▢ 서 론
우리나라 65세 이상 인구 중 두 가지 이상의 만성질환을 동시에 지닌 복합이환율(multimorbidity)은 54.9%에 달한다. 만성질환 종류별로는 고혈압, 당뇨병, 고지혈증, 관절염, 요통/좌골신경통, 골다공증, 백내장, 위십이지장궤양, 협심증/심근경색증, 뇌졸중 등 심뇌혈관계, 내분비계, 근골격계, 소화기계 등 여러 신체 기관에 걸친 유병상태를 보인다. 복합이환 상태는 장애를 유발하며, 장애의 전 단계인 노쇠의 위험을 높인다. 복합이환은 장애에 직접적인 영향을 미치기도 하지만, 누적된 건강결핍으로 노쇠를 유발하고 이로 인해 간접적으로 장애의 위험을 높인다. 이와 같이 복합이환과 노쇠는 서로 밀접한 관련이 있기 때문에 장애 예방을 위해 복합이환과 노쇠의 관리는 매우 중요하다. 우리나라 인구는 급속한 고령화를 보이고 있어 노년층의 증가와 이에 따른 노년기 복합이환과 노쇠의 관리가 중요한 과제로 대두되고 있다.
이 연구의 목적은 우리나라 노년기 복합질환관리 통합의료체계 구축을 위한 연구 설계 및 정책 방향을 제시하는 데 있다. 구체적인 목적은 (1) 노인의 복합만성질환 관련 요인 및 중재 파악, (2) 노년기 복합이환 실태분석, (3) 국내외 복합이환 관리 정책·제도 현황분석, (4) 노쇠 현황분석 및 가이드라인에 근거한 정책/사업화 모델 및 실행전략 우선순위 제안, (5) 노년 복합이환 통합관리 모형 개발을 위한 연구 및 정책 로드맵 마련이다.
▢ 연구방법
Ⅰ. 문헌고찰
복합이환의 관련 요인 및 중재에 관한 국내외 연구에 대하여 기존의 체계적 문헌고찰을 이용한 신속 문헌고찰을 실시하였다. 복합이환의 관련요인과 중재에 대한 문헌고찰 결과를 토대로 노년기 복합이환 연구의 주요 영역 및 주제를 발굴하였다.
Ⅱ. 노년기 복합이환 실태분석
자료분석은 국민건강보험공단 노인코호트 DB(2002-2015)를 활용하였다. 건강보험 청구자료의 진료내역에서 분류되는 상병코드를 주상병으로 진단받았으며, 이에 대하여 입원 1일, 외래 3일 이상일 경우를 만성질환으로 정의하고, 이러한 만성질환을 3개 이상 보유하고 있을 경우를 복합만성질환으로 정의하였다. 노인 복합만성질환자에서 성별, 연령, 보험종별에 따른 복합만성질환 현황을 파악하고 다빈도 만성질환 패턴을 분석하였다. 의료이용 실태는 외래, 입원, 내원일수를 분석하였다. 복합만성질환군에서 입·내원일수 및 의료이용비에 따라 군집분석을 시행하되, 만성질환의 중증도를 고려하기 위해 외래 및 입원이용 각각에 대한 군집분석을 시행하였다. 복합이환의 장애 발생과 사망 위험도를 분석하였다.
III. 정책 및 제도 현황분석
국내의 주요 노인보건복지사업과 만성질환관리사업의 최신 자료와 핵심 관계자와의 심층면담을 통해 각 사업의 노년기 복합질환 문제에 대한 인식, 접근법, 한계, 시사점 등을 종합적으로 분석하였다. 또한 주요 국가의 정책과 제도, 관리도구 등을 종합적으로 고찰 및 분석하고 국내 시사점을 도출하였다.
IV. 노쇠의 현황분석 및 근거기반 정책/사업화 모델 및 실행전략 및 정책 제안
국내 노쇠 연구에 있어 관련 요인, 진단 및 중재, 예후에 관한 문헌고찰을 통하여 노쇠 관련 연구의 근거를 종합하였다. 주요 국내 노쇠 관련 연구, 시범사업과 정책을 파악하고, 임상진료지침 등을 조사하였다. 세계보건기구의 노인을 위한 통합관리 모델(Integrated Care for Older People: ICOPE)의 실행전략 틀을 이용하여 국내 노쇠 관련정책에 대한 현황을 파악하기 위해 전문가 델파이조사를 실시하였다. 이를 바탕으로 근거기반의 노쇠관리 정책 방안을 제안하였다.
