▢ 연구 배경
당뇨의 합병증인 당뇨망막증 (diabetic retinopathy, DR)으로 인한 실명을 막기 위한 주요 치료로 범망막광응고술 (panretinal photocoagulation, PRP)과 항혈관내피성장인자 유리체 주입술(anti-vascular endothelial growth factor intravitreal injection, anti-VEGF 주입술)이 주로 사용되고 있다. 최근 당뇨망막증에서 anti-VEGF 주입술의 사용빈도가 현저하게 증가하고 있는데, 이 두 치료법은 극명하게 대비되는 장단점을 가지고 있어 의료서비스 의사결정에 어려움이 있다.
PRP는 건강보험에서 급여적용을 받는 치료로서 비용이 상대적으로 저렴하고, 보다 영구적인 효과를 가진 반면, 치료 시에 필연적으로 동반되는 망막위축으로 치료 후 시야가 좁아진다. 또한 당뇨황반부종 등의 합병증이 발생하여 치료 후 시력을 오히려 떨어뜨리는 위험성이 있다. Anti-VEGF 주입술은 비급여 치료법으로서 시술 후 시야손상이 없고, 황반부종이 발생하지 않는 등 안구의 조직손상이 거의 없는 장점이 있다. 그러나 의료비용으로 인한 경제적 부담이 높으며, 치료효과가 비교적 단기적이어서 치료를 반복해야 하는 단점이 있어 결과적으로 반복된 치료로 비용 부담이 더욱 높아지게 된다.
두 치료의 특징 및 장단점이 극명하게 나뉘고 있어서 실제 임상에서 환자의 선호도 또는 치료자의 선택에 따라 시술이 이루어지고 있는 실정이다. 그러므로 PRP와 anti-VEGF 주입술의 경제성 평가를 통해 의료서비스 의사결정 근거자료를 제공하는 것이 필요하다.
▢ 연구 목적
당뇨망막증의 주요한 치료방법인 PRP와 anti-VEGF 주입술 간 경제성 평가를 시행하여, 의료서비스 의사결정 시 비용-효과성에 대한 근거를 제공하기 위함이다.
▢ 연구 방법
당뇨망막증의 역학적 특징, PRP와 anti-VEGF 주입술의 치료 효과 및 부작용, 당뇨망막증 환자의 건강관련 삶의 질 및 선행 경제성 평가 연구에 대한 문헌고찰을 수행하였다. 모형에 투입할 파리미터로서 치료의 효과 및 부작용과 관련된 확률 산출을 위해 병원 익명화 데이터를 분석하였고 이와 관련된 선행연구를 검토하였다. 비용 산출을 위해 공단 청구자료를 분석하고 이를 바탕으로 비용 추정을 위해 의료패널, 통계청, 고용노동부 자료 등을 사용하였다. 마콥상태의 질가중치 산출을 위해 당뇨망막증 환자 대상 설문조사를 실시하였다. 경제성 평가를 위해 마콥모형을 구축하여 코호트 시뮬레이션을 통해 비용-효용분석 및 비용-효과분석을 실시하였다. 비교대안으로는 현재 급여로 인정되고 있는 PRP만 사용하는 전략(PRP only), anti-VEGF만 사용하는 전략(Anti-VEGF only), PRP와 anti-VEGF를 함께 사용하는 전략(PRP first, Anti-VEGF first)으로 4개의 대안을 설정하였다. 결과지표는 비용-효용분석의 경우 질보정수명(quality-adjusted life year, QALY) 개선을, 비용-효과분석의 경우 실명 상태에 머문 기간 감소를 사용하였다. 또한 연구 전반에 대해 임상 전문가 및 경제성 평가 전문가의 자문을 실시하였다.
▢ 연구 결과
보건의료체계 관점에서 비용-효용분석 결과 현재 급여체계에서 인정되고 있는 PRP로만 치료하는 ‘PRP only’ 전략 대비 ‘Anti-VEGF only’ 전략의 점증적 비용-효과비(incremental cost-effectiveness ratio, ICER)는 4,175만 원/QALY으로 가장 작았으나 1QALY 개선 당 지불의사금액(willingness to pay, WTP)인 3,050만 원을 기준으로 할 때 비용-효과적이지 않았다. ‘PRP only’ 전략 대비 ‘Anti-VEGF first’ 전략의 ICER는 4,296만 원/QALY로서 ‘Anti-VEGF only’ 전략과 유사한 수준으로 역시 비용-효과적이지 않았다. ‘PRP only’ 전략과 ‘Anti-VEGF only’ 전략에 대한 결정론적 민감도분석으로서 일원 민감도분석 시 PRP 대비 anti-VEGF 치료 효과(RR) 증가, PRP 대비 anti-VEGF 주입술 성공 후 증식성 DR 재발(RR) 감소, PRP 후 주변시야결손으로 인한 불효용 증가, 실명 상태 질가중치 감소, 증식성 DR 치료 시 anti-VEGF 주입술 비용 감소, 비증식성 DR 상태 질가중치 증가에 따라 ICER가 3,050만 원 이하로 감소하여 비용-효과적이었다. 이러한 일원 민감도분석 결과는 ‘PRP only’ 전략 대비 ‘Anti-VEGF first’ 전략에서도 동일하였다. 4개 전략에 대한 확률적 민감도분석 시 WTP가 약 5천만 원까지의 범위에서는 ‘PRP only’ 전략이 비용-효과적일 확률이 가장 컸고, 그 이후에는 ‘Anti-VEGF only’ 전략이 비용-효과적일 확률이 가장 큰 것을 확인하였다.
