▢ 연구 배경
중증 선천심장질환(critical congenital heart disease, CCHD)은 출생 초기인 신생아기부터 생명을 위협하는 심각한 증상이 나타나는 경우로 적절한 시기에 수술이나 카테터를 이용한 중재적 시술이 꼭 필요한 질환이다. 선천심장질환 중 약 25%를 차지하고 있다. 우리나라는 2016년 10월부터 산전 정밀 초음파를 급여로 인정하고 있어, 산전 선천심장질환 진단율이 높은 편이다. 산전에 진단되지 않은 신생아의 경우는 출생 후 신생아 진찰만으로는 심각한 증상이 나타나기 전에 진단하기에는 한계가 있다. 새로운 진단법인 맥박산소측정법을 이용할 경우 보다 빠르고 정확하게 진단할 수 있는 장점이 있다.
▢ 연구 목적
맥박산소측정을 이용한 신생아 중증 선천심장질환의 선별검사에 대한 국내 적용의 타당성 및 비용효과성을 분석을 통해 국내에서 출생하는 모든 신생아에게 신생아 중증 선천심장질환 선별검사를 적용하는 국가적 보건정책사업으로 수행할 근거를 마련하고자 한다.
▢ 연구 방법
1. 진단정확도
맥박산소측정의 진단 정확성과 경제성 평가 모델 구축을 위하여 신속 문헌검토를 수행하였다. 진단 정확성에 대해서는 기존 체계적문헌고찰을 검토하기로 하였고, 경제성 평가모델 구축을 위해서는 주요 국가 수준의 경제성평가문헌을 탐색하였다.
2. 건강보험청구자료 분석
(1)건강보험자료를 이용하여 CCHD 분류별의 유병률, 1년내 사망률, 1년간 의료비를 산출하였다. (2)건강보험자료를 이용하여 CCHD 중 신생아실 퇴원 전 진단(출생 3일 이내 진단) 받지 못한 경우의 규모를 CCHD 분류별로 파악하고, (3)분류별로 신생아실 퇴원 전 진단받은(즉, 출생 3일 이내 진단되었다고 가정할 수 있는) CCHD와 진단받지 못한(즉, 출생 4일 이후에 진단되었다고 가정) CCHD의 1년 내 사망위험을 비교하는 것이었다.
3. 경제성 분석
모든 신생아를 대상으로 중증 선천심장질환을 선별하기 위하여 기존 신체검사와 맥박산소측정을 함께 시행하는 것과 신체검사만을 시행하는 전략의 비용-효과성을 비교하였다. 본 연구에서는 검사방법에 따른 경제성 분석을 수행하였다.
보건의료체계관점으로 의료비용만을 고려하였으며 분석 기간은 1년으로 결정수형을 적용하였다. 본 연구에서는 진단 시점에 따른 생존율을 주요 결과 변수로 고려하였다.
▢ 연구 결과
1. 진단정확도
2019년 코크란에 발표된 맥박산소측정법의 유용성에 대한 19개의 연구(N = 457,195명)에 대한 체계적문헌고찰에서는, CCHD 진단을 위한 맥박산소측정법의 전반적인 민감도는 76.3% (95% CI 69.5 – 82.0), 특이도는 99.9% (95% CI 99.7 - 99.9)였고, 위양성률은 0.14% (95% CI 0.07 – 0.22)라고 보고 하였다. 또한 연구자들은 CCHD의 유병률은 후기 미숙아 또는 만삭아의 6/10,000명으로 보고 하면서, 맥박산소측정법에 의한 선별검사는 이들 중 5명의 CCHD를 찾아낼 수 있고 다른 한 명의 경우를 놓칠 것으로 보고하였다. 또한 맥박산소측정법으로 검진을 하면 1만명 중 14명의 영아가 CCHD가 없는데도 CCHD가 의심되는 것으로 허위 판별하게 된다고 보고하였다. 따라서 연구자들도 맥박산소측정법은 매우 낮은 위양성률로 CCHD를 검출하기 위한 매우 구체적이고 적당히 민감한 검사라고 하면서, 증상이 없는 신생아들이 병원에서 퇴원하기 전에 CCHD에 대한 정기적인 검사가 필요하다는 근거를 제시하였다.
