▢ 연구 배경 신장이식은 말기신부전 환자가 선택할 수 있는 최선의 신대체요법이다. 만성콩팥병 환자의 수명 연장 및 삶의 질 향상과 함께 개인 및 사회적인 비용을 절감할 수 있는 중요한 치료법이다. 현재 국내에서 연간 2,000건 이상의 신장 이식이 이루어지고 있고, 이 중 900-1,000 건의 신장 이식이 생체 공여자로부터 공여받은 신장에 의해 이루어지고 있다. 그런데 생체 신장 공여자의 공여 후 합병증 및 예후에 대한 국내 근거가 매우 부족하다. 공여 후 장기 생존율, 말기신부전 발생 위험도, 공여 후 신 손상 회복에 미치는 인자와 같은 생체 신장 공여자의 안전에 중요한 의학적 문제에 대한 연구가 매우 부족한 실정이다. 또한 생체 신장 공여자의 심리적, 사회 경제적 문제와 같은 비의학적 문제에 대하여도 알려진 바가 많지 않다. 따라서 생체 신장 공여자의 단ㆍ장기 합병증 및 예후 확인, 공여 후 합병증 발생의 위험 요인 규명, 공여자의 수술 관련 합병증, 공여 후 신기능 회복 정도에 대한 임상적 분석과 함께 공여 후 심리적 상태, 사회 경제적 문제에 관련된 실태 조사가 필요하다.
▢ 연구 목적 본 연구는 국내 생체 신장 공여자의 합병증 및 예후에 대한 임상적 분석과 설문을 통한 공여 전후 변화를 분석하여 국내 생체 신장 공여의 기준 및 공여 후 장기 관리 지침을 제시하고, 생체 신장 공여의 다면적 지원을 위한 정책 제안의 근거를 마련하고자 한다.
▢ 연구 방법 Ⅰ. 후향적 다기관 등록 자료 분석 결과 일차적으로 각 국립대학교병원의 병원자료를 가지고 임상 정보와 검사 결과 및 수혜자와의 관계, 새롭게 발생한 합병증 등에 대한 자료를 수집하였고, 신장 공여자의 임상적 예후를 정확하게 파악하기 위해 신장 공여자 관련 코호트 자료와 국민건강보험공단이 보유한 청구자료, 자격 자료와의 연계를 시행하여 연구의 결과 변수를 타당성 있게 파악하였다 ● 연구 대상자 - 국립대병원(서울대학교병원, 분당서울대학교병원, 서울특별시 보라매병원, 전남대학교병원, 전북대학교병원, 경북대학교병원, 부산대학교병원)에서 1979년 1월부터 2018년 12월까지 신장이식을 위해 신장 적출술을 받은 생체 신장 공여자
● 대조군 - 서울대학교병원 및 분당서울대학교병원 건강검진 수검자 가운데 신기능에 대한 확인이 가능하며 수검 당시 신기능이 정상인 수검자
공여자와 건강한 일반 인구 집단 매칭을 수행하여 연구에 등록된 공여자의 기저 특성이 군 간의 차이가 있는지를 분석하였으며, 연구 대상자의 성별, 소변검사결과와 같이 범주형 변수는 빈도와 백분율로 제시하였다. 연령, 검진요인(혈압, 체질량지수 등)등 연속형 변수는 중앙값 및 사분위 범위로 제시하였고, 세부적으로 정의된 범주형 기준에 따라 빈도와 백분율로 제시하였다. 연속형 변수의 경우 독립표본 t-검정, 범주형 변수의 경우 카이제곱 검정 방법을 적용하여 두 군간 차이가 있는지를 분석하였다. 두 군에서 기저 특성이 연도 군집 간의 차이가 있는지 연속형 변수의 경우 일원분산분석을 적용하였고, 범주형 변수의 경우 카이제곱 검정 방법을 적용하여 분석하였다. 연도가 변화함에 따라 기저 특성이 증가 또는 감소의 추세가 있는지 연속형 변수의 경우 피어슨 상관계수를 이용한 상관분석, 범주형 변수의 경우 카이제곱 추세검정, 코크란-아미티지 추세 검정방법을 적용하여 분석하였다. 대사 위험도에 대해 시간에 따른 두 군 간의 추세선 차이가 있는지를 확인하고자 로지스틱 회귀모형에 공여 여부와 연도 군집 간의 상호작용 항을 추가하여 유의확률을 제시하였다. 제시된 유의확률이 매칭으로 인한 변동성 및 편향을 갖는지 확인하기 위해 부트스트랩 방법을 사용하여 민감도 분석을 시행하였다. 장기 예후 지표를 구하기 위해 콕스 비례 위험 모형을 적용하여 유의한 예후 지표를 선정 후 단변수 분석에서 유의한 지표들을 다변수 모형에 포함함으로써 예후 예측 인자를 확인하였다. 이후 기존 예후 예측 모형을 본 연구 대상 집단에 적용 후, 이를 한국인 자료로만 구성한 예후 예측 모델과 예측력을 Harrell의 일치성 지수를 통해 비교하였다.