제한적 사회적 관점과 보험자관점에서는 기본분석 시 ‘PRP only’ 전략 대비 ‘Anti-VEGF only’ 전략이 비용을 절감시키고 효과를 증가시키는 우등한 전략이었으며, ‘Anti-VEGF only’ 전략은 다른 3개의 전략에 비해 비용이 가장 적고 효과가 가장 큰 전략이었다.
▢ 결론 및 정책적 제언
이 연구는 증식성 당뇨망막증 치료시 실제 임상에서 매우 빈번하게 사용하지만 비급여 치료인 anti-VEGF 주입술과 급여 적용을 받는 PRP 치료에 대해 4개의 대안을 설정하여 마콥 모델링을 통해 코호트 시뮬레이션 기법으로 경제성 평가를 실시한 연구다. 보건의료체계 관점에서 ‘PRP only’ 전략 대비 ‘Anti-VEGF only’ 전략의 ICER가 4,175만 원/QALY으로 가장 작았으나 비용-효과적이지는 않았다. 결정론적 민감도분석에서 anti-VEGF 주입술의 치료효과 및 anti-VEGF 주입술 후 재발, PRP후 주변시야감소 및 비증식성 DR과 실명 상태의 삶의 질, 증식성 DR 치료 시 anti-VEGF 비용의 값이 변함에 따라 비용-효과적인 결과를 나타냈다. ‘PRP only’ 전략 대비 ‘Anti-VEGF first’ 전략의 기본분석에서도 ICER 4,296만 원/QALY로 유사한 수준이었고 및 결정론적 민감도 분석에서도 상기한 동일한 변수들의 증감에 따라 비용-효과적인 결과를 나타냈다. 4개 전략에 대한 확률적 민감도 분석 시에는 WTP 5천만 원 이전에는 ‘PRP only’ 전략이, 그 이후에는 ‘Anti-VEGF only’ 전략이 비용-효과적일 확률이 가장 큰 것을 확인하였다.
이러한 경제성 평가 분석 결과를 통해 증식성 DR에서 ‘Anti-VEGF only’ 전략 또는 ‘Anti-VEGF first’ 전략의 비용-효과성에 대해 보건의료체계관점에서 WTP 3,050만 원 기준에서는 비용-효과적이지 않았으나 결정론적 민감도분석 시 일부 파라미터의 변동에 따라 비용-효과적이었고, ‘Anti-VEGF only’ 전략의 경우에는 확률적 민감도분석 시 WTP가 5천만 원 이후인 경우 비용-효과적일 확률이 가장 높은 것을 확인할 수 있었다.
그간 anti-VEGF 주입술은 망막 조직에 인위적으로 손상을 주어 합병증을 일으키는 PRP 치료에 비해 부작용이 없어 효과적인 대안으로 여겨져 임상에서 빈번하게 사용됨에도 불구하고 높은 비용으로 인해 환자의 경제적 부담 등의 이유로 사용 시 여러 제한점이 있었다. 본 연구에서 anti-VEGF 주입술의 비용-효과성에 대한 다양한 분석결과를 확인하였으므로, 증식성 DR에서 anti-VEGF 주입술을 단독 또는 PRP 치료와 병합의 형태의 급여권에 도입에 대한 의사결정 시 중요 근거자료로 사용할 수 있다.

▢ Background
Panretinal photocoagulation (PRP) and anti-vascular endothelial growth factor intravitreal injection (anti-VEGF injection) are mainly used to prevent blindness due to diabetic retinopathy, a complication of diabetes. Recently, the frequency of use of anti-VEGF injection in diabetic retinopathy has increased significantly. However, they have sharply contrasting advantages and disadvantages, making it difficult to make medical service decisions. While PRP is relatively inexpensive and has a more permanent effect, the visual field is narrowed after PRP due to retinal atrophy that is inevitably accompanied during treatment. In addition, complications such as diabetic macular edema occur, and there is a risk of lowering the visual acuity after treatment. Anti-VEGF injection has the advantage of almost no tissue damage to the eye, such as no visual field damage and no macular edema after the procedure. However, the economic burden due to medical costs is high, and the treatment effect is relatively short-term, so the treatment has to be repeated. As a result, the cost burden becomes higher due to repeated treatment.
Since the characteristics and advantages and disadvantages of the two treatments are sharply divided, in actual clinical practice, the procedure is performed according to the preference of the patient or the choice of the doctors. Therefore, it is necessary to provide supporting data for medical service decision making through economic evaluation of PRP and anti-VEGF injection.