2. 건강보험청구자료 분석
2014~2018년의 CCHD 신생아 2,334명 중 출생 입원 건이 존재하지 않는 93명을 제외한 2,241명에 대해서 현황분석을 하였다. CCHD 신생아는 23개 질환으로 분류되었다. CCHD 분류는 수술 및 시술 내역의 주상병을 기준으로 결정되었고, 수술 또는 시술 내역이 존재하지 않는 경우에는 질환분류에 해당되는 '입원' 시작일 기준으로 90일 이내에 사망한 건의 주상병을 기준으로 결정되었다. 출생 3일 이내 진단 여부 판단하기 위해 출생 이후 심장초음파 진료내역과 엄마의 태아초음파 내역을 살펴보았다. 2,241명 중 출생 후 3일 이내에 심장초음파를 받은 경우는 1,546명(69.0%)이고, 엄마내역에 태아초음파가 있는 경우는 419명(18.7%)이었다. 심장초음파를 처음 받은 시점이 출생 4일 이후거나 출생 1년 이내에 심장초음파를 받지 않은 경우이면서, 엄마의 건강보험자료를 확인할 수 없는 경우 111명에 대해서는 출생 3일 이내 진단 여부를 판단할 수 없었다. 결과적으로 출생 3일 이내 진단 여부는 2,241명 중 95.0%인 2,130명에서만 분류되었고, 출생 3일 이내 진단된 CCHD군은 1,553명, 출생 4일 이후 진단된 CCHD군은 577명이었다.
CCHD 신생아 중 엄마내역이 연결된 1,862명의 출생 전 엄마의 진료내역을 살펴본 결과, 엄마가 산전검사로 태아심장초음파를 받은 경우는 22.5%(419명)였다. 엄마의 진료내역에 일반 임산부 초음파검사가 없는 경우는 60.3%, 태아심장초음파 또는 정밀 임산부 초음파검사가 없는 경우는 65.1%로, CCHD 신생아의 절반 이상이 출생 전 시기에 임산부 초음파검사를 받지 않은 경우로 파악되었다.
3. 경제성 분석
우리나라에서 태어나는 모든 신생아에서 생후 신체 검사와 함께 맥박산소측정을 시행하게 된다면, 맥박산소측정 비용으로 1,443,720,000원/년이 필요하다. 또한 맥박산소측정의 특이도 0.9994를 적용할 경우, 163명이 위양성으로 나타날 수 있어, 이들에게 추가적으로 심장 초음파 등 입원이 이루어질 경우 30,746,582원이 소요된다.
효과와 비용 결과를 요약하면, 맥박산소측정을 추가로 시행할 경우 추가 생존연수는 2.34 LYG 차이가 있고, 비용은 1,427,356,977원 차이이다. 1 LYG 당 610,063,240원으로 나타났다.
▢ 결론
산모의 진료내역에 일반 임산부 초음파검사가 없는 경우는 60.3%, 태아심장초음파 또는 정밀 임산부 초음파검사가 없는 경우는 65.1%로, CCHD 신생아의 60% 이상이 산전 초음파 검사를 받지 않거나 산전 초음파를 받았더라도 정밀 검사를 시행하지 않는 경우로 파악된다. 상당수의 산모가 산전 초음파를 시행하지 않고 있으며 산전초음파 이후에 정밀 초음파를 하지 않는 경우의 수는 앞으로 임신중절이 자율적 의사에 따라 결정됨에 따라 그 빈도가 증가할 것으로 판단되며 중요한 정책 목표로 삼을 필요가 있다.
국내 CCHD의 유병률은 출생아의 0.144%로 CCHD 환아의 27.1%가 생후 4일 이후에 늦게 진단되는 것으로 분석되었다. CCHD의 질환별로 조기 진단율과 이에 따른 의료비와 사망률이 상이하였으며, 이는 질환의 특성과 중증도에 따라 다를 것으로 판단되었다. 하지만, 출생 전 혹은 출생 후 가능한 빨리 CCHD를 진단하여 이에 대한 조치와 치료를 시작한다면, 그 질환으로 인한 합병증과 후유증 그리고 사망률을 줄일 수 있고 삶의 질을 향상시킬 수 있다.
