이상지질혈증은 흡연, 고혈압, 당뇨병, 비만과 함께 심뇌혈관질환의 중요한 위험인자로 최근 질병부담이 증가하고 있다. 현재 국내에서 제작된 이상지질혈증에 대한 진료지침, 치료기준 및 건강보험심사평가원의 보험급여기준이 서로 일치하지 않고 있으며, 또 한국인의 질병양상을 제대로 반영하고 있지 못하고 있다. 이에 본 연구에서는 한국형 이상지질혈증 진료지침 개발을 위한 방향을 제시하고자 한다.
본 연구는 2004년 이후 출판된 각국 진료지침 중 질평가를 통해 선별한 8개 지침을 비교 분석하여, 향후 진료지침에 포함되어야 할 핵심질문을 도출하였다. 이러한 핵심질문은 한국인의 역학적 특성과 지질저하제 처방 현황을 고찰하면서 보다 구체화되었고, 관련 학회 및 진료지침 전문가들과의 합의회의를 통해 한국형 이상지질혈증 진료지침의 개발에 대한 방향을 제시하였다.
선택된 스코틀랜드, 영국, 캐나다, 유럽, 싱가포르, 미국, 일본, 한국의 8개 진료지침은 진단기준, 위험도 평가, 치료 목표와 목표치, 치료 시작 기준, 약물치료 추적 검사의 5개 영역으로 세분화하여 비교하였다. 이를 기준으로 다섯 개의 핵심질문과 9개의 세부질문을 도출하였다.

최근 서구에서는 저밀도지단백 콜레스테롤을 목표치 아래로 가능한 더 낮출수록 좋다는 개념(the lower, the better)으로 강화되어 가는 추세이나, 우리나라의 질병발생 양상은 서구와 다르다. 한국의 심뇌혈관질환 역학은 최근 출혈성 뇌졸중은 감소하고 허혈성 심장질환과 허혈성 뇌졸중이 증가하고 있으나 서구에 비해서는 여전히 뇌혈관질환이 상대적으로 사망과 장애의 주요 원인이다.
또한 이상지질혈증의 유병률은 고콜레스테롤혈증 10.9%, 고중성지방혈증 17.3%로 증가추세이며, 서양에 비해 고콜레스테롤혈증은 낮으나 고중성지방혈증의 비율이 높다(미국 nhanes 2006년 각각 16%, 13%). 아시아인은 백인에 비해 더 낮은 스타틴 용량으로도 비슷한 정도의 지질개선 효과를 얻을 수 있다고 나타나 이를 고려한 기준 설정이 필요하다.
지질저하제 처방은 최근 계속 증가하는 추세이나, 2차 예방을 위한 처방이 충분하게 이루어지지 못하고 있으며, 관련 전문과의 경우 2차 예방을 위한 처방률이 높지만 장기적으로 관찰했을 때 치료목표달성이 충분하지 못했으며 대부분 초기치료약제로 스타틴을 사용하고 있는 것으로 나타났다.
외국과 다른 한국인의 역학적 특징을 반영하기 위해, 우선적으로 필요한 근거 개발을 위한 지원이 이루어져야하는 영역은 진단기준, 위험도 평가 결과 변수, 1·2차 치료목표치, 위험군별 약물치료 시작기준 및 시행기간이며, 이를 위해 장기적으로는 대규모 환자 코호트를 구축되어 대표성 있는 자료를 확보하면서, 공공기관의 보건의료 자료원을 융합하여 국민건강에 직결되는 근거들을 생성해야 한다.
즉, 자료융합 직후 개인정보를 완전 삭제하는 체계 속에서, 국민건강영양조사의 이상지질혈증 환자의 의료이용 현황과 동반질환 여부를 건강보험심사평가원 자료와 연계하고, 최종 사망여부를 통계청 사망자료와 연결한다면, 보건의료의 경쟁력을 가진 훌륭한 근거들을 효율적으로 생성해 낼 수 있다.
