분석을 시행하였다.
(2) 심평원 청구자료 분석
2008년 뇌졸중 신환자의 의료이용 현황을 분석하기 위하여 심평원의 요양기관 청구자료 중 2006~2010년 5년간 청구자료를 이용하여 주부상병 기준 상병코드가 I60~64인 환자를 대상으로 2008년 뇌졸중 신환자의 조작적 정의를 내리고 이 환자들의 뇌졸중 발생 후 1년간 의료이용 패턴과 일반적 특성, 의료이용 총량 등을 분석하였다. 임상적 차이를 고려하기 위하여 뇌경색과 뇌출혈 환자의 경우 별도의 하위그룹 분석을 수행하였으며, 다회 병원 이동 환자 및 장기 입원 환자에 대한 특이적 형태를 파악하기 위하여 이들의 특성을 추가적으로 분석을 하였다. 의료이용 총량의 경우 입원과 외래로 나누어 요양기관종별, 주 단위 심결금액 총액 및 의료기관 이용환자수를 산정하였으며 항목별 의료비에 대한 세부분석도 수행하였다. 항목별 의료비의 시간에 따른 증감을 확인하기 위하여 입원/외래 각각에서 1인당 평균 의료비용을 주 단위로 계산하고 뇌경색, 뇌출혈의 주요 경로에 대한 분석도 수행하였다.
3. 연구결과
(1) 환자조사자료 분석
전체 환자 중 뇌졸중 상병코드로 입원한 자는 13,508명(2.3%)으로 이 중 뇌출혈 2,894명(21.6%), 뇌경색 7,982명(59.0%), 불명 522명(3.9%), 후유증 2,109명(15.6%)으로 나타났다. 성별로 구분했을 때 남성의 발생이 더 높았고 40대 이후부터 급격하게 증가하는 경향을 보인다. 입원기관은 3차 병원을 이용하는 비중이 가장 높게 나타났고 병원, 요양병원 순으로 나타났다. 입원 환자의 재원기간을 살펴보면 7일 이내의 비중이 가장 높고 그 다음으로 7~14일, 15~21일, 22~28일로 시간이 갈수록 낮아지다 한 달이 지난 시점부터는 다시 높아지는 경향을 보인다. 뇌졸중으로 퇴원하는 환자의 치료결과를 살펴보면 호전완쾌되어 퇴원하는 경우, 호전안되는 경우, 사망의 수가 높게 나타났다. 뇌경색은 호전완쾌가 46.4%, 호전안됨 7.8%, 사망 3.5%로 나타났다. 뇌출혈의 경우 호전
완쾌 15.2%, 사망 2.9%, 호전안됨 2.7%로 나타났다. 입원환자의 경우 퇴원 시 정상 퇴원하는 경우가 가장 많았고 전원하여 다른 의료기관으로 이동하는 경우, 스스로 퇴원을 결정한 경우 순으로 나타났다. 뇌졸중으로 환자가 입원할시 이용하는 진료과는 신경과, 신경외과, 재활의학과 내과 순으로 나타났다.
(2) 심평원 청구자료 분석
심평원 청구자료를 분석한 결과, 본 연구의 조작적 정의에 따라 2008년 청구된 뇌졸중 신환자는 68,829명이었으며, 전체 환자 중 뇌경색 환자가 67.3%, 뇌출혈 환자가 28.9%를 차지하였다. 뇌졸중 신환자의 연령대의 특성을 보면 뇌경색은 70대에서, 뇌출혈은 50대에서 가장 많았다. 뇌출혈 환자의 경우 뇌경색 환자에 비하여 높은 사망률과 긴 입원기간을 가지는 것으로 나타났다. 뇌졸중 신환자의 약 88.1%가 전체 의료기관 중 상급종합병원과 종합병원을 이용하는 것으로 나타났다.
의료기관별로 평균 입원일수 차이를 확인해 본 결과 뇌경색은 상급종합병원, 종합병원, 병원에서 각각 19.2일, 22.9일, 30.6일로, 뇌출혈은 각각 35.3일, 41.7일, 39.3일로 유의한 차이를 보이는 것으로 나타났으며, 전반적으로 뇌경색이 뇌출혈에 비하여 입원일수가 10일 가량 짧게 나타났다.