미국의 CCHD 신생아의 연간 유병률은 출생아의 0.227%을 보이고 있으나 우리나라의 CCHD의 유병률은 0.144%로 미국에 비해 약 1/2의 수준을 보이고있다. 이러한 유병률의 차이가 인종적인 차이에 의해 나타나는 것인지 사회적인 보장제도의 미비로 인해 임신중절 등을 결정하는데 영향을 준 것인지 불분명하며 이에 대한 조사가 필요하다.
CCHD의 3일 이내의 조기 진단된 군과 3일 이후의 조기진단 되지 않은 군 간의 일반적인 특성을 비교해 볼 때, 미숙아일수록, 몸무게가 작을수록 조기 진단될 확률이 높아지는데, 이는 미숙아나 저출생 체중아일수록 심장 이외에 다른 여러 기관의 문제를 동반하는 경우가 많아, 심장에 대한 평가가 조기에 이뤄져 조기 진단될 확률이 높아진다고 할 수 있다. 각 질환별 CCHD의 특성을 비교해 보면, 대부분의 CCHD 질환들에서 조기 진단된 경우가 많았으며, 조기 진단과 조기 진단 되지 않은 군의 빈도가 같은 질환(50:50%)들로는 대동맥폐동맥중격결손, 대동맥판의 선천협착이 있었으며 조기 진단 안 된 질환들로는 선천성 폐동맥판협착(38:62%), 대동맥의 폐쇄(33:67%) 등이 있었다. 생후 3일 이내에 조기 진단된 질환들은 초기부터 심한 호흡곤란이나 청색증을 보인 질환들로 증상과 징후로 쉽게 진단이 되어 조기에 발견된 경우라 볼 수 있다(대동맥의 폐쇄 환자수는 3명으로 너무 적어서 통계학적 의미는 없다).
국내에서 CCHD 환아를 조기에 선별하기 위하여, 모든 신생아에게 맥박산소측정을 시행한다는 가정으로 분석한 경제성 평가에서, 매년 3명의 신생아를 살릴 수 있고, 이에 추가생존연수는 2.34 LYG로, 비용은 약 14억/년이 필요하다고 추계되었다. 하지만 이는 추가로 생존한 3명에 대한 비용으로, 조기 진단으로 생존한 신생아들의 합병증과 후유증을 줄이는 비용과 잠재적 건강 편익에 대한 비용을 감안한다면 우리나라의 현재 경제 수준으로 보았을 때 충분한 가치가 있다고 판단된다.
또한, 맥박산소측정 검사에서 위 양성 검사 결과가 나온 신생아는 심장 초음파 등 불필요한 추가 검사를 받을 수 있다. 하지만, 이러한 저산소혈증의 원인에 대한 규명이 필요하며, 이에 대한 조기 진단 및 치료를 함으로서 이로 인한 심각한 후유증 및 사망률을 추가로 감소시킬 수 있어 그 유용성은 더욱 증가하겠다.
따라서, 본 연구 결과로 국내에서 출생하는 모든 신생아에게 맥박산소측정을 이용한 신생아 중증 선천심장질환 선별검사를 적용하는 국가적 보건정책 사업으로 수행할 근거가 마련되었다고 판단된다.

▢ Background
Critical congenital heart disease (CCHD) refers to a group of heart defects that cause serious, life-threatening symptoms in the neonatal period and requires timely surgical or catheter interventions, and CCHD accounts for approximately 25% of congenital heart disease (CHD). In South Korea, prenatal targeted ultrasound has been covered by health insurance since October 2016, and consequently, the country features a high rate of prenatal CHD diagnosis. It is challenging to make the diagnosis postnatally solely by examination prior to the onset of serious symptoms in neonates who had not been prenatally diagnosed.
In 2002, a pulse oximetry approach was first introduced as a screening test for CCHD, and CCHD screening tests are performed in most advanced countries since. All 50 states of the United States administer CCHD screening tests on all neonates, and the number of hospitals administering CCHD screening using pulse oximetry is growing in Europe.
Based on existing hospital data, the morbidity rate of CCHD in Korea is 1.1 per 1,000 live births, and assuming that approximately 20% are born without a prenatal diagnosis of CHD, the screening test is anticipated to help additionally diagnose about 600,000 newborns with CCHD annually.
Initiating appropriate treatment based on such screening test before reaching an irreversible state would improve the neonates’ survival rate and prognosis.
▢ Objectives
To collate evidence for incorporating mandatory neonatal CCHD screening test as a national public health project for all neonates born in Korea by analyzing the validity and cost-effectiveness of neonatal CCHD screening using pulse oximetry in Korea.
▢ Methods
1. Diagnostic accuracy
We performed a rapid literature review to establish models for the diagnostic accuracy and economic evaluation of pulse oximetry. We decided to review existing systematic reviews to examine the diagnostic accuracy and to review key national-level studies on economic evaluation to establish an economic evaluation model.
The literature search was conducted on OVID-MEDLINE using the following key words: pulse oximetry, neonate, newborn, and congenital heart disease. The search filters presented in SIGN were used to search for systematic reviews and economic evaluation studies. We also performed manual searches to supplement the search results.
Instead of a quality appraisal, we comparatively analyzed all studies that qualified per the key words (pulse oximetry, neonate, newborn, and congenital heart disease).
2. Analysis of health insurance claims data
(1) We will compute the prevalence, one-year mortality, and one-year medical expenditure for different types of CCHD using the national health insurance (NHI) data. (2) The number of neonates with CCHD who had not been diagnosed prior to discharge from the hospital (diagnosis made within three days of birth) is calculated by type of CCHD using NHI data, and (3) the one-year mortality risk is compared between neonates diagnosed with CCHD prior to discharge from hospital (that is, diagnosis is assumed to have been made within three days of birth) and neonates not diagnosed prior to discharge (that is, diagnosis is assumed to have been made after four days of birth) by type of CCHD.
“Diagnosis made within three days of birth” is defined as “cardiac ultrasound (Table 5)” specified in the medical records during hospital stay following birth or “prenatal diagnosis” present in the neonate’s mother’s medical records. “Prenatal diagnosis” is defined as undergoing prenatal fetal cardiac ultrasound (medical fee code E9438, EB436) (Table 6) during 240 days (8 months) prior to the date of delivery.
3. Economic evaluation
We analyzed the cost-effectiveness of pulse oximetry by comparing the group of neonates who received a combination of a physical examination and pulse oximetry and group of neonates who only received a physical examination.
We examined the benefits of early diagnosis by adding pulse oximetry. The analyses were structured to shed light on the effects of early diagnosis of CCHD on mortality rate.
The data obtained from the health insurance claims data show the results of performing physical examination alone, and based on this data, we compared the cost of pulse oximetry by examining the effects of adding pulse oximetry on the number of cases of early diagnosis and consequent reduction in the mortality rate.
▢ Results
1. Diagnostic accuracy
Four existing systematic reviews were selected to analyze the diagnostic accuracy of pulse oximetry for CCHD in neonates. By country of publication, three studies were published in the United Kingdom, and one was published in Columbia.
In conclusion, the 2018 Cochrane review presents the highest quality and most comprehensive results pertaining to the diagnostic accuracy. The diagnostic accuracy of pulse oximetry for CCHD screening features a high specificity (99.9%) and low false positive rate (0.14). Further, combining pulse oximetry with the routine physical examination is the standard in clinical practice, so based on the 2019 systematic review published in Columbia, the pooled sensitivity can be enhanced from 76.5% (pulse oximetry alone) to 92% (combined with physical examination).
