인구 노령화에 따라 골감소증 및 골다공증의 유병률이 급격하게 증가하고 있다. 골감소증 환자는 골다공증 환자의 약 1.5~6배로 많고 현재 노인 인구의 절반 가량이 골감소증으로 추정된다. 골감소증은 뼈의 밀도 및 강도가 약해져서 부러지기 쉬운 상태를 말하는데, 이는 골 감소와 골절 증가로 이어지므로 향후에 발생할 골절을 예방하기 위해 골감소증 및 골다공증을 치료하는 것은 중요하다.
골감소증은 골의 감소와 골절 증가로 이어지므로 향후에 발생할 골절을 예방하기 위해 골감소증 및 골다공증을 치료하는 것은 중요하며, 특히 골감소증 환자에서 어떤 환자를 치료할 것인지는 중요한 과제이다. 미국과 유럽 등 대부분의 나라에서는 FRAX를 이용하고 있으며, 골밀도가 낮더라도 골절 고위험군인 환자들을 치료하고 있다. 그에 비해 국내 골다공증 약제의 급여 기준은 골밀도 점수에만 의존하고 있어 한계점이 있으며, 우리나라 골감소증 환자에서 골절 고위험군을 찾는 노력이 필요하다.
실제 노인환자에서 골절이 미치는 사회/경제적인 효과를 고려해 볼 때, 골다공증뿐만 아니라 골감소증에서 골절이 실제로 얼마나 발생하는지, 골절 발생위험 요인이 무엇인지에 대한 정확한 평가가 필요하다. 또한 골감소증 환자에서 어떤 약제로 치료하여야 하는지에 대한 근거도 아직 부족하다. 최초의 골절이 발생하기 전에 골다공증을 예방하고, 조기에 진단하고 치료하는 것이 무엇보다 중요하며, 골절 고위험군을 선별하여 선제적인 치료를 하는 것은 골절을 예방함으로써 국민 건강을 증진하고 사회경제적인 비용을 절감하는데 도움을 줄 수 있다. 먼저 국내의 골감소증 환자에서 골절이 얼마나 많이 발생하는지를 파악하고 치료 필요성을 확인하는 것이 우선되어야 하며, 치료가 필요한 고위험군을 분류하여 효과적인 치료 대상군을 정의하는 것이 필요하다. 따라서, 본 연구에서는 국내 골밀도 수준 및 골절 현황을 파악하고, 국내 골감소증 환자 중 치료가 필요한 골절 고위험군을 구분하고자 한다.
▢ 연구 목적
본 연구는 국내 골밀도 수준 현황과 골밀도 수준에 따른 골절 발생을 파악하고, 골감소증 환자에서 골절 위험요인 분석을 통해 골절예방치료의 필요성을 평가하고자 한다. 또한, 골절 고위험군 분석 및 골절예측모형 개발을 통해 골절 고위험군을 선별 및 관리하기 위한 골절예방 치료의 과학적 근거를 제시하고자 한다.
▢ 연구 방법
골감소증 환자에서 골다공증 약제치료의 효과를 분석하고자 체계적 문헌고찰 및 국내 환자자료 분석을 실시하였으며, 국내 골밀도 수준 및 골절 현황을 파악하고 골절 고위험군을 선별하기 위해 국민건강보험공단의 건강보험청구자료, 국가건강검진자료를 이용하여 후향적 코호트 연구를 실시하였다.
첫째, 골감소증 환자에서 골다공증 약제치료의 효과를 연구하기 위해 체계적 문헌고찰을 수행하였으며, 2007년 이후 서울대학교병원을 내원하여 골밀도 검사를 받은 50세 이상 환자자료와 국민건강보험공단 자료를 연계하여 분석하였다.
둘째, 국내 골밀도수준 현황을 파악하고, 골절 빈도 및 골절의 위험인자를 분석하기 위해 2007년부터 2019년까지 건강보험공단 청구자료. 자격자료 및 생애전환기 국가건강검진 자료를 이용하여 분석하였다.
셋째, 골감소증 환자에서 골다공증 치료제 사용이 임상적으로 유용할지에 대해 대한골대사학회 및 대한내분비학회 임원진을 대상으로 전문가 의견 조사를 실시하였다.
▢ 연구 결과
Ⅰ. 골감소증 환자에서 약제 효과 분석
1. 체계적 문헌고찰
국내외 5개 데이터베이스에서 검색된 문헌 3,194편 중 문헌선정제외 기준에 따라 최종 19편이 포함되었다. 전체 19편의 문헌 중 안전성에 대해 보고된 문헌은 총 15편이었으며 대부분의 문헌에서 경미한 부작용이 보고되었고, 골다공증 약제치료에 따른 유의한 부작용은 확인되지 않았다. 골다공증성 골절 발생에 대한 메타분석 결과, 골다공증 약제 치료군의 골절 발생 위험이 대조군에 비해 통계적으로 유의하게 낮았다(RR 0.64, 95% CI 0.52 – 0.79). 골밀도의 경우, 골다공증 약제치료군에서 12개월 시점 Lumbar spine, femonral neck, total hip 골밀도 변화량이 통계적으로 유의하게 더 높았다.
2. 국내병원 환자자료 분석
2008~2011년 골밀도 검사를 받은 50세 이상 서울대학교병원 골감소증 환자 9,331 명을 대상으로 골다공증약제 사용 여부에 따라 골절 위험도를 분석하였다.
골다공증약제 약제 사용여부에 따라 골절 위험을 분석한 결과, 약제 사용군이 미사용군에 비해 골절 위험도가 높았다. 과거 골절력으로 층화 분석을 시행한 경우 골절력이 있는 경우에는 약제 사용에 따른 골절 위험도의 차이가 없었으나, 골절력이 없는 경우 약제 사용한 경우에 골절 위험도가 더 높았다.
II. 국내 골밀도 수준 및 골절 현황 분석
생애전환기 건강검진을 받은 66세 여성의 골밀도 수준 및 골밀도 수준에 따른 골절 발생 현황을 파악하기 위해 2007년 1월 1일부터 2019년 12월 31일 생애전환기 건강검진 수검 여성의 자료 전수를 분석하였다. 2007년 이후 연도별로 골감소증 환자는 계속 증가하는 추세를 보이며 2019년 골감소증 환자는 66세 여성의 절반을 차지하였다. 골밀도 수준에 따라 골절 발생률을 분석한 결과, 골밀도 수준이 정상인 군에 비해 골감소증, 골다공증 환자군의 골절 발생률이 높았다.
III. 골절 위험요인/고위험군 분석 및 골절 예측모형 개발
골절예측모형 개발을 위해 2008년-2009년 생애전환기 건강검진 수검자 중 골밀도 검사를 시행한 만 66세 여성을 대상으로 2019년까지 골절 발생 추적하였다. 골다공증성 골절, 고관절 골절 발생에 영향을 주는 위험요인이 조금씩 달랐으나, 최종 개발된 골절 예측모형에는 낙상, 당뇨병, 뇌혈관질환, 천식, 과거골절력이 5가지 요인이 포함되었다. 개발된 골절예측모형은 전체 골다공증성 골절의 경우 훈련데이터에서 73.2%의 적합도, 검정데이터에서 72.6%의 예측력을 보였다. 고관절골절의 경우, 훈련데이터에서는 74.3%의 적합도, 검정데이터에서는 74.5%의 예측력을 보였다.
IV. 전문가 의견조사
대한골대사학회 및 대한내분비학회 임원진을 대상으로 구조화된 설문지를 이용하여 온라인 설문조사를 실시하였으며, 총 44명의 전문가가 설문에 응답하였다.
설문에 응답한 44명의 전문가 모두 골감소증 환자에서도 골다공증 약제치료가 선택적으로 필요하다고 응답하였다. 골감소증 환자 중 골다공증 약제의 치료가 필요하지만 치료하지 못하는 이유, 실제 치료를 하는 환자 수와 차이가 나는 이유에 대해서는 주로 해당약제가 보험급여 되지 않기 때문이라고 응답하였으며 실제 치료가 필요한 환자에게 약제를 처방하기 위해서는 보험 급여 확대가 요구된다고 응답하였다.
▢ 결론 및 정책적 제언
본 연구는 건강보험 빅데이터를 이용하여 국내 골밀도수준 및 골절 발생 현황을 파악하고 골감소증 환자에서 골절 위험요인을 분석하였다. 우리나라의 골감소증 환자는 2007년 이후 지속적으로 증가 추세이며, 2019년 기준 66세 여성의 절반이 골감소증 환자임을 확인하였다. 또한 실제로 골밀도 정상군에 비해 골감소증 환자의 골절 위험도가 더 높은 것을 확인함으로써, 정책적으로 골감소증 환자에게 적극적인 치료를 고려해할 시점이 왔음을 시사한다.
건강보험청구자료, 자격자료, 일반건강검진자료, 생애전환기 건강검진 자료를 광범위하게 이용하여 66세 골감소증 여성 환자에서 골절위험인자를 확인하고 골절 예측 모형을 구축하였다. 낙상, 흡연, 신체활동 등 사회학적인 요소 및 혈액 검사, 동반 상병 등 여러 가지 요소들 중 낙상, 당뇨병, 뇌혈관질환, 천식, 과거골절력이 유의한 골절 위험 인자로 포함되었다. 본 연구에서 개발된 골절예측모형은 기존 골절 예측 모델에서 가장 중요한 인자로 꼽히는 나이 및 성별이 고정된(66세 여성) 모델임에도 불구하고 골절을 예측하는 성능이 적합도와 예측력 모두 73% 내외를 보여 골감소증 환자에서 골절 위험을 평가하는 유용한 지표로 사용될 수 있을 것으로 기대된다. 66세 여성만을 대상으로 하였으므로 다른 연령대, 남성에서의 확대 적용에 대해서는 추가 연구가 필요하고, 외부 데이터에서의 실제 검증이 더 이루어지는 것이 필요하다. 이러한 한계에도 불구하고 이전 공단자료분석에서는 확인할 수 없었던 골밀도 자료를 기반으로 하였다는 점, 실제로는 매우 중요한 골절위험인자이나 FRAX에 포함되어 있지 않은 낙상이 골절위험모델로 실제로 포함했다는 점은 본 연구의 가장 큰 강점이다.
골감소증 환자에서 치료가 추가적인 효과가 있을 것인가에 대한 질문에 답하기 위해 체계적 문헌고찰을 수행하였다. 총 19편의 최종 선택된 논문을 바탕으로 시행한 메타 분석에서, 골감소증 환자에게 골다공증 약제로 치료한 경우 골절 발생 위험이 대조군에 비해 통계적으로 유의하게 낮음을 확인하였다. 또한 척추 및 고관절 골밀도가 유의하게 증가하였음을 확인할 수 있었다. 이를 통해 실제 골감소증 환자에서도 골다공증 약제를 사용하였을 때 추가적인 골절 감소 위험 및 골밀도 증가 효과가 있음을 확인하였다. 실제 서울대학교병원의 골감소증 환자를 대상으로 약제 사용군과 미사용군을 후향적으로 비교하였을 때에는 메타분석에서의 결과와 달리 약제 사용군에서 골절 위험도가 더 높은 것을 확인할 수 있었다. 그 원인으로는, 실제 임상에서는 골감소증인 환자 중에서 골절 위험도가 높을 것으로 의학적으로 판단되는 환자에게 약제를 쓰기 때문인 것으로 판단되는데, 실제로 약제 미사용군에 비해 약제 사용군이 유의하게 골밀도가 낮은 환자군이며, 과거골절력 빈도가 높았다. 또한 골밀도를 보정하더라도, 낙상력, 기저질환 등 보정하기 어려운 다양한 골절에 위험을 미치는 인자들에 의한 영향으로 무작위비교군 연구와는 달리 결과가 나온 것으로 판단된다. 하지만, 이러한 결과는 골감소증 환자 중에서도 골절의 위험도가 높은 환자군이 존재하며, 이들 환자에서는 약제를 사용한 상태임에도 불구하고 골절의 위험도가 높았음을 확인할 수 있었다. 골감소증 환자 중에서도 골절 고위험군은 임상현장에서 적절한 치료시점을 놓치지 않고 적극적인 치료를 고려하는 것이 필요하겠다.
대한내분비학회, 대한골대사학회를 대상으로 실시한 전문가 의견조사 결과에서도 골감소증 환자에서 적극적인 치료가 필요하나 보험 등 실제 처방하는 환자의 수는 적다고 응답한 전문가의 수가 많았다. 나아가서, 실제 처방을 더 적극적으로 하기 위해서 가장 필요한 점이 보험 급여의 확대라고 다수의 전문가들이 공감대를 형성하였다.
골감소증 환자에서 어떤 환자를 치료할 것인지는 중요한 과제로, 골밀도가 골다공증 수준으로 낮지 않더라도 골절 고위험군인 환자들을 선별하여 치료하는 것이 필요하다. 본 연구 결과를 바탕으로 향후에는 골감소증 환자 중 골절의 고위험군에 대해서는 좀 더 적극적인 치료가 가능해 질 수 있는 정책적 변화가 있기를 기대한다. 추가적으로, 골감소증 환자에서 약물치료효과에 대한 재정영향 분석 또는 비용효과분석 등 추가 연구가 필요할 것으로 사료된다.