평균 입원기간 파악을 위한 선행 분석으로 입원한 뇌졸중 환자의 평균 의료기관 이동 횟수를 분석한 결과, 약 63.7%가 1회 입원 후 퇴원하는 것으로 나타났으며, 20.3%의 환자가 2회, 8.2%의 환자가 3회 이동하는 것으로 나타났다.
첫 입원 에피소드의 입원기간이 1일인 환자를 별도로 추출하여 첫 입원 시 일반적인 특성을 살펴보았다. 연령별, 성별 특성의 경우 뇌졸중 신환자의 특성과 유사하게 나타났다. 전체 환자 중 하루 입원 후 당일 혹은 다음날 전원한 환자는 556명이었으며 뇌졸중 타입 별로는 뇌경색 환자(266명) 중 하루 입원 후 당일 혹은 다음날 전원한 환자는 141명(53.0%), 뇌출혈 환자(270명) 중 하루 입원 후 당일 혹은 다음날 전원한 환자는 114명(42.2%)로 나타났다.
뇌경색과 뇌출혈 환자의 3차 입원 에피소드까지의 의료기관 이용 패턴을 확인한 결과, 두 그룹 모두 첫 입원 시에는 대부분의 환자가 상급종합병원과 종합병원 등을 이용하였지만 2차, 3차 입원으로 갈수록 병원과 요양병원의 비중이 커지는 것을 확인하였다.
뇌졸중 환자의 중증도를 반영하기 위하여 인공호흡기 사용 여부에 따라 중증 환자를 분류하였고 이에 대한 하위그룹 분석을 수행하였는데 인공호흡기를 사용한 환자그룹이 그렇지 않은 환자 그룹에 비하여 사망률이 더 높게 나타났고 두 번째 입윈 이후에는 요양병원의 이용률이 높고 한방병원의 이용률은 낮은 형태를 보였다.
3개월, 6개월 및 1년 이상 한 의료기관에 장기 입원한 환자의 특성을 살펴보면 첫 번째 입원에서 장기 입원을 한 경우 약 60%가 뇌출혈 환자였고 인공호흡기 이용률이 높았으며 상급종합병원과 종합병원이 큰 비중을 차지하며 특히 종합병원의 이용률이 높게 나타났다.
두 번째 입원에서 장기 입원을 한 경우는 약 74%가 뇌경색 환자로 나타났고 인공호흡기 이용률은 상대적으로 감소하였다.
3회 이상 입원환자와 3회 미만 입원환자로 분류하여 특징을 살펴본 결과, 3회 이상 입원환자가 3회 미만 입원환자에 비하여 CCI 점수가 2점 이상(3회 이상 82.6% vs. 3회 미만 74.3%)의 비율이 높게 나타났다. 재원기간의 경우 3회 이상 입원환자가 3회 미만 입원환자에 비하여 첫 입원 시 7일 이내 퇴원 비율(3회 이상 22.0% vs. 3회 미만 29.6%)이 낮게 나타났다.
뇌졸중 환자의 외래 이용 패턴을 살펴보면 뇌졸중 발병 후 2주 후부터 외래 이용이 증가하다가 그 뒤로 조금씩 낮아짐을 확인할 수 있다. 외래 이용 환자의 동반상병을 확인해 본 결과 고혈압으로 인한 방문이 가장 큰 비중을 차지하는 것으로 나타났다.
국내에서 뇌졸중 입원 환자의 의료이용 총량을 심결금액총액과 입원환자수로 확인하였는데 전반적으로 뇌졸중 발병 초기에 의료이용 총량이 매우 많음을 확인할 수 있었다. 의료기관 타입별로 보면 모두 뇌졸중 초기 의료이용에서 상급종합병원과 종합병원이 차지하는 비중이 매우 크며 시간이 갈수록 병원이나 요양병원에서의 의료이용이 증가함을 확인하였다.
