▢ 연구 배경 2017년도 국민건강영양조사에 따르면 중등도 이상의 만성콩팥병 유병률은 연령대가 높아질수록 증가하는 추세를 보이며, 특히 70대 이상인 경우 유병률은 16%에 달한다. 2020년 대한신장학회 말기신부전환자 등록사업(KORDS)의 자료에 의하면 65세 이상 투석이 필요한 말기신부전 환자 비율은 90년에 2.1%에서 2000년 17.4%, 2010년 36%, 2020년에는 54.6%로 급격하게 증가하는 추세로 보이고 있다. 최근 10여 년간 주요 국가의 만성콩팥병 환자에 대한 임상진료지침이 개발되었으나 노인 만성콩팥병 또는 말기신부전 환자에 대한 진료지침은 거의 전무한 상태이다. 노인에 대한 권고를 포함하는 유일한 지침으로는 유럽신장학회에서 2017년에 개발한 지침으로 만성콩팥병 3b 단계 이상의 노인 만성콩팥병 환자에서 언제 투석을 시작할지, 이를 위한 신기능 평가법, 예후 예측모델 안내, 환자의 영양상태 평가와 노쇠 고령 환자에서의 투석치료 여부에 대한 지침을 제공하고 있으나 임상근거가 충분하지 못하여 이후 축적된 과학적 임상근거에 바탕을 둔 진료 권고안의 개발은 지속적으로 필요한 상황이다. 국내 노인 만성콩팥병 환자에 대한 진료 권고의 제정은 이루어지지 않은 상태로 본 연구를 통한 과학적 근거의 체계적 검토 결과가 국내 임상진료지침 개발의 근거자료로 활용되는데 이바지하고자 한다. 더불어 본 연구는 수년 이내 국내 초고령 사회로의 진입 및 증가하는 노인 만성콩팥병 환자의 건강수명을 연장하고 삶의 질을 개선하는데 기초연구로서 진료 권고안 의사결정의 근거자료로 활용되어 임상적 근거기반 의사결정을 지원하고자 한다. ▢ 연구 목적 본 연구는 노인 만성콩팥병 환자의 투석치료와 관련하여 치료효과 및 예후요인을 분석하여 실제 임상에서 진료 시 투석치료 여부를 결정하는 데 있어 합리적 의사결정에 도움이 되는 객관적인 근거를 제시하고자 한다. 이를 위한 구체적인 연구목적은 다음과 같다. 첫째, 노인 만성콩팥병 환자에서 투석치료와 보존적 관리 간의 임상효과를 비교분석한다. 둘째, 노인 만성콩팥병 환자에서 혈액투석과 복막투석 간의 임상효과를 비교분석한다. 셋째, 노인 만성콩팥병 환자에서 투석치료의 사전 계획 여부가 생존에 미치는 영향이 어떠한지와 계획되지 않은 투석치료를 받은 노인 환자에서 독립적 예후요인은 무엇인지 파악한다.
▢ 연구 방법 노인 만성콩팥병 환자에서 투석치료는 보존적 치료에 비해 효과적인지, 복막투석은 혈액투석에 비해 효과적인지, 계획되지 않은 투석치료가 생존에 미치는 영향은 어떠한지에 대해 체계적 문헌고찰을 수행하였다. 문헌검색은 국외 3개 데이터베이스(Ovid-Medline, Ovid-EMBASE, Cochrane Central Register of Controlled Trials)와 국내 3개 데이터베이스(KoreaMed, KMbase, KISS) 및 수기검색을 병행하였으며, 출판연도 및 언어에 제한을 두지 않았다. 문헌선택은 2명의 연구자가 독립적으로 검토한 뒤 의견 일치를 통하여 최종 문헌을 선택하였다. 최종 선택문헌들의 비뚤림 위험 평가는 RoBANS 2.0도구 및 QUIPS 도구를 사용하였으며 가능할 경우, 랜덤효과모형 기반의 메타분석을 수행하였고, 양적합성을 수행할 수 없는 경우 질적으로 기술하였다. ▢ 연구 결과 1. 중재 체계적 문헌고찰 결과 1) 노인 만성콩팥병 환자에서 투석치료는 보존적 치료에 비하여 임상적 안전성 및 효과성이 어떠한가? 최종 선정된 문헌은 관찰연구 21편으로 모두 국외에서 수행한 연구였으며, 노인을 대상으로 한 연령 기준이 60세 이상 2편, 65세 이상 5편, 70세 이상 5편, 75세 이상 7편, 80세 이상 2편이었다. 투석방법이 혈액투석 단독 3편, 복막투석 단독 1편, 두 가지 투석방법을 모두 대상으로 한 경우는 17편이었다. 