▢ 연구배경 및 목적
우리나라 위암 발생률은 세계적으로 가장 높은 수준이며, 1983년 공식적인 암발생 통계를 집계하기 시작한 이래 줄곧 남성 암발생률 1위를 차지하고 있다. 2019년에 발표된 암등록 통계자료에 의하면 2017년 국내 위암 신환자는 2만 9,685명으로 전체 암 발생자 23만 2,255명의 12.8%를 차지하였으며 인구 10만 명 당 조발생률은 57.9건으로 이는 미국의 약 10배 수준에 달한다.
우리나라는 위암을 조기에 발견하여 사망률을 낮추면서 환자 예후 개선 및 삶의 질을 유지하기 위해 국가 암검진 사업에 위암 검진을 포함하여 만 40세 이상 성인을 대상으로 위장조영술 또는 상부위장관 내시경 검사를 2년마다 시행하고 있다. 이와 더불어 소화기내시경 검사의 보편화와 국민의 건강검진에 대한 인식 개선은 위암의 조기 발견에 일조하였으며 조기 위암 발견과 더불어 위선종의 진단 역시 지속적으로 늘어나고 있다.
위선종은 위암의 전구병변으로 간주되고 있기 때문에 고도 위선종의 경우 내시경적 절제술을 시행하는 것을 치료 원칙으로 하고 있으며, 저도의 경우에도 논란이 있기는 하나 내시경적 절제술을 시행하는 추세이다. 위선종 발생 건수가 매년 증가함에 따라 내시경적 절제술 수행 빈도 및 의료비 규모 역시 증가 추세를 보이고 있으나 그 치료효과에 대한 근거는 매우 부족한 상황이다. 서양에서 발생률이 높은 대장암의 경우 위암과 마찬가지로 대장용종이 진행하여 암이 발생한다고 알려져 있으며 치료 방법 역시 내시경적 절제술을 통한 제거를 원칙으로 한다. 대장용종의 내시경적 절제술에 대한 치료효과 연구는 다수가 존재하며 대장용종 제거 시 대장암 발생위험과 대장암 관련 사망이 감소한다고 알려져 있다 Winawer SJ, Zauber AG, Ho MN, O'Brien MJ, Gottlieb LS, Sternberg SS, et al. . 또한 대장용종 절제 시 용종의 개수, 조직형, 크기 등에 따라 추적 검사의 시기가 가이드라인에 반영되어 있다. 반면 위선종에 대한 내시경적 절제 이후 위암 발생과 사망률이 감소되는지 여부에 대한 추적 관찰 연구가 부재하며, 따라서 위선종 절제술 시행 이후 관리 방안에 대한 가이드라인도 제시되어 있지 않다.
이에 본 연구에서는 국민건강보험공단 건강보험 청구자료, 국립암센터 암등록통계자료, 통계청 자료 등 국내 보건의료 빅데이터를 활용하여 위선종을 내시경적 절제술로 제거한 이후 전체 사망률, 위암 발생 및 위암 관련 사망률을 확인하였다. 또한 단일기관 환자자료 분석을 통해 빅데이터를 통한 대상자 정의 및 임상결과의 타당성을 검토하였다.
▢ 연구방법
본 연구는 위암의 전구병변으로 알려져 있는 위선종을 내시경적 절제술로 제거하는 치료 방법의 효과를 확인하기 위해 보건의료 분야 빅데이터 분석 연구를 수행하였다. 국민건강보험공단 건강보험 청구자료 맞춤형 자료를 통해 2011년 1월 1일~2013년 12월 31일 사이에 위선종을 진단 받고 6개월 이내에 내시경 절제술을 시행받은 40세 이상 환자를 중재군(위선종(adenoma)군)으로 정의하였다. 대조군은 2011년 1월 1일~2013년 12월 31일 사이에 국가검진 또는 민간에서 상부위장관내시경을 받은 40세 이상 대상 중 중재군과 성별, 연령 및 입적연도를 매칭한 대상자(대조군1, 비위선종(non-adenoma) 대조군)와 일반인구(대조군2)로 정의하였다. 연구 대상 중 입적일 이전 2년 이내에 위선종으로 내시경 절제술을 받은 경우, 위궤양 등의 위수술을 받은 경우 및 암(ICD-10 C code) 발생 내역이 있는 경우는 제외하였다. 또한 위선종 및 위암의 발생 기간 등 임상적 특성과 최소 추적관찰 기간을 고려하여 입적일 이후 1년 이내에 관심 결과지표(위암 발생, 사망)가 발생한 환자의 경우 제외하였으며 입적일 이후 1년 이내에 위선종을 진단받은 대조군을 추가 제외하였다.
