▢ 서 론
세계 보건기구(WHO)는 2020년부터 2030년까지 10년간을 건강노화 10년으로 정하고 이와 관련한 계획을 공표하였다. WHO의 “건강 노화”는 “노년기에 웰빙을 가능하게 하는 기능적 능력을 개발하고 유지하는 과정”으로 정의한다. WHO는 건강 노화 10년을 위한 전략 중 내재적 역량 강화, 기능적 능력 증진, 노인 통합 케어(ICOPE) 패키지 시행을 제안했다. 즉, 노인의 건강관리에서 기능의 유지 및 감소 예방은 매우 중요하며, 따라서 노쇠한 노인의 관리는 건강노화에 매우 중요한 사업이다.
본 연구의 목적은 노쇠의 의료적 비용을 산출하기 위해 첫째, 일차의료에서 통합 노쇠 관리 프로그램을 적용하여 노쇠의 선별검사 및 진단을 실시하고 일차의료에서 통합 노쇠 관리를 통하여 환자의 일상생활기능, 노쇠상태, 보행능력, 사회참여활동, 급성기 진료 및 건강 습관의 평가 및 개선을 확인하고 이를 적용하여 통합관리의 비용효과성을 분석하는데 있다. 둘째, 한국노인노쇠코호트(Korean Frailty and Aging Cohort Study, KFACS 코호트 대상자의 건강보험청구자료를 이용하여 의료이용 행태 및노쇠 상태에 따른 의료 비용 산출 규모를 파악하여 을 통하여 대상자의 경제적 질병 부담을 산출하기 위함이다.
▢ 연구 방법
○ 일차의료에서 노쇠의 통합 관리
이 연구는 6개의 일차의료의원에서 진행되었다. 클리닉의 간호사와 의사는 1차 진료에 대한 한국 노쇠 지수(KFI_PC)를 적용하여 의원을 방문하는 노인 환자의 기능 저하 및 노쇠를 평가하였다. KFI_PC는 다음 10개 영역 55개 항목으로 구성된다.
(1) 섬망 또는 치매를 포함한 인지 상태; (2) 기분; (3) 시각, 청각 및 언어를 포함한 의사 소통; (4) 이동성; (5) 균형; (6) 장 기능; (7) 방광 기능; (8) 일상 생활 활동을 수행하는 능력; (9) 영양; (10) 사회적 자원
KFI_PC의 모든 항목은 모바일 안에 내장되어 표시되어 간호사와 의사가 환자를 쉽게 평가하고, 노쇠 지수 점수를 결정하며, 노쇠에 대한 위험 요인을 평가할 수 있도록 모바일 안에 자동으로 표시된다. ICOOP_Frail 연구에서 KFI_PC 환자는 노쇠한 것으로 정의되는 점수 ≥ 0.25는 시험군 또는 대조군으로 무작위 배정되었다.
시험군에서는 1차 진료의가 기능저하 또는 위험인자를 설명하고 환자에게 자동으로 표시되는 권장사항에 따라 기 제작된 노쇠 노인에 대한 영양 및 운동 교육자료를 이용하여 영양이나 운동에 대해 교육하고 약물을 조절하며 질병을 관리하였다. 1개월 이내에 건강 코치는 전화를 통해 환자에게 연락하여 환자를 모니터링하고 단백질이 풍부한 균형 잡힌 식단 또는/및 운동 연습을 권장하였다. 건강 코칭을 위한 전화 통화는 총 6개월 동안 매월 반복하였다.
대조군에서는 기 제작된 노쇠 노인에 대한 영양 및 운동 교육자료를 나누어 주며 추가적인 중재는 하지 않았다.
시험군과 대조군 모두 해당 의원에서 지속적으로 진료를 받고 있는 분들이며, 일상적인 진료는 동일하게 진행되었다.
KFI_PC는 3개월 및 6개월 후에 재평가되었고, 결과 변수는 한국 일차의료 노쇠지수(KFI_PC)와 일상생활활동, 건강습관(운동, 식이요법), 사회활동, 보행능력 등이 포함되어 있다.
○ KFACS 코호트 대상자의 의료비용 및 질병 부담 산출
KFACS 코호트대상자의 의료이용 행태와 의료비용 산출은 노쇠점수별 ( 1~5점), 동반질환별 (예, 고혈압, 당뇨, 근골격질환), 노쇠상태변화를 구분하여 의료이용 행태와 의료비용 산출 및 회귀분석 시행하였다. KFACS 코호트대상자의 경제적 질병부담(Economic burden)은 직접비용과 간접비용의 합으로 측정하였다.
▢ 연구 결과
○ 일차의료에서 노쇠의 통합 관리
KFI_PC는 대조군과 비교해 시험군에서 0.01만큼 점수가 감소하는 경향으로 나타났고, 시간의 경우 베이스라인과 비교해 3개월 후 0.01, 6개월 후 0.03만큼 점수가 감소하였다(p<0.05). 또, 시간과 그룹의 교호작용을 통해 베이스라인에서 대조군에 비해 시험군의 차이보다 3개월 후에 대조군에 비해 시험군이 0.03만큼 점수가 더 감소하였다(p<0.05).
만성질환을 제외한 KFI_PC_re는 대조군과 비교해 시험군에서 0.005만큼 점수가 감소하는 경향으로 나타났고, 시간의 경우에는 베이스라인에 비해 3개월 후 0.02, 6개월 후에는 0.05만큼 점수가 감소하였으며 통계적으로 유의하였다(p<0.05). 시간과 그룹의 교호작용을 통해 확인한 결과, 베이스라인에서 대조군에 비해 시험군의 차이보다 3개월 후에 대조군에 비해 시험군이 0.05만큼 점수가 더 감소하였고, 이는 통계적으로 유의하였다(p<0.05).
○ KFACS 코호트 대상자의 의료비용 및 질병 부담 산출
외래 의료비는 노쇠가 건강군 대비 27만원 더 많았고 입원과 외래 합한 의료비는 건강군 대비 노쇠가 145만 원 더 높은 것으로 나타났다(p<0.05). 2018년 직접비용을 1인당으로 환산한 결과, 건강군은 3.9백만 원, 전노쇠군은 5.2백만 원, 노쇠군은 6.5백만 원으로 산출되었다. 성별로는 남성이 여성보다 높았으며, 연령별로는 노쇠 남성 75-79세의 1인당 직접비용이 11.4백만 원으로 가장 높은 것으로 나타났다.

▢ Introduction
The World Health Organization (WHO) recently endorsed the proposal for a Decade of Healthy Ageing (2020–2030). The WHO defines “healthy aging” as “the process of developing and maintaining the functional ability that enables wellbeing in older age.” Among the strategies for the Decade of Healthy Ageing, the WHO has suggested enhancing intrinsic capacity, promoting functional ability, and implementing the Integrated Care for Older People (ICOPE) package. The WHO has defined steps for ICOPE evaluation and scale-up and is performing a prospective study in 2–3 countries (low and middle income, high income) to test its feasibility in 2021–2022 and a multinational randomized study to validate its clinical efficacy and effectiveness in 2022–2024. Intrinsic capacity and frailty represent two faces of the same coin, with one indicating the reserves of the individual and the other indicating the deficits that accumulate with age.
First, the purpose of this study is to improve the frailty status, gait ability, social involvement activity, acute medical visit and healthy habits of patients through comprehensive integrated frailty management in primary care by applying the screening test and diagnosis. Second, it is to calculate the economic burden of disease of the subjects of the KFACS cohort through the calculation of medical usage behavior and medical expenses.
▢ Methods
○ Comprehensive integrated frailty management in primary care
This study was conducted in six primary care clinics. Nurses and doctors at the clinics apply the Korean Frailty Index for Primary Care (KFI_PC) to evaluate the functional decline and frailty of older adult patients visiting the clinics.
The KFI_PC consists of 54 items in the following 10 domains: (1) cognitive status, including delirium or dementia; (2) mood; (3) communication, including vision, hearing, and speech; (4) mobility; (5) balance; (6) bowel function; (7) bladder function; (8)ability to perform activities of daily living; (9) nutrition; and (10) social resources. All items in the KFI_PC are already embedded in and displayed on a mobile notepad so that the nurses and doctors can easily evaluate the patient, determine the frailty index score, and assess risk factors for frailty, which are automatically displayed on the notepad. In the ICOOP_Frail study, patients with KFI_PC scores ≥ 0.25, which is defined as frail, are randomized into experimental or control groups.
In the experimental group, primary care physicians explain functional declines or risk factors and educate patients about nutrition or exercise, adjust medication, and manage diseases based on the recommendations automatically displayed for the patient. Within 1 month, a health coach contacts the patient via telephone to monitor the patient and encourage their consumption of a protein-rich balanced diet or/and practicing exercise, as well as connects the patient to community resources for social support, if needed. The phone call for health coaching is repeated monthly for a total of 6 months. The KFI_PC is re-evaluated after 3 and 6 months. The primary outcome variables were defined as frailty index and the number of acute medical visits, while activities of daily living, health habits (exercise, diet), social activities, and gait ability were defined as the secondary outcomes.
○ Calculation of medical expenses and disease burden for KFACS cohort subjects
The KFACS cohort subjects' medical use behavior and medical cost calculations were performed by classifying frailty scores (1-5 points), comorbid diseases(e.g., hypertension, diabetes, musculoskeletal disorders), and changes in frailty status to calculate medical usage behaviors and medical expenses. A regression analysis was performed. And the economic burden of disease was measured as the sum of direct and indirect costs.
▢ Results
○ Comprehensive integrated frailty management in primary care
In KFI_PC, the score decreased by 0.01 in the experimental group compared to the control group, and in the case of time, the score decreased by 0.01 after 3 months and 0.03 after 6 months compared to the baseline. In addition, through the interaction between time and groups, the score of the experimental group decreased more by 0.03 compared to the control group after 3 months than the difference between the experimental group at baseline compared to the control group (p<0.05).
The score of KFI_PC_re decreased by 0.005 in the experimental group compared to the control group, and in the case of time, the score decreased by 0.02 after 3 months and 0.05 after 6 months compared to the baseline, which was statistically significant (p<0.05). As a result of confirming through the time and group interaction of model 3, the score of the test group decreased by 0.05 more than the control group after 3 months compared to the control group at baseline, which was statistically significant (p<0.05).
○ Calculation of medical expenses and disease burden for KFACS cohort subjects
Outpatient medical expenses were 270,000 won more in the frail group than in the robust group, and the combined inpatient and outpatient medical expenses were 1.45 million won higher in the frail group than in the robust group. (p<0.05). As a result of converting direct costs per person in 2018, the robust group was 3.9 million won, the pre-frail group was 5.2 million won, and the frail group was 6.5 million won. In terms of gender, men were higher than women, and by age, frail men aged 75-79 years had the highest direct cost of KRW 11.4 million per person.