이상지질혈증은 외국연구결과를 그대로 받아들일 수 없는 역학적 특징이 있으며, 이를 고려하지 않을 경우 실제 진료와 지침 사이의 차이를 극복할 수 없다. 따라서 한국인을 대상으로 한 양질의 임상연구를 하기 위해 적극적인 투자가 필요하며, 본 연구에서 제시된 핵심질문과 국내 자료에 기반한 역학적 특징을 고려하여 근거중심 이상지질혈증 진료지침이 개발되어야 할 것이다.
Dyslipidemia is an important risk factor, along with smoking, hypertension, diabetes, and obesity, for cardiovascular and cerebrovascular diseases and the burden it causes has recently increased. At present, guidelines and treatment standards for dyslipidemia, and the HIRA guideline for insurance payment standard have differences, and are not also reflective of the disease aspects of Koreans. Thus, this study suggests the directions for the development of Korean dyslipidemia guideline.
This study performed comparative analysis of 8 guidelines selected through quality assessment of guidelines published in other countries since 2004, and drew key questions that should be included in future guidelines. Such key questions took more concrete shapes while considering the epidemiological characteristics of Koreans and the current status of prescriptions for lipid-lowering agents, and suggested directions
for the development of Korean dyslipidemia guideline through the agreement meetings with related scientific societies and professionals for guidelines.
Eight selected guidelines of Scotland, UK, Canada, Europe, Singapore, USA, Japan, and Korea were segmented into 5 areas of diagnosis standard, risk assessment, treatment goal and value, treatment initiation standard, and drug therapy follow-up test. These guidelines were then compared. Based on these works, 5 key questions and 9 detailed questions were drawn.

Recently, 'the lower, the better' concept tends to be increased in western societies in which the LDL-cholesterol level should be lowered below the goal value, but the aspect of the disease in Korea is different from those of other western countries. The epidemiology of cardiovascular and cerebrovascular diseases in Korea has shown recently that hemorrhagic stroke has decreased and ischemic heart disease and ischemic stroke has increased, but cerebrovascular diseases are still major causes of death and disability compared to the western countries.
In addition, the prevalence of dyslipidemia tends to increase by 10.9% in hypercholesterolemia and by 17.3% in hypertriglycerodemia, showing that hypercholesterolemia is lower and hypertriglycerodemia is higher than those of the western values (USA 2006 NHANES 16% and 13%, respectively). Asians are known to have similar lipid-improving effect with lower statin level than Caucasians, and it is thus necessary to establish the standard by considering it.
The prescription for lipid-lowering agents has recently increased, but is not enough for secondary prevention, and although the prescription rate for secondary prevention is higher in related clinical department, still the treatment goal has not sufficiently accomplished in the long-term, and statins have been shown as the initial treatment in most cases.
The areas that need support for developing the basis to reflect the Korean epidemiological characteristics, which are different from those of foreign countries, are diagnosis standard, risk assessment result variables, 1st·2nd treatment goal values, drug therapy initiation standard and period by risk groups. To accomplish these in the long-term basis, a large-scale patients cohort should be established to ensure representing data, and the basis that directly related to the national health should be
generated by combining public health data sources of public organizations.
That is, in the system in which individual information is completely deleted right after data convergence, if the current status of using medical services and the presence of accompanying diseases of dyslipidemic patients in the KNHANES are connected to the HIRA data, and if the death of a patient is connected to the mortality data of the National Statistical Office, then the excellent basis with competitiveness in the public health can be efficiently generated.
Dyslipidemia has certain epidemiological characteristics that foreign study results cannot be directly applied, and if such aspects are not considered, it is difficult to overcome the difference between actual treatment and the guidelines. Therefore, active investment is needed to perform good quality clinical studies for Koreans, and basis-oriented dyslipidemia guidelines should be developed by considering key questions suggested in this study and epidemiological characteristics based on domestic data.
