▢ 서 론 ‘치료연관 골수계 종양(therapy-related myeloid neoplasms)’은 선행암에 대한 항암화학요법이나 방사선치료에 의한 유전적 손상으로 인하여 새롭게 발생한 골수형성이상증후군, 급성 골수성 백혈병 등 골수계 혈액암을 의미한다. 항암화학요법과 방사선치료로 인한 세포의 손상으로 짧게는 1년에서 길게는 10년 이내에 발생하는 것으로 알려져 있으며 대다수는 5년 이내에 발생한다. 치료연관 골수계 종양은 1) 예후가 좋지 않고, 2) 선행암의 치료로 이미 상당한 부담을 겪은 환자들에게서 치료에 고비용이 소요되는 난치 혈액암의 속발로 인한 신체적, 경제적, 사회적 파탄의 우려가 있으며, 3) 학술적으로는 이차암 발생 기전 연구에 유용한 모델이라는 점에서 중요한 이차암 질환군이다. 최근 정밀한 유전자 분석 기술의 발전에 따라 선행암 진단 시 혈액검사에서 ‘클론성 조혈증(clonal hematopoiesis; 혹은 clonal hematopoiesis of indeterminant potential; CHIP)’으로 불리는 혈액 조혈세포의 일부 특정 유전자 변이가 존재할 경우 치료연관 골수계 종양 발생 위험이 증가한다는 점이 알려졌다. 이를 통하여 치료연관 골수계 종양은 혈액암의 병인 탐구를 위한 더욱 중요한 모델로 떠올랐다. 그러나 절대적인 발생률이 낮아 선행암으로 치료받은 환자들을 장기간 추적 관찰하는 양질의 대규모 암 코호트를 만들고 유지하기 어려워 그 중요성에 비하여 잘 구성된 연구가 드물다. 따라서 본 연구는 암등록자료와 건강보험 청구자료를 연계하여 치료연관 골수계 종양의 국내 현황을 파악하고 치료연관 골수계 종양의 발생 고위험군을 적절히 정의함으로써, 선제적인 유전자 검사와 암 백신 등 새로운 화학적 예방요법(chemo-prevention)에 대한 임상연구의 적절한 대상을 제시하고자 하였다. ▢ 연구방법 치료연관 골수계 종양의 발생 현황과 고위험군을 선별하기 위하여 국립암센터 암등록자료와 국민건강보험공단의 건강보험 청구자료를 연계하여 분석하였다. 2009년 1월 1일부터 2013년 12월 31일까지 선행암을 진단받은 암등록자료에 등록된 성인으로 선행암 코호트를 구축하였으며, 해당 대상자들의 2002년 1월 1일부터 2019년 12월 31일까지 건강보험 청구자료를 연계하여 치료연관 골수계 종양 발생에 대한 후향적 추적관찰 코호트(retrospective cohort)를 구축하였다. 연구대상자는 암이 발생한 신환자 중 항암화학요법 또는 방사선요법을 받은 환자로 정의하였으며 선행암으로는 일부 희귀암과 골수계 종양을 제외하고 독립적인 특징이 있는 원발성 고형암과, 림프계 종양, 형질세포종양을 분석 대상으로 정의하였다. 본 연구의 선행암 코호트에 등록된 환자들을 2009년 7월 1일부터 2019년 12월 31일까지 추적관찰하여 대상자의 치료연관 골수계 종양 발생 여부를 분석하였다. 대상자의 추적관찰은 치료연관 골수계 종양이 발생하거나, 본 연구의 최대 추적관찰 기간인 2019년 12월 31일이 되는 경우 종료하였다. 치료연관 골수계 종양은 급성 골수성 백혈병 (acute myeloid leukemia, AML)과 골수형성이상증후군(myelodysplastic syndrome, MDS)으로 정의하였으며 산정특례(V193, V194) 2회 이상 청구와 함께 국제질병분류(ICD-10)에 따라 아래의 상병코드를 이용하였다. 치료연관 골수계 종양의 위험도(risk) 분석은 선행암 코호트에 등록된 환자들 중 항암화학요법(단클론항체 치료 등 표적 치료 포함), 방사선치료, 방사성동위원소 치료 중 적어도 한 가지 이상의 치료를 받은 대상자의 선행암 진단 시 평균연령, 성별, 원발암의 병기와 치료 내역(항암화학요법 단독, 방사선치료 단독, 두 요법 병합, 방사성동위원소 단독 등으로 분류)을 선행암종별로 분석하고 대상자에서의 치료연관 골수계 종양의 표준화 발생비와 누적발생률을 분석하였다. 치료연관 골수계 종양의 발생 위험은 표준화 발생비(standardized incidence ratio, SIR=observed/expected)와 95% 신뢰구간(Poisson-based 95% confidence intervals)으로 추정하였다. 표준화 발생비는 일반인구집단에서의 골수계 종양 발생률에 대비하여 본 연구의 최종 코호트에서의 골수계 종양 발생률을 비교하는 비율의 형태이다. 이때 비교집단이 되는 일반인구집단은 우리나라 전체 인구로 정의하고 건강보험공단에 청구된 모든 골수계 종양으로 5세 연령 그룹 및 성별에 따라 기대빈도를 계산하였다.
▢ 연구결과 국립암센터 암등록자료를 통해 확인된 2009년부터 2013년까지 본 연구에서 정의된 선행암 경구암, 식도암, 위암, 결장암, 직장암, 항문암, 간암, 췌장암, 담도암, 후두암, 폐암, 골암, 연조직암, 유방암, 자궁경부암, 자궁암(Corpus), 난소암, 상피암, 생식세포암, 고환암, 방광암, 신경암(Nerves), 신장암, 전립샘암, 악성림프종, 형질세포종 이 발생한 성인 환자는 총 755,965명이었다. 이중 국립암센터 암등록자료를 통해 2개 이상 선행암을 가지고 있는 환자(n=30,554)가 제외되어 건강보험 청구자료와 연계한 대상자는 총 725,441명이었다. 건강보험 청구자료 연계 후 선행암 진단 1년 이내 사망한 환자(n=1,116), 선행암 진단 이전 암 진단 또는 관련 치료 청구력이 있는 환자(n=31,707), 항암화학요법 또는 방사선치료를 받지 않은 환자(n=330,682)가 제외되었으며, 최종 분석대상자는 총 342,875명이었다. 선행암 코호트 대상자 342,875명(갑상샘암 포함) 중 추적관찰 기간 동안 치료연관 골수계 종양이 확인된 환자는 629명으로 단순 비율 계산 시 0.18%에 해당하였다. 