▢ 서 론
전 세계적으로 보건의료 재정 효율화의 핵심요소로써 의료기술평가(Health Technology Assessment, 이하 ‘HTA’)의 중요성이 강조되고 있으며, 2014년 제67차 세계보건총회에서는 “보편적 의료보장을 달성하기 위한 보건의료 중재 및 의료기술평가(Health intervention and technology assessment in support of universal health coverage)” 결의안이 채택되어 HTA에 대한 국제적 관심과 수요가 증가하고 있는 추세이다. 이러한 흐름에 부합하여 HTA의 효과적 수행을 도모하고 연구결과 등 관련 정보를 공유하며, HTA의 저변 확산을 위해 HTA 국제협의체 및 관련 기관 간 협력 활동이 확대되고 있다. 특히 유관기관 간 연수교육 프로그램을 제공하거나 국제학회를 개최하여 정보를 공유하고 연구 역량 강화를 도모하고 있으며, 이를 통해 축적된 연구 방법론과 경험을 토대로 개발도상국의 HTA 체계 구축과 연구 수행을 지원하고 있다.
영국과 태국의 HTA 기관인 National Institute for Health and Care Excellence(NICE)와 Health Intervention and Technology Assessment Program(HITAP)은 국제부(International Unit)를 설립하여 국제협력 강화 및 국제보건의료 증진을 위한 노력을 기울이고 있으며, 중·저개발 국가의 보건부와 업무협력협약(Memorandum of Understanding, 이하 ‘MOU’)을 체결하여 보건의료 체계 구축, HTA 수행역량 강화 등을 지원하고 있다. 또한 HTA 유관기관 및 World Bank, World Health Organization(WHO) 등 다양한 국제기구와의 협력관계를 유지하고 있다.
이러한 세계적 동향에 적극적으로 대응하고 역내 리더십을 제고하기 위하여 보다 다각적인 국제협력 활동을 수행할 필요가 있다. 따라서 본 연구에서는 첫째, 한국보건의료연구원 국제협력 업무의 효율성을 높이기 위하여 국제협력 관련 업무 체계를 구축하고자한다. 둘째, HTA 국제협의체 및 국외 학술활동 참여를 확대하고 국외 유관기관과의 교류를 통해 국제 네트워크를 강화하고자 한다. 셋째, 중장기적 국제협력 활동의 방향성을 확립하여 한국보건의료연구원 국제협력의 지속적 발전을 도모하고자 한다.
▢ 국제협력 활동에 대한 체계화
국제협력 활동의 효율성을 높이기 위하여 업무를 표준화하고 관련 자료의 데이터베이스화가 진행되었다. 국외 HTA 유관기관 및 협의체와 HTA international(HTAi)과 같은 보건의료 분야 국제학회에 대한 조사를 수행하고 관련 정보를 데이터베이스화하는 한편 이 중 한국보건의료연구원이 수행하는 연구와의 유사성을 고려하여 선별된 5개 국외기관(미국 Agency for Healthcare Research and Quality(AHRQ), National Guideline Clearinghouse, U.S. Preventive Services Task Force, 영국 NICE, 유럽 EuroScan)의 동향에 대한 자료를 정기적으로 배포하였다. 또한 해외출장 업무에 대한 표준화가 이루어지지 않고 기 구축된 자료의 공유가 원활하지 않아 업무의 중복성이 발생하는 문제점을 해결하기 위하여 개원 이후 발생한 모든 해외출장 보고서를 연도별·국가별로 정리하고 기관 방문과 학회 참석을 구분하여 해외출장 보고서 양식을 개발하였다. 데이터베이스화 된 자료는 원내 요청이 있을 경우 인쇄물이나 전자파일의 형태로 반출이 가능하도록 하여 정보 공유의 용이성을 제고하였다.
▢ 국제협력 활동 및 국제적 위상 강화
한국보건의료연구원에서는 기 구축된 국제 네트워크를 강화하기 위하여 HTA 국제협의체 활동 및 학회 참여를 확대하였다. HTAsiaLink의 사무국으로서 기존 및 신규 회원 관리와 관련한 업무를 진행하였으며 2014년에는 총 4개 기관이 신규 회원으로 협의체에 가입하였다. 또한 2014년 5월 제3회 HTAsiaLink 연례회의에 참석하여 연구원 수행 연구결과의 국제적 확산을 도모하였다. 그 외에도 HTAsiaLink 홈페이지의 부분 개편을 단행하여 회원기관 간 보다 효율적인 정보 공유를 지원하였다. 국제학회 활동과 관련하여, 한국보건의료연구원은 HTAi 연례회의 및 HTAi Policy Forum, INAHTA와 RedETSA 연례회의 등에 참여하여 연구 성과를 발표하고 관련분야 전문가와의 교류를 통하여 향후 HTA의 발전방향 및 전략에 대하여 논의하였다.
또한 보건의료 및 HTA 관련 유관기관과의 네트워크를 형성하고 국제 공동사업의 개발 및 추진을 위한 노력으로 2014년 2월, 영국 NICE와 MOU를 체결하였다. 이를 통하여 HTA 및 보건의료 관련 정보의 적극적 공유와 공동 워크숍 개최 등의 국제적 협력 활동을 수행하고자 한다. 이러한 국제 유관기관과의 MOU를 통한 네트워크 형성은 향후 지속적으로 진행될 예정이며 2015년에는 캐나다의 Canadian Agency for Drugs and Technologies in Health(CADTH)와의 MOU 체결이 준비 중에 있다.
HTA 전문기관으로서의 국제적 위상을 제고하기 위하여 한국보건의료연구원은 HTA를 바탕으로 한 보건의료분야 공적개발원조(Official Development Assistance, ODA) 사업을 진행하였다. 그 한 예로 HTA의 필요성은 인지하고 있으나 그 체계가 구축되지 않은 인도네시아 정부 및 학계 관련자를 대상으로 HITAP과 함께 HTA 교육 워크숍을 진행하여 관련자들의 HTA 능력 배양에 기여하고자 하였다.
연구원 및 연구원 수행 연구결과에 대한 국제 확산활동의 일환으로 해외 유관기관에 배포할 수 있는 기관 영문책자를 제작하였으며 연구 성과의 확산을 지원하기 위하여 2013년 한국보건의료연구원의 연구보고서 총 17건을 영국의 Centre for Reviews and Dissemination(CRD)에 게재하였다.
▢ 대외협력 역할 및 중장기 계획 수립
한국보건의료연구원의 국제협력 활동에 대한 인식과 업무에 대한 수요를 파악하기 위하여 연구원 내 구성원을 대상으로 2주에 걸쳐 온라인 설문조사를 수행하였다. 설문조사 결과 국외동향 배포와 HTA 국제협의체 활동에 대한 인지도는 높은 편으로 나타났으나, 해외기관의 본원 방문 및 해외 보건의료 종사자를 대상으로 한 연수교육에 대한 인지도는 낮게 나타났다. 국제협력 업무에 대한 만족도 조사에서 국외동향 자료가 업무에 도움이 된다고 답한 비율은 71%였으며, 그렇지 않다고 응답한 경우 관련 사유로는 현재 수행 중인 연구과제와의 낮은 연관성, 광범위한 분야의 약식 정리 등으로 나타났다. 2014년 영국 NICE와 체결된 MOU에 대한 인지도 역시 높게 나타났으며, MOU 후속활동으로 공동 워크숍/심포지엄 개최와 연수기회 제공을 통한 인적교류, 공동연구 수행이 제안되었다. 한국보건의료연구원의 국제협력 업무에 추가되어야 할 항목으로 해외 유관기관과의 협력활동 강화, 연구원 수행연구에 대한 국제 확산, 국제협력 활동 내역에 대한 공유 채널 마련과 내부 교육 수행 등으로 나타났으며, 국제협력 업무에 대한 관리 능력을 강화하고 업무의 내실화를 도모해야한다는 의견이 제시되었다.
기 수행 중인 국제협력 업무에 대한 현황 분석과 원내 설문조사 결과를 바탕으로 설립된 한국보건의료연구원 국제협력 활동의 중장기적 방향은 다음과 같다. 첫째, HTA 국제 확산 활동을 강화한다. HTA의 역할이 강조되고 있는 현 추세에 맞추어 연구원의 경험과 HTA 관련 연구수행 능력을 바탕으로 역내 HTA 확산에 기여하고 국제적 리더십을 제고하고자 한다. 둘째, 국외 유관기관과의 MOU 추진을 바탕으로 협력 관계를 확대하고 연구수행 노하우 및 정보 교류를 바탕으로 내적 성장을 도모하고자 한다. 셋째, 해외 유관기관의 국외 동향 및 한국보건의료연구원 국제협력 활동에 대한 공유 체계를 구축하여 활동성과가 연구원 수행 업무에 활용될 수 있는 기반을 확보하고자 한다.
▢ 결론 및 정책적 제언
현재 HTA에 대한 국제사회의 관심이 증가하고 있으며, 이에 따른 한국보건의료연구원의 국제협력 활동 역시 새로운 도약이 필요한 시점이다. 본 연구는 국제협력 활동을 강화하기 위하여 기 수행 중인 활동을 파악하고 업무의 체계화 및 내실화를 도모하였다. 또한 국제 네트워크를 확대하고 연구원의 국제적 위상 제고를 위한 다양한 활동을 수행하였으며 국제협력 활동에 대한 원내 수요조사를 시행하여 중장기적 활동 방향을 모색하고자 하였다.
한국보건의료연구원의 국제협력 활동 현황을 분석하고 한국보건산업진흥원, 영국 NICE, 태국 HITAP 등 국내외 유관기관의 활동을 비교한 결과 타 기관들이 중·저개발국가의 보건의료 체계 구축을 지원하는 등 보다 다각적 활동을 수행하고 있는 반면 한국보건의료연구원의 국제협력 활동은 HTA 국제협의체와 학술대회 참여가 가장 큰 비중을 차지하고 있었다. 국제적 흐름에 부응하고 HTA 선진기관으로서의 리더십 제고를 위하여 연구수행 능력과 결과를 국제사회에 확산하기 위한 보다 다양한 노력이 필요할 것으로 보인다. 2015년에는 HTA의 국제 확산을 위해 Asia-Pacific Economic Cooperation(APEC)/WHO 및 국내외 유관기관과 연계하여 HTA 국제 워크숍을 개최할 예정이며 이를 바탕으로 구축된 인적 네트워크를 활용하여 보다 다양한 중·저개발국의 HTA 체계 구축에 기여하고자 한다. 현재 연구원의 국제협력 활동은 담당자 및 참여자를 제외한 전체 구성원 내 공유가 원활하지 못하다는 지적이 있다. 따라서 수준 높은 국제 공동 연구를 수행하고 연구결과를 확산하는 등 연구원의 국제적 위상 제고를 위해 내부연구진의 국제 감각을 향상시키고 국제협력 활동을 통해 축적된 경험을 공유할 수 있는 교류 체계가 구축되어야 할 것이며, 보다 다양한 구성원이 국제협력 활동에 참여할 수 있도록 하여 내적 역량 강화를 지향하고자 한다.
▢ Introduction
The importance of Health Technology Assessment (HTA) as an integral component for financial efficiency of healthcare has been underlined worldwide. In 2014, the 67th World Health Assembly adopted a resolution titled, "Health intervention and technology assessment in support of universal health coverage," increasing the interest in and demand for HTA. Following this trend, cooperation among HTA international networks and organizations has expanded in order to devise ways to carry out HTA effectively, share research results and related information, and spread HTA globally. Especially, training programs between HTA agencies is provided or international conferences are held to share information and strengthen research capabilities. With accumulated research methodology and experiences through the forementioned activities, developing countries are supported to establish HTA systems and build HTA capacity.
National Institute for Health and Care Excellence (NICE) and Health Intervention and Technology Assessment Program (HITAP) have established International Units to concentrate effort on improving the international cooperation and global healthcare. They have also signed MOUs with the department of health of less developed nations to help them develop healthcare systems and strengthen their ability to carry out HTA. In addition, they have been maintaining cooperative relationships with various non-profit organizations such as HTA related agencies, the World Bank, and WHO.
To actively react to these global trends and improve the regional leadership, there is a need to carry out a more multilateral international cooperation effort. Accordingly, this study aims to enhance the effectiveness of international cooperation services of the National Evidence-based Healthcare Collaborating Agency (NECA) by refining a system for activities related to international cooperation. In addition, by expanding the participation in HTA international networks and conferences, we aim to strengthen the global network of the agency.
▢ Systematization of international collaboration efforts
To increase the effectiveness of international collaborations, work has been standardized and databases have been created for related materials. Studies have been done on HTA related agencies, networks, and international conferences on healthcare, and searched information has been organized into databases. Among the organizations, recent activities and trends of five international agencies including Agency for Healthcare Research and Quality (AHRQ), National Guideline Clearinghouse, and US. Preventive Services Task Force, NICE, and EuroScan, selected based on similarities to research performed by the NECA, were distributed regularly. Also, in order to reduce the amount of duplication work arising from unstandardized work system regarding overseas business trip and inadequate sharing for the accumulated information, we have collected every report on overseas business trips by year and country and developed a new template for the report, separating agency visits and participation in international conferences. The data are stored into databases and are available as a print or as an electronic file when there is a demand within the agency for the purpose of facilitating information sharing.
▢ Enhancement of the international collaborations
NECA has expanded its involvement in international networks and conferences on HTA to strengthen the previously established international collaborations. As the secretariat of HTAsiaLink, it performed tasks on managing existing and new members. In 2014, four agencies such as Essential Medicines and Technology Division (EMTD) of Bhutan joined the network. NECA representatives also attended the third annual meeting of HTAsiaLink in May of 2014 to plan a global dissemination of study results conducted by NECA. In addition, the HTAsiaLink website was revised and updated to support a more effective way of sharing information between HTAsiaLink member agencies. In terms of international conferences, the NECA has participated in HTAi annual meetings, HTAi Policy Forum, INAHTA, and RedETSA annual meetings to present its research results and to discuss the future directions and strategies of HTA by exchanging ideas with experts in the related fields.
In March of 2014, the NECA signed an MOU with NICE as an ongoing effort to form a network with healthcare and HTA related agencies and to develop and promote international joint collaborations. Through the MOU, sharing of HTA and healthcare related information will become more active, and international cooperation such as co-hosting workshops are expected to take place. Establishing networks by signing MOUs with international agencies will continue, and signing an MOU with the Canadian Agency for Drugs and Technologies in Health (CADTH) is in progress.
To improve its status as an expert on HTA, NECA has proceeded with healthcare Official Development Assistance (ODA) projects, focusing on HTA education. For example, collaborating with HITAP, the agency provided HTA educational workshops to government and academic personnel in Indonesia, who have acknowledged the need for HTA, but have not established it systematically, in order to support their capacity building for HTA.
As a part of an ongoing effort for worldwide dissemination, the NECA has produced English brochure to distribute to international organizations. In addition, a total of 17 studies conducted in 2013 was posted on Centre for Reviews and Dissemination (CRD) to promote distribution of research results.
▢ Role of International Relations and establishment of mid-to-long term goals
An online survey was conducted on every member of NECA for two weeks to understand how much the members were aware of the agency's international collaboration activities and what kind of demand there was. It was found that the distribution of global trends and the participation in HTA international networks were well recognized. However, not many were aware about foreign agencies visiting NECA and the education program given to the trainees. In terms of the satisfaction on the agency's international cooperation efforts, 71% answered that the information on global trends has been useful to conduct researches. People who answered not being satisfied with the provided data indicated that the following reasons: the data having low relevance to the research they were conducting and the data being an abbreviated summary of such a wide range of subjects. There was a high awareness on the MOU signed between the agency and NICE in 2014, and it was proposed that co-hosting workshops/symposiums, providing training opportunities to exchange human resources, and conducting research together should follow the MOU. Many also proposed that NECA should add to its international collaborations the following: strengthening collaboration with foreign agencies, internationally spreading research findings, setting up channels to share the details of international cooperations, carrying out an educational seminar inside the agency, and reinforcing research capacities and doing a sound development of international cooperation efforts.
Based on the survey and the analysis on international collaboration already being carried out, NECA has set the following mid-to-long term goals in its international collaborating activity. First, it will strengthen its efforts to spread HTA globally. To follow the current trend that emphasizes the role of HTA, it aims to contribute to the spread of HTA and improve international leadership based on the researchers' experience and research capabilities. Second, by signing MOUs with foreign agencies, NECA will expand its cooperative relationships with international healthcare and HTA related agencies and pursue internal growth. Third, by establishing a system of sharing global trends and international collaboration activities, it will attempt to set foundation where those can be utilized in NECA researches.
▢ Conclusions and Implementations
Currently, there is an increasing interest in HTA among the international society. Therefore, NECA should also take a leap in its international cooperation efforts. This study aimed to strengthen the international cooperation efforts by analyzing previously ongoing activities and seeking to systemize the agency's work and ensure its internal stability. In addition, this study performed various activities to expand the international network and improve its researchers' international status. It also conducted a survey inside the agency to analyze the demand for international cooperation efforts, trying to establish mid-to-long term goals for the agency.
After analyzing the international cooperation efforts of NECA and comparing them to the efforts of related domestic and foreign agencies such as Korea Health Industry Development Institute, NICE, and HITAP, we have come to the conclusion that the NECA's international cooperation activities consist largely of participating in international consultative bodies and international conferences. To meet the global trends and improve its leadership as an advanced agency in HTA, a wide range of efforts to spread NECA's research capabilities and findings is needed. NECA is planning to host an international HTA workshop in 2015 by cooperating with APEC/WHO and other domestic and foreign organizations to promote spreading HTA internationally. By utilizing the human network gained from the workshop, NECA would like to contribute to the establishment of HTA system in middle- and low-income countries.
There have been some comments that there should be more opportunities to share the information of international cooperation activities with all members in NECA. Therefore, a system for exchanging HTA experiences internationally is needed to improve NECA's international status by conducting international joint research projects at a high level and spreading the findings. Finally, in order to improve its inner capacity, more members in NECA should participate in international cooperation activities.
