▢ 연구 배경 전세계적으로 소화성 궤양 질환(peptic ulcer disease, PUD) 및 위식도 역류질환(gastro-esophageal reflux disease, GERD)은 높은 이환율을 보이고 있으며, 이에 따라 소화성 질환 치료제들의 사용 또한 증가하고 있다. 소화성 질환에 보편적으로 사용되는 약물로는 H2 수용체 길항제(H2 receptor antagonists, 이하 H2RA), 양성자 펌프 억제제(proton pump inhibitors, PPI) 등이 있다. 이중 양성자 펌프 억제제는 작용기전상 H2RA에 비해 효과적으로 산분비를 억제하는 것으로 알려져 있어 국내에서의 사용 빈도가 급속하게 증가하고 있다. 양성자 펌프 억제제를 장기간 사용할 경우 골절 증가에 영향을 줄 수 있음이 보고되고 있으나 국내에서 양성자 펌프 억제제의 장기간 사용으로 인한 골절 발생의 위험에 대한 연구는 미미하다. 따라서 실제 임상현장에서 대규모 자료를 사용하여 양성자 펌프 억제제의 사용과 골절 발생의 연관성을 검증할 필요가 있다.
▢ 연구 목적 본 연구에서는 양성자 펌프 억제제의 사용과 골다공증성 골절 발생 간의 연관성을 파악하고자 한다. 구체적으로는 양성자 펌프 억제제 처방력, 누적 사용일수, 사용 용량, 골절 발생 최근에 사용했는지의 유무, 골절 발생 최근에 정기적으로 처방 받았는지의 여부 등에 따른 골절 발생 위험양상을 살펴본다.
▢ 연구 방법 국민건강보험공단의 건강보험청구자료, 자격자료 및 국가일반건강검진자료를 이용하여 코호트 내 환자-대조군 연구(nested case-control study)를 수행하였다. 코호트 내 연구대상자는 2006년 1월 1일부터 2015년 12월 31일까지 소화성 궤양 및 위식도 역류질환의 상병코드(ICD-10 code: K21, K25, K26, K27, K28)에 의한 의료이용이 있고, 양성자 펌프 억제제 또는 H2RA 처방 이력이 있는 사람들이다. 코호트 입적일(cohort entry date)은 최초로 약물(PPI 또는 H2RA)을 처방받은 날이다. 임상적 대상자 제외 기준은 코호트 입적일을 기준으로 50세 미만인 환자, 코호트 입적 이전 1년 이내에 양성자 펌프 억제제 또는 H2RA를 처방받은 이력이 있거나 암으로 인한 의료 이용이 있는 환자, 후천성면역결핍증(AIDS) 또는 골다공증 및 골절을 진단받았거나 치료받은 경험이 있는 환자로 정의하였다. 또한, 국가일반건강검진 수검 기록이 없는 경우 역시 자료의 제한으로 인해 제외하였다. 환자군은 코호트 입적일로부터 2015년까지의 기간 동안 최초로 골다공증성 골절이 발생(발생일: index date)한 자이다. 대조군은 결과지표가 발생하지 않은 환자들 중에서 성별, 5세 단위 연령군, bisphosphonates 사용 여부, 코호트 입적일, 동일한 추적관찰 기간 등에 의한 1:5 짝짓기(matching)를 통해 선별하였다. 추적관찰 기간 동안 최소 1회 이상의 양성자 펌프 억제제 처방 여부, 양성자 펌프 억제제의 누적 사용일수, 저용량/표준용량 이상 PPI 제제 사용여부, 골절 발생일을 기준으로 최근 1년간 양성자 펌프 억제제 사용여부, 최근 1년간 28일 이상 양성자 펌프 억제제를 처방받은 분기의 횟수별로 골절 발생의 양상을 관찰하였다. 조건부 로지스틱 회귀분석(conditional logistic regression)을 통하여 결과지표 오즈비(odds ratio, OR)를 추정하였고, 95% 신뢰구간(confidence interval, CI) 및 유의확률(p-value)을 함께 제시하였다. 공변량으로는 성, 연령, 거주 지역, 소득분위 등의 인구·사회학적 요인과 신장, 체중, 체질량 지수(BMI), 음주, 흡연, 운동력 등의 검진/문진 변수, 골절 발생에 직접적 또는 간접적으로 영향을 줄 수 있는 병용약물 및 동반질환을 포함하였다. 양성자 펌프 억제제 사용에 따른 골다공증성 골절의 발생에 영향을 미치는 요인에 대한 추가적인 검토를 위해 짝짓기 변수로 사용된 성별, 연령, bisphosphonates 사용 여부에 따른 하위그룹 분석을 수행하였다.