갑상샘암을 제외할 경우에는 총 250,155명 중 555명(0.22%)에서 치료연관 골수계 종양이 발생하였다. 이를 통하여 치료연관 골수계 종양의 절대적인 발생률 자체는 낮음을 확인하였다. 선행암 코호트의 환자 중 갑상샘암을 제외한 전체 환자에서의 SIR은 3.40(95% CI 3.13-3.70) 이었다. 치료연관 골수계 종양 발생 555예 중 선행암 진단 후 1년 후부터 5년 경과시점까지 진단 받은 경우가 366명으로 다수를 차지하였고 SIR도 선행암 진단 첫 5년 이내가 더욱 높았다. 치료연관 골수계 종양 내에서 살펴보면, 여성 유방암 환자가 115명으로 전체의 20.72%로 가장 많았고, 림프종 68명, 형질세포종 44명의 순이었다. 림프종과 다발골수종을 합쳐서 ‘림프계종양’으로 묶을 경우 유방암과 발생자 수가 거의 같았다. 이와 같이 치료연관 골수계 종양 환자 내에서 유방암과 림프계종양이 가장 흔한 선행암이었는데 이는 위험에 노출된 분모 환자 수가 많은 것이 영향을 미친 것으로 판단되며 서양의 기존 소규모 관찰 연구들의 결과와 일치한다. 그 외 폐암, 난소암, 대장암, 위암의 순으로 발생자가 많았다. 개별 선행암 내에서 살펴보면, 골암에서 SIR 값이 가장 높았고, 형질세포종, 연부조직암, 림프종, 난소암이 높은 SIR 값을 보였다(표 14). 경구암, 담도암, 유방암, 자궁경부암, 위암, 폐암, 간암, 전립샘암도 SIR 값의 95% 신뢰 구간 하한선이 1.0을 초과하여 일반 인구집단보다 유의하게 높은 발생률을 보였다. 이를 통하여 선행암 치료를 받는 모수가 적어 치료연관 골수계 종양 환자의 주된 선행암에 들지 못하지만 암종 자체의 절대적인 위험도가 상당히 높은 암종들을 확인하였다. 만 20-49세에서의 SIR은 4.30(95% CI 3.64-4.95)으로 만 50-64세 및 만 65세 이상에서의 SIR보다 높았다(표 20). 급성 골수성 백혈병과 골수형성이상증후군의 경우 일반 인구집단에서 고령일수록 발생률이 급격히 증가하는 질환임을 감안할 때, 치료연관 골수계 종양은 선행암에 대한 세포독성치료라는 특별한 병인으로 인하여 고령이 발생의 주요한 위험요인이 되지 않는다는 점을 확인하였다. 유방암과 난소암의 경우에도 폐경 전 연령대인 만 20-49세에서 SIR이 높았다. 남성과 여성 모두에서 치료연관 골수계 종양의 SIR은 증가하였다. 여성에서는 유방암과 난소암이 치료연관 골수계 종양 환자의 다수를 이루어 여성암의 치료에서 치료연관 골수계 종양의 위험이 높음을 확인하였다. 남성에서는 간암과 폐암을 선행암으로 둔 치료연관 골수계 종양 환자들이 많았다. 림프종과 형질세포종은 남녀 모두에서 영향을 미쳤다. 분석 대상 선행암 코호트 환자군 중 항암화학요법과 방사선치료를 동시에 받은 경우의 SIR은 항암화학요법만 단독으로 받은 경우보다 유의하게 높았다. 반면, 방사선치료 단독으로 치료받은 경우는 SIR 1.16(95% CI 0.76-1.56)으로 일반 인구에서의 골수계 종양 발생과 비교할 때 통계적으로 유의한 SIR의 증가를 보이지 않았다. 기존의 연구 결과들을 참고하여 알킬화제, 제2형 토모이소머라제 억제제, 그리고 백금계 화합물 3가지를 백혈병 호발제제(leukemogenic agent)로 분류하고 이들의 사용 여부에 따른 치료연관 골수계 종양 발생의 위험을 분석한 결과 백혈병 호발제제를 사용하지 않은 경우에 비하여 사용한 경우에서 더 높은 SIR 값을 보였다. 백혈병 호발제제 중 한 가지를 단독으로 사용한 경우에서의 SIR을 확인한 결과 알킬화제를 단독으로 사용한 경우 SIR이 4.75(95% CI 3.46-6.04), 토포이소머라제2 억제제를 단독으로 사용한 경우는 6.57(95% CI 5.1-8.5)으로 높았다(표 18). 특히 2개 이상의 백혈병 호발제제를 사용한 경우에는 SIR이 8.94(95% CI 7.95-9.94)로 더욱 상승하여 약제의 중복 사용이 위험 요인임을 확인하였다. 반면, 비세포독성항암제인 단클론항체제제 혹은 티로신키나제 억제제만을 사용한 환자에서는 치료연관 골수계 종양의 위험이 일반인구집단 대비 높지 않았다. 치료연관 골수계 종양 환자에서 진단 후 1년 이내와 3년 이내의 의료비용을 분석한 결과 진단 후 첫 1년 간의 1인당 의료비용은 50,412,061원, 3년 이내의 평균 전체 비용은 62,015,873원으로 대부분의 진료비용이 첫 1년에 집중되는 것을 확인하였다. 건강보험심사평가원 암 상병 진료비 통계자료를 통하여 분석할 때 2015년 건강보험통계연보의 연령별 성별 신규 중증(암)등록환자의 진료현황에 보고된 신규 암환자(새롭게 진단 받고 1년 동안 실제로 진료를 받은 자로 정의) 실 인원 1인당 진료비 총액이 9,324,610원임을 감안하면, 치료연관 골수계 종양은 1인당 진료비가 매우 높은 고액암임을 알 수 있었다. 고강도 항암화학요법과 조혈모세포이식이 이루어지는 만 20세-64세 연령대로 한정 시 치료연관 골수계 종양 발생 후 첫 1년 간 진료비는 57,738,338원으로 더욱 증가함을 확인하였다.
▢ 결론 및 정책적 제언 본 연구에서는 2009년부터 2013년까지의 국내에서 발생한 25개 암환자에서 치료연관 골수계 종양의 발생 현황을 확인하고 SIR을 통한 위험도와 주요 특성을 확인하였다. 이를 통하여 여러 암종 및 특성에 따라 항암화학요법과 방사선치료 전 치료연관 골수계 종양과 관련된 유전적 소인에 대한 사전검사 적용과 치료 후 보다 세심한 혈액학적 추적 관찰이 필요한 환자군을 확인하였다. 앞으로 치료연관 골수계 종양의 발생 위험을 줄이기 위한 화학적 예방요법(chemoprevention), 암백신을 통한 암 예방법(cancer vaccines) 등 새로운 중재요법에 대한 임상시험 시 본 연구가 대상자의 선정 기준으로 활용될 수 있을 것으로 기대된다. 최근 치료연관 골수계 종양의 발생에 클론성 조혈증과 유전성 암 소인 증후군 등 암 진단 전 환자가 이미 보유한 기저의 유전 이상이 큰 영향을 끼치는 점이 밝혀졌으며, 미국에서는 2018년부터 혈액암의 발생 위험을 모니터하기 위한 클론성 조혈증 클리닉을 개설하였고 진단 기술과 새로운 연구결과들의 누적에 따라 그 운영이 점차 활발해지고 있다. 국내에서도 치료연관 골수계 종양 발생 위험 예측을 위한 상세 유전자 검사에 대한 관심과 수요가 항암화학요법과 방사선치료를 받는 암환자 내에서 늘어날 것으로 예상된다. 그러나 이와 같은 유전자 검사들은 현재 고가이며, 위험군 규명이 명확하게 정립되지 않아 실제 적용 시 과다한 검사의 우려가 있다. 따라서 치료연관 골수계 종양 위험 예측을 위한 의학적 목적의 클론성 조혈증과 유전암소인증후군 유전자검사를 모든 암환자에서 지나치게 무분별한 시행이 이루어지지 않도록 제한이 필요하며, 위험도가 명백히 높은 환자군에서 선별적으로 허용하여 새로운 고급 진단 기법 도입으로 인한 의료 현장의 혼란을 막고 잠재적 이익이 있을 수 있는 환자들에게는 수준 높은 의료 서비스가 조기에 제공될 수 있도록 할 수 있는 정책이 필요하다. 지금까지 암과 관련된 대부분의 임상 연구는 효과적인 치료에 집중되어 왔다. 암 예방에 대한 연구는 상대적으로 긴 시간이 필요하며 무엇보다 이후의 암 발생 여부를 예측하기 어렵기 때문에 유효성과 안전성에 대한 통계적인 증명이 어려운 한계가 있다. 그러나 아직도 대다수 선진국의 사망원인 1위를 암이 차지할 정도로 암이 정복되지 않은 상황에서 치료 외에도 예방과 조기 발견에 대한 수준 높고 장기간 추적관찰을 요하는 전향적 연구가 필요하다. 또한 암백신 등을 통한 면역학적인 방법으로 암을 예방하려는 연구가 태동하고 있는 바, 이에 가장 일차적인 연구 대상이 될 것으로 예상되는 치료연관 골수계 종양에 대한 고위험군 환자들의 전향적 코호트 구축을 장려할 필요가 있다. 따라서 명백한 고위험군에 한정하여 치료연관 골수계 종양 병인의 상세 규명과 효과적인 예방적 중재법 규명을 위한 장기적인 전향적 코호트 구축과 임상시험 지원을 제안한다.
▢ Introduction : Therapy-related myeloid neoplasms result from genetic damage caused by chemotherapy or radiotherapy for previous cancers and include myeloid dysplasia syndrome (MDS), acute myeloid leukemia (AML), and other myelogenous hematological cancers. They occur within 1 to 10 years of the original therapy, usually within 5 years. Furthermore, they have poor prognosis and cause physical, economic, and social disruption. In particular, they involve recurring, incurable hematological cancers, which are expensive to treat in patients who have already suffered considerable burden due to prior cancer treatment. Moreover, they are a scientifically important secondary cancer group because they provide a useful model for research into the mechanism of secondary cancers. Recently developed techniques for precise gene analysis have shown that the risk of therapy-related myeloid neoplasms is greater in individuals with specific gene mutations in hematopoietic cells. These mutations are known as clonal hematopoiesis of indeterminant potential (CHIP) and can be identified in a blood test performed at the time of the original cancer diagnosis. Because of CHIP mutations, therapy-related myeloid neoplasms have emerged as an important model for exploring the etiology of hematological cancer. However, CHIP mutations have a low absolute incidence rate, so it is difficult to create and maintain a high-quality, large-scale cohort to carry out long-term monitoring of patients treated for cancer. Therefore, the present study used data from cancer registration and health insurance claims to identify therapy-related myeloid neoplasms and define high-risk groups, which constitute an appropriate target for clinical research into new chemo-prevention methods, such as pre-emptive genetic testing and cancer vaccines.