뇌경색과 뇌출혈 환자 모두 종합병원에 입원 후 퇴원한 경로가 가장 많았으며 1인당 의료비용을 주 단위로 분석한 결과 입원 초기에는 뇌경색 환자의 경우 검사비가 높게 나타나고 뇌출혈 환자의 경우 검사비와 처치 및 수술비가 높게 나타나는 것을 확인하였다.
외래이용의 경우, 상급종합병원에 입원한 환자의 경우 검사비의 비중이 가장 높았으며 그외 종합병원, 병원, 요양병원, 의원에서는 재활치료비가 가장 높은 비중을 차지하였다.
4. 결론
본 연구는 전 국민을 대상으로 하는 건강보험 청구자료를 이용하여 뇌졸중 환자의 의료이용 패턴을 분석하였다는데 의의가 있다. 본 연구의 결과는 뇌졸중 환자에 대한 의료자원의 효율적 이용 등 관련 보건의료분야의 정책적 결정에 있어서 근거자료로 활용될 수 있을 것이다.
1. Study background
Stroke is one of major causes of death worldwide and according to the 2011 Korea National Statistical Office data, cerebrovascular diseases was the 2nd leading cause of death in Korea. In addition, stroke requires continued use of medical facilities after disease onset; however, there have been few local studies on the pattern of medical care utilization of stroke patients.
The aim of this study was to analyze the status of medical care utilization of local stroke patients, including the use for each type of medical facilities based on diagnosis and follow-up timing and to identify factors associated with medical care utilization by using the patient survey data and the Health Insurance Review & Assessment Service (HIRA) claims data.
2. Study methodology
(1) Analysis of patient survey data
The ‘Patient Survey' data from the year of 2009 were used to investigate general characteristics of stroke patients. The study subjects were defined as patients who used medical facilities among those with disease codes of I60-62 (Cerebral hemorrhage), I63 (Cerebral infarction), I64 (Hemorrhage or infarction, unspecified), and I69 (Sequelae of a cerebrovascular disease) that correspond to the stroke disease in the ICD-10 criteria. As general characteristics, sex, age, medical facility type, inpatient route, context of hospital visit, number of beds, department, length of stay, treatment outcome, discharge type, and medical fee payment method were analyzed.
(2) Analysis of HIRA claims data
To analyze medical care utilization status of incident cases in 2008, HIRA’s health insurance claims data over 5 years (2006~2010) were obtained. The operational definition of new stroke patients in 2008 was developed and medical care utilization pattern over 1 year after stroke onset, demographic characteristics, and total volume of medical care utilization were analyzed in the defined patient group. To take clinical differences into account, subgroup analyses were performed for patients with cerebral infarction and cerebral hemorrhage. In order to define specific characteristics of patients with multiple hospital transfers and long-term stays in hospital, additional analyses were conducted. The total volume of medical care utilization was estimated by the total amount of medical fee (per week, per type of medical facilities) and by the total number of patients who used medical facilities among inpatients and outpatients separately. Medical expenses were itemized for the further analyses. To investigate the increase/decrease of medical expenses by item over time, per-capita, per-week mean medical expenses were calculated for inpatients/outpatients separately. The key patterns of medical utilization were also measured for cerebral infarction and cerebral hemorrhage patients.
3. Study results
(1) Analysis of 'Patient Survey' data
Of all patients, 13,508 patients (2.3%) were hospitalized with a stroke disease code; of these, 2,894 (21.60%) had cerebral hemorrhage, 7,982 (58.97%) had cerebral infarction, 522 (3.86%) were unspecified, and 2,109 patients (15.57%) had sequelae. By sex, the incidence was higher for men and rapidly increased after 40 years old. In case of inpatient facilities, tertiary hospitals indicated the highest proportion of use, followed by hospitals, and nursing hospitals. For the length of stay among inpatients, less than 7 days accounted for the highest proportion of patients, followed by 7~14 days, 15~21 days, and 22~28 days. The number of inpatients decreased gradually over time; however, it started to increase from 1 month after hospitalization. In terms of treatment outcomes for the discharged patients with stroke, discharge with improvement and complete recovery showed the highest percentage, followed by no improvement, and death. In case of cerebral infarction, improvement and complete recovery made up 46.42%, no improvement was 7.82%, and death was 3.50%. For cerebral hemorrhage, improvement and complete recovery represented 15.17%, death was 2.92%, and no improvement was 2.70%. Three most common types of discharge were leaving hospitals, transferring to another facilities, and voluntary decision of discharge. For inpatients with stroke, the most frequently visited departments were Department of Neurology, Neurological surgery, Rehabilitation, and internal medicine in order.