▢ 연구 결과 지난 10년간 소화성 궤양 및 위식도 역류질환으로 약물치료를 경험한 환자들은35,520,188명이었다. 제외 기준 적용 후, 연구 대상자 2,388,137명 중 78,465명(3.3%)에서 분석기간 중 골다공증성 골절이 발생했다. 짝짓기 결과, 골절군(case)은 59,240명, 비골절군(control)은 296,200명으로 정의되었고, 대상자의 78.0%가 여성이었다. 추적관찰 기간 동안 양성자 펌프 억제제를 1회 이상 복용한 환자들의 경우, 그렇지 않은 경우에 비해 골절 발생의 위험이 1.11배(95% CI: 1.08, 1.13) 높았으며 이는 통계적으로 유의하였다. 또한 양성자 펌프 억제제의 누적 사용일수가 높아질수록 골절 발생 위험이 높아지는 것으로 나타났다(p for trend <0.0001). 특히, 양성자 펌프 억제제를 1년 이상 사용하였을 경우는 전혀 사용하지 않은 경우에 비해 골절의 오즈비가 1.42(95% CI: 1.32, 1.52)로 비교적 높게 나타났다. 저용량 양성자 펌프 억제제 사용에 따른 골절 발생의 오즈비는 1.17(95% CI: 1.14, 1.21), 고용량 양성자 펌프 억제제 사용에 따른 골절 발생의 오즈비는 1.06(95% CI: 1.04, 1.21)로 큰 차이는 없었다. 골절 발생 최근 1년 이내 양성자 펌프 억제제를 사용한 경우는 그렇지 않은 경우에 비해 골절 발생의 위험이 1.30(95% CI: 1.27, 1.33)배로 높게 나타났다. 골절 발생 최근 1년 이내의 기간을 4분기로 나누었을 때, 모든 분기에서 4주 이상 양성자 펌프 억제제를 처방받은 경우 골절 위험의 오즈비는 1.37(95% CI: 1.26, 1.50)배였다. 남성과 여성 모두 양성자 펌프 억제제 사용에 따른 골다공증성 골절의 위험이 높아지는 것으로 나타났다. 연령대가 높을수록 양성자 펌프 억제제 사용에 따른 골절 위험이 증가하는 경향을 보였다. Bisphosphonate를 복용하고 있는 경우와 그렇지 않은 경우 모두에서 양성자 펌프 억제제 사용에 따른 골절위험이 큰 것으로 나타났다.
▢ 결론 및 정책적 제언 우리나라 국민의 상당수가 소화성 궤양 및 위식도 역류질환으로 약물치료를 경험하고 있었으며 노령층에서 양성자 펌프 억제제 복용은 골절의 위험을 증가시켰다. 양성자 펌프 억제제를 지속적으로 복용하는 경우 골절의 위험은 더 증가하는 것으로 나타났다. 따라서 양성자 펌프 억제제 처방 시 환자의 투약 이력을 확인할 필요가 있다. 특히 장기처방이 요구되는 고령의 환자들에게는 양성자 펌프 억제제의 위험성을 고지하고 불필요한 투약을 자제하며 장기투약자에 대한 골절 예방, 골다공증 관리 등의 노력도 병행되어야 할 것이다.