Ⅱ.생체 신장 공여에 대한 설문조사 생체 신장 공여에 대한 인식 및 태도와 공여 전후의 변화를 조사하기 위하여 일반인 집단을 대상으로 인터넷 설문을 진행하였으며, 생체 신장 공여에 대한 사전 인식, 공여의 목적/대상, 공여 이후 예상 변화, 공여 이후의 지원 필요 여부 등을 조사하였다. 또한 공여예정자/공여자를 대상으로 구조화된 설문지를 이용하여 대면 설문조사를 각각 시행하였으며, 공여 동기, 공여 시 우려되는 부분, 삶의 질, 의학적/사회경제적 평가, 통증, 공여에 대한 의견, 공여 시 지출한 의료 및 관련 비용 내역 등을 조사하였다.
● 설문 대상자 - 일반인 집단 설문 조사 2020년 1월 주민등록인구현황에 근거하여 지역별, 성별, 연령별 비례할당 후 무작위 익명화 추출된 일반인 - 공여예정자/공여자 설문 조사 서울대학교 병원과 계명대학교 병원에서 생체 신장 공여를 계획한 예정자 및 생체 신장 공여를 진행한 공여자
일반인 집단 설문 조사는 2020년 1월부터 2020년 5월까지 진행되었으며 총 설문 참여자는 1,000명이었다. 공여예정자/공여자 설문조사는 2020년 2월부터 2020년 11월 까지 시행되었고, 총 설문 참여자는 공여 전 115명, 공여 후 276명이었다.
각 조사문항자료에 대해서는 응답 빈도 및 분율을 확인하기 위해 빈도분석을 하였고, 맥니마 검정 및 대응표본 T 검정 방법을 적용하여 제시문 노출 전/후 및 공여 전/후 차이가 있는지 분석하였다.
▢ 연구 결과
후향적 다기관 등록 자료 분석 결과 ⦁생체 신장 공여자의 수가 급격히 증가하고 있으며, 60세 이상의 공여자의 수 역시 10%에 달할 정도로 증가하였고, 배우자 간 이식이 증가하고 있는 추세임. ⦁고요산혈증, 수축기혈압 140 mmHg 이상인 환자의 비율 및 과체중/비만 환자는 시간이 지나면서 증가하는 추세임. 그러나 이 증가 추세는 비슷한 건강 상태를 가진 일반인구집단과 비교하였을 때 공여자와 일반인구집단에서 큰 차이를 보이지 않았음. ⦁초기 추정 사구체여과율과 1개월 째 추정 사구체여과율의 누적 위험 정도를 평가하였을 때, 초기 추정 사구체여과율이 낮은 경우, 1개월 째 추정 사구체여과율의 변화량에 따라 신장 생존 위험이 뚜렷하게 낮아짐을 확인하여, 공여 후 신기능에 대한 면밀한 관찰이 필요함을 확인함. ⦁생체 신장 공여자는 매칭된 대조군에 비해 장기 추적 관찰 시 사망률이 높아지는 것으로 보였으나, 사회 경제적 수준 및 거주 지역을 추가로 보정하였을 때 사망률의 차이가 없어지는 것을 확인함. 이는 생체 신장 공여자의 사망률에 사회 경제적 수준 및 거주 지역이 중요한 변수로 작용할 수 있음을 의미하는 것으로 생각됨. ⦁본 연구를 통해 확인된 예측인자를 사용하여 기존보다 비열등 또는 우월한 공여자 말기신부전 진행 예후 예측 모형을 확립함. ⦁신장 공여자는 대조군과 비교하여, 공여 직후 고용 불평등, 건강 가입자격의 하향 변화 등의 경제적 불평등을 겪고 있음을 확인함.
생체 신장 공여에 대한 설문조사 결과 ⦁일반 인구 집단 설문에서 공여 설명문 노출 후 공여를 안전하게 생각하는 비율이 유의하게 늘었으며(31.9% → 48.2%), 약 73% 응답자가 공여자에 대한 경제/사회적 정책 지원의 필요성에 동의하였고, 공여 설명문 노출 후 그 비율이 81.3%로 유의하게 늘어남을 확인함. ⦁공여예정/공여자 설문에서 공여 후의 주관적 건강 점수, 삶의 질, 우울 척도가 유의하게 악화됨을 확인하였으며, 공여자 중 약 64%가 공여 후 신체적 문제를 보였고, 약 34%에서 경제/사회적 악화를 경험했다고 확인됨.