V. 노년 복합이환 통합관리 로드맵 개발
국내 연구와 제도의 약점과 한계, 보충이 필요한 연구영역과 개발내용, 국내외에서 제시된 발전방안을 토대로 단계적인 연구계획 및 정책설계 계획을 수립하였다. 로드맵 개발 시 관련 이해관계자(임상 및 정책전문가 포함)로 구성된 자문위원회 의견을 수렴하였다. 로드맵 개발과 정책 설계 마련을 위해 Sustainable intEgrated care modeLs for multimorbidity: delivery, FInancing and performancE (SELFIE) 모델을 분석틀로 활용하였다. 이를 바탕으로 시범사업과 연구과제를 제시하였다.
▢ 노년기 복합이환의 문헌고찰 및 실태
Ⅰ. 복합이환의 위험요인 및 중재에 관한 문헌고찰
기존 체계적 문헌고찰을 이용한 신속 문헌고찰 방법을 통해 노인 복합이환의 위험요인에 관한 체계적 문헌고찰 문헌 5편, 중재법에 관한 체계적 문헌고찰 문헌 7편을 선정하였다. 복합이환 발생에 위험을 주는 요인으로는 교육수준, 소득 수준, 신체적 노쇠 등이었으며, 사회적 노쇠의 경우 복합이환 노출에 관하여 양의 연관성을 보였다. 간호사 사례관리 중재의 경우 재입원율에 유의미한 감소를 보였다. 건강관리에 대한 의사결정에 복합이환 노인을 참여시키는 중재의 효과성의 경우 환자 워크샵 및 개별 코칭, 인지-행동치료, 실무자 교육, 조직적 중재, 전문가-환자 공동 목표 설정이 긍정적인 결과를 보였다. 관리 조정, 자가관리 지원을 포함한 복합중재가 제시되었으며, 복합중재의 우울, 복약순응도, 건강행위 개선 효과가 보고되었으나, 연구 수의 부족과 이질성으로 인해 근거 합성에 제한이 있었다.
Ⅱ. 노년기 복합이환 실태
노인의 복합만성질환의 특성과 패턴을 분석하기 위해 국민건강보험공단 노인코호트 DB를 활용하였다. 65세 인구가 포함된 2007년을 기준으로 자격이 유지된(사망 제외) 대상자 47만 904명을 연구대상자로 포함하였다. 3개 이상의 만성질환을 지닌 복합만성질환의 유병률은 20.2%이었다. 복합만성질환자의 경우 여성과 의료급여 수급자의 분율이 비만성질환자나 만성질환자 군에 비해 높았으며, 연령군별로는 복합만성질환자의 분율이 60대에서 가장 낮고, 70대에서 가장 높았다. 다빈도 복합만성질환으로는 고혈압, 고도시력감퇴, 당뇨병, 만성요통, 무릎관절증 등을 보였다. 복합만성질환자는 비복합만성질환자에 비해 외래, 입원 의료비 지출과 입·내원일수가 더 많았다. 외래이용 환자의 내원일수와 외래 의료이용비로 군집분석을 시행한 결과 의료비 지출이 많은 군집일수록 중증도가 더 높은 만성질환을 보유하고 있었다. 위험도분석 결과 연령을 보정하더라도 복합만성질환이 있는 경우 장애발생 및 사망의 위험이 통계적으로 유의하게 높았다.
▢ 국내외 복합이환 관리사업 및 제도
국내 주요 만성질환 관리사업들의 목적이나 대상자가 유사하지만, 사업별로 참여 의료기관·지원센터 등을 별도로 운영하고 있으며, 지원 내용과 정책적 수단 등이 달라 포괄적으로 사업을 수행하기 어려운 한계점을 가지고 있다. 복합만성질환자에 대한 통합 관리 모형이 부재하고, 만성질환 예방보다 치료중심의 의료서비스가 제공되고 있으며, 지원 서비스 질 관리가 미흡하다. 따라서 대상자 선정기준 및 질환 확대, 임상전문가(영양사, 사회복지사 등) 인력 활용 방안 마련, 통합관리전산시스템 마련(자료 연계), 지역사회 자원(보건소)과 일차의료기관과 협력이 필요하다.