(2) Analysis of HIRA claims data
According to the operational definition in this study, there were 68,829 patient who claimed with a stroke diagnosis (I60~64) in 2008. Cerebral infarction patients were approximately 67%, and cerebral hemorrhage patients were 29% of the identified patients. In terms of characteristics at the initial admission, the proportion was the highest in the 70~79 years age group for cerebral infarction patients and in the 50~59 years age group for cerebral hemorrhage patients. Cerebral hemorrhage patients exhibited higher mortality and longer hospitalization period compared to cerebral infarction patients. Approximately 88% of incident cases used advanced general hospitals and general hospitals For the mean length of hospital stay by the type of medical facilities and type of stroke, a significant difference was noted among advanced general hospitals, general hospitals, and hospitals; admission period was about 10 days shorter for cerebral infarction than for cerebral hemorrhage.
As preliminary analysis to identify the mean length of hospitalization, the mean number of medical facility transfer was analyzed; about 63.7% of patients left hospitals after one time admission, 20.3% and 8.2% patients transferred twice and three times, respectively.
The pattern of medical facility use up to the 3rd hospitalization episodes was estimated for cerebral infarction and cerebral hemorrhage patients; although both groups mostly used advanced general hospitals and general hospitals for the first admission, the proportion of hospitals and long-term care hospitals became higher for 2nd and 3rd admissions.
In order to take severity of stroke patients into account, subgroup analysis was conducted with groups classified by a mechanical ventilation use. According to the results, the patient group that used mechanical ventilation had a higher mortality, more frequent use of long-term care facilities and less usage of oriental medicine hospitals after the 2nd admission, compared to the group that did not use any ventilation.
Characteristics of inpatients who stayed in one medical facility for 3 months, 6 months and 1 year or longer were analyzed; in case of long-term inpatients at the 1st admission, approximately 60% were cerebral hemorrhage patients, the ventilator use rate was high, advanced general hospitals and general hospitals accounted for a high proportion, and the use rate was particularly high for general hospitals. In case of long-term inpatients at the 2nd admission, approximately 74% were cerebral infarction patients and the ventilator use rate was relatively lower.
Regarding the pattern of medical care utilizations among outpatients, the number of patients increased from 2 weeks after stroke onset but subsequently, slowly declined over time. The disease distribution of outpatients indicated that hypertension was the most common reason for visits. Based on the total amount of medical costs and the number of inpatients, overall, the total volume of medical care utilization was concentrated on the early phase of treatment course. By the type of medical facilities, advanced general hospitals and general hospitals made up a very high proportion at the beginning of treatments; however, use of hospitals or nursing hospitals increased over time.
For both cerebral infarction and cerebral hemorrhage patients, discharge after admission to a general hospital was the most common pattern. According to the results from per-capita, per-week medical expenses analyses, medical examination fees accounted for a largest part of the expenses for cerebral infarction patients at the beginning of hospitalization, whereas not only medical examination fees, but also treatment and surgery expenses were high for cerebral hemorrhage patients.
In case of outpatient, fees for medical examination took up the highest proportion for the patients admitted to advanced general hospitals; while cost of rehabilitation treatment represented the highest share in general hospitals, hospitals, nursing hospitals, and clinics.
4. Conclusion
This study has a major implication as it analyzed the pattern of medical care utilization of stroke patients by using the health insurance claims data that involved the general public. Results from this study can be used as the grounds for political decision making in relevant public health areas such as efficient use of medical resources for stroke patients.