종합적으로, 본 연구는 골절 위험의 사각지대에 있는 국내 골감소증 환자에서 높은 골절 발생률을 확인하였고 국내 골감소증 환자, 특히 골절 발생 위험이 가장 높은 고령 여성에서의 주요 골절 위험인자를 포함하는 골절 예측 모형을 개발하였다. 본 연구 결과는 향후 전문가들이 골감소증 환자에서의 골다공증약제 투약 여부를 결정하는 임상진료지침에 중요한 근거로 활용될 수 있을 것으로 기대한다.

▢ Background
The prevalence of osteopenia and osteoporosis is rising sharply, with that of osteopenia being approximately 1.5 to 6 times higher than that of osteoporosis, and with an estimated half of the current older adult population suffering from osteopenia. Osteopenia refers to a state of reduced bone mineral density (BMD), bone mass, and strength that leads to a high fracture risk. Treating osteopenia and osteoporosis is therefore vital to prevent potential fractures.
Identifying the indications for osteoporotic treatment among patients with osteopenia is crucial. Most countries, such as the United States and European countries, use the fracture risk assessment tool (FRAX) and treat patients with a high fracture risk even if they have low BMD. In contrast, Korea’s medical coverage for osteoporosis is solely dependent on BMD, regardless of clinical risk factors for fractures among patients with osteopenia.
Fractures due to osteoporosis in the elderly have a significant socioeconomic impact. In addition, considering the there is insufficient evidence as to which drugs are effective in treating people with osteopenia, accurately assessing the incidence and risk factors for fractures among patients with osteopenia and osteoporosis is essential to prepare preventative care. Preemptively treating patients with a high fracture risk could promote public health while cutting socioeconomic costs. Such preventative measures first require an understanding of the incidence of fracture in the osteopenia population in Korea and identifying their treatment needs, followed by categorizing at-risk groups to define indications for treatment. Thus, this study aimed to examine the epidemiology of BMD and fractures in Korea and to identify patients with osteopenia with a high fracture risk who require preventative treatment.
▢ Aim
The aim of this study was to examine BMD and its association with fracture incidence in Korea. Additionally, we sought to identify and analyze fracture risk factors among patients with osteopenia to develop a fracture prediction model, which could be used to inform future screening and fracture prevention practices.
▢ Methods
First, we performed a systematic review and meta-analysis to investigate the effectiveness of anti-osteoporotic medications in patients with osteopenia.
Second, we conducted a retrospective cohort study using NHIS claims data and the national health examination data at 2007–2019 to examine the epidemiology of BMD in Korea, the incidence of fracture and its association with BMD, and fracture risk factors. The association between anti-osteoporotic medication use and fractures was analyzed in 9,331 patients (age ≥ 50 years) with osteopenia who underwent bone density testing at the Seoul National University Hospital between 2008 and 2011. A cohort was constructed using data from 66-year-old women who underwent medical examinations between 2008 and 2019, and fracture incidence and fracture risk factors were analyzed. In addition, We developed a fracture prediction model using the cohort.
Third, we conducted a survey among experts, namely the management team at the Korean Society for Bone and Mineral Research and the Korean Endocrine Society about the clinical utility of anti-osteoporotic medications in patients with osteopenia.
▢ Results
Ⅰ. Effectiveness of anti-osteoporotic medications in patients with osteopenia
1. Systematic review
Of 3,194 search results in five Korean and overseas databases, a total of 19 studies were included per the inclusion and exclusion criteria. Fifteen studies reported data on safety, with most reporting mild side effects and no cases of serious adverse reactions from the use of anti-osteoporotic medications. A meta-analysis of the incidence of osteoporotic fractures revealed that the risk for fractures was statistically significantly lower in patients using anti-osteoporotic medications than in untreated patients (RR 0.64, 95% CI 0.52–0.79). Compared with the untreated group, the rate of change in BMD in the lumbar spine, femoral neck, and total hip at 12 months was statistically significantly higher in the anti-osteoporotic medication group.
2. Patient data from Korean hospitals
Overall, fracture risk was higher in patients treated with anti-osteoporotic medications than in untreated patients. In an analysis stratified by fracture history, among patients with a history of fracture, the risk of fracture was not significantly influenced by the use of medication, but among patients with no history of fracture, the fracture risk was higher in the anti-osteoporotic medication group.
II. Epidemiology of BMD and fractures in Korea
Since 2007, the prevalence of osteopenia has consistently risen year by year, reaching approximately 50% of all women aged 66 years in 2019. The incidence of fractures was higher in the osteopenia and osteoporosis group than in the normal BMD group.
III. Analysis of fracture risk factors/at-risk patient groups and development of fracture prediction model
While the risk factors for osteoporotic fracture and hip fracture differed slightly, the finalized fracture prediction model comprised five factors (falls, diabetes mellitus, cerebrovascular disease, asthma, and fracture history). The developed fracture prediction model demonstrated a 73.2% fit with the training data and 72.6% predictive power with the test data for all osteoporotic fractures. For hip fractures, the model had a 74.3% fit with training data and 74.5% predictive power with test data.
IV. Expert survey
All 44 experts who completed our survey stated that anti-osteoporotic medications are selectively indicated for patients with osteopenia. The experts pinpointed the lack of health insurance coverage as the primary barrier to patients acquiring indicated anti-osteoporotic medications and the reason for the discrepancy between those requiring and those receiving this medication, and suggested that health insurance coverage must be expanded to allow the prescription of these medications to patients in whom they are indicated.
▢ Conclusion and policy recommendations
This study examined the epidemiology of BMD and fracture rates in Korea and identified fracture risk factors in patients with osteopenia using health insurance big data. The prevalence of osteopenia in Korea has been continuously on the rise since 2007, with approximately half of all women aged 66 years being identified as having osteopenia in 2019. Furthermore, our results showed that the fracture risk was higher among patients with osteopenia than in those with normal BMD, highlighting the importance of implementing policies to promote the consideration of active treatment for patients with osteopenia.
We identified fracture risk factors in women aged 66 years with osteopenia using an extensive review of health insurance claims data, eligibility data, general health examination data, and turning point health examination data and established a fracture prediction model accordingly. After evaluating various factors such as falls, smoking, physical activity, laboratory results, and comorbidities, we included falls, diabetes mellitus, cerebrovascular disease, asthma, and fracture history as the significant risk factors for fractures. The fracture prediction model developed in this study featured a high fit and predictive power of approximately 73% despite being adjusted for age (66 years) and sex (female)—the two most critical factors in existing fracture prediction models—and thus is expected to serve as a useful tool for predicting fracture risk in patients with osteopenia. Because we only studied women aged 66 years, the applicability of the model to other age groups and male populations must be analyzed in follow-up studies, and the model should be verified using external data. Despite these limitations, the greatest strength of this study is that we used BMD data, which is not available in existing NHIS data, and that falls, a component neglected in the FRAX despite being a key fracture risk factor, was included in the fracture risk model.
Our meta-analysis of 19 studies confirmed that the risk of fracture is statistically significantly lower among patients with osteopenia who have been treated with anti-osteoporotic medications than in untreated patients. Furthermore, BMD in the spine and hips was significantly increased in patients treated with anti-osteoporotic medication. These results confirm that using anti-osteoporotic medications reduces fracture risk and increases BMD in patients with osteopenia. Our retrospective comparison of patients with osteopenia receiving and not receiving anti-osteoporotic medication at the Seoul National University Hospital showed a higher fracture risk in the anti-osteoporotic medication group, contradicting the results of our meta-analysis. This may be attributable to the fact that in clinical practice, anti-osteoporotic medications are only prescribed to patients with osteopenia who are clinically suspected to have a high fracture risk. In support of this hypothesis, the patient group receiving medication showed a significantly lower BMD and a higher rate of past fractures. In addition, the discrepancy may also be due to various factors that affect fracture risk but are difficult to adjust for, such as falls history and pre-existing conditions. However, these results suggest that a subset of patients show a higher risk of fracture even among the osteopenia population, and even with the use of anti-osteoporotic medications. Ensuring timely preventative treatment of these patients is crucial in a clinical setting.
A questionnaire survey of experts of the Korean Society for Bone and Mineral Research and the Korean Endocrine Society showed that many experts recognize the need for aggressive treatment in patients with osteopenia, but that the number of anti-osteoporotic medication prescriptions is low, largely due to inadequate insurance coverage. Moreover, many experts agreed on the pressing need to expand insurance coverage to facilitate access to these essential medications.
Identifying the specific indications for treatment among patients with osteopenia is an essential task, and it is important to identify and treat patients at a high fracture risk even if their BMD is not as low as that in osteoporosis. Based on our findings, we hope that policies that enable more aggressive treatment for patients with osteopenia and high fracture risk are implemented in the future. Additionally, subsequent studies should also analyze the economic impact and cost-effectiveness of pharmacological treatment for patients with osteopenia.
In summary, this study found a high incidence of fracture among patients with osteopenia in Korea and developed a fracture prediction model comprising the key risk factors for fractures in patients with osteopenia, particularly older women, who have the highest risk of fracture. The results of this study will be useful in developing clinical practice guidelines by offering evidence for the indications for anti-osteoporotic medications in patients with osteopenia.