▢ Objective
The purpose of this study is to provide a basis for cost-effectiveness when making medical service decisions by conducting an economic evaluation between PRP and anti-VEGF injection, which are the main treatment methods for proliferative diabetic retinopathy.
▢ Methods
Epidemiologic characteristics of diabetic retinopathy, therapeutic effects and side effects of PRP and anti-VEGF injection, health-related quality of life for diabetic retinopathy patients, and prior studies of economic evaluation were reviewed. Hospital anonymization data were analysed and previous literature were reviewed to calculate the probability related to the effectiveness and adverse events of treatment, and a survey was conducted for diabetic retinopathy patients to calculate the quality weights. We analyzed the insurance claim data for cost calculation. For economic evaluation, a Markov model was built and cost-effectiveness analysis and cost-utility analysis were performed through cohort simulation
We structured four strategies for economic evaluation. (1) PRP only: using only PRP, which is currently covered only for proliferative diabetic retinopathy, (2) Anti-VEGF only: using only anti-VEGF injection, (3) or (4) PRP first or Anti-VEGF first: using PRP and anti-VEGF together. As a outcome, quality-adjusted life year (QALY) gain was used for cost-utility analysis, and the avertion of the period of stay in blindness was used for cost-effectiveness analysis. In addition, the overall study was consulted by clinical and economic evaluation experts.
▢ Results
As a result of the cost-utility analysis from the healthcare system perspective, the incremental cost-effectiveness ratio (ICER) of the 'Anti-VEGF only' strategy compared to the 'PRP only' strategy was the smallest at 41.75 million KRW/QALY, but it was not cost-effective based on the willingness to pay (WTP) of 30.5 million KRW per 1QALY gain. Compared to the ‘PRP only’ strategy, the ICER of the ‘Anti-VEGF first’ strategy was 42.96 million KRW/QALY, which was similar to the ‘Anti-VEGF only’ strategy and was not cost-effective. The results of one-way sensitivity analysis between the 'PRP only' strategy and the 'Anti-VEGF only' strategy, were as follows. The ICERs of the 'Anti-VEGF only' strategy decreased to 30.5 million KRW or less, indicating cost-effective, when some input parameters changed: increase in anti-VEGF treatment effect compared to PRP, decrease in proliferative DR recurrence after successful anti-VEGF injection, increase in disutility of peripheral visual field defect after PRP, decrease in quality weight of blindness, decrease in anti-VEGF cost in proliferative DR treatment, and increase in value of quality weight in non-proliferative DR. The results of this one-way sensitivity analysis were the same in the 'Anti-VEGF first' strategy compared to the 'PRP only' strategy. In the probabilistic sensitivity analysis of the four alternatives, the 'PRP only' strategy was most likely to be cost-effective when the WTP was up to about KRW 50 million, and after that, the 'Anti-VEGF only' strategy was likely to be cost-effective.
From a limited social perspictive and an payer’s perspective, the 'Anti-VEGF only' strategy was a superior strategy to saver costs and increase the QALYs compared to the 'PRP only' strategy, and the 'Anti-VEGF only' strategy was the least costly and most effective strategy.
▢ Conclusions
This study established the four alternatives to anti-VEGF injection, which is very frequently used in clinical practice, and PRP treatment, which is already covered, in the treatment of proliferative diabetic retinopathy, and economical evaluation was conducted under the cohort simulation technique through Markov modeling. From the healthcare system perspective, the ICER of the 'Anti-VEGF only' strategy was the smallest at 41.75 million KRW/QALY compared to the 'PRP only' strategy, but it was not cost-effective. In a deterministic sensitivity analysis, as the therapeutic effect of anti-VEGF injection, recurrence after anti-VEGF injection, disutility of peripheral vision field defect after PRP, quality of life in non-proliferative DR and blindness, and the cost of anti-VEGF treatment in proliferative DR treatment were changed, the ICERs of 'Anti-VEGF only' strategy decreased. However, in the probabilistic sensitivity analysis, the 'PRP only' strategy before WTP 50 million KRW and the 'Anti-VEGF only' strategy after that had the greatest probability of being cost-effective.
Through the results of this economic evaluation analysis, the cost-effectiveness of the 'Anti-VEGF only' strategy or the 'Anti-VEGF first' strategy in proliferative DR was not cost-effective, but deterministic sensitivity from the healthcare system perspective at WTP 30.5 million KRW, they are were cost-effective according to the change of some parameters, and in the case of probabilistic sensitivity analysis, it was confirmed that it was cost-effective when the WTP was 50 million won KRW or more.
So far, anti-VEGF injection has been considered an effective alternative to PRP treatment, which causes complications by artificially damaging retinal tissue. Since various analysis on the cost-effectiveness of anti-VEGF injection and PRP were conducted in this study, it can be used as important evidence when decision-making for national health insurance benefit of anti-VEGF injection alone or in combination with PRP treatment in proliferative DR.