Ⅰ. 후향적 다기관 등록 자료 분석 결과
1. 생체 신장 공여자 현황 1982년부터 2016년까지 7개 국립대학병원에서 다른 사람에게 신장 공여를 한 생체신장 공여자는 총 2,898명으로, 생체 신장 공여자 수는 1982년 1명에서 매년 꾸준히 증가하여 2016년에는 242명까지 증가하였으며, 2010년 이후로 더욱 급속한 속도로 증가하였다. 공여자의 평균 연령은 2000년대 중반에는 37세 전후였으나, 점차 증가하여 2016년에는 46.2세까지 늘어났으며, 60세 이상의 환자 비율이 1990년대 후반 2%였던 것에 비해 2016년에는 10%에 달하여 고령에서의 생체 신장 이식이 이전에 비해 활발하게 이루어지고 있음을 알 수 있었다. 공여자의 성별은 연도에 따라 아주 큰 변화는 없었으나, 대부분의 연도에서 여성의 비율이 50% 이상으로 조금 더 높은 수치를 보였다. 공여자-수혜자 관계는 배우자 간 신장 공여가 1988년 이전에 3.6%였던 것에 반해 2016년에는 31.0%까지 급격히 상승함을 확인할 수 있다. 1988년 이전에는 부모-자식, 형제자매, 친척, 비혈연관계, 배우자 순이었으나, 2016년에는 부모-자식, 배우자, 형제자매 순으로 그 순위가 변동되었다. 2. 대사 위험도 비교 신장 공여자와 건강검진을 시행받았던 건강검진 수검자를 나이, 성별, 기저 사구체여과율, 연도를 기준으로 1:1 매칭을 시행하였고, 생체 신장 공여자 2,018명, 대조군 2,018명이 선정되었다. 본 연구에서는 대사 위험도를 다음과 같은 세 가지 기준으로 확인하였다: 1) 고요산혈증(남성 7 mg/dL 이상, 여성 6 mg/dL 이상), 2) 수축기 혈압 140 mmHg 이상, 3) 체질량지수 25 kg/m2 이상의 과체중 및 비만 여부. 고요산혈증의 유병률은 공여자 및 매칭된 대조군 모두에서 시간이 지남에 따라 급격히 상승하였으며, 두 군 간 유병률 상승의 경향에는 큰 차이가 없는 것으로 확인되었다. 혈압의 경우, 공여자의 경우 수축기혈압 140 mmHg 이상인 공여자 비율이 8-9%사이로 유지되었으며, 매칭된 대조군의 경우 2006년까지는 공여자와 비슷한 수준이었으나, 2007-2011년, 2012-2016년에는 각각 12.2%, 11.9%로 이전에 비해 상승하였다. 두 군 간 수축기 혈압 140 mmHg 이상인 환자의 비율의 경향에는 차이가 없는 것으로 확인되었다. 과체중 및 비만이었던 환자의 비율은 전 기간 동안 공여자에서 매칭된 대조군에 비해 높은 편이었다. 공여자 및 매칭된 대조군의 과체중 및 비만 관련 경향 비교 분석에서는 두 군간 차이는 없는 것으로 확인되었다.
3. 생체 신장 공여자의 공여 후 신기능 회복에 대한 연구 1982년부터 2018년까지 총 7개 국립대학교병원에서 신장 공여를 위한 신절제술을 받은 환자 3,456명 중 1년 이상의 추적 기간을 가지고, 1개월 째 혈청 크레아티닌 값을 갖는 환자 총 1,358명을 대상으로 분석을 시행하였다. 평균 연령은 43.8세, 635명(46.8%)이 남성, 평균 초기 추정 사구체여과율은 99.0 mL/min/1.73 m2, 평균 추적 기간은 61.8 개월이었고, 약 4년의 추적 기간 중 만성콩팥병 3~5기로 진행한 환자는 총 183(13.4%)이었으며, 이 중 2명이 4기, 1명의 공여자가 5기까지 진행하였다. 마지막 추적 추정 사구체여과율 기준으로 만성콩팥병 3기에 해당하는 공여자는 총 183명(13.5%)였고, 1-2기에 해당하는 환자군과 비교한 결과 나이, 성별, 수축기 혈압, 체질량지수, 헤모글로빈, 혈당, 요산, 초기 추정 사구체여과율, 1개월 째 추정 사구체여과율, 1개월 째 추정 사구체여과율 변화율이 양군 간 유의한 차이를 보였다. 만성콩팥병 발생에 유의한 위험인자인 초기 추정 사구체여과율과 1개월째 추정 사구체여과율 변화율의 누적 위험 정도를 평가하기 위해 각각의 변수에 대해 중앙값을 이용하여 두 군으로 나누고, 두 가지 변수를 조합하여 총 4개의 그룹에 대한 신장 생존 분석을 시행하였을 때, 초기 추정 사구체여과율이 좋은 경우, 1개월 째 추정 사구체여과율의 변화량에 따라 신장 생존에 유의한 차이가 없었으나, 초기 추정 사구체여과율이 상대적으로 낮은 경우, 1개월째 추정 사구체여과율의 변화량에 따라 신장 생존 위험이 뚜렷하게 차이 나는 것을 확인하였다.
4. 생체 신장 공여자의 장기 추적관찰 결과에 대한 연구 2003년부터 2016년까지 7개 국립대학교병원에서 신장 공여를 한 생체 신장 공여자와 대조군으로 서울대학교병원에서 건강검진을 시행한 일반인구집단 105,055명 중 일부 배제 기준을 만족하는 환자를 제외하고 나이, 성별, 기저 신기능, 체질량지수, 당뇨 및 고혈압 유무, 알부민뇨의 유무를 가지고 매칭한 공여자 및 대조군은 각각 1,701명이었고, 매칭 후 환자를 기준으로 하였을 때는 생체 신장 공여자 중 4명이, 건강검진 대조군에서는 말기신부전으로 진행한 환자가 없는 것으로 나타났다. 사회경제적 수준 및 거주 지역을 포함하여 다변수 분석을 시행하였을 때, 생체 신장 공여자와 대조군 사이에 사망률에는 차이가 없는 것으로 본 연구에서 확인하였다.