▢ Methods : To identify the prevalence of therapy-related myeloid neoplasms and high-risk groups, the data on cancer registration at the National Cancer Center were analyzed in conjunction with the data on health insurance claims from the National Health Insurance Service. A prior cancer cohort was established comprising all adults who had been diagnosed with cancer between January 1, 2009 and December 31, 2013. A retrospective cohort of therapy-related myeloid neoplasms was set up by linking the data on health insurance claims by these subjects between January 1, 2002 and December 31, 2019. The subjects comprised all new patients with cancer who had received chemotherapy or radiotherapy. Prior cancers were defined as primary solid cancers, lymphoid tumors, or plasma cell tumors with independent characteristics, excluding some rare cancers and myeloid neoplasms. Patients enrolled in the prior cancer cohort were followed up from July 1, 2009 to December 31, 2019 to see whether they developed therapy-related myeloid neoplasms. Follow-up was terminated upon the development of therapy-related myeloid neoplasms or when the final follow-up date (December 31, 2019) was reached. Therapy-related myeloid neoplasms comprised AML and MDS, with the following disease codes were used in accordance with the International Statistical Classification of Diseases and Related Health Problems 10th revision (ICD-10) : V193, V194. In addition, at least two of the claims were made under exceptional circumstances. In these groups, the standardized and cumulative incidence values of therapy-related myeloid neoplasms were analyzed. The risk of developing therapy-related myeloid neoplasms was estimated using standardized incidence ratio (SIR = observed/expected) with Poisson-based 95% confidence intervals (CIs). The SIR compared the incidence of myeloid neoplasms in the final cohort to that in the general population of South Korea, which constituted the control group. The expected frequency was calculated in each 5-year age and sex group using all claims mentioning myeloid neoplasms in the National Health Insurance Service.
▢ Results : The total number of adult patients with cancer between 2009 and 2013 was 755,965, according to the cancer registration data of the National Cancer Center. Among them, those with at least two prior cancer diagnoses (n = 30,554), according to the cancer registration data of the National Cancer Center, were excluded. Thus, the total number of subjects whose data were linked to National Health Insurance claims was 725,441. Next, patients who died within 1 year of the original cancer diagnosis (n = 1,116) were also excluded, as were those with claims mentioning cancer diagnosis or related treatments before the original cancer diagnosis (n = 