▢ Background With the high morbidity rate of peptic ulcer disease (PUD) and gastro-esophageal reflux disease (GERD) around the world, currently there is an increase in the use of digestive disease medications. Commonly used medications for the diseases include H2 receptor antagonists (H2RAs) and proton pump inhibitors (PPIs). PPIs are known to inhibit acid secretion more effectively compared to H2RAs in terms of their underlying mechanism. In Korea, there is a dramatic increase in the frequency of use of the medication. Recent studies have indicated that the long-term use of PPIs may have an effect on increasing bone fracture. However, few studies have investigated the risk of bone fracture by long-term use of PPIs in Korea. Accordingly, it is required to examine the association between PPI use and bone fracture using large-scale data collected in clinical settings. ▢ Objective In the present study, we explore the association between PPI use and osteoporotic bone fracture. Specifically, we investigate any use, cumulative use, dose, recent use, and regular use of PPIs are associated with a increased risk of bone fracture. ▢ Methods The nested case-control study was conducted using insurance claims data, qualification data, and national health examination data provided in the Korea National Health Insurance Service. People treated with PPIs or H2RAs under the diagnosis codes of PUD and GERD (ICD-10 code: K21, K25, K26, K27, and K28) from 1 January 2006 to 31 December 2015, were eligible for subjects in the cohort. The cohort entry date is the day of the first drug administration (PPIs or H2RAs). We excluded patients who: (1) were aged less than 50 years based on the cohort entry date; (2) were treated with PPIs or H2RAs during the year before cohort entry date; or (3) had a diagnosis (or treatment) of cancer, AIDS, osteoporosis/bone fracture during the same period. People without the record of national health examination during the year before or after cohort entry date were also excluded due to data loss. Cases were defined as patients who experienced osteoporotic fracture for the first time (index date: fracture occurrence date) in the period from the cohort entry date to 2015. For each case, up to five controls were matched on sex, age (±5 year), bisphosphonate use, cohort entry date and follow-up duration. Drug exposures include any PPI use, cumulative number of days of PPI use, low dose use/ high dose use of PPI during the follow-up period, recent use of PPI (PPI use during one year before fracture), and recent regular use of PPI (the number of quarters when PPIs were administered for more than 28 days during one year before fracture). Conditional logistic regression models were employed to estimate odds ratio (OR), and provide 95% confidence interval (CI) and significance probability (p-value). Covariates include sociodemographic factors (such as sex, age, residential district, and income quintile), variables for medical examination and medical examination by interview (such as height, weight, BMI, drinking, smoking, and locomotion), and comorbidities (defined by specific drug use, or several diagnosis codes) that may have a direct or indirect effect on fracture. To examine additionally the factors affecting the occurrence of osteoporotic fracture by PPI use, we analyzed subgroups using matching variables such as sex, age, and bisphosphonate use. ▢ Results The number of patients treated with medications for PUD or GERD for the past ten years were 35,520,188 persons. After applying the exclusion criteria, osteoporotic fracture was found in 78,465 subjects (3.3%) among 2,388,137 during the study period. Subjects were 59,240 matched cases and 296,200 matched controls. 78.0% of total subjects were female. During the follow-up period, subjects administered with any PPI showed higher risk of fracture (OR 1.11; 95% CI: 1.08 - 1.13) compared to those who didn't take it. This result was statistically significant. Moreover, there was a higher risk of fracture in the greater cumulative number of days of PPI administration (p for trend <.0001). In particular, patients treated with PPIs for one year or more showed higher risk of fracture (OR 1.42; 95% CI: 1.32 – 1.52) compared to those who didn't take it. Both low dose taker or high dose taker showed higher risk of fracture compared to those who didn't take it. Subjects administered with PPIs during one year before fracture showed higher risk of fracture (OR 1.30; 95% CI: 1.27 - 1.33) than those who didn't take it. Dividing the one year period by 4 quarters, subjects administered with PPI for four weeks or more in all quarters showed higher risk of fracture (OR 1.37; 95% CI: 1.26 - 1.50). In addition, there was an increased risk of osteoporotic fracture by PPI use in both male and female subjects. The risk of fracture by PPI use tended to increase in highly aged subjects. Subjects who were treated, as well as not treated, with bisphosphonates showed also a high fracture risk by PPI use. ▢ Conclusions A great number of Korean people have been treated with medication for PUD and GERD. However, PPI administration in older age groups increased the risk of bone fracture. Moreover, it was found that continued PPI administration had a greater effect on increasing the risk. Therefore, it is required to check medical histories of patients before PPI administration. Especially, for elderly patients requiring long-term PPI administration, the risk of PPI use should be informed and needless administration should be limited. Also, for those who have been administered with PPIs for a long time, we should make an effort to prevent bone fracture and manage osteoporosis.