5. 생체 신장 공여자의 장기 예후 예측 모형 개발 기존에는 연령, 성별, 체질량지수, 수축기혈압, 기저 신기능, 고혈압, 당뇨 등의 인자를 이용한 신기능 장기 예후 예측 모형을 구성하였다. 본 연구에서는 말기 신부전 예측 인자로 연령, 거주 지역, 기저 신기능, 혈청 알부민 수치, 당뇨, 고혈압, 악성 종양의 병력, 혈청 고밀도 콜레스테롤 수치가 확인되었고, 사망 예측 인자로 연령, 성별, 소득분위, 수축기 혈압, 기저 빈혈 여부, 혈청 인, 요산, 알부민, 적혈구 침강 속도, C 반응성 단백, 고밀도 콜레스테롤, 중성지방 수치, 간 기능 관련 혈청 검사 결과, 기저 신기능, 기저 단백뇨, 당뇨, 고혈압, 악성 종양의 병력, 심혈관계 질환의 병력이 확인되어 말기신부전으로의 진행 예후 예측 모형을 구성하였으며, 본 연구 결과를 통해 구성한 예후예측 모형이 비열등 또는 우월한 예후 예측 결과를 가짐을 알 수 있었다.
6. 생체 신장 공여자의 공여 전후 건강보험 가입자격 변화에 대한 분석 신장 공여자는 신장 공여 직후에 기존의 피고용 상태를 유지하지 못할 확률이 높았으며 새로 고용 상태를 획득할 확률이 대조군에 비해 유의하게 낮았다. 단 이러한 고용 불평등은 이식 후 2년째부터는 나타나지 않았다. 생체 공여자들은 이식 후 건강보험 가입 자격이 급여 가입자로 변동하는 경우가 일부에서 발생하였으며 절대적인 비율은 약 2% 정도로 적었지만 대조군에 비해서는 유의미하게 높았다. 생체 이식 공여자들은 세대 경제력 분위가 대조군에 비해 상승하는 경우가 적었고, 하강하는 경우는 유의하게 높았다. 이러한 경제력 불평등은 공여 후 5년 후까지 지속적으로 확인이 되었다. Ⅱ.생체 신장 공여에 대한 설문조사 결과
1. 일반인 인구 집단 설문 일반 인구 집단 설문 결과 생체 신장 공여 자체에 대한 인지도는 81.1%로 높은 편이나 실제 생체 신장 공여의 자세한 내용에 대한 인지는 42.5%로 나타났다. 생체 신장 공여의 안전 여부 및 건강 영향 여부를 묻는 설문에서는 실제 공여 시 사용되는 설명문 노출 이후 생체 신장 공여를 좀더 안전하고, 건강에 영향을 주지 않을 것이라는 방향으로 유의하게 변화하였다. 다만 이와 같은 변화가 실제 신장 공여 의향 여부로 연결되지는 않은 것으로 나타났다. 일반 인구 집단에서는 공여 의향의 1순위(69.7%, 한 사람의 생명을 구하는 것에 보람을 느껴서), 2순위(67.7%, 수혜자의 건강을 위해서) 모두 이타적 이유로 나타났으며, 공여를 꺼리는 이유는 1순위(69.1%, 공여 후 발생할 수 있는 합병증에 대한 두려움), 2순위(54.4%, 공여 후 남은 신장 및 다른 장기에 영향을 줄까 두려워서) 모두 신체적 문제로 나타났다. 또한 공여자에 대한 경제/사회적 지원의 필요성을 73.2%의 응답자가 공감하였으며, 이는 설명문 노출 이후 81.3%로 더 증가하였다.
2. 공여예정자·공여자 설문 공여 예정자를 대상으로 한 설문에서도 공여를 결정한 이유로 이타적인 이유가 1순위(59.1%, 수혜자 개인의 건강과 행복)와 2순위(29.6%, 가족 혹은 수혜자·본인이 포함된 집단 전체의 행복과 이익)로 나타났으며, 공여 후 가장 우려되는 항목에 대한 설문에서 1순위(22.6%, 수술과 관련된 합병증), 2순위(20.0%, 본인의 신장 기능 악화) 모두 신체적 문제로 나타났다. 또한 공여자를 대상으로 수행한 공여 전후 비교 설문에서 공여 이후 유의한 수준으로 주관적 건강 점수의 하락이 관찰되었으며, 삶의 질(Short Form 36 Health Survey) 및 우울 척도(Patient Health Questionnaire-9)의 유의한 악화가 확인되었다. 공여자를 대상으로 한 설문에서는 약 64%의 환자가 공여로 인한 합병증을 겪었고, 공여 이후 통증이 합병증 중에서 가장 높은 비율(38%)로 나타났다. 공여 이후 경제적·사회적인 변화는 약 34%의 환자가 겪었으며 공여 과정에서 지출한 검사 및 수술 비용 등으로 인한 경제적 어려움을 다수의 공여자가 경험하였다. 현재 보험 체계에서 이러한 부분에 대한 환급 절차가 있으나, 실제 환급 여부를 묻는 설문에서 약 26%의 환자가 환급을 받지 않았다고 답변하였고, 실제 환급 비용을 묻는 설문의 응답률도 47.5% 였다.