국외 복합이환 관리 사례를 통해 효과적인 프로그램/정책/사업의 고유한 특성을 파악할 수 있으며, 복합이환 환자 관리는 다차원적 돌봄 욕구를 반영해야 함을 시사해 준다. 복합이환 통합관리 모형은 양질의 복합이환 서비스를 전달하기 위해 효율적으로 작동하는 시스템을 제시하며, 복합이환 환자 돌봄/관리가 어떤 방식으로 수행되어야 하는지에 대한 전반적인 구조 및 방향성을 보여준다. 복합이환 통합관리 모형은 복합이환 환자 치료 및 관리와 관련된 주요 쟁점을 파악ㆍ분석하고 이를 체계적으로 정리하여 효과적인 복합이환 대응 전략 및 권고안을 제시하여야 한다.
▢ 노쇠 연구 및 관리 현황과 정책 방향
I. 국내 노쇠 관련 연구 현황
국내에서 진행된 노쇠 관련 연구 현황을 살펴보고 노쇠 연구 현황과 노쇠와 관련된 요인을 파악함으로써 향후 이와 관련된 연구 분야를 탐색하기 위한 서술적 조사연구를 실시하였다. 본 연구에서는 최종적으로 45건을 연구 대상 문헌으로 포함하여 연구 기준에 따라 분류하였다. 이들 문헌은 노쇠 관련 요인의 상관성 또는 건강결과에 대한 영향 연구와 노쇠 예방 중재프로그램에 대한 효과를 평가한 연구로 구분되었다. 노쇠는 여성, 고령, 만성질환, 낙상, 낮은 일상활동, 의료이용, 우울, 인지기능 저하 등과 유의한 연관성을 보였다. 노쇠는 사망의 위험을 통계적으로 유의하게 증가시켰으며, 삶의 질 지표가 낮아지고 기능장애지수가 높아짐을 보고하였다. 노쇠 예방 중재 프로그램은 운동중재, 복합중재, 낙상 관련 중재, 기타 중재로 구분되었으며, 대부분의 문헌에서는 주로 신체능력 및 건강 요인에 대한 결과를 제시하였다.
지금까지 노쇠에 관련한 연구는 노쇠 그 자체의 극복이나 복합적인 결과를 파악하기 보다는 신체 능력의 회복이나 건강요인의 긍정적인 변화, 정신 인지 요인 등의 단편적인 부분의 개선을 살펴보는 연구에 집중되어 있다. 노쇠 예방을 위한 효과적인 중재요법의 근거는 아직 불충분한 실정이다. 노쇠 예방을 위한 근거창출을 위해서는 코호트 연구와 임상중재 연구의 활성화, 지역사회 및 임상적 예방 프로그램 및 임상지침 개발, 노쇠 예방 정책 및 제도 수립 등 다각적인 중재가 필요하다.
II. 노쇠 관련 사업/정책 분석
세계보건기구는 건강한 노화 전략 및 액션 플랜에서 2020년부터 2030년까지 건강 노화를 위한 국제적 파트너십 및 근거 수립과 더불어 모든 사람에게 기능적 능력을 최대화하기 위한 근거기반의 정책 방향을 제시하였다. 이를 위해 노인 통합관리 서비스 제공 모델(ICOPE)을 개발하였으며, 시스템과 서비스 수준의 실행방안을 제시하였다.
전문가 델파이 조사를 통해 WHO-ICOPE 틀을 활용하여 국내에서 실시되어 온 노인 대상의 다양한 사업 및 정책에 대한 전문가의 의견을 수렴한 결과 전문가와 사업담당자 모두 시스템 영역보다 서비스 영역에 대한 문항의 구현이 더 잘되어 있다고 응답하였다. 세부 문항별로 살펴보면 서비스 영역에서는 “다학제 차원에서 공급자별 제공되는 서비스 조정ㆍ지원” 중 ‘돌봄을 필요로 하는 노인을 찾기 위한 적극적인 노력 실시’와 ‘지역사회 기반 서비스 및 인력을 활용한 치료(care) 서비스를 제공함’ 두 개 항목에서 적정 수준의 합의가 도출되었다.
III. 노쇠 통합관리 정책 방향
지역사회 거주 노인에서 노쇠를 조기에 발견하여 예방 또는 악화를 방지하는 것은 노인 개개인의 기능 저하를 예방하고 그로 인해 독립적인 삶을 가급적 오래 유지할 수 있게 함으로써 지역사회의 부양 부담 또한 덜어줄 수 있게 하는 방법이다. 노쇠는 영국의 사례에서 보듯이 복합이환과 같이 설계되어 국가가 관리할 필요가 있다. 노인 대상의 통합관리서비스 제공에서 노쇠와 복합이환자를 함께 국가적 관리가 필요한 대상으로 정하고 질환관리 정책과 보건의료제도적 관점에서 통합적으로 접근할 필요성을 제안한다. 이를 위해서는 미흡한 노쇠 연구와 정책 개발에 대한 지원이 필요하며, 국가 가이드라인을 개발적용할 필요가 있다.
▢ 결론 및 정책 제언
Ⅰ. 노인 복합이환 통합관리를 위한 로드맵
노인 복합이환 통합의료의 목표는 (1) 개인과 인구집단 건강 결과 향상, (2) 환자중심성 및 환자경험 개선, (3) 지속가능한 의료비 지출구조로의 변화이다. 로드맵은 SELFIE 프로그램을 기반으로 하여 핵심 추진 항목들을 (1) 보건의료체계 6대 영역(서비스 공급, 관리/경영구조, 인력, 재정, 의료기술/의약품, 정보 및 연구)와 (2) 진료부문/조직부문/정책부문의 구분 틀로 제시하였다.
Ⅱ. 시범사업 및 후속 연구과제
구체적인 시범사업으로는 장기요양 건강주치의, 재택의료센터, 병원 입원자 대상의 복합이환 노인환자의 진료 질 향상을 위한 급성기병원 노인병센터(노인병클리닉) 운영, 지역사회 거주 노인 대상의 장기요양 등급외 노인 중 복합이환 노인환자의 노인주치의 적용 등을 제안한다.

▢ Introduction
Among people aged 65 years or older, 54.9% have multiple chronic conditions. Multimorbidity includes chronic conditions affecting cardiovascular, endocrine, musculoskeletal, and gastrointestinal systems, such as hypertension, type 2 diabetes, hyperlipidemia, arthritis, back pain/sciatica, osteoporosis, cataracts, gastro-duodenal ulcers, angina/myocardial infarction, and stroke. Multimorbidity increases the risk of frailty, contributing to disability onset. Although multimorbidity directly influences disability, it also indirectly increases the risk of disability through the pathway of frailty. Because multimorbidity and frailty are closely related, managing both conditions is critical in preventing disability. With the rapid increase in the aging of the population, there has been an increasing need for developing policies for the management of multimorbid conditions and frailty in older people.
The study’s aim is to propose research and policy directions for establishing an integrated healthcare system for managing older people with multimorbidity. Specific aims include (1) identifying risk factors of and effective interventions for multimorbid chronic conditions, (2) examining current multimorbidity profiles of older people, (3) reviewing national policies and management strategies for multimorbidity, (4) prioritizing action plans for developing model policies and programs for the management of frailty, and (5) generating a research and policy roadmap for developing an integrated care model for people with multimorbid conditions.
▢ Methods
I. Literature review
A rapid systematic review of risk factors and interventions for multimorbidity was conducted. Systematic review articles were searched and examined to identify the current evidence base and to develop future research agenda on multimorbidity in older people.
II. Analysis of the current status of multimorbidity in older adults
The National Health Insurance Service Senior (NHIS-Senior) claim database (2002-2013) was used in the analysis. Multimorbidity was defined as having three or more chronic conditions, with one inpatient day or three or more outpatient days. Multimorbidity status was analyzed by gender, age, and insurance types. Use of health care was analyzed by outpatient, inpatient, and length of stay. Cluster analysis was performed, taking into account visits/admissions and healthcare costs by outpatient and inpatient service use. The risk of disability and mortality of those with multimorbidity was also analyzed.
III. Analysis of policy and programs
Major programs addressing chronic disease management were reviewed, and opinions of experts were collected to identify the current approaches, limitations, and implications in managing multimorbidity of older adults. The policy frameworks and plans of major countries in addressing multimorbidity were also analyzed. Expert opinions were obtained by a self-administered survey using the SELFIE framework on multimorbidity.
IV. Analysis of policies and programs of frailty management
A literature review of risk factors, diagnostic tools, interventions, and prognosis of frailty was conducted. In addition, studies, demonstrations projects, programs, and clinical practice guidelines regarding frailty management were analyzed. Based on the World Health Organization’s ICOPE framework, a Delphi survey of experts was administered to identify the current situation of frailty management and produce policy recommendations.
V. Development of a roadmap for integrated care of multimorbidity
Based on the current evidence on multimorbidity programs and policies, a research and policy roadmap for managing people with multimorbid chronic conditions was developed. Plans of action were grouped according to the SELFIE framework. Recommendations for demonstration programs and an agenda for research were proposed.
▢ Current evidence and status of multimorbidity
I. Systematic review of evidence
A rapid review of the current systematic reviews on multimorbidity resulted in 5 articles on risk factors and 7 on interventions. Risk factors included education, income, physical frailty, and social frailty. Among the intervention studies, case management by nurses reduced readmission rates significantly. Patient workshop and coaching, cognitive behavioral therapy, education of personnel, organized intervention, and common goal setting by specialists and patients demonstrated positive effects. Care management and self-care support were commonly implemented, with multicomponent interventions exhibiting positive changes for depression, medication adherence, and health behaviors. However, an insufficient number of studies and heterogeneity among the reported studies limited the synthesis of the evidence.
II. Current status of multimorbidity
Among the 470,904 aged 65 years or older in the NHIS-Senior database in 2007, there were 20.2% with three or more chronic conditions. Those with multimorbid conditions tended to be women and insured by Medical Aid. The proportion of multimorbidity tended to be the lowest among those in their 60’s but highest in their 70’s. Chronic conditions commonly observed in the multimorbid group included hypertension, vision loss, chronic back pain, and knee osteoarthritis. People with three or more chronic conditions, compared with those having one or two chronic conditions, had higher healthcare costs and longer admission days. In addition, those in the higher class membership defined by the cluster analysis demonstrated higher disease severity. Multimorbidity was associated with an increased risk of disability and mortality.
▢ Programs and policies for managing multimorbidity
Although many chronic disease management programs exist, they vary in organization, support, and implementation that limit their effective operation. In addition, there is a lack of an integrated care model for multimorbid patients, where the current focus is on treatment rather than prevention, with insufficient support to ensure delivery of quality care. Developing eligibility criteria and expanding coverage of chronic conditions, training and recruiting clinical specialists (e.g., nutritionists, social workers), establishing an electronic health information system for data linkages, and collaboration with community resources (e.g., public health centers) and primary care are warranted.
Programs and policies in other countries indicate the need to develop models that can incorporate the distinctive characteristics of multimorbidity and address the diverse needs of multimorbid patients. An integrated model for managing multimorbidity should be effective and efficient in delivering quality services to multimorbid individuals. In addition, it should identify key problems and issues facing multimorbid management and provide effective strategies and recommendations.
▢ Research and policy directions for the management of frailty
I. Research in frailty
A literature search on frailty in Korea was conducted, resulting in 45 articles. These included studies on the risk factors and outcomes of frailty and the effectiveness of interventions. Frailty was associated with female gender, being older, chronic diseases, falls, low physical activity, healthcare use, depression, and cognitive decline. Frailty significantly increased mortality risk, and lowered quality of life and increased functional disability. Interventions to prevent frailty included programs on physical exercise, multicomponent interventions, and fall prevention, demonstrating effects on physical capacity and health status.
The present frailty interventions report results that demonstrate improvement in individual health dimensions, such as physical capacity, health status, mental and cognitive status, rather than frailty as a whole. The current evidence on the effects of frailty prevention is still limited. It is necessary to build the evidence base by funding cohort studies, clinical intervention studies, and community intervention studies to contribute to policymaking.
II. Analysis of policy and programs on frailty
The World Health Organization (WHO) has put forward the strategic action plan, Decade of Healthy Ageing 2020-2030, that proposes establishing an international partnership and building evidence for achieving healthy aging by maximizing functional ability. The ICOPE has been developed as a model to disseminate integrated care models at the system and service levels.
Based on the WHO-ICOPE framework, an expert Delphi has reported the service dimension being more firmly established than the system dimension. At the service dimension, “actively seek and identify older people in need of care in the community” and “deliver care through a community-based workforce, supported by community-based services” items received a high level of consensus.
III. Policy directions for integrated care of frailty
Early identification of frailty in community-dwelling older adults helps prevent functional decline and maintain independent living. As seen in the United Kingdom, frailty, as with multimorbidity, in late-life needs to be designed as part of the national plan. The government should recognize the need to construct an integrated care system for older people with frailty and multimorbidity. Funding research and development in frailty is essential.
▢ Conclusions and policy agenda
I. Roadmap for integrated care of multimorbidity
The aim of the roadmap is (1) to improve health outcomes of the individual and population, (2) to enhance patient-centeredness and patient experience, and (3) to maintain a structure of sustainable health care financing. Strategic action plans based on the SELFIE framework include (1) 6 components of the healthcare system (service delivery, leadership and governance, workforce, financing, technologies and medical products, information and research), and (2) clinical/organizational/policy settings.
II. Demonstration programs and research proposal
Development and testing of the integrated care model require demonstration projects. A family physician for people eligible for long-term care, a center for medical house calls, a geriatric clinic for acute care hospitals, and a family geriatrician for multimorbid, frail older adults are proposed.
Future research agenda might include:
