▢ 연구배경
폐쇄성 수면무호흡증(obstructive sleep apnea, OSA)은 수면 중 반복적인 부분적 또는 완전한 기도 폐쇄가 특징적으로 나타나는 질환이다. 따라서 OSA 환자는 수면 중 저산소 혈증에 반복적으로 노출되는 데 이것은 수면의 질을 저하시킬 뿐만 아니라, 만성질환의 발병과 악화에 위험요인으로 작용하여 전신 건강에 영향을 미치므로 반드시 치료가 필요하다.
OSA의 대표적인 치료 방법으로는 양압기 또는 구강내장치를 이용하는 비수술적 방법과 수술적 방법이 있다. 이 중 구강내장치 치료는 수면 중 기도를 확보하여 OSA를 치료하는 방법으로 여러 가지 형태가 있지만 하악을 앞으로 내밀어 착용하는 하악전방위치 장치(Mandibular Advancement Device, MAD) 형태가 가장 일반적으로 사용되고 있다. 구강내장치는 다른 치료 방법과 비교시 편리하여 순응도가 높은 편이며, 적합한 환자에게 적용했을 때 양압기 치료에 준하는 치료효과를 나타낸다. 하지만 적응증에 해당하지 않는 환자에게 적용하거나 잘못된 방법으로 적용하는 경우에는 유효성 및 안전성이 감소할 수 있으며, 관련 부작용이 적절한 시기에 관리되지 않는다면 이 문제는 더욱 악화될 수 있다.
따라서 효과적이고 안전한 구강내 치료를 위해서는 구강내장치 치료 경험이 있는 환자를 대상으로 연구를 수행하여 효과적인 구강내장치 치료에 대한 근거를 확립하고, 이러한 결과를 바탕으로 의료진이 표준화된 치료를 제공할 수 있도록 임상 가이드라인 개발이 필요하다.
▢ 연구 목적
구강내장치 치료를 받은 OSA 환자를 대상으로 설문조사, 임상 검사, 방사선학적 검사 및 수면다원검사를 시행하여 구강내장치 치료에 대한 안전성과 유효성을 평가하고 OSA 환자 치료에 대한 구강내장치 치료 임상 가이드라인을 개발한다.
▢ 연구 방법
2000년 1월부터 2020년 2월까지 서울대학교치과병원, 연세대학교치과병원, 서울아산병원, 단국대학교치과병원 및 경북대학교치과병원에 수면무호흡증을 주소로 내원하여 OSA를 진단받고 구강내장치 치료를 받은 환자를 대상으로 치료효과, 치료 반응자의 특성, 치료 만족도 및 부작용을 평가하기 위하여 설문조사 및 임상검사를 수행하였다. 설문 조사를 통해 구강내장치 치료의 주관적 만족도, 순응도 및 부작용 등을 평가하였으며, 수면다원검사, 측모두부계측 방사선사진 촬영, 턱관절장애 평가 및 구강검진을 포함하는 임상검사를 통하여는 구강내장치 치료 전·후 변화를 비교 분석하였다. 이때, 임상검사는 추가 검사 시행에 동의한 대상자에 한하여 진행하였으며, 추가 내원이 어려운 환자 중 치료 전·후 임상결과 기록이 있는 경우, 후향적 의무기록조사를 통하여 임상검사 결과를 기록하고 그 데이터를 분석 시 함께 포함하여 진행하였다.
치료 반응자는 무호흡-저호흡 지수(apnea-hypopnea index, 이하 AHI)가 치료 후 10미만으로 감소하였거나 AHI가 치료전에 비하여 50% 이상 감소한 대상자로 정의 하였으며, 치료 반응자와 비반응자의 임상적인 특징, 수면다원검사 및 측모두부계측 분석 결과를 비교 평가하여 치료 반응자의 특징을 파악하였다. 통계학적 분석을 통해 유의미한 차이를 보이는 항목과 문헌 고찰을 고려하여, 구강내장치 치료에 영향을 미친다고 평가된 변수를 이용하여 Logistic regression과 Multiple linear regression 분석을 시행하였으며 이를 통하여 구강내장치 치료의 예측인자를 확인하였다.
설문조사 분석은 현재 장치 착용자와 미착용자로 나누어 비교 분석하였으며, 부작용과 관련된 항목의 분석을 통하여 구강내장치 치료의 안전성과 사후 관리에 대한 평가를 시행하였다.
이후 본 연구의 결과와 문헌 고찰 내용을 근거로 하고, 국내 보건의료시스템의 특성을 고려하여 연구 참여자와 전문가자문위원의 합의를 통하여 구강내장치 치료에 대한 임상 가이드라인을 개발하였다.
▢ 연구결과
1. 구강내장치 치료의 효과 (유효성)
가. 유효성은 AHI<10에서 42.0%, AHI 50% 감소에서 55.0%로 나타났다.