위선종 내시경절제술의 효과를 확인하기 위해 위암 발생, 위암 사망 및 전체 사망을 결과지표로 포함하였으며 결과지표에 영향을 줄 수 있는 공변량으로 성, 연령, 소득수준 등의 일반적 특성과 헬리코박터 치료여부, 아스피린, 스타틴 등 약물복용력, 동반질환 여부 등의 임상적 특성을 고려하였다. 또한 일반건강검진 이력이 있는 대상자(검진군)의 경우에는 흡연, 음주, 체질량지수를 추가로 고려하였다. 위선종군과 건강보험 청구자료를 통해 정의한 상부위장관내시경을 받았으나 위선종 또는 위암이 없는 대조군 간 결과지표 발생률을 비교하기 위해 콕스비례위험모형을 통한 위험비를 추정하고 생존분석을 수행하였다. 기본 분석 시 사망 여부는 건강보험공단의 자격자료를 바탕으로 정의하였으며 위암 발생 여부는 암등록통계자료를 기준으로 정의하였다. 이 때 자료원의 타당성을 확인하기 위해 건강보험 청구자료와 암등록자료를 모두 포함하여 위암 발생 여부를 확인하고 이에 대한 민감도 분석을 수행하였다.
위선종 중재군과 일반인구 대조군과는 통계청 사망률 및 위암 발생률 자료를 이용하여 성-연령 표준화 사망률 및 성-연령 표준화 위암발생률을 산출하였다. 일반인구의 경우 대상자 제외 기준이나 공변량을 적용할 수 없으므로 자료추출 시 오류인 경우를 제외한 나머지 대상자 제외 기준을 적용하기 이전의 위선종군과 일반인구 간의 비교분석을 민감도 분석으로 추가 수행하였다.
▢ 연구결과
건강보험 청구자료를 통해 추출된 분석 대상자는 총 634,533명(중재군 57,685명, 대조군 576,848명)이었으며, 제외 기준을 적용한 후 최종 분석 대상자는 위선종 중재군 44,405명, 대조군 527,945명이었다. 성향점수 매칭 이후 군 간 대상자수는 39,826명이었으며, 이 중 국가검진을 받은 검진군은 23,330명, 비검진군은 16,496명이었다. 성향점수 매칭 이후 기저특성의 분포를 검진 여부에 따라 층화하여 비교하였으며 검진군 내 중재군의 평균 연령은 63세(std 9.9), 대조군은 62.9세(std 9.9)로 유의하였으나 비검진군 내 중재군과 대조군의 평균 연령은 각각 63.9세(std 10.1), 62.6세(std 10.5)로 유의한 차이를 보였다. 성별 분포의 경우 대조군에 비해 중재군에서 남성의 비율이 높았으며 소화성 궤양 여부는 매칭 이후에 대조군에서 다소 높게 나타났다.
콕스비례위험모형을 통해 위암 발생 위험을 분석한 결과 성향점수 매칭 후 비위선종 대조군과 비교하여 위선종 중재군에서 위암 발생 위험이 유의하게 높은 것으로 나타났다(HR=2.84, 95% CI: 2.51-3.21). 이는 검진 여부로 층화한 결과 및 추적관찰 상부위장관내시경 시행 빈도로 층화한 결과에서도 동일하게 나타났다. 일반인구 대조군과 비교한 성-연령 표준화 위암 발생률에서도 중재군의 위암 발생 위험은 유의하게 높은 것으로 나타났다(SIR=1.98, 95% CI: 1.84-2.13).
위암 사망은 위암 환자에서 발생한 사망으로 정의하였으며 성향점수 매칭 후 위암 사망은 대조군에 비해 중재군에서 유의하게 높게 나타났다(HR=1.66, 95% CI: 1.19-2.31). 검진 여부로 층화 시 검진군에서는 군 간 유의한 차이를 보이지 않았으며(HR=1.20, 95% CI: 0.77-1.88), 비검진군에서는 대조군에 비해 중재군의 위암 사망 위험이 약 2.4배 높은 것으로 나타났다(HR=2.44, 95% CI: 1.46-4.07). 반면 일반인구 대조군과 비교한 표준화 사망률의 경우 위선종 중재군의 위암 사망률은 대조군 대비 약 50% 낮은 것으로 나타났으며(SMR=0.50, 95% CI: 0.40-0.62) 제외 기준을 적용하지 않은 중재군에서도 같은 경향을 보였다.
전체 사망 발생을 분석한 결과 성향점수 매칭 이후 비위선종 대조군과 비교하여 중재군에서의 사망 발생이 유의하게 낮은 것으로 나타났다(HR=0.93, 95% CI: 0.89-0.97). 검진 여부로 층화하여 분석한 결과 검진군 내 중재군의 사망 발생 위험은 6% 낮았으나 통계적으로 유의하지 않았으며(HR=0.94, 95% CI: 0.88-1.00), 비검진군 내 중재군의 사망 발생 위험은 대조군에 비해 유의하게 낮게 나타났다(HR=0.92, 95% CI: 0.86-0.98). 위선종 중재군의 사망발생과 일반인구 대조군에서 산출한 기대 사망 발생을 비교한 성-연령 표준화 사망률의 경우 위선종군의 사망발생률이 일반인구 대비 50% 낮은 것으로 나타났으며(SMR=0.50, 95% CI: 0.48-0.52), 대상자 제외 기준을 적용하지 않은 경우에도 유사한 결과를 보였다.
빅데이터 분석과 별개로 서울성모병원 임상데이터자료원을 활용하여 위선종 내시경절제술을 받은 중재군에 대해 타당성을 확인하였다. 빅데이터 분석 연구와 동일한 기준으로 대상자를 정의한 후 환자의무기록을 통한 병리결과와 대조하여 진단정확도를 산출하였다. 분석결과 위선종 상병코드 및 내시경절제술 진단코드를 통한 대상자 분류의 정확도는 약 95%로 이상인 것으로 나타났다.
▢ 결론 및 정책적 제언
본 연구는 국내 공공자료원을 이용하여 위선종을 내시경적 절제술을 통해 제거하는 치료법의 효과를 확인하였다. 위선종은 위내시경을 통해 진단되며 이후 내시경 절제술로 제거하게 된다. 이 때 위선종을 진단받았으나 내시경 절제술을 시행하지 않은 환자는 대부분 고령이거나 출혈의 위험이 있는 등 위선종 내시경절제술을 받은 환자군과 확연히 다른 특성을 갖는다. 이에 위선종을 내시경절제술로 제거한 군과 제거하지 않은 군 간의 비교분석은 부적절하다는 임상전문가의 의견에 따라 대장용종 선행 연구 등의 연구설계를 바탕으로 위선종이 없는 군과 일반인구를 대조군으로 설정하여 분석을 수행하였다. 이 때 위선종이 없는 군의 경우 중재군 선정 기간과 동일한 기간에 상부위장관내시경을 받은 대상으로 제한하여 건강 관련 특성, 건강 행동 등의 동질성을 확보하고자 하였다.
연구결과 위암 발생의 경우 중재군에서 발생률이 유의하게 높았으며 이는 위선종을 제거한 이후에도 위선종이 발생한 군은 위암 발생의 고위험군임을 의미한다. 대장 용종과 마찬가지로 위선종 역시 위점막의 손상에 의해 발생된다고 여겨지며 위선종을 제거한 환자의 경우 남은 점막도 손상되었을 가능성이 있어 위암 발생의 위험이 높을 수 있다. 이는 표본코호트를 활용한 예비연구와 서울성모병원 환자자료를 통해 위내시경 절제술을 받은 조기위암군과 위선종군의 위암 발생률을 비교한 연구결과와도 유사하다. 위암 사망의 경우 비위선종 대조군에 비해서는 전반적으로 유의하게 높게 나타났으나 일반인구 대조군과 비교하여 약 50% 낮은 것으로 나타났다. 이는 위암 발생 및 사망에 대해 비교적 저위험군에 해당하는 비위선종 대조군에 비해서는 발생률이 높으나 일반인구 대조군에 비해서는 발생률이 낮음을 보여주는 것으로 대장 용종에 대한 선행 연구와도 유사한 결과를 나타내고 있다. 반면 전체 사망 발생위험의 경우 비위선종 대조군과 비교하여 중재군에서 전반적으로 유의하게 감소하는 경향을 보였으며, 일반인구 대비 사망률 역시 약 50% 낮게 나타났다.
본 연구는 위선종 내시경절제술이 위암 발생 및 사망에 미치는 효과를 확인한 최초의 연구이며 위선종 내시경절제술을 받은 군에서 위암 발생은 높으나 사망의 위험은 낮추는 것을 확인할 수 있었다. 특히 보건의료 빅데이터를 통해 실제 임상현장에서의 결과를 확인할 수 있었으며 건강보험 청구자료와 암등록통계자료 간 연계자료원을 구축하여 위암을 정의하고 요약병기 별 결과를 제시하고 있다. 또한 위선종을 제거한 이후에도 위암 발생 위험이 높은 것을 확인함으로써 해당 환자들에 대한 보다 철저한 추적관찰과 함께 추적 검사의 빈도 및 강도에 대한 임상적 가이드라인 개발의 필요성을 확인하였다.
본 연구 결과를 해석할 때 고려해야할 한계점은 다음과 같다. 첫째, 위선종을 진단 받았으나 위선종 절제술을 받지 않은 군 간의 직접 비교가 불가능하였다. 둘째, 일반인구 대조군의 경우 대상자 제외 기준 적용 또는 잠재적 교란요인 통제가 불가능하다. 이는 일반인구 대조군을 사용하는 연구들이 갖는 일반적 제한점이나 본 연구의 경우 최소한의 제외 기준만을 적용한 중재군을 바탕으로 민감도 분석을 수행함으로써 결과의 타당성을 검증하였다. 셋째, 제거된 위선종의 조직검사 결과를 확인할 수는 없었다. 저도와 고도 선종은 Vienna classification 구분에 의해 각각 카테고리 III과 IV에 해당하며, 악성 위험도는 다르다. 하지만 이번 연구의 목적은 전암 병변 제거 후의 암 발생이므로 절제된 일차병변의 병리보다는 이후 추적 검사에 주안점을 두었다. 추가로, 위에서 제시한 단일기관 연구에서는 저도 선종 제거 후에도 고도 및 위암 제거 후에 발생하는 위암 위험도와 크게 다르지 않음을 제시하였다. 넷째, 위암 사망을 위암 환자 중 사망한 경우로 정의함으로써 위암 관련 사망이 과대추정되었을 우려가 있다. 이에 대해 원격전이 위암 환자에서 사망한 경우로 위암 사망을 정의하여 민감도 분석을 수행하였으나 암등록통계자료를 기준으로 최초 암이 원격전이로 진단되는 경우는 매우 낮기 때문에, 보다 정확한 사망원인 자료를 바탕으로 한 후속 연구가 필요할 것으로 보인다.
▢ Background
The incidence rate of gastric cancer in South Korea is one of the highest in the world. Since the national cancer statistics has reported in 1983, it has been ranked first in the male cancer incidence rate. Based on the statistics in 2017, there were 29,685 newly diagnosed gastric cancer patients, accounting for 12.8% of 232,255 cancer incident cases. The age adjusted incidence rate was 57.9 per 100,000, which was 10 times higher than the rate in US.
In Korea, gastric cancer screening has been conducted in every two year for adults aged 40 years or above as a part of the national cancer screening program, in order to lower the mortality rate, improve the prognosis, and maintain the quality of life of patients by detecting gastric cancer at an early stage. In addition, expansion of gastrointestinal endoscopy use and improvement of public awareness of health examinations contributed to the early detection of gastric cancer, resulting in increse of gastric adenoma diagnosis.
Gastric adenoma is a premalignant lesion of gastric cancer. It is detected during endoscopy as a gastric mucosal lesion of the polypoid type with histological dysplasia. For gastric adenomas, the gastric epithelial cells may undergo atypical changes to mediate the progression of gastric adenomas with low-grade or high-grade dysplasia to gastric cancer. According to a previous study, the probability of progression from gastric adenoma with high-grade dysplasia to gastric cancer is 6%/yr and that of progression from gastric adenoma with low-grade dysplasia to gastric cancer is 0.6%/yr.
The treatment principles for gastric adenoma vary according to the grade. Nevertheless, in many countries including those in Europe, endoscopic resection is recommended for gastric adenomas with high-grade dysplasia, while it is also recommended for accurate pathological diagnosis of gastric adenomas with low-grade dysplasia. While the frequency of endoscopic resection of gastric adenomas is continuously increasing each year, evidence regarding whether such resections lower the incidence of gastric cancer or the mortality related to gastric cancer is insufficient compared to those of colon adenoma with a similar structure. Hence, there is no guideline on patients’ management after gastric adenoma resection. In a previous study on the incidence of gastric cancer after the endoscopic resection of gastric adenomas, the incidence was found to be similar to the incidence of metachronous gastric cancer after the early resection of gastric cancer. The result implied the need for increased awareness of the risk of gastric cancer even after the removal of gastric adenomas, but as the study was conducted on a single institution and had a small sample size, evidence from data derived from a nationwide registry is required.
▢ Objective
This study aimed to investigate overall mortality, gastric cancer incidence as well as gastric cancer-related mortality after removing gastric adenoma through endoscopic resection based on the national health care bigdata such as health insurance claims data from the National Health Insurance Service, cancer statistics from the National Cancer Center, and data from the Korean Statistical Information Service. In addition, definition of study subjects applied and outcomes drwan from the bigdata was validated by comparing with analysis results using patient data retrospectively obtained from a single medical center.
▢ Methods
Among the study subjects, the intervention group contained patients aged 40 yrs or above, who were diagnosed with gastric adenoma between 2011 and 2013 and underwent endoscopic resection within 6 months, based on the health insurance claim data of the NHIS. The control group comprised healthy individuals aged 40 yrs or above, who received upper gastric endoscopy in the same period as the patients in the intervention group but had neither gastric adenoma nor cancer. The control group was matched on age, sex, and index year with the intervention group with 1:5 ratio.
The exclusion criteria were patients aged below 40 yrs, patients diagnosed with cancer (C code), and patients who underwent gastric surgery due to gastric ulcer or endoscopic resection of a gastric adenoma within two years before the index date. In addition, patients with any outcome of interest or controls diagnosed with gastric adenoma within a year after the index date were excluded. The primary outcome was gastric cancer mortality, and the secondary outcomes were gastric cancer incidence and overall mortality. The covariates were the potential risk factors of gastric cancer incidence; age, gender, smoking and drinking, BMI, history of drug use, history of gastric ulcer, and history of helicobacter therapy. Regarding helicobacter therapy, positive cases were those with a recorded prescription of 7–14 days of triple or quadruple drug therapy within two years of the index year, as well as those with a cumulative of 180 days or more of drug prescription within two years of the index year. Variables of physical examination such as smoking, and BMI were extracted from the national screening data recorded before or after six months of the index date.
In order to compare the gastric cancer incidence and mortality between general population and patients who underwent gastric adenoma resection, the KOSTAT data were used to estimate the age·sex standardized rates of gastric cancer incidence and mortality as well as the overall mortality.
Lastly, to validate the subjects definitions and estimated results, patient data from a single medical center were analyzed. The study subjects were defined based on the same conditions with the main analysis using bigdata, that was patients who were diagnosed with gastric adenoma between 2011 and 2013 and underwent endoscopic resection within 6 months. The control group comprised patients who received the upper gastric endoscopy in the same period as the patients in the intervention group but had neither gastric adenoma nor cancer.
▢ Results
A total of 634,533 subjects were initially extracted from the claims data. After applying the exclusion criteria, 44,405 subjects were included in the intervention group and 527,945 subjects were in the control group, making a total of 572,350 subjects. After propensity score matching, the number of subjects in each group was 39,826, of which 23,330 were received the national health screening. The distribution of baseline characteristics were stratified based on whether health screening was received or not. For the screened intervention group, the mean age was 63(std 9.9), while for screened control group, the mean age was 62.9(std 9.9). In terms of sex, male proportion was statistically higher in the intervention group, compared to the control group.
The results from the Cox proportional hazard model indicated that the gastric incidence rate was approximately 3 times higher in the intervention group (HR=2.84, 95% CI: 2.51-3.21). The trend remained the same after stratifying the subjects based on the status of national health screening and the frequency of follow-up upper gastrointestinal endoscopy. Age-sex standardized gastric cancer incidence was also higher in the intervention group than in the general population control group (SIR=1.98, 95% CI: 1.84-2.13).
Gastric cancer related mortality, defined as death in gastric cancer patients, was statistically significantly higher in the intervention group as well (HR=1.66, 95% CI: 1.19-2.31). However, after stratification by health screening status, the result was no longer significance in the screened group (HR=1.20, 95% CI: 0.77-1.88). In terms of the standardized mortality rate comparing to the general population, patients in the intervention group were less likely to die from gastric cancer (SMR=0.50, 95% CI: 0.40-0.62).
