보철치료란 치아를 상실했을 경우 인공적으로 치아를 만들어 주는 시술로 임플란트, 브릿지, 틀니 등이 이에 속한다. 치아의 손상 정도에 따라 한두 개의 치아를 상실한 환자들은 임플란트 또는 브릿지 치료를 받을 수 있으며, 치아가 하나도 없는 경우는 고정성 임플란트 보철, 가철성 임플란트 보철, 완전 틀니 중 한 가지를 선택하게 된다. 이들 보철치료는 가격, 수명, 부작용 등이 모두 다르기 때문에 치료 선택시 이에 대한 고려가 필요하다.
본 연구에서는 국내 보철치료 현황을 파악하기 위하여 국민건강영양조사 제4기(2007-2009) 자료를 활용하였다. 보철치료군별 특성을 알아보기 위해 구강검사 결과에 따른 보철물 존재군, 임플란트 존재군, 보철물 필요군의 비중을 확인하였다. 우리나라 인구의 27.2%가 보철물이 존재하는 것으로 나타났고 임플란트 존재군은 2.7%, 보철물 필요군은 12.9%인 것으로 나타났다. 남자보다는 여자가 보철물이나 임플란트 존재비중이 높은 것으로 나타났으며, 보철물 필요의 경우 큰 차이를 보이지 않았다. 연령이 증가할수록 보철물이나 임플란트 존재, 보철물 필요비중이 증가하는 것으로 나타났다. 특히, 40-69세의 경우 대상자 중에서 보철물이 존재하는 비중은 46.9%, 임플란트 존재 비중은 4.9%, 보철물이 필요한 경우가 전체의 20.2%인 것으로 나타났다. 저학력, 저소득층 에서 보철물이 존재하는 비중이 상대적으로 높았으나, 임플란트가 존재하는 비중은 학력이 높을수록, 소득이 많을수록 증가하는 것으로 나타났다.
최근 1년 동안 치과진료가 필요하다고 생각하였으나 진료를 받지 못한 적이 있는지를 묻는 질문에서 “예”라고 응답한 경우에 경제적 이유를 진료를 받지 못한 가장 주된 이유로 꼽았다. 그 다음으로 “직장이나 학교를 비울 수 없어서”, “다른 문제들에 비해서 덜 중요하다고 느껴서”, “치과진료를 받기가 무서워서”라는 응답이 뒤를 이었다.
구강검진 전 면접을 통해 현재 본인이 치과진료가 필요하다고 생각하는지를 확인한 결과, 전체의 58.5%가 필요하다고 응답하였으며, 구체적으로 해당되는 진료를 모두 응답하라는 문항에서는 충치치료, 잇몸병치료, 보철치료 순으로 나타났다.
보철치료 현황 및 만족도, 구강건강 관련 삶의 질, 치료비용, 보철치료에 대한 미충족 의료 등을 파악하기 위해 일반인과 보철치료를 받은 환자를 대상으로 설문조사를 실시하였다. 먼저 일반인 대상 설문조사의 경우 보철치료 경험 여부와는 무관하게 연구대상자를 선정하였고, 만 40-69세의 성인 남녀 152명을 대상으로 하였다. 응답자 152명 중 27.63%에서 보철치료 경험이 있는 것으로 조사되었으며 보철치료를 한 환자 중 브릿지 치료가 52.38%로 가장 많았고 다음으로 단일치아 임플란트 보철치료 23.81%, 부분 틀니가 16.37%를 차지하였다. 또한 보철치료에 대한 미충족 의료 경험률을 파악하기 위해 “스스로 보철치료가 필요하다고 생각하거나 의사로부터 보철치료가 필요하다고 진단받았음에도 불구하고 여러 가지 요인으로 인해 제 때에 적절한 치료를 받지 못한 경우”를 미충족 의료로 정의하여 분석하였다. 그 결과 보철치료에 대해 미충족 의료를 경험한 경우는 39.62%로 13.21%는 치료를 뒤늦게 받았고 24.5%에서는 아직까지 치료를 받지 않고 있는 것으로 조사되었다. 보철치료에 대한 미충족 의료에 영향을 주는 요인을 파악하기 위해 인구사회학적 요인, 의료서비스에 대한 필요 요인, 건강행동 요인으로 변수를 구분하여 로지스틱 회귀분석을 실시하였다. 그 결과 인구사회학적 요인에서 가구소득과 필요요인 중 구강질환 유무 변수만이 보철치료에 대한 미충족 의료에 통계적으로 유의한 영향을 미치는 것으로 나타났다.