나. OSA의 중증도에 따른 유효성의 평가는 성공기준 AHI<10에서 경증 78.1%, 중등증 58.1%, 중증 17.6%였으며 성공기준 AHI 50%에서 경증 43.8%, 중등증 67.7%, 중증 54.4%로 관찰되었다.
다. OSA의 표현형(phenotype)에 따른 치료효과를 분석한 결과, positional OSA에서 non-positional OSA보다 더 높은 유효성을 보였다.
1. 구강내장치 치료의 예측인자 분석
라. 최저 산소포화도와 superior oral airway space (SOAS)가 구강내장치 치료 예측인자로 관찰되었다.
2. 구강내장치 치료의 순응도
구강내장치의 평균 착용 시간은 4.8일/주, 6.3시간/일로 관찰되었는데, 현재 착용자의 경우에는 5.5일/주, 6.6시간/일, 현재 미착용자의 경우에는 3.9일/주, 5.8시간/일 (착용 당시 기준)으로 착용자의 평균 착용 시간이 더 높음을 확인할 수 있었다.
3. 구강내장치 치료의 부작용 (안전성)
마. 설문조사에 따르면 대상자의 41.8%가 구강내장치 치료 중 1회 이상의 부작용을 경험했다고 보고하였으며, 주요 부작용으로는 턱관절 통증, 치아 통증 및 교합변화였다.
가. 치료 전·후 측모두부계측 분석에서 2년 이상의 착용자의 palatal plane to mandibular plane angle, Frankfort-mandibular plane angle (FMA), U1 to Frankfort horizontal plane (U1 to FH) 변화가 통계적으로 유의하게 관찰되었으며, 착용 기간에 상관없이 대부분의 그룹에서 수평 및 수직 피개량 감소가 관찰되었다.
▢ 결론 및 정책적 제언
구강내장치 치료를 효과적이고 안전하게 시행하기 위해서는 치료 반응이 높은 대상자를 선정하여 치료를 시행하고 부작용의 예방 및 관리를 철저히 해야 한다. 특히 적합한 대상자를 선정하고 치료효과를 평가하기 위해서는 AHI 뿐만 아니라 많은 임상 기록과 수면다원검사 결과 등 다양한 항목의 검토가 필요한데, 이러한 과정은 치과 수면의학 전문의가 아닌 일반 의료인에게는 어렵게 느껴질 수 있다. 따라서 문진부터 치료, 향후 추적관리까지의 진단 및 치료 전 과정에 대한 표준화된 임상 가이드라인을 마련하면 보다 많은 의료인이 양질의 진료를 제공할 수 있게 될 것이다.
이를 위하여 본 연구에서는 설문조사와 다양한 임상기록을 통하여 구강내장치의 유효성, 치료 반응자의 임상적 특징, 순응도 및 부작용에 관한 내용을 검토하고, 통계분석을 통해 구강내장치 치료의 예측인자를 평가하였다. 이러한 결과와 문헌 고찰 및 전문가 자문을 바탕으로 국내에서 처음으로 OSA 환자의 구강내장치 치료에 대한 임상 가이드라인을 개발하였다.
이러한 노력을 통해 단기적인 효과를 확인하기는 어려우나, 중․장기적으로 OSA 환자에 대한 표준화된 양질의 치료가 제공되어 질환의 악화로 인한 사회·경제적 손실과 잘못된 치료로 인한 보건·의료비용 감소를 기대해 볼 수 있다. 또한 현재의 AHI 결과에 따른 획일화된 치료방법의 결정과 보험적용에서 벗어나 다양한 평가요인과 치료방법에 대한 보험적용 및 이와 관련한 정책 결정에 참고가 될 수 있을 것이다. 더불어 임상 가이드라인의 의미있는 활용을 지속하기 위해서는 후속 연구 및 문헌 고찰을 통해 최신 지견 추가하고 보건의료 정책 변화를 수용할 수 있도록 임상 가이드라인을 지속적으로 업데이트 해야 할 것이다.

▢ Background
Obstructive sleep apnea is characterized by repetitive partial or complete airway obstruction during sleep. Therefore, OSA patients are repeatedly exposed to hypoxemia, which not only worsens the quality of sleep but also acts as a risk factor for the onset and exacerbation of chronic diseases. Considering this major effect on systemic health, its prompt treatment is essential. Representative treatment methods for OSA include conservative approaches using continuous positive airway pressure (CPAP) or oral appliances and invasive surgical methods.
Oral appliance treatment is a method of treating OSA by maintaining an open airway during sleep and the mandibular advancement device (MAD) is the most commonly used type. Oral appliances are convenient leading to its high compliance rate compared to other treatment methods. Also it demonstrates a therapeutic effect comparable to that of CPAP when applied to an appropriate patient. However, when applied to a patient that does not fall under the indications or applied in the wrong way the efficacy and safety may be reduced. This problem could be further aggravated if related side effects are not managed in a timely manner.
Therefore, for effective and safe oral appliance treatment research on patients who have experience in oral appliance treatment should be conducted to establish the evidence for effective oral appliance treatment. Then, based on such results clinical practice guideline development is called upon for medical personnel to provide standardized therapy.
▢ Objective
The aim of this study was to evaluate the safety and efficacy of oral appliance treatment for the treatment of OSA by questionnaire surveys, polysomnography, clinical and radiographic examination. The clinical practice guideline of oral appliance treatment was then developed based on such results.
▢ Methods
From January 2000 to February 2020, questionnaire and clinical examinations were performed on patients who visited five medical centers※ with complaints of sleep apnea, were diagnosed with OSA, and received oral device treatment. Subjective satisfaction level, compliance, and side effects of oral appliance treatment were evaluated through a questionnaire survey and comparison between before and after treatment through clinical examination results including PSG, lateral cephalometric analysis, TMD evaluation, and oral examination were done.
Treatment responders were defined as subjects whose apnea-hypopnea index (AHI) decreased to less than ten after treatment or whose AHI decreased by 50% or more compared to before treatment. The clinical features, PSG and lateral cephalometric analysis results between responder and non-responder were analyzed to confirm features of responders. Logistic regression and multiple linear regression analysis were performed to evaluate factors that may have an effect on oral appliance treatment. Factors were selected when there was a significant difference in statistical analysis and identified through literature review.
The analysis of the questionnaire was performed based on two groups, wearer and non-wearer. Safety of oral appliance and after treatment management were evaluated through the analysis of side effects.
Based on the results of this research and literature review, the clinical practice guideline for oral appliance therapy was developed through consensus of research participants and expert advisors considering unique characteristics of Korea's health care system.

▢ Results
1. Effects of oral appliance therapy (efficacy)
1.1. Efficacy was observed to be 42.0% for AHI <10, and 55.0% for AHI >50% reduction..
1.2. The evaluation of efficacy according to the severity of OSA was mild 78.1%, moderate 58.1%, and severe 17.6% based on AHI <10, and mild 43.8%, moderate 67.7%, and severe 54.4% based on AHI >50% reduction.
1.3. When analyzing treatment efficacy according to the phenotype of obstructive sleep apnea, positional OSA had higher efficacy than non-positional OSA.
2. Analysis of predictors of oral appliance treatment
2.1. The minimum oxygen saturation and superior oral airway space (SOAS) were observed as predictive factors for successful oral appliance treatment.
3. Compliance of oral appliance treatment
3.1. The total average wearing time was observed to be 4.8 days/week and 6.3 hours/day. For the current wearer, 5.5 days/week, 6.6 hours/day, and for the current non-wearer, 3.9 days/week, 5.8 hours/day (at the time of wearing). 4. Side effects of oral appliance treatment (safety)
4.1. Based on questionnaire, 41.8% of the subjects reported that they had experienced any side effects at least once during oral appliance treatment, and the main side effects were temporomandibular joint pain, toothache, and occlusal change.
4.2. In the lateral cephalometric analysis between before and after treatment, the change in palatal plane to mandibular plane angle, Frankfort-mandibular plane angle (FMA), and U1 to Frankfort horizontal plane (U1 to FH) in wearers over 2 years or more was observed be statistically significant. A decrease in overbite and overjet was also observed in all subjects.
▢ Conclusions
In order to effectively and safely implement oral appliance treatment, it is necessary to select appropriate patients and prevent and manage side effects thoroughly. In particular, in order to select suitable subjects and evaluate treatment effects, it is necessary to review various factors such as AHI in addition to many other polysomnography results and clinical records. However, this process may feel difficult for the general practitioners that is not a dental sleep medicine specialist. In this case, a standardized clinical guideline on the whole diagnosis and treatment process from interview to treatment to follow-up will help a broader range of medical personnel to provide high quality treatment.
To this end, the efficacy of oral appliance treatment, clinical features of treatment responders, compliance, and prevalence of side effects were reviewed through questionnaire surveys and various medical records, and predictors of successful oral appliance treatment were evaluated through statistical analysis. Based on such results and literature review and experts opinion, a clinical practice guideline for the treatment of oral appliance for OSA patients was developed for the first time in Korea.
Through this effort standardized, high-quality treatment will be provided for OSA patients in the long-term, which can reduce socioeconomic losses due to disease exacerbation and medical expense due to wrong treatment. And, it will also act as a reference for policy making related to decisions on standardized treatment methods and insurance application based on AHI results. In order to maintain the value of the guideline, it will be necessary to add latest knowledge through follow-up researches and literature review and to continuously update the clinical guideline to accommodate healthy-policy changes.