After the propensity score matching, the overall mortality was significantly lower in the intervention group (HR=0.93, 95% CI: 0.89-0.97). As a result of a stratified analysis by screening status, the risk of overall death in the intervention group was 6% lower, but it was not statistically significant (HR=0.94, 95% CI: 0.88-1.00). The risk of death in the non-screened intervention group was 8% lower than that in the non-screened control group, which was statistically significant.
▢ Conclusions
This study investigated the clinical effectiveness of treatment to remove gastric adenoma through endoscopic resection using public data sources. Gastric adenoma is diagnosed through gastroscopy and is then removed by endoscopic resection. At this time, patients who were diagnosed with gastric adenoma but did not undergo endoscopic resection have significantly different characteristics from the group of patients who underwent endoscopic gastric adenoma resection, such as being elderly or at risk of bleeding. According to the opinion of clinical experts that a comparative analysis between the group in which the gastric adenoma was removed by endoscopic resection and the group in which it was not removed by endoscopic resection was inappropriate, the group without gastric adenoma and the general population were set to be control group refered to the colorectal polyps study. In terms of comparability and homogeneity of baseline characteristics among study subjects, control groups were selected from subjects who received upper gastrointestinal endoscopy during the same period as the intervention group selection period.
As a result of the study, the incidence of gastric cancer was significantly higher in the intervention group, that is, the group that developed gastric cancer even after removal of the gastric adenoma was a high-risk group for gastric cancer. Like colon polyps, gastric adenoma is also considered to be caused by damage to the gastric mucosa, and in the case of a patient who has had the gastric adenoma removed, the remaining mucous membrane may also be damaged, which may increase the risk of gastric cancer. This was similar to the results of a preliminary study using a sample cohort and a study comparing the incidence of gastric cancer in the early gastric cancer group and the gastric adenoma group that underwent gastroscopic resection through patient data at Seoul St. Mary's Hospital. In the case of gastric cancer death, it was found to be significantly higher in the intervention group compared to the non-gastric adenoma control group, but it was found to be about 50% lower than that of the general population control group. This shows that the incidence rate was higher than that of the non-gastric adenoma control group, which was a relatively low-risk group for gastric cancer occurrence and death, but the incidence rate was lower than that of the general population control group, showing similar results to previous studies on colon polyps. On the other hand, the overall mortality risk showed a tendency to decrease significantly in the intervention group compared to the control group, and the mortality rate was also about 50% lower than that of the general population.
This study was the first study to investigate the effectiveness of endoscopic gastroadenoma resection on the incidence and mortality of gastric cancer. In particular, we were able to generate the real world evidence through health and medical big data, and established a linked data between health insurance claim data and cancer registration statistical data to define gastric cancer cases and analyze based on the cancer stage. In addition, by confirming the high risk of gastric cancer even after gastric adenoma was removed, the necessity of developing clinical guidelines for the frequency and intensity of follow-up examinations along with more thorough follow-up of the patients was confirmed.
The limitations to be considered when interpreting the results of this study are as follows. First, a direct comparison between the groups diagnosed with gastric adenoma but not undergoing gastric adenoma resection was not possible. Second, in the case of the general population control group, it is impossible to apply the criteria for exclusion of subjects or control potential confounding factors. This is a general limitation of studies using the general population control group, but in this study, the validity of the results was verified by performing a sensitivity analysis based on the intervention group that applied only the minimum exclusion criteria. Third, the biopsy result of the removed gastric adenoma could not be confirmed. Low-grade and high-grade adenomas fall into categories III and IV, respectively, according to the Vienna classification, and the risk of malignancy is different. However, the purpose of this study was to focus on the follow-up examination rather than the pathology of the resected primary lesion, since cancer was developed after removal of the precancerous lesion. In addition, the single-center study presented above suggested that even after removal of the low-grade adenoma, it was not significantly different from the high-grade and gastric cancer risk after gastric cancer removal. Fourth, there is a possibility that gastric cancer-related deaths may have been overestimated by defining gastric cancer deaths as deaths among gastric cancer patients. In this regard, a sensitivity analysis was performed by defining gastric cancer death as the case of death in a patient with distant metastasis gastric cancer. However, a follow-up study may be necessary using more accurate data with a cause of death.