2. Analysis of health insurance claims data
Of 2,334 neonates with CCHD between 2014–2018, 93 without a record of hospitalization after birth were excluded, and the remaining 2,241 neonates were analyzed. Per the classification criteria, CCHD was classified into 23 types (There were no cases of #9 AVSD PS). CCHD classification was based on the primary diagnosis shown in the surgery and procedure details, and in cases in which surgery or procedure details are lacking, it was determined based on the primary diagnosis of the case of death within 90 days from “hospital day (HD) 1.” To determine whether the diagnosis was made within three days of birth, records of postnatal neonatal cardiac ultrasound and prenatal fetal ultrasound were reviewed. Of 2,241 neonates, 1,546 (69.0%) underwent cardiac ultrasound within three days after birth, and mothers of 419 neonates had a record of prenatal fetal ultrasound (18.7%). Whether the diagnosis was made within three days of birth could not be determined for 111 neonates, as they either had their first cardiac ultrasound on or after day 4 of birth or did not undergo cardiac ultrasound in the first year of life and their mothers’ health insurance data could not be reviewed. Ultimately, diagnosis within three days of birth was reviewed in 2,130 out of 2,241 (95.0%) neonates. Of them, 1,553 were diagnosed with CCHD within three days, while 577 were diagnosed on or after day 4 of birth.
A total of 1,862 neonates with CCHD whose mother’s medical records were available were reviewed, and mothers of 22.5% (n=415) of the neonates had prenatal fetal cardiac ultrasound. On the other hand, 60.3% had no records of routine prenatal ultrasound, while 65.1% had no records of fetal cardiac ultrasound or targeted ultrasound, showing that more than half of the neonates with CCHD were not examined through prenatal ultrasound prior to birth.
3. Economic evaluation
For the cost-effectiveness analysis for CCHD screening test in this study, we used a duration of one year, diagnostic accuracy as the clinical endpoint, and LYG as the effectiveness indicator, and we were only able to analyze medical cost incurred at hospitals, such as testing and treatment costs.
Our analysis revealed that adding pulse oximetry to the routine neonatal physical examination leads to 2.34 of LYG and a cost difference of 1,427,356,977 KRW, showing a ICER 610,063,240 KRW/LYG.
▢ Conclusions
Our data showed that more than 60% of neonates with CCHD did not have a prenatal ultrasound or prenatal targeted ultrasound (60.3% lacked record of routine prenatal ultrasound, 65.1% lacked record of fetal cardiac ultrasound or prenatal targeted ultrasound). A considerable number of pregnant women opt out from prenatal ultrasound, and the odds that pregnant women would not undergo targeted ultrasound following routine prenatal ultrasound are anticipated to increase as abortion becomes an individual choice.
The annual prevalence of neonatal CCHD in Korea (0.144%) is about half of that in the United States (0.227%). It is unclear whether this gap in prevalence is attributable to racial differences or the impact of a lack of social insurance system affecting individuals’ choice of abortion, calling for relevant investigations.
In our comparison of neonates diagnosed with CCHD within three days of birth with those diagnosed with CCHD on or after day 4 of birth, the probability of early diagnosis increased for preterm infants and infants with low birth rate, which could be attributable to the fact that preterm infants or low birth weight infants often have other organ problems as well that lead to an early heart evaluation. By type of CCHD, most types of CCHD were diagnosed early. Disease featuring an equal ratio of early and non-early diagnosis (50:50%) included PA window and aortic stenosis, and diseases that were not diagnosed early included PV stenosis (38:62%) and aortic atresia (33:67%). Diseases that were diagnosed early (≤ 3 days of birth) were characterized by severe dyspnea or cyanosis, and these diseases were probably detected early owing to their apparent signs and symptoms (results for aortic atresia lack statistical significance due to the small number of cases (3 patients)).
The mean medical cost was higher in the early diagnosis group, presumably because the more severe subtypes of each disease were mostly included in the early diagnosis group.
Regarding mortality rate, most types of CCHD showed a significantly higher mortality rate in the early diagnosis group (≤ 3 days of birth), and as mentioned above, this is associated with the average medical cost. The reason for the high mortality rate despite a high early diagnosis rate pertains to the high percentage of patients with severe conditions that induce serious heart rate within three days of birth, as increased severity translates to increased odds of early diagnosis as well as mortality. In this sense, it is not an exaggeration to say that early diagnosis, reduced average medical cost, and reduced premature mortality rate are not at all associated. Thus, research on CCHD should focus on the early diagnosis rate as opposed to the premature mortality rate in the early diagnosis group. The mean rate of early diagnosis is approximately 73%, and such research would be valuable in terms of increasing the early diagnosis rate for undetected CCHD cases.
More than half of the neonates with CCHD were found to have not undergone a prenatal fetal ultrasound, rendering this an important policy target.