31,707) and those who did not receive chemotherapy or radiotherapy (n = 330,682). Thus, the final analysis included 342,875 subjects. In this prior cancer cohort, which included patients with thyroid cancer, 629 patients (0.18%) had therapy-related myeloid neoplasms during the follow-up period. When thyroid cancer was excluded, 555 out of 250,155 patients (0.22%) developed therapy-related myeloid neoplasms. Clearly, the absolute incidence of therapy-related myeloid neoplasms was low. The SIR among all patients, excluding those with thyroid cancer, was 3.40 (95% CI: 3.13–3.70). Among the 555 patients with therapy-related myeloid neoplasms, 366 were diagnosed from 1 to 5 years after the original cancer diagnosis; the SIR was also higher within the first 5 years of the original cancer diagnosis. However, among patients with therapy-related myeloid neoplasm, there were 115 cases (20.72%) of breast cancer, 68 lymphomas, and 44 plasmacytomas. When lymphoma and multiple myeloma were combined and grouped as “lymphoid tumors,” the number of cases was almost equal to that of breast cancer. Thus, breast cancer and lymphoid tumors were the most common prior cancers among patients with therapy-related myeloid neoplasms, probably because a large number of the patients were exposed to risks for these cancers. This is consistent with previous small-scale observational studies in the West. In addition, lung, ovarian, colorectal, and stomach cancers were common. When the prior cancers were individually examined, bone cancers had the highest SIR value, followed by plasmacytoma, soft tissue cancer, lymphoma, and ovarian cancer. Oral, biliary tract, breast, cervical, stomach, lung, liver, and prostate cancers also showed a significantly higher incidence than in the general population, as the lower limit of the 95% CI of the SIR exceeded 1.0. In this way, we identified carcinomas that had a very high absolute risk in themselves, but which did not rank among the most common prior cancers in patients with therapy-related myeloid neoplasms because few of the patients had received. The SIR in patients aged 20–49 years was 4.30 (95% CI: 3.64–4.95), which was higher than those in patients age 50–64 years and in patients aged over 65 years (Table 20). In the case of AML and MDS, given that the incidence in the general population increases rapidly with age, the results confirmed that old age was not a major risk factor for therapy-related myeloid neoplasms because cytotoxic therapy for prior cancers constitutes a special etiology. In the case of breast and ovarian cancer, the SIR was high in the premenopausal group (women aged 20–49 years). The SIR of therapy-related myeloid neoplasms increased in both men and women. Among women, most patients with therapy-related myeloid neoplasms had breast cancer and ovarian cancer, and the results confirmed that the risk of therapy-related myeloid neoplasms was high in the treatment of female cancer. In men, many patients with therapy-related myeloid neoplasms had liver or lung cancer as their prior cancers. Lymphoma and plasmacytoma affected both men and women. In the prior cancer cohort, the SIR of patients who received chemotherapy and radiotherapy at the same time was significantly higher than in those who received chemotherapy alone. In contrast, in the case of radiotherapy alone, the SIR was 1.16 (95% CI: 0.76–1.56), which did not indicate a significant increase over the incidence of myeloid neoplasms in the general population. Based on the results of previous studies, alkylating agents, type 2 topoisomerase inhibitors, and platinum-based compounds were classified as leukemogenic agents, and the risk of developing therapy-related myeloid neoplasms associated with their use was evaluated. The results showed that the SIR was higher when leukemogenic agents were used than when they were not. When only alkylating agents were used, the SIR was 4.75 (95% CI: 3.46–6.04), while it was 6.57 (95% CI: 5.1–8.5) when topoisomerase 2 inhibitors were used alone (Table 18). When two or more leukemogenic agents were used, the SIR increased to 8.94 (95% CI: 7.95–9.94), confirming that repeated use of these drugs was a risk factor. On the other hand, patients who only received non-cytotoxic anticancer drugs, such as monoclonal antibodies or tyrosine kinase inhibitors, had a similar risk of therapy-related myeloid neoplasms as the general population. When medical expenses within 1 or 3 years of diagnosis were analyzed in patients with therapy-related myeloid neoplasms, treatment expenses were concentrated in the first year, with the average medical expenses per person totaling 50,412,061 won 1 year after diagnosis and 62,015,873 won 3 years after diagnosis. Among new cancer patients reported by age and sex in the new severe cancer registration of the 2015 Health Insurance Statistical Yearbook, which was based on statistical data of cancer medical expenses from the Health Insurance Review and Assessment Service, the total cost of treatment was 9,324,610 won per patient. It follows that therapy-related myeloid neoplasms are very expensive to treat. When the age of high-intensity chemotherapy and hematopoietic stem cell transplantation was restricted to 20 to 46 years, the treatment cost after the first year of therapy-related myeloid neoplasms was even higher, at 57,738,338 won per person. ▢ Conclusions and policy suggestions : The present study investigated the incidence of therapy-related myeloid neoplasms in patients with 25 cancers that commonly occurred in South Korea between 2009 and 2013, identifying the risk and major characteristics using SIR. The results identified