전체 생존율을 보고한 14편에 대해서, 1, 2, 3년 시점의 생존율의 중앙값 및 IQR을 살펴보면, 투석군의 1년 시점 생존율은 85% (interquartile range (IQR) 77~91%), 2년 시점 생존율은 73%(IQR 62~79%), 3년 시점 생존율은 58%(IQR 51~71%)로 나타났다. 보존적 치료군의 1년 시점 생존율은 69%(IQR 63~73%), 2년 시점 43%(IQR 30~51%), 3년 시점 25%(IQR 20~35%)이었다. 중앙값 생존기간은 투석치료군 중앙값 38개월(IQR 36~51개월), 보존적 치료군 중앙값 20개월(IQR 14~27개월)이었다. 전체 생존율 확인 결과, 모든 시점에서 투석 치료군의 생존율이 높았고, 생존기간도 더 길었다. 사망위험에 대한 메타분석 결과, 보존적 치료대비 투석치료의 비보정 사망위험(hazard ratio, HR)은 0.43(n=11, 95% confidence interval (CI) 0.37~0.50, I2=43%), 보정된 HR의 통합추정치는 0.44(n=8, 95% CI 0.32~0.60, I2=82%)로 투석군의 사망위험이 유의하게 낮았다. 보정된 사망위험의 문헌간 이질성의 원인을 탐색하기 위하여 사전에 정한 요인에 따른 하위군 분석 결과, 이질성의 원인으로 투석종류가 확인되었고, 전반적으로 투석치료군의 사망위험이 유의하게 낮은 경향을 나타냈으며, 연령이 70대 이상인 경우, 혈액투석 혹은 복막투석이 혼재된 경우, 추적관찰 기간이 3년 이내인 경우 및 단일기관 연구인 경우 문헌간 이질성이 낮고 사망위험도 투석군에서 유의하게 낮았다. 삶의 질을 보고한 문헌은 7편으로, 문헌별로 다양한 척도를 이용하여 다양한 결과보고 양식을 사용하고 있는 것으로 나타났다. 전반적인 삶의 질을 측정한 문헌 2편과 SF 도구의 하부척도인 신체요약지표(physical component score, PCS)과 정신요약지표(mental component score, MCS)의 값을 보고한 문헌 2편을 이용하여 메타분석한 결과, SF-12 정신요약지표에서만 투석군에서 유의하게 더 높았다(SMD 0.28, 95% CI 0.09~0.47, I2=25%). 질병특이 삶의 질 측정도구인 kidney disease quality of life instrument (KDQoL)을 이용한 결과값의 통합분석결과 ‘증상 또는 문제 영역’과 ‘일상생활에 대한 신장질환의 영향’ 영역에서는 투석치료군의 삶의 질이 유의하게 좋은 것으로 나타났고, ‘신장질환의 부담’ 영역은 보존적 치료군에서 유의하게 좋은 결과를 보였다. 재원기간에 대해서 보고한 문헌은 7편으로, 문헌별로 결과변수 정의 및 결과보고 양식이 다양하였고 합성이 가능한 3편을 이용하여 메타분석한 결과, 두 군간 유의한 차이가 없었다(mean difference (MD) –0.03, 95% CI –0.95~0.89). 입원에 대해 보고한 문헌은 3편으로, 문헌별로 다양한 결과변수 정의 및 결과보고 양식에 차이가 있어 양적합성은 수행하지 않았다. 6개월 이내 1회 이상 입원 비율은 투석군에서 유의하게 높았지만, 입원횟수나 연간 1인당 입원율은 두 군간 차이가 없었다. 응급실 입원율은 1편의 문헌에서 투석군 1.63회, 보존적 치료군은 3.51회로 보존적 치료군에서 응급실 입원율이 유의하게 높게 보고되었다. 질병특이 사망률을 보고한 문헌은 3편으로 감염관련, 심혈관질환 관련, 암 관련, 치료 중단, 4개 영역 모두 두 군간 유의한 차이는 없었다. 치료 만족도를 보고한 문헌은 1편으로 renal treatment satisfaction questionnaire (RTSQ)를 이용하여 측정한 결과, 보존적 치료는 30.5, 혈액투석은 31, 복막투석은 32로서 복막투석의 치료만족도가 가장 높게 보고되었다.
2) 노인 만성콩팥병 환자에서 복막투석은 혈액투석에 비해 임상적 안전성 및 효과성이 어떠한가? 복막투석과 혈액투석이 만성콩팥병 노인 환자에게 미치는 영향을 알아보기 위해 실시한 체계적 문헌고찰에서 최종 선정된 문헌은 관찰연구 34편이었다. 노인만을 포함한 연구는 16편, 노인 대상의 결과를 보고한 혼합대상 연구는 18편이었다. 1, 3, 5년 시점의 복막투석과 혈액투석에서 생존율의 중앙값 및 IQR을 살펴보면, 1년시점 생존율 78.8%(IQR 67.6~85.9%) 및 77.2%(IQR 75.1~88.7%), 3년 시점 45.6%(IQR 36.3~60.0%) 및 48.2%(IQR 45.1~57.9%), 5년 시점 26.2%(IQR 20.7~41.9%) 및 27.5%(IQR 25.7~37.0%)였다. 중앙값 생존기간의 경우 복막투석군은 중앙값 30.7개월(IQR 23.0~33.9개월), 혈액투석군은 중앙값 32.5개월(IQR 27.3~39.3개월)이었다. 3년, 5년 시점에서 혈액투석군의 생존율이 다소 더 높은 수준이었고 생존기간도 더 길었다. 사망위험에 대한 메타분석 결과 혈액투석 대비 복막투석의 비보정 HR의 통합추정치는 1.13 (n=14, 95% CI 1.05~1.22, I2=33%)이었으며, 보정된 HR의 통합추정치는 1.15로 복막투석의 사망위험이 더 높았으나 문헌간 이질성이 높았다(n=20, 95% CI 1.11~1.20, I2=81%). 보정된 HR의 하위군 분석결과, 이질성 원인은 탐색되지 않았으나 당뇨가 없는 경우, 단일기관 연구 및 RoBANS 도구의 비교가능성 영역에서 비뚤림 위험이 낮은 경우가 이질성이 낮았고 복막투석의 사망위험이 혈액투석에 비해 유의하게 더 높게 나타났다. 질병특이 사망률을 메타분석한 결과, 심혈관계질환으로 인한 사망의 경우 혈액투석군 대비 복막투석군의 상대위험도(relative risk, RR)가 1.63 (95% CI 1.35~1.97, I2=0%), 감염관련 사망의 경우가 RR 2.09 (95% CI 1.38~3.17, I2=0%)로 통계적으로 유의하게 더 높았다. 암관련 사망률은 두 군간 차이가 없었다(RR=0.91, 95% CI 0.52~1.60). 입원에 대해 보고한 문헌은 4편으로, 문헌별로 결과변수 정의 및 결과보고 양식이 다양하였다. 양적합성이 가능했던 입원율에 대한 분석결과 두 군 간 통계적으로 유의한 차이는 없었다(n=2, RR=1.09, 95% CI 1.00~1.19, I2=23%). 삶의 질(일반 3편, 질병특이 2편), 일상생활개선지표(2편), 영양상태(3편), 치료만족(1편)에 대해 보고한 문헌의 경우 두 군간 통계적으로 유의한 차이를 보고하는 문헌은 없었으며 메타분석에 필요한 자료를 정확히 보고하고 있지 않아 통합분석은 불가능하였다. 근소실증 및 응급실방문횟수를 보고한 문헌은 없었다.
2. 예후 체계적 문헌고찰 결과 1) 계획되지 않은 투석치료를 받은 노인 만성콩팥병 환자에서의 생존 예후요인 계획되지 않은 투석치료를 받은 만성콩팥병 노인 환자에서 생존에 유의한 영향을 미치는 요인을 분석하기 위하여 최종 선정된 문헌은 관찰연구 3편(n=823명)이었다. 65세 이상 노인만을 포함한 연구는 1편이었으며 나머지 2편은 성인 대상과 혼합된 연구였다. 3편의 연구에서 계획되지 않은 투석치료를 받은 대상자에서 생존에 유의한 영향을 미치는 요인으로 연령, 저칼륨혈증, 투석이후 동정맥루 수술 여부가 보고되었다. 연령은 높아질수록(n=2) 또는 고연령군에서(n=1) 사망위험이 높았으며, 저칼륨혈증에서 혈중칼륨수치가 증가할수록(HR 0.678, 95% CI 0.487~0.970, P=0.032), 계획되지 않은 투석이후 도관을 지속적으로 유지하여 투석한 경우보다 동정맥루를 만들어 투석을 지속한 경우 사망 위험이 유의하게 낮은 것으로 나타났다(HR 0.11, 95% CI 0.03~0.38). 투석종류는 2편에서 일관되지 않은 결과를 보고하고 있었다. 초고령 대상문헌은 없었으며 고령 대상문헌 1편(Zang, 2020)에서 당뇨병을 동반한 대상에서 혈중알부민 수치, 혈중칼륨수치, 투석종류가 생존에 영향을 미치는 유의한 요인으로 보고되었다. 혈중알부민 수치가 증가할수록(HR 0.926, 95% CI 0.861~1.000, P=0.049), 혈중칼륨수치가 증가할수록(HR 0.258, 95% CI 0.126~0.538, P<0.001) 사망위험은 유의하게 낮았으며 계획되지 않은 복막투석에 비해 계획되지 않은 혈액투석을 받은 대상에서 사망위험이 유의하게 높게 나타났다(HR 2.813, 95% CI 1.092~7.330, P=0.033). 2) 노인 만성콩팥병 환자에서 계획되지 않은 투석치료는 계획된 투석치료에 비해 사망에 미치는 영향이 어떠한가? 만성콩팥병 노인 환자에서 계획되지 않은 투석치료는 계획된 투석치료에 비해 사망에 미치는 영향의 차이를 분석하기 위하여 최종 선택된 문헌은 관찰연구 12편(n=13,268명)이었다. 9편이 고령 대상문헌(≥60세)이었으며 3편이 성인과 혼합된 문헌이었다. 이 중 초고령 대상(≥80세)문헌은 2편이 포함되었다. 1년 이내 비보정 사망위험(HR)에 대한 메타분석 결과, 계획되지 않은 투석치료의 사망위험이 계획된 투석에 비해 2.49배 유의하게 높았다(n=3, HR 2.49, 95% CI 2.11~2.94, I2=0%). 고령 대상 문헌만의 분석결과에서는 사망 위험이 2.29배(n=1, 95% CI 1.09~4.81), 초고령 대상의 경우는 2.55배(n=1, 95% CI 1.16~5.61) 유의하게 높은 결과를 보였다. 그러나 보정된 사망위험에 대한 통합분석 결과, 두 군간 사망위험 차이는 없었으며(HR 1.51, 95% CI 0.74~3.08, I2=58%), 초고령 대상 문헌의 결과에서는 계획되지 않은 투석치료의 사망위험이 계획된 투석에 비해 3.98배로 비보정 사망위험 보다 더 높았다(n=1, 95% CI 1.18~13.42). 1년 이상 비보정 사망위험에 대한 통합결과, 계획되지 않은 투석치료의 사망위험이 계획된 투석에 비해 2.06배 유의하게 높았으며 문헌간 이질성도 높았다(n=7, HR 2.06, 95% CI 1.53~2.76, I2=75%). 사전에 정한 하위군 분석을 통해 연구국가에 따른 분석에서 이질성은 해소되었다(아시아 HR 3.59, 95% CI 2.16~5.97, I2=48%; 비아시아 HR 1.50, 95% CI 1.35~1.68, I2=0%). 하위군 및 민감도 분석결과, 추적관찰 1년 시점의 문헌들에서 2.84배(I2=39%), 레지스트리로 연구한 경우 1.52배(I2=16%), 초고령 대상 문헌에서 2.84배(I2=39%)로 계획되지 않은 투석치료군의 사망위험이 유의하게 높았으며 문헌간 이질성은 낮았다. 1년 이상 보정된 사망위험의 통합결과, 계획되지 않은 투석치료의 사망위험이 계획된 투석에 비해 1.98배 유의하게 높았으나 문헌간 이질성은 높았다(n=5, HR 1.98, 95% CI 1.25~3.12, I2=78%). 연구국가별로 나누어 분석한 하위군 분석(아시아 HR 3.10, 95% CI 1.76~5.45, I2=48%; 비아시아 HR 1.30, 95% CI 0.97~1.75, I2=45%)과 보정변수에 따른 하위군 분석결과 이질성은 해소되었다(보정변수 5개 미만, HR 3.10, 95% CI 1.76~5.45, I2=48%; 보정변수 5개 이상, HR 1.30, 95% CI 0.97~1.75, I2=45%). 초고령 대상문헌 1편에서는 보정된 사망위험이 계획되지 않은 투석치료군에서 유의하게 높았다(HR 3.19, 95% CI 1.51~6.74). ▢ 결론 및 제언 본 연구는 체계적 문헌고찰 방법을 통해 노인 만성콩팥병 환자에서 투석치료 여부 및 투석치료법 간의 임상적 효과, 사전 계획 여부에 따른 예후 영향 및 예후요인을 평가하였다. 노인 만성콩팥병 환자에서 투석치료가 보존적 치료에 비해 더 나은 생존 이득을 나타냈으며, 삶의 질 측면에서는 정신적 영역 및 증상·문제 영역에서 투석 치료가 더 나을 수 있다는 근거를 확인하였다. 복막투석과 혈액투석간의 임상효과 비교에서는 혈액투석이 복막투석보다 더 좋은 생존율을 나타내는 관련성을 보였지만, 근거수준이 낮고 주요 결과변수들에 대해 두 치료법간 효과 차이를 입증하는 근거가 불명확하여 명확한 결론을 내리기 어렵다. 노인 및 초고령 대상의 질 높은 추가연구가 필요하며 투석치료 방법간의 의사결정은 각 국가 마다 임상현실이 다양한 점을 감안할 때, 환자의 상태와 각 투석 방법의 장단점을 고려하여 투석 방법을 결정하는 것이 필요하겠다. 노인 만성콩팥병 환자에서 계획된 투석치료에 비해 계획되지 않은 투석치료를 받을 경우 사망위험을 유의하게 높인다는 근거와 정량적 영향 정도를 확인하였으며, 계획되지 않은 투석치료를 받은 노인 만성콩팥병 환자에서의 유의한 생존 예후요인으로 연령, 저칼륨혈증, 계획되지 않은 투석이후 동정맥루 시행 여부가 확인되었으므로 이들 지표의 관리를 통한 생존 이득을 향상시킬 필요가 있다. 고령 및 초고령 대상을 명확한 연구대상자로 선정하고 이들 관련한 주요 교란요인을 충분히 조사하고 분석에 반영하는 추가 연구가 필요하다. 특히, 국내 노인 만성콩팥병 환자에 대한 인지심리적 상태, 기능상태 및 영양상태 등 임상적으로 의미있게 고려되는 교란요인을 충분히 보정한 근거 생성이 요구되며 지속적인 근거합성의 체계적 문헌고찰 연구가 필요하겠다.