보철치료군 설문조사는 설문시점에서 해당 보철치료를 완료한 이후 최소 6개월이 경과한 만 40-69세의 성인 남녀를 대상으로 하였다. 대상군은 크게 단일치아 상실군과 완전 무치악군으로 구분된다. 단일치아 상실군은 임플란트 보철치료군과 브릿지 치료군으로 나누고 완전 무치악군은 고정성 임플란트 보철군, 가철성 임플란트 보철군, 완전 틀니군으로 구분하였다. 단일치아 상실군은 임플란트 보철 31명, 브릿지 32명으로 총 63명을 조사하였고, 완전 무치악군은 고정성 임플란트 보철 29명, 가철성 임플란트 보철 27명, 완전 틀니 30명으로 총 86명에서 조사가 완료되었다. 본 연구에서는 여러 보철치료를 받은 경험이 있는 경우 가장 최근에 받은 보철치료를 기준으로 설문 대상군을 정의하였으며 보철치료부위, 보철물 재료, 정기적인 치과검진 여부 및 관리 비용, 부작용 치료 경험 및 치료 실패로 인한 재시술 경험 등을 조사하였다. 보철치료에 대한 만족도는 단일치아 상실군의 경우 임플란트 보철과 브릿지 두 군에서 통계적으로 유의한 차이를 보이지 않았다. 완전 무치악군에서는 저작 기능, 사회적 기능, 전반적인 만족도 모두 고정성 임플란트 보철군에서의 만족도가 가장 높게 나타났으며 가철성 임플란트 보철군이 그 다음이었고, 완전 틀니군의 만족도가 가장 낮은 것으로 분석되었으며 세 군에서 유의한 차이를 보였다. 보철치료 후에 불만족한 부분에 대해서는 임플란트 치료인 단일치아 임플란트 보철, 고정성 임플란트 보철, 가철성 임플란트 보철은 “비용이 비싸서”가 가장 높은 비율을 차지한 반면, 브릿지의 경우 “건강한 치아를 손상시켜야 해서” 불만족하는 경우가 가장 많았고 완전 틀니는 “틀니 사용이 불편해서”라는 응답이 가장 많았다.
한편, 구강건강영향지수(Oral health impact profile-14, OHIP-14)를 이용하여 구강건강 관련 삶의 질을 측정한 결과 5가지 보철치료 모두 각 영역 및 총점에서 기능적 제한, 신체적 동통, 심리적 불편, 신체적 능력저하, 심리적 능력저하, 사회적 능력 저하, 사회적 불리를 포함한 7가지 영역 및 OHIP-14 총점에서 보철치료 전에 비해 치료 후 구강내의 문제로 인해 받는 삶의 질 제한이 통계적으로 유의하게 감소된 것으로 나타나 보철치료 후에 구강건강 관련 삶의 질이 개선되었음을 확인할 수 있었다. 단일치아 상실군에서의 보철치료 전, 후의 구강건강 관련 삶의 질 개선 정도는 임플란트 보철과 브릿지 간에 통계적으로 유의한 차이를 보이지 않았다. 완전 무치악군에서는 삶의 질 개선 정도가 고정성 임플란트 보철, 가철성 임플란트 보철, 완전 틀니 순이었으나 고정성 임플란트 보철과 가철성 임플란트 보철군과의 비교에서 통계적으로 유의한 차이를 보이지 않았다. 완전 틀니와 비교해서는 고정성 임플란트 보철 및 가철성 임플란트 보철치료에서 통계적으로 유의하게 구강건강 관련 삶의 질 개선 정도가 더 큰 것으로 나타났다
보철치료비용에 대해 조사한 결과 단일치아 임플란트 보철 1개당 치료비용의 중앙값은 치과의원 179만원, 치과병원 300만원이었고, 브릿지 비용은 치과의원에서는 115만원, 치과병원은 200만원으로 병원에서의 치료비용이 통계적으로 유의하게 더 높았다. 완전 무치악군의 경우 하악 전체에 고정성 임플란트 보철치료를 받을 때의 치료비용의 중앙값은 치과의원 1,150만원, 치과병원 2,450만원으로 병원의 치료비용이 통계적으로 유의하게 더 높았다. 반면 상악 전체와 상 ․ 하악 전체 치아에서는 치료기관 종별 차이가 통계적으로 유의하지 않았고, 상악/하악/상 ․ 하악 전체 치아 간 치료부위별 비용차이도 유의하지 않았다. 가철성 임플란트 보철치료비용이 고정성 임플란트 보철치료보다 저렴하였으나, 치료기관 종별, 치료부위별로 통계적으로 유의한 차이를 보이지 않았다. 상악 또는 하악 전체에서 완전 틀니의 병원치료를 받은 대상자는 없었고 치과의원에서는 상악 전체 196만원, 하악 전체 153만원이었으며 상 ․ 하악 전체 치아는 치과의원 398만원, 치과병원 350만원으로 조사되었다. 완전 틀니 비용의 경우 치료기관 종별에 따른 차이는 통계적으로 유의하지 않았지만 치료부위에 따라서는 유의한 차이를 보이는 것으로 분석되었다.
각 보철치료들의 생존율과 치료에 따른 삶의 질을 파악하기 위해 체계적 문헌고찰을 수행하였다. 국내 검색 DB로는 KoreaMed, KMbase, KISS를, 국외 검색 DB로는 Ovid-medline, Embase, Cochrane을 이용하였다. 국내 DB에서 7,287건, 국외 DB에서 21,699건의 문헌이 검색되었으며 1차, 2차 선택배제(screening) 과정을 거쳤다. 단일 치아 상실군에 대한 문헌은 국내 21건(임플란트 19건, 브릿지 2건), 국외 50건(임플란트 41건, 브릿지 9건)이었으며 두 보철물을 직접 비교한 문헌은 없었다. 단일치아 임플란트 보철과 브릿지는 직접 비교한 문헌이 없어 각각에 대한 생존율을 메타분석한 결과 추적관찰기간에 상관없이 상악에 식립한 임플란트의 생존율은 94.4%, 하악은 97.0%로 하악이 상악보다 높았다. 4-6년 생존율은 상악 90.7%, 하악 96.8%였다. 브릿지의 경우 생존율을 보고한 문헌의 수가 적어 상악과 하악을 구분하여 생존율을 분석하지 않았다. 브릿지의 4-6년 생존율은 84.2%로 단일치아 임플란트 보철보다 생존율이 낮았다. 그러나 본 메타분석 결과는 두 보철치료를 직접 비교한 문헌에 대한 분석 결과가 아니기 때문에 해석에 주의가 필요하다.
완전 무치악군에 대한 문헌은 국내 7건(고정성 임플란트 보철 1건, 가철성 임플란트 보철 1건, 완전 틀니 6건), 국외 68건(고정성 임플란트 보철 21건, 가철성 임플란트 보철 51건, 완전 틀니 17건)이었다. 이 중 완전 무치악군이 선택할 수 있는 세 가지 중재를 모두 동시에 비교한 문헌은 없었다. 세 가지 중재 중 두 가지를 직접 비교한 문헌은 총 22건이었으며 이 중 5건은 고정성 임플란트 보철과 가철성 임플란트 보철의 생존 또는 실패를 비교, 1건은 고정성 임플란트 보철과 가철성 임플란트 보철의 합병증에 대한 연구였고 16건은 삶의 질 및 만족도를 비교하는 문헌(고정성 임플란트 보철과 가철성 임플란트 보철의 비교 3건, 가철성 임플란트 보철과 완전 틀니의 비교 12건, 고정성 임플란트 보철과 완전 틀니의 비교 1건)이었다. 직접비교연구 4건에 대하여 고정성 임플란트 보철과 가철성 임플란트 보철 각각의 생존율을 메타분석한 결과 랜덤효과모형에서 추정된 고정성 임플란트 보철의 생존율은 97.9%, 가철성 임플란트 보철의 생존율은 92.5%로 나타났다. 직접 비교한 문헌의 수가 적어 임플란트 식립 위치별, 추적관찰기간별 분석은 수행하지 않았다. 다음으로 비교연구와 단일군 시험 연구(single arm study)의 결과를 모두 포함하여 메타분석하였다. 고정성 임플란트 보철의 경우 상하악에 식립한 임플란트의 4-6년의 생존율은 98.4%였다. 고정성 임플란트 보철의 생존율을 보고한 문헌의 수가 적어 추적관찰기간별로 분석시에는 임플란트 식립 위치별 생존율을 고려할 수 없었다. 가철성 임플란트 보철의 4-6년 생존율은 상하악 전체 94.9%, 상악 90.7%, 하악 97.2%로 식립 위치별로 생존율에 차이가 있었으며 단일치아 임플란트 보철과 마찬가지로 하악에서의 생존율이 더 높았다. 고정성 임플란트 보철과 가철성 임플란트 보철을 비교하였을 때 고정성 임플란트 보철의 생존율이 높게 나타났다. 그러나 위의 결과는 각각의 보철물에 대한 단일군 시험 연구(single arm study)의 생존율까지 포함하여 수행한 메타분석 결과이기 때문에 해석에 주의가 필요하다.
단일치아 상실 환자에서 임플란트 보철과 브릿지 치료간의 경제성을 확인하였다. 분석기간은 5년과 10년이었으며 효과는 생존율, 비용은 의료비용, 교통비용, 시간비용을 포함하였다. 임플란트 보철과 브릿지 치료의 생존율을 직접 비교한 문헌이 없었다. 따라서 본 연구에서는 각각의 단일군 시험 연구(single arm study)들에 대한 메타 분석결과를 사용하였다. 메타 분석 결과 임플란트 보철치료의 4-6년 생존율은 94.5%(95% CI 92.5-96.5%), 7-10년 생존율은 91.7%(95% CI 85.9-97.4%)였음. 브릿지의 경우 4-6년 생존율은 84.2%(95% CI 69.7-98.6%)였으며 7-10년은 81.3%(95% CI 71%-89.1%)로 나타났다.
의료비용에는 시술비용과 정기적 관리비용, 합병증 비용을 고려하였다. 시술비용의 경우 환자 조사 자료를 이용하였으며 정기적 관리비용 및 합병증 비용은 임상 전문의를 대상으로 조사하여 도출된 값을 사용하였다. 교통비용은 임플란트 보철치료와 브릿지 치료의 스케줄 및 정기적 관리 스케줄을 조사하여 의료기관 방문 횟수를 산출하였으며 단위 비용은 2008년 의료패널 조사 자료에서 보고한 치과의원/치과병원 편도 교통비를 사용하였다. 시간비용의 경우 각 의료기관 방문 스케줄과 연계하여 총 소요시간을 산출하고 시간당 임금 및 고용율을 고려하여 분석하였다.
경제성분석 결과 분석기간이 5년일 때 두 보철치료의 효과 차이는 10.3%, 비용의 차이는 367,000원(의원)-485,000원(병원)으로 단일치아 보철 한 개를 5년간 보존하는데 임플란트가 3,560,000원(의원)-4,711,000원(병원)의 비용이 추가로 소요되는 것으로 나타났다. 분석기간을 10년으로 연장할 경우 효과의 차이는 10.4%, 비용의 차이는 330,000원(의원)-415,000원(병원)으로 임플란트와 브릿지의 비용 차이가 더 적어졌다. 그러나 경제성 분석을 수행함에 있어 효과지표(생존율)에 대한 근거의 질이 낮고 삶의 질, 환자 만족도 등 치료의 다각적인 측면을 충분히 고려하지 못하였다.
본 연구는 다음과 같은 한계를 갖는다. 첫째, 본 연구에서는 환자의 동질성 확보를 위해 분석 대상 환자군을 제한하였다. 따라서 가장 많은 비중을 차지하는 부분 무치악 환자들의 경우는 고려하지 못하였다. 그러나 부분 무치악 환자에서도 임플란트 보철치료와 브릿지 등이 이루어지고 있으므로 이들을 비교하는 부분은 부분 무치악 보철치료에도 의미있다고 판단된다. 둘째, 단일치아 임플란트 보철치료와 브릿지, 고정성 임플란트 보철과 가철성 임플란트 보철, 완전 틀니에 대한 직접 비교 임상 연구가 거의 없었다. 따라서 각 보철치료의 임상적 효과의 우월성을 확인하는데 제한적이었다. 셋째, 본 연구에서는 분석기간을 5년과 10년으로 하여 경제성 분석을 수행하였다. 그러나 자료의 한계로 보철치료 실패 후 재치료에 따른 생존율을 고려하지 못하였으며 합병증 비율 및 치료비용을 적용하는데 제한적이었다. 넷째, 실제 임플란트 보철치료의 주요 장점으로 알려져 있는 인접 치아를 손상시키지 않고 주변 치아 상태에 관계없이 치료가 가능한 점, 심미적인 장점 등을 경제성 분석의 효과에 반영하지 못하였다. 다섯째, 비용 및 만족도 조사에 있어 각 군의 환자수를 많이 배정하지 못하였다. 따라서 자료의 대표성의 문제가 발생한다. 또한 환자를 대상으로 한 설문조사였기 때문에 회상 바이어스에 의해 영향을 받을 수 있다. 따라서 본 연구에서는 관련 임상의에게 가격에 대한 타당성을 검증받고 다양한 민감도 분석 등을 통하여 이를 극복하고자 하였다.
치아상실 환자는 보철 생존율, 만족도, 합병증, 치료비용 및 치료기간 등을 고려하여 자신의 구강상태에 맞는 치과 보철치료를 선택하는 것이 필요하다. 그러나 비교 연구가 거의 없어 근거 수준이 높지 않고 관련 자료가 부족하여 종합적인 결론을 내리는 데 한계가 있다. 본 연구는 이러한 제한된 환경에서 접근 가능한 자료원과 분석 방법을 사용하여 다양한 측면에서 여러 정보를 제공하고자 하였다. 본 연구가 치과 보철치료를 합리적으로 선택하는데 있어 관련 근거를 제공하는 밑거름이 되기를 기대한다.
□ Prosthetic treatment status in Korea
This study was performed to investigate the current state of dental prosthetic treatment (fixed prostheses, removable prostheses, implant prostheses) and unmet needs for prosthetic treatment in Korea.
The data from the Fourth Korea National Health and Nutrition Examination Survey (KNHANES Ⅳ 2007-2009) was utilized. To determine the characteristics of various prosthetic treatments, the proportion of people with the conventional prosthetic treatment and implants, as well as the demanding prosthetic treatment was estimated through the results of oral examination.
It was found that the 27.2% of the Korean population experienced prosthetic treatment at least once, 2.7% had implants, and 12.9% were in need of further prosthetic treatment. The proportions of those who have any kinds of prostheses or implants were higher in women compared to men, and the demand for prosthetic treatment increases with age. Specifically, in the subjects aged 40-69 years, 46.9% experienced prosthetic treatment, 4.9% had dental implants and the need for prosthetic treatment was 20.2%. While the conventional prosthetic treatment ratio was found relatively higher in the poorly educated, low-income socioeconomic classes, the survey showed those who highly educated and have high income tend to have more implants. The main causes of unmet needs were related to economic reasons and it comprised 58.5%.
The proportion of people with prosthetic treatment is increasing in Korea however the unmet need of prosthetic treatment is still high due to economic reasons.
□ A survey with general people and patients
A survey was conducted to the general people and to patients that had undergone prosthetic treatment, to examine satisfaction with each dental prosthesis, quality of life related to oral health, treatment costs and unmet medical need for prosthetic treatment. 27.63% of the 152 people who participated in the survey were found to have had prior prosthetic treatment. Within the group with prior prosthetic treatment, those who had bridges were found to be the highest (52.38%), followed by single-tooth implants (23.81%), and partial dentures (16.37%). Additionally, unmet needs rate for prosthetic treatment was 39.62%, in which 13.21% were treated later than necessary, and 24.5% had not yet received treatment. To identify the factors that influence the unmet need for prosthetic treatment, demographic and social factors, treatment necessity factors, and health behavior factors were selected as variables for a logistic regression analysis. The results showed that the family income as well as the presence of oral disease had statistical significance in the unmet need for prosthetic treatment. A survey for patients with dental prosthesis included patients
who, between ages 40-69, have elapsed a minimum of 6 month period after their prosthetic treatment. Patients with single missing tooth were categorized into those with a dental implant and those with a bridge. Edentulous patients were dividing into three categories - implant-supported fixed prostheses, implant supported removable prostheses, and complete dentures. Patients with single missing tooth were comprised of 31 patients who had received dental implants, and 32 patients who had received the bridge treatment, for a total of 63 patients. Edentulous patients included 29 with implant-supported fixed prostheses, 27 with implant-supported removable prostheses, and 30 with complete dentures, for a total of 86 patients. The satisfaction difference with prosthetic treatment for single tooth loss was not statistically significant. For edentulous patients, the satisfaction with prosthetic treatment was the highest in those with implant-supported fixed prostheses, followed by implant-supported removable prostheses and then completes dentures. The major reason of dissatisfaction in dental implants, implant-supported fixed prostheses, and implant-supported removable prostheses was the high costs, while in bridges, most of respondents expressed dissatisfaction with damaging healthy teeth for treatment. Those who were treated with complete dentures mentioned discomfort of the dentures as the highest source of dissatisfaction (83.3%).
Additionally, the quality of life related to oral health was measured using the Oral health impact profile-14 (OHIP-14). The oral health quality of life was significantly increased after prosthetic treatment for all the sub-scales of OHIP-14. For patients with single tooth loss, difference between bridge treatment and implants on oral health related quality of life was not statistically significant. Edentulous patients showed only a minimal difference between implant-supported fixed prostheses and implant-supported removable prostheses, but complete dentures showed significant differences from the other two types of treatments.
Analysis of treatment costs showed that the median cost for a single tooth implant was 1,790,000 Korean won in clinics, and 3,000,000 Korean won in hospitals. Median cost for bridges were 1,150,000 Korean won in clinics and 2,000,000 Korean won in hospital, revealing higher hospital costs to be statistically significant. In the case of edentulous patients, the median cost for implant-supported fixed prostheses in the lower jaw was 11,500,000 Korean won in clinics and 24,500,000 Korean won in hospitals, showing statistically higher costs in hospitals. Costs for implant-supported
removable prostheses were found to be about 30% lower than fixed implants, ranging about 7,000,000 Korean won in clinics and 15, 000,000 in hospitals. Dentures of upper or lower jaw separately were about 2,000,000 Korean won, and about 4,000,000 Korean won for both jaws. For costs regarding complete dentures, analysis showed significant differences depending on the location of dental prosthesis.
□ A systematic review of the 5-10 year survival rates with implant and bridge (3-unit FPD) in patients with single tooth loss
Patients with single tooth loss can be rehabilitated with implant prosthesis or bridge. Systematic review was performed to determine the survival rate of these dental prosthetic treatments and the change in quality of life according to treatment option.
The electronic search was conducted to identify randomized clinical trials, prospective and retrospective cohort studies on survival rates with implant prosthesis or bridge. The databases were searched from Ovid-Medline, Cochrane library, EMBASE, and Korean DB with publication year limit from 1995 to 2011. The full texts to meet the inclusion criteria or to make a unclear decision, were obtained. Quality assessment of the identified studies and data extraction was performed independently by two reviewers. Survival rates were analyzed using random effect model to obtain summary estimates of 5 - 10 year survival rates.
The search provided 18,967 titles and 514 abstracts. Full text analysis of 21 Korean and 50 abroad studies regarding patients with single tooth loss were performed. There were no studies directly comparing the two prostheses. The result of meta-analysis indicated an estimated survival rate of implant prosthesis is 94.5% after 5 year of loading and 91.7% after 10 year of loading. The survival rate of bridge is 84.2% after 5 year of function and 81.3% after 10 year of function. Two studies meeting the inclusion criteria on the subject of quality of life were identified. However it cannot be synthesized.
Comparing the 5 year survival rate for implant prosthesis and bridge, the survival rate of implant prosthesis might be higher than that of bridge in patient with single tooth loss. However, there was no comparative study to determine the relative effectiveness of two prosthesis treatment options.
□ A systematic review of the survival rates and quality of life of implant-supported fixed prostheses and implant-supported removable prostheses
The purpose of this systematic review was to determine the differences of survival rates and quality of life among implant-supported fixed prostheses, implant-supported removable prostheses and complete dentures for totally edentulous patients.
Electronic searches were conducted in Ovid-Medline, Cochrane and Ovid-EMBASE to identify studies on implant-supported fixed prostheses, implant-supported removable prostheses and complete dentures for totally edentulous patients. Survival rates were calculated for implant-supported fixed prostheses and implant-supported removable prostheses using random effect model to obtain summary estimates.
Electronic search yielded 22 implant-supported fixed prostheses, 53 implant-supported removable prostheses and 21 complete dentures. There were no studies comparing all three types of treatment concurrently. 17 studies compared 2 of the 3 types of treatments for edentulous patients and 5 of which were reporting the survival or failure rates of both fixed and removable implant-supported prostheses. The remaining 12 studies out of 17 comparative studies reported the quality of life. The result of the meta-analysis of the risk ratio of fixed implant-supported prostheses and removable implant-supported prostheses was 0.36. Meta analysis also revealed that the survival rate of fixed implant-supported prostheses was 97.9% and that of removable implant-supported prostheses was 92.5%. When single arm studies were included in meta-analysis, the survival rate of fixed implant-supported prostheses was 98.4% and that of removable implant-supported prostheses was 94.9% after 4-6 years of loading. There was no study about survival rate of complete dentures.
The survival rate of fixed implant-supported prostheses might be higher than that of removable implant-supported prostheses in totally edentulous patients.
□ Economic evaluation of single tooth replacement: Dental implant vs. bridge
This study was conducted to assess the cost-effectiveness of dental implant compared with bridge in patients with single tooth loss from societal perspective.
The decision model was used with a 5 and 10 year period respectively. To determine the effectiveness of single-tooth implant and bridge, data were extracted from meta-analysis including single-arm studies. To examine current practical data in Korea, medical costs were categorized to initial treatment costs, maintenance costs and complication treatment costs. Patient survey data were utilized to obtain the initial treatment costs of single-tooth implant and bridge. Maintenance costs and complication treatment cost were based on consulting to clinical professionals. Transportation costs were calculated based on the number of follow-up visits for maintenance of implant or bridge. The one-way fares for dental clinics and dental hospitals reported in the 2008 Korea Health Panel were used as unit costs. The total time spent for the treatment was calculated based on the visit schedules, and time costs were calculated using the hourly wage and employment rate.
The results of a 5 year period model showed dental implants cost 367,000 Korean won (clinic setting) to 485,000 Korean won (hospital) more than 3-unit bridge and also had higher survival rates by 10.3%. The incremental cost-effectiveness ratio was 3,560,000 Korean won (clinic) - 4,711,000 Korean won (hospital) per a preserved prosthesis for 5 years. When the period of economic evaluation on this study is extended to 10 years, single-tooth implant had longer survival rates by 10.4% requiring additional cost of 330,000 Korean won (clinic) or 415,000 (hospital) compared to bridge. The longer period examined, the additional cost of single-tooth implant compared to bridge was decreased.
Although the economic evaluation could not take all aspects of into consideration for single-tooth implant or bridges, this study would be useful as one of the evidences choosing the mode of treatment in patients with single tooth loss.
□ Conclusion
The patients with tooth loss should choose their dental prosthesis after careful consideration of survival rates, complications, treatment costs and treatment period of dental prostheses. However, the lack of head-to-head clinical studies which compare the effectiveness of each dental prosthesis and insufficient related evidences make it difficult to draw a comprehensive and definite conclusion. This study was conducted with such limitations, but attempts to provide several pieces of information in various aspects by using accessible resources and methods. This study expects to contribute tomaking a reasonable decision on choosing appropriate dental prostheses.