which patients require preliminary examination for the genetic predisposition associated with therapy-related myeloid neoplasms before chemotherapy and radiotherapy, as well as which require more detailed hematological follow-up after treatment based on the type and the characteristics of their carcinomas. The results could be used as patient selection criteria for new interventional therapies, such as chemoprevention and cancer vaccines to reduce the risk of developing therapy-related myeloid neoplasms. Recent studies have shown that underlying genetic abnormalities possessed by patients before cancer diagnosis, such as clonal hematopoiesis and hereditary cancer predisposition syndrome, have a significant impact on the occurrence of therapy-related myeloid neoplasms. In the United States, clinics for clonal hematopoiesis have been opening since 2018 to monitor the risk of hematological cancer. These clinics are gradually becoming more active as diagnostic technology and new research results accumulate. In South Korea, the interest and demand for detailed genetic tests to predict the risk of therapy-related myeloid neoplasms are expected to increase among cancer patients receiving chemotherapy and radiotherapy. However, these genetic tests are currently expensive, and no clear risk group has been established, so excessive testing may be impractical. As such, genetic testing for clonal hematopoietic and genetic cancer biomarkers to predict the risk of therapy-related myeloid neoplasms should be restricted. It should not be performed indiscriminately in all cancer patients. Moreover, a policy is needed to prevent any confusion that may be caused by new advanced diagnostic techniques and to ensure that high-quality medical services are provided early to patients who are most likely to benefit. To date, most clinical studies on cancer have focused on effective treatment. Research into cancer prevention necessitates relatively long study durations. More importantly, it is difficult to predict the occurrence of cancer in the future, so it is difficult to statistically demonstrate efficacy and safety. Nonetheless, cancer remains the number one cause of death in most developed countries, and high-level studies with longer duration are needed for prevention, early detection, and treatment. With the emergence of cancer prevention using immunological methods such as cancer vaccines, future studies should recruit a prospective cohort of high-risk patients for therapy-related myeloid neoplasms, who are likely the primary study targets. Therefore, we propose that a long-term, prospective cohort be established and that supportive clinical trials be carried out to ascertain the detailed etiology of therapy-related myeloid neoplasms and identify effective preventive interventions in high-risk groups.