▢ Background According to the 2017 Korea National Health and Nutrition Examination Survey, the prevalence of moderate to severe chronic kidney disease (CKD) is 16% for those aged ≥70 years and increases with age. In the 2020 Periodic Report by the Korean Society of Nephrology, the percentage of patients with CKD who were aged ≥65 years drastically increased from 2.1% in 1990 to 17.4% in 2000, 36% in 2010, and 54.6% in 2020. While clinical practice guidelines for CKD have been developed in major countries in the last decade, few guidelines are available for CKD the elderly or end-stage kidney disease. The only guideline with recommendations for the elderly is the one developed by the European Renal Association in 2017. The guideline provides recommendations regarding when to begin dialysis for elderly patients with CKD of stage 3b or higher, how to assess renal function, prognostic prediction models, how to assess nutritional status, and when to perform dialysis for frail elderly patients. However, the guideline lacks clinical evidence, and there is a continued need to develop clinical practice guidelines based on clinical evidence collected over time. A clinical practice guideline for CKD in elderly patients is yet to be established in Korea. This study aimed to systematically review scientific evidence that can form the basis of a domestic clinical practice guideline. This study will serve as basic research to the quality of life of elderly patients with CKD and support evidence-based decision-making. ▢ Objectives This study aimed to present objective evidence that can support the decision-making related to dialysis for elderly patients with CKD through a systematic literature review. The purpose of this study is to present objective evidence to help rational decision-making in deciding whether to receive dialysis treatment in actual clinical practice by analyzing treatment effects and prognostic factors related to dialysis treatment for elderly patients with CKD. The purpose of this study is 1) to identify the therapeutic effect related to dialysis treatment through comparative analysis of clinical effects between dialysis treatment and conservative management in elderly CKD patients, and 2) comparative analysis of clinical effects between hemodialysis and peritoneal dialysis in elderly chronic kidney disease patients, and 3) to prepare a scientific basis by analyzing the effects of prior planning of dialysis treatment on survival and independent prognostic factors in elderly patients with CKD.
▢ Methods A systematic review was performed to examine whether dialysis is more effective than conservative care, whether peritoneal dialysis is more effective than hemodialysis, and the effect of unplanned dialysis on the survival of elderly patients with CKD. Studies were searched in three international databases (Ovid-Medline, Ovid-EMBASE, Cochrane Central Register of Controlled Trials) and three domestic databases (KoreaMed, KMbase, KISS) and were hand-searched additionally. The search was not restricted by publication date and language. Two researchers independently reviewed each study and mutually agreed upon a final selection of studies. RoBANS 2.0 and QUIPS were used to assess the risk of bias in the finally selected studies. A meta-analysis, which is a type of quantitative analysis, was performed if evidence synthesis was possible. In case the synthesis was not possible, a qualitative review was performed. A meta-analysis was performed using a random-effects model and the generic inverse variance model to present the effect of a variable on survival in terms of hazard ratios (HRs) or odds ratios (ORs) and their 95% confidence intervals. Heterogeneity between studies was visualized on a forest plot and then assessed using the Cochrane Q statistic (P<0.10) and I2 statistics (≥50%) (Higgins et al., 2008). Subgroup analyses pre-defined age cut-offs for the elderly, country of research, research facility, study design, and the risk of bias were performed. In order to perform the sensitivity analysis, we analyzed studies that included only the elderly or studies that included advanced aged subjects (over 80 years old) or subjects with diabetes. If possible (≥10 studies selected), the risk of publication bias was analyzed using a funnel plot and statistical tests (Higgins et al., 2008). Statistical tests were performed using RevMan 5.4. A significance level of 5% was used to analyze the difference between the effect of planned dialysis and that of unplanned dialysis.
▢ Results 1. Systematic reviews on intervention 1) How do the safety and effectiveness of dialysis for elderly patients with CKD compare with those of conservative care? A systematic review was performed to compare the safety and effectiveness of dialysis for elderly patients with CKD to those of conservative care. Two studies examined the elderly aged ≥60 years, 5 examined those aged ≥65 years, 6 examined those aged ≥70 years, 7 examined those aged ≥75 years, and 80 examined those aged ≥80 years. The method of dialysis was hemodialysis in 3 studies, peritoneal dialysis in 1 study, and both in 17 studies. 1-, 2-, and 3-year survival rates were investigated in 14 studies that reported on overall survival rates. The 1-, 2-, and 3-year survival rates in the dialysis group were 85% (interquartile range (IQR) 70~96%), 73% (IQR 53~89%), and 58% (IQR 39~90%), respectively. The 1-, 2-, and 3-year survival rates in the conservative care group were 69% (IQR 29~83%), 43% (IQR 15~62%), and 25% (IQR 5~41%), respectively. The median survival time was 38 months (IQR 36~51 months) for the dialysis group and 20 months (IQR 14~27 months) for the conservative care group. The conservative care group had lower survival rates than the dialysis group at all time points and had lower survival time. In a meta-analysis of mortality, the unadjusted HR of mortality for dialysis relative to conservative care was 0.43 (95% confidence interval (CI) 0.37-0.50, I2=43%). In a subgroup analysis by the HRs of mortality, dialysis had significantly low mortality with low heterogeneity across the studies (HR 0.42, 95% CI 0.34~0.53, I2=53%). The adjusted HR for dialysis was 0.44 (95% CI 0.32~0.60, I2=82%). In a subgroup analysis by follow-up period, mortality was significantly low if the follow-up period was less than 3 years with low heterogeneity across the studies (HR 0.55, 95% CI 0.44~0.69, I2=0%). In subgroup analyses by age cut-off, dialysis modality, publication year, center type (single/multi center), and ‘comparability’ from the RoBANS, the HR for mortality in the dialysis group relative to the conservative care group was less than 1 and was statistically significant. I2 was 26% if the age cut-off was ≥70 years and 0% if patients received both hemodialysis and peritoneal dialysis; thus, these factors reduce the heterogeneity between the studies. Seven studies reported on the quality of life using different assessment tools and reporting formats. A meta-analysis of the EQ-5D, SF-12 physical component score (PCS), and SF-12 mental component score (MCS), which are quality-of-life assessment tools, showed a standard mean difference (SMD) of 0.28 (95% CI 0.09~0.47, I2=25%) for SF-12 MCS. The dialysis group scored significantly high in the “symptom or problem list” and “effects of kidney disease” domains of the Kidney Disease Quality of Life (KDQoL), which is a disease-specific quality of life assessment tool. The conservative care group scored significantly low in the “burden of kidney disease” domain of the KDQoL. Seven studies reported on the hospital days. Since the studies varied in their definitions of outcome variables and reporting formats, a meta-analysis was conducted on 3 studies for which synthesis was possible. No significant difference in the hospital days was found between the dialysis and conservative care groups (mean difference (MD) –0.03, 95% CI –0.95~0.89). Three studies reported on hospitalization rates. Since the studies varied in their outcome variables and reporting formats, quantitative synthesis could not be performed. The hospitalization rate was significantly higher in the dialysis group during 6 months, but there was no difference between the two groups in the number of hospitalizations or the annual hospitalization rate per capita. Three studies reported on disease-specific mortalities. A meta-analysis was performed after categorizing mortalities into infection-, cardiovascular disease-, and treatment termination-related mortalities. No significant differences were found for all four mortality categories. One study reported on treatment satisfaction measured using the Renal Treatment Satisfaction Questionnaire (RTSQ). Satisfaction scores were 30.5, 31 and 32 for conservative care, hemodialysis groups and peritoneal dialysis, respectively; the peritoneal dialysis group had the highest satisfaction score, indicating that peritoneal dialysis achieves the highest treatment satisfaction.
2) How is the clinical safety and effectiveness of peritoneal dialysis compared to hemodialysis in elderly patients with CKD? Thirty four studies were selected for a systematic review conducted to investigate the effects of peritoneal dialysis and hemodialysis on elderly patients with CKD. Sixteen studies included elderly patients only, and 18 included elderly and adult patients. The median 1-, 3-, and 5-year survival rates and their IQRs in the peritoneal dialysis and hemodialysis groups were 78.8% (IQR 67.6~85.9%) and 77.2% (IQR 75.1~88.7%) for 1-year survival, 45.6% (IQR 36.3~60.0%) and 48.2% (IQR 45.1~57.9%) for 3-year survival, and 26.2% (IQR 20.7~41.9%) and 27.5% (IQR 25.7~37.0%) for 5-year survival, respectively. The median survival time (MST) was 31 months (IQR 23.0~33.9 months) for the peritoneal dialysis group and 32.5 months (IQR 27.3~39.3 months) for the hemodialysis group. In a meta-analysis of mortality, the unadjusted and adjusted HRs of mortality for peritoneal dialysis relative to hemodialysis was 1.13 (n=14, 95% CI 1.05~1.22, I2=33%) and 1.15 (n=20, 95% CI1.11~1.20, I2=81%), respectively, indicating higher mortality for peritoneal dialysis. In a subgroup analysis by age cut-off for the elderly, diabetes, publication year, country of research, number of participating institutions, and “comparability” in the RoBANS, the HR of mortality for peritoneal dialysis relative to hemodialysis was greater than 1 and was statistically significant. I2 for adjusted HRs was 53% for studies not including diabetic patients and 0% for those with a low risk of bias in the comparability domain of the RoBANS. In case of disease-specific mortality, the relative risk (RR) of the peritoneal dialysis group compared to the hemodialysis group for cardiovascular disease was 1.63 (n=4, 95% CI 1.35~1.97, I2=0%), and for infection 2.09 (n=4, 95% CI 1.38~3.17, I2=0%). For cancer, the RR of the peritoneal dialysis group compared to the hemodialysis group was 0.91 (n=3, 95% CI 0.52~1.60, I2=0%), which was not statistically significant. As a result of meta-analysis of RR for hospitalization in the peritoneal dialysis group compared to the hemodialysis group, there was no statistically significant difference between the two groups (n=2, RR=1.09, 95% CI 1.00~1.19, I2=23%). No significant differences were found between peritoneal dialysis and hemodialysis in the studies reporting on the quality of life, indicators of improvement in daily life, nutritional status, and treatment satisfaction. No study reported on sarcopenia and the number of visits to an emergency room.
2. Systematic review on prognosis 1) Significant prognostic factors of survival in elderly patients with CKD on unplanned dialysis Three observational studies (n=823) were reviewed to investigate the significant prognostic factors of survival in elderly patients with CKD on unplanned dialysis. One study was conducted on the elderly aged ≥65 years, and 2 on both adults and the elderly. The factors significantly affecting the survival of patients on unplanned dialysis in the three studies were age, blood potassium levels, dialysis type, and undergoing AVF surgery following the dialysis The risk of death was significantly lower with increasing age (n=2) or in the older age group (n=1) (HR 0.11, 95% CI 0.03~0.38). Mortality significantly decreased as blood potassium levels increased for patients with hypokalemia (HR 0.678, 95% CI 0.487~0.970, p=0.032) and was significantly low in patients who received AVF surgery after unplanned dialysis (HR 0.11, 95% CI 0.03~0.38). Results according to the modality of dialysis were not consistent between the two studies. One study reported a higher risk of death for unplanned hemodialysis compared to unplanned peritoneal dialysis (HR 2.220, 95% CI 1.298~2.790, p=0.004), and no difference was reported in the other study. No study examined patients of advanced age. One study on the diabetic elderly (Zang, 2020) identified blood albumin and potassium levels and receiving unplanned hemodialysis rather than unplanned peritoneal dialysis as prognostic factors significantly affecting survival. As blood albumin (HR 0.926, 95% CI 0.861~1.000, p=0.049) and potassium levels (HR 0.258, 95% CI 0.126~0.538, p<0.001) increased, mortality significantly decreased. Mortality was significantly higher among patients who were on unplanned hemodialysis than those on unplanned peritoneal dialysis (HR 2.813, 95% CI 1.092~7.330, p=0.033) 2) How does mortality compare between planned dialysis and unplanned dialysis in elderly patients with CKD? Twelve observational studies (n=13,268) were reviewed to investigate the difference between the effect of unplanned dialysis and that of planned dialysis on elderly patients with CKD. Nine studies were on elderly patients (≥60 years), and 3 on elderly and adult patients. Of these, 2 were on patients of advanced age (≥80 years). In a meta-analysis of mortality based on univariate analysis results on short-term survival (<1 year), mortality was 2.49 times higher for unplanned dialysis than planned dialysis (n=3, HR 2.49, 95% CI 2.11~2.94, I2=0%). In an analysis of studies on elderly patients only, mortality was 2.29 times (n=1, 95% CI 1.09~4.81) higher for unplanned dialysis than planned dialysis. For patients of advanced age, mortality was 2.55 times higher (n=1, 95% CI 1.16~5.61) for unplanned dialysis than planned dialysis. However, after combining the results of multivariate analyses, the difference in mortality between the two groups was no longer found (HR 1.51, 95% CI 0.74~3.08, I2=58%). In an analysis of studies on patients of advanced age, mortality was 3.98 times higher for unplanned dialysis than planned dialysis; this difference in mortality was higher than the one based on the combined univariate analysis results (n=1, 95% CI 1.18~13.42). Based on the combined results of univariate analyses, overall mortality (≥1 year) was 2.06 times higher for unplanned dialysis than planned dialysis; however, high heterogeneity was found among the studies (n=7, HR 2.06, 95% CI 1.53~2.76, I2=75%). Heterogeneity was reduced through a subgroup analysis by country of research (Asia HR 3.59, 95% CI 2.16~5.97, I2=48%; Non-Asia HR 1.50, 95% CI 1.35~1.68, I2=0%). In subgroup and sensitivity analyses, mortality was 2.84 times higher for patients with unplanned dialysis in studies with one-year follow-up (I2=39%), 1.52 times higher in studies using registries (I2=16%), and 2.84 times higher in studies on patients of advanced age (I2=39%) with insignificant heterogeneity across the studies. After combining the results of multivariate analyses of ≥1-year mortality, mortality was 1.98 times higher for patients on unplanned dialysis than those on planned dialysis, but the heterogeneity among the studies remained high (n=5, HR 1.98, 95% CI 1.25~3.12, I2=78%). Heterogeneity was resolved via subgroup analysis by country of research (Asian countries HR 3.10, 95% CI 1.76~5.45, I2=48%; Non-Asian countries HR 1.30, 95% CI 0.97~1.75, I2=45%) and confounding variables (HR 3.10, 95% CI 1.76~5.45, I2=48% for (<5 confounding variables; HR 1.30, 95% CI 0.97~1.75, I2=45% for ≥5 confounding variables). In one study on patients of advanced age, mortality was significantly higher for patients on unplanned hemodialysis (HR 3.19, 95% CI 1.51~6.74).
▢ Conclusions This study examined the clinical effect of dialysis and the factors influencing the prognosis of unplanned dialysis in elderly patients with CKD through a systematic review. In elderly patients with chronic kidney disease, dialysis group showed better survival benefits than conservative care. We confirmed that overall quality of life or symptom improvement in CKD may be better in dialysis group. In the comparison of peritoneal dialysis and hemodialysis in elderly patients with CKD, mortality was higher for patients on peritoneal dialysis. Therefore, well-designed studies are needed to produce more evidence for dialysis. It was confirmed that the mortality was significantly increased when receiving unplanned dialysis treatment compared to planned dialysis treatment in elderly patients with chronic kidney disease. The factors significantly affecting the survival of patients on unplanned dialysis in the three studies were age, blood potassium levels, dialysis type, and undergoing AVF surgery following the dialysis Overall, evidence is insufficient and many methodological limitations of the study have been identified. High-quality observational studies of elderly chronic kidney disease patients in Korea or long-term follow-up on a large scale require the clinically meaningful outcomes such as cognitive, functional, and nutritional status.