▢ 결론 및 정책적 제언 본 연구에서는 전국 7개의 국립대학교에서 1982년 이후 신장이식을 시행 받은 환자들의 병원 내 의무기록 자료와 건강보험공단 자료의 연계를 통해 기존에 부족하였던 국내 생체 신장 공여자에 대한 근거 자료를 마련하였다. 먼저, 국내 생체 신장 공여자의 기본적인 역학 조사를 통해 대사위험도를 보이는 공여자의 증가 추세를 확인하였다. 이는 공여 후 장기 예후에 영향을 미칠 것으로 생각되는데, 실제 공여자 선정의 기준 변화가 원인인지, 일반적인 한국인의 대사위험도 변화가 원인인지를 확인하기 위해 비슷한 건강 상태를 확보한 대조군과 비교하였고, 결과적으로 국내 생체 신장 공여자의 선정 기준의 변화가 원인이 아님을 확인하였다. 국내 생체 신장 공여자의 장기 생존율은 의학적 기준만을 고려했을 때, 비슷한 건강 상태를 확보한 대조군에 비해 사망률이 높아지는 결과를 보였으나, 사회 경제적 수준 및 거주 지역과 같은 비의학적 요인을 추가하여 분석하였을 때 이러한 사망률의 차이가 상쇄되는 결과를 보여, 공여 후 사회 경제적 변화에 대하여 분석하였고, 공여 후 사회경제적 변화 상태를 건강보험료를 기반으로 추산한 소득 분위를 통해 추정하였을 때, 생체 신장 공여자는 공여 후 경제적 상태가 악화될 가능성이 있음을 확인하였다. 공여 후 신장 예후에 대하여는 공여 후 1개월 째 사구체여과율의 변화량에 따라 신장 생존 위험이 뚜렷하게 낮아짐을 확인하여 공여 후 신기능에 대한 면밀한 관찰이 필요함을 확인하였다. 또한 본 연구를 통하여 확인된 예측 인자를 사용하여 공여자의 사망 및 말기 신부전 진행 예후 예측 모형을 확립하였으며, 일반인 및 공여 경험자에 대한 면밀한 설문 조사를 통해 공여 후 신체적, 심리적, 경제적, 사회적 변화를 겪고 있음을 확인하였다. 대한민국은 전 세계적으로 생체 신장 공여에 의한 신장 이식 수혜율이 높은 국가이다. 생체 신장 공여자의 장기 안전성에 대한 우려가 증가하는 상황에도 불구하고, 본 연구를 통해 국내 생체 신장 공여자의 장기 생존율은 비슷한 의학적 건강 상태를 확보한 대조군과 다르지 않음을 확인하였다. 그러나 과거에 비해 증가하여 외국과 차이가 줄어들고 있는 대사합병증의 증가를 고려할 때, 향후 국내 생체 신장 공여자의 장기 생존에 미칠 영향에 대하여는 면밀한 관찰 및 주의가 필요할 것이다. 또한 의학적 요건이외에 사회경제적 요인에 의해 생체 신장 공여자의 장기 생존이 낮아질 수 있고, 실제 공여 후 경제적 상태가 악화될 수 있다는 본 연구의 분석 결과와 공여 경험자의 설문조사를 통해 확인된 공여에 의한 경제적 부담 및 공여 후 사회적 상태 변화를 고려할 때, 공여 후 사회 경제적 안전성을 확보하기 위한 정책적 대안을 마련할 필요가 있다. 국내 생체 신장 공여자는 지속적으로 증가할 가능성이 높다. 생체 신장 공여자의 안전을 위한 의료 관리 시스템의 구축 및 관리 방안을 정립하기 위해 살아있는 신장 공여자의 공여 전 선정 및 공여 후 추적 관찰에 대하여 전향적으로 관리할 수 있는 국가 시스템을 만들고 체계적으로 관리할 수 있도록 하는 것이 시급하다. 이를 통하여 보다 정확한 자료를 확보할 수 있고, 타인에 대한 이타적인 마음으로 신장을 공여한 많은 생체 신장 공여자들에 대한 보건 정책 결정에 필요한 결과들을 도출해 낼 수 있을 것이다. 주요어 생체 신장 공여, 말기 신부전, 신장 이식, 공여 후 합병증, 생체 신장 공여자의 사회경제적 문제, 예후
▢ Background Kidney transplantation (KT) is the best renal replacement therapy for patients with end-stage kidney disease (ESKD). It is an important treatment that can reduce personal and social costs, prolong lifespan, and improve the quality of life of patients with chronic kidney disease (CKD). Of more than 2,000 KT performed annually in South Korea, 900-1,000 are from living donors. However, there is little evidence regarding the complications of kidney donation and the prognosis of living kidney donors in South Korea. There is a lack of research on medical issues that are crucial to the safety of living kidney donors such as long-term survival after donation, risk of ESKD, and factors affecting the recovery from acute kidney injury (AKI) after donation. In addition, little is known about non-medical problems such as psychological and socio-economic problems of living kidney donors. Therefore, investigation of the actual conditions related to psychological status and socio-economic problems of living kidney donors after kidney donation is necessary, including clinical analyses of the short-and long-term complications and prognosis of living kidney donors, risk factors for post-donation complications, complications related to kidney donation, and the degree of recovery from AKI.
▢ Objective By analyzing clinical factors before and after kidney donation and the survey of living kidney donors on changes after donation, we aimed to determine the criteria for living kidney donation, provide long-term management guidelines after donation, and propose policies for the multi-dimensional support of living kidney donors.
▢ Methods Ⅰ. Retrospective analysis of multicenter registration data Data on the clinical information of living kidney donors, laboratory results, their relationship to the recipient, and complications newly developed after donation were collected from the medical records data of the National University Hospitals. The research outcomes were also identified by linking the cohort data of kidney donors to the health insurance claims and qualification data managed by the National Health Insurance Service (NHIS). ● Subjects Living kidney donors included individuals who underwent nephrectomy for KT between January 1979 and December 2018 at the National University Hospitals (Seoul National University Hospital, Seoul National University Bundang Hospital, Seoul Metropolitan Government Seoul National University Boramae Medical Center, Chonnam National University Hospital, Jeonbuk National University Hospital, Kyungpook National University Hospital, and Pusan National University Hospital). Controls were people with test results showing normal kidney function at the time of medical checkup from Seoul National University Hospital and Seoul National University Bundang Hospital.
● Statistical methods We matched enrolled living kidney donors and controls and analyzed the differences in baseline characteristics at the time of donation and health examination, respectively. Categorical variables such as sex and urine dipstick results were presented as frequencies and percentages. Continuous variables such as age and medical checkup parameters (blood pressure, body mass index (BMI), etc.) were presented as medians and interquartile ranges and as frequencies and percentages according to the categorical criteria defined in detail. Differences between the groups were analyzed by independent-sample t-tests and chi-square tests for continuous and categorical variables, respectively. The differences in baseline characteristics between the clusters by years in the two groups were assessed by one-way analysis of variance and chi-square tests for continuous and categorical variables, respectively. The trends of increasing or decreasing baseline characteristics as annual changes were assessed in correlation analyses using Pearson correlation coefficients in continuous variables and chi-square trend and Cochran-Armitage trend tests for categorical variables. To evaluate the differences in the trend lines of metabolic risk between the two groups, the significance probability was presented by adding interaction parameters; namely, the donor group and year cluster, to the logistic regression model. Sensitivity analysis was performed using the bootstrap method to check for variability and bias in the significance probability due to matching. To obtain long-term prognostic factors, the Cox proportional-hazards model was applied to confirm significant prognostic factors, which were then added to the multivariable model to identify prognostic predictors. Subsequently, the existing prognosis prediction model was applied to the enrolled subjects of this study. We then compared the predictive power between the newly developed and existing prognosis prediction models using Harrell's concordance index.
Ⅱ. Survey of living kidney donation Internet surveys were conducted on the general population to investigate the changes in perceptions and attitudes regarding the living kidney donation and prior recognition of living kidney donation, the purpose of donation, relationship to the potential recipient, expected changes after donation, and the need for support after donation. Face-to-face surveys using a structured questionnaire were conducted of potential kidney donors and living kidney donors to assess their willingness to donate, concerns during donation, quality of life after donation, changes in medical and socio-economic state, pain after donation, opinions on the donation, and medical and related expenses that occurred at the time of donation.
● Subjects The general population survey was applied to the randomly extracted anonymized general population after proportional allocation by region, sex, and age based on the registration population in January 2020. The living kidney donor survey was applied to potential kidney donors and/or living kidney donors from Seoul National University Hospital and Keimyung University Dongsan Hospital. The general population survey was conducted from January to May 2020 and included a total of 1,000 survey participants. The survey of potential kidney donors and living kidney donors was conducted from February to November 2020 and included a total of 115 potential kidney donors and 276 living kidney donors.
● Statistical methods For each questionnaire, a frequency analysis was performed and McNemar's and paired t-tests were applied to determine whether there was a difference in responses before and after the description of kidney donation operation and before and after kidney donation.
▢ Results
Retrospective analysis of multicenter registration data ● The number of living kidney donors is increasing rapidly, the number of elderly donors over 60 years of age has increased to 10%, and the trend of KT between spouses is increasing. ● The proportion of patients with hyperuricemia and/or systolic blood pressure over 140 mmHg and/or overweight and/or obese tended to increase over time. However, this trend did not differ significantly between the living kidney donors and the general population with similar health conditions. ● Evaluation of the initial estimated glomerular filtration rate (eGFR) and the cumulative risk of eGFR at 1 month confirmed a significantly reduced risk of kidney survival if the initial eGFR was low. These results confirmed the need for close observation of kidney function after donation. ● In long-term follow-up, the mortality rate of living kidney donors was higher than that in the matched controls. However, this difference disappeared after adjusting for the socio-economic level and residential area. Thus, socio-economic level and residential area could be important variables related to the mortality rate of living kidney donors. ● Using the predictors identified in this study, we established a new model for predicting the progression of ESKD in living kidney donors. This new model was non-inferior or superior to the existing prognosis prediction model. ● We confirmed that living kidney donors experienced economic inequality such as employment inequality immediately after donation as well as a downward change in health insurance coverage status compared to those in the controls.
Survey of living kidney donation ● In the survey of the general population, the percentage of respondents who thought that donation was safe increased significantly after receiving a description of the kidney donation operation (31.9% → 48.2%) and approximately 73% of respondents agreed that there was a for socio-economic policies to support living kidney donors, a proportion that increased significantly to 81.3% after exposure to a description of the kidney donation operation. ● The results of the survey of potential kidney donors and living kidney donors confirmed that the subjective health score, quality of life, and depression scale significantly deteriorated after donation. Approximately 64% of living kidney donors experienced physical problems after donation and approximately 34% experienced socio-economic deterioration.
Ⅰ. Retrospective analysis of multicenter registration data 1. Status of living kidney donors From 1982 to 2016, there were 2,898 living kidney donors at seven National University Hospitals. The number of living kidney donors steadily increased from 1 in 1982 to 242 in 2016, with a rapid increase since 2010. The average age of living kidney donors was about 37 years in the mid-2000s and has gradually increased to 46.2 in 2016. The proportion of living donor kidney donors over 60 years of age reached 10% in 2016 compared to 2% in the late-1990s. Thus, living donor kidney transplantation is increasingly performed in older age groups. The sex of donors did not differ much from year to year, although the proportion of women was slightly higher (over 50%) in most years. Regarding the relationship between the donor and recipient, KT between spouses increased sharply to 31.0% in 2016, compared to 3.6% before 1988. Before 1988, the relationships between donor and recipient were in the order of parent-child, sibling, relative, non-related, and spouse, however, in 2016, the ranking had changed to parent-child, spouse, and sibling, in order.
2. Metabolic risk comparison After 1:1 matching for age, sex, baseline eGFR, and year, the living kidney donor and control groups each comprised 2,018 individuals, who were included in the analysis. In this study, the metabolic risk was assessed based on three criteria: 1) hyperuricemia ( > 7 mg/dL in men and > 6 mg/dL in women), 2) systolic blood pressure >140 mmHg, and 3) overweight and obese by BMI over 25 kg/m2. The prevalence of hyperuricemia increased rapidly over time in both living kidney donors and matched controls, with no significant difference in the trend of prevalence between the two groups. Regarding blood pressure, 8-9% of donors had systolic blood pressure >140 mmHg, while the proportion of matched controls with systolic blood pressure >140 mmHg was similar to that in the donor group until 2006; however, in 2007-2011 and 2012-2016, the proportions increased to 12.2 % and 11.9%, respectively. No difference in the trend of the proportion of patients with systolic blood pressure > 140 mmHg between the two groups was observed. The proportion of overweight and/or obese living kidney donors during the entire follow-up period was higher than that in the matched controls. A comparative analysis of trends related to overweight and obesity between donors and matched controls showed no differences between the two groups.
3. Recovery of renal function after living kidney donation From 1982 to 2018, among 3,456 patients who underwent nephrectomy for living kidney donation at seven national university hospitals, 1,358 patients with a follow-up period of 1 year after donation and available serum creatinine values at 1 month after donation were finally included in the analysis. The average age was 43.8 years, 635 (46.8%) were male, the average initial eGFR was 99.0 mL/min/1.73 m2, and the average follow-up period was 61.8 months. During the follow-up period of 4 years, 183 (13.4%) donors progressed to stage 3-5 CKD, including two who progressed to stage 4 and one who progressed to stage 5. Based on the eGFR at the last follow-up, 183 donors (13.5%) had stage 3 CKD. Significant differences were observed in age, sex, systolic blood pressure, BMI, hemoglobin, serum glucose level, uric acid concentration, initial eGFR, eGFR at 1 month after donation, and rate of eGFR change at 1 month after donation compared to those in donors with CKD stage 1-2. To evaluate the cumulative risk of the initial eGFR and rate of eGFR change at 1 month after donation, which are significant risk factors for CKD, we divided the donors into two groups using the median value for each variable and performed kidney survival analysis for four groups obtained by combining each group. If the initial eGFR was good, no significant difference in kidney survival was observed according to the rate of eGFR change at 1 month after donation; however, if the initial eGFR was relatively low, the risk of kidney survival differed significantly according to the rate of eGFR change at 1 month after donation.
4. Long-term follow-up of living kidney donors Initially, living kidney donors at the National University Hospitals from 2003 to 2016 and 105,055 individuals from the general population who underwent health checkups at Seoul National University were identified. After matching by age, sex, baseline eGFR, BMI, history of diabetes mellitus (DM), history of hypertension, and presence of albuminuria, 1,701 living kidney donors, and 1,701 controls were included in the analysis. After matching, four living kidney donors progressed to ESKD and no patients progressed to ESKD in the controls. Multivariate analysis including parameters of socio-economic level and residential area was performed, and there was no difference in mortality between living kidney donors and controls. 5. Development of a model for predicting long-term prognosis of living kidney donors The factors constituting the existing long-term prediction model for kidney function were age, sex, body mass index, systolic blood pressure, basal renal function, hypertension, and DM. In this study, we identified age, residential area, baseline eGFR, serum albumin level, history of DM, history of hypertension, history of malignancy, and high-density lipoprotein level as prognostic predictors of ESKD. We also identified age; sex; income status; systolic blood pressure; anemia; erythrocyte sedimentation rate; concentrations of serum phosphorus, uric acid, albumin, C-reactive protein , triglyceride, aspartate aminotransferase, alanine aminotransferase, and gamma-glutamyl transferase; baseline eGFR; baseline proteinuria level; and histories of DM, hypertension, malignancy, or cardiovascular disease as prognostic predictors of mortality. Using the predictors identified in this study, a model for predicting the progression of ESKD in living kidney donors was then established, which was non-inferior or superior to the existing prognosis prediction model.
6. Changes in health insurance qualification status after donation of living kidney donors Living kidney donors tended to lose their jobs immediately after donation; moreover, the probability of getting a new job was significantly lower than that of controls. However, this employment inequality disappeared in the second year after donation. Approximately 2% of living kidney donors changed health insurance qualifications to medical care after donation, a significantly higher rate than that in the controls. The income quartile status of living kidney donors was less likely to increase and the decrease in income quartile status was significantly higher compared to those in the controls. These economic inequalities were continuously observed until 5 years after donation.
Ⅱ. Survey of living kidney donation 1. General population survey In the general population, 81.1% were aware of living kidney donation, while 42.5% knew the details of living kidney donation. In the questionnaire asking whether or not living kidney donation was safe and whether it affected donor health, there was a significant increase in those believing that living kidney donation was safer and would not affect health after exposure to the description of the kidney donation operation. However, this change did not lead to an increased intention to donate. In the general population, the most common reasons explaining a willingness to donate a kidney were altruistic (69.7%, saving a life feels rewarding; 67.7%, for the recipient's health). The most common reasons to explain a reluctance to donate a kidney were physical problems (69.1%, fear of possible medical complications after donation; and 54.4%, fear of effects on the kidneys and other organs after donation). In addition, 73.2% of the respondents agreed that there was a need for socio-economic support for living kidney donors, which increased to 81.3% after exposure to the description of the operation.
2. Living kidney donor survey Among potential kidney donors, the most common reasons for deciding kidney donation were altruistic (59.1%, for the recipient's health and happiness; 29.6%, for the happiness and benefit of the family or group including the donor and recipient). The most common concerns after donation were related to physical problems (22.6%, fear of possible medical complications after donation; 20.0%, fear of deteriorating kidney function after the donation). In the comparison survey before and after donation among living kidney donors, significant deteriorations in subjective health score, quality of life (Short Form 36 Health Survey), and depression scale (Patient Health Questionnaire-9) were observed after donation. Approximately 64% of living kidney donors experienced medical complications after donation, most commonly pain (38%). About 34% of patients reported experiencing socio-economic changes, including financial difficulties due to the costs of tests and surgery in the donation process. Although the current insurance system provides reimbursement, approximately 26% of patients reported that they did not receive a reimbursement; among those who did receive reimbursement, the total cost of reimbursement was 47.5%.
▢ Conclusions The results of this study provide additional data on living kidney donors, which were previously insufficient, by linking the medical records of patients with kidney transplantation since 1982 at seven national universities nationwide to data from the Health Insurance Service in South Korea. First, the increasing trend of metabolic risk in living kidney donors was confirmed through epidemiological investigations of living kidney donors. As this increased metabolic risk could affect the long-term prognosis of living kidney donors, we compared donors to controls of similar health status to differentiate between a change in donor selection criteria or in the metabolic risk of the general South Korean population. The results confirmed that there was no problem with the selection criteria for living kidney donors. The long-term survival rate of living kidney donors in South Korea was higher than that in the controls with similar health status when considering only medical criteria; however, the difference disappeared after considering non-medical factors such as socio-economic level and residential area. Therefore, we investigated the socio-economic change after donation in living kidney donors by analyzing the earning quintiles estimated based on health insurance premiums and confirmed a worsened economic status of living kidney donors after donation. We confirmed a significantly lowered risk of renal survival according to the rate of eGFR change at 1 month after donation. Therefore, close observation of kidney function after donation is required. We also established a new model for predicting the progression of death and ESKD among living kidney donors that used the predictors identified in the present study. Finally, we confirmed the physical, psychological, and socio-economic changes after the donation by applying a questionnaire survey to the general population and living kidney donors/potential kidney donors. South Korea has a high rate of living donor KT compared to other countries worldwide. Despite the growing concern regarding the long-term safety of living kidney donors, we confirmed that the long-term survival rate of living kidney donors in South Korea did not differ from that of the control group of participants with similar medical health status. However, close observation and attention should be paid to the effect of the long-term survival of living kidney donors in the future, considering the trend of increasing complications of metabolic disease compared and decreasing differences from other countries. In addition, a policy is needed that ensures socio-economic safety after donation based on the possibility of decreased long-term survival of living kidney donors due to socio-economic factors, deterioration of financial status after donation, and the economic burden of donation with changes in socio-economic status after donation observed in the present study. The number of living kidney donors will likely continue to increase. The establishment of a medical management system and management plan for the safety of living kidney donors urgently requires a national system to prospectively manage the selection of living kidney donors before donation and follow-up after donation. Through this system, more accurate data could be obtained, from which it would be possible to derive the data needed to make health policy decisions for living kidney donors